Abstract

Description
本願は、米国仮特許出願第60/788,296号(2006年3月31日出願)に基づく優先権の利益を主張する。この先の出願は、明確に、その全体が本明細書に参考として援用される。
(発明の分野)
本発明は、腫瘍細胞に対する免疫応答を誘導するための細胞組成物および方法に関する。この細胞組成物は、抗細胞傷害性Tリンパ球関連抗原−4(CTLA−4)抗体およびサイトカインを発現するように遺伝的に改変された細胞を含み、それによって、免疫化部位における局所的な細胞ベースの抗CTLA−4抗体の持続発現が、全身投与と比較して、治療濃度を顕著に減少させる。
癌に対する1つの治療的アプローチは、抗腫瘍免疫を増強するためのワクチンとしての自己由来または同種異系の癌細胞の使用である(非特許文献1;非特許文献2ならびに非特許文献3)。このアプローチの拡張は、免疫療法部位において局所的にサイトカインを発現する遺伝的に改変された腫瘍細胞の使用を伴う。

自己由来サイトカイン発現腫瘍細胞の使用は、各々の患者の腫瘍が、別の患者からの組織学的に類似する、MHCが一致する腫瘍細胞上で見出されるものとは異なり得る腫瘍抗原の独特のセットを発現するので、利点を提供する。例えば、Kawakamiら、J.Immunol.、148、638−643(1992年);Darrowら、J.Immunol.、142、3329−3335(1989年);およびHomら、J.Immunother.、10、153−164(1991年)を参照のこと。対照的に、異なる供給源由来のMHCが一致する腫瘍細胞は、本発明の細胞ベースの癌免疫療法を調製するために、患者を外科手術に供してその腫瘍のサンプルを入手する必要がない、という利点を提供する。

本発明の細胞ベースの癌免疫療法は、非修飾の腫瘍細胞、サイトカインおよび抗CTLA4抗体を発現するように修飾された腫瘍細胞または非腫瘍細胞、抗CTLA4抗体を発現するように修飾された腫瘍細胞または非腫瘍細胞、サイトカイン、例えばGM−CSFなどを発現するように修飾された腫瘍細胞または非腫瘍細胞から選択される1つ以上の異なる細胞集団を含み得る。本発明の細胞ベースの癌免疫療法は、被験体への投与の前に増殖不能にされる。本明細書で使用される場合、「増殖不能」または「不活性化された」という用語は、複数ラウンドの有糸分裂を受けることが不可能であるが、サイトカイン、腫瘍抗原または抗原性タンパク質などのタンパク質を発現する能力をなお保持している細胞をいう。このことは、当業者に公知である多数の方法を通して達成されてもよい。被験体への投与の前に、本発明の細胞ベースの癌免疫療法は、典型的には、約50〜約200ラド/分、または約120〜約140ラド/分の線量で、被験体への投与の前に照射される。典型的には、照射を使用する場合に、必要とされる合計レベルは、2,500ラド、5,000ラド、10,000ラド、15,000ラド、または20,000ラドである。好ましくは、細胞は、実質的に100%の細胞を、さらなる増殖から阻害するために十分な合計線量で照射される。
細胞ベースの癌免疫療法は、1つ以上のサイトカインのコード配列を含んでもよい。「サイトカイン」は、非限定的に、局所的に作用して血液中を循環せず、本発明に従って使用される場合、個体の免疫応答の変化をもたらすホルモンを含む。典型的には、サイトカインはヒトサイトカインである。
本発明の細胞ベースの癌免疫療法において、少なくとも1つの腫瘍抗原が発現される。この腫瘍抗原は、遺伝子改変の前に、自己由来細胞、同種異系細胞またはバイスタンダー細胞集団によってか、または腫瘍抗原をコードするベクターからのネイティブなコード配列の発現によって発現させることができる。本発明を実施する際の目的の腫瘍抗原は、処置下にある癌と関連し、免疫応答の増強が所望される腫瘍抗原である。本発明の組成物および方法を使用する処置のための例示的な癌標的には以下が含まれるが、これらに限定されない:膀胱癌、乳癌、結腸癌、頭頸部癌、腎臓癌、肝臓癌、肺癌、卵巣癌、子宮頚癌、膵臓癌、直腸癌、前立腺癌、胃癌、表皮癌;リンパまたは骨髄細胞系列の造血器腫瘍;線維肉腫または横紋筋肉腫などの間葉起源の腫瘍;黒色腫、奇形癌、神経芽細胞腫、神経膠腫、腺癌、および非小細胞肺癌などの他の腫瘍型。処置される腫瘍細胞の型が前立腺癌である場合、前立腺腫瘍細胞株は、DU145、PC−3およびLnCaPからなる群より選択されてもよい。
CTLA4(CD152)は、種々のモデル系において、抗原提示細胞上のB7分子(APC:B7−1(CD80)およびB7−2(CD86))との相互作用によって、T細胞活性化をネガティブに調節する、T細胞によって発現される免疫調節分子である(例えば、Kormanら、Curr.Opin.Invest.Drugs 6:582−591(2005年)を参照のこと)。CTLA4ノックアウトマウスは、4週間未満で死亡をもたらす大量のリンパ球増殖を示す。CTLA4は広範に研究されてきたが、T細胞増殖を減少させる際のその作用の機序は完全に理解されてはいない。この機序には、APCおよびT細胞界面におけるB7リガンドの競合;IL−2をダウンレギュレートするシグナル伝達事象;APCによるインドールアミン2,3−ジオキシゲナーゼの誘導;およびネガティブ調節サイトカインの誘導が含まれると考えられている。
抗体は、重鎖および軽鎖のヘテロ二量体である免疫グロブリンタンパク質であり、哺乳動物培養発現系において単一のベクターから全長で発現させることは困難であることが判明している。3つの方法が、脊椎動物抗体の産生のために現在使用されている。「ポリクローナル」抗体を産生するための動物のインビボ免疫化、モノクローナル抗体を産生するためのB細胞ハイブリドーマのインビトロ細胞培養(Kohlerら、Eur.J.Immunol.、6:511、1976;Antibodies:A Laboratory Manual、Cold Spring Harbor Laboratory、1988年)、および組換えDNA技術(例えば、Cabillyら、米国特許第6,331,415号に記載されている)である。

腫瘍抗原、抗CTLA4抗体またはサイトカインのコード配列は、細胞による発現をもたらすために有効である任意の方法を使用して、細胞に導入されてもよい。典型的には、目的のコード配列を含むベクターは、当業者によって慣用的に使用される慣用的な分子生物学的技術を使用して調製される。

BALB/cマウスCT26腫瘍モデルにおける以下の研究は、抗CTLA4抗体を発現する、レトロウイルスで形質導入されたサイトカイン発現癌免疫療法が、より低い全身抗体濃度で、腫瘍保有動物の生存を改善することを実証する。0日目に、BALB/cマウスに、5×105個の生きているCT26腫瘍細胞を皮下接種した。3日後、マウスを、1×106個の照射されたGM−CSF分泌CT−26を用いてか、またはGM−CSFおよび抗CTLA4抗体の両方を分泌する同じ数の照射されたCT−26細胞を用いて免疫化した。対照群は、3日目に、1×106個の照射されたGM−CSF分泌CT−26腫瘍細胞、および組換え抗CTLA4抗体の全身投与(腹腔内−150μg、10μg)または局所的皮下投与(サイトカイン発現癌免疫療法と混合−10μg)を用いて免疫化した。マウスを、明白な腫瘍の形成について週に2回モニターし、腫瘍が壊死性になるか、サイズが1500mm3を超える場合、屠殺した。
異なる腫瘍モデルにおける抗CTLA4抗体の局所的送達を評価するために、上記のGM−CSFに加えて抗CTLA4抗体を分泌するB16F10細胞を試験した。0日目に、C57BL/6マウスを、2×105個の生きているB16F10腫瘍細胞でチャレンジした。マウスを、1日目に、3×106個の照射されたGM−CSF分泌B16F10腫瘍細胞を用いてか、またはGM−CSFおよび抗CTLA4抗体の両方を分泌する同じ数の照射されたB16F10細胞を用いて免疫化した。別個の群を、照射されたGM−CSF分泌腫瘍細胞、および組換え抗体の全身(腹腔内)の投与で、2日目(150μg)および5日目(100μg)に免疫化した。免疫化および抗体注射の第2のラウンドを、14日目に開始して投与した。腫瘍量をモニターし、マウスを、チャレンジ腫瘍が1500mm3に達するか、または重篤な潰瘍が発生した場合に、屠殺した。
