Abstract
Description
本出願は、2014年6月13日に出願された米国仮特許出願第62/012,109号、および2014年9月4日に出願された米国仮特許出願第62/045,808号に対する優先権の利益を主張する。上記の出願のそれぞれの開示は、その全体が本明細書に援用される。
成熟した赤血球(red blood cell)、すなわちエリスロサイト(erythrocyte)は、脊椎動物の循環系において酸素の輸送を担っている。赤血球は、比較的高い酸素分圧(pO2)の肺において酸素に結合し、そして、比較的低いpO2の身体の領域へと酸素を運搬するタンパク質であるヘモグロビンを高濃度で含む。
したがって、貧血と関連する潰瘍を処置または予防するための、代替的な方法を提供することは本開示の目的である。
(1.概要)
トランスフォーミング増殖因子−β(TGF−β)スーパーファミリーは、共通の配列エレメントと構造モチーフを共有する、種々の増殖因子を含む。これらのタンパク質は、脊椎動物および無脊椎動物の両方における広範な種々の細胞型に対して生物学的作用を発揮することが公知である。このスーパーファミリーのメンバーは、胚発生の間に、パターン形成および組織の特異化において重要な機能を果たし、そして、脂質生成、筋発生、軟骨形成、心臓発生、造血、神経発生および上皮細胞分化を含む種々の分化プロセスに影響を及ぼし得る。TGF−βファミリーのメンバーの活性を操作することによって、しばしば、生物において顕著な生理学的変化を引き起こすことが可能である。例えば、ウシのPiedmonteseおよびBelgian Blue品種は、GDF8(ミオスタチンとも呼ばれる)遺伝子に機能喪失変異を有しており、筋肉量の顕著な増加を引き起こしている。例えば、Grobetら(1997年)、Nat Genet.、17巻(1号):71〜4頁を参照されたい。さらに、ヒトでは、GDF8の不活性な対立遺伝子は、筋肉量の増加、および、報告によれば、例外的な強度と関連している。例えば、Schuelkeら(2004年)、N Engl J Med、350巻:2682〜8頁を参照されたい。
本明細書で提示されるデータは、ActRIIのアンタゴニスト(阻害剤)(例えば、SMAD2/3および/またはSMAD1/5/8のシグナル伝達など、ActRIIAおよび/またはActRIIBのシグナル伝達のアンタゴニスト)を、インビボにおける赤血球レベルを高めるのに使用し得ることを実証する。特に、本明細書では、このようなActRIIアンタゴニストは、種々の貧血のほか、例えば、皮膚潰瘍を含む、貧血の種々の合併症(例えば、障害/状態)も処置するのに有効であることが示される。したがって、本開示は部分的に、単独で、あるいは1つもしくは複数の赤血球生成刺激因子(例えば、EPO)または他の支持療法[例えば、ヒドロキシウレアによる処置、輸血、鉄キレート化療法、および/または疼痛管理(例えば、オピオイド鎮痛剤、非ステロイド系抗炎症薬、および/またはコルチコステロイドの1つまたは複数による処置)]と組み合わせて、それを必要とする対象において貧血を処置もしくは予防し、かつ/または貧血を有する患者において皮膚潰瘍を処置もしくは予防するのに使用され得る、種々のActRIIアンタゴニスト因子を提供する。
A.ActRIIポリペプチドおよびGDFトラップ
ある特定の実施形態では、本開示は、本明細書で開示されるActRIIポリペプチドおよびGDFトラップポリペプチド(そのフラグメント、機能的改変体、および融合タンパク質を含む)をコードする、単離核酸および/または組換え核酸を提供する。例えば、配列番号12は、天然に存在するヒトActRIIA前駆体のポリペプチドをコードするが、配列番号13は、ActRIIAのプロセシング後の細胞外ドメインをコードする。さらに、配列番号7は、天然に存在するヒトActRIIB前駆体ポリペプチド(上記のR64改変体)をコードするが、配列番号8は、ActRIIBのプロセシング後の細胞外ドメイン(上記のR64改変体)をコードする。対象核酸は、一本鎖でもよく、二本鎖でもよい。このような核酸は、DNA分子でもよく、RNA分子でもよい。これらの核酸は、例えば、本開示のActRIIベースのリガンドトラップポリペプチドを作製するための方法において使用することができる。

他の態様では、本明細書で開示される方法(例えば、それを必要とする対象において貧血を処置もしくは予防する方法、および/または貧血の1つもしくは複数の合併症(例えば、皮膚潰瘍を含む)を処置もしくは予防する方法)に従う使用のための作用因子は、フォリスタチンポリペプチドである。用語「フォリスタチンポリペプチド」は、フォリスタチンの任意の天然に存在するポリペプチドのほか、有用な活性を保持するその任意の改変体(変異体、フラグメント、融合物、およびペプチド模倣物形態を含む)も含むポリペプチドを含み、フォリスタチンの任意の機能的な単量体または多量体もさらに含む。ある特定の実施形態では、本開示のフォリスタチンポリペプチドは、アクチビン、特に、アクチビンAに結合し、かつ/またはこれらの活性(例えば、SMAD2/3のシグナル伝達など、ActRIIAおよび/またはActRIIBのシグナル伝達の、アクチビンに媒介される活性化)を阻害する。アクチビンへの結合特性を保持する、フォリスタチンポリペプチドの改変体は、フォリスタチンとアクチビンとの相互作用に関するかつての研究に基づき同定することができる。例えば、WO2008/030367は、アクチビンへの結合に重要であることが示されている、特異的なフォリスタチンドメイン(「FSD」)について開示している。下記の配列番号18〜20に示される通り、フォリスタチンのN末端ドメイン(「FSND」;配列番号18)、FSD2(配列番号20)は、フォリスタチン内の例示的なドメインであって、アクチビンへの結合に重要なドメインを表し、程度は劣るが、FSD1(配列番号19)も、フォリスタチン内の例示的なドメインであって、アクチビンへの結合に重要なドメインを表す。加えて、上記では、ActRIIポリペプチドの文脈において、ポリペプチドのライブラリーを作製し、これについて調べるための方法についても記載したが、このような方法はまた、フォリスタチンの改変体を作製し、これらについて調べることにも関する。フォリスタチンポリペプチドは、任意の公知のフォリスタチン配列に由来するポリペプチドであって、フォリスタチンポリペプチドの配列に少なくとも約80%同一である配列を有し、任意選択で、少なくとも85%、90%、95%、96%、97%、98%、99%、またはこれを超える同一性を有するポリペプチドを含む。フォリスタチンポリペプチドの例は、成熟フォリスタチンポリペプチド、または、例えば、WO2005/025601において記載されている、ヒトフォリスタチン前駆体ポリペプチド(配列番号16)の短いアイソフォームもしくは他の改変体を含む。
ある特定の態様では、本開示は、対象のActRIIポリペプチド(例えば、ActRIIAポリペプチドおよびActRIIBポリペプチド)およびGDFトラップポリペプチドの使用であって、ActRIIBポリペプチドのアゴニストまたはアンタゴニストである化合物(作用因子)を同定するための使用に関する。このスクリーニングを介して同定される化合物を試験して、赤血球レベル、ヘモグロビンレベル、および/または網状赤血球レベルを調節し、ならびに皮膚潰瘍に作用するそれらの能力を評価することができる。これらの化合物は、例えば、動物モデルにおいて試験することができる。
5.例示的な治療的使用
潰瘍および貧血
潰瘍および鎌状赤血球症
ある特定の態様では、任意選択で、1つまたは複数の因子および/または支持療法と組み合わせた、本開示のActRIIアンタゴニスト因子を使用して、それを必要とする対象において無効赤血球生成を処置または予防することができる。鉄動態研究に基づき、再生不良性貧血、出血、または末梢溶血とは元来識別される(Rickettsら、1978年、Clin Nucl Med、3巻:159〜164頁)、無効赤血球生成は、成熟RBCの生成が、骨髄中に存在する赤芽球系前駆体(赤芽球)の数が与えられたときに予測されるより少ない、多様な貧血群を説明している(Tannoら、2010年、Adv Hematol、2010巻:358283頁)。このような貧血では、エリスロポエチンレベルの上昇にもかかわらず、成熟RBC生成の無効に起因して、組織低酸素症が遷延する。最終的には、エリスロポエチンレベルの上昇が赤芽球の大規模な増殖を駆動する悪循環が発生し、これは、髄外赤血球生成に起因する脾腫(脾臓の腫大)(Aizawaら、2003年、Am J Hematol、74巻:68〜72頁)、赤芽球誘導性骨病態(Di Matteoら、2008年、J Biol Regul Homeost Agents、22巻:211〜216頁)を潜在的にもたらし、治療的RBC輸血の非存在下においてもなお、組織鉄過負荷(Pippardら、1979年、Lancet、2巻:819〜821頁)を潜在的にもたらす。したがって、赤血球生成の有効性を促進することにより、本開示のActRIIアンタゴニストは、前述の悪循環を打破し、根底にある貧血だけでなく、エリスロポエチンレベルの上昇、脾腫、骨病態、および組織鉄過負荷といった関連する合併症も緩和することができる。ActRIIアンタゴニストは、貧血およびEPOレベルの上昇のほか、脾腫、赤芽球誘導性骨病態、および鉄過負荷、皮膚潰瘍ならびにそれらの付随する病態などの合併症を含む、無効赤血球生成を処置することができる。脾腫に関して、このような病態は、胸痛または腹痛および網内系過形成を含む。髄外造血は、脾臓内だけでなく、潜在的には他の組織内でも、髄外造血性偽腫瘍の形態で生じる場合がある(Musallamら、2012年、Cold Spring Harb Perspect Med、2巻:a013482頁)。赤芽球誘導性骨病態に関して、付随する病態は、低骨塩密度、骨粗鬆症、および骨疼痛を含む(Haidarら、2011年、Bone、48巻:425〜432頁)。鉄過負荷に関して、付随する病態は、ヘプシジン抑制および食餌中の鉄の過剰吸収(Musallamら、2012年、Blood Rev、26巻(補遺1号):S16〜S19頁)、複数の内分泌障害および肝線維症/肝硬変(Galanelloら、2010年、Orphanet J Rare Dis、5巻:11頁)、ならびに鉄過剰性心筋症(Lekawanvijitら、2009年、Can J Cardiol、25巻:213〜218頁)を含む。
他の貧血適応
ある特定の態様では、本開示の1つまたは複数のActRIIアンタゴニスト作用因子(例えば、GDF−ActRIIアンタゴニスト、ActRIIAポリペプチド、ActRIIBポリペプチド、GDFトラップなど)は、単独で投与することもでき、医薬製剤(治療用組成物または薬学的組成物とも称される)の成分として投与することもできる。医薬製剤とは、その中に含有される活性成分(例えば、本開示の作用因子)の生物活性が効果的であることを可能とするような形態にあり、製剤が投与される対象に対して許容不可能に毒性である、さらなる成分を含有しない調製物を指す。本主題のActRIIアンタゴニスト作用因子は、ヒト医療または獣医療における使用のために好都合な任意の方式における投与のために製剤化され得る。例えば、本開示の1つまたは複数のActRIIアンタゴニスト作用因子は、薬学的に許容されるキャリアと共に製剤化することができる。薬学的に許容されるキャリアとは、医薬製剤中の、活性成分以外の成分であって、対象に対して一般に非毒性である成分を指す。薬学的に許容されるキャリアは、緩衝剤、賦形剤、安定化剤、および/または保存剤を含むがこれらに限定されない。一般に、本開示における使用のための医薬製剤は、対象に投与されるとき、発熱物質非含有であり、生理学的に許容される形態にある。本明細書に記載のもの以外の治療的に有用な作用因子であって、任意選択で、上記の製剤中に含まれ得る作用因子を、本開示の方法における主題のActRIIアンタゴニスト作用因子と組み合わせて投与することができる。

ActRIIa−Fc融合タンパク質
本出願人は、間に最小限のリンカーを有する、ヒトActRIIaの細胞外ドメインを、ヒトFcドメインまたはマウスFcドメインに融合させた、可溶性ActRIIA融合タンパク質を構築した。構築物を、それぞれ、ActRIIA−hFcおよびActRIIA−mFcと呼ぶ。

ActRIIA−hFcタンパク質の特徴付け
ActRIIA−hFc融合タンパク質は、安定的にトランスフェクトされたCHO−DUKX B11細胞内で、pAID4ベクター(SV40 ori/エンハンサー、CMVプロモーター)から、配列番号24の組織プラスミノーゲンリーダー配列を使用して発現させた。上記の実施例1で記載した通りに精製されたタンパク質は、配列番号22の配列を有した。Fc部分は、配列番号22において示される、ヒトIgG1のFc配列である。タンパク質解析は、ActRIIA−hFc融合タンパク質は、ジスルフィド結合によるホモ二量体として形成されることを明らかにする。

研究では、各々、雄カニクイザル5匹および雌カニクイザル5匹ずつによる4つの群を使用し、群当たり性別当たり3匹ずつを、29日目に終了するようにスケジュールにし、群当たり性別当たり2匹ずつを、57日目に終了するようにスケジュールにした。各動物に、1、8、15、および22日目において、静脈内(IV)注射を介して、ビヒクル(群I)または1、10、もしくは30mg/kg(それぞれ、群2、3、および4)の用量のActRIIA−Fcを投与した。投薬容量は、3mL/kgで維持した。赤血球レベルの種々の尺度は、初回投与の2日前、ならびに初回投与後15、29、および57日目(残りの2匹の動物について)に評価した。
ActRIIA−hFcは、ヒト患者において赤血球レベルを高め、かつ骨形成のマーカーを増加させる
実施例1で記載したActRIIA−hFc融合タンパク質を、主に、健常な閉経後女性におけるタンパク質の安全性を評価するように実施された、無作為化、二重盲検、プラセボ対照研究において、ヒト患者に投与した。48例の対象を、6つのコホートに無作為化して、単一用量のActRIIA−hFcまたはプラセボを施した(5つが実薬(active):1つがプラセボ)。用量レベルは、静脈内(IV)で0.01〜3.0mg/kgの範囲とし、皮下(SC)で0.03〜0.1mg/kgの範囲とした。全ての対象は、120日間にわたり追跡した。薬物動態(PK)解析に加えて、ActRIIA−hFcの生体活性もまた、骨形成および骨吸収についての生化学的マーカーの測定ならびにFSHレベルの測定により評価した。
貧血患者の、ActRIIA−hFcによる処置
0.1mg/kg、0.3mg/kg、および1.0mg/kgという30の用量レベルで、30日ごとに投薬を行う、複数回用量のActRIIA−hFcにより患者を処置するように、臨床研究をデザインした。試験において、正常な健常患者は、一部の場合に、ヘモグロビン(Hg)およびヘマトクリット(Hct)が、正常な範囲を超えて増大することを除き、実施例4で報告した、第I相臨床試験において見られる増大と符合するヘモグロビンおよびヘマトクリットの増大を呈示した。ヘモグロビンレベルがおよそ7.5g/dLである貧血性患者にもまた、1mg/kgのレベルで2用量を施したところ、2カ月後において、およそ10.5g/dLのヘモグロビンレベルが結果としてもたらされた。患者の貧血は、慢性鉄欠損により引き起こされると考えられる、小球性貧血であった。
代替的なActRIIA−Fcタンパク質
本明細書で記載される方法に従い使用され得る、様々なActRIIA改変体は、参照によりその全体において本明細書に組み込まれる、WO2006/012627として公開されている国際特許出願(例えば、59〜60頁を参照されたい)において記載されている。代替的な構築物は、C末端テール(ActRIIAの細胞外ドメインの最後の15アミノ酸)を欠失させている場合がある。このような構築物の配列を下記(Fc部分に下線を付す)(配列番号28)に提示する。
ActRIIB−Fc融合タンパク質の生成
本出願人は、間に最小限のリンカー(3つのグリシンアミノ酸)を有する、ヒトActRIIBの細胞外ドメインを、ヒトFcドメインまたはマウスFcドメインに融合させた、可溶性ActRIIB融合タンパク質を構築した。構築物を、それぞれ、ActRIIB−hFcおよびActRIIB−mFcと呼ぶ。

ActRIIB−Fcは、非ヒト霊長動物における赤血球生成を刺激する
カニクイザルを、7つの群(群当たり性別当たり6匹ずつ)に割りつけ、ActRIIB(20〜134)−hFcを、投与量0.6、3、または15mg/kgで、2週間ごとまたは4週間ごとに、9カ月間にわたる皮下注射として投与した。対照群(群当たり性別当たり6匹ずつ)には、ActRIIB(20〜134)−hFc処置動物と同じ投薬容量(用量当たり0.5ml/kg)で、ビヒクルを施した。一般的な臨床病態パラメータ(例えば、血液学パラメータ、臨床化学パラメータ、凝固パラメータ、および尿検査パラメータ)の変化について、動物をモニタリングした。血液学パラメータ、凝固パラメータ、および臨床化学パラメータ(鉄パラメータ、リパーゼ、およびアミラーゼを含む)は、投薬を開始する前、ならびに59、143、199、227日目、および267日目(4週間ごとに投薬される群について)または281日目(2週間ごとに投薬される群について)において、2回評価した。267/281日目における評価は、最終用量を投与した2週間後に行った。
GDFトラップの生成
本出願人は、以下の通りに、GDFトラップを構築した。アクチビンAへの結合を、GDF11および/またはミオスタチンと比べて大幅に低減した(配列番号1内の79位における、ロイシンからアスパラギン酸への置換の帰結として)、改変ActRIIBの細胞外ドメイン(L79D置換を有する配列番号1のアミノ酸20〜134)を有するポリペプチドを、間に最小限のリンカー(3つのグリシンアミノ酸)を有する、ヒトFcドメインまたはマウスFcドメインに融合させた。構築物を、それぞれ、ActRIIB(L79D 20〜134)−hFcおよびActRIIB(L79D 20〜134)−mFcと呼ぶ。79位にアスパラギン酸ではなく、グルタミン酸を有する代替的形態についても、同様に実施した(L79E)。下記の配列番号36に照らした226位において、バリンではなく、アラニンを有する代替的形態もまた、生成し、調べる全ての点において、同等に実施した。79位におけるアスパラギン酸(配列番号1に照らした;または配列番号36に照らした60位)は、下記で二重下線により指し示す。配列番号36に照らした226位におけるバリンもまた、下記で二重下線により指し示す。

GDF−11およびアクチビンに媒介されるシグナル伝達についてのバイオアッセイ
A−204レポーター遺伝子アッセイを使用して、ActRIIB−Fcタンパク質およびGDFトラップの、GDF−11およびアクチビンAによるシグナル伝達に対する効果を評価した。細胞株:ヒト横紋筋肉腫(筋肉に由来する)。レポーターベクター:pGL3(CAGA)12(Dennlerら、1998年、EMBO、17巻:3091〜3100頁において記載されている)。CAGA12モチーフは、TGFベータ応答性遺伝子(例えば、PAI−1遺伝子)内に存在するので、このベクターは、SMAD2およびSMAD3を介してシグナル伝達する因子のために一般的に使用される。
1日目:A−204細胞を、48ウェルプレートに分ける。
2日目:A−204細胞に、10ugのpGL3(CAGA)12またはpGL3(CAGA)12(10ug)+pRLCMV(1μg)およびFugeneをトランスフェクトする。
3日目:因子(培地+0.1%のBSA中に希釈された)を添加する。細胞に添加する前に、1時間にわたり、阻害剤を、因子と共に、あらかじめインキュベートする必要がある。6時間後、細胞を、PBSですすぎ、溶解させた。
ActRIIB−Fc改変体の細胞ベースの活性
ActRIIB−Fcタンパク質およびGDFトラップの活性を、上記で記載した細胞ベースのアッセイで調べた。結果を、下記の表にまとめる。一部の改変体を、異なるC末端切断構築物で調べた。上記で論じた通り、5または15アミノ酸の切断は、活性の低減を引き起こした。GDFトラップ(L79D改変体およびL79E改変体)は、GDF−11の野生型阻害をほとんど保持しながら、アクチビンA阻害の実質的な喪失を示した。

12週齢の雄C57BL/6NTacマウスを、2つの処置群(N=10)の1つに割り当てた。マウスに、ビヒクルまたは改変体ActRIIBポリペプチド(「GDFトラップ」)[ActRIIB(L79D 20〜134)−hFc]を、皮下注射(SC)により、10mg/kgで、毎週2回ずつ、4週間にわたり投薬した。研究終了時において、全血液を、心臓穿刺により、EDTAを含有する管へと回収し、HM2血液解析器(Abaxis,Inc)を使用して、細胞分布について解析した。
GDFトラップは、インビボにおける赤血球レベルを高めるのに、ActRIIB−Fcより優れている
19週齢の雄C57BL/6NTacマウスを、3つの群の1つに無作為に割り当てた。マウスに、ビヒクル(10mMのトリス緩衝生理食塩液、TBS)、野生型ActRIIB(20〜134)−mFc、またはGDFトラップであるActRIIB(L79D 20〜134)−hFcを、皮下注射により、毎週2回ずつ、3週間にわたり投薬した。血液は、ベースラインおよび3週間の投薬後における回収頬部採血であり、、血液解析器(HM2、Abaxis,Inc.)を使用して、細胞分布について解析した。
短縮型ActRIIB細胞外ドメインを含むGDFトラップの生成
実施例9で記載した通り、ActRIIB(L79D 20〜134)−hFcと呼ばれるGDFトラップを、N末端における、TPAリーダーの、ロイシンからアスパラギン酸への置換(配列番号1内の残基79における)を含有するActRIIB細胞外ドメイン(配列番号1内の残基20〜134)への融合と、C末端における、ヒトFcドメインの、最小限のリンカー(3つのグリシン残基)による融合とにより生成した(図16)。この融合タンパク質に対応するヌクレオチド配列を、図17Aおよび17Bに示す。
二重短縮型ActRIIB細胞外ドメインを有するGDFトラップによる選択的リガンド結合
GDFトラップおよび他のActRIIB−hFcタンパク質の、いくつかのリガンドに対するアフィニティーを、インビトロにおいて、Biacore(商標)装置により評価した。結果を、下記の表にまとめる。Kd値は、複合体の会合および解離が極めて急速であり、konおよびkoffの正確な決定が妨げられるために、定常状態アフィニティーの当てはめにより得た。
代替的なヌクレオチド配列によるActRIIB(L79D 25〜131)−hFcの生成
ActRIIB(L79D 25〜131)−hFcを生成するために、天然の79位(配列番号1)においてアスパラギン酸置換を有し、N末端およびC末端を短縮した(配列番号1内の残基25〜131)ヒトActRIIB細胞外ドメインを、N末端において、天然のActRIIBリーダーの代わりに、TPAリーダー配列と融合させ、C末端において、最小限のリンカー(3つのグリシン残基)を介して、ヒトFcドメインと融合させた(図18)。この融合タンパク質をコードする1つのヌクレオチド配列を、図19(配列番号42)に示し、正確に同じ融合タンパク質をコードする代替的なヌクレオチド配列を、図22Aおよび22B(配列番号46)に示す。このタンパク質は、実施例9で記載した方法を使用して、発現させ、精製した。
短縮型ActRIIB細胞外ドメインを有するGDFトラップは、マウスにおける赤血球系前駆細胞の増殖を高める
ActRIIB(L79D 25〜131)−hFcを、評価して、赤血球系前駆細胞の増殖に対するその効果を決定した。雄C57BL/6マウス(8週齢)を、1および4日目において、ActRIIB(L79D 25〜131)−hFc(10mg/kg、s.c.;n=6)またはビヒクル(TBS;n=6)で処置し、次いで、8日目に、脾臓、脛骨、大腿骨、および血液を回収するために安楽死させた。脾臓および骨髄の細胞を単離し、5%のウシ胎仔血清を含有するIscoveによる改変ダルベッコ培地中で希釈し、特製のメチルセルロースベースの培地中に懸濁させ、2または12日間にわたり培養して、それぞれ、コロニー形成単位赤芽球(CFU−E)期およびバースト形成単位赤芽球(BFU−E)期にあるクローン生成性前駆細胞のレベルを評価した。BFU−Eを決定するためのメチルセルロースベースの培地(MethoCult M3434、Stem Cell Technologies)は、CFU−Eを決定するためのメチルセルロース培地(MethoCult M3334、Stem Cell Technologies)中には存在しない、組換えマウス幹細胞因子、インターロイキン3、およびインターロイキン6を含む一方、いずれの培地も、他の成分の中でもとりわけ、エリスロポエチンを含有した。BFU−EおよびCFU−Eのいずれについても、各組織試料に由来する、二連の培養プレート内でコロニー数を決定し、結果についての統計学的解析は、処置群当たりのマウスの数に基づいた。
短縮型ActRIIB細胞外ドメインを有するGDFトラップは、非ヒト霊長動物において赤血球レベルを高める
2つのGDFトラップである、ActRIIB(L79D 20〜134)−hFcおよびActRIIB(L79D 25〜131)−hFcを、カニクイザルにおける赤血球生成を刺激するそれらの能力について評価した。サルを、GDFトラップ(10mg/kg;n=4匹の雄/4匹の雌)、またはビヒクル(n=2匹の雄/2匹の雌)で、1および8日目に、皮下処置した。血液試料は、1(処置前のベースライン)、3、8、15、29、および44日目に回収し、赤血球レベル(図24)、ヘマトクリット(図25)、ヘモグロビンレベル(図26)、および網状赤血球レベル(図27)について解析した。ビヒクルで処置されたサルは、全ての処置後時点において、反復した血液サンプリングの予想される効果である、赤血球、ヘマトクリット、およびヘモグロビンのレベルの低下を呈示した。これに対し、ActRIIB(L79D 20〜134)−hFcまたはActRIIB(L79D 25〜131)−hFcによる処置は、最初の処置後時点(3日目)までに、これらのパラメータを増大させ、それらを、実質的に上昇したレベルで、研究期間にわたり維持した(図24〜26)。ActRIIB(L79D 20〜134)−hFcまたはActRIIB(L79D 25〜131)−hFcで処置されたサルにおける網状赤血球レベルが、8、15、および29日目において、ビヒクルと比較して実質的に上昇した(図27)ことは、重要である。この結果は、GDFトラップによる処置が、赤血球前駆体の生成を増大させ、赤血球レベルの上昇を結果としてもたらすことを実証する。
ActRIIB5に由来するGDFトラップ
他の研究者らも、ActRIIBの膜貫通ドメインを含むエクソン4が、異なるC末端配列により置きかえられた、ActRIIBの代替的な可溶性形態(ActRIIB5と表記される)について報告している。例えば、WO2007/053775を参照されたい。
マウスにおける、EPOおよび短縮型ActRIIB細胞外ドメインを有するGDFトラップによる組み合わせ処置の効果
EPOが、赤血球系前駆体の増殖を増大させることにより、赤血球の形成を誘導するのに対し、GDFトラップは、EPOの効果を補完または増強する形で、赤血球の形成に潜在的に影響を及ぼし得る。したがって、本出願人らは、EPOおよびActRIIB(L79D 25〜131)−hFcによる組み合わせ処置の、赤血球生成パラメータに対する効果について探索した。雄C57BL/6マウス(9週齢)に、組換えヒトEPO単独(エポエチンアルファ、1800単位/kg)、ActRIIB(L79D 25〜131)−hFc単独(10mg/kg)、EPOおよびActRIIB(L79D 25〜131)−hFcの両方、またはビヒクル(トリス緩衝生理食塩液)の単回i.p.注射を施した。血液、脾臓、および大腿骨を回収するために、マウスを、投薬の72時間後に安楽死させた。

から選択される、請求項110に記載の使用。 - 前記GDFトラップポリペプチドが、配列番号1に照らして、79位に酸性アミノ酸を含む、請求項111に記載の使用。
- 前記ポリペプチドが、ActRIIA、ActRIIB、またはGDFトラップのポリペプチドドメインに加えて、インビボ半減期、インビトロ半減期、投与、組織局在化または分布、タンパク質複合体の形成、および精製のうちの1つまたは複数を増強する1つまたは複数の異種ポリペプチドドメインを含む融合タンパク質である、請求項106から112のいずれか一項に記載の使用。

- 前記融合タンパク質が、前記ActRIIA、ActRIIB、またはGDFトラップのポリペプチドドメインと前記免疫グロブリンFcドメインとの間に位置するリンカードメインをさらに含む、請求項114から116のいずれか一項に記載の使用。

から選択されるポリペプチドを含むGDFトラップポリペプチド−Fc融合タンパク質である、請求項113から118のいずれか一項に記載の使用。 - 前記GDFトラップポリペプチド−Fc融合タンパク質が、配列番号1に照らして、79位に酸性アミノ酸を含む、請求項121に記載の使用。
- 前記ポリペプチドが、グリコシル化アミノ酸、PEG化アミノ酸、ファルネシル化アミノ酸、アセチル化アミノ酸、ビオチニル化アミノ酸、脂質部分に結合体化されたアミノ酸、および有機誘導体化剤に結合体化されたアミノ酸から選択される、1つまたは複数のアミノ酸修飾を含む、請求項106から122のいずれか一項に記載の使用。
