Description
本発明の別な観点によれば、添付の特許請求の範囲の請求項3に記載されているような、自己拡張型ステントと経皮経管型の搬送カテーテルを含む自己拡張型ステント搬送装置が提供される。
2. ステント部位にステントを設置している際に、搬送用鞘部材を近位方向に引き出している最中にステントが近位方向に移動するのを抑止する、
という上記目的のために、ステントに必ずや賦課されなければならない力をステント管の管腔の全長にわたって満遍なく分散させることにより、ステント管内の応力がステント管の一端に集中するのではなく、概ね均質に分布するように応力分散を管理することができる。プッシャから鞘部材までの間の一連の応力分布で連接役としてステントの被覆材を利用することにより、金属ステント管内のいかなる点でも全く、所定の設計最大値を超えたレベルの応力を受けることを更に回避することができる。ステント被覆材は、本来、ステント管そのものよりも可撓性に富んでいるため、金属製のステントプッシャ上の一点からステント管の素材の一面積または一容積の間で応力を分散させることができる。
図3は搬送システムの遠位先端部のみを例示しているが、システムの残余の部分は本発明が当該技術に貢献することになる部分ではなく、少なくとも、当業者には馴染みのある部分である。自己拡張型ステントの従来の搬送システムの基本構成要素は、内側カテーテルと外側鞘部材であり、外側鞘部材の目的は、ステント設置部位でステントが解放されるまでの間、自己拡張型ステントを放射方向に拘束することで小径の搬送形状に維持することである。内側カテーテルの目的は、鞘部材が近位方向に引き出されている間に、鞘部材と一緒にステントが近位方向に移動することがないように抑止することである。
Claims (9)
- ステント搬送システムを製造するに際して搬送鞘部材に自己拡張型ステントを装填する方法であって、放射方向に拘束された搬送形状を取っているステントが、ステント設置部位に搬送され、ステント設置部位で、鞘部材が引き出されてステントを解放し、その部位でステントを放射方向に拡張させるようにするために、軸線方向に前進させられることで鞘部材の中に入れられる、搬送鞘部材に自己拡張型ステントを装填する方法において、
(i)ステントを被覆ステントとして設ける段階を含み、被覆ステントは、管腔外被および非管腔外被の輪郭を定めている複数表面がステントの壁厚さ分だけ互いに間隔を離されているステント配列と、ステント配列に接着されて管腔外被の放射方向内側に位置している被覆材とから構成されており、
(ii)ステントの管腔内にステントプッシャを設ける段階を含み、ステントプッシャはステント管腔の長さに沿って分布する、放射方向外向きに張出した突起部を設けており、
(iii)突起部が被覆材を変形させて、それでもなお、ステント配列の管腔外被が突起部によって放射方向に変形されないままであるまで、ステントを放射方向内向きに圧縮する段階と、
(iv)ステントプッシャに端部からの力を付与することにより圧縮状態のステントを前進させて鞘部材の中に入れ、ステントプッシャの突起部からステント配列へ押し力を被覆材に伝達させるようにする段階とを含んでいる、
ことを特徴とする方法。 - 突起部を螺旋状に配置し、鞘部材の内側で、ステント管腔に対してステントプッシャを反対にねじることで、ステントの管腔からステントプッシャを引き出すことができるようにするようにした段階を更に含んでいることを特徴とする、請求項1に記載の方法。
- 自己拡張型ステントと経皮経管型の搬送カテーテルを含む自己拡張型ステント搬送装置であって、前記経皮経管型の搬送カテーテルは、前記自己拡張型ステントを取り囲み、近位方向に引き出されるとステント設置部位でステントを解放する鞘部材と、鞘部材の内部に置かれて、鞘部材を引き出している間もステントをステント設置部位に保持するプッシャとを有している、自己拡張型ステント搬送装置において、
(i)プッシャはステントの管腔に沿って延びるとともに、ステント管腔の長さに沿って分布させられた放射方向外向きに張出した突起部を設けており、
(ii)ステントは、管腔外被および非管腔外被の輪郭を定める複数表面がステントの壁厚さ分だけ互いに間隔を離されているステント配列と、ステント配列に接着されて管腔外被の放射方向内側に位置している被覆材とから構成されている被覆ステントであり、
(iii)突起部が被覆材を変形させて、それでもなお、ステント配列の管腔外被が突起部によって放射方向に変形されないままであることを特徴とする、自己拡張型ステント搬送装置。 - 前記ステント配列は金属製であり、前記被覆材は発泡ポリテトラフルオロエチレン製であることを特徴とする、請求項3に記載の自己拡張型ステント搬送装置。
- 前記ステント配列は複数開口が設けられており、前記被覆材は複数開口を通して非管腔ステント被覆層に接着されていることを特徴とする、請求項3または請求項4に記載の自己拡張型ステント搬送装置。
- 前記ステント配列はニッケルチタン形状記憶合金から形成されていることを特徴とする、請求項4または請求項5に記載の自己拡張型ステント搬送装置。
- 前記突起部は螺旋状の渦巻き体であることを特徴とする、請求項3から請求項6のいずれか1項に記載の自己拡張型ステント搬送装置。
- 先細り状の遠位先端部は前記鞘部材に設けられていることを特徴とする、請求項3から請求項7のいずれか1項に記載の自己拡張型ステント搬送装置。
- 先細り状の遠位先端部は前記鞘部材より遠位で前記プッシャに設けられていることを特徴とする、請求項3から請求項7のいずれか1項に記載の自己拡張型ステント搬送装置。
