Abstract
Description
本出願は、参照により本明細書に組み込まれる、「Embolectomy devices and methods for treatment of acute ischemic stroke condition」と題する、Jason Galdonikらによる、2010年4月13日に出願された米国仮特許出願第61/323,461号明細書に対する優先権を主張する。

血塊除去の処置は、概して、繊維ベースの血塊係合要素を血管内で血塊を通り越して送達すること、血塊を近位方向に引っ張ることと、血管から血塊を吸引することとを含む。プロセスは、血塊除去を容易にするために血塊を解体することをさらに含むことができる。血塊係合装置は、概して、先のセクションで説明した構造のうちのいずれをも含むことができる。処置は、血塊の除去中に塞栓として血塊の断片のいかなる放出も低減または排除するように設計されている。血塊の除去に続き、繊維ベースの装置は、場合によっては断片の除去を制限または防止する連続した吸引によって回収される。
可撓性ワイヤおよび血塊係合器具の繊維ベースの要素は、概して滅菌パッケージに合わせて包装される。好適な滅菌処置は本技術分野において既知であり、他の処置も開発され得る。システム全体のさらなる構成要素を合わせて包装してもしなくてもよい。たとえば、ガイドカテーテルの設計に応じて、別個に包装することができる従来の構成要素を用いてもよい。同様の問題は、システム全体の他の構成要素に関連する。概して、本システムで用いられる固有の構成要素を、便宜上合わせて包装することができる。
Claims (33)
- 1本または複数本の可撓性送達ワイヤと、不織繊維の少なくとも1つの束および第1取付要素を備えた繊維ベースの血塊係合要素と、を具備し、前記束の各繊維が一端において前記第1取付要素に固定されている、急性脳卒中治療装置であって、
前記第1取付要素が、前記送達ワイヤの上を並進することができるスライド、または前記送達ワイヤの円周の固定位置に固定されているアンカーのいずれかを含み、
前記第1取付要素が前記送達ワイヤに固定されたアンカーを有する場合、前記繊維の他端が、作動構造への固定取付なしに、第2取付要素において固定されていないかまたは束で固定されており、
前記繊維の束は、第1薄型送達形態と、前記繊維の固定されていない部分が、血管の変化する内周に沿うように好適な直径を有するように、前記送達ワイヤに対して外側に広がる第2形態と、を有し、前記繊維が、前記第1形態と前記第2形態との間で自発的に移行しない、装置。 - 前記第1形態の前記不織繊維が、前記束において前記送達ワイヤに沿って互いに対して実質的に平行である、請求項1に記載の装置。
- 前記第2形態の前記繊維が、前記繊維が織られていないままである一方で、前記マトリックス内でサイズが分散している有効な孔を備えた3次元濾過マトリックスを形成する、請求項1に記載の装置。
- 前記第1取付要素に放射線不透過性バンドをさらに具備する、請求項1に記載の装置。
- 前記繊維ベースの要素が、複数の不織繊維束を備えている、請求項1に記載の装置。
- 不織長繊維束が、前記可撓性ワイヤの円周に前記繊維に沿った1つまたは複数の位置で係留されて、互いに相互接続される複数の繊維束を形成する、請求項5に記載の装置。
- 前記複数の繊維束が、各々、それ自体の取付要素を有する、請求項5に記載の装置。
- 繊維束が、前記繊維束の一端または両端が固定されている、請求項5に記載の装置。
- 前記繊維束の一部の周囲に熱収縮ラップをさらに具備し、それにより、前記ラップが、前記第2形態で前記繊維ベースの構造の一部の周囲にジャケットを形成する、請求項1に記載の装置。
- 前記可撓性ワイヤに取り付けられるかまたは前記可撓性ワイヤで送達される補助的係合構造をさらに具備する、請求項1に記載の装置。
- 前記補助的係合構造が、複数のポリマーフィラメントを備えるフィラメント構造であり、前記ポリマーフィラメントが、前記繊維束の前記繊維より実質的に厚い、請求項10に記載の装置。
- 前記補助的係合構造が、複数の自己拡張型Nitinolワイヤを備えるNitinolフレームである、請求項10に記載の装置。
- 前記可撓性ワイヤで送達される機械的治療装置をさらに具備する、請求項1に記載の装置。
- 前記機械的治療装置が、アテローム切除装置、ステントまたは血管形成バルーンである、請求項13に記載の装置。
- 作動器具または展開器具を用いて、前記繊維ベースの要素を前記第1形態から前記第2形態に移行させる、請求項1に記載の装置。
- 前記作動器具がマイクロカテーテルである、請求項15に記載の装置。
- 前記展開器具が、複数のフィラメントを備えたフィラメントカートリッジを含む、請求項15に記載の装置。
- 急性脳卒中を治療するシステムであって、
請求項1に記載の繊維ベースの血塊係合要素と、
管状遠位端を有する作動器具であって、前記作動器具の前記遠位端が前記送達ワイヤの上を摺動して、前記繊維ベースの血塊係合要素の前記繊維を前記第2の広がった形態に展開するような内径を有する作動器具と、
遠位開口部を有する、内腔を備える吸引カテーテルであって、前記吸引カテーテルを前記可撓性送達ワイヤで送達することができる、吸引カテーテルと、
を具備するシステム。 - 大脳動脈内で血塊係合装置を送達する方法であって、
内頸動脈内にガイドカテーテルの遠位開口部を位置決めするステップと、
前記血塊係合装置を、前記内頚動脈から下流の大脳動脈にアクセスするように前記ガイドカテーテルを通じて送達するステップであって、前記血塊係合装置が、可撓性ワイヤによって支持される繊維ベースの血塊係合要素を備える、ステップと、
作動要素を前記可撓性ワイヤ上で前進させて、前記繊維ベースの要素を、前記繊維が前記動脈の内周に沿うように拡張形態に展開するステップであって、前記作動要素の移動が、前記可撓性ワイヤの上で拘束されない、ステップと、
を含む方法。 - 前記可撓性ワイヤが、前記大脳動脈にアクセスするように直接前進する、請求項19に記載の方法。
- 前記送達ステップが、
ガイドワイヤを、前記大脳動脈にアクセスするように送達するステップと、
マイクロカテーテルを、前記大脳動脈にアクセスするように前記ガイドワイヤで送達するステップと、
前記ガイドワイヤを回収するステップと、
前記血塊係合装置を、前記大脳動脈にアクセスするように前記可撓性ワイヤで前記マイクロカテーテルを通じて送達するステップと、
を含む、請求項19に記載の方法。 - 前記血塊係合装置の後方に前記マイクロカテーテルの前記遠位開口部を引っ張って、前記血塊係合装置を前記動脈内に露出させ、その後、前記マイクロカテーテルを前記血塊係合装置に対して前方に引っ張って前記装置を作動させるステップをさらに含む、請求項21に記載の方法。
- 前記作動要素が、フィラメントカートリッジを備えた展開器具に結合されたプッシュカテーテルであり、前記フィラメントカートリッジが、前記血塊係合装置を展開するのを支援する前に拡張形態になるように展開される、請求項19に記載の方法。
- 前記フィラメントカートリッジが、フィラメント束、三角形状フィラメントまたは星形フィラメントであり得る複数のフィラメントを含む、請求項23に記載の方法。
- 前記プッシュカテーテルを用いて、前記展開された展開器具を押して前記血塊係合装置を展開するステップをさらに含む、請求項23に記載の方法。
- 急性脳卒中事象をもたらす大脳動脈からの血塊を除去する方法であって、
前記大脳動脈内で、繊維ベースの血塊係合装置を送達ワイヤ上で前記血塊の遠位に位置決めするステップと、
前記繊維ベースの血塊係合装置を、前記繊維の少なくとも一部が前記送達ワイヤに対して外側に拡張して前記大脳動脈の内周に沿うように、拡張形態に展開するステップと、
前記展開された血塊係合装置を、内頚動脈内に位置付けられた吸引カテーテルに向かって引っ張り、それにより、前記血塊係合装置が前記血塊と係合するステップであって、前記血塊係合装置の前記繊維が、前記動脈の変化する内周に沿ったままであり、それにより、前記引張プロセス中に前記血塊を係合し続ける、ステップと、
前記吸引カテーテルを通じて吸引を施す一方で、前記血塊係合装置の近位移動により前記血塊を前記吸引カテーテル内に引き込むステップと、
を含む方法。 - 機械的治療装置により前記血塊を遊離させるステップをさらに含む、請求項26に記載の方法。
- 前記機械的治療装置が、アテローム切除装置、ステントまたは血管形成バルーンである、請求項26に記載の方法。
- 前記血塊係合装置が、補助的係合構造をさらに備える、請求項26に記載の方法。
- 前記補助的係合構造が、複数のポリマーフィラメントを含むフィラメント構造、複数の自己拡張型Nitinolワイヤを含むNitinolフレームまたはそれらの組合せである、請求項29に記載の方法。
- 吸引中に頸動脈を閉塞するように前記吸引カテーテルの前記遠位端の近くに取り付けられた閉塞バルーンを展開するステップをさらに含む、請求項26に記載の方法。
- 前記血塊係合装置が、血液が通過するのを可能にする、請求項26に記載の方法。
