Description
本発明は、一般に、身体脈管構造、導管等の治療のための移植片(インプラント)に関する。より詳細には、本発明は小径脈管への適用で特に有用となる薄型断面形状の脈管ステントに関する。
発明の背景
疾患または他の原因で損傷を受けた脈管構造を治療する1つの方法は、従来、脈管ステント及び/または移植体(グラフト)の移植によって脈管構造の開存性を維持することであった。冠状動脈をバイパスした時か、または後日に伏在静脈移植体が部分的または完全に閉塞した時に、伏在静脈バイパス移植体にこの装置を移植することも知られている。
ワイヤステントは一般に大きな脈管での使用には許容できるが、小さな脈管では血流のために利用可能な断面積が一般に減縮されているため、ワイヤステントを使用すると脈管の管腔内部を許容できないほど侵襲することが多く、血球を損傷させ、凝固を引き起こしかねない。同様に、2つかそれ以上の重なり合う螺旋形状部材から形成されたステントは、頸動脈、冠状動脈等といった小さい脈管の管腔に侵襲の問題を起す。ワイヤから形成された移植体に関するさらなる問題は、小さなサイズの脈管を通して挿入したり、そこに配置することができる断面形状までに移植体を(例えば折り畳み、半径方向圧縮または他の縮小方法によって)縮小することが困難なことである。
一連の相互に接続された環から形成され、その環がステントの長手方向軸線に対してほぼ垂直になっているステントがさらに知られている。この種のステントは、長手方向軸線に沿って断面質量が変化するため、最も弱い位置、例えば一般には環が相互に接続された位置で曲がる傾向がある。
種々のステントとステント移植体が説明されたが、それらは前述の1つかそれ以上の欠点を含んでいる。ピンチャク(Pinchuk)の米国特許第5,163,958号明細書は、熱分解炭素の層で被覆された、螺旋状に巻かれた波状ワイヤステントを開示している。このワイヤステントには複数の概略円周状部分が含まれ、これらの部分は同じ連続する、ほぼ螺旋状に巻かれた、波状長さ部分から形成されている。
ロー(Lau)らの米国特許第5,421,955号明細書は、単 数又は複数の相互接続要素によって相互に接続された複数の半径方向に拡張可能な筒状要素から作られた拡張可 能なステントを開示している。この筒状要素は波状要素からそれぞれ形成され得る。ステント全体が1本のチューブから作られ得る。
シュネップ・ペッシュ(Schnepp−Pesch)らの米国特許第5,354,309号明細書は、周囲温度より高い体温より低い遷移温度で半径方向に広がる記憶合金部分を含むステントを開示している。このステントは、図4a〜図4bに示されるような螺旋状に巻かれたワイヤを含むことがで きる。
レビーン(Leveen)らの米国特許第4,820,298号明細書は、医療用熱可塑性材料から作られた螺旋部で構成された柔軟なステントを開示している。螺旋部の隣接するループが、エラストマーの紐によって相互に接続される。このためステントは引き延ばされて、ある程度延ばされた直線形状になり得ると共に、伸張力を開放すると螺旋形状を回復することができる。
ロー(Lau)らの米国特許第5,514,154号明細書は、単 数又は複数の相互接続要素によって相互に接続された複数の独立した半径方向に拡張可能な筒状要素から作られた拡張式ステントを開示している。この筒状要素は、波状要素からそれぞれ形成され得る。ステント全体は1本のチューブからつくられ得る。筒状要素には、拡張したステントの断面形状を大きくする半径方向に外向きに延びる掛止突起が含まれる。
要約すると、種々のステントが説明(前述)されたが、成功の度合いは様々であった。そこで必要とされ、本発明が取り組んでいるのは、比較的小さな直径の曲がりくねった経路を通じて進めるために高度の柔軟性を有し、容易に拡張させることができ、移植された管腔の開存性を維持するのに十分な機械的強度を有する一方で、管腔侵襲の量を最小にして血栓形成の危険を減らすステントである。
発明の概要
本発明は、長手軸線に沿って比較的柔軟性を有すると同時に、柱強度を増大させる構造体を備えた拡張可能なステントを含んでいる。
本発明の一つの実施態様によれば、自己拡張式ステントは、略管状形状を形成する複数の螺旋状巻回を有する構造体と、螺旋状巻回を相互に接続する橋部分であるブリッジとを備える。好ましくは、ブリッジは構造体内に螺旋状に配置される。ステントは自己拡張するのが好ま しい。しかし、バルーン拡張式ステントを含む実施態様も、本発明に含まれる。
本発明によるステントは、薄壁チューブから形成され得る。好ましくは、ステントは、当業技術分野で公知であるレーザー切断またはEDM(すなわち放電加工)技術によってチューブから切断して作られる。しかしながら、様々なエッチング技術も使用することができる。この薄壁チューブはまたステントの断面積(プロファイ ル)を小さくすることに寄与している。
ブリッジは、螺旋に沿って周上に、互いにほぼ等角に配置される。ブリッジは、螺旋状巻回360゜当たりおよそ2〜4つのブリッジとなる間隔で設けられるのが好ま しい。ブリッジは、螺旋状巻線360゜当たりおよそ3つのブリッジとなる間隔で設けられるのがより一層好まし い。
ブリッジは、略直線形状のブリッジとして形成され得る。あるいはまた、少なくとも1つのブリッジ(及びすべてのブリッジ)は所定のばね定数を有するばねを含むか、またはかかるばねの役目を果たすものであってもよい。ばねは波状ばねとして形成され得る。あるいはまた、ばねが、同等のばね定数を与える板ばねまたは他の等価なばね機構として形成されてもよい。
好ましくは、少なくとも1つのばねは、略管状形状の長手方向軸線と略平行な方向に位置合わせされる。
螺旋状構造体の螺旋状巻回とブリッジほぼ等しい幅を有することができる。あるいはまた、1つかそれ以上のブリッジの幅を変化させて、ステントの柔軟性を変えてもよい。螺旋状構造体の螺旋状巻回の幅に対するブリッジの幅を縮小する変更がなされ、ステントの柔軟性を増大させるようにするのが好ましい。
好ましくは、螺旋状構造体の巻回は、略管状形状の長手方向軸線と略平行な方向に波状になっている。本発明の小さい断面積(プロファイル)の自己拡張式ステントは、長手方向軸線を有する略管状形状を形成する複数の 巻回を有する単一螺旋状構造体を備え、その単一螺旋状構造体は薄壁チューブから形成されるのが好ましい。
また、ステントは、螺旋状構造体の巻回を相互に接続するブリッジと、巻回の波状形とを備えるのが好まし い。波状形はステントの拡張性を向上させる。さらに、ブリッジは、略管状形状の長手方向軸線と略平行な方向に位置合わせされてもよい。好ましくは、ブリッジが、その隣り合うブリッジに対して、螺旋に沿って周上にほぼ等角に配置される。
ブリッジは構造体に螺旋状に配置され得る。ブリッジは巻回360゜当たりおよそ3つのブリッジとなる比率となるように配置されるのが好ましい。ステントは、少なくとも1つの螺旋状巻回に非対称波状形をさらに備え、ステントの螺旋の性質によって発生する不均一な拡張を補償してもよい。

図1は、本発明の原理によって構成された自己拡張式ステントを例示している。比較的小さな直径の脈管で使用するためには(例えば、頸動脈、冠状動脈、伏在静脈移植体)、これまで大きな直径の脈管で使用されてきた利用可能なステントを単純に小型化するだけでは移植用 としては一般に不満足であった。例えば、ニチノールワイヤと延伸膨張ポリテトラフルオロエチレンを使用する公知の設計でサイズ(特に直径寸法)を縮小すると、半径方向の剛性が許容され得る下限を下まわるまで低下した。
ステントの柔軟性は、冠状動脈、頸動脈及び伏在静脈移植体を含むがそれらに制限されない曲がりくねった身体管腔を通るステントの搬送を容易にするが、前記管腔では、脈管が曲がりくねっているだけでなく、その直径が小さくなっている。
図1では、自己拡張式ステント10は、螺旋パターンの螺旋状巻回に形成されてほぼ筒状の管状構造11を形成し た連続的な網目パターンの正弦波状または波状の部材15を通常備えている。波状部材は波打っており、筒状構造11の長手方向軸線の方向に概略反対向きに方向付けされた曲げ部15p及び15vを形成する。ステントに柱強度及び半径方向強度とを与え、さらに拡張の際にステントの長さの変化を最小にする安定性を与えるために、波状部材によって形成された螺旋状巻回は橋部分であるブリッジ18によって接合されている。ブリッジ18は、さらに、ステント10が曲がる際のねじれ抵抗を改善させ、例えば動脈瘤を橋絡するために移植されるときにステントの曲が りを防止する。ブリッジのない螺旋状ステントは柱圧縮と曲げに対してより影響を受けやすい。この問題は、ステントまたはステント移植体が動脈瘤を形成する拡張部分をまたいで配置される動脈瘤の治療で特に注目される。ブリッジのないステントはステントの上流端部を通る血流によって加えられる力のために曲がることが多いが、この力はその端部の柱強度に対して局所的に作用する傾向がある。その結果、ステントまたはステント移植体が動脈瘤の輪郭に追従するようにステントまたはステント移植体の中央部分が曲がることになる。最終的には、ステントまたはステント移植体の上流端部が動脈瘤の頸部から外れるように引張られて動脈瘤嚢に入り、それによって血流が完全にステントまたはステント移植体を回避してしまうことがある。この位置での動脈瘤嚢の液圧隔離に失敗したことから、この結果として、ステント移植体の場合には完全な機能不全となる。
ブリッジ18は、前述のように、ステント10の軸線方向剛性と柱強度を増大させる。ステント10の上流端部を通る血流によって加えられる力は、ブリッジ18を介してステント10に沿って軸線方向に分散される。こうして、ステント10が動脈瘤にまたがっているときでさえも、ステント10を通る血流の力の一部は、動脈瘤の反対側端部のステント10の遠位端部に伝達される。遠位(下流)端部もまたステントが移植された脈管と少なくとも摩擦接触していることから、血流に逆らう力がステント10の上流端部と下流端部の両方で発生し得る。これは上流端部を脈管経路に沿って押し下げる全体的な傾向を減少させ、さらには、移植体が動脈瘤の部位に移動して拡張した脈管の経路に追従する傾向を緩和させる。たとえある程度の曲げが発生しても、螺旋の巻きの軸回方向間隔を一定 に保つ傾向を有するブリッジ18は、この状況では、ばねとして作用し、その後ステントを曲がっていない状態に回復させるよう作用するエネルギーを保存する。ブリッジのないステントは、この点で、はるかに限定された能力を有する。
ステント中のブリッジ18の数は、搬送の際のステントの断面形状を最小にすることを可能とするのに許容可能な最小値に維持されるべきである。好ましくは、螺旋一巻き(すなわち360゜)当たりおよそ2〜4つのブリッジとなる比率となるようなブリッジ構成が許容可能と考えられ、好ましい構成は、例えば図3に示されているように、螺旋一巻き当たりおよそ3つのブリッジとなる比率である。螺旋一巻き当たり3つのブリッジとなるブリッジ構成は、隣り合う巻回または巻きの間のブリッジをオフセットして配置する。こうした配置は事実上すべての方向でステントの軸線方向曲げ柔軟性を維持するが、このことは曲がりくねった経路を通して設置する場合に重要となる。
ブリッジ18は、好ましくは、波状螺旋巻回の隣り合う曲げ部15pと15vの間で相互に接続され、拡張時にステントが短くなることを防止するようにする(図3及び図4参照)。しかしながら、こうした構成は拡張時のステントの長さ維持のために絶対的に必要なわけではなく、ブリッジ18がステント全体に渡って隣り合う巻回の同じ対応する位置の間で相互に接続される限り長さはほぼ維持できることが注目される。例えば、各隣り合う巻回の曲 げ部の間の中間で隣り合う巻回間にブリッジが相互に接続され、残りのブリッジを一貫して対応するように配置することもできる。さらに、巻回は曲げ部15p及び15vが隣り合うようにすることが好ましいが(「同位相」巻 回)、他の巻回構成が可能であることに注意されたい。例えば、ブリッジが隣り合う曲げ部15p及び15pと長手方向に位置合わせされこれらを接続するように、螺旋状巻 回を配置する(「異位相」巻回)のことも可能である。さらに、他の巻回配置も可能である。
好ましくは、ステントの構造全体が薄壁チューブから形成される。この構造は、ステントが設置される脈管の管腔内で、ステントの壁厚と管腔侵襲を最小にする。同時に、柔軟性または搬送時の断面形状を犠牲にすることなく、半径方向及び長手軸線方向の強度が維持される。これは血流断面形状の破壊に関連する血球の損傷と血栓形成の危険性を最小に抑える。
ステントは、公知の化学エッチング技術を含む多くの様々な方法によって、好ましくはレーザー切断(例えばNd:Yag)によってチューブから作られ得る。本発明によるステントを作る他の好ましい方法は、当業技術分野で公知の技術である放電加工(すなわちEDM)によるものである。エッチングの好ましい方法には、ニッケル−チタンチューブのような薄壁チューブ部材を化学エッチング液に抵抗性を有する材料で被覆するステップと、被覆の一部を除去して下にあるチューブの除去すべき部分を露出させるが、ステントのための望ましいパターンでチューブの被覆された部分を残して、続いて行われるエッチングで金属チューブの露出した部分を除去してチューブのステントを形成する部分を相対的にもとのままに維 持させるステップとが含まれる。次に、エッチング液に抵抗性を有する材料が、公知の方法による機械制御レーザーによってステントから除去されてもよい。
好ましくは、ステントは、当業技術分野で公知の幾つかの技術の何れかによる電気化学的研磨の仕上げ工程を経る。このような研磨はステントの部材の全体的な寸法を減少させ、それによって研磨前の特性に対してステントを弱くするが、この作用は、単に、電気化学的研磨によって除去される材料の付加的な寸法を「あらかじめ設計に盛り込み」、最終的に望ましい寸法と強度特性を有するステントが得られるようにすることで克服される。電気化学的研磨から得られる利点は表面がより平滑になることであり、これによって血栓形成を減少させ、血流に対する抵抗を減少させて、ステントがより生体適合性を持つようにする。電気化学的研磨は、さらに、ステントの耐疲労性を向上させ、自己拡張可能ではなくバルーンカテーテルを使用した拡張が必要なステントの場合にバルーン破裂の危険を減少させる。さらに表面が平滑になることで、以下論じられるようにステントを圧縮するために使用される漏斗状筒との摩擦を低くすることができ、それによってステントの圧縮をさらに容易にする。
チューブは、ステンレス鋼、チタン、タンタル、エルギロイ(Elgiloy)(Co−Cr合金)、超弾性NiTi合金(例えば「ニチノール」)、及び高強度熱可塑性ポリマーといった適宜な生体適合性材料から作られ得る。好ま しい材料はNiTi合金であり、詳細には「二元ニチノール」(すなわち重量比が50%のNiと50%のTi)である。
所望のパターンが、すでに拡張され周知の方法によってヒートセットされたチューブから切断されるか、または小さい直径のチューブから切断された後大きい直径に拡張されてヒートセットすることによって形成される。ステントがニチノールから作られるときには、前述のヒートセットするステップが含まれる。しかし、既に述べ たように、ステントは、ステンレス鋼(例えば316Lステンレス)及び他の材料のような、自己拡張式ステントを形成せず、バルーンカテーテルによる拡張といった他の方法で拡張する必要のある材料から作成されてもよい。これらの例では、ヒートセットするステップは不必要であり、行われない。
図2に示されるように、ブリッジ18の断面形状は、この実施態様では同じ断面を有する波状部材15と同様、矩形である。この構成は、ワイヤステントによって与えられる円形断面と比較して、所定の壁厚さに対してより大きな半径方向の剛性を与える。その結果、所定の半径方向強度に対して、本発明による半径方向に薄いチューブから形成されたステントは、ワイヤから形成されたステントよりかなり薄くすることができるので、既に述べた 他の利点に加えて、管腔内断面形状を小さくし、血流のインピーダンスを減らす。波状部材とブリッジの厚さは、比較した幅が異なることがあるが、本発明のすべての実施態様において互いにほぼ等しくなっていることが注記される。
また、前述の大きな半径方向剛性のため、ステントは単一の螺旋状構造として形成することができ、この構造が管腔内断面形状を大幅に縮小させる。ステントは拡張 された形状では掛止突起を有さないが、これはさらにステントの小さい断面積(プロファイル)に寄与している。ブリッジは、ブリッジのない螺旋状構造に比較してステントに長手方向により強い剛性を与え、拡張された時の長さの変化がはるかに小さくなる。
さらに、本発明の強度、柔軟性及び拡張性は、縫合のような副次的な取り付け方法の必要性をなくし、こうした副次的取り付け方法は厚さを増加させることで管腔侵襲を増大させて管腔表面を粗くして、血流断面形状の破 壊を増大させたり、ステントの搬送断面形状に有害な影響を与える恐れがある。
また、本発明による螺旋状ステントは、図10及び図11に関連して以下で論じられるように、個々の環または他の環状構造から形成されるステントより小さい搬送断面形状に圧縮することができ、ワイヤまたは二重螺旋形タイプの構成より確実に小さくなると考えられる。
さらにまた、本発明による螺旋構成は、特に軸線方向または長手方向で、環状タイプのステントよりも柔軟であることが見出された。また、環状タイプのステントの環は独立して拡張可能であるが、これは拡張された断面形状の不連続性につながる恐れがある。それと対照的に、本発明による螺旋状ステントは連続的に拡張可能であるので、装置の拡張の際に不連続性すなわち「段」を形成する危険を冒さず、その結果、より平滑な管腔が得られる。その結果、移植されたときにステントを通してより良好な血行動態が得られ、それによって血栓形成の危険性を減少させる。
図4は、本発明によるステントの第二実施態様の平らにした部分の平面図を示している。この実施態様では、曲げ部25pと25vは第一実施態様のものより著しく鋭角なので、第一実施態様(図3参照)の比較的湾曲した曲げ 部15p、15vと比較してかどばった山と谷に近づく。図4の実施態様は拡張した状態で図3のものより強い剛性を有するステントを提供する。しかしながら、同時に、図3の実施態様は、より均一に開くので、図4の実施態様に比べて拡張状態における不規則性と間隙が少なくなる。
ステントの柔軟性の設計に付加的な制御を提供するために、ブリッジの構造が、直線支柱タイプの設計18から、図5a、図5b及び図5cがそれぞればねタイプブリッジ38a、板ばねタイプブリッジ38b及びブリッジ38cによって示しているように改変されてもよい。ばねタイプブリ ッジ38a、板ばねタイプブリッジ38b及びブリッジ38cは本発明の第一及び第二実施態様の波状部材と組み合わせて示されているが、改変されたブリッジは本願で開示された何れかの実施態様及び本発明全般に概ね適用し得ることが注記される。
図5aでは、ブリッジは波状のばねタイプブリッジ38aを形成するように改変されており、筒状ステントの長手方向軸線に整列した方向により大きな圧縮性を与える。ばねタイプブリッジ38aは、さらに、半径方向の曲げ性を増大させる(すなわち、曲げ強度を低減させる)。さらに、所定のブリッジ18だけを別個にばねタイプブリッジ38aとして設計することによって、ステントが非対称的な曲げ及び強度特性となるように特に調製することもできるということが注目される。すなわち、ゼロまたは1つのブリッジ18をばねタイプブリッジ38aとして形成するか、ステントのすべてのブリッジをそのように形成することができる。一般に、対称的な曲げ及び強度特性を与えるように、すべてのブリッジ18、またはブリッジ18の対称構造の部分がばねタイプブリッジ38aとして形成されることが好ましい。しかしながら、必ずしもこういうわけではなく、本発明はそのように制限されるものではない。
図5bは、板ばねタイプブリッジ38bを形成されるように改変されたブリッジを示しているが、これも筒状ステントの長手方向軸線と整列した方向により大きな圧縮性を与える。同様に、板ばねタイプブリッジ38bもまた半径方向の曲げ性を増大させる(すなわち、曲げ強度を低減させる)。ばねタイプブリッジ38aと同様に、所定のブリッジ18だけを板ばねタイプブリッジ38bとして別個に設計することによって、ステントが非対称的な曲げ及び強度特性となるように特に調製することもできる。すなわち、ゼロまたは1つのブリッジ18を板ばねタイプブリッジ38bとして形成するか、ステントのすべてのブリッジをそのように形成することができる。一般に、対称的な曲げ及び強度特性を与えるように、すべてのブリッジ18、またはブリッジ18の対称構造の部分が板ばねタイ プブリッジ38bとして形成されることが好ましい。しかしながら、必ずしもこういうわけではなく、本発明はそのように制限されるものではない。
上述のばねタイプの設計によってステントの圧縮性を高めることによって、結果として得られるステントの折り畳みまたは圧縮断面形状が悪い影響を受けることがあり得る。図5cは、結果として得られるステントの折り畳みまたは圧縮断面形状に悪い影響を与えることなく、圧縮性と柔軟性を増大させる第三の代替的態様を示してい る。この実施態様では、ブリッジ38cの幅38wを縮小することによって、単数又は複数のブリッジをより圧縮性及 び屈曲性が高まるようにする。したがって、ブリッジ38cの幅38wは波状部材15、25等の幅より小さくなる。この構成は結果として得られるステントの圧縮または折り畳み断面形状に悪い影響を与えないだけでなく、実際にはこの断面形状に良い影響を与えることもあり得、さらに結果として得られるステントの全体的な重量を減少させる。図5a及び図5bの実施態様と同様、ゼロまたは1つのブリッジ18を細いブリッジ38cとして形成するか、ステントのすべてのブリッジをそのように形成することができる。一般に、対称的な曲げ及び強度特性を与えるように、すべてのブリッジ18、またはブリッジ18の対称構造の部分を細いブリッジ38cとして形成することが好ましい。しかしながら、必ずしもこういうわけではなく、本発明はそのように制限されるものではない。
さらに、図5dの実施態様も、拡張された位置にあるときに結果として得られるステントの強度を増大させるために利用し得ることが注意される。これは、単数又は複 数のブリッジ18の幅を増大させて幅広いブリッジ38dを形成することによって達成される。これは一般に本発明の好ましい実施態様ではないが、ステントの設計者に利用可能な選択肢である。もちろん、ステントの強度を増大させるもう1つの選択肢として、波状部材とブリッジを含むステント全体の構造を幅広くしてもよい。ブリッジの波状部材に対する幅の比は一般に約0.5:1から約1.5:1までであり、好ましい比は約1:1以下である。
図6は、本発明のステントの第三実施態様を示してい るが、そこには、好ましくは小さな直径のチューブに切断された後、使用時の大きな直径に拡張させられて、その大きな直径でヒートセットされて自己拡張特性を与えるパターンが含まれる。例えば、図6に示される実施態 様のパターンでは、約2.0mmの直径を有するニチノールチューブに切断され、約4.0mmの直径に拡張された後ヒートセットされることができる。
この実施態様では、拡張の前に、曲げ部及び谷35p、35vは事実上半円を形成するように概略丸められていることが注記される。曲げ部及び谷35p、35vを相互に接続する部材35mは、ステントが切断される筒状チューブの長手方向軸線とほぼ整列している。しかしながら、拡張の際には、以下次の実施態様に関して論じ且つ示されているように、接続部材35mはステントの筒状形状の長手方向軸線に対してほぼ横向きになる。
前述の実施態様の他の変形は、拡張時のステントの短縮を防止するためにブリッジ18が、好ましくは、波状螺旋巻回の隣り合う曲げ部15p、15v(図3参照)の間に相互に接続されているが、ブリッジ18が接続される特定の谷35v'(図6参照)が、接続されていない曲げ部及び谷 35p、35vからわずかに変更されることが可能で、接続された谷35v'が1つのブリッジ18と共に2つのほぼ半円をブリッジ18の各側に形成するようになっている。この変更によって、筒状部分の拡張の際、ブリッジ18に関して接続部材35mが谷35v'からより均一に拡張することが可能となる。
この実施態様及び他のすべての実施態様では、曲げ部15pと15vが図1〜図11に示されるようなステントの特定の配置の影響を受けることが注記されるべきである。従って、曲げ部15pと15vがその実施態様の説明全体を通じ て矛盾なく交換される限り、本願で説明された何れかの実施態様に関連してそれらは交換できる。こうした交換は、その実施態様の詳細な説明によって参照される特定の図の配置などを逆にすることと同等である。
本発明によるステント設計の螺旋の性質は、最終製品の結果として得られる筒状構造にある程度の形の変形を必要とするが、これは以下のさらなる実施態様によって対処され得る。
図7は、図6の実施態様の変形を示しているが、そこでは、ステントを形成する筒状部分の端部が変形され、両端部が「整然とした端部」、すなわち、ステント30'の筒状形状の長手方向軸線に対してほぼ垂直な円を形成している。この「整然とした端部」を実現するために、曲げ部及び谷35p、35v(35v')を接続する部材の長さが徐々に増加され、螺旋のピッチ角を補償する(例えば、35m、35m'、35m''...の長さの推移を参照)。さらに、長くなっていく接続部材(35m'、35m''等)によって接続される曲げ部及び谷35p、35vを相互に接続するブリッジもまた段々と長くなる全体体系に従わなければならない(例えば、18、18'、...参照)。
図8はヒートセットされる拡張された状態のステント30'を示している。図6の同様の実施態様に関して認め られたように、拡張の前には、曲げ部及び谷35p、35vは事実上半円を形成するように概略丸くなっており(図7参照)、曲げ部及び谷35pと35v、35v'を相互接続する接 続部材35m、35m'、35m''は、ステント30'が切断される筒状チューブの長手方向軸線と概略整列している。しかしながら、拡張すると、接続部材35m、35m'、...はステント30'の筒状形状部分の長手方向軸線にほぼ垂直になると共に、ブリッジ18、18'...は長手方向軸線に対してほぼ平行な位置を維持する。こうして、ブリッジはステント30'を長手方向に強化する最大の潜力を維持する。
上記で説明されたステント構造の螺旋の性質によって要求される他の問題は、ステント全体を通じて一部の接 続部材35m''が、標準長さの接続部材35mと比較していくらか長くならざるを得ないことである。これは前の隣り合う螺旋状巻回から徐々に離れ、螺旋のピッチ角を補償し標準ブリッジ長さを維持するためにはいくらか長い接 続部材を必要とする螺旋状巻回の性質のためである。すべての接続部材の長さが等しいわけではないため、ステントの拡張時には、ブリッジ18と接続部材、例えば35m、35m''の間に幾つかの不均一または不均等な間隙も発生する。間隔のこうした変則性を補償するために、図9に示されるステント40には、各ブリッジの両側で一方の端部に接続する非対称な接続部材44m及び45mが含まれる。接続部材44mは接続部材45mより大きな曲率を有するので、接続部材44mの側ではより大きな度合いの拡張が可能となり、螺旋状巻回によって発生する拡張の不均一性が補償される。
既に述べたように、本発明による螺旋状ステントは、個々の環または他の環状タイプ構造から形成されるステントより小さな搬送断面形状に圧縮することができると考えられる。図10は、図9に示されている実施態様と同様の螺旋状ステントの平らに延ばした部分70を示してい るが、そこではステントが長手方向軸線と平行に長手方向に切断され、平らに延ばされてほぼ平面の構造となっている。図11は、図10に示されているステントと同じ波状パターンによって形成されたステントの平らに延ばされた部分80を示しているが、比較の目的で螺旋状構造と対立するものとしての環状構造になっている。
仮想線75及び85は、それぞれステント部分70及び80の長手方向軸線にほぼ垂直に引かれている。線75が交差する構造の合計数(ブリッジと部材を含む)は11であり、線85が交差する13の構造と比較される。この差は、図10の螺旋状構造がステントの全長に沿って構造の質量をより連続的に分散することで説明される。他方、図11に示されている環状タイプステントの質量は環部分により集中しており、環の間を接続する部分では集中度が低い。ステントを縮小できる最小断面形状は、搬送のためにステントを縮小した後で最大直径を有するステントの部分によって制限される。したがって、螺旋状ステントを通る断面の半径の中の構造数11と比較して、環状タイプステントの最大断面にはその半径内に13の構造が含まれることから、環状タイプステントの断面形状は螺旋状ステントより大きくなることが予想される。
図12a〜図12cは、展開配置のために本発明によるステントを準備すると共にステントを展開配置するための好 ましい方法で使用される様々な機器を示す。好ましく は、自己拡張式ステントは半径方向に押しつぶされるか圧縮されて、移植対象の脈管に導入するのに適した縮小された直径を有する。あるいはまた、ステントは折り畳まれて、導入段階の間折り畳まれた状態に保持されるか、またはステントが小さい直径に形成されて脈管内に導入された後でバルーンカテーテル等によって拡張されてもよい。
好ましくは、自己拡張式ステントは、以下詳細に論じられるように、漏斗状筒を通して引張ることによって圧縮される。ステントはスリーブ内で圧縮された状態に保持される。スリーブ内には図12aに示されているようなカテーテル90が配置される。カテーテル90は脈管を通し て移植位置までステントと装置全体を案内する機能を果たす。カテーテル90には、圧縮状態のステントの内径よりも大きな外径を有する拡大直径部分124が含まれる。こうして、拡大直径部分124は、圧縮されたステント95がカテーテル90の近位端部へ向かう方向に滑動することを防止する機能を果たす。カテーテル90の遠位端部は「オリーブ」91を受容する構造になっている。オリーブ91の外径は圧縮状態のステントの内径よりも大きい。したがって、オリーブ91をカテーテル90の遠位端部に付加することは、ステントを移植する前に圧縮されたステントがカテーテル90の遠位端部から滑り落ちる傾向を防止する機能を果たす。カテーテル90は好ましくはポリイミドから作られるが、このような目的に適した他の公知の同等の材料で代用することもできる。
ステントを圧縮するために漏斗状筒を通してステント95を引張るに十分な引張り力を加えるために、フィラメント96が、好ましくは、ステント95の部材の中を通して編まれ、図12bに示されているように、ステント95の両端から延びるループ97及び98に形成される。フィラメント96は好適には商業的に入手可能である縫い糸であり、好ましくはCV−7ゴアテックス縫い糸(W.L.Gore社によって製造される)である。もちろん、他の規格の縫い糸材料が代用されてもよく、ステンレス鋼ワイヤ、様々なポリマーフィラメント等の他の材料が同様に使用されてもよい。好ましくはフィラメント96より太いフィラメントが、次に、ループ97及び98を通して輪に結ばれ、それぞれ短い引張り線100と長い引張り線99を形成する。引張り線99及び100は、好ましくは、5.5ゲージ縫い糸材料から形成されるが、フィラメント96の代用品と同様、他の代用品が使用されてもよい。
スリーブ110(図12c)は、カテーテル90と同様、好ま しくはポリイミドから作られるが、この目的に適した他の公知の同等の材料で代用されてもよい。スリーブ110の内径は、圧縮状態のときのステント95の目標の外径とほぼ等しい又はわずかに大きくなるように設計される。スリーブ111の近位端部は広がって拡大操作把手112となるが、これはスリーブを引込めるためにステント95の展 開配置の際に把握されることができる。
フィラメント96をステント95と編み合わせて、引張り線99及び100を接続した後、図13aに示されるように、漏斗状筒130をスリーブ110と軸線方向に整列させることによって、ステント95の展開配置準備が続けられる。漏斗状筒130は、好ましくは、ステンレス鋼から形成されるが、ステント材料に対して低い摩擦特性を示す他の比較的剛性を有した材料が使用されてもよい。例えば、高密度熱可塑性ポリマーまたは熱硬化性ポリマーが、低摩擦内側被覆材料を塗布するかまたはしない状態で使用でき る。チタン、タンタル、銀及び金といった他の金属も使用され得る。十分に非免疫原性であることが知られており、本発明に従ってステントを圧縮するに十分な強度を示すと共に当該ステント材料に対する低い摩擦特性を示す他の任意の材料も使用できる。
漏斗状筒130は、非圧縮状態にあるときのステント95の外径よりわずかに大きい遠位端内径131を有する。漏斗状筒130の内径は遠位端内径131からスリーブ110の内径よりわずかに小さい近位端内径132へと徐々に減少するので、漏斗状筒130を通してステント95が引張られると、結果として得られる圧縮されたステント95はスリーブ110の中に容易に滑り込み、そこでステント95は圧縮された状態に維持される。
漏斗状筒130をスリーブ110と軸線方向に整列させた後、長い引張り線99を漏斗状筒130とスリーブ110に通し、図13aに示されるようにスリーブ110の近位端部から突出させる。次にステント95が漏斗状筒130と軸線方向に整列され、引張り線99を介してわずかな引張り力を加えることによってこの位置に維持される。ステント95の操作を助けて適切な軸線方向整列を確実に行うために、短い引張り線100が使用されてもよい。徐々にかつ着実に引張り線99の引張り力を増大させることによって、ステント95は、漏斗状筒130の連続的に内径が減少する表面に沿って引張られるにつれ、圧縮され始める。
漏斗状筒130の近位端部(すなわち近位内径)を通して引張られると、ステントはその最終圧縮状態よりわずかに小さい外径となるので、比較的容易にスリーブ110へ滑り込む。図13bの破線で示されているように、ステントが完全にスリーブ110へ引き込まれると、引張り力が止められる。スリーブ110に入ると、ステント95はわずかに拡張して、スリーブ110の内周に当接し、最終圧縮直径となる。ステント95からのフィラメント96の引き出しは少なくとも2つの異なった態様で達成することが できる。短い引張り線100が切断され、ループ97との連結を解かれてもよい。その後で、引張り線99がスリーブ110から引き出され、それと共にフィラメント96を引き出す。あるいはまた、引張り線99が切断され、ループ98との連結を解かれてもよい。その後で、引張り線100が漏斗状筒130から引き出され、それと共にフィラメント96を引き出す。
引張り線99、100とフィラメント96を除去した後、漏斗状筒130が除去され、スリーブ110内に圧縮されたステント95が残る。次に、図13cに示されているように、カテーテル90の近位端部が圧縮されたステント95の管状開口とスリーブ110の中に通して挿入される。カテーテル90は、拡大直径部分124が圧縮されたステント95に当接し、カテーテル90の遠位端部がスリーブ110の遠位端部とほぼ整列するまで、スリーブ110の中に完全に引き込まれる。
次に、オリーブ91が、図13dに示されているようにカテーテル90の遠位端部に固定して取り付けられ、圧縮されたステント95の遠位端方向への移動を防止するようにスリーブ110の遠位端部に当接する。オリーブ91は、好 ましくは、当業者に容易に理解され且つ利用可能である様々な周知の生体適合性接着剤の何れかを使用して、カテーテル90に接着結合される。あるいはまた、オリーブ91が、ねじ止め、熱接着、スピン溶接、または本発明の目的に沿った様々な他の公知の技術によってカテーテル90に固定され得る。この段階で、本装置は、ステント95を展開配置するため、脈管部位または身体器官への挿入に適するように完全に組み立てられている。
装置が所望の移植部位に挿入された後、手術者は操作把手112とカテーテル90の両方を把握して、ステント95の展開配置を開始する。手術者は、カテーテル90の位置を維持しながら、着実にゆっくりと操作把手112を移植部位から引き出す。その結果、スリーブ110がステント95に対して滑って徐々に連結を解かれると、拡大直径部分124はステント95の近位端部に当接することによってステント95を所望の位置に維持する。こうして、スリーブ110を移植部位から引き出す際に、拡大直径部分124によって、ステント95は所望の移植部位に残り、スリーブ110と一緒に引き出されることが防止される。
図13eは、スリーブ110がステントとの接触を解かれるとステント95が自己拡張することを示している。スリーブ110とステント95の接触が完全に除去されると、ステントは図13fに示されるように、以前の圧縮されていない形状を回復し、それによって移植された脈管の壁に当接する。次に、手術者は、移植が行われる生体からカテーテル90とオリーブ91が完全に引き出され、引き続き閉鎖処置の実行を可能にするまでカテーテル90の引き出しを開始する。
図14a〜図14cは、本発明によるステントの展開配置準備と展開配置で使用される代替装置を示す。この実施態 様では、スリーブ140は、上述の実施態様の場合のように、手術者による直接操作のために移植部位から生体の外までずっと延びるようには設計されていない。むしろ、スリーブ140はステント95よりわずかに長くなっていて、ステント95が圧縮状態で完全かつ確実にその内部に維持されることを保証しているだけである。スリーブ140は、好ましくは、ポリイミドから形成されるが、スリーブ110に関して論じたように代替材料も利用できる。
カテーテル150は展開配置前の圧縮されたステントを所定の位置に保持するために遠位オリーブ151と近位オリーブ152の両方を備える。ステント95は、スリーブ110に関する前述の説明とほぼ同じようにしてスリーブ140内で圧縮される。次に、カテーテル150がカテーテル90に関する前述の説明とほぼ同じようにして挿入され、次いで遠位オリーブ151がオリーブ91に関する前述の説明とほぼ同じようにして接続される。
カテーテル150には、スリーブ140とステント95を支持するカテーテルの遠位部分154からカテーテルを移行させるための近位移行部153と、近位移行部153に対して近 位にあるカテーテルの残りの部分であるカテーテルの近位部分155がさらに含まれる。つなぎ線または引張りひも156はスリーブ140の近位端部140aに固定される。つなぎ線または引張りひも(以下、つなぎ線とする)156はステンレス鋼ワイヤ、高強度生体適合性ポリマー繊維、又は当業技術分野で公知の同等品から形成され得る。つなぎ線156はさらに位置153aで近位移行部153に滑動可能に固定され、そこでつなぎ線156は小さい直径のカテーテルの近位部分155の内部を通る。つなぎ線156は手術者による操作のために、小さい直径のカテーテル150の近位端部(図示せず)から外に延びている。
図14bに示されているように、手術者が小さい直径のカテーテル150の遠位端部、したがってステント95を所望の位置にうまく設置すると、ステント95の展開配置が開始される。次に手術者はスリーブ140をステント95の周囲の位置から引込めるように、着実かつ徐々につなぎ線156を引張り始める。その結果、ステント95の一部が連続的に自由になっていくに連れ、ステント95が連続的に自己拡張を開始する。オリーブ152はステント95の圧縮された近位端部が小さい直径のカテーテル150に対して滑るのを防止し、ひいてはステント95がスリーブ140と共に引込むのを防止する。
スリーブ140の引込みとステント95の拡張が完了すると、小さい直径のカテーテル150、スリーブ140及びつなぎ線156を含む展開配置装置を、引き続き閉鎖処置を行うために、一体として生体から引き出すことができる。
本発明の実施態様が添付の図面と本願中で示された特定の構造を参照して説明されたが、当業者によって、以下の請求の範囲によって規定される本発明の範囲から逸脱することなく多くの修正と変更をなし得ることは明らかである。
Claims (22)
- 各々が頂点を有する複数の波状形を含み螺 旋状に延びる波状螺旋部材であって、長手方向軸線周り に配置され、前記長手軸線周りに複数の巻回を有すると 共に第1及び第2の端部を有した略管形状をなす波状螺 旋部材と、
略長手方向に延びており、1つの螺旋状巻回の少なくと も1つの頂点を隣接する螺旋状巻回の少なくとも1つの 頂点と接続する複数のブリッジ部材と、
を有する螺旋状構造体を備え、
相互に接続された頂点が前記第1の端部に向かって延び ていることを特徴とするステント。 - 前記ブリッジ部材の各々は、隣接する巻回 の波状形と同位相の巻回の波形状を相互に接続する、請 求項1に記載のステント。
- 前記ブリッジ部材が前記長手方向軸線と略平行な方向に位置合わせされている、請求項1に記載のステント。
- 前記ステントは、拡張された形状になっているとき、掛止突起の無い状態となる、請求項1に記載のステント。
- 前記ブリッジ部材が前記構造体に螺旋状に配置されている、請求項1に記載のステント。
- 前記螺旋状構造体が薄壁チューブから形成される、請求項1に記載のステント。
- 前記螺旋状構造体が前記薄壁チューブからレーザー切断される、請求項6に記載のステント。
- 前記螺旋状構造体が前記薄壁チューブから切断される、請求項6に記載のステント。
- 前記螺旋状構造体が前記薄壁チューブから放電加工プログラミングによって切断される、請求項6に記載のステント。
- 前記ブリッジ部材が、該ブリッジ部材と隣り合うブリッジ部材に関して、前記螺旋に沿って周方 向にほぼ等角に設けられる、請求項1に記載のステント。
- 前記ブリッジ部材が、前記螺旋部材の360゜当たりおよそ2〜4つのブリッジ部材となる比率となるように配置される、請求項9に記載のステント。
- 前記ブリッジ部材が、前記螺旋部材の360゜当たりおよそ3つのブリッジとなる比率となるように配置される、請求項11に記載のステント。
- 前記ブリッジ部材の少なくとも1つは略直線の柱状要素からなる、請求項1に記載のステント。
- 前記ブリッジ部材の少なくとも1つは予 め定められたばね定数を有するばね要素からなる、請求項1に記載のステント。
- 前記ばね要素は波状ばねからなる、請求項14に記載のステント。
- 前記ばね要素は板ばねからなる、請求項14に記載のステント。
- 前記ブリッジ部材の少なくとも1つが、前記略管形状の長手方向軸線に略平行な方向に位置合わ せされたばねからなる、請求項14に記載のステント。
- 前記螺旋部材と前記ブリッジ部材はほぼ等しい厚さを有する、請求項1に記載のステント。
- 前記螺旋状構造体と前記ブリッジ部材はほぼ等しい幅を有する、請求項1に記載のステント。
- 前記ブリッジ部材の少なくとも1つは前記螺旋部材の幅より実質的に小さい幅を備える、請求項1に記載のステント。
- 前記ステントは、搬送のために圧縮されることができると共に圧縮力から解放されると自己拡張することができる自己拡張式ステントからなる、請求項1に記載のステント。
- 前記ステントはバルーンカテーテルによる力の付加によって拡張することができる、請求項1に記載のステント。
