Description

本発明は、脱毛症の治療方法に関する。
外皮(または皮膚)は、身体の最大の器官であり、身体の外表面を覆う高度に複雑な器官である。それは様々な身体の開口部で、消化管および他の管の粘膜と融合する。外皮は、体温および水分損失を調節することによる一定の内部環境の維持;汗腺による排出などのいくつかの必須機能を実行するが、より深部の組織に対する物理的、化学的および生物学的因子の作用に対する保護障壁として主に作用する。皮膚は伸縮性があり、足裏、手のひら、および耳などのいくつかの領域を除いて、下層組織に緩く結合している。また、それはまぶたでは厚さ0.5mm(0.02インチ)(薄い皮膚)から、手のひらおよび足裏では4mm(0.17インチ)以上(厚い皮膚)まで変化する(Ross MH, Histology: A text and atlas、第3版、Williams and Wilkins, 1995: Chapter 14; Burkitt HGら、Wheater's Functional Histology、第3版、Churchill Livingstone, 1996: Chapter 9)。
円形脱毛症(AA)は、あらゆる民族群にわたる男性および女性を含む米国だけでも約460万人が罹患している最も高頻度に見られる自己免疫疾患の1つであり、その生涯リスクは1.7%である(1)。AAにおいては、毛包に対する自己免疫が生じ、パッチとして開始し得る非瘢痕性の脱毛をもたらし、癒着および進行して頭皮全体に広がる(完全脱毛症、AT)か、または最終的には全身に広がる(全身性脱毛症、AU)(図1)。AAは、それが頭皮にわたってゆっくりと広がる際の曲がりくねった経路の脱毛に起因して、「ヘビ」を意味する用語「蛇行状脱毛症(ophiasis)」を用いて、紀元30年にCornelius Celsusによって初めて記載された。Hippocratesは初めてギリシャ語「alopekia」(キツネ疥癬)を使用し、現代用語「円形脱毛症(alopecia areata)」は、Sauvageによって、Lyons、Franceで1760年に発行された彼のNosologica Medicaにおいて初めて使用された。
本発明は、公知の治療剤、例えば、JAK/STATタンパク質Jak1、Jak2、Stat1またはStat2などの、サイトカインシグナリングに関与するタンパク質チロシンキナーゼ(PTK)の阻害剤(例えば、INCB018424; Incyte)を、脱毛症の治療のために用いることができるという発見を提供する。脱毛症の非限定例としては、アンドロゲン性脱毛症、円形脱毛症、休止期脱毛、円形脱毛症、完全脱毛症、および全身性脱毛症が挙げられる。
本発明は、当業者には利用可能な従来の分子生物学、微生物学、および組換えDNA技術を利用する。そのような技術は当業者には周知であり、文献で完全に説明されている。例えば、Maniatis, Fritsch & Sambrook、「Molecular Cloning: A Laboratory Manual」(1982):「DNA Cloning: A Practical Approach」、Volumes I and II (D. 真核発現ベクターを用いて細胞をトランスフェクトして、ベクターのヌクレオチド配列によりコードされたタンパク質を産生させることができる。哺乳動物細胞(例えば、毛球に由来する単離された細胞;例えば、真皮鞘細胞および真皮乳頭細胞)は、当業界で公知の方法により好適な宿主細胞中に発現ベクターを導入することによって発現ベクター(例えば、Jak1、Jak2、Stat1またはStat2タンパク質またはポリペプチドをコードする遺伝子を含有するもの)を含有してもよい。
培養される宿主細胞に関して、様々な培養パラメータを用いることができる。哺乳動物細胞のための好適な培養条件は当業界で周知であり(Cleveland WLら、J Immunol Methods, 1983, 56(2): 221-234)、または当業者によって決定することができる(例えば、Animal Cell Culture: A Practical Approach、第2版、Rickwood, D.およびHames, B. D.(編)(Oxford University Press: New York, 1992)を参照されたい)。細胞培養条件は選択された宿主細胞の型に応じて変化し得る。市販の培地を用いることができる。培地の非限定例としては、例えば、最少必須培地(MEM, Sigma, St. Louis, Mo.); Dulbeccoの改変イーグル培地(DMEM, Sigma); HamのF10培地(Sigma); HyClone細胞培養培地(HyClone, Logan, Utah); RPMI-1640培地(Sigma); および様々な細胞型のために考案された化学規定(CD)培地、例えば、CD-CHO培地(Invitrogen, Carlsbad, Calif.)が挙げられる。
Jak1、Jak2、Stat1もしくはStat2遺伝子などの遺伝子、またはその変異体によりコードされるポリペプチド分子を、Jak1、Jak2、Stat1もしくはStat2タンパク質もしくはポリペプチドをコードする核酸配列のin vitroもしくはin vivoでの発現を介する、Jak1、Jak2、Stat1もしくはStat2遺伝子によりコードされるタンパク質もしくはポリペプチドを発現するヒト細胞からの精製によって、または直接化学合成によって取得することができる。
本発明は、被験体における毛髪増殖(例えば、毛髪密度)または毛髪色素を制御および/または調節するために用いることができる化合物を同定する方法を提供する。本発明は脱毛症と関連する遺伝子としての本明細書に列挙された遺伝子の同定を提供したため、本発明はまた、Jak1、Jak2、Stat1またはStat2の遺伝子および/またはタンパク質の発現または活性を調節する化合物を同定する方法も提供する。さらに、本発明は、脱毛症の治療に用いることができる化合物を同定する方法を提供する。本発明はまた、貧毛症(例えば、遺伝性先天性貧毛症(HHS))の治療に用いることができる化合物を同定する方法も提供する。脱毛症の非限定例としては、アンドロゲン性脱毛症、円形脱毛症、休止期脱毛、円形脱毛症、完全脱毛症、および全身性脱毛症が挙げられる。前記方法は、Jak1、Jak2、Stat1もしくはStat2によりコードされるポリペプチド分子に結合し、および/またはJak1、Jak2、Stat1もしくはStat2遺伝子によりコードされるタンパク質の生物活性またはその発現に対する刺激または阻害効果を有する試験化合物または薬剤(例えば、ペプチド(抗体もしくはその断片など)、小分子、核酸(siRNAもしくはアンチセンスRNA)、または他の薬剤)を同定した後、これらの化合物が被験体における毛髪増殖を調節することができるかどうか、またはin vivoアッセイ(すなわち、毛髪増殖の増加もしくは低下を試験する)において脱毛症と関連する症候に対する効果を有することができるかどうかを決定することを含んでもよい。
Jak1、Jak2、Stat1またはStat2調節化合物:Jak1、Jak2、Stat1またはStat2化合物は、in vivoおよび/またはin vitroでのJak1、Jak2、Stat1またはStat2タンパク質の活性および/または発現に影響する化合物であってよい。Jak1、Jak2、Stat1またはStat2調節化合物は、Jak1、Jak2、Stat1またはStat2タンパク質のアゴニストおよびアンタゴニストであってよく、発現を介して、翻訳後修飾を介して、または他の手段によってJak1、Jak2、Stat1またはStat2タンパク質の活性に対してその効果を示す化合物であってよい。
本発明はまた、被験体における脱毛症を治療または予防する方法も提供する。一実施形態においては、前記方法は、被験体からのサンプル中のJak1、Jak2、Stat1もしくはStat2遺伝子の変化の存在、脱毛症を示す変化の存在、または脱毛症に対する素因を検出すること、および脱毛症に対する治療的処置を必要とする被験体に投与することを含む。治療的処置は、薬剤投与(例えば、Jak1、Jak2、Stat1またはStat2核酸に対するsiRNAを含む医薬組成物)であってもよい。一実施形態においては、投与される治療分子は、配列番号1、3、5または7のアミノ酸配列の少なくとも約75%、少なくとも約80%、少なくとも約85%、少なくとも約90%、少なくとも約93%、少なくとも約95%、少なくとも約97%、少なくとも約98%、少なくとも約99%、または100%を含む、Jak1、Jak2、Stat1またはStat2遺伝子によりコードされるポリペプチドを含み、Jak1、Jak2、Stat1またはStat2遺伝子によりコードされるタンパク質の発現を低下させる機能を示す。これは、毛包または皮膚から誘導される細胞中での毛髪増殖を開始する能力を修復させることができる。別の実施形態においては、投与される治療分子は、配列番号2、4、6または8の核酸配列の少なくとも約75%、少なくとも約80%、少なくとも約85%、少なくとも約90%、少なくとも約93%、少なくとも約95%、少なくとも約97%、少なくとも約98%、少なくとも約99%、または100%を含む、ポリペプチドをコードするJak1、Jak2、Stat1またはStat2遺伝子を含む核酸配列を含み、Jak1、Jak2、Stat1またはStat2遺伝子によりコードされるタンパク質の発現を低下させる機能を有するポリペプチドをコードし、かくして、毛包または皮膚から誘導される細胞中での毛髪増殖を開始する能力を修復させる。
本明細書で考察される通り、Jak1、Jak2、Stat1もしくはStat2遺伝子により占有される染色体領域における変化またはJak1、Jak2、Stat1もしくはStat2遺伝子の発現における変化を、配列の変化またはJak1、Jak2、Stat1もしくはStat2遺伝子によりコードされるポリペプチドの発現レベルの変化をスクリーニングすることによって検出することもできる。特異的抗体などの、様々な型のリガンドを用いることができる。一実施形態においては、サンプルを、Jak1、Jak2、Stat1もしくはStat2遺伝子によりコードされるポリペプチドに特異的な抗体と接触させた後、免疫複合体の形成を決定する。免疫複合体を検出するための様々な方法、例えば、ELISA、ラジオイムノアッセイ(RIA)および免疫酵素アッセイ(IEMA)を用いることができる。
生細胞中への核酸の送達を、ウイルスベクター(例えば、レンチウイルス、アデノウイルス、アデノ随伴ウイルス、もしくはレトロウイルス)などのベクターの使用によりex vivo、in situもしくはin vivoで、または物理的DNA導入法(例えば、リポソームもしくは化学的処理)の使用によりex vivoで行うことができる。in vitroでの哺乳動物細胞中への核酸の導入にとって好適な非限定的技術としては、リポソームの使用、エレクトロポレーション、マイクロインジェクション、細胞融合、DEAE-デキストラン、およびリン酸カルシウム沈降法が挙げられる(例えば、Anderson, Nature, supplement to vol. 392, no. 6679, pp. 25-20 (1998)を参照されたい)。本発明のポリペプチドをコードする核酸または遺伝子の導入は、染色体外基質(一過的発現)または人工染色体(安定発現)を用いて達成することもできる。また、細胞を本発明の治療用組成物の存在下でex vivoで培養して、そのような細胞に対する所望の効果またはそのような細胞中での活性を増加させるか、またはもたらすこともできる。次いで、処理された細胞を治療目的でin vivoで導入することができる。
本発明のJak1、Jak2、Stat1またはStat2タンパク質およびJak1、Jak2、Stat1またはStat2調節化合物を被験体に1回投与することができる(例えば、単回注射または沈着物として)。あるいは、Jak1、Jak2、Stat1またはStat2タンパク質およびJak1、Jak2、Stat1またはStat2調節化合物を、約2〜約28日、または約7〜約10日の期間にわたって、それを必要とする被験体に1日1回または2回投与することができる。Jak1、Jak2、Stat1またはStat2タンパク質およびJak1、Jak2、Stat1またはStat2調節化合物を、年に1、2、3、4、5、6、7、8、9、10、11回、またはその組合せの期間にわたって、被験体に1日1回または2回投与することもできる。さらに、本発明のJak1、Jak2、Stat1またはStat2タンパク質およびJak1、Jak2、Stat1またはStat2調節化合物を、別の治療剤と同時投与することができる。投薬レジメンが複数の投与を含む場合、被験体に投与されるJak1、Jak2、Stat1またはStat2タンパク質およびJak1、Jak2、Stat1またはStat2調節化合物の有効量は、全投薬レジメンにわたって投与される遺伝子産物の全量を含んでもよい。
サイトカインは、危険シグナルを互いに連絡し、炎症応答を拡散および増幅させるために近隣の細胞を活性化するために産生される。長年にわたって、遮断抗体を用いてこれらのサイトカインを中和する方法と、応答細胞中でのそのシグナリングを阻害する方法の両方が、両手法のために存在する小分子タンパク質チロシンキナーゼ阻害剤FDA認可薬剤、例えば、サイトカインシグナリングを遮断するIL-2およびTNF遮断抗体ならびに小さい経口利用可能な分子Gleevec(商標)によって研究された。可能な場合はいつでも、中心は、全身毒性を制限しながら有効性を改善するべきである局所小分子製剤を追跡し(治療指数の改善)、生物製剤ピペリン中の他の小分子阻害剤のAAにおける臨床評価を促進する。
本発明の阻害剤またはアゴニストを、投与にとって好適な医薬組成物、例えば、阻害剤および製薬上許容し得る担体中に組み入れることができる。
本発明のより完全な理解を容易にするために、実施例が以下に提供される。以下の実施例は、本発明を作製し、実施する例示的様式を例示するものである。しかしながら、同様の結果を得るために代替的な方法を用いることができるので、本発明の範囲は、例示のためだけのものであるこれらの実施例に開示される特定の実施形態に限定されない。

円形脱毛症(AA)は主要な医学的問題であり、米国における最も高頻度の自己免疫疾患(表8)であり、あらゆる民族群にわたって男性および女性を含む約460万人が罹患し、その生涯リスクは1.7%である(Kyriakis KP, Paltatzidou K, Kosma E, Sofouri E, Tadros A, Rachioti E. Alopecia areata prevalence by gender and age. J Eur Acad Dermatol Venereol. 23:572-3, 2009)。さらに、AAは、アンドロゲン性脱毛症に次いで2番目に最も一般的な形態のヒトの脱毛であり、罹患した個体の外観が有意に損なわれ、心理的苦痛を引き起こす(図1)。AAは、全身性エリテマトーデス(LE)、1型糖尿病(T1D)、乾癬、多発性硬化症(MS)および慢性関節リウマチ(RA)などの多くの他の自己免疫疾患を合わせたよりも多くの個人が罹患している(表8)。
NKG2D受容体(NKG2D)は、危険シグナルを発する細胞を排除するように機能し、この認識プロセスの調節障害は自己免疫の発生を誘導することが多い。理論によって束縛されるものではないが、NKG2DL発現が転写レベルで阻害される場合、JAKおよびNF-kb阻害の影響が追求される。JAK1/JAK2阻害剤(po/局所)であるINCB18424は、筋増殖/筋線維障害および乾癬について第II相臨床試験中である。
AA皮膚に由来する全細胞集団の免疫染色およびフローサイトメトリー分析を用いて、CD8+NKG2D+T細胞が毛包中に浸潤し(図17)、正常な皮膚ではなくAA中で増殖する(図18および図4A)ことがわかった。さらに、AAマウスは、AA皮膚リンパ節および血液の両方におけるCD8+NKG2D+集団の劇的な全身的増殖に少なくとも一部起因して、皮膚リンパ節症を示し、全LN細胞の全部で6±1%が全PBMCの2.4±1.3%であり、それぞれ正常なC3Hマウスにおいて認められるものよりも10倍以上高いことを示している。これらのCD8+NKG2D+T細胞は、HF真皮鞘細胞に対して強い細胞傷害性を示す(図21)。CD8+NKG2D+T細胞のさらなる免疫表現型分析により、この集団がT記憶マーカーCD44+および複数の「NKマーカー」、例えば、DX5、NKG2A/C/EおよびNKp46を発現することが示された(図19)。
C3H/HeJ皮膚移植モデル:円形脱毛症のC3H/HeJマウスモデルは、1994年に初めて報告されたこのモデルであるため、これを研究した(29、30、31)。円形脱毛症の他のマウスモデルも見出されてたが(32)、このモデルは依然として最も研究されており、当業界で今日でも用いられている。長年にわたってこのモデルを用いて様々な薬剤の試験が行われ(33)、様々なプロトコルが最適化されている(34)。
局所送達は、全身曝露および毒性の制限において明らかな利点を有する。前臨床および臨床試験により、クリーム中の小分子PTKi送達が皮膚障壁を乗り越え、全身曝露の限定と共に真皮中の有効な局所濃度を達成することが示された(78)。AAにおけるエフェクターT細胞に対して中枢的な重要なPTK、すなわち、サイトカインIFNγが標的化される。
これらの前臨床研究における治療結果に付随する免疫学的事象の同定は、提唱された作用機構を検証するため、およびAA患者におけるこれらの同じ治療の潜在的な臨床評価を知らせる有用なバイオマーカーを提供するための両方において重要である。臨床評価においては、生物標本は血液および皮膚サンプルに限定されるため、これらの部位から容易にサンプリングされるマウス中のマウスバイオマーカーに焦点が当てられる。例えば、GWASおよび転写プロファイリングを用いる手法、ならびに例えば、T細胞サブセットの免疫染色およびFACを用いる手法が、AAマウスの皮膚および血液中のいくつかの病因バイオマーカーを同定するために取られてきた。脱毛症のC3H/HeJマウスにおける研究は、IFN誘導遺伝子などの、AAにおいて上方調節されるいくつかの皮膚バイオマーカーを既に同定している(図22)。スペクトル型データ(図20)は、これらの循環NKG2D+CD8 T細胞エフェクターが真正の自己抗原駆動性エフェクターT細胞である顕著な証拠を提供した。治療を行った、および行わなかったマウスにおけるこれらの血清学的皮膚および細胞バイオマーカーの一時的評価により、治療結果を予測し、臨床バイオマーカーの探索/利用を知らせる、力学的な、機構的に重要な、予見的炎症バイオマーカーを同定することができる。
頭皮からの皮膚生検を、最少で100人の円形脱毛症患者および約100人の対照から収集する。被験者集団はAA患者および毛髪移植を受けている罹患していない対照被験者からなる。これらのサンプルの収集のための認可は、それらが廃棄される組織であるため、迅速な審査/免除を受けた。AA患者からの頭皮生検の収集のためのプロトコルは、準備中である。最少で100人のAA被験者を動員するが、100人を超える集団が有益であり、この手法の力を増大させる。
若いC3H/HeJマウス(2〜3ヶ月)および可視的脱毛を示している引退したブリーダーを、Jackson Labsから獲得する。動物を、動物施設中で標準的な条件下で保持する。
生物標本中の経路を研究するためには、皮膚および血液中の異種細胞集団の分析が必要である。ヒト自己免疫の臨床調査において、円形脱毛症はその関連する微小環境中の病原性免疫エフェクターの研究/単離を可能にする末端器官の接近可能性のため、独特の機会を提供する。必要とされる分析道具および個人は、これらの正確な末端器官皮膚生物標本、および同じ個体から得られた血液中のその細胞/血清対応物の最適な研究を提供する。
以下のものは、血液の分析を補助するために提供される:
i)血清サンプルからの多分析物サイトカイン/ケモカイン分析、
ii)AA被験者からのヒトPBMCのサイトカイン分析を含む、フローサイトメトリー免疫表現型決定、
iii)T細胞サブセットを用いて下流の適用のためのフロー選別(RNAプロファイリング、スペクトル型決定)。
i)一次毛包と、機能的(細胞傷害性)および分析研究(T細胞クローニング、スペクトル型決定)のためのT細胞構成要素の両方の生細胞画分の調製/分析、
ii)全皮膚の免疫染色および転写プロファイリング、
iii)ヒトおよびマウスのAA皮膚に由来する真皮リンパ球のフローサイトメトリー免疫表現型決定。
a.AA被験者の末梢血中の病原性HF特異的細胞サブセットを同定する。この目的のために、スペクトル型分析を、循環しているヒト脱毛症T細胞集団を同定するための道具として使用し、同様に全皮膚T細胞中に認められるスペクトル型と、同じ患者から得られた末梢血中の特異的に選別された末梢血T細胞サブセットのスペクトル型とを一致させることにより、マウスモデルについて行った。
1.免疫モニタリング:
a.血清サンプルからの多分析物サイトカインおよびケモカイン分析。
CD8+NKG2D+細胞が、自然脱毛症を有するマウスの皮膚から収穫された真皮白血球集団で優位であることが、多パラメータフローを用いて確立された(図25)。ヒトでの状況に対処するために、3つのFACSチューブ(T細胞サブセットチューブ、CD4、CD8およびNK細胞のためのNKマーカーチューブならびに第3の非T細胞チューブ)を用いるNKおよびCD4およびCD8 T細胞集団に焦点を当てる30種のPBMCサブセットのいずれか;系列マーカー:CD3、CD4、CD8α、CD8β、CD14、CD19、CD20、CD56、CD64;T細胞サブセットマーカー:CD45Ra、CD45Ro、CD56、CD62L、CCR7、CD127、FoxP3、Helio;皮膚ケモカイン受容体分析:CCR4、CCR8、CCR10、CLA、EおよびP-セレクチン;NK免疫受容体ファミリーメンバー:阻害:KIR/CD158、CD94/NKG2A、LILR/ILT、LAIR1 NKR-P1 活性化:NKp46、NKp30、NKp44、NKG2C、NKG2Dにおける量的変化を定義するためのフローパネルが開発された。4人のAA被験者からのPBMCの初期評価において、NKG2D発現の増加がCD56+CD8+T細胞およびNK細胞上で観察されたが、CD4+T細胞上では観察されなかった(図26)。
目標は、下流の機能分析のための潜在的な脱毛症T細胞を提供することである。例えば、フロー選別されたT細胞サブセットは、RNAプロファイリング、例えば、NK型T細胞の転写プロファイリングのために提供される。バイオマーカー研究のためには、好適な細胞サブセット中でのホーニング(honing)は、さもなければ混合異種PBMC集団に由来するRNAの存在により希釈される分析の分解能を改善する。
組織または表現型で分離されたリンパ球サブセットからのRNAのスペクトル型決定またはTCRβ鎖長分布分析は、浸潤T細胞のTCRクローン型使用/レパートリーの定性的ポートレートを提供する。近年では、この技術は、サンプル中のクローン増殖したT細胞の割合を説明するための高感度かつ正確な方法として他者(5〜7)により用いられてきた。オリゴクローン性、TCRが制限されたT細胞のサブセットの増殖の証拠は、炎症プロセスにおける抗原駆動を示唆し、病原性T細胞クローンを同定するための第1のステップとして用いることができる。それぞれのTCR再配置は3つのヌクレオチドのうちの複数においてCDR3長が変化するため、T細胞の集団内のCDR3長の異種性を、TCR多様性の尺度として用いることができる。この技術の原理は、コンビナトリアルおよび接合VDJ連結により作製されたCDR3領域に共に広がる上流の特異的Vβファミリーメンバープライマーおよび下流の定常領域プライマーを用いてT細胞集団から得られたcDNAをPCR増幅することである。次いで、PCR産物を、蛍光標識された定常領域プライマーを用いるプライマー伸長(「ランオフ」)反応において蛍光色素標識する。次いで、前記産物をABI PRISM 3700 DNA分析装置上で泳動する。Gene Mapperソフトウェアを用いて、個別のCDR3に対応するピークのサイズを表示させ、分析することができる(図20および27)。データを輸出し、参照ポリクローナルレパートリーと比較し、Hamming距離などのオリゴクローン性の尺度を算出する。それぞれのヒストグラム中のピーク面積は、各ファミリー内の各CDR3長の発現頻度を反映する。皮膚におけるクローン増殖の優位性は、疾患の誘導における真皮抗原の主要な役割を示唆し、病原性T細胞の集中的調査を可能にする。対照的に、増殖しなかったクローンのポリクローナルレパートリーは、非クローン特異的ケモカイン受容体による動員を示唆する。同じ個体の非病変皮膚および血液におけるTCRレパートリーの比較は、クローン増殖された皮膚在住T細胞集団が循環中にも見出され得るかどうかを示唆する。同様に、選別されたT細胞サブセットを、レパートリー分析のための起源RNA材料のために用いることができる。例えば、図20および27は、C3Hマウスにおいて、オリゴクローン集団が、NKG2D陽性集団中で過剰に現れるが、陰性CD8リンパ節集団には現れないレパートリーである、病変皮膚中に見出されることを示す。これらのデータは、CD8+NKG2D+T細胞が皮膚中に見出されるオリゴクローンT細胞集団の多く(全てではないが)を含有することを確認する。いくつかのTCR/クローン、例えば、Vβ10は流出皮膚リンパ節中ではなく、皮膚中で見出されることに留意されたい。これらのものは、AA皮膚中に見出されるが、選別されたCD8+LN画分中に存在しないCD4+T細胞集団を含む可能性がある。この手法は、「抗原誘導」のための強力な証拠を提供し、ここで示されるようなフロー選別と共に、細胞の集団間の病原性T細胞サブセットを同定する。
侵襲性の薬剤難治性紅斑性狼瘡をマーキングする転写特徴を、全PBMCのRNAを用いた場合ではなく、CD8 T細胞コンパートメント内で同定することができる(8)。かくして、フロー選別と転写プロファイリングとの組合せは、分析能力に大きな力を与える。円形脱毛症においては、病原性細胞サブセットは、毛包を包み、両セリアック病(4、10)、I型糖尿病(11)および慢性関節リウマチ(12〜14)などの自己免疫において広く病原性に関連する(9)ものであり得るCD8+NKG2Dhi T細胞であると考えられる。セリアック病(4、10)におけるこれらの細胞の転写プロファイリングは、選別精製された循環および皮膚由来CD8 T細胞中で同様に評価されるこれらの細胞のNK様転写プログラミングを以前に記載した。これらの細胞集団の転写プロフィールに特異的に対処して、これらの細胞にとって中心的な生物経路を同定することが重要である。
活性化PBMCのフローサイトメトリー分析を、細胞内サイトカイン分析および/またはリン酸化タンパク質のシグナリング活性化のために用いることができる。古典的抗原特異的な、分裂促進反応または混合リンパ球反応を、CFSE染色されたT細胞またはチミジン取込みを用いて理解することができる。利用可能な装備としては、細胞収穫装置およびMicrobeta/Triluxプレートリーダー(チミジンに基づく増殖およびクロム放出細胞傷害アッセイのため)、ELISAプレートリーダー/ウォッシャー、ELISPOTリーダーが挙げられる。
機能的(細胞傷害性)、分取(T細胞クローニング)および分析研究(スペクトル型決定、RNAプロファイリング)のための一次毛包およびT細胞構成要素の調製。研究上にあるAA患者からの生検標本は貴重であり、免疫染色および転写プロファイリングはこれらの2つの手法の実現性および情報力を考慮すると、最も高い優先度を取る。日常的にヒト頭皮皮膚を対照個体(毛髪移植ドナー)から入手して、皮膚および毛包内の個々の細胞集団の一次培養を確立する。
i)HF標的:対照またはAAに罹患した個体から入手したヒト頭皮皮膚を用いて、皮膚および毛包(図28)内の個々の細胞集団の一次培養を確立して、一次HF集団中でのNKG2DLの調節を研究し、CTLアッセイにおける標的としてHF集団を評価する。毛包内皮膚をジスパーゼ処理して、表皮および真皮構成要素を分離し、酵素処理してケラチノサイトおよび線維芽細胞の一次培養を確立する。毛包を顕微解剖して間葉構成要素を分離し、真皮鞘細胞(DSC)および真皮乳頭細胞(DPC)を培養するためにさらに用いる。KondoおよびPhilpottら(15)からのプロトコル後にモデル化された実験室中で確立された無血清毛包器官培養物(図30)を用いて、対照個体、ならびにAA患者に由来する病変(可能な場合)および対照皮膚に由来する個々の毛包を日常的に培養する。毛包を無血清増殖条件で培養すると、7〜10日間通常の成長期を示し、次いで、退行期に入り、マトリックスの崩壊を示す(16)。細胞傷害性アッセイのために、毛包を、4〜8h、細胞傷害性T細胞またはNK細胞(MyeloCult、Stemcell Technologies)を保持する培地に移す。皮膚外植片は正常な皮膚構造、ならびに免疫微小環境を維持し、機能研究に用いる(17)。
本明細書に記載の研究のために皮膚-免疫相互作用を評価するための確立されたプロトコルが利用可能である。一次培養細胞、ならびに器官培養された毛包を、対照もしくは患者の末梢血リンパ球に由来する細胞傷害性T細胞、または皮膚から誘導されたものを用いる標的として用いることができる(図29)。免疫エフェクター細胞を、CFSE標識された標的細胞/毛包と同時培養し、LDH放出により細胞溶解を比色分析により測定するか、またはあるいは、死んだ細胞を7-AADで染色し、細胞数をフローサイトメトリーにより計数する。これらのアッセイを用いて、細胞傷害性などの、免疫エフェクターとHF標的との相互作用のための分子要件を決定することができる。例えば、本明細書に示される通り、細胞傷害性には、NKG2Dの関与ならびにNKG2DLの転写および表面発現を上方調節することが知られるサイトカイン/TLRリガンドを用いる標的の前もっての感作が必要である。
AAはいくつかの自己免疫障害との高い相関を示し、毛包を、自己免疫の基本的機構を研究するための高度にアクセス可能な器官にする。発症の初期代用生物マーカーの同定における実質的な興味があった。そのようなものとして、AAは広範囲の疾患との関連性を有し得るバイオマーカー開発のためのモデル系である。ヒトおよびマウスの両方におけるAAの発生および治療に対する応答をモニターするために、以下のバイオマーカーが現在用いられている。
頭皮からの皮膚生検を、AA患者および毛髪移植を受けている罹患していない対照被験者からなる正常被験者から収集する。さらなる生検を、臨床試験上にある被験者から取得する。
若いC3H/HeJマウス(2〜3ヶ月)および可視的脱毛を示している引退したブリーダーを、Jackson Labsから獲得する。それぞれの参加する基礎研究者により確立されたマウス株を、それぞれの動物飼育室で飼育する。マウスに水およびペレット食(5010げっ歯類用餌、LabDiet、PMI Nutrition International)を自由に与える。
Claims (14)
- 脱毛症の治療を必要とする哺乳動物被験体における脱毛症を治療する方法であって、Jak1および/またはJak2阻害剤を該被験体に投与することを含む、前記方法。
- 脱毛症の治療を必要とする哺乳動物被験体における脱毛症を治療する方法であって、Stat1および/またはStat2阻害剤を該被験体に投与することを含む、前記方法。
- 被験体における毛髪増殖を誘導する方法であって、
(a)有効量のJak/Stat阻害剤を被験体に投与し、それによって被験体における毛髪増殖を制御すること、
を含む、前記方法。 - 阻害剤が配列番号1、3、5または7を含むタンパク質に特異的に結合する抗体を含む、請求項1、2または3に記載の方法。
- 被験体が脱毛症に罹患している、請求項3に記載の方法。
- 脱毛症がアンドロゲン性脱毛症、休止期脱毛、円形脱毛症、休止期脱毛、頭部白癬、完全脱毛症、貧毛症、遺伝性先天性貧毛症、または全身性脱毛症を含む、請求項1、2または5に記載の方法。
- 投与された阻害剤が、該阻害剤を用いる治療の前の脱毛症に罹患した被験体の毛髪増殖と比較して、該被験体において毛髪増殖を誘導したかどうかを決定するステップ(b)をさらに含む、請求項1、2または3に記載の方法。
- 阻害剤が、Jak1、Jak2、Stat1もしくはStat2タンパク質をコードする遺伝子の発現を特異的に阻害するアンチセンスRNA;Jak1、Jak2、Stat1もしくはStat2をコードする遺伝子を特異的に標的化するsiRNA;または小分子である、請求項1、2または3に記載の方法。
- 阻害剤がINCB018424である、請求項1、2または3に記載の方法。
