Abstract
Description

を含む、対象の生物試料におけるFimH発現プロテオバクテリアの存在を検出するためのin vitro方法に関する。

b)前記ヌクレオチド分画におけるfimオペロン転写のOFFからONへのスイッチをqPCRによって測定することによってfimH遺伝子の発現を検出する工程
を含む、対象の便試料におけるFimH発現プロテオバクテリアの存在を検出するための、又はその腸管に多量のFimH発現プロテオバクテリアを保有する対象を同定するためのin vitro方法に関する。

- ヘプチルマンノース(HM)は、最も効率的なFimHアンタゴニスト及び強力なin vitro AIEC接着阻害剤の1つである(Bouckaertら、2005、Bouckaertら、2013)。HMは一般的に、抗接着アッセイにおいて基準として使用されているが、in vivoでは効果を有しないことが証明されている。実際に、マウス膀胱炎モデルにおいて有意な細菌負荷の低減を観察するためにはミリモル濃度が必要であり(Wellensら、2008)、CEABAC 10クローン病モデルではAIECに関して効果を有しなかった(Sivignon 2015)。

本発明者らは、補完的診断として使用されるFimHスイッチ機構に基づいてqPCRアッセイを開発した。
1.FimSスイッチ機構とEB8018に対する凝集との間の連鎖を実験によって確認する、
2.便試料においてFimSスイッチ機構を評価するqPCRに基づくアッセイFimSスイッチ機構を開発する。
1.便の収集及びDNA抽出
CD患者の探索的長期的研究CrohnOmeter(Enterome社が依頼)を、2012年7月から2014年12月の間に、St. Antoine及びSt. Louis Hospitals(Paris、France)、並びにFrench patient association l'Association Francois Aupetit(AFA、Paris、France)において実施した。本試験に自発的に参加した対象98人の適格性を評価した。組み入れ基準は、年齢≧18歳、内視鏡、放射線、及び/又は組織学特色に従うCDの確立された診断であった。主な除外基準は、組み入れ前の8週間における抗生物質の使用、腸切除の既往、及び組み入れ前の3カ月間における結腸鏡鏡のための腸洗浄であった。患者は、約9カ月間長期的に追跡され、自宅で1カ月に1回収集した便試料を提供した。
La Girona University(Margarita Medina-Martinez Lab- Girona-Spain)及びCornell(Kenneth Simpson Lab - Ithaca - USA)からの110株の大腸菌株を、MTAを通して入手した。これらの単離株は、IBD患者又は対照の便又は回腸/結腸生検から以前に抽出されており、そのAIEC表現型に関して特徴付けされていた(Cornell株に関しては、別紙に要約されているDogan B.ら、2014;Baumgart M.ら、2007を参照されたい)。それらの110株を、全ての臨床検査を担当するSMALTIS社(Besancon、France)に送付した。更に3株の非大腸菌株を、陽性及び陰性対照としてそれぞれ役割を果たすLF82及びLF82デルタFimHと共に注文した。

DNA抽出は、ZymoBIOMICS DNA PrepKitを使用して、製造元の説明書に従ってSMALTIS社が実施した。ゲノムシークエンシングは、GATC社(Konstance、Germany)が実施し、Illumina社のV4ケミストリーを使用してMiSeqによって300bpペアエンドとして実行したが、これは継続性を確保するためにこのプロジェクトの期間変化させない。陰性対照を各シークエンシングランに添加し、最終のphredスコア分析を、適した品質レベルを確保するために行った。混入に関してシークエンシングプロセスを制御するために、本発明者らはPhiX対照を使用した。
fimE、fimS、及びfimA遺伝子の領域を、Ensembl細菌データベースの大腸菌LF82ゲノムから選択した。本発明者らの内部データベースにおいて利用可能な391個のメタゲノミクスCD試料をfimE/fimS/fimA領域にマッピングして、カバレッジ及び多型を調べ、qPCRプライマーの候補領域を選択した。図3に示すように、2つのプライマーをfimS領域(フォワード及びリバース鎖上)について設計し、一方、他の2つをfimE(フォワード鎖)及びfimA(リバース鎖)領域について設計する、4つのプライマー戦略を開発した。
DNAを、DNase/RNaseフリー蒸留水で20倍希釈した後、分析した。qPCR分析をLC480(Roche社)機器において、TATAA SYBR(登録商標)GrandMasterミクス(TATAA Biocenter AB社)を使用して、4つのCD試料について二重の10μl反応の8つのアッセイ(SYBRに基づく)によって実施した。希釈した試料2μlを反応に添加した。EpMotion 5070(Eppendorf社)ロボットが全てのピペット操作を実施した。TATAAプレート間キャリブレータ(IPC)及びNTCを全てのプレートに含めた。全ての試料を同じプレートにおいて4アッセイ/ランで実行した(2組のプライマー間でTmが異なるため)。以下のプロトコール及び条件をqPCR分析のために使用した。
ONスイッチと、単離株におけるEB80018に対する細菌の凝集との間の関連
110株の単離大腸菌株と共に追加の3株の非大腸菌株におけるEB8018の凝集試験によって以下の結論を得た。
EB8018に対する凝集とFimSスイッチとの間の強い関連により、FimSのON位置、FimSのOFF位置、及び基準としてFimH領域を標的とするqPCRアッセイを開発した。
1.MOBIDIC試験
クローン病患者における分子マーカー及び接着侵襲性大腸菌(AIEC)検出試験(A clinical study for MOlecular BIomarkers and Adherent and Invasive Escherichia coli(AIEC) Detection study In Crohn's disease(MOBIDIC))患者の臨床試験を、2016年9月から2017年10月の間に実施した。この多施設試験におけるクローン病患者122人の適格性を、評価した。組み入れ基準は、(i)年齢18歳〜75歳の男性又は女性で、(ii)組み入れ前少なくとも3カ月間の回腸又は回腸-結腸クローン病の診断、(iii)試験組み入れ前に予定していた回腸結腸鏡検査、(iv)参加及びインフォームドコンセント用紙に署名する同意、(v)自宅で便の収集を実施できることを含むことであった。主な除外基準は、(i)モントリオール分類に基づくL2表現型の結腸クローン病、(ii)広範囲の小腸切除(>100cm)、又は短腸症候群、(iii)回腸結腸鏡の禁忌である小腸狭窄(stricture)/狭窄(stenosis)、(iv)現在、人工肛門又は回腸肛門パウチを使用している、(iv)現在、中心静脈栄養を受けている、(v)過去3カ月以内に受けた腸管前処置、(vi)出血リスクの増加(抗凝固剤/抗血小板療法を受けている患者)、(vii)腸癌又は結腸直腸がん、アルコール又は薬物の乱用、制御されない慢性障害の既往であった。
便試料は、CD患者が自宅で、保存緩衝液9mLを満たしたSarstedt社チューブ(Numbrecht、Germany)を使用して収集した。受領後、チューブを-80℃で保存した。分割及びDNA単離は、Eurofins/GATC Biotech社(Konstanz、Germany)に外注した。チューブをバッチ毎に分割し、抽出するまで-80℃で保存した。細菌内容物の単離のために、市販の抽出キットであるQIAamp便DNAミニキット(Qiagen社、Hilden、Germany)を使用した。DNA濃度を、Qubit蛍光分析定量(Life Technologies社、Carlsbad、California、United States)を使用して測定した。
CD患者から単離した粘膜標本を、MOBIDIC試験から収集した。各患者に関して、4つの生検を収集し、2又は3つを更なる試験のために収集し、1又は2つをバックアップ試料としてCell&Co社(Clermont-Ferrand、France)で保存した。回腸標本を、15%グリセロールを有する冷蔵DMEM 1.8mLを含有する予め重さを測定したバイアルに直ちに入れた。バイアルの重さを測定し、試料の質量を確立した。理想的には、生検のサイズは、5〜10mgの間でなければならない。-80℃で凍結後、バイアルをCell&Co社に送付し、受領時に-80℃で保存した。
MOBIDIC生検から単離した細菌株のEB8018に対する凝集は、SMALTIS社(Besancon、France)に外注している。主な手順を以下に記載する。

5.1.MOBIDIC試験の便試料を、QIAamp便DNAミニキットを使用して分割及びDNA単離のためにEurofins/GATC Biotech社(Konstanz、Germany)に送付した。各々の便試料に関して、3つのアリコートを得て、最初の2つのアリコートを抽出した。幾つかの便試料に関して、第3のアリコートも抽出した。濃度及び分解を調べることによって、品質管理を実施した。試料を2つのバッチで処理し、第1のバッチは試料30個であり、残りの試料は第2のバッチであった。本発明者らは、第1のバッチに関してゲル電気泳動において尾を引くバンドを観察し、一部の試料は品質管理に合格しなかった。しかしこれらの品質チェックを、更なるシークエンシングの対照としてEurofins/GATC社が調整し、本発明者らは最善のアリコートを選択することによって、それらをqPCR分析に関して使用することを決定した。
5.4.1.FimS OFFアッセイの結果
以下の表は、FimS OFF標的化アッセイの記述統計値及び2つの検定(ウィルコクソンの順位検定及びピアソンのカイ二乗検定)のp値、及びその有意性(Sは有意を表し、NSは有意差なしを表す)を表す。

大腸菌遺伝子FimHを標的とするqPCRアッセイもまた開発した。技術開発及び技術的検証を、ISO17025を遵守して実施した。
FimH遺伝子における4つの領域を、プライマーの設計のために選択した。4つの領域の各々に関して、3つのアッセイを設計した。加水分解プローブに基づくアッセイ設計を、Allele ID及びBeacon designer(PREMIER Biosoft社)を使用して実行した。プローブを、5'末端でFAMによって標識し、3'末端でIBFQクエンチャーに加えて内部ZENクエンチャー(PrimeTime ZENプローブ、IDT社)によって標識した。全てのアッセイのプローブ配列、センス及びアンチセンスプライマーを、以下の表に報告する(Table 9)(表12)。
FimHに関するqPCRアッセイの技術開発及び検証は、TATAA biocenter社が行った。
MOBIDIC試験において、患者35人が「活動性」(すなわち、CDAI>150を有する)と分類され、患者75人が「非活動性」(すなわち、CDAI≦150を有する)と分類された。2つのFimS-ONアッセイは、疾患活動度(p1.p3 ON、p=0.027 ;p6.p8 ON、p=0.031)と有意に関連した。CDAIスコアは当技術分野で周知である。これは例えば、Bestら、1976に記載されている。
i)患者の便試料におけるFimH発現細菌の存在が、疾患の活動度と相関するが、炎症レベル及び症状とは相関しないこと;
ii)そのままの便試料において本発明のバイオマーカーの存在をアッセイすることによって、クローン病の活動度を予後判定することが可能であるが、SES-CDスコアなどの潰瘍マーカーでは不可能であること
を意味する。
単離株のシークエンシングによって、本発明者らはFimS領域のDNA配列と、EB8018に対する凝集との間に強い関連を同定することができた(p<0.0001、ウィルコクソンの順位検定)。FimSスイッチ及びFimH遺伝子を標的とする9つのqPCRアッセイの技術開発及び検証を行った。MOBIDIC試験によって、本発明者らは、CD患者から便及び生検試料を得ることが可能であった。細菌DNAを便試料から単離し、9つのqPCRアッセイをDNA試料について実施した。同時に、EB8018に対する凝集試験を、回腸生検から単離した単離形態型について実施した。統計分析は、全てのqPCRアッセイとEB8018に対する凝集との間に、特にFimS-ON標的化アッセイに関して強い関連を示した(p5p7に関してp<0.001、ウィルコクソンの順位検定)。
Claims (36)
- 対象の便試料におけるfimH遺伝子の発現を検出する工程を含む、その腸管に多量のFimH発現プロテオバクテリアを保有する対象を同定するためのin vitro方法。
- 前記検出する工程が、前記便試料のヌクレオチド分画におけるfimオペロン転写のOFFからONへのスイッチを測定する工程によって実施される、請求項1に記載の方法。
- fimオペロン転写のOFFからONへのスイッチが、fimオペロンのFimS、FimA、及びFimE領域内の配列番号13〜配列番号33のヌクレオチド領域の部分を増幅するプライマーを使用することによって、好ましくは配列番号1〜8のプライマー対を使用することによって、より好ましくは[配列番号5;配列番号7]のプライマー対を使用することによって、qPCRによって検出される、請求項2に記載の方法。
- 前記検出する工程が、前記便試料のヌクレオチド分画におけるfimH遺伝子の存在量を測定する工程によって実施される、請求項1に記載の方法。
- 前記検出する工程が、配列番号35〜40のプライマー対を使用することによってqPCRによって実施される、請求項1又は4に記載の方法。
- 対象の便試料におけるfimH遺伝子の発現を検出する工程を含む、対象が、FimH遮断剤による処置から利益を得るか否かを予測するためのin vitro方法。
- 前記便試料のヌクレオチド分画におけるfimオペロン転写のOFFからONへのスイッチを測定する工程を含む、請求項6に記載の方法。
- 前記便試料のヌクレオチド分画におけるfimH遺伝子の存在量を測定する工程を含む、請求項6又は7に記載の方法。
- 前記測定する工程がqPCRによって実施される、請求項6から8のいずれか一項に記載の方法。
- a)前記対象から便試料を得る工程、

- 配列番号13〜33を有する特定のヌクレオチド領域又はその相同領域の部分を特異的に増幅する少なくとも2つのヌクレオチドプライマーを含有するキット。
- 以下のプライマー対:

- 対象がFimH遮断剤による処置から利益を得るか否かを予測するための、又はFimHプロテオバクテリア関連疾患に罹患している対象における処置の治療効率を試験するための、又はFimH遮断剤による処置に対して感受性であるか又は無応答性である患者のサブセットを同定するための、又は対象におけるFimHプロテオバクテリア関連疾患を診断及び/若しくはモニターするための、請求項32若しくは33に記載のキット又は請求項34に記載のプライマーの使用。
