末熟児網膜症(ROP)などの網膜の新生血管疾患の医学管理における進歩にもかかわらず、網膜神経血管疾患は、依然として世界的中の失明の主要原因である。

網膜感度と血管系の関係

黄斑変性症は、高視力(high−acuity vision)を担う、黄斑と呼ばれる中心網膜の一部における光受容体の喪失を意味する。黄斑変性症は、細胞外マトリックス成分、及び網膜色素上皮と血管脈絡膜の間の膜内の崩壊蓄積物の異常蓄積に関連している。この崩壊蓄積物様の物質は、ドルーゼンと呼ばれている。ドルーゼンは眼底検査で観察される。正常な眼はドルーゼンが存在しない黄斑を有しており、ドルーゼンはまた網膜周辺に多く存在し得る。黄斑視覚にいかなる喪失もない状態における黄斑内の軟質ドルーゼンの存在は、AMDの初期段階と考えられる。
加齢性黄斑変性症(AMD)は、網膜黄斑に異常を生じる疾患を意味する;これは、ヨーロッパ及び米国における視力低下の主要原因である。日本では、この疾患は、高齢人口のため確実に増加している。黄斑は網膜の中心に位置し、その領域は光受容体細胞のうちの錐体細胞と共に密に存在している。外部から来る光線は、角膜と水晶体によって屈折し、次いで、黄斑、特に中心窩に集められる。文字を読む能力は、この領域の機能による。加齢性黄斑変性症では、上記のような重要領域である黄斑が年齢により変性し、視力障害、主として変形視症(斜視)及び中心暗点が発症する。
脈絡膜血管新生(CNV)は、眼の脈絡膜層での新生血管の形成を意味する。CNVは、脈絡膜の最も内側の層のブルック膜に欠損のある個体を急速に生じ得る。さらに、これは過剰量の血管内皮細胞増殖因子(VEGF)に関連している。ウェット型AMDと同様に、CNVはまた、珍しい遺伝病弾性線維性偽性黄色腫を、また稀により一般的な視神経乳頭ドルーゼンを併発することが多い。またCNVは、強度近視又は悪性の近視性変性にも関連しているが、脈絡膜血管新生では、主として、ラッカークラックとして知られている網膜(特に)黄斑組織にクラックが存在して生じる。
ほとんどの眼の手術、外科処置、治療及び検査は、眼に明るい光を直接曝露することが必要であり、多くの場合、この曝露は持続される;本明細書に記載の化合物は、このような曝露による眼に対する望ましくないダメージの制限又は予防に有用である。

本明細書は、本明細書で提供されている組成物の治療上有効な量を患者に投与することによって、(一若しくは複数の症状、又は一若しくは複数の症状の停滞を軽減する)患者の糖尿病性網膜症を治療する方法を提供する。治療は、患者の状態を改善することができ、一又は複数の次の因子を判定することによって評価することができる:黄斑浮腫の減少、又は視力の向上が生じたかどうか。本明細書に記載した化合物はまた、糖尿病性網膜症の治療用薬剤に使用することもできる。
糖尿病性網膜症は、根底にある糖尿病状態の結果生じ、その進行リスクを軽減するか、その進行を遅延させるためのさらなる手段は、最適な血糖レベルを維持すること;規則的且つ完全な眼の検査を行うこと;健康的な食事計画に従うこと:異なる種類の食品を食べ、各食事で正しい量の炭水化物を食べること;定期的なエクササイズを行うこと;処方通りに薬を正確に服用すること;正常なレベルにコレステロールと血圧を維持するための低脂肪及び減塩の食事を食べること;喫煙しないこと;制御下に血圧値及びコレステロール値を保つこと;妊娠中、血圧を注意深くモニターすることである。
レーザー光凝固術は、1960年代以来、非増殖性糖尿病性網膜症、黄斑浮腫及び増殖性糖尿病性網膜症の治療に使用されてきた。
このタイプのレーザーエネルギーは、影響を受けた領域に直接向けられるか、又は損傷を受けた眼の組織を破壊するために含まれる穏やかなグリッド状のパターンで適用され、難視領域と視力低下の一因となる傷跡を取り除く。レーザー治療のこの方法は、一般に、特定の個別の血管を対象とする。
汎網膜光凝固術は、増殖性糖尿病性網膜症治療の第1選択である。これは、網膜の最も外側の(辺縁)領域にレーザーエネルギーの約1,200〜1,800微小スポットを適用し、内部の部分には触れないままでおいておく。このレーザー治療は、異常血管を収縮させることができる。この治療は、網膜周辺の虚血性網膜細胞を凝固又は焼成する目的で、網膜の広範囲をレーザー処理することを含む。
凍結療法(凝固)は、糖尿病性網膜症の治療を促進し得る。硝子体が血液によって曇っている場合、血液が消失又は透明になるまで、レーザー外科療法は使用することができない。これらの症例のうちのいくつかでは、網膜凍結療法は、異常血管を収縮させ、網膜を眼の後部へ接合させるのに有用となり得る。
蛍光眼底血管造影法は、糖尿病性網膜症のある眼の網膜循環における診療上の病理学的変化を理解する際の研究ツールとして有用であった。またそれは、糖尿病性網膜症を分類し、蛍光眼底血管造影法の基準特性、特に毛細血管の非灌流のパターンからの進行を予測するのに役立った。
硝子体切除術は、眼の中央からの硝子体ゲルの外科的除去であるが、これは多くの場合、進行性の網膜疾患のある患者に対して使用される。この処置は、網膜の完全な剥離を予防するものである。この処置は、一般に、取り除かれない硝子体出血、硝子体黄斑牽引、網膜上膜、及び網膜剥離の治療に使用される。
眼の屈折矯正手術は、角膜又は白内障の外科再形成の様々な方法を含む(例えば、放射状角膜切除術は、ダイヤモンドナイフで製造されたスポーク形状切開を使用する)。場合によっては、エキシマーレーザーが角膜の曲率を再形成するために使用される。眼の屈折矯正手術は、一般的な視覚障害、例えば、近視、遠視及び乱視など、並びに、円錐角膜のような変性障害を軽減又は矯正することができる。別のタイプの眼の屈折矯正手術としては、角膜曲率形成術(角膜乳頭の剥離、急速凍結、切削加工−研削後、その本来の曲率への復元)、自動層状角膜移植術(ALK)、レーザー光線による近視手術(LASIK)、イントラレーシック、レーザーによる上皮下角膜曲率形成術(LASEK aka Epi−LASIK)、レーザー屈折矯正角膜切除術、レーザーサーマル角膜移植術、伝導式角膜形成術、角膜輪部減張切開術、乱視角膜切開術、放射状角膜切開術、ミニ非対称放射状角膜切開術(mini asymmetric radial keratotomy)、六角形角膜切開術、表層角膜移植術、角膜内リング又は角膜内リングインプラント(Intacs)、コンタクトレンズインプラント、老眼反転術、前方毛様体強膜切開術、老眼のレーザー反転術、強膜拡大バンド法(scleral expansion bands)、及びカムラインレーが挙げられる。
角膜手術の例としては、限定するものではないが、角膜移植手術、全層角膜移植術、人工角膜移植術、治療的角膜表層切開術、翼状片切除術、角膜染色術、及び歯根部利用人工角膜移植術(OOKP)が挙げられる。場合によっては、角膜手術にレーザーを必要としない。別の例においては、角膜手術はレーザーを使用する(例えば、治療的角膜表層切開術であって、これは表面の角膜混濁と表面凹凸を除去する)。ある場合には、患者に、これらの処置の後、患者の眼を明るい光から保護するサングラスが渡される。
白内障手術は、レンズの外科的除去とプラスチック眼内レンズによる交換を含む。典型的には、外科医を補助するため光が使用される。
コルチコステロイドは、血管透過性を低下させ、血液網膜関門の機能障害を軽減する。これは、VEGF遺伝子の転写及び翻訳と、血管壁への白血球接着を阻害する。コルチコステロイドは、特に、血管透過性の増大と関係する合併症に対応する。
現在、糖尿病性網膜症の管理に使用されている4種の抗VEGF薬剤があり、例えば、ペガプタニブ(Macugen; Pfizer, Inc., New York, USA))、ラニビズマブ(Lucentis; Genentech, Inc., South San Francisco, California, USA)、ベバシズマブ(Avastin; Genentech, Inc.)、及びVEGF Trap−Eye(Regeneron Pharmaceuticals, Inc., Tarrytown, New York, USA))が挙げられる。
持続性びまん性黄斑浮腫(DME)に対する黄斑前の後部硝子体膜を除去する硝子体切除術は、広範囲で急速に受け入れられている。硝子体切除術の効果に関する多くの一連の評価は、(内境界膜剥離の有無に関わらず)矛盾する結果となった。結果において、内境界膜を剥離する硝子体切除術は、事前に従来のレーザー治療には反応せず、距離や読書視力並びに浮腫の形態に確実に影響を受けている、持続性びまん性糖尿病性黄斑浮腫(DME)のある眼における所見よりも優れていたことが観察された。また別の研究では、場合により内境界膜を剥離する硝子体切除術は、レーザー光凝固術に難治性であるびまん性の非牽引性無反応糖尿病性黄斑浮腫(DME)のある眼において、解剖学的及び視覚的な効果を提供し得ることが示唆された。
証明の際、後部硝子体剥離(PVD)と硝子体液化の併用の誘導するマイクロプラスミンの硝子体内注入は、硝子体内の酸素分圧を増加させることが観察された。一方、後部硝子体剥離(PVD)誘導のないヒアルロニダーゼ誘導型硝子体液化は、硝子体内の酸素分圧を増加させなかった。さらに、マイクロプラスミンで治療した動物が100%酸素に曝露された場合、コントロール又はヒアルロニダーゼで治療した眼に比較して、硝子体内腔の酸素レベルの上昇が加速された。これらの所見は、硝子体腔中の酸素分圧の増加における外科的硝子体切除術の有効な効果は、PVD及び硝子体液化の酵素的誘導により再現することができ、手術の時間、リスク及び費用がなくてすむことを示唆する。2009年には、硝子体切除術を伴わない自己由来プラスミン酵素を硝子体内注入すると、完全PVDが誘導され、難治性びまん性糖尿病性黄斑浮腫に起因する黄斑の肥厚が効果的に低減され、視力が改善することが示された。したがって、硝子体切除術を伴わない、内境界膜と後部硝子体膜の間を完全に切断する後部硝子体皮質の外傷性薬理学的分離は、網膜裂孔などの手術時の医原性損傷、神経繊維への損傷、及び術後の後遺症のリスクを減らすことができる。
フィブラートは、異脂肪血症の治療で広く処方されている脂質効果薬である。ペルオキシソーム増殖因子活性化受容体アルファの活性化により媒介される、これらの主な臨床効果は、総コレステロール及び低密度リポタンパク質コレステロール濃度の穏やかな減少、トリグリセリドの顕著な減少、及び高密度リポタンパク質コレステロールの増加である。フェノフィブラート投与と糖尿病おける事象低減(FIELD)に関する研究では、この効果の機序は脂質の血漿中濃度と関係しているようには見えないが、フェノフィブラートによる長期的な脂質降下療法は、糖尿病性網膜症の進行を抑制し、2型糖尿病患者におけるレーザー治療の必要性を低減することを示した。最近、ACCORD研究グループ(2010)は、異脂肪血症の集中治療用フェノフィブラートが2型糖尿病の人の糖尿病性網膜症の進行速度を抑制することを示した。
いくつかの研究によれば、RAS遮断薬が糖尿病性網膜症の負担を軽減することができることが示唆されている。Eurodiab Controlled trial of Lisinopril in Insulin−dependent Diabetes (EUCLID)の所見は、アンギオテンシン変換酵素阻害剤のリシノプリルによるレニン−アンギオテンシン系の遮断は、1型糖尿病における網膜症の発生率及び進行をともに低減することができることを示唆している。
ペルオキシソーム増殖因子活性化受容体ガンマ(PPAR−γ)アゴニスト
PKCは、糖尿病のいくつかの眼の合併症に影響を及ぼす。これはVEGFによって活性化され、糖尿病性網膜症の治療における有望な対象である。
最新の研究では、島移植転帰の改善としては、島単離の促進、グルココルチコイドを使用しない免疫抑制、体重1kg当たり10,000を超える島当量の適切な島量の提供を観察することができた。これらの改善によって、1型糖尿病患者に対する効果、例えば、長時間のC−ペプチド分泌、血糖コントロールの改善、及び低血糖エピソードの低減などが達成された。
オリゴヌクレオチドは、疾患過程における特定の関連性を標的とする新しい治療実体の1つを表す。2つの主なカテゴリーのオリゴヌクレオチド治療薬:アンチセンスオリゴヌクレオチド、例えば、低分子干渉RNA(siRNA)と、オリゴヌクレオチドアプタマーがある。
成長ホルモン及びインシュリン増殖因子(IGF)は、網膜内皮前駆細胞の機能をモジュレートし、低酸素による網膜血管新生を作動する;またIGF 1は、血液網膜関門を破壊し、網膜の血管透過性を高め得る。
硝子体内ヒツジヒアルロニダーゼ注入は、硝子体出血を取り除くのに有効である。いくつかの一連のヒトの症例では、自己由来プラスミン酵素の硝子体内注入が糖尿病性黄斑浮腫及び増殖性糖尿病性網膜症を治療する硝子体手術に対する安全且つ有効な補助剤であることが示された。
糖尿病性網膜症の一次予防は、厳密な血糖、脂質及び血圧のコントロールを含む。糖尿病性網膜症を予防するためにコントロールされるべき全身的因子のいくつかを以下に詳述する。
高血糖は、最終的には糖尿病性網膜症の発症に結びつく事象のカスケードを誘発する。したがって、糖尿病性網膜症の進行を遅くするために使用することができる治療の一つは、血糖コントロールである。血糖コントロールは、発症のリスクを低減し、1型及び2型糖尿病両方における糖尿病性網膜症の進行を抑制する。
高血圧症は、血流の増加と血管内皮細胞の力学的損傷(伸縮)により糖尿病性網膜症を悪化させ、VEGFの放出を刺激する。厳しい血圧コントロールは、2型糖尿病の人において、網膜症進行のリスクを約3分の1まで、視力障害のリスクを半分まで、レーザー治療の必要性を3分の1まで減らす。また血圧コントロールは、糖尿病性網膜症の発症及び進行も低減することができる。
異脂肪血症は、糖尿病性網膜症の病因に関与する。網膜症の重症度は、トリグリセリドの増加と関連し、逆にHDLコレステロールと関連していた。ヒドロキシメチルグルタリルコエンザイムA(HMG CoA)阻害剤は、異脂肪血症の患者における糖尿病性網膜症(DR)及び糖尿病性黄斑浮腫(DMO)の管理に役立ち得る。

糖尿病性網膜症を治療するための化合物の使用
中心部を含む臨床的に重大な糖尿病性黄斑浮腫(DME)を有する患者における、本明細書に記述する化合物の経口投与の生物学的活性を判断するために、及び、関連するどんな有害事象でも報告するために、単一施設、非盲検、用量漸増予備研究を開始する。黄斑の中心部を含むDMEを有し、試験眼における最高矯正視力(BCVA)が、20/63から20/400の間である患者を登録する。
マンガン造影磁気共鳴画像法(MEMRI)のプロトコル
実験期間を開始する前に、ラットを、通常の実験室の照明(12時間明期、12時間暗期)下に維持する。光曝露、退色、暗順応は、コホートにより変動する(下記参照)。
内径12mmのリニア表面コイルを使用して、軸及び矢状面で、網膜を二等分する単一の撮像スライスを得るために、スナップショットFLASH反転回復(IR)撮像シーケンスを使用する。撮像パラメーターは、TR/TE=2000/2.7ms、掃引幅=73.5kHz、取得数=32;スライス厚=0.7mm、視野=12mm×12mmと、47ミクロンの面内解像度をもたらす192×192データマトリックス(256×256にゼロ詰めされる)である。6つの反転時間[TI=50、150、300、400、900、1800ms]で得た信号を使用して、T1マップを得た。
群1〜3のための時系列を図1に示す。
この試験の目的は、ACU−4429の単回(高)用量投与が、退色後の暗順応の後に網膜の再度の陽イオン活性(Mn2+の取り込み)を低下させるかどうかを決定することである。群1〜4(ACU−4429対ビヒクル)に投与し、それらを室内光下に2時間置いておく。群5及び6(レチニルアセトアミド対ビヒクル)には、退色の18時間前に投与する。それらの動物は、瞳孔が拡張し、それらの動物を、退色させる適度な白色光(5,000ルクスの拡散性白色蛍光灯)に10分間曝す。退色直後、それらの動物に、Mn2+を腹腔内(IP)注射し、その後に、4時間の暗順応を行う(動物は、撮像を待っている間、暗順応状態にさせたままである)。群3の動物を、調光として機能する周囲室内光下に残しておく(網膜が、まるで明順応しているように振る舞うように、レチニルアセトアミド及びACU−4429で治療されると期待する)。MRI撮像(動物1匹あたり30分〜1時間)を、Mn2+注射(i.p.)の4時間後に実行し、撮像の前に動物が入れられていたのと同じ光条件で行う。動物への投与は、投与から撮像の時間がすべての動物について同じであることを確実にするようにずらす。
この試験の目的は、時間をかけた、通常の周期的な光におけるACU−4429(10mg/kg/日)による反復治療が、暗順応後の網膜の再度の陽イオン活性(Mn2+の取り込み)を低下させるかどうかを試験することである。3つの群:群1:1日2回、5mg/kg(10mg/kg/日)のACU−4429;群2:ビヒクル(暗順応している);群3:ビヒクル(室内光)。すべての投与は、通常の周期的な光(約100ルクスの拡散性白色蛍光灯に12時間)への曝露という条件下で、6日間、明かりを付けた状態及び明かりを消した状態で行う。7日目の朝の投与の直後、硫酸アトロピン(1%)を1滴、すべての動物の両眼に点眼し、瞳孔を開かせる。最終用量の投与の6時間後、及び通常の光での少なくても6時間後、群1(ACU−4429)及び群2(暗順応している)に、Mn2+をIP注射し、その後、4時間の暗順応を行い(動物は、撮像を待っている間、暗順応状態にさせたままである)、群1及び群2を、暗所で撮像する(動物1匹あたり30分〜1時間)。最終投与の6時間後に、群3(室内光)に、Mn2+をIP注射し、群3を、撮像までさらに4時間、通常の室内光下に置いたままにする。群3の撮像は、通常の光の下で行う。
ラットにおける酸素誘発網膜症の低下
目的:試験化合物を、ヒトの未熟児網膜症(ROP)の一般的なモデルである、酸素誘発網膜症(OIR)を有するラットにおいて評価する。OIRもROPも、網膜血管系の異常、及び神経網膜の長期的な機能障害を特徴とする。
ROPのラットモデルにおける視覚サイクル調節及び桿体機能
ROPのラットモデルは、光受容体と網膜血管系との関係を研究及び操作できる、好都合なin vivo系をもたらす。
レーザー誘発脈絡膜血管新生及び黄斑変性の動物モデル
脈絡膜血管新生のネズミモデル
本明細書に記述するVCM化合物の効果を、脈絡膜血管新生のネズミモデルにおいて評価することができる。
動物は、加齢黄斑変性(AMD)自体を発症しないが、AMDに見られるものに類似した脈絡膜血管新生は、ブルッフ膜及びその上に重なる網膜色素上皮(RPE)における局所的破壊をもたらすレーザーを使用することによってもたらすができる。この傷害は、下にある脈絡膜毛細血管の、RPE層及び網膜下腔への異常な成長を刺激する。ブルッフ膜の破壊は、ウェット型AMDを特徴づけるものを含めた、すべての形態の脈絡膜血管新生(CNV)に共通している。
慢性の光誘発脈絡膜血管新生における有効性研究
目的:この研究の目的は、ウィスターラットを使用して、in vivoでの3000ルクスの光損傷からの保護のための、0.3及び3mg/kg/日の臨床開発候補による、3カ月(90日)の1日1回の経口処置の有効性を試験することであった。ラットにおける長期の光損傷(3カ月)は、光受容体の退化及び脈絡膜血管新生(CNV)をもたらすことを示している。光誘発ONL及びCNVからの保護における、例示的な臨床開発候補の有効性を判断した。
結果:臨床開発候補は、光誘発ONL厚減少から網膜を保護する。臨床開発候補による処置は、脈絡膜血管新生に対する重大な保護をもたらした。
新規の視覚サイクル調節剤であるACU−4429の、健康なボランティアにおける第I相の用量範囲探索試験
視覚サイクル調節(VCM)は、網膜における、光子の電気信号への生物学的変換を指す(図4A及び図4B参照)。
・光への曝露時のフリーラジカル産生;
・洗剤のような特性は、RPE細胞膜を損傷し得る;
・RPEリソソーム阻害(ドルーゼン形成につながる);及び
・補体因子の活性化。
55歳〜80歳の健康なボランティアにおいて、安全性及び忍容性を観察した。用量依存性の視覚サイクル調節を、網膜電図(ERG)によって観察した。
125人の健康な対象に、ACU−4429を投与した。ACU−4429は、DMCに関係するAEを有しないこれらの健康な対象において、忍容性が良好であった。一部の対象において、頭痛が見られたが、一時的なものであり、薬に無関係であり得た。軽く一時的な視覚AEが観察された。ACU−4429は、低用量でさえも、非常に良好な薬理反応をもたらした。錐体ERGにおいて、変化は、全く観察されなかった。

図8は、ACU−4935が、低酸素条件によって引き起こされるVEGF上方制御を減少させるかどうかを試験するために使用するプロトコルを示す。手短に言えば、動物を、暗所に16時間順応させ、次いで、ACU−4935を投与した。投与の2時間後に、動物を、50000ルクスで10分間光退色させ、その後、2時間暗所で回復させた。6%のO2で、6時間低酸素を誘発した。一部の動物を安楽死させ、時間0で、サンプルを収集した。別の一部の動物を、2時間、暗所に戻してから、安楽死させ、サンプルを収集した。
ビーグル犬における[14C]−ACU−4429の眼内分布

最初の試験日に、動物を、各同腹から生まれた子の半分(数が奇数の場合は切り上げる)が、光シグナル伝達の活性化及び不活性化、並びに受容体後の網膜機能の研究に参加し、残りが、退色実験に参加するように、危険が半分になるように割り付ける。桿体の光応答の特性は、5つの最も明るいフラッシュによって引き出されるa波に対する光シグナル伝達の活性化を含む生化学的過程のLamb及びPugh(1992;Pugh及びLamb、1993)モデルの、Hood及びBirch(1992)の定式化のパラメーターに適合させることによって、ERGから見積もる:

同じ動物において、桿体媒介性の受容体後機能を、ERGのb波から判断する。およそ0.075から約300R*からもたらされる、一連の13の「緑色」のフラッシュを使用して、b波応答を引き出す。そのような応答の振幅(μV)に対して、Naka−Rushton関数のパラメーター

コホートで行われる第2のセットの実験において、退色からの暗電流の回復を評価する。暗順応した眼に与えられる、桿体を飽和させるPF(10,000R*)を使用して、暗電流の大きさを決定する。退色曝露の後、PFへの応答を、およそ40分間、2分間隔でモニタリングする。各時間で、回復した暗順応の分率(1−SFt)を計算する。飽和した桿体の光応答が50%回復する時間、すなわち、t50を、関数のパラメーターを最適化することによって求め、
次いで、P=50%の式を解く。この式において、t(P)は、a波が、Pパーセントの暗順応値に達するのに必要な時間であり、t0は、再生の時間定数であり、P0は、暗順応したa波(100%)の正規化された振幅であり、Bは、スカラーである。しばしば、t50は、記録するセッションよりも長く、したがって、外挿する。
ERG刺激のタイミングと強度は、コンピューターで制御される。受容体及び受容体後応答の感度及び振幅を評価するために使用する、一連の強度について平均化された、刺激時間間隔及び掃引の数を、以下に詳細に示す。不活性化実験について、調整フラッシュへの応答は、平均で8回であり、プローブフラッシュへの応答は、平均で4回であり、ダブルフラッシュ状態において、すべてのトレースは、1分の開きを記録された、平均で2つの掃引である。退色実験において、プローブフラッシュを、2分ごとに1回送達する。
血管の蛇行を、縦断研究において必要なものである非侵襲的技術を使用して、対象の両眼において判断する。この研究において用いられるOIRモデルは、NV率が100%であることを特徴とし;蛇行した網膜血管も特徴とする。患者において、後極は、危険性の高いROPの診断に最も重要である領域である。それに対応して、各ERGセッション後、網膜の主要な血管を示す眼底の広範囲画像を得て、後極(渦静脈に巻きつかれ、視神経乳頭と同心である円内の領域とここでは定義し、渦静脈は、赤道部を規定する)の全体像を示すように画像を合成する。細動脈を特定し、それらの蛇行を、上記したとおり、RISAソフトウェアを使用して測定する(Akulaら、2007; Akulaら、2008; Gelmanら、2005; Hansenら、2008; Martinez-Perezら、2002、2007)。手短に言えば、各血管を、メインの画像から切り取り、個々に分割する。必要に応じて、分割画像を、手動で編集して、無関係な特徴、例えば、背景の脈絡血管系を取り除く。RISAは、骨格を作成し、終点及び分岐点に印を付けた。次いで、使用者は、分析のために、血管セグメントを選択し、RISAは、選択された各血管セグメントについて、統合的屈曲(IC)を自動的に計算する。ICは、直線コースからのいかなる離脱をもとらえているものであり、血管の長さによって正規化された、血管に沿った角の合計である(ラジアンpixel−1)。したがって、理論的にまっすぐな血管は、IC=0である。高い値のICは、医師が、蛇行的であると呼ぶであろう十分な血管をとらえる。細動脈蛇行(TA(ラジアンpixel−1)を、両眼におけるすべての測定可能な細動脈の平均の統合的屈曲として、各対象について計算する(中央値10)。
未熟児網膜症に対するヒト臨床試験
