バイオマーカーパネル
本発明は、LYVE1、REG1(REG1Aおよび/またはREG1B)、ならびにTFF1を含む、膵管腺癌 (PDAC) の診断に有用なバイオマーカーパネルを提供する。特に、本発明は、生物学的試料中のLYVE1、REG1(REG1Aおよび/もしくはREG1B)、ならびにTFF1、またはこれらの組み合わせからなる群より選択される遺伝子またはタンパク質の発現レベルを検出する段階を含む、膵管腺癌 (PDAC) を診断、スクリーニング、または検査する方法を提供する。好ましい態様では、LYVEI、REG1、およびTFF1のうちの少なくとも2つが用いられる。より好ましい態様では、3つすべてが用いられる。
a) 2〜10ng/ml LYVE1
b) 120〜500 ng/ml REG1A
c) 40〜100 ng/ml REG1B、および/または
d) 2.5〜5 ng/ml TFF1。

本発明はPDACの診断に有用である。PDACは、早期PDAC、例えばI期もしくはII期PDACであってよく、または後期PDAC、例えばIII期もしくはIV期PDACであってもよい。しかしながら、本発明は、早期PDAC、特にI期〜IIA期PDACを発見する上で特に有用である。

生物学的試料は、尿試料、全血試料、血清試料、または生検標本(膵臓組織試料など)であってよいが、尿試料が特に有用である。本方法は、生物学的試料を採取または提供する段階を含み得るか、または代わりに、試料は、例えばエクスビボ方法で患者から既に採取されたものであってもよい。
本発明は、PDACの発見において、ならびに特に早期PDACを後期PDAC、CP、および膵炎と鑑別診断する上で有用なバイオマーカーのパネルを提供する。

を含む、PDACの処置に有用な薬物を同定する方法が提供され、2つの試料間でタンパク質の1つまたは複数の発現レベルまたは濃度が低下していることにより、薬物候補がPDACの可能な処置として見なされる。いくつかの態様において、本方法は、パネルバイオマーカータンパク質のすべてを使用し、すなわち段階 (a) および段階 (c) において、LYVE1、REG1、およびTFF1のすべてを定量化しなくてはならない。いくつかの態様において、生物学的試料は尿試料である。いくつかの態様において、薬物は化合物、抗体、または抗体断片である。
本発明のなおさらなる態様において、LYVE1、REG1(REG1Aおよび/もしくはREG1B)、もしくはTFF1、またはこれらの組み合わせの発現または濃度を定量化する手段を含む、膵管腺癌を検査するための部品のキットが提供される。手段は、任意の適切な検出手段であってよい。
健常、CP、およびPDAC尿標本を、発見段階に関してRoyal London Hospital (RLH) から (n=18)、ならびに検証目的のためにRLHおよびUniversity College London(後に併せて「LON」と称する)、the Department of Surgery, Liverpool University(「LIV」)、およびthe CNIO Madrid, Spain(「SPA」)から(合計で尿371件)入手した。他の良性および悪性の肝胆道病態を有する患者の尿試料 (n=117) は、LIVから入手した。試料はすべて、関連センターから倫理審査の全面的な承認を得て、および尿試料を供与した全個人からインフォームドコンセントを得て採取した。いずれの参加センターにおける標本も、同一の標準操作手順を用いて採取した:クリーンキャッチ中間尿を採取し、採取の2時間以内に凍結し、使用時まで-80℃で貯蔵した。重要なことには、試料はすべて、腎疾患の病歴のない患者に由来した;潜在的ビリルビン血症、タンパク尿、細菌汚染、および血尿を排除するために、ディップスティック検査解析 (Bayer multistix SG 08935414) もまた行った。試料は、手術または化学療法処置の前に採取した。CA19.9を測定するための対応する血漿試料は、RLHおよびLIVから入手可能であった。
各群(H、CP、およびPDAC)につき尿試料6件(男性3件および女性3件;合計で試料18件)を使用した:H 男性/女性の年齢45、50、60歳/44、45、54歳;CP男性/女性の年齢46、48、51歳/47、69、74歳;PDAC 男性/女性の年齢44、74、84歳/71、73、77歳;男性のPDAC病期は全員がIIB/女性は2名がIB、1名がIIA。尿試料はすべて、以前に記載されているように (Weeks ME et al. "Analysis of the urine proteome in patients with pancreatic ductal adenocarcinoma", Proteomics Clinical Applications 2008; 2:1047-57) 脱塩し、濃縮した。1群当たり試料3件の各前処理済みプール20μgを、4〜12%ミニゲル (Invitrogen) にて2つ組で分離した;女性と男性の尿を別々に解析した。ゲルをColloidal Coomassieで染色し、各試料レーンを、グリッドを用いて40個の同じ大きさのスライスに切断した。ゲルスライスをトリプシンでロボット制御により消化し、結果として得られたペプチドを、LTQ Orbitrap XLタンデム質量分析計 (ThermoFisher) に接続されたnanoAcquity (Waters) を用いてナノLC/MS/MSにより解析した。Mascotを用いて産物イオンデータをヒトIPIタンパク質データベースに対して検索し、続いて非重複タンパク質リストへの照合のためにScaffold ソフトウェア (Proteome Software) で解析した。逆データベース検索を用いて偽発見率を評価し、標的タンパク質FDRは試料当たり<0.5%とした。PDAC、CP、および健常試料間の相対タンパク質存在量の半定量的評価は、スペクトル計数アプローチ (Liu H et al., "A model for random sampling and estimation of relative protein abundance in shotgun proteomics", Analytical Chemistry, 2004; 76:4193-201) を用いることにより得た。
尿中の総タンパク質濃度は、Bradfordアッセイ(Coomassie Protein Assay Reagent、Pierce)により決定した。ヒトLYVE-1(Cat# SEB049Hu、Uscn Life Science Inc.)およびヒトTFF1(Cat# ELH-LYVE1-001、RayBiotech, Inc.)の定量的決定は、製造業者の説明書に従って行った:ヒトReG1Aレベルは、最初は本発明者らの研究室で、その後BioVendor Analytical Testing Service (BioVendor - Laboratorni medicina a.s) により評価した。評価する各タンパク質について、精製標準物質を用いて検量線を作成した。製造業者の説明書に従って、4パラメータロジスティック回帰により曲線の当てはめを達成した。ELISAアッセイの各々の検出限界および変動係数 (CV) は、以下の通りであった:LYVE-1 - 8.19 pg/ml、アッセイ内CV - 9%、アッセイ間CV - 12%、TFF1 - 0.037 ng/ml、アッセイ内CV -9%、アッセイ間CV - 12%。REG1A - 0.094 ng/ml、アッセイ内CV - 9%、アッセイ間CV 20%;REG1B- 3.13 pg/ml、アッセイ内CV - 3.9%、アッセイ間CV - 2.7%。尿中クレアチニンは、Clinical Biochemistry Laboratory, RLH (London, UK) において、Roche Cobas 8000システム((Roche Diagnostics、Mannheim, Germany) を用いてJaffe法により測定された。血漿中のCa19.9のレベルは、通例の手順に従ってRoche Modular E170装置を用いて、Clinical Biochemistry Laboratory, RLHにおいて測定された。
バイオマーカーの発現を評価する際に使用した組織マイクロアレイおよびスコアリング手順の詳細は、以前に記載されている (Ene-Obong A et al., "Activated pancreatic stellate cells sequester CD8+ T cells to reduce their infiltration of the juxtatumoral compartment of pancreatic ductal adenocarcinoma", Gastroenterology, 2013; 145:1121-32)。IHCは、標準プロトコール(sCC1、1Hインキュベーション)に従って、Ventana Discoveryシステムを使用して抗REG1A抗体(Abcam、ウサギポリクローナル、ab47099、1:100希釈)、抗TFF1抗体(Abcam、ウサギポリクローナル、ab50806、1:100希釈)、および抗LYVE1抗体(Acris、ウサギポリクローナル、DP3500PS、1:100希釈)を用いて行った。
MSデータから潜在的尿中バイオマーカーを同定するため、Arraytrackソフトウェア (http;edkb.fda.go/webstart/arraytrack) およびt検定を用いて、標準化データ(スペクトルカウント/試料の和に基づく)に対して統計解析を行った。任意の2つの試料群間のp値および変化倍率の両方に従って、データにさらにフィルターをかけた。
R「pROC」パッケージ (Robin X et al., "pROC: an open-source package for R and S+ to analyse and compare ROC curves", BMC Bioinformatics, 2011; 12:77) の「ci.閾値」手順により算出して、
最少((1 - 感度)2 + (1 - 特異度)2)。
このアプローチは、バイオマーカーの最適カットポイントの推定において良好な性能を有することが示されている。
PDAC、CP、および健常 (H) 個体に由来する18件の尿標本(1群当たり6件、男性3件、女性3件)のGeLC/MS/MSによる徹底的なプロテオミクス解析(図6A)を行った。この解析により、およそ1,500個(男性尿において1,198個および女性尿において1,061個)の非重複タンパク質が同定された。これらのタンパク質はすべての細胞区画に由来し、IPA(Ingenuity pathway analysis、http://www.ingenuity.com/)を用いていくつかの細胞機能および疾患に対してマッピングされ、このことから、尿がそれらの起源および機能的役割に関して多様なタンパク質の豊富な供給源を提供することが確認された。
続いて、3つのセンター:LondonおよびLiverpool, UK、ならびにMadrid, Spainから収集された371件の尿試料に対してELISAアッセイを用いて、選択されたバイオマーカーを評価した。研究に含めた患者および健常参加者の人口統計および臨床的特徴を、表1(次ページ)に示す。
ELISA解析から、健常試料 (n=87) と比較した場合に、PDAC患者 (n=192) の尿中の各候補バイオマーカーの尿中濃度が有意により高いことが示された(いずれもp<0.0001、図1)。留意すべきは、REG1BとREG1AのELISAアッセイが、同様の結果をもたらしたことである(図1)。
次に、早期癌を健常個体と識別する際のバイオマーカーの能力を評価した。腫瘍病期の情報は、148名 (77%) のPDAC患者について入手可能であった。各バイオマーカーの濃度は、健常者 (n=87) と比較して、より後期(III〜IV期、n=77、いずれもp<0.001)、I〜II期(n=71、いずれもp<0.001)、およびI〜IIA期(リンパ節転移のない局所浸潤性疾患、n=16、p<0.05)で有意に上昇していた(図3)。LYVE1およびTFF1の濃度は、I期癌においてもより高かった(それぞれp<0.001およびp<0.05;データは示さず)。限られた数のI期尿試料しか入手できなかったため (n=7)、組み合わせPDAC I〜II期データについての尿中マーカーの診断精度を評価した。最初に、新たな訓練データセット(試料の70%;それぞれPDAC I〜II期 n=56および健常 n=61)において、PDAC I〜II期尿と健常尿を識別する際の個々のマーカーおよびパネルの性能を評価した。この訓練データセットを用いて新たな5パラメータモデルを構築し、データの残り(試料の30%;PDAC I〜II期 n=15、健常 n=26)を用いて検証した(図4A、B)。パネルは、訓練および検証データセットのそれぞれにおいて、AUC 0.900(95% CI 0.843〜0.957)および0.926(95% CI 0.843〜1.000)を達成した(図4C)。したがって、尿中バイオマーカーパネルは、高精度で早期PDAC試料を健常試料とで区別し得る。
PDACをCPと区別する際のバイオマーカーパネルの能力を評価した。
全3つのバイオマーカーの尿中濃度は、CP試料 (N=92) と比較してPDAC試料 (n=192) において、いずれもp<0.001を有してより高く(図1)、PDACと同様に、CPデータにおいてもバイオマーカー濃度は相互に正に相関していた(図7)。訓練データセット(PDAC n=143、CP n=62)において、LYVE1およびREG1は、SN 77〜78%およびSP 66〜69%(それぞれのAUC値は0.775(95%CI 0.704〜0.846)および0.722(95%CI 0.643〜0.801))を有して2つの群を識別することができたのに対して(図8)、TFF1のSPは、同様のSNに対して50%に届くにすぎなかった。3つのバイオマーカーを組み合わせてパネルにすると、訓練データセット(AUC= 0.815、95%CI 0.752〜0.878)および検証データセット(PDAC n=49、CP n =30、AUC= 0.839、95%CI 0.751〜0.928)において評価した場合に、単独のLYVE1およびREG1の性能がわずかに改善されただけであった。
バイオマーカー尿中濃度は、CP (n=87) と比較してI〜II期 PDAC (n=71) において、3つのバイオマーカーのそれぞれについてp<0.001を有して有意に上昇していた(データは示さず)。パネルは、訓練データセット(PDAC I〜II期 n=56、CP n=66)および検証データセット(PDAC I〜II期 n=15、CP n=26)の両方において高いSN (>85%) を達成したが、個々のバイオマーカーについて認められたSPと同様に、SPは比較的低く(66.7%および50%)、それぞれのAUCは0.831(95%CI 0.762〜0.901)および0.846(95%CI 0.730〜0.963)であった(図8D〜F)。
最後に、いくつかの他の良性または悪性肝胆道病態を有する患者から採取された尿標本中のバイオマーカーの発現を調べ、早期PDAC患者における発現と比較した(図10)。PDAC I〜II期試料におけるLYVE1の尿中レベルは、IPMN、AMP、および膵臓NET標本よりも高かった;REG1Aレベルは唯一、IPMNと比較して早期PDACにおいて有意により高かった。血漿CA19.9レベルは、膵臓NETおよびDuCA試料と比較してPDAC I〜II期において有意により高かった。このことから、他の良性または悪性肝胆道病態を早期PDACと区別する際のLYVE1およびREG1Aの潜在的有用性が示唆され得る。
早期癌患者を健常個体と区別する際のパネルの良好な性能が実証され、次に膵臓組織におけるバイオマーカーの発現を確証しようと努めた。自家構築のPDAC組織マイクロアレイを用いて、免疫組織化学検査 (IHC) を行った。REG1Aの強力な発現は、組織学的に正常な隣接腺房細胞において認められたが、染色はまた44/60の腫瘍 (73%) においても認められた(図11A)。TFF1は正常膵臓には存在しなかったが、PDACの43/60 (72%) において発現されていた(図11B)。LYVE1発現は癌細胞のいずれにおいても認められなかったが、PDAC組織8件のリンパ管において稀に認められた(図11C)。次に、全3つのバイオマーカーのレベルを、手術の前後に試料が採取された7名のPDAC患者由来の尿で測定した(図11D)。おそらくは術後に腫瘍量が実質的に減少したために、すべての患者において、LYVE1およびREG1Aのレベルが術後に低下し、これはTFF1に関しても患者7名のうち6名において認められた(患者1以外、この場合、最初の術後尿試料は手術の4カ月後に採取された)。
