Abstract
Description

るインビボ研究は、眼の手術外傷後、プロスタグランジンレベルは最長7時間の期間、上昇したままであることを示す。以下の実施例2で説明する、フェニレフリンおよびケトロラク溶液の前房内投与後の網膜および他の眼組織におけるケトロラク濃度を決定するためのイヌにおけるインビボ研究は、術中の網膜および他の眼組織によるケトロラクの取り込みは、驚くべきことに、COX-1およびCOX-2レベルを、薬物投与後少なくとも8時間眼組織で少なくとも90%、および薬物投与後少なくとも10時間眼組織で少なくとも85%阻害するのに十分なレベルであることを示す。したがって、本発明は、補完的な散瞳および抗縮瞳効果を通して外傷を低減させることにより、かつプロスタグランジン放出を先行して阻害することにより、手術処置中に炎症を阻害し、術後シクロオキシゲナーゼレベルが最も上昇している期間中、炎症を阻害し続ける。
NSAIDおよびα-1アドレナリン作動性レセプターアゴニストを、キャリアとしての水性溶媒中に含有させて、薬物組成物または溶液を得る。上記水性キャリアは、適切には、注射用水(WFI)であり、これは、蒸留水の滅菌溶質非含有調製物である。代わりに、眼内組織に有害でなく上記製剤の安定性に有害な影響を与えない他の水性キャリアが使用され得る(例えば、脱イオン水、または安定性に対する潜在的影響について最初に評価した後には、食塩水もしくは以下に記載されるものなどの平衡塩類溶液)。
散瞳維持および術後疼痛予防のための、白内障手術および眼内レンズ置換におけるフェニレフリン1%/ケトロラク0.3%を評価する臨床研究
この実施例は、上記の表1で説明するように調合したフェニレフリン1%およびケトロラク0.3%注射の、白内障手術および眼内レンズ(IOL)置換中の散瞳維持および白内障手術および眼内レンズ置換後の術後疼痛予防のために使用された場合の、有効性および安全性を評価するために行われた2つのフェーズ3臨床研究を説明する。
術中の散瞳維持、術中の縮瞳予防、および白内障手術およびIOL置換に伴う術後初期の眼疼痛の低減のための、フェニレフリン1%およびケトロラク0.3%注射(OMS302)の使用を支持するために、2つの主要、多施設、無作為化、並行群、二重盲検、プラセボ制御フェーズ3研究(研究1および研究2)を行った。米国およびオランダの全部で20箇所からの被験体が、これらの研究に登録された。
各研究を、独立して行った。2つの研究についてのサンプルサイズ計算は同一である:各研究における全部で400人の被験体(治療アーム当たり200人の被験体)は、両側t検定を用いてα=0.05で、ベースラインからの平均曲線下面積(AUC)瞳孔直径変化における0.6mm(標準偏差[SD]:0.7mm)の差を検出するための99%検出力、および術後最初の12時間の眼疼痛VASの平均AUCにおける5.0mm(SD:13.3mm)の差を検出するための96%検出力を提供した。
OMS302は、白内障手術またはIOL置換処置中の散瞳の維持でプラセボより優れていた。瞳孔直径の決定のための使用可能なビデオ画像がある759人の被験体のうち、瞳孔直径のベースラインからの平均AUC変化は、プラセボ群(n=380)の-0.50mmと比較してOMS302群(n=379)について0.08mmであり、CMH加重平均差(OMS302-プラセボ)(標準誤差[SE])は0.58mm(0.04)(95%信頼区間[CI]:0.51、0.65;p<0.0001)であった。手術開始後、ベースライン瞳孔直径はOMS302治療で維持されたが、一方で、プラセボ治療では進行性の縮瞳が観察された(図1)。また、皮質浄化の完了時に、および手術中の任意の時間に6mm未満の瞳孔直径を有する被験体の発生率を評価する二次有効性分析の結果は、OMS302治療に有利であった(表2)。皮質浄化完了時の6mm未満の瞳孔直径、手術中の任意の時間の6mm未満の瞳孔直径、および2.5mm以上の術中瞳孔収縮度を有する被験体の割合は全て、プラセボ治療被験体よりOMS302治療被験体で顕著に低かった(各エンドポイントについてp<0.0001)。1mmを超える術中瞳孔収縮を経験したOMS302治療被験体は、プラセボ被験体よりかなり少なかった(図2)。
統合安全性分析に含まれる808人の被験体(403人のOMS302、405人のプラセボ)のうち、513人(63.5%)は、少なくとも1回の治療で発現した有害事象(TEAE)を経験した。TEAEを報告した被験体の割合は、プラセボ(271/405[66.9%])よりOMS302を与えた被験体(242/403[60.0%])で、わずかに低かった。TEAEの大部分は、重症度が軽度または中程度であった。2つの研究において、ただ1つの重篤な有害事象が報告された。また、この事象(研究薬物に無関係であると思われる感電による死亡)は、研究からの早期の中断をもたらした唯一の事象であった。
OMS302は、IOL置換中の散瞳維持、およびIOL置換後の眼疼痛の低減についてプラセボより優れていた。瞳孔直径のベースラインからの平均曲線下面積(AUC)変化は、プラセボの-0.50mmと比較して、OMS302について0.08mmであった(p<0.0001)。術後12時間以内の被験体の眼疼痛の視覚的アナログスケール(VAS)スコアの平均AUCは、プラセボ(平均AUC=9.06mm)より、OMS302(平均AUC=4.16mm)について50%超低かった(p<0.001)。全ての二次有効性分析の結果は、OMS302と関連づけられる顕著な治療効果を示した。治療で発現した有害事象は、IOL置換を受けた集団について予想されるとおりであり、安全性尺度における臨床的有意差は治療群間で観察されなかった。
眼内レンズ置換中のイヌへのフェニレフリン1%/ケトロラク0.3%投与後のケトロラクの眼組織分布
この実施例は、イヌにおけるIOL置換中の、表1で説明するように調合したフェニレフリン1%およびケトロラク0.3%注射(OMS302)の前房内投与後の、網膜および他の眼組織中のケトロラク濃度を決定するための、イヌにおけるインビボ研究の結果を説明する。
20匹の雌ビーグルで、超音波水晶体乳化吸引によるIOL置換を行った。処置中に、OMS302を、灌注および処置直後の前房内注射によりBSS溶液に投与した。ケトロラクの標的用量レベルは5.71mg/眼であり、BSS溶液中に希釈したOMS302の標的用量体積は250mL/眼であった。処置後0、2、6、8、および10時間で、時点当たり4匹の動物を屠殺した。血液および房水のサンプルを回収した。眼球除去した眼を、網膜、網膜色素上皮-脈絡膜、角膜、虹彩-毛様体、硝子体液、強膜、および水晶体嚢の回収のために凍結および解剖した。ケトロラクの組織濃度を、液体クロマトグラフィー/質量分析(LCMS)法を用いて定量化した。ケトロラクによるシクロオキシゲナーゼ(COX)阻害の公表されているIC50値(Waterburyら、Curr Med Res Opin、22巻(6号):1133~40頁(2006年))を用いて、阻害パーセントの見積もりを各時点について得た。
図4~6は、OMS302の前房内用量後の特定の時点での平均ケトロラク濃度を示し、図4は、角膜、水晶体嚢、虹彩-毛様体(ICB)、房水、および前部強膜中のケトロラク濃度を示し、図5は、眼球結膜および眼瞼結膜中のケトロラク濃度を示し、図6は、硝子体液、網膜、脈絡膜-RPE(末梢)、脈絡膜-RPE(輝板)、および後部強膜中のケトロラク濃度を示す。図7は、COX-1について20nMのIC50および10nMのKi、ならびにCOX-2について120nMのIC50および60nMのKiに基づいた、t=0~t=10時間での網膜組織におけるCOX-1およびCOX-2の平均阻害パーセントを示す。網膜中のケトロラク濃度は、IOL置換の終了直後に1400±1004ng/g、および処置後8時間で164±39ng/gであり、t=0で99.3%/96.0%、およびt=8時間で98.4%/91.1%の見積もられたCOX-1/COX-2阻害に対応した。網膜半減期は、約3.8時間であった。驚くべきことに、t=8時間での房水、硝子体液、およびRPE-脈絡膜中の組織濃度は、COX-1およびCOX-2の90%超の阻害と一致した。また、驚くべきことに、t=10時間で、網膜組織濃度はCOX-1について97.74%(0.36%の標準偏差)、およびCOX-2について87.82(1.75%の標準偏差)であった。ケトロラクの平均血漿レベルは、t=0で4.73±1.46ng/mLであり、t≧2時間では検出不能なレベルまで低下した。
この研究では、IOL置換手術中のOMS302の使用は、前房内灌注溶液中での薬物投与後に、少なくとも8時間にわたり90%を超え、少なくとも10時間にわたり85%を超えて、眼内組織でCOX-1およびCOX-2レベルを阻害するのに十分なレベルでの、網膜および他の眼組織によるケトロラクの取り込みをもたらし、その活性持続時間は予想外であった。全身曝露は低く、一過性であった。
白内障手術前の局所的ケトロラク投与後の前房内ケトロラク濃度を評価する臨床研究
この実施例は、白内障手術前に局所的ケトロラクを与えた被験体における術後前房内ケトロラク濃度を決定するための臨床研究の結果を説明する。
白内障摘出とレンズ置換(CELR)を受ける患者が適格であった。14人の被験体から書面でのインフォームドコンセントを得、その各々に外科医の通常の実施に従って、手術1日前から始めて、局所的眼科ケトロラクを与えた。最初の外科的切開直前に、外科医は30ゲージのツベルクリンシリンジで、手術中の眼から房水の100μLサンプルを取り出した。前眼房の最終的再膨張および創傷閉鎖前のCELRの終わりに、外科医は前眼房から別の100μLサンプルを取り出した。前房内流体サンプルのケトロラク濃度を、分析研究室で分析した。
14人の被験体のうちの13人が手術前日に4用量のケトロラクを使用し、1人の被験体が手術前日に3用量を使用した。全ての14人の被験体に、手術日に手術センターで局所的ケトロラクを与えた。不注意で、2人の被験体から房水サンプルが回収されなかった。サンプルを回収した12人の被験体の術前ケトロラク濃度は4.9~369ng/mLの範囲であった。処置の最後のサンプルは、1.0未満(定量化の下限またはLLOQ)~6.32ng/mLの範囲であり、12人の被験体のうちの8人は、LLOQ未満のケトロラクレベルを有した。
局所的ケトロラクの在宅コンプライアンスは概して良く、92.9%の被験体が指示されたとおりに局所的ケトロラクを使用した。CELR後、手術処置の最後の房水中のケトロラクレベルは、おそらく灌注洗浄のために低かった。なぜなら、被験体の66.7%が検出不能な濃度のケトロラクを有したからである。
