Abstract
Description
本発明は、概して、溶解素、特に、連鎖球菌の溶解素、特に、溶解素PlySs2と1つまたは複数の抗生物質との組み合わせを用いた、ブドウ球菌を含むグラム陽性菌の防止、回復および処置に関する。
多種多様の疾病および他の状態に対して使用する抗生物質が増えるにつれ、薬物耐性菌が発生することが、医療における大きな問題である。より多くの抗生物質を使用することおよび耐性を示す細菌の数のせいで、処置時間が長くなる。さらに、広域の非特異的抗生物質が、現在、頻繁に使用されているが、そのうちのいくつかは、患者に有害作用を及ぼす。この使用の増加に関連する問題は、多くの抗生物質が、粘液内層(mucus lining)を容易に透過しないということである。

本願は、グラム陽性菌の迅速かつ有効な殺滅のためのバクテリオファージ溶解素(複数可)と抗生物質との組み合わせに関する。本発明によると、複数の細菌、特に、StaphylococcusおよびStreptococcusの菌株を含むグラム陽性菌に対する広範な殺滅活性を示す溶解素PlySs2は、抗生物質(複数可)との併用において、注目すべき相乗作用を提供し、抗生物質(複数可)に対して必要とされる有効MIC用量を有意に減少させ得る。

aerobically,Vol.32(Wayne(PA):Clinical and Laboratory Standards Institute(US),2012)。ペニシリンファミリーのメンバーであり、バンコマイシンまたはダプトマイシンのいずれとも異なる様式で細菌を殺滅する抗生物質オキサシリンを用いて、相乗作用研究をさらに調べた。単独または溶解素PlySs2の存在下でのオキサシリン研究の結果を図8に示す。臨床MRSA分離菌(図8C〜8F)またはMSSA分離菌(図8A〜8B)に対して、MIC未満の濃度のPlySs2、ダプトマイシン、バンコマイシンおよびオキサシリンを単独または併用で使用して、時間殺菌曲線を作成した。相乗作用は、最も活性な単剤と比較したときの、併用についての、6時間の時点におけるCFU/mLの≧2log10減少と定義された。この基準に基づくと、PlySs2は、評価したすべての代表的なMRSAおよびMSSA菌株に対して評価したすべての抗生物質と相乗的に作用した(図4〜8を参照のこと)。拡大した分離菌セットを同様に調査したところ、オキサシリン、バンコマイシンおよびダプトマイシンを含む異なる抗生物質と併用されたPlySs2を用いたとき、MSSAについては49個の解析中45個において、MRSAについては26個の解析中24個において、相乗作用が観察された(下記に提供される表7〜11)。

5xMICは、各濃度によって表されるMICのパーセンテージを表す。例えば、0.5というxMIC値は、特定の薬物についての最適な相乗作用濃度が、特定の分離菌または菌株の特定のMIC値の1/2であることを意味する。ゆえに、xMIC値は、相乗作用時間殺菌アッセイにおいて使用された薬物濃度を、還元剤の非存在下の特定の菌株に対するその薬物のMICで除算した値である。

チェッカーボードアッセイでは、薬物相互作用を、各組み合わせについてのFICminに基づいて、相乗作用、相加作用または拮抗作用のいずれかと定義する。薬物についてのFICは、併用の薬物のMICを、単独で使用された薬物のMICで除算したものと定義される。FICminは、各薬物についてのFICの合計に基づく。FICminが≦0.5である場合、その併用は、相乗作用と解釈され;>0.5〜≦2の場合、相加作用と解釈され;>2の場合、拮抗作用と解釈される。

S.aureusを含むグラム陽性菌をPlySs2の存在下で殺滅するのに有効であるために必要とされる抗生物質の量が、PlySs2の非存在下より有意に少ない、項目1に記載の方法。
(項目3)
S.aureusを含むグラム陽性菌を抗生物質の存在下で殺滅するのに有効であるために必要とされるPlySs2の量が、抗生物質の非存在下より有意に少ない、項目1に記載の方法。
(項目4)
前記PlySs2溶解素が、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、前記抗生物質の存在下で前記グラム陽性菌を殺滅するのに有効である、そのバリアントを含む、項目1〜3のいずれかに記載の方法。
(項目5)
前記抗生物質が、バンコマイシンまたは関連抗生物質、リネゾリドまたは 関連抗生物質、ダプトマイシンまたは関連抗生物質およびオキサシリンまたは関連抗生物質のうちの1つまたは複数から選択される、項目1〜3のいずれかに記載の方法。
(項目6)
前記抗生物質が、ダプトマイシン、バンコマイシン、リネゾリドまたはオキサシリンである、項目1〜3のいずれかに記載の方法。
(項目7)
抗生物質耐性グラム陽性菌を阻害する方法であって、該方法は、グラム陽性菌を殺滅することができる溶解素および1つまたは複数の抗生物質と該細菌とを接触させる工程を含む、方法。
(項目8)
前記溶解素が、PlySs2である、項目7に記載の方法。
(項目9)
前記溶解素が、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含む、項目7に記載の方法。
(項目10)
前記細菌が、StaphylococcusおよびStreptococcusから選択される、項目7に記載の方法。
(項目11)
前記細菌が、Staphylococcus aureusである、項目7に記載の方法。
(項目12)
グラム陽性広域スペクトル抗生物質とグラム陽性菌を殺滅することができる溶解素との組み合わせを投与することによって、該広域スペクトル抗生物質を投与した際の抗生物質耐性の発生を減少させる方法。
(項目13)
前記抗生物質と前記溶解素の両方が、最小有効量より少ない用量で投与される、項目12に記載の方法。
(項目14)
前記抗生物質が、推奨用量より低い(lower that)用量で投与される、項目12に記載の方法。
(項目15)
前記溶解素が、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含む、項目12に記載の方法。
(項目16)
グラム陽性抗生物質(gram−positive antibiotic)の有効性を高める方法であって、該方法は、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含むPlySs2溶解素とともに該抗生物質を投与する工程を含み、ここで、該抗生物質は、Plyss2との併用において少なくとも10倍有効である、方法。
(項目17)
前記抗体が、PlysS2溶解素との併用において少なくとも16倍有効である、項目16に記載の方法。
(項目18)
前記抗体が、PlysS2溶解素との併用において少なくとも50倍有効である、項目16に記載の方法。
(項目19)
前記PlySs2溶解素が、抗体との併用において少なくとも2倍有効である、項目16に記載の方法。
(項目20)
前記PlySs2溶解素が、抗体との併用において少なくとも4倍有効である、項目16に記載の方法。
(項目21)
サーファクタント様活性を有する生物学的流体中のグラム陽性菌に対する抗生物質活性を増強する方法であって、該方法は、図29に提供されているアミノ酸配列(配列番号1)、または図29のポリペプチド(配列番号1)に対して少なくとも80%の同一性、85%の同一性、90%の同一性、95%の同一性もしくは99%の同一性を有し、グラム陽性菌を殺滅するのに有効である、そのバリアントを含むPlySs2溶解素と併用して抗生物質を投与する工程を含み、ここで、該抗生物質は、該抗生物質がPlySs2の非存在下では無効である用量でもPlySs2との併用において有効である、方法。
(項目22)
前記抗生物質が、ダプトマイシンまたは関連化合物である、項目19に記載の方法。
(項目23)
前記細菌が、S.pneumoniaeである、項目19または20に記載の方法。
(項目24)
Staphylococcus、Streptococcus、EnterococcusおよびListeriaから選択されるグラム陽性菌を阻害する際に使用するための、PlySs2溶解素ポリペプチドおよび1つまたは複数の抗生物質を含む組成物。

前記抗生物質が、バンコマイシンまたは関連抗生物質、リネゾリドまたは関連抗生物質、オキサシリンまたは関連抗生物質およびダプトマイシンまたは関連抗生物質から選択される、項目24に記載の組成物。

前記抗生物質が、ダプトマイシン、バンコマイシン、オキサシリンまたはリネゾリドである、項目24または25に記載の組成物。
(項目27)
前記抗生物質の用量が、通常の臨床用量より少なくともX倍低い、項目24または25に記載の組成物。

- StaphylococcusおよびStreptococcus細菌を殺滅するための組み合わせ物であって、該組み合わせ物は、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅するのに有効であるPlySs2溶解素と、1つまたは複数の抗生物質とを含み、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、
ここで、S.aureusを含むStaphylococcusおよびStreptococcus細菌を該抗生物質の存在下で殺滅するのに有効であるために必要とされるPlySs2の最小阻止濃度(MIC)用量は、該抗生物質の非存在下より有意に少なく、かつS.aureusを含むグラム陽性菌をPlySs2の存在下で殺滅するのに有効であるために必要とされる該抗生物質のMIC用量は、PlySs2の非存在下より有意に少ない、組み合わせ物。 - StaphylococcusおよびStreptococcus細菌を殺滅するための組成物であって、該組成物は、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅するのに有効であるPlySs2溶解素を含み、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、

- 抗生物質耐性StaphylococcusおよびStreptococcus細菌を阻害するための組み合わせ物であって、該組み合わせ物は、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅するのに有効であるPlySs2溶解素と、1つまたは複数の抗生物質とを含み、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、

- StaphylococcusおよびStreptococcus細菌の抗生物質耐性の発生を減少させるための組み合わせ物であって、該組み合わせ物は、グラム陽性広域スペクトル抗生物質と、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅することができるPlySs2溶解素とを含み、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、該抗生物質および該溶解素の両方は、MIC用量で、もしくはMIC用量未満で投与されることを特徴とする、組み合わせ物。
- StaphylococcusおよびStreptococcus細菌の抗生物質耐性の発生を減少させるための、グラム陽性広域スペクトル抗生物質と、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅することができるPlySs2溶解素とを含む組成物であって、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、該抗生物質および該溶解素の両方は、MIC用量で、もしくはMIC用量未満で投与されることを特徴とする、組成物。
- StaphylococcusおよびStreptococcus細菌の抗生物質耐性の発生を減少させるための組成物であって、該組成物は、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつStaphylococcusおよびStreptococcus細菌を殺滅することができるPlySs2溶解素を含み、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、該組成物は、グラム陽性広域スペクトル抗生物質と併用され、該抗生物質および該溶解素の両方は、MIC用量で、もしくはMIC用量未満で投与されることを特徴とする、組成物。
- 前記抗生物質と前記溶解素の両方が、MIC用量より少ない用量で投与される、請求項10に記載の組み合わせ物、または請求項11もしくは12に記載の組成物。
- 前記抗生物質が、MIC用量より低い用量で投与される、請求項10に記載の組み合わせ物、または請求項11もしくは12に記載の組成物。
- グラム陽性抗生物質のStaphylococcus細菌及びStreptococcus細菌に対する有効性を高めるための組成物であって、該組成物は、PlySs2溶解素を含み、該組成物は、グラム陽性抗生物質と併用されることを特徴とし、該PlySs2溶解素は、配列番号1のポリペプチドに対して少なくとも80%の同一性を有するアミノ酸配列を含み、かつグラム陽性菌を殺滅するのに有効であり、ここで、該PlySs2溶解素は配列番号1のポリペプチドに対して90%未満の同一性を有するアミノ酸配列を含み、ここで、該抗生物質は、PlySs2との併用において少なくとも10倍有効である、組成物。
- 前記抗生物質が、PlySs2溶解素との併用において少なくとも16倍有効である、請求項15に記載の組成物。
- 前記抗生物質が、PlySs2溶解素との併用において少なくとも50倍有効である、請求項15に記載の組成物。
- 前記PlySs2溶解素が、抗生物質との併用において少なくとも2倍有効である、請求項15に記載の組成物。

- 前記抗生物質が、ダプトマイシンである、請求項1、4〜6、9、10、13および14のいずれかに記載の組み合わせ物、または請求項2〜5、7〜9、および11〜20のいずれかに記載の組成物。
- 前記Streptococcus細菌が、S.pneumoniaeである、請求項21に記載の組み合わせ物または組成物。
- 前記抗生物質耐性細菌が、抗生物質感性細菌と比較して増加したPlySs2溶解素への感受性を有する、請求項10に記載の組み合わせ物、または請求項11もしくは12に記載の組成物。
