Description
本発明は、心房細動の処置のための方法および装置に関する。より具体的には、本発明は、非侵襲的に高密度焦点式超音波を使用して、心房細動の一因となる異常な心臓の電気経路を中断することに関する。
1980年代まで、以前には処置されなかった広汎な種々の状態を処置するための新規な外科的方法の開発が、劇的に増加した。過去20年間にわたり、侵襲性手術から移って、そのような疾患の低侵襲性処置を可能にするデバイスおよび方法の発明へ、その後、低侵襲性手術へ、そして介入技術へと向かう明らかな傾向があった。最終的には、全く非侵襲性の治療へと移ることが、望ましい。
上記のことを考慮して、本発明の目的は、心房性細動および他の伝導欠陥を、その組織から離れた組織を焼灼すること(これにより、その手順は、非侵襲性に実施され得る)によって処置するための、方法および装置を提供することである。
本発明は、高密度焦点式超音波(HIFU)を使用して、完全に非侵襲性の様式で、心臓の壁に損傷部を作製するための方法および装置に関する。以前から公知であるHIFUシステム(例えば、Epicor MedicalまたはTransurgicalによって開発されたシステム)は、HIFUデバイスを、標的組織に近く近接させる必要がある。本発明の方法および装置は、より離れた距離から心臓の壁に損傷部を作製することを可能にするシステムを提供することによって、この欠点を克服する。
Claims (17)
- 身体の外側で組織から距離を置いて該組織を焼灼することにより心房細動を処置するための装置であって:
ハウジング;
該ハウジング内に配置された超音波画像化システム;
該ハウジング内に、該超音波画像化システムと整列して配置された、要素のアレイを備えた高密度焦点式超音波システムであって、独立して操縦可能なアクチュエータ上に位置するHIFU(高密度焦点式超音波エネルギー)要素を備える高密度焦点式超音波システム;
該超音波画像化システムおよび高密度焦点式超音波システムに作動可能に接続された、制御器;および
該高密度焦点式超音波システムの焦点の位置を調節するために該ハウジングに連結された、流体を満たされたバルーンを備え、該制御器が該高密度焦点式超音波システムを制御するためにプログラムされ、該組織に対して該ハウジングの移動なくして心臓組織中に連続する直線状の焼灼を形成する、装置。 - 前記制御器、前記超音波画像化システムおよび前記高密度焦点式超音波システムが、共有する変換器を備える、請求項1に記載の装置。
- 前記制御器が、前記高密度焦点式超音波システムの焦点に対応するマーカーを表示するようにプログラムされている、請求項1に記載の装置。
- 前記制御器が、前記高密度焦点式超音波システムの焦点の位置を、該高密度焦点式超音波システムの軸に対して垂直な二次元の面内で調節するようにプログラムされている、請求項3に記載の装置。
- 前記制御器が、前記高密度焦点式超音波システムの焦点の深さの位置を調節するようにプログラムされている、請求項3に記載の装置。
- 身体の外側で組織から距離を置いて該組織を焼灼することにより心房細動を処置するためのシステムであって:
ハウジングであって、該ハウジングは、超音波画像化システム、および該超音波画像化システムと整列して配置された要素のアレイを備えた高密度焦点式超音波システムを有し、該ハウジングが患者の身体に対して接触されるように構成され、該高密度焦点式超音波システムが独立して操縦可能なアクチュエータ上に位置するHIFU(高密度焦点式超音波エネルギー)要素を備える、ハウジング;
該超音波画像化システムを作動させて、心臓組織の一部分の画像を作成するための手段;および
該患者の身体と該ハウジングとの間に配置されように構成され、該高密度焦点式超音波システムの焦点の位置を調節するように構成されている、流体を満たされたバルーンを備え、
該高密度焦点式超音波システムが、手動で作動され、そして該画像によって案内されるよう構成され、該組織に対して該ハウジングの移動なくして標的部位における心臓組織に連続する直線状の焼灼を形成する、システム。 - 前記高密度焦点式超音波システムの焦点に対応するマーカーを作成し、そして該マーカーを、前記画像上に表示するための手段をさらに備える、請求項6に記載のシステム。
- 前記高密度焦点式超音波システムの焦点の位置を調節することによって、前記標的部位の位置を改変するための手段をさらに備える、請求項6に記載のシステム。
- 前記患者の左心房の壁上の一連の垂直焼灼線に沿って、心臓組織を焼灼するための手段をさらに備える、請求項6に記載のシステム。
- 心房細動を管腔内処置するための装置であって:
遠位部分ハウジングを有するカテーテル;
該カテーテルの遠位部分ハウジング内に配置された超音波画像化システム;
該カテーテルの遠位部分ハウジング内で、該超音波画像化システムと整列して配置された、要素のアレイを備えた高密度焦点式超音波システムであって、独立して操縦可能なアクチュエータ上に位置するHIFU(高密度焦点式超音波エネルギー)要素を備える高密度焦点式超音波システム;ならびに
該超音波画像化システムおよび高密度焦点式超音波システムに作動可能に接続された、制御器、を備え、
該制御器が、該高密度焦点式超音波システムの焦点に対応するマーカーを表示するようにプログラムされている、装置。 - 前記制御器、前記超音波画像化システムおよび前記高密度焦点式超音波システムが、共有する構成要素を備える、請求項10に記載の装置。
- 前記制御器が、前記高密度焦点式超音波システムの焦点の位置を、該高密度焦点式超音波システムの軸に対して垂直な二次元の面内で調節するようにプログラムされている、請求項10に記載の装置。
- 前記制御器が、前記高密度焦点式超音波システムの焦点の深さの位置を調節するようにプログラムされている、請求項10に記載の装置。
- 前記高密度焦点式超音波システムが、線状の焼灼標的に沿って集束させるように構成されている、請求項10に記載の装置。
- 心房細動を管腔内処置するためのシステムであって:
遠位部分ハウジングを有するカテーテルであって、該カテーテルは、超音波画像化システム、該超音波画像化システムと整列して配置された要素のアレイをもつ高密度焦点式超音波システムおよび高密度焦点式超音波要素のアレイの周りに流体を循環し、組織への損傷を防ぐための水ジャケットを収容しており、該アレイが該高密度焦点式超音波システムの焦点の位置を調節することにより標的部位の位置を改変するための独立して操縦可能なアクチュエータを備え、該高密度焦点式超音波システムが独立して操縦可能なアクチュエータ上に位置するHIFU(高密度焦点式超音波エネルギー)要素を備え、該カテーテルの遠位部分が、患者の身体の食道内に配置されるように構成されている、カテーテル;
該超音波画像化システムを作動させて、心臓組織の一部分の画像を作成するための手段;を備え、そして該高密度装填式超音波システムが手動で作動され、そして該画像によって案内されるよう構成され、標的部位における心臓組織を焼灼し、そして直線状の焼灼標的に沿って心臓組織を焼灼する、システム。 - 前記高密度焦点式超音波システムの焦点に対応するマーカーを作成し、そして該マーカーを、前記画像上に表示するための手段をさらに備える、請求項15に記載のシステム。
