Abstract
Description
本発明は、大部分の骨髄系細胞に見出され患者における急性骨髄白血病(AML)を診断するために使用される細胞表面糖タンパク質CD33へ免疫細胞の特異性および反応性を向け直すことができる組換えキメラタンパク質であるキメラ抗原受容体(CAR)に関する。本発明によるCARは、T細胞またはNK細胞において発現された時、CD33を保持する悪性細胞を処置するために特に有用である。得られた操作された免疫細胞は、免疫療法のための安全性および効率を付与する、悪性細胞に対する高レベルの特異性を示す。
エクスビボで生成された自己の抗原特異的T細胞の移入を含む養子免疫療法は、ウイルス感染および癌を処置するための有望な戦略である。養子免疫療法のために使用されるT細胞は、抗原特異的T細胞の増大または遺伝子操作を通したT細胞の向け直しのいずれかによって生成され得る(Park, Rosenberg et al. 2011)。ウイルス抗原特異的T細胞の移入は、移植に関連したウイルス感染および稀なウイルス関連悪性疾患の処置のために使用されている、よく確立された手法である。同様に、腫瘍特異的T細胞の単離および移入は、黒色腫の処置において成功が示されている。
本明細書において特に定義されない限り、使用される技術用語および科学用語は、全て、遺伝子治療、生化学、遺伝学、および分子生物学の領域の当業者によって一般的に理解されるのと同一の意味を有する。

本発明は、本発明による前記のCARのいずれか一つをコードするポリヌクレオチド、ベクターにも関する。
本発明は、以前に記載されたように、CD33 CARの一つをコードするポリヌクレオチドまたはベクターを免疫細胞へエクスビボで導入する工程を含む、免疫療法のための免疫細胞を調製する方法を包含する。
例えば、キャンパス(アレムツズマブ)およびグルココルチコイドによる処置の薬物標的であるCD52およびグルココルチコイド受容体(GR)を、これらの処置に対して細胞を耐性にし、特異的なCD33 CARを付与されていない患者自身のT細胞と比べて競争有利性を与えるため、不活性化することができる。別の免疫抑制薬であるテプリズマブに対する耐性を付与するため、CD3遺伝子の発現を抑制するかまたは低下させることもできる。化学療法において、具体的には、急性リンパ芽球性白血病の処置のため、一般的に使用される細胞分裂阻害剤である6-チオグアニンに対する耐性を付与するため、HPRTの発現を本発明に従って抑制するかまたは低下させることもできる。「GLl1」遺伝子の発現は低下し得る。
上記の様々な方法は、CARを細胞へ導入する工程を含む。非限定的な例として、CARは、1種のプラスミドベクターによってコードされたトランスジーンとして導入され得る。プラスミドベクターは、ベクターを受容した細胞の同定および/または選択を提供する選択マーカーも含有し得る。
本発明は、細胞を操作する方法によって入手可能な単離された細胞または細胞株にも関する。具体的には、単離された細胞は、少なくとも1種の上記のCARを含む。別の態様において、単離された細胞は、各々異なる細胞外リガンド結合ドメインを含むCARの集団を含む。具体的には、単離された細胞は、CARをコードする外因性のポリヌクレオチド配列を含む。本発明の遺伝子改変免疫細胞は、活性化されており、抗原結合機序と無関係に増殖する。
本発明の遺伝子改変免疫細胞は、活性化されており、抗原結合機序と無関係に増殖するが、本発明の免疫細胞、具体的には、T細胞は、T細胞の遺伝子改変の前であっても後であっても、例えば、米国特許第6,352,694号;第6,534,055号;第6,905,680号;第6,692,964号;第5,858,358号;第6,887,466号;第6,905,681号;第7,144,575号;第7,067,318号;第7,172,869号;第7,232,566号;第7,175,843号;第5,883,223号;第6,905,874号;第6,797,514号;第6,867,041号;および米国特許出願公開第20060121005号に記載されるような方法を一般に使用して、さらに活性化させ増大させることができる。T細胞は、インビトロまたはインビボで増大させることができる。
別の態様において、既に記載されたような異なる方法によって入手された単離された細胞または単離された細胞に由来する細胞株は、医薬として使用され得る。別の態様において、医薬は、その必要のある患者において、癌を処置するため、具体的には、B細胞リンパ腫および白血病の処置のため、使用され得る。別の態様において、本発明による単離された細胞または単離された細胞に由来する細胞株は、その必要のある患者における癌の処置のための医薬の製造において使用され得る。
(a)既に記載された方法のいずれか一つによって入手可能な免疫細胞を準備する工程、
(b)形質転換された免疫細胞を患者へ投与する工程

(a)抗CD33発現CAR免疫細胞(または本発明の操作された免疫細胞のいずれか)を調製するため、本発明の方法のいずれか一つによって入手可能な免疫細胞を準備する工程、
(b)抗CD33発現CAR免疫細胞を患者へ投与する工程
のうちの少なくとも一つを含む、その必要のある患者を処置する方法に依り、任意で、本発明のCD33 CAR T細胞は、頑強なインビボT細胞増大を受け、より長期にわたり、存続することができ、具体的には、より長期にわたり、CD33に結合することができる。
好ましい態様において、本発明は、60歳を超える患者、20歳未満の患者、特に小児の患者における、白血病、特にAMLに対する医薬を提供する。
一つの具体的な態様において、本発明は、患者、具体的には、AMLに関連した合併症を起こしている患者の健康状態を改善する医薬を提供する。より好ましくは、本発明の操作された抗CD33 CAR発現T細胞は、SEQ ID NO:1、およびSEQ ID NO:19、SEQ ID NO:20、SEQ ID NO:21、SEQ ID NO:22、SEQ ID NO:27、SEQ ID NO:28、SEQ ID NO:29、SEQ ID NO:30、SEQ ID NO:35、SEQ ID NO:36、SEQ ID NO:37、SEQ ID NO:38、またはそれらの組み合わせとの少なくとも80%の同一性を有するポリペプチドを含む少なくとも1種の抗CD33 CARを発現しており、AMLに関連した合併症の処置のための医薬として使用される。
一つの態様において、本発明は、AMLに関連した病理学的状態の処置のための抗CD33 CAR発現T細胞も提供する。好ましくは、本発明は、AMLに関連した病理学的状態の処置のための、SEQ ID NO:48、SEQ ID NO:50、SEQ ID NO:52、SEQ ID NO:54、SEQ ID NO:56、SEQ ID NO:58、SEQ ID NO:66、SEQ ID NO:68、およびSEQ ID NO:70より選択されるポリペプチドとの少なくとも80%の同一性を有するポリペプチドを含む、少なくとも1種の抗CD33 CARを発現する細胞を提供する。
本発明は、本発明による抗CD33発現T細胞と薬学的に許容される媒体とを含む組成物を提供する。
他に特記されない限り、「a」、「an」、「the」、および「少なくとも1」は、交換可能に使用され、1または複数を意味する。ポリペプチド配列内のアミノ酸残基は、1文字コードによって本明細書において表記され、例えば、QはGln、すなわちグルタミン残基を意味し、RはArg、すなわちアルギニン残基を意味し、DはAsp、すなわちアスパラギン酸残基を意味する。

他に示されない限り、類似性スコアは、BLOSUM62の使用に基づくであろう。BLASTPが使用される時、パーセント類似性は、BLASTP陽性スコアに基づき、パーセント配列同一性は、BLASTP同一性スコアに基づく。BLASTP「同一性」は、同一である高スコア配列対における全残基の数および画分を示し;BLASTP「陽性」は、アライメントスコアが正の値を有しており、相互に類似している残基の数および画分を示す。本明細書に開示されたアミノ酸配列との同一性もしくは類似性のこれらの程度または同一性もしくは類似性の中間の程度を有するアミノ酸配列が、本開示によって企図され包含される。類似したポリペプチドのポリヌクレオチド配列は、遺伝暗号を使用して推定され、特に、遺伝暗号を使用して、そのアミノ酸配列を逆翻訳することによって、従来の手段によって入手され得る。
初代T細胞における特定の遺伝子の不活性化
初代T細胞における特定の遺伝子の不活性化を、T細胞へのCAR導入の前または後に実施することができる。
核酸−ベクター
本発明の抗CD33 CARをコードする核酸(mRNAまたはレンチウイルスベクター)を、図2または図3において設計された構成によって構築する。
初代T細胞培養物
フィコール勾配密度媒体を使用して、EFS(Etablissement Francais du Sang,Paris,France)によって提供されたバフィーコート試料から、T細胞を精製する。PBMC層を回収し、市販されているT細胞濃縮キットを使用してT細胞を精製する。精製されたT細胞を、ヒトIL-2およびDynabeads Human T activator CD3/CD28を使用してX-Vivo(商標)-15培地(Lonza)において活性化する。
T細胞の精製および活性化の後、4日目または11日目に、異なるCAR構築物をコードするCAR mRNAのトランスフェクションを行った。
様々なレベルのCD33タンパク質を発現する等量の細胞と共に、T細胞をインキュベートする。共培養を、少なくとも6時間、維持する。共培養の開始時に、蛍光性抗CD107a抗体を添加することによって、細胞刺激中にCD107a染色を行う。6時間のインキュベーション期間の後、細胞を、固定可能な生死細胞判定色素および蛍光色素結合型抗CD8によって染色し、フローサイトメトリーによって分析する。CD8+/CD107a+細胞の%として、そしてCD8+細胞におけるCD107a染色についての平均蛍光強度シグナル(MFI)を決定することによって、脱顆粒活性を決定する。
mRNAトランスフェクションの24時間後、様々なレベルのCD33タンパク質を発現する細胞株と共に、37℃で24時間、抗CD33 CAR発現T細胞をインキュベートする。上清を回収し、細胞培養上清中のIFNγ検出をELISAアッセイによって行う。
同一のウェルにおいて、(様々なレベルのCD33を発現する)標的細胞または(CD33陰性)細胞と共に、抗CD33 CAR発現T細胞をインキュベートする。標的CD33+細胞および対照CD33-標的細胞を蛍光性の細胞内色素(例えば、CFSEまたはCell Trace Violet)によって標識した後、37℃で4時間共培養する。このインキュベーション期間の後、細胞を固定可能な生死細胞判定色素によって標識し、フローサイトメトリーによって分析する。各細胞集団(標的細胞またはCD33陰性対照細胞)の生存率を決定し、特異的細胞溶解の%を計算する。細胞傷害アッセイは、mRNAトランスフェクションの48時間後に実施される。
免疫不全NOGマウスに、CD33発現ルシフェラーゼ細胞を静脈内(iv)注射する。任意で、マウスは、抗CD33 CAR+T細胞を注射する前に、抗癌処置を受容する。次いで、異なる用量の試験される抗CD33 CAR+T細胞、またはCARレンチウイルスベクターによって形質導入されていないT細胞を、(腫瘍細胞株の注射の2日後または7日後のいずれかに)マウスにiv注射する。異なる動物における腫瘍進行を追跡するため、T細胞注射の当日(D0)、T細胞注射後7日目、14日目、21日目、28日目、および40日目に生物発光シグナルを決定する。
CARの異なる構成、即ち、V1〜V6を設計した(図3)。第一工程として、T細胞においてキメラ抗原受容体(CAR)(SEQ ID NO:48〜SEQ ID NO:71)を生成するため、M195、DRB2、m2H12、およびMy9.6に由来する4種の異なるscFvを構築した。
初代T細胞培養物
フィコール勾配密度媒体を使用して、EFS(Etablissement Francais du Sang,Paris,France)によって提供された様々なバフィーコート試料から、T細胞を精製した。PBMC層を回収し、T細胞を市販されているT細胞濃縮キット(Stem Cell Technologies)を使用して精製した。精製されたT細胞を、20ng/mLヒトIL-2、5%ヒト血清、およびビーズ:細胞比1:1のDynabeads Human T activator CD3/CD28(Life Technologies)が補足されたX-Vivo(商標)-15培地(Lonza)において活性化した。活性化の後、20ng/mLヒトIL-2および5%ヒト血清が補足されたX-Vivo(商標)-15培地(Lonza)において細胞を増殖させ維持した。
T細胞の精製および活性化の後、4日目または11日目に、様々な抗CD33抗体断片を使用して生成された各CD33 CARのトランスフェクションを行った。異なるCAR構築物をコードする15μgのmRNAによって500万個の細胞をトランスフェクトした。mMESSAGE mMACHINE T7キット(Life Technologies)を使用してCAR mRNAを作製し、RNeasy Mini Spin Columns(Qiagen)を使用して精製した。最終体積200μlの「Cytoporation buffer T」(BTX Harvard Apparatus)において、0.4cmギャップキュベットにおいて、3000V/cmで0.1mSパルスを2回適用した後、325V/cmで0.2mSパルスを4回適用することによって、Cytopulseテクノロジーを使用してトランスフェクションを行った。細胞をX-Vivo(商標)-15培地(Lonza)で直ちに希釈し、5%CO2と共に37℃でインキュベートした。電気穿孔の2時間後に、20ng/mLのIL-2を添加した。
T細胞形質導入
抗CD33 CARの発現のための組換えレンチウイルスベクターによるT細胞の形質導入を、T細胞の精製/活性化の3日後に実施した。Vectalys SA(Toulouse,France)によって作製されたレンチウイルスベクターを使用した。T細胞の表面の抗CD33 CAR検出を、(LakePharmaによって作製された)マウスIgG1 Fc断片に融合されたヒトCD33タンパク質の細胞外ドメインからなる組換えタンパク質を使用して行った。このタンパク質の抗CD33 CAR分子との結合を、タンパク質のマウスFc部分を標的とするPE結合型二次抗体(Jackson Immunoresearch)によって検出し、フローサイトメトリーによって分析した。
CD33遺伝子の不活性化
CD33遺伝子を不活性化するため、CD33遺伝子内の10bpまたは15bpのスペーサーによって分離された(TALEN左が結合する配列およびTALEN右が結合する配列と呼ばれる、表11参照)2種の配列を標的とするヘテロ二量体TALEヌクレアーゼを、下記のように設計し作製した。各半標的は、表11にリストされた半TALEヌクレアーゼのリピートによって認識される。次いで、TCRα遺伝子を不活性化するために設計されたものと共に、同時に、構築物をT細胞へ導入した。
様々な抗CD33抗体断片を使用して生成されたCD33 CARの脱顆粒活性
前記のM195、m2H12、およびMy9.6の構築物V1、V3、およびV5の活性を、まず、ヒト初代T細胞における一過性発現によって決定した。
次いで、様々なレベルのCD33タンパク質を発現する等量の細胞と共に、96穴プレートにおいてT細胞をインキュベートした(40,000細胞/ウェル)。共培養を、5%CO2で37℃で6時間、100μlの最終体積のX-Vivo(商標)-15培地(Lonza)において維持した。共培養の開始時に、1μg/mlの抗CD49d、1μg/mlの抗CD28、および1×モネンシン溶液と共に蛍光性抗CD107a抗体を添加することによって、細胞刺激中にCD107a染色を行った。6時間のインキュベーション期間の後、細胞を、固定可能な生死細胞判定色素および蛍光色素結合型抗CD8によって染色し、フローサイトメトリーによって分析した。CD8+/CD107a+細胞の%として、そしてCD8+細胞におけるCD107a染色についての平均蛍光強度シグナル(MFI)を決定することによって、脱顆粒活性を決定した。脱顆粒アッセイは、mRNAトランスフェクションの24時間後に実施された。
このため、様々なドナーに由来するT細胞をバフィーコート試料から単離し、CD3/CD28ビーズを使用して活性化した。細胞を、活性化の後、11日目に、異なる候補をコードするmRNAによって一過性トランスフェクトした。
96穴プレートにおいて、様々なレベルのCD33タンパク質を発現する細胞株と共に、抗CD33 CAR発現T細胞をインキュベートした(40,000細胞/ウェル)。共培養を、5%CO2で37℃で24時間、100μlの最終体積のX-Vivo(商標)-15培地(Lonza)において維持した。このインキュベーション期間の後、プレートを5分間1500rpmで遠心分離し、上清を新しいプレートに回収した。細胞培養上清中のIFNγ検出をELISAアッセイによって行った。IFNγ放出アッセイは、mRNAトランスフェクションの24時間後に細胞共培養を始めることによって実施された。
96穴プレートにおいて、同一のウェルにおいて、10,000個の標的細胞(CD33発現)および10,000個の対照細胞(CD33陰性)と共に、抗CD33 CAR発現T細胞をインキュベートした(100,000細胞/ウェル)。標的細胞および対照細胞は、CAR+T細胞との共培養の前に蛍光性の細胞内色素(CFSEまたはCell Trace Violet)によって標識された。共培養物を5%CO2で37℃で4時間インキュベートした。このインキュベーション期間の後、細胞を固定可能な生死細胞判定色素によって標識し、フローサイトメトリーによって分析した。各細胞集団(標的細胞またはCD33陰性対照細胞)の生存率を決定し、特異的細胞溶解の%を計算した。細胞傷害アッセイは、mRNAトランスフェクションの48時間後に実施された。
Claims (42)
- 図2に図示するV1、V3、およびV5より選択されるポリペプチド構造のうちの一つと少なくとも80%の同一性を有するCD33特異的キメラ抗原受容体(CAR)であって、該構造が、
(a)モノクローナル抗CD33抗体由来のVHおよびVLを含む細胞外リガンド結合ドメイン、
(b)FcRIIIαヒンジ、CD8αヒンジ、およびIgG1ヒンジより選択されるヒンジ、
(c)CD8α膜貫通ドメイン、ならびに
(d)CD3ζシグナリングドメインおよび4-1BB由来の共刺激ドメインを含む細胞質ドメイン
を含む、CD33特異的キメラ抗原受容体(CAR)。 - 構造V3がCD8αヒンジおよびCD8α膜貫通ドメインを含む、請求項1記載のCD33特異的CAR。
- CD8αヒンジがSEQ ID NO:4と少なくとも80%の同一性を有する、請求項2記載のCD33特異的CAR。
- SEQ ID NO:27、SEQ ID NO:28、SEQ ID NO:29、およびSEQ ID NO:30と少なくとも80%の同一性を有するポリペプチド配列を含む、請求項2または3記載の構造V3のCD33特異的CAR。
- 構造V1がFcγRIIIαヒンジおよびCD8α膜貫通ドメインを含む、請求項1記載のCD33特異的CAR。
