Description

細書中に提供される。治療用組成物または薬学的組成物は、1つまたは複数の溶解ポリペプチド(複数可)を含み得、1つまたは複数の抗生物質と併用され、好適な賦形剤、キャリアまたはビヒクルと必要に応じて組み合わされる、天然、トランケート型、キメラまたはシャフリングした溶解酵素を必要に応じて含み得る。本発明は、細菌感染症または関連する状態を含むグラム陽性菌の殺滅、軽減、除菌、予防または処置において使用するための、抗生物質と組み合わされる、PlySs2を含む溶解素の治療用組成物または薬学的組成物を提供する。本発明は、細菌感染症または関連する状態を含むグラム陽性菌の殺滅、軽減、除菌、予防または処置において使用するための、バンコマイシン、リネゾリドまたはダプトマイシンと組み合わされる、PlySs2を含む溶解素の治療用組成物または薬学的組成物を提供する。本発明は、細菌感染症または関連する状態を含むグラム陽性菌の殺滅、軽減、除菌、予防または処置において使用するための、ダプトマイシンと組み合わされる、PlySs2を含む溶解素の治療用組成物または薬学的組成物を提供する。ダプトマイシンを含む抗生物質と組み合わせてPlySs2溶解素(その短縮化物またはバリアントを含む)を含む組成物は、特に、Streptococcus pyogenesおよび抗生物質耐性Staphylococcus aureusを含む、Streptococcus、Staphylococcus、EnterococcusまたはListeriaの細菌感染症または関連する状態を含むグラム陽性菌の殺滅、軽減、除菌、予防または処置において使用するために本明細書中に提供される。
わち、1つまたは複数の従来の抗生物質を含む。抗生物質は、細胞壁ペプチドグリカン生合成に影響するものおよびグラム陽性菌におけるDNA合成またはタンパク質合成に影響するものに広く下位分類され得る。ペニシリンおよびそのような抗生物質を含む細胞壁合成阻害剤は、堅い外側の細胞壁を破壊し、その結果、相対的に支えられていない細胞は膨張し、最終的には破裂する。補完的な作用物質は、溶解酵素の治療効果を相乗的に増強するのに有効である量の、抗生物質(例えば、エリスロマイシン、クラリスロマイシン、アジスロマイシン、ロキシスロマイシン、マクロライドファミリーの他のメンバー、ペニシリン類、セファロスポリン類およびそれらの任意の組み合わせ)であり得る。実質的に他の任意の抗生物質が、変更されたおよび/または変更されていない溶解酵素とともに使用され得る。細胞壁ペプチドグリカン生合成に影響する抗生物質には:N-アセチルムラミン酸(NAM)およびN-アセチルグルコサミン(NAG)ペプチドサブユニットをペプチドグリカンマトリックスに組み込むことを妨げることによってペプチドグリカン合成を阻害するグリコペプチド類が含まれる。利用可能なグリコペプチド類としては、バンコマイシンおよびテイコプラニン;ペプチドグリカン架橋の形成を阻害することによって作用するペニシリン類が挙げられる。ペニシリン類の官能基であるβ-ラクタム部分は、細菌中でペプチドグリカン分子を連結するDD-トランスペプチダーゼに結合して、それを阻害する。加水分解酵素は、細胞壁を壊し続け、浸透圧に起因して、細胞溶解または細胞死をもたらす。一般的なペニシリン類には、オキサシリン、アンピシリンおよびクロキサシリン;ならびに原形質膜の外側にペプチドグリカン基本要素を有する分子であるC55-イソプレニルピロホスフェートの脱リン酸化に干渉するポリペプチドが含まれる。細胞壁に悪影響を及ぼすポリペプチドは、バシトラシンである。他の有用なおよび関与性の抗生物質としては、バンコマイシン、リネゾリドおよびダプトマイシンが挙げられる。
中性の塩が挙げられる。グリセリンまたはグリセロール(1,2,3-プロパントリオール)は、薬学的使用のために商業的に入手可能である。DMSOは、局所的に適用される多くの薬物の透過を高める注目すべき能力を有する非プロトン性溶媒である。特に、静脈内溶液が調製されるとき、キャリアビヒクルには、リンガー溶液、緩衝液およびデキストロース溶液も含まれ得る。
D-グルコピラノシド、および生物学的に存在する界面活性物質、例えば、脂肪酸、グリセリド、モノグリセリド、デオキシコレートおよびデオキシコレートのエステルが挙げられる。
粧品業界および製薬業界において頻繁に使用される親水性およびハイドロアルコール(hydroalcoholic)のゲル化剤が含まれる。他の好ましいゲル化ポリマーとしては、ヒドロキシエチルセルロース、セルロースガム、MVE/MAデカジエンクロスポリマー、PVM/MA共重合体またはそれらの組み合わせが挙げられる。

for dilutional antimicrobial sensitivity tests for bacteria that grow aerobically.Volume 32(Wayne[PA]:Clinical and Laboratory Standards Institute[US],2012)に記載されているように、ブロス微量希釈法を使用して決定した。この値は、MICアッセイにおいて還元剤(例えば、DTTまたはBMS)の存在下で決定されるMICである。還元剤は、MIC値を決定する際のアッセイ間の再現性を改善する目的で加えられる。

の異なるMSSAおよび42個のMRSA菌株にわたって1×MIC薬物濃度を試験する時間-殺菌アッセイ(Mueller Mら(2004)Antimicrob Agents Chemotherapy 48:369-377)を使用した。PlySs2は、30分以内に殺菌レベルに達した(Methods for dilution antimicrobial susceptibility tests for bacteria that grow aerobically.Vol.32(Wayne(PA):Clinical and Laboratory Standards Institute(US),2012)(CFUの≧3log10減少)(図4AおよびB)。対照的に、ダプトマイシンは、殺菌レベルに達するために6時間を要し、バンコマイシンおよびオキサシリンは、6時間以内に2log10の殺滅しか達成しなかった。迅速な殺滅動態は、15個の異なる最近のMSSA分離菌(図4C)またはMRSA分離菌(図4D)のセットに対するPlySs2の場合にも得られ、関与性の臨床分離菌に対するPlySs2の効率的な殺菌活性が例証された。PlySs2の強力な活性は、処理のわずか15秒後にS.aureus cocciの広範な溶菌を示す電子顕微鏡検査によってさらに例証され;PlySs2の作用の速度は、接触直後の殺菌効果と一致する(図4E)。

実施例3
aerobically,Vol.32(Wayne(PA):Clinical and Laboratory Standards Institute(US),2012)。ペニシリンファミリーのメンバーであり、バンコマイシンまたはダプトマイシンのいずれとも異なる様式で細菌を殺滅する抗生物質オキサシリンを用いて、相乗作用研究をさらに調べた。単独または溶解素PlySs2の存在下でのオキサシリン研究の結果を図8に示す。臨床MRSA分離菌(図8C~8F)またはMSSA分離菌(図8A~8B)に対して、MIC未満の濃度のPlySs2、ダプトマイシン、バンコマイシンおよびオキサシリンを単独または併用で使用して、時間殺菌曲線を作成した。相乗作用は、最も活性な単剤と比較したときの、併用についての、6時間の時点におけるCFU/mLの≧2log10減少と定義された。この基準に基づくと、PlySs2は、評価したすべての代表的なMRSAおよびMSSA菌株に対して評価したすべての抗生物質と相乗的に作用した(図4~8を参照のこと)。拡大した分離菌セットを同様に調査したところ、オキサシリン、バンコマイシンおよびダプトマイシンを含む異なる抗生物質と併用されたPlySs2を用いたとき、MSSAについては49個の解析中45個において、MRSAについては26個の解析中24個において、相乗作用が観察された(下記に提供される表7~11)。

4相乗作用は、CLSI21によって、CFU/mLの≧2log10の減少と定義される。相加的な相互作用は、CFU/mLの<2log10の減少と定義される。
5xMICは、各濃度によって表されるMICのパーセンテージを表す。例えば、0.5というxMIC値は、特定の薬物についての最適な相乗作用濃度が、特定の分離菌または菌株の特定のMIC値の1/2であることを意味する。ゆえに、xMIC値は、相乗作用時間殺菌アッセイにおいて使用された薬物濃度を、還元剤の非存在下の特定の菌株に対するその薬物のMICで除算した値である。

チェッカーボードアッセイでは、薬物相互作用を、各組み合わせについてのFICminに基づいて、相乗作用、相加作用または拮抗作用のいずれかと定義する。薬物についてのFICは、併用の薬物のMICを、単独で使用された薬物のMICで除算したものと定義される。FICminは、各薬物についてのFICの合計に基づく。FICminが≦0.5である場合、その併用は、相乗作用と解釈され;>0.5~≦2の場合、相加作用と解釈され;>2の場合、拮抗作用と解釈される。

養物については、25日間の連続継代実験の終わりのバンコマイシンのMIC値は、有意に低かった(4、4および2μg/ml(2~4倍増加))。ゆえに、MIC未満の濃度のPlySs2は、バンコマイシン単独の条件と比べて、MRSA菌株がバンコマイシン耐性を増加させる能力を4~8倍抑制した。ダプトマイシンに対する耐性は、PlySs2溶解素の存在下では、たった約4倍しか増加しなかった。

- グラム陽性菌を殺滅する方法であって、該方法は、PlySs2溶解素と1つまたは複数の抗生物質との組み合わせと該細菌とを接触させる工程を含み、ここで、S.aureusを含むグラム陽性菌を抗生物質の存在下で殺滅するのに有効であるために必要とされるPlySs2の量は、抗生物質の非存在下より有意に少なく、かつ/またはS.aureusを含むグラム陽性菌をPlySs2の存在下で殺滅するのに有効であるために必要とされる抗生物質の量は、PlySs2の非存在下より有意に少ない、方法。
- S.aureusを含むグラム陽性菌をPlySs2の存在下で殺滅するのに有効であるために必要とされる抗生物質の量が、PlySs2の非存在下より有意に少ない、請求項1に記載の方法。

- グラム陽性抗生物質(gram-positive antibiotic)の有効性を高める方法であって、該方法は、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含むPlySs2溶解素とともに該抗生物質を投与する工程を含み、ここで、該抗生物質は、Plyss2との併用において少なくとも10倍有効である、方法。
- 前記抗体が、PlysS2溶解素との併用において少なくとも16倍有効である、請求項16に記載の方法。
- 前記抗体が、PlysS2溶解素との併用において少なくとも50倍有効である、請求項16に記載の方法。

- サーファクタント様活性を有する生物学的流体中のグラム陽性菌に対する抗生物質活性を増強する方法であって、該方法は、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含むPlySs2溶解素と併用して抗生物質を投与する工程を含み、ここで、該抗生物質は、該抗生物質がPlySs2の非存在下では無効である用量でもPlySs2との併用において有効である、方法。
- 前記抗生物質が、ダプトマイシンまたは関連化合物である、請求項19に記載の方法。
- 前記細菌が、S.pneumoniaeである、請求項19または20に記載の方法。
- Staphylococcus、Streptococcus、EnterococcusおよびListeriaから選択されるグラム陽性菌を阻害する際に使用するための、PlySs2溶解素ポリペプチドおよび1つまたは複数の抗生物質を含む組成物。
