Abstract

動物、抗TGFβ処置および組織回収
Crtap−/−マウスを生成し、混合C57Black/6J/129Sv遺伝的バックグラウンドを維持した。Col1α2遺伝子(Col1α2tm1.1Mcbr)におけるG610C変異を保有するマウスを得て、野生型C57Bl/6Jマウスに交配させた。Col1α2tm1.1Mcbr対立遺伝子に対してヘテロ接合性のマウスも実験に使用した。Smad2/3依存のTGFβシグナル伝達経路に応答してルシフェラーゼを発現するTGFβ−リポーターマウス(SBE−Lucマウス)を取得し、Crtap+/−マウスと2世代交配してリポーター導入遺伝子を発現するCrtap−/−マウスおよび野生型同腹子を生成した。マウスは全て小動物施設に収容し、実験は動物実験委員
会(IACUC)の承認プロトコルにしたがって実施した。
急速凍結したP3頭蓋冠試料からタンパク質を抽出し、300μlの溶解バッファー(0.0625M Tris−HCl pH7.5、2%SDS、5mM NaF、2mM

TGFβリポーター導入遺伝子を発現するP10Ctrap−/−マウスおよび野生型同腹子(SBE−Lucマウス)に、D−ルシフェリン(Goldbio、150mg/kg、IP)を注入し、イソフルランで麻酔をかけ、生物発光画像システム(Xenogen)を使用して、注入後10分間画像処理した。
骨髄細胞をおよそ2か月齢のCrtap−/−および野生型マウスの脛骨および大腿骨から分離し、10%FBS、100U/mLペニシリンおよび100ug/mLストレプトマイシンを補足したα−MEMで培養した。培地は2日に1回ごとに代え、非接着細胞は廃棄した。7日後に、骨髄間質細胞(BMSC)と定義される接着細胞を、24ウェルプレートに、1cm2あたり2.5×104細胞で再播種し、骨形成培地(α−MEM、10%FBS、500μMアスコルビン酸および10mM β−グリセロホスフェート)中で3日間培養した。調整培地を回収し、PAIルシフェラーゼ・リポーター・ミンク肺上皮細胞と共にインキュベートした。24時間後、細胞溶解物をルシフェラーゼ活性アッセイのために回収し、Dual−Luciferase Reporter System(Promega)を使用して測定した。結果は、Micro BCA試薬(Pierce)を使用して定量した全タンパク質に対して標準化した。
腰部脊椎と大腿骨は、Scanco μCT−40マイクロCTを使用してスキャンし、骨梁および皮質骨パラメータを定量した。脊椎および大腿部の骨梁骨パラメータを、手動で椎体L4の骨梁ならびに大腿骨の遠位骨端セクションの輪郭をとることにより、Scanco分析ソフトウェアを使用して分析した。大腿部の中軸中心にある皮質骨パラメータは、ソフトウェアに含まれた自動閾値アルゴリズムを使用して定量した。

Crtap−/−およびWTの大腿骨を、3点曲げ試験によって、Instron5848装置(Instron Inc.,Norwood MA)で6mmのスパンを用いて試験した。大腿骨は全て室温で、湿らせて試験した。これらを0.05N/秒の速度で5秒間、1Nで予荷重した。予荷重に続き、大腿骨を破壊されるまで0.1mm/秒の速度で荷重した。荷重と変位のデータはBLUEHILLソフトウェア(Instron5848)を使用して40Hzの速度で得た。
血清オステオカルシン(OCN)は、Biomedical Technologies Inc.からのマウス・オステオカルシンEIAキットを使用して定量した。骨コラ
ーゲン(CTX)のC末端架橋テロペプチドは、Immunodiagnostic Systems Ltd.からのRatLaps(商標)EIAキットを使用して定量した。両方の分析は、製造業者のプロトコルにしたがって実施した。
質量分析については、I型コラーゲンを、Crtap−/−および野生型脛骨から製造した。骨を、クロロホルム/メタノール(3:1 v/v)で脱脂し、0.5MのEDTA、0.05MのTris−HCl、pH7.5で脱塩し、これらの脱塩工程は全て4℃で実施した。骨を細かく刻み、コラーゲンをSDS−PAGE試料バッファー中での熱変性(90℃)によって可溶化した。コラーゲンα鎖をSDS−PAGEゲルから切断し、インゲル・トリプシン消化にかけた。Electrospray MSを、トリプシンペプチドについて、C8キャピラリーカラム(300μm×150mm;Grace Vydac208 MS5.315)を使用したインライン液体クロマトグラフィー(LC)(ThermoFinnigan)を装備したLCQ Deca XPイオントラップ質量分析計を使用して、4.5μl分で溶出して実施した。Sequest検索ソフトウェア(ThermoFinnigan)を使用して、NCBIタンパク質データベースを使用してペプチド同定を行った。
表面プラズモン共鳴実験はBIACore X機器(GE Healthcare Bio−Science Corp.)を使用して実施した。野生型およびCrtap−/−マウス由来の精製された天然マウス腱I型コラーゲンを、CM5センサー・チップ上で、それぞれ約0.05ナノグラム/mm2(500RU)および0.08ナノグラム/mm2(800RU)の濃度でのアミドカップリングによって固定化した。実験は、10μl/分の流速にて20℃でHBSPバッファー(10mM Hepesバッファー、pH7.4、150mM NaClおよび0.005%界面活性剤P20を含有)中で実施した。組み換えヒトデコリンコアタンパク質(R&D systems)を両方のI型CM5チップ上に注射した。ヒトデコリン原液の濃度はアミノ酸分析によって決定した。デコリンの野生型およびCrtap−/−マウスI型コラーゲンに対する結合応答は、CM5センサー・チップ上の固定化I型コラーゲンの量によって標準化した。3つのデコリン濃度(3、5および12μM)を使用して、各濃度について分析を3回繰り返した。この実験は、各時間の異なるマウスから単離したコラーゲンで2回実施した。
2群間の比較は、不対両側スチューデントt検定を使用して行った。3群間の比較については、群の等分散が確認された場合には一元配置分散分析(ANOVA)を実施し、次いでHolm−Sidak法を使用して全ペアワイズ多重比較を行った。等分散試験が不合格となった場合は、クラスカル=ウォリスの順位に基づく一元配置分散分析を実施し、次いでTukeyテストを使用して、全ペアワイズ多重比較を行った。0.05未満のP値を、スチューデントt検定、ANOVA、およびクラスカル=ウォリスの順位に基づく一元配置ANOVAで統計的に有意であるとみなした。事後ペアワイズ多重比較については、各P値について、P値の順位および比較総数に依存する臨界レベルと比較し、群間の差異が有意であるかを測定した。Sigma Plot V11.0(Systat Software Inc.)を統計分析に使用した。
1と対照処置OIマウスとの間の90%出力でのマイクロCTによる骨量(BV/TV)の最小差異20%を検出した。1群のサイズは8匹のマウスを必要とした。
Crtap−/−マウスおよび年齢一致野生型(WT)同腹子対照を、活性化pSmad2、TGFβシグナル伝達経路のメンバー、およびTGFβの下流の他の標的の発現について分析した。頭蓋骨を切除し、RNAおよびタンパク質を、リアルタイム−PCRおよびウェスタンブロットによってそれぞれ抽出し、分析した。図1Aおよび1Bにおいて見出し得るように、Crtap−/−マウスは、ウェスタンブロットで測定し、デンシトメトリで定量化するとき、WTマウスと比較して全Smad2に対する活性化pSmad2の比率が100%高く、TGFβシグナル伝達がCrtap−/−マウス中で上昇していることが示された。TGFβの転写標的、例えばCol1α1およびp21は、RT−PCRによって測定するとき、図1Cおよび図1Dで実証されるように、WT対照と比較して、それぞれ上昇していた。線維化促進性ECMタンパク質結合組織成長因子(CTGF)を測定すると、RT−PCRによって測定するように、および図1Eで実証されるように、WT対照と比較してCrtap−/−マウスでは約50%高いことがわかった。図1Fにおいて示されるように、RT−PCR分析は、サイクリン依存性キナーゼ阻害剤p27の発現が、Crtap−/−マウスまたはWTマウスにおいて変わらないことが明らかになった。
Crtap−/−マウスを、TGFβシグナル伝達の活性化に応じてルシフェラーゼを発現するTGFβリポーターマウスと交配させた(Jackson Laboratory;B6.Cg−Tg(SBE/TK−luc)7Twc/J)。P9マウスに、基質D−ルシフェリン(150mg/kg)を注入し、10分後にイメージングした(Xenogen;IVISカメラシステム)。図2Aで実証されるように、Crtap−/−マウスは、WT対照と比較して、尾骨、長骨および頭蓋冠において発光がかなり高く、crtap−/−マウスにおいてTGFb活性が増加していることを示していた。図2Bは、頭蓋冠でのルシフェラーゼ活性を定量したものである。
8週齢成体Crtap−/−マウス(1群あたりN=6)に、配列番号14のアミノ酸配列を含む重鎖と、配列番号15のアミノ酸配列を含む軽鎖とを含む、TGFβに結合する汎特異的抗体のマウス代替物である1D11(10mg/kg、I.P.,3回/週、全8週)を投与した。無関係の13C4抗体を、Crtap−/−マウスと対照としてのWTマウス(N=6)の個別群に投与した。16週齢マウス(週8〜16から処置)のL4椎体を、μ−CTで画像化した。TGFβ中和抗体1D11(Genzyme;10mg/kg、I.P.,3回/週)で8週間処置した8週齢Crtap−/−マウス(1群
あたりn=6)ならびに対照抗体(13C4プラセボ)で処置した野生型(WT)および対照Crtap−/−マウスからの椎体L4のマイクロCTデータを図3に示した。図3に示されるように、Crtap−/−脊椎は、WT対照脊椎と比較して海綿状であった。しかしながら、1D11処置により、WT状態と同様の骨格表現型に帰着した。
μ−CTに加えて、抗体処置マウス、プラセボ処置マウスおよびWTマウスの脊椎骨を、組織形態計測によって分析した。図5Aおよび5Bに示されるように、μ−CTの結果を組織形態計測により確認した。さらに、組織切片を、破骨細胞マーカーTRAPの発現について染色した。分析で明らかにされるように、Crtap−/−マウスの骨表面では、WT対照と比較して、より多くの破骨細胞が被覆され(N.Oc/BSおよびOc.S/BS)、破骨細胞活性が増加していることが示された。1D11抗TGFβを用いた処置により、全ての破骨細胞特異的パラメータがWT数未満に減少した。したがって、破骨細胞は、TGFβ抗体、より詳細には、汎特異的抗TGFβ抗体に対する有望な標的として同定された。
生体力学試験は、インストロン5848装置(Instron Inc.,Norwood MA)を用いた標準3点曲げ試験を使用して、6mmのスパンで、1N/秒の速度で5秒間1Nを予荷重して、16週齢マウス(処置後8〜16週目)の切除した大腿骨に実施した。予荷重に続き、大腿骨を破壊されるまで0.1mm/秒の速度で圧縮した。荷重と変位のデータはBLUEHILLソフトウェア(Instron5848)を使用して40Hzの速度で得た。
Crtap−/−マウスは、低い骨量、糸球体硬化症および肺形成障害によって現れる全身性結合組織疾患を有した(Baldridgeら; PLoSone 5(5):e10560(2010))。TGFβ発現の増加が、pSmad2の免疫染色陽性により証拠づけられ、図7Aで実証されるように、Crtap−/−マウスの肺で見られた。組織学的に、Crtap−/−マウスは、図7Bに示されるように、WTマウスと比較して、遠位気道空間が増加していることが示された。図7Aおよび図7Bで実証され、図7Cで定量化されるように(*P<0.05対対照Crtap−/−;1マウスあたり10画
像解析、1群あたりn=8マウス)、1D11処置(10mg/kg、IP、3x/週を8週間)により、Crtap−/−マウスにおけるpSmad2発現が減少し、遠位気道空間が減少し、肺表現型が改善した。
Crtap−/−マウスの骨および肺における調節不全TGFβシグナル伝達の基礎を理解するためにTGFβ発現の転写制御因子を調べた。ECMにおいてTGFβを制御可能な細胞外タンパク質の主なクラスとしては、小ロイシンリッチプロテオグリカン(SLRP)、例えばデコリンが挙げられる。図8に示されるように、免疫染色により、WT対照肺と比較して、Crtap−/−肺におけるデコリンの発現が増加していることが明らかになった。
OIは、脆弱骨、低骨量、骨変形および骨折を特徴とする。さらに、肺異常を含む骨外性の症状発現は実質的に病的状態および死を導く。OIの症例の多くは、I型コラーゲンをコードする遺伝子の常染色体優性突然変異(COL1A1とCOL1A2)によって引き起こされる。近年、コラーゲンの翻訳後修飾に関与するタンパク質をコードするさらなる遺伝子の突然変異がOIの退行形態を引き起こすものとして同定された。記述された第1番目の変異は、I型コラーゲンのプロリン残基986α1(I)の3−ヒドロキシル化に関わるプロリル−3−ヒドロキシラーゼ複合体の一員である軟骨関連タンパク質(CRTAP)にあった。低形質CRTAP変異は、原線維性コラーゲンにおける3−ヒドロキシプロリン(3Hyp)の部分的損失および他の残基の過剰修飾を導き、優性形態の重度OIと臨床的に重複する退行性OI VII型に帰着した。3Hypの生理学的作用は、完全には理解されていないが、生化学的研究によって、コラーゲンの安定性に否定的に影響するよりもむしろコラーゲン−タンパク質相互作用に関係していることが示唆された。
れた。さらに、Crtap−/−マウスは肺において肺胞気道空間の拡大を示すが、これはマルファン症候群モデルマウスで観察される状態と類似しており、TGFβシグナル伝達の増加が、肺病理の主な要因であることが示された。したがって、OI退行の場合のTGFβシグナル伝達の状態をCrtap−/−モデルマウスにおいて調べた。
対照Crtap−/−マウスと比較して、TGFβ阻害が、骨体積/全体積、骨梁数、骨梁幅を含む骨梁骨パラメータを、WTレベルの近くまで顕著に改善したことを示した(図11Aおよび11B、および図17)。同様の有益な効果が、Crtap−/−マウスの大腿部の骨梁骨で観察され、TGFβ阻害により骨梁骨パラメータが顕著に改善された(図18)。1D11を用いた骨格におけるTGFβ阻害の効果は、WTマウスと、正常TGFβ成熟の機能不全によりTGFβ活性が増加したモデルであるEsl−1−/−マウスとにおいて、以前に報告されている。1D11はWTマウスの背骨で骨梁BV/TVを33%、穏やかに増加させたが、Esl−1−/−マウスでは、BV/TVの106%増加が示された。このことは、骨格中で増加している病態生理学状況でTGFβを標的とすることで、相対的により明白な肯定的な結果に結びつきうることを示唆した。本研究において、1D11は、Crtap−/−マウスの背骨で骨梁BV/TVを235%増加させ、TGFβシグナル伝達の調節異常がCrtap−/−マウスにおける低い骨量の重要な要因であることを支持した。大腿骨中軸では、Ctrap−/−マウスにおいて、皮質の幅、直径、横断面積および横断面の慣性モーメントを含む皮質のアーキテクチャーのパラメータが、WTマウスと比較して顕著に低下していた。1D11処置後、これらのパラメータは、もはやWTマウスと有意な差異はなかった(図19)。皮質および骨梁骨におけるこれらの変化が、骨強度の改善に転換されるどうかを試験するために、3点曲げ試験による大腿骨の生体力学試験を実施した。TGFβ阻害により、処置されたCrtap−/−マウスにおいて最大荷重と極限強度が増加されたことがわかり、全体の骨および組織強度の向上と骨折に対する耐性の改善が示された。しかしながら、1D11処置は、対照および1D11処置Crtap−/−マウスの両方で降伏後変位の低下が示されるように(図20)、OI骨の脆性増加には影響を与えなかった。これはおそらく、変化したコラーゲン構造に関連した固有の異常なミネラル化を反映していると考えられた。まとめると、これらの知見から、TGFβシグナル伝達の増加が、Crtap欠損に起因する退行性OIの骨表現型の主要な要因であり、TGFβシグナル伝達調節異常の阻害により、骨量、微細構造パラメータが回復し、全骨強度が改善することが示された。
期の報告と一致して、WTレベルより低いOc数およびOb数が観察されたことでも、骨再構築プロセスの間の通常のOc数およびOb数を調整するためにTGFβの局所的量の生理学的必要性が強調された。我々の知見は、WTマウスについての1D11処置によりOc数が低下し、Ob数が増加したという従来の研究とは異なった。これは、WTマウスの正常骨と比較して、増加したTGFβシグナル伝達および増加した骨再構築を伴う病態生理学的状況におけるTGFβ阻害の明白な細胞効果を反映している可能性がある。TGFβは骨芽細胞前駆細胞の分化を阻害することが示されており、TGFβシグナル伝達の増加により、未熟な骨芽細胞系細胞の割合が高くなる可能性がある。他方、骨表面における未熟Obの増加または高比率により、これらの細胞による分泌TGFβの量の増加が引き起こされ得る。1D11を用いたTGFβ阻害により、Crtap−/−マウスにおいて増加したOb数が著しく低下するという知見は、TGFβシグナル伝達の増加が、骨芽細胞系細胞の増加に必然的に関与することを示唆している。
のG610C変異(Col1α2tm1.1Mcbr)を保有するノックイン・マウスは、アーミッシュ系集団で最初に同定された優性遺伝される軽度OI型の表現型模写体である。Col1α2tm1.1Mcbrマウスの骨試料において、TGFβ目標遺伝子p21およびPAI−1の発現増加が確認され、TGFβシグナル伝達が上方調節されていることが示唆された(図13A)。一貫して、Col1α2tm1.1Mcbrマウス由来の骨抽出物の免疫ブロット分析においても、活性化pSmad2/全Smad2の比率増加が示され、Crtap−/−マウスにおける我々の観察と類似していた(図13Bおよび13C)。
Claims (24)
- それを必要とする対象における骨形成不全症(OI)を処置する方法であって、該対象に、トランスフォーミング成長因子ベータ(TGFβ)に特異的に結合する治療有効量の抗体またはその抗原結合性断片を投与することを含む前記方法。
- 抗体またはその抗原結合性断片は、配列番号4、5および6からなる群から選択されるアミノ酸配列を有する3つの相補性決定領域(CDR)を含む重鎖可変領域と、配列番号7、8および9からなる群から選択されるアミノ酸配列を有する3つのCDRを含む軽鎖可変領域とを含む、請求項1に記載の方法。
- 抗体またはその抗原結合性断片は、配列番号10のアミノ酸配列を含む重鎖可変領域と、配列番号11のアミノ酸配列を含む軽鎖可変領域とを含む、請求項1または2に記載の方法。
- 抗体またはその抗原結合性断片は、ヒトIgG4定常領域をさらに含む、請求項1〜3のいずれか1項に記載の方法。

- それを必要とする対象における骨形成不全症(OI)を処置する方法であって、該対象に、トランスフォーミング成長因子ベータ(TGFβ)に結合する治療有効量の抗体またはその抗原結合性断片と、少なくとも1つの治療薬剤とを組み合わせて投与することを含む前記方法。
- 薬剤はビスホスホネートである、請求項23に記載の方法。
