Description
よび治療方法に関するものであり、特に一般に一般にス
テントと呼ばれる動脈内補形物に関するものである。
びレーザ血管成形術、アテレクトミーおよびバイパス手
術を含む種々の治療法が使用されている。代表的なバル
ーン血管成形術においては、予成形された遠位端を有す
る案内カテーテルが通常のセルジンガー技術によって経
皮的に大腿動脈を通して患者の心臓血管系の中に導入さ
れ、この案内カテーテル遠位端が所望の冠状動脈の開口
部に着座するまで心臓動脈系の中を前進させられる。拡
張カテーテルの内腔の中にガイドワイヤが配置され、こ
れらのガイドワイヤと拡張カテーテルが前記の案内カテ
ーテルを通してその遠位端まで前進させられる。まずガ
イドワイヤが案内カテーテルの遠位端の外部に突出させ
られて、このガイドワイヤの遠位端が拡張される損傷部
位を横断するまで患者の冠状動脈の中を前進させられ、
つぎに遠位端に拡張性バルーンを備えた拡張カテーテル
が前記のガイドワイヤに沿って患者の冠状動脈の中を前
進させられ、拡張カテーテルのバルーンが損傷部位を横
断するように適正に配置される。比較的非弾性材料から
成るバルーンが損傷部位を横断する位置に達すると、比
較的高圧の(例えば4気圧の)放射線不透過性液をもっ
て所定サイズまで膨張させられ、損傷部位の動脈硬化症
粘膜斑を動脈壁の内側面に押圧し、またはその他の形で
動脈の内腔を拡張させる。つぎにバルーンを収縮させる
と、拡張された動脈を通して血行が再開し、この損傷部
位から拡張カテーテルを取り出す事ができる。拡張カテ
ーテル、ガイドワイヤおよび血管成形術の付属装置の詳

とする。
際して生じる問題点は、血管成形術の終わりにバルーン
を収縮させた時に内膜フラップが形成され、これが崩壊
して動脈を閉塞する事である。バルーン血管成形術の第
2の問題点は、多数の患者が治療された動脈中において
再狭窄症にかかる事である。再狭窄症の場合、治療され
た動脈損傷部位に再びバルーン成形術を実施し、あるい
はもしバルーン血管成形術が是認されなければバイパス
外科手術など他の治療法を実施する。しかし、バルーン
を収縮させた後に切断された動脈内皮の崩壊による冠状
動脈の部分的または完全閉塞に際して、患者は非常に危
険な状態におかれ、特に冠状動脈の場合には即刻医療を
必要とする。
主としてステントと呼ばれる内部補形物に集中されてい

装置は損傷した動脈中に、これを解放状態に保持するた
めに配置される。この装置は再狭窄症を予防し、血管治
療の直後に血管の開存を保持するために使用される。ま
た二、三の場合には、この装置は狭窄部位を拡張して拡
張状態に保持するための一次的治療装置として使用され
る。

その他の人体内腔を開いた状態に保持する。

速簡便に使用する事のできるステント送入システムであ
る。

内腔を備えた細長いスリーブと、ガイドワイヤを受ける
ように成された前記スリーブの遠位端の第1ポートと、
同じくガイドワイヤを受けるように成された遠位端から
近位端側に離間された第2ポートとを含み、これらの両
方のポートが前記スリーブの内腔と流体連通するように
成される。また前記ステント送入システムは、前記スリ
ーブの内腔の中に滑動自在に配置された血管カテーテル
を含み、このカテーテルはその遠位端に、拡張性ステン
トを外側に受けるように成された拡張性部材を受ける。
前記カテーテルは、案内部材を受ける遠位端の第1ポー
トと、前記カテーテルの遠位端から近位側に離間されて
ガイドワイヤを受けるように成された第2ポートとを有
し、これらの両方のポートがカテーテルの内腔と流体連
通するように成される。前記ガイドワイヤを受ける第2
ポートは、カテーテルの遠位端上の拡張性部材から近位
側に離間されなければならない。また、前記カテーテル
と前記スリーブの相対軸方向位置を調整して前記拡張性
部材の周囲に配置された拡張性ステントを露出し、拡張
性部材の膨張によってステントが拡張された血管壁体に
押圧されるように成す手段を含む。

ルの遠位端のポートを通して挿入する。ガイドワイヤの
近位端の挿入を容易にするため、カテーテルを送入スリ
ーブの内腔の中に配置し、カテーテルの遠位端を送入ス
リーブの遠位端のポートから突出させる。拡張性ステン
トを取り付けられたカテーテルの遠位端上の拡張性部材
が送入スリーブの内腔の中に引き戻されるように、送入
スリーブとカテーテルとの軸方向相対位置を調整する。

からスリーブとカテーテルを引き離す事ができる。スリ
ーブとカテーテルは、ガイドワイヤを露出するのに十分
な距離、案内カテーテルの近位端の外部に近位側に引き
出される。露出されたガイドワイヤ部分を例えば手で固
定すれば、スリーブとカテーテルをガイドワイヤの近位
端から引き離す事ができる。

バルーン14の外部に取り付けられている。

に詳細に示すように、カテーテル本体13の近位端38
が細長い駆動部材34の通路39の中を通り、リューエ
ルロック35の中に接着剤などの適当手段によって固着
される。ハウジング31の遠位端に回転自在に取り付け
られたキャップ33は、細長い駆動部材34の外側ネジ
山とネジ係合する内側ネジ山40を備える。キャップ3
1の回転によって、駆動部材34を軸方向に移動させ、
これによりスリーブ10と拡張カテーテル12との相対
軸方向運動を生じる。
システムは、血管内手術の結果、患者の動脈粘膜が損傷
されて粘膜が動脈通路中に崩壊して血管を通る十分な血
行を阻害する事を防止するため、粘膜を支持する必要の
ある場合に使用される。これらの場合、ガイドワイヤ2

遠位端とスリーブの遠位端が動脈損傷部位に隣接しまた
は横断するまで、ステント送入システムの前進が継続さ
れる。この時点で、ハウジング31の近位端のキャップ
33を回転させてステント送入システムの近位端のマニ
ピュレータ15を作動し、スリーブ10をカテーテル1
2に対して近位側に移動させてバルーン14上に取り付
けられたステント16を露出させる。

をガイドワイヤ26に沿って患者の体内から引き出し、
図8に図示のように拡張したステント16を損傷部位の
中に残し、この部位の開存を保持する。

マニピュレータ15を保持し、保持した手の指でキャッ
プ33を回転させて、拡張カテーテル12をスリーブ1
0の遠位端の中に戻し(または場合によってスリーブの
遠位端をカテーテル12の遠位端の上に押し込み)、つ
ぎにガイドワイヤ26を含む組立体全体を患者の体内か
ら取り出す。

ック56の中に固着され、このリューエルロックはカテ
ーテルの膨張内腔27と流体連通するので、膨張流体が
リューエルロックを通してカテーテル上のバルーン14
の中に噴射され、バルーン14とその上に取り付けられ
たステント16とを拡張させる。図10から明らかなよう
に、ハウジング51の外側面に取り付けられた要素53
の運動により、バルーン14上に取り付けられたステン
ト16を露出するために必要なスリーブ10とカテーテ
ル12との相対軸方向運動を生じる。スロット55はそ
の両端近くに、垂下要素54の幅より少し狭い狭窄部分
を有するので、滑動要素53の位置がロックされる。ハ
ウジング51の下側面は波形面57を備え、この波形面
は操作者の指を受けてハウジングの把持を容易にする。

程度に大きくする。スリーブとカテーテル軸部は通常の
ポリエチレンで形成する事ができる。
ステントを送入する場合について説明したのであるが、
このステント送入システムは、尿道または卵管などの他
の内腔の内部部位にステントを送入し、これらの内腔の
開存を保持するようにステントを拡張させるために使用
する事ができる。本発明は前記の説明のみに限定される
ものでなく、その主旨の範囲内において任意に変更実施
できる。

Claims (9)
- 【請求項1】案内部材に沿って拡張性ステントを人体内
腔の中まで送入するシステムにおいて、 a)近位端と遠位端とを有し、内部に内腔を備え、遠位
端の第1ポートおよび遠位端から近位端側に離間された
第2ポートを有し、これらの両方のポートが前記内腔と
流体連通するように成された細長いスリーブと、 b)前記スリーブの内腔の中に配置された細長いカテー
テルであって、このカテーテルは、近位端および遠位端
と、前記カテーテルの遠位端の近位側に隣接配置され拡
張性ステントを外側に受けるように成された拡張性部材
と、内腔とを有し、前記カテーテル内腔はその中に案内

成す手段とを含むステント送入システム。 - 【請求項2】前記スリーブはその第1ポートと第2ポー
トとの間に延在するスリットを有する壁体を含む事を特
徴とする請求項1に記載のステント送入システム。 - 【請求項3】前記カテーテルは、その第2ポートから拡
張性部材の近位側部位まで延在するスリットを備えた壁
体を有する事を特徴とする請求項1または2のいずれか
に記載のステント送入システム。 - 【請求項4】拡張性カテーテルとスリーブの相対軸方向
位置を調整する前記手段はマニピュレータを含み、この
マニピュレータは、 近位端および遠位端を有する細長いハウジングおよび内
部チャンバと、 ネジ孔を備え前記細長いハウジングの一端に回転自在に
取り付けられたキャップと、 外側ネジ山を有する長手方可動の駆動手段であって、こ
の駆動手段は前記細長いハウジングの内部チャンバの中
に少なくとも部分的に配置され、また前記キャップのネ
ジ孔を通して延在する遠位端を有し、前記キャップの回
転によって前記駆動部材の軸方向運動を生じるようにし
た駆動部材とを含む事を特徴とする請求項1乃至3のい
ずれかに記載のステント送入システム。 - 【請求項5】前記長手方可動駆動部材は、カテーテルの
近位端を受ける中心通路を有する事を特徴とする請求項
4に記載のステント送入システム。 - 【請求項6】カテーテルの近位端がマニピュレータハウ
ジングに固着される事を特徴とする請求項5に記載のス
テント送入システム。 - 【請求項7】前記スリーブは、マニピュレータの遠位端
から突出した長手方可動の駆動部材の遠位端に対して固
着されている事を特徴とする請求項5または6のいずれ
かに記載のステント送入システム。 - 【請求項8】a)近位端と遠位端とを有し、内部に内腔
を備え、遠位端の第1ポートおよび遠位端から近位端側
に離間された第2ポートを有し、これらの両方のポート
が前記内腔と流体連通するように成された細長いスリー
ブと、 b)前記スリーブの内腔の中に配置された細長いカテー
テルであって、このカテーテルは、近位端および遠位端
と、前記カテーテルの遠位端の近位側に隣接配置され拡
張性ステントを外側に受けるように成された拡張性部材
と、内腔とを有し、前記内腔はその中に案内部材を滑動
自在に受けまた前記カテーテルの第1ポートと第2ポー
トとの間に延在するように成された細長いカテーテル
と、 c)前記カテーテルと前記スリーブの相対軸方向位置を
調整して前記拡張性部材の膨張に際してこの拡張性部材
の周囲に配置された拡張性ステントが拡張されるように
成す手段とを含み、 前記スリーブはその第1ポートと第2ポートとの間に延
在するスリットを有する壁体を含む事を特徴とするステ
ント送入システム。 - 【請求項9】a)近位端と遠位端とを有し、内部に内腔
を備え、遠位端の第1ポートおよび遠位端から近位端側
に離間された第2ポートを有し、これらの両方のポート
が前記内腔と流体連通するように成された細長いスリー
ブと、 b)前記スリーブの内腔の中に配置された細長いカテー
テルであって、このカテーテルは、近位端および遠位端
と、前記カテーテルの遠位端の近位側に隣接配置され拡
張性ステントを外側に受けるように成された拡張性部材
と、内腔とを有し、前記内腔はその中に案内部材を滑動
自在に受けまた前記カテーテルの第1ポートと第2ポー
トとの間に延在するように成された細長いカテーテル
と、 c)前記カテーテルと前記スリーブの相対軸方向位置を
調整して前記拡張性部材の膨張に際してこの拡張性部材
の周囲に配置された拡張性ステントが拡張されるように
成す手段とを含み、 前記カテーテルは、その第2ポートから拡張性部材の近
位側部位まで延在するスリットを備えた壁体を有する事
を特徴とするステント送入システム。
