Abstract
Description
本出願は、2014年7月23日に出願された米国特許出願第62/028,286、及び、2015年5月28日に出願された米国特許出願第62/167,624についての米国特許法第119条の優先権を主張する。それらの内容は、当該参照によって本明細書に組み入れられる。

本発明は、心不全を治療するための装置及びその使用方法に関している。本発明の一つの特徴は、例えばシャントを形成して当該シャントの治療効果を高めるべく当該シャントを通る血流を選択的に調整することによって、心腔内で血圧を変更(例えば低減)するために用いられ得る装置に関している。本発明は、更に、例えば鬱血性心不全及びその関連状態、例えば心臓の左心房内部で増大された圧力によって引き起こされる急性心原性肺水腫、を治療する際において、そのような装置を利用する方法に関している。
Claims (12)
- 患者の心房中隔内への移植のための装置であって、
中央開口を取り囲む複数のコア要素を有するコア領域と、
前記コア要素から延在する複数の遠位保持要素を有する遠位保持領域と、
前記コア要素から延在する複数の近位保持要素を有する近位保持領域と、
前記近位保持要素から延在する複数の撤回部材を有する撤回領域と、
を備え、
前記コア領域は、前記心房中隔の開口内に配置されるように適合ないし構成されており、
前記遠位保持要素は、前記隔壁の左心房側の組織に係合するように適合されており、
前記近位保持要素は、前記隔壁の右心房側の組織に係合するように適合されており、
各撤回部材は、近位端部にコネクタを有しており、
前記コネクタは、供給システムに接続するように適合されており、
当該装置は、供給形態と配置形態とを備えており、
前記コア領域、前記遠位保持領域及び前記近位保持領域は、各々、前記供給形態において前記配置形態よりも小さい直径を有しており、
前記撤回部材の前記コネクタは、前記配置形態において、前記開口に対して近位であって径方向外側に配置されている
ことを特徴とする装置。 - 前記コネクタは、前記配置形態においてよりも、前記供給形態において、より径方向内側に配置されている
ことを特徴とする請求項1に記載の装置。 - 前記撤回領域は、2つの撤回部材を有している
ことを特徴とする請求項1に記載の装置。 - 前記撤回領域は、4つの撤回部材を有している
ことを特徴とする請求項1に記載の装置。 - 前記コネクタは、孔(eyelet)を有している
ことを特徴とする請求項1に記載の装置。 - 前記コネクタは、前記配置形態において、前記撤回部材の端部から径方向内方に延在している
ことを特徴とする請求項1に記載の装置。 - 前記コネクタは、前記配置形態において、前記撤回部材の端部から先端方向に延在している
ことを特徴とする請求項1に記載の装置。 - 当該装置は、撤回形態を更に備えており、
前記撤回形態において、前記コネクタは、前記配置形態時の位置から径方向内方に位置し、前記近位保持要素及び前記遠位保持要素は、各々、前記配置形態におけるのと略同じ位置にある
ことを特徴とする請求項1に記載の装置。 - 前記撤回部材は、前記撤回形態においてよりも、前記供給形態において、前記近位保持領域から更に近位に延在している
ことを特徴とする請求項8に記載の装置。 - 患者の心臓の心房中隔内に圧力開放装置を移植する方法であって、
前記装置は、
遠位保持領域と、
近位保持領域と、
前記遠位保持領域と前記近位保持領域との間に配置されたコア領域と、
前記遠位保持領域と前記コア領域と前記近位保持領域とを貫く開口と、
前記近位保持領域に対して近位に配置された複数の撤回部材と、
を含み、
前記方法は、
前記患者の心臓の左心房において、縮んだ供給形態から配置形態へと前記遠位保持領域を拡張する工程と、
前記患者の心臓の前記左心房と右心房との間の前記心房中隔における開口内で、縮んだ供給形態から配置形態へと前記コア領域を拡張する工程と、
前記右心房において、縮んだ供給形態から配置形態へと前記近位保持領域を拡張する工程と、
供給システムから前記撤回部材を開放する工程と、
前記開放工程の後、前記開口から径方向外方の位置へと前記撤回部材を移動する工程と、
を備えたことを特徴とする方法。 - 前記移動工程は、前記開口に対して近位であって前記開口の外側境界より径方向内側の位置から、前記開口の前記外側境界より径方向外側の位置へと、前記撤回部材を移動する工程を含む
ことを特徴とする請求項10に記載の方法。 - 前記装置は、更に、撤回形態を更に備えており、
前記撤回形態において、前記撤回部材は、前記開口の前方の位置にあり、前記近位保持要素及び前記遠位保持要素は、各々、前記配置形態におけるのと略同じ位置にあり、
前記方法は、更に、前記開放工程前に前記撤回形態へと前記装置を拡張する工程を備えており、
前記遠位保持領域と前記コア領域と前記近位保持領域とを配置形態へと拡張する工程は、前記開放工程の後に実施される
ことを特徴とする請求項10に記載の方法。
