Description
3つの天然メチルキサンチンアルカロイド、すなわち式(I)のテオフィリン、式(II)のカフェインおよび式(III)のテオブロミンが茶(Thea sinensis)の葉、カカオ(Theobroma cacao)種子およびコーヒー(Coffea arabica)果粒中に存在することが知られている。これらの植物はいくつかの広く消費されている商品および飲料のソースであるため、ヒトの食生活に深い関わりを持っている。
茶では、テオフィリンとテオブロミンの他にカフェインが主要なアルカロイドである。コーヒーにおいてはカフェインが主なアルカロイドであり、一方チョコレートの基本原料であるココアパウダーとチョコレート液ではテオブロミンが主要アルカロイドであるが、多少のカフェインも含んでいる。
3つの天然メチルキサンチン(I、II、III)は複雑な生物学的作用を持つが、化学的に緊密に類似しているにも関わらず、効力とそれらの作用スペクトルに関しては大きな相違がある。
2つの主要な分子機序から、メチルキサンチンの広いスペクトルの薬理作用が推論できることは文献において一般的に認められている。ひとつは環状ヌクレオチドホスホジエステラーゼ類(PDE)の非特異的阻害であり、2番目はアデノシンレセプタ類の拮抗作用である。これまでに数多くのアルキルキサンチンが検討されており、一般原則として、対応する1,3−ジアルキルキサンチンと比較して、キサンチン部分のN1原子上に置換基がないか、もしくはN7原子が置換基を含んでいれば、上述した両方の作用が低下することが確認されている。
N1原子は置換されていないが、N7原子が置換されているテオブロミンの場合には、一般原則に従ってテオブロミンの活性はカフェインあるいはテオフィリンの活性よりも弱いはずである。実際には、PDE阻害とアデノシン拮抗作用に関する3つの天然に発生するメチルキサンチンの活性順位は次の通りである:テオフィリン>カフェイン>テオブロミン。
in vivoおよびin vitroの両方で様々な種類の平滑筋を弛緩することは、メチルキサンチン、特にテオフィリンの非常に重要で且つ臨床的に高い関連性を持つ特徴である。テオフィリンの気管支平滑筋弛緩作用と他の優れた治療特性は喘息の治療に広く使用されている。テオブロミンとカフェインの同様の作用はテオフィリンの作用よりもはるかに低い。
カフェインとテオフィリンは強力な中枢神経系(CNS)刺激作用を有することが知られており、茶とカフェイン含有飲料が高い人気で消費されていることがそれを裏付けている。テオブロミンのCNSへの作用はカフェインあるいはテオフィリンのどちらよりも弱い(Mumford,G.K.ら、Psychopharmacology,1994,115:1)。
カフェインとテオフィリンは心筋および心臓血管系に強力で複雑な作用を及ぼす。これらの化合物は著明な末梢血管拡張作用を持つことが知られているが、より高用量では頻拍も生じさせる。テオブロミンの心臓血管作用ははるかに弱い。
利尿作用はこれら3つのメチルキサンチン(I、II、III)、特にテオフィリンの非常に特徴的な性質である。テオブロミンはテオフィリンあるいはカフェインに比して弱い利尿作用しか有していない。
天然メチルキサンチンの相違は、有用な治療作用に存在するが、それらの副作用や毒性作用でも認められうる。テオフィリンは治療用量範囲が狭いため、過量の危険性が高いが、カフェインの場合にはその可能性が低い。テオブロミンによって引き起こされる危険な副作用の可能性は、テオフィリンあるいはカフェインに比べて無視しうるものである(Stavric,B.,Fd.Chem.Toxic.1988,26:725)。
天然に生じるメチルキサンチン、特にテオフィリンとカフェインを取り上げた学術文献は極めて広範囲にわたっている。数千もの学術論文、特許明細書、この話題に関する総説や書物の中にデータが認められる。これらの試験は、テオブロミンの生物活性が低いため、この化合物には治療的重要性がないことを示唆している。上記の意見は最も広く知られている薬理学書物のひとつによって支持されている:
「テオフィリン、カフェイン、およびテオブロミンは、治療的興味のあるいくつかの薬理作用を共有している。これらは平滑筋、特に気管支平滑筋を弛緩し、中枢神経系(CNS)を刺激し、心筋を刺激し、さらに腎臓に作用して利尿を生じさせる。テオブロミンはこれらの薬理作用において低い効力しか示さないため、治療のシーンからほとんど姿を消してしまったのである。」(Rall,T.V.,Goodman and Gilman’s,The Pharmacological Basis of Therapeutics,第8版、p.620、Pergamon Press,New York,1990)から引用)。テオブロミンの鎮咳作用に関する言及は見出せなかった。
上記の事実から、我々が、テオブロミン(III)が強力で長時間持続性の鎮咳作用を示すという予想外の結果に至ったことは非常な驚きであった。このテオブロミンの作用は、最も広く使用されている鎮咳性モルヒネ誘導体のひとつであるコデインと同等である。
テオブロミンは毒性が低く、危険な副作用がないため、この予想外で有益な鎮咳作用は治療において卓抜に使用することができる。モルヒネ構造の鎮咳化合物と異なって、テオブロミンは呼吸抑制作用が全くなく、さらにその鎮咳作用に加えていくつかの有利な気管支肺への作用を有しているという事実の重要性を強調しておきたい:我々の現在のデータによれば、テオブロミンは、周知の粘液溶解物質であるブロムヘキシンと同様に粘膜線毛のクリアランス機能を有意に刺激する。
文献によれば、テオフィリンはモルモットにおいてクエン酸噴霧によって誘発した咳に対して弱い鎮咳活性を持つ。この弱い鎮咳活性は、有害な副作用が発現するため、テオフィリンの用量を増加しても改善することができない(Forsberg,K.ら、Respiration,1992,59:72)。アミノフィリン(テオフィリンエチレンジアミン)は、同じ試験モデルにおいて検討したすべての用量で実際上無効である(Franzone,J.S.ら、Il Farmaco,Ed.Sc.1981,36:201)。
カフェインあるいはテオブロミンでは同様の鎮咳作用は認められなかった。我々は、コデイン−HClを標準化合物として用いて、クエン酸スプレー誘発の咳モデルに対するそれらの鎮咳活性を検討した。
急性鎮咳活性に関する実験を、体重250−300gの雌雄の両方のハートレー白色モルモット(Charles River)において実施した。Tardosの方法に重要でない修正を加えて採用した(Tardos,L.,Erdely,I.,Arzneim.Forsch.(Drug Res.)1966,16:617)。噴霧は、透明なチェンバー(容量6.7リットル)に接続した噴霧器によって生成した。0.16リットル/秒の流量の圧縮空気と0.5バールの圧で噴霧を生成した。蒸気は一定な空気流によって推進され、短い管を通してチェンバー内に入り込んだ。動物を個体別に透明チェンバーに入れ、クエン酸(蒸留水に溶解した15w/v%クエン酸)エアロゾルに3分間暴露した。熟達した観察者が噴霧暴露の開始から終了まで、咳の数をカウントした。動物の咳は、腹部の強い収縮とそれに続く動物の開いた口を通しての強制呼気と定義した。物質を試験するにあたって、最初のトライアルにおいて3分間に6回またはそれ以上の咳を示した動物だけを使用した。選んだ動物を、0.1%メチルセルロース中の懸濁液(処置群)として被験キサンチンあるいは標準コデインによって、あるいは0.1%メチルセルロース単独(対照群)によって経口で1回処置した。0.1ml/100g体重の容量の賦形剤を適用した。被験物質による処置の1時間後に2回目のクエン酸発作が誘発された。鎮咳活性は、2回目と1回目の誘発チャレンジ間での咳の数のパーセント低下として計算した。薬剤処置群を賦形剤処置対照群と比較した。
図1に示すように、テオブロミンと標準コデインは単回経口処置後、用量依存的に咳の回数を減少させた。
2回目の咳発作の1時間前に動物を経口で処置した。
賦形剤は0.1%メチルセルロースであった。
テオブロミンの鎮咳作用はコデインの作用と同じ程度に強力であった。テオブロミンとコデインの鎮咳作用は、それぞれ4および8mg/kg用量で既に有意であった。テオブロミンの64mg/kg用量で咳数の大きな低下(69.0±1.9%)が達成された。同じ用量のコデインはわずかに低い度合で(54.2±5.7%)咳数を減少させた。テオブロミンとコデインの算定したED50値はそれぞれ37mg/kgと49mg/kgであった。
これに対し、カフェインは2−16mg/kgの用量範囲で鎮咳作用を及ぼさない。より高用量では(32および64mg/kg)、カフェインは咳の数を有意に低下させたが(それぞれ17.5±3.7%と48.4±3.1%)、これらの用量はこの化合物の毒性用量に近く、従ってカフェインの鎮咳作用は治療には不適当である。
ラットにおける3つの化合物の急性経口毒性は次の通りである(LD50mg/kg):カフェイン:192;テオブロミン:1265;コデイン:427(The Sigma−Aldrich Library of Chemical Safety Data,編集、Lenga R.E.編集、第II版、Sigma−Aldrich Corporation,USA,1988)。
得られたED50とLD50値から計算したテオブロミンの鎮咳治療指数LD50(/ED50)は34で、コデインの指数は8.7である。テオブロミン鎮咳治療指数はコデインの指数よりもほぼ4倍高い。
さらなる実験により、テオブロミンの優れた鎮咳作用は数時間持続することが証明された。経口投与した32mg/kgのテオブロミンのハートレーモルモットに対する鎮咳作用の時間依存性を、1時間の急性実験で述べたようにして測定した。
図2に示すように、テオブロミンは長時間持続性の鎮咳活性を有している。
モルモットにおいて経口投与した32mg/kgのテオブロミンが誘発する鎮咳作用の時間経過は、同様に32mg/kgの経口用量で測定したテオブロミンの血漿レベルの時間経過と概ね平行している(図2参照)。これら2つの時間経過の間のこの緊密な平行性を明らかにすることは、本発明の主題に関して非常に重要であり、有益である。薬剤の経口投与後短い時間内に期待される治療効果に達することは重要な特徴である。最近のヒトでの治験の結果は、チョコレートベースのテオブロミン製剤を純粋なテオブロミン含有カプセルと比較すると、両製剤とも等しいテオブロミン用量を使用したが、前者はテオブロミンの腸吸収率と血漿濃度の両方を有意に改善することを示した。カプセルの投与後3時間でテオブロミンの最大血漿レベルに達したが、チョコレートベースの製剤を用いると吸収ははるかに速やかであり、治療後2時間で最大血漿濃度に達した(Mumford,G.K.ら、Eur.J.Clin.Pharmacol.1996,51:319)。モルモットでは、処置から4時間後のテオブロミン血漿レベルの低下が比較的急激であるのに対し、ヒトにおいてはテオブロミンの血漿半減期が長い(t1/2=6.1〜10時間;Shively,C.A.ら、Clin.Pharmacol.Ther.1985,37:415)。ヒト血漿中でのコデインの半減期ははるかに短い:2〜4時間(Raisine T.,Pasternak G.,In:Goodman and Gilman’s The Pharmacological Basis of Therapeutics,第9版,McGraw−Hill,New York,1996,p.534)。
テオブロミンのヒト血漿レベルの低下がテオフィリンあるいはカフェインよりも緩徐であることも言及しておく価値がある(Birkett,D.J.,Methylxanthine metabolism in man.In:Anti−Asthma xanthines and adenosine,p.235,Anderson K.E.とPersson C.G.A.編集、Excerpta Medica,Amsterdam,1985)。
さらに、モルモットにおけるテオブロミンの血漿レベルとその鎮咳作用間の緊密な関係も、ヒトでの長時間持続性の鎮咳作用を明白に示唆している。
モルモットにおけるテオブロミンの用量反応曲線、その有利な毒性プロフィールおよび軽微な副作用を考慮すると、テオブロミンの成人の鎮咳用量として提案されるのは、200〜500mgを1日2回または3回である。
前述したように、テオブロミンのチョコレートベースの製剤はカプセル製剤に比べてヒトでより高い血漿レベルを達成するのに役立つため(Mumford,G.K.ら、Eur.J.Clin.Pharmacol.1996,51:319)、我々は、単に普通のチョコレートを食べることによって求められる鎮咳効果を達成できるのかどうかを検討した。
ミルクチョコレートからビター(ダーク)チョコレートまで様々な種類の普通のチョコレートが様々な量のテオブロミンを含有することが知られている。ビターチョコレートはミルクチョコレートの約3倍のテオブロミンを含む。特殊なビターチョコレートバー(41g)(Hershey’s Special Dark,New and Improved)は、テオブロミン185mgとカフェイン36mgを含む(Mumford,G.K.ら、Eur.J.Clin.Pharmacol.1996,51:319)。これは、必要な鎮咳効果を達成するためには、この特殊なビターチョコレートを少なくとも1〜3本(41〜123g)、1日に2〜3回食べなければならないことを意味する。もちろん、テオブロミンの含有量が少ないチョコレートの場合はもっとたくさんのバーを消費しなければならない。それほどたくさんのチョコレートを特に長期間にわたって食することは(慢性投与)、明らかに健康上の理由から禁忌である。特徴的なCNS刺激や他の薬理作用を引き起こしうるチョコレートのカフェイン含量はより重大な問題である(Mumford,G.K.ら、Psychopharmacology,1994,115:1)。
普通のココア製品(ココア飲料およびココア含有食品)にテオブロミンの鎮咳用量を適用したいと望む場合も、種々のココア製品のテオブロミンおよびカフェイン含量に基づくと、状況はチョコレートの場合と同様である(Zoumas,B.L.,Methylxanthine composition and consumption patterns of cocoa and chocolate products,in:CAFFEINE,Spiller,G.編集、CRC Press,USA,1997)。
テオブロミンの強力な鎮咳作用(カフェインおよびテオフィリンの鎮咳作用よりもはるかにすぐれている)の分子機序は不明である。先に述べたように、テオブロミンの他のすべての作用が天然に生じる他の2つのキサンチン誘導体よりも劣っているため、この作用は本当に意外である。
我々は、メチルキサンチンのいくつかの複雑な作用が鎮咳作用に役割を果たすと考える。テオブロミンの極めて良好な鎮咳活性は、おそらく、その複雑で多くの段階にわたる作用から、アデノシンレセプタ(主としてA1)の拮抗作用がテオフィリンあるいはカフェインよりも有意に弱いという事実から生じるのであろう。
習慣形成の研究のため、いくつかの実験を行った。
これらの実験の目的は、本剤の反復(慢性)投与後テオブロミンの鎮咳活性が不変のままであるかどうかを調べることであった。上述したのと同じ方法を採用した。選択したハートレー白色モルモットを1日1回14日間、0.1%メチルセルロース懸濁液として32mg/kgのテオブロミンで(処置群)、あるいは0.1%メチルセルロース単独で(対照群)処置した。1日目−7日目−14日目に鎮咳作用を検査した。先に示した日に処置の直前と処置の1時間後に咳の回数を数えた。対照群(n=12)にはテオブロミン処置群(n=13)と同じプロトコールを実施した。
対照群に合わせて調整した鎮咳作用は、1日目、7日目、14日目にそれぞれ46.1±2.3%、54.1±3.4%、49.0±4.9%であった。モルモットにおけるテオブロミンの鎮咳活性は、連続14日間の高い(32mg/kg)経口用量による処置後も習慣形成によって低下しなかった。
ヒトにおいてビターチョコレートの形態で1週間投与した比較的高い1日用量のテオブロミン(6mg/kg/日)は、テオブロミンの代謝あるいはそのクリアランスのいずれも変化させなかったことに留意しなければならない(Shively,C.A.ら、Clin.Pharamcol.Ther.1985,37:415)。
テオブロミンの代謝に関連して、我々の実験によれば、テオブロミンの主たる代謝産物である3−メチルキサンチンは狭い用量範囲で多少の鎮咳作用を持つことにも留意しなければならない。しかし、より高用量では用量活性曲線は破壊される。
呼吸気道の主要な防御機序である粘膜線毛クリアランスは慢性気管支炎によって大きく損傷される(Dirksen,H.ら、Eur.J.Resp.Dis.1987,71(補遺153):145)。気管支炎を有する患者は最も頻繁に鎮咳薬を使用する患者である。コデインのような伝統的なオピオイド鎮咳薬は呼吸パラメータを損傷するだけでなく、粘膜線毛への抑制作用も及ぼす(Melville,G.N.,Iravani,J.,Can.J.Physiol.Pharmacol.1975,53:1122)。ヒトおよびin vitro動物実験において、テオフィリンは有意の粘膜線毛クリアランス改善活性を持つことに注目しなければならない(Wanner,A.,Am.J.Med.1985,79(補遺6A):16;Wagner,U.ら、Eur.J.Pharmacol.,1996,298:265)。我々の試験の目的は、テオブロミン(テオフィリンと異なって、コデインと同等の強い鎮咳活性を持つ)がウサギにおいて呼吸気道の粘膜線毛クリアランスに影響を及ぼすかどうかを検討することであった。粘膜線毛クリアランス活性へのテオブロミンの作用を調べるため、Achterrath−Tuckermanの方法を採用した(Achterrath−Tuckerman,U.ら、Lung,1992,170:201)。既知の粘液溶解剤であるブロムヘキシン−HClを標準物質として使用した。
このモデルは、99mテクニチウム標識した同種赤血球懸濁液の吸入による投与を含む。標識赤血球の吸入の30分前に、テオブロミンと標準物質であるブロムヘキシンを静脈内投与した。噴霧化した細胞懸濁液の最後の吸入後、非開胸でガンマカメラにより60分間放射能を測定した。時間活性曲線を作成し、測定点に適合させた。粘膜線毛クリアランス値は、単位時間(時)におけるパーセント排泄としての放射性マーカの量で表した。粘膜線毛活性が事前の薬剤処置によって増強された場合には、粘膜線毛クリアランス値の上昇によって定量的に示された。統計評価のため、スチューデントt検定を用いて薬剤処置群を賦形剤処置群と比較した。2、4および8mg/kg用量の静脈内テオブロミンは粘膜線毛クリアランスを用量依存的に上昇させた。最も高い注入用量のテオブロミン(8mg/kg)は、クリアランスの60%(p<0.05)の上昇を生じさせた。20mg/kgのブロムヘキシンが同様のクリアランスを誘発させることができた。
テオブロミンは、我々の上述した試験で確立した鎮咳用量範囲内でウサギにおいて粘膜線毛クリアランスを有意に上昇させた。
鎮咳薬は一般に、咳−刺激−咳という悪循環を導く有害な刺激咳を断つために投与される。
咳の完全なあるいはあまりに強い抑制は生産的な咳も抑圧してしまうため望ましくないことを強調しておかねばならない。
理想的な鎮咳薬は咳の頻度を低下させ、苦痛を軽減するが、咳反射は損傷しないまま残す、言い換えれば、理想的鎮咳薬は有害な咳を抑制することができるが、有用な咳は損傷しない。さらに、理想的鎮咳薬は気管支拡張活性を有し、粘膜線毛クリアランスを刺激し、毒性副作用を持たない。
テオブロミンは上述した必要条件のすべてを満たすと思われる。
我々の発明によれば、テオブロミンは鎮咳薬として種々の疾患の治療に有利に使用することができる。その粘液溶解作用を考慮すると、単独あるいは合併した気道疾患(気管支炎、インフルエンザ等)の薬剤治療において特に有用であろう。
上記に基づき、本発明は、任意に他の既知の有効成分と不活性の固体あるいは液体担体、添加物および補助物質と混合した式(III)のテオブロミンおよび/またはその塩および/またはその複合体の治療上活性な量を含有する、咳症候群を軽減するのに適した製薬組成物に関する。
本発明のさらなる局面は、咳症候群を軽減するのに適した製薬組成物を製造するための、任意に他の既知の有効成分と不活性の固体あるいは液体担体、添加物および補助物質と混合したテオブロミンおよび/またはその塩および/またはその複合体の使用に関する。
任意に他の既知の有効成分と適当な不活性の固体あるいは液体担体、添加物および補助物質と混合したテオブロミンおよび/またはその塩および/またはその複合体の有効量を投与する段階を含む、咳の軽減が望ましい状態にあるヒトあるいは動物被験者を治療する方法も、本発明の目的に属する。
テオブロミン含有薬剤あるいはパラメディシンを製造するためには、様々な製剤、たとえば注射剤、シロップ、糖剤、錠剤、香錠、坐剤、カプセル、ならびにリポソームのようなこれらすべての遅効性形態が適する。ヒトの吸収データに基づくと、チョコレートベースの香錠と糖剤製剤、特にテオブロミン強化チョコレート製品とリポソームが有望である。好ましくは、咳症候群を軽減するのに適した製薬組成物を製造するために、テオブロミンおよび/またはその塩および/または複合体をココアパウダー、インスタントココアあるいはチョコレートの成分として部分的あるいは全面的に使用する。列挙した本発明に従った製剤は、従来の製薬および食品技術を用いて製造される。我々の発明を利用するための医薬製剤のテオブロミン含量は0.1〜99.9w/w%、好ましくは1〜30w/w%である。1日用量は、患者の年齢と投与経路に応じて、10〜3000mgである。
本発明を以下の製剤例によって裏付けるが、鎮咳物質としてのテオブロミンの使用はこれらの例に限定されない。
実施例1.錠剤:
混合した化合物をそれ自体が既知の方法によって顆粒化し、840mg錠剤1000錠を圧縮すると、各錠剤は500mgの有効成分を含有する。
実施例2.デポ糖剤:
有効成分であるカルボキシメチルセルロースとステアリン酸をブレンドし、200cm3の酢酸エチル−エタノール中のフタル酸酢酸セルロース溶液で顆粒化し、800mg錠剤を圧縮して、それ自体が既知の方法によって糖を含む5%ポリビニルピロリドン水溶液で被覆する。すべての糖剤が450mgの有効成分を含有する。
実施例3.カプセル:
すべての成分を混合し、それ自体が既知の方法によって顆粒化する。1000個のカプセルにこの顆粒を充填し、350mgの有効成分を含有するカプセルを得る。
実施例4.坐剤:
固相脂肪組織を融解し、有効成分をその中で撹拌する。この懸濁液を100個の冷却した坐剤鋳型に注いで、500mgの有効成分を含有する坐剤を得る。
実施例5.シロップ:
蒸留水40gとサッカロースを煮沸する。それを冷却した後、アロマ、着色剤、および水を加えたカルボキシメチルセルロース(蒸留水10cm3+カルボキシメチルセルロース1g)に懸濁したテオブロミンを混合し、蒸留水を加えてシロップを100gにする。このシロップ1gは250mgの有効成分を含有する。
実施例6.チョコレート香錠:
ココアパウダーのテオブロミン含量を考慮に入れて有効成分を測定し、チョコレート組成物250gにつきテオブロミン50gとする。ココアバター150gを融点のちょうど上で融解し、ココアパウダー、サッカロース、粉乳、エチルバニリンおよび当該有効成分をそれに混合する。この懸濁液に融解したココアバターを加えて250gにする。この混合物を冷却した鋳型に注ぐ。それぞれ500mgのテオブロミンを含有する100個のチョコレート香錠が得られる。
実施例7.リポソーム:
製剤手順に従って成分を組み合わせる。液相の温度をリン脂質の転移温度まで上昇させ、等しい温度の水を加える。混合物を45℃で20分間、約50RPMで撹拌し、加熱をやめて、製剤が25℃に達するまで撹拌を続ける。
