Abstract

本发明涉及一种外科术中应用的分支人造血管支架,具有一绦纶织物血管及内衬有较硬记忆合金丝的大支架,近端留有1cm长的无合金丝人造血管作为缝合环,其特征是大支架近端缝合有一小支架,该小支架由绦纶织物血管及内衬的较软记忆合金丝组成。本发明创造性地增加一分支架,用以支撑左锁骨下动脉的真腔,保持左锁骨下动脉和胸降主动脉远端的畅通和完整,克服现行手术的缺陷,即能避免左锁骨下动脉的吻合,并且远端吻合口位于无名动脉及左颈总动脉之间,从而解决了技术最难点。

Description

外科术中应用的分支人造血管支架
技术领域:
本发明属于医疗器材领域,特别涉及一种急性主动脉夹层行全弓替换手术治疗用的外科术中应用的分支人造血管支架。
背景技术:
目前常规应用四分叉人造血管及降主动脉内释放支架来完成急性主动脉夹层的全弓替换术,在这种常规手术中需分别吻合左锁骨下动脉、左颈总动脉、无名动脉以及左锁骨下动脉远端的胸降主动脉吻合。但左锁骨下动脉吻合及胸降主动脉的远端吻合因其位置深,操作困难,并且容易损伤喉返神经,一旦出现吻合口出血,极难准确止血。现术中采用的人造血管支架为一管状结构,由绦纶织物人造血管及内衬有较硬金属丝组成,近端留有1cm长的无金属丝人造血管作为缝合环,以便与四分叉人造血管缝合,血管支架主要释放于胸体主动脉的真腔内,挤闭假腔。
发明内容:
本发明的目的是发明一种人造血管支架近端带有一小血管支架的外科术中应用的分支人造血管支架。
本发明技术方案是这样实现的:一种外科术中应用的分支人造血管支架,具有一绦纶织物血管及内衬有较硬记忆合金丝的大支架,近端留有1cm长的无合金丝人造血管作为缝合环,其特征是大支架近端缝合有一小支架,该小支架由绦纶织物血管及内衬的较软记忆合金丝组成。
上述的缝合是指大支架近端5-7mm的人造血管上造一个10-12mm的孔眼,小支架近端留有5mm长的无支架绦纶织物血管缝合环,将该缝合环垂直吻合于大支架孔眼上。
本发明创造性地增加一分支架,用以支撑左锁骨下动脉的真腔,保持左锁骨下动脉和胸降主动脉远端的畅通和完整,克服现行手术的缺陷,即能避免左锁骨下动脉的吻合,并且远端吻合口位于无名动脉及左颈总动脉之间,使远端吻合口位置浅表,从而解决了技术最难点。

具体实施方式:
参照图1和图2,外科术中应用的分支人造血管支架,包括一大支架1和一垂直于大支架且缝合于近端的小支架2组成,大支架1和小支架2均由绦纶织物人造血管3及内衬有的记忆合金4制成,由合金将人造血管撑开,对血管真腔实现扩张作用,保证血液的流通。大支架1内衬有较硬的合金丝4,而小支架2即内衬有较软的合金丝。
大支架近端留有1cm长的无合金丝人造血管3作为缝合环5,提供与动脉缝合之需,在大支架1近端5-7mm的人造血管上造一个10-12mm的孔眼,小支架2近端留有5mm长的无支架绦纶织物血管缝合环,将该缝合环垂直吻合于大支架孔眼上。制成合成支架,大小支架1、2分别用线收紧待释放于相应的血管腔内。
参照图2,进行临床手术操作,常规体外循环降温至肛温24℃,停循环,切开主动脉弓,选择性直接插管于无名动脉7以及左颈总动脉8灌注脑保护,于无名动脉7及左颈总动脉8之间横断主动脉弓,将收紧的分支合成支架插入胸降主动脉6及左锁骨下动脉9的真腔内,依次释放小、大支架2、1,重建近端断端,将三分叉人造血管与近端断端吻合,从三分叉人造血管顺行灌注,依次吻合左颈总动脉及无名动脉,再行近端吻合,完成全弓替换手术。

Claims (2)

1、一种外科术中应用的分支人造血管支架,具有一绦纶织物血管及内衬有较硬记忆合金丝的大支架,近端留有1cm长的无合金丝人造血管作为缝合环,其特征是大支架近端缝合有一小支架,该小支架由绦纶织物血管及内衬的较软记忆合金丝组成。
2、根据权利要求1所述的外科术中应用的分支人造血管支架,其特征是缝合是指大支架近端5-7mm的人造血管上造一个10-12mm的孔眼,小支架近端留有5mm长的无支架绦纶织物血管缝合环,将该缝合环垂直缝合于大支架孔眼上。
