免疫系は、病原体、すなわち、細菌、ウイルス、真菌などによって引き起こされる疾患、ならびに、身体自身の細胞および組織の異常増殖によって引き起こされる疾患(すなわち癌性腫瘍)に対抗する、身体の一次防御である。通常、免疫系は、身体の正常細胞または「自己」と外来病原体もしくは異常細胞または「非自己」とを区別することができる。免疫系が自分自身の身体に反応しないようにするプロセスは、寛容性と呼ばれる。時折、免疫系は、「自己」を正常と認識する能力を失い、組織または細胞を対象とするその後の応答は寛容性を失い、自己免疫の状態となる。自己免疫に起因する病態は、重篤な臨床転帰をしばしば有し、かつ、世界中、特に先進国における主要な健康問題の1つである。
1つの局面において、本発明は、SEQ ID NO: 29のアミノ酸残基20番目〜145番目に対して少なくとも80%の同一性およびSEQ ID NO: 37のアミノ酸残基21番目〜126番目に対して少なくとも80%の同一性を含む抗ヒトIL-21モノクローナル抗体を提供する。一定の態様において、これらの抗体は、SEQ ID NO:31の重鎖可変領域CDR1中に存在する、同一性が少なくとも80%の変化を含む。

IL-21は、最適なCD8+T細胞性免疫、NK細胞活性化、ならびに、抗体産生およびB 細胞成熟など最適な体液性応答のために重要であるCD4+T細胞由来のサイトカインである。IL-21は、IL-18、IL-15、IL-5、IL-6、IL-7A、IL-17F、TNFRII、sCD25、およびRANTESなどいくつかの炎症誘発性のケモカインおよびサイトカインを誘導することが示されている。IL-21はまた、IV注射またはSC注射によって投与された場合に、非ヒト霊長類およびヒトにおいて、急性期応答も誘導する(Dodds et al., Cancer Immunol Immunother 2008 Oct 17[電子出版])。インビトロにおいて、多発性骨髄腫細胞および急性T細胞白血病など特定の新生物性免疫細胞集団の増殖を刺激する(Brenne et al Blood 99(10):3756-62 (2002), diCarlo E, et al Cancer Immunol Immunother 56(9):1323-1324 (2007))。IL-21はまた、ホジキンリンパ腫においてホジキン病リード・シュテルンベルク細胞によっても産生される(Lamprecht et al., Blood 112(8):3339-47, 2008)。IL-21受容体発現の増大が、全身性硬化症(Distler et al., Arthritis & Rheumatism 52:865-864, 2004)および関節リウマチ滑膜線維芽細胞(Jungel et al., Arthritis & Rheumatism 50:1468-1476, 2004)に罹患している患者の表皮において示されている。さらに、自己免疫性の糖尿病NODマウスでも、IL-21受容体発現が増大していた(King et al., Cell 117:265-277, 2004)。IgGおよびIL-21の発現が、自己免疫性エリテマトーデス様疾患を発症するBXSB-Yaaマウスモデルにおいて増大していることが示された(Ozaki et al., J. Immunol. 173:5361-5371, 2004);狼瘡易発性サンロケ(sanroque)マウスにおいて、IL-21発現はより強い(Vinuesa et al. Nature 435:452, 2005);クローン病患者の炎症を起こしていない腸組織よりも炎症を起こした組織の方が、IL-21発現が強い(Monteleone, et al., Gastroenterology 128:687-694, 2005)。また、IL-21は、セリアック病患者の粘膜でも過剰産生される(Finn et al. Gut, PMID:17965065, 2007)。
アレルギー性接触性皮膚炎は、皮膚と接触する抗原に対するT細胞性免疫反応と定義される。アレルゲン依存性のT細胞応答が、CLA+細胞集団にほとんど限局されているため、CLA+T細胞集団は、皮膚炎の開始に関与していると考えられている(Santamaria-Babi, et al., J Exp Med 181:1935,(1995)を参照されたい)。最近のデータから、一般的な接触過敏症アレルゲンであるニッケルに応答して増殖し、かつ1型(IFN-γ)サイトカインおよび2型(IL-5)サイトカインの両方を産生するのは、記憶(CD45RO+)CD4+CLA+T細胞のみであり、CD8+T細胞は産生しないことが判明した。さらに、CD4、CD45RO(記憶)、またはCD69と組み合わせてCLAを発現する細胞は、ニッケル特異的な刺激の後に増加し、かつ、ケモカイン受容体CXCR3、CCR4、CCR10を発現するが、CCR6は発現しない。Moed et al., Br J Dermatol 51:32,(2004)を参照されたい。
アトピー性皮膚炎(AD)は、この数10年の間に発病率が劇的に増加している、慢性的に再発する炎症性皮膚疾患である。臨床的に、ADは、慢性的な再発性経過を示す、著しくそう痒性で、しばしば擦りむけた斑および丘疹を特徴とする。ADの診断は、主として、主要な臨床所見および軽微な臨床所見に基づく。 Hanifin J.M., Arch Dermatol 135: 1551(1999)を参照されたい。組織病理学により、急性病変における海綿状態、過錯角化、および限局的な錯角化が明らかになるのに対し、過錯角化および錯角化、アカントシス/顆粒層肥厚、ならびにリンパ球および大量の肥満細胞による真皮の血管周囲浸潤を伴う著しい表皮過形成は、慢性病変の特徴である。
変形関節炎、関節リウマチ、および傷害が原因で関節炎を起こした関節などを含む関節炎は、抗炎症性の抗体および結合ポリペプチドの治療的使用から利益を受けると思われる一般的な炎症性状態である。例えば、関節リウマチ(RA)は、身体全体に影響を及ぼす全身性疾患であり、関節炎の最も一般的な形態の1つである。これは、疼痛、こわばり、熱感、発赤、および腫脹を引き起こす、関節の内側を覆う膜の炎症を特徴とする。炎症細胞は、硬骨および軟骨を消化し得る酵素を放出する。関節リウマチの結果として、炎症を起こした関節の内層、すなわち滑膜は、硬骨および軟骨に浸入し、かつそれらを損傷して、他の生理的作用に混じって関節の劣化および重度の疼痛をもたらし得る。巻き込まれた関節は、その形状および配列を失って、疼痛および動作低下をもたらし得る。
関節リウマチ(RA)は、重度の障害および死亡率の上昇を招く、炎症およびそれに続く組織損傷を特に特徴とする免疫介在性疾患である。様々なサイトカインが、リウマチ関節において局所的に産生される。多数の研究により、2種のプロトタイプの炎症誘発性サイトカインであるIL-1およびTNF-αが、滑膜炎症および進行性の関節破壊に関与するメカニズムにおいて重要な役割を果たしていることが実証された。実際、TNF-αおよびIL-1の阻害物質をRA患者に投与すると、炎症の臨床徴候および生物学的徴候が劇的に改善し、かつ、骨びらんおよび軟骨破壊の放射線医学的徴候が減少した。しかしながら、これらの有望な結果にもかかわらず、かなりの比率の患者がこれらの作用物質に応答しないことから、他のメディエーターも関節炎の病態生理に関与していることが示唆されている(Gabay, Expert. Opin. Biol. Ther. 2(2):135-149, 2002)。
アメリカ合衆国では、約500,000人の人々が炎症性腸疾患(IBD)に罹患しており、この疾患は、結腸および直腸(潰瘍性大腸炎)、または小腸および大腸の両方(クローン病)のいずれかを冒し得る。これらの疾患の病因は不明であるが、これらは罹患組織の慢性的な炎症を伴う。潰瘍性大腸炎(UC)は、一般に結腸(colon)と呼ばれる大腸の炎症性疾患であり、結腸の粘膜または最も内側の壁の炎症および潰瘍を特徴とする。この炎症は、結腸の頻繁な排便を引き起こし、結果として下痢を生じる。症状には、ゆるい大便、ならびに付随する腹部の有痛性痙攣、発熱、および体重減少が含まれる。UCの正確な原因は不明であるが、最近の研究により、身体の天然の防御が、免疫系が「非自己」であると考える身体中のタンパク質に対抗して作用すると示唆されている(「自己免疫反応」)。おそらく、これらのタンパク質が腸中の細菌タンパク質に似ているため、それらは、結腸の内壁を破壊し始める炎症プロセスを扇動または刺激する可能性がある。結腸の内壁が破壊される間に、潰瘍が形成して、粘液、膿、および血液を放出する。この疾患は、通常、直腸領域で始まり、最終的には大腸全体に拡大し得る。炎症のエピソードが繰り返されると、瘢痕組織によって腸および直腸の壁が肥厚する。結腸組織の死滅または敗血症が、重度の疾患と共に発生し得る。潰瘍性大腸炎の症状の重症度は様々であり、かつ、それらの発症は漸進的または急激であり得る。呼吸器感染症またはストレスを含む多くの因子によって、発病が引き起こされ得る。
乾癬は、700万人を超えるアメリカ人に発症する慢性皮膚病態である。乾癬は、新しい皮膚細胞が異常に増殖した結果、炎症を起こし、隆起し、かつ鱗屑を有する皮膚部分を生じ、その場所でのケラチノサイトの最終分化が変化する場合に発生する。最も一般的な型である尋常性乾癬は、銀白の鱗屑で覆われた、皮膚の炎症部分(「病変」)を特徴とする。乾癬は、少数のプラークに限定されるか、または中程度から広範囲の皮膚領域を含む場合があり、頭皮、膝、肘、および胴に最も一般的に現れる。極めて目立つが、乾癬は、接触感染性疾患ではない。これらの疾患の病因は、T細胞活性化、抗原提示の変化、および炎症性樹状細胞によるサイトカイン産生、ならびに罹患組織の慢性的炎症を含む。本発明の抗IL-21抗体は、乾癬、他の炎症性皮膚疾患、皮膚アレルギーおよび粘膜アレルギー、ならびに関連した疾患における炎症および病理学的作用を減少させる有益な治療物質としての機能を果たす可能性がある。
全身性エリテマトーデス(SLE)は、遍在する自己抗原を対象とする慢性的なIgG抗体(例えば抗dsDNA)産生を特徴とする、免疫複合体に関連した障害である。糸球体腎炎がいくつかの症例(すなわちループス腎炎)をもたらす場合があるものの、SLEの作用は、特定の器官に限局されるよりはむしろ全身性である。多数の染色体座位がこの疾患に関係付けられており、かつ、抗dsDNA抗体および糸球体腎炎など、疾患の様々な局面をもたらしている可能性がある。CD4+T細胞は、SLEのマウスモデルにおいて活動的な役割を果たすことが示されている(Horwitz, Lupus 10:319-320, 2001; YellinおよびThienel, Curr. Rheumatol. Rep., 2:24-37, 2000)。CD8+T細胞の役割は、明瞭に定義されていないが、狼瘡患者において「サプレッサー」CD8+T細胞機能が障害されていることを示唆する証拠がある(Filaci et al., J. Immunol., 166:6452-6457, 2001; Sakane et al, J. Immunol., 137:3809-3813, 1986)。
固形臓器を移植されたレシピエントは、組織適合性不一致が原因で同種移植片の急性拒絶または慢性拒絶を発症する場合がある。これらの患者におけるHLA分子を対象とする抗体(同種抗体)の生成は、T細胞への外来抗原の提示の結果として起こる。同種抗体は、結合された同種抗体によって指示される免疫複合体の形成、補体結合、および抗体を媒介とした細胞障害活性を介して、移植片に組織損傷をもたらし得る。また、補体カスケードは、内皮細胞を活性化し、移植片内部に脈管障害を引き起こす局所的因子を放出する。補体生成物C4dは、急性移植拒絶および慢性移植拒絶両方の初期マーカーであり、明白な病理学的変化より前に無症候性症例において検出することができる(RacusenおよびHaas, Clin J Am Soc Nephrol 1: 415-420, 2006; MollおよびPascual, Am J Transplantation 5: 2611-2618, 2005; TinkamおよびChandraker, Clin J Am Soc Nephrol 1: 404-414, 2006)。移植前に、患者は抗HLA同種抗体(パネル反応性抗体)についてスクリーニングされる。患者は、以前の同種移植失敗、輸血、または多胎妊娠が原因で著しく感受性が高い場合がある。感受性の高い移植患者中に同種抗体が存在すると、免疫抑制療法の強化が必要とされる場合があり、急性拒絶の可能性が高いため、治療が複雑になる(Baid et al., Curr Opin Immunol 13:577-581, 2001)。場合によっては、B細胞標的物質(ミコフェノール酸またはリツキシマブ)が使用されるが、この治療戦略は、抗体分泌形質細胞を直接標的とはしない。また、プラスマフェレーシスも、循環血中の免疫グロブリンを減少させるために使用される。すべての場合において、移植レシピエントは、拒絶リスクを低下させるためのT細胞を標的とした免疫抑制剤で治療し、移植片に対する寛容性が確立されるにつれ、これらの治療計画からゆっくりと「離れ」させてよい(Seyfort-MargolisおよびTurka, J Clin Invest 118(8): 2684-2685, 2008; Taylor et al., Crit Rev Oncol Hematol 56:23-46, 2005; AmanteおよびEjercito, Transplant Proc 40: 2274-2280, 2008)。
A. 免疫化およびハイブリドーマ
IL-21タンパク質を、その全体が本明細書に組み入れられる米国特許第7,250,274号で説明されているようにして作製した。可溶性IL-21受容体タンパク質を、いずれもその全体が本明細書に組み入れられる米国特許出願第2007-0122413号および米国特許第6,777,539号で説明されているようにして作製した。抗IL-21モノクローナル抗体を、マウス抗体を生成する野生型マウスおよび完全ヒト抗体を生成するトランスジェニックマウスにおいて産生させた(Medarex, Princeton, NJ)。マウスをヒトIL-21タンパク質で免疫化した。簡単に説明すると、BSA(Imject Pharmalink Immunogen Kit, Pierce)と結合させた精製組換えIL-21(ZymoGeneticsにより大腸菌中で作製された)30μgを皮下注射し、製造業者の取扱い説明書に従ってCpG(オリゴヌクレオチド、マウスTLR9リガンド)およびGM-CSF (Granulocyte Macrophage Colony Stimulatory Factor、R&D, Minneapolis, MN)ならびにEmulsigen(登録商標)-Pアジュバント(MVP Laboratories, INC, Omaha, NE)と組み合わせて投与して、マウスを最初に免疫化した。最初の免疫化に続いて、Emulsigen(登録商標)-Pアジュバントに溶かしたIL-21 30μgを週に一度の間隔でさらに3回各マウスに皮下経路で与えた。4回目の免疫化後7日目に、後眼窩神経叢経由でマウスから採血し、IL-21に結合する能力を解析するために血液から血清を分離した。
2つの発現カセットを用いて、ヒト抗ヒトIL-21モノクローナル抗体(ハイブリドーマクローン362.78.1.44由来)をCHO細胞において発現させた。改変したヒトIgG1定常領域にVH鎖を融合させた。改変IgG1であるIgG1.1は、エフェクター機能を低下させる5つのアミノ酸置換を含んだ。配列内リボソーム進入部位(IRES)配列を有するジヒドロ葉酸還元酵素(DHFR)選択マーカーにヒト抗ヒトIL-21重鎖を連結した。ヒト抗ヒトIL-21重鎖およびDHFR選択マーカーの発現を、ヒトサイトメガロウイルス(CMV)エンハンサーおよび骨髄増殖性肉腫ウイルス(MPSV)エンハンサー/プロモーターの融合物からなる構成的な合成プロモーターによって指示した。サルウイルス40(SV40)ポリアデニル化シグナルを用いて、DHFR選択マーカーの末端で転写を終結させた。ヒト免疫グロブリンκ定常領域にVL鎖を融合させた。IRES配列を有するピューロマイシン耐性(puroR)選択マーカーにヒト抗ヒトIL-21軽鎖を連結した。ヒト抗ヒトIL-21軽鎖およびpuroR選択マーカーの発現を、ヒトCMVエンハンサーおよびMPSVエンハンサー/プロモーターの融合物からなる構成的な合成プロモーターによって指示した。SV40ポリアデニル化シグナルを用いて、puroR選択マーカーの末端で転写を終結させた。ヒト抗ヒトIL-21重鎖及び軽鎖の発現カセットをCHO DXB-11宿主細胞に同時トランスフェクトした。ピューロマイシン選択に続いてメトトレキサート選択を行って、ヒト抗ヒトIL-21モノクローナル抗体の高く安定な発現を得た。IL-21 mAbクローン362.78.1.44をCHOで発現させた種類を以下の実施例では「362.78-CHO」と呼ぶ。
2A. ペプチドの結合および中和
抗ヒトIL-21結合性モノクローナル抗体および抗ヒトIL-21中和性モノクローナル抗体がヒトIL-21、変異ヒトIL-21タンパク質、およびヒトIL-21配列由来の合成ペプチドに結合する能力を、直接(Direct)ELISAアッセイ形式で実証した。
表面プラズモン共鳴を用いて、ヒト組換えIL-21およびカニクイザル組換えIL-21に対する結合親和性に関して抗IL-21モノクローナル抗体362.78-CHOを評価した。
抗ヒトIL-21抗体がマウスIL-21タンパク質もしくはカニクイザルIL-21タンパク質またはヒトIL-21配列由来合成ペプチドと交差反応し結合する能力の測定
特異的抗体の別の重要な特徴は、抗体が標的タンパク質に結合し拮抗するが、関連するタンパク質(非標的)と有意な程度には結合しない能力である。抗ヒトIL-21抗体が関連サイトカインに結合する能力をウェスタンブロット形式で試験した。γc-サイトカインファミリーの全メンバーの試料を獲得し、SDS-PAGEで電気泳動し、ブロッティング用のニトロセルロース膜に移した。組換えヒトIL-2(202-IL/CF)、ヒトIL-4(204-IL/CF)、ヒトIL-7(207-IL/CF)、ヒトIL-9(209-IL/CF)、ヒトIL-15(247-IL/CF)(すべてR&D Systems, Minneapolis MNから入手)、ヒトIL-21(ZymoGenetics)、およびヒトIFN-λ(米国特許第6,927,040号;同第7,252,969号)を用いて抗体の特異性を評価した。試験した抗体はすべて、ヒトIL-21を除くγc-ファミリーサイトカインに対してバックグラウンドを上回る検出可能な結合を示さず(ヒトIL-21の場合は明らかな結合が観察された)、これらの抗体を用いた先のウェスタンブロットと一致していた(実施例3を参照されたい)。
エピトープビニング実験を実施して、どの抗IL-21モノクローナル抗体がヒトIL-21に同時に結合できるかを判定した。ヒト抗体およびマウス抗体の両方を示した。抗原上の同じまたは共通部分がある結合部位(エピトープ)を得るために競合する抗IL-21モノクローナル抗体は同時に結合することができず、1つのファミリーまたは「エピトープビン」に機能的にグループ分けされる。抗原上の同じ結合部位を得るために競合しない抗IL-21モノクローナル抗体は同時に結合することができ、別々のファミリーまたは「エピトープビン」にグループ分けされる。実験はBIACORE T100(商標)機器を用いて実施した。可溶性ヒトIL-21(ZymoGenetics)を抗原として用いてエピトープビニング実験を実施した。
どの抗ヒトIL-21モノクローナル抗体がIL-21可溶性受容体と同時にIL-21に結合できるかを判定するために、競合実験を実施した。抗原上の同じまたは共通の部分がある結合部位(エピトープ)を得るために可溶性受容体と競合する抗ヒトIL-21モノクローナル抗体は同時に結合することができない。抗原上の同じ結合部位を得るために可溶性受容体と競合しない抗IL-21モノクローナル抗体は同時に結合することができる。可溶性組換えヒトIL-21を抗原として用いて競合実験を実施した。IL-21可溶性受容体と競合させる前に、IL-21抗原をモノクローナル抗体に結合させた。これらの研究でモノクローナル抗体を解析するために、2種類のIL-21可溶性受容体(両方ともZymoGenetics社製)を使用した。1種類目の受容体は、ヒト免疫グロブリンに由来するFc分子に融合されたIL-21受容体の細胞外ドメインから構成されるホモ二量体受容体(IL-21R-Fc)からなる。第2の可溶性受容体の形態は、その全体が参照により本明細書に組み入れられる共同出願の米国特許第6,777,539号で説明されているように、ヒト免疫グロブリンに由来するFc分子に融合されたIL-21受容体の細胞外ドメインを含む1つのサブユニットおよびヒト免疫グロブリンに由来するFc分子に融合された共通のγ共通鎖の細胞外ドメインを含む第2のサブユニットから構成されるヘテロ二量体受容体(IL-21R/γc-Fc)であった。
下記のリン酸化STAT3バイオアッセイ法を一次スクリーニングとして用いて、マウス血清中の中和性抗IL-21の力価ならびにハイブリドーマ上清および精製抗IL-21抗体によるIL-21中和の相対的レベルを測定した。Baf3/KZ134/huIL-21R細胞におけるリガンド-受容体相互作用後のSTAT3リン酸化レベルを測定することによってIL-21活性を測定した(SpolskiおよびLeonard, Annu Rev Immunol. Nov 8; 2007を参照されたい)。EC50濃度のIL-21およびアンタゴニストの漸増物(titration)を用い、リン酸化STAT3レベルの低下に基づいて、相対的中和活性を決定した。
この24時間アッセイ法では、Baf3/KZ134/huIL-21Rをトランスフェクトされた細胞においてIL-21によって誘導されたSTAT-ルシフェラーゼ活性を測定する。96ウェル平底乳白色培養プレート(Corning/Costar, Lowell, MA)中に40,000細胞/ウェルで播種する前に、アッセイ用培地(5%ウシ胎児血清、1×Glutamax、1%ピルビン酸ナトリウム、および2μM β-メルカプトエタノール(すべてInvitrogen(Carlsbad, CA)社製)を添加したフェノールレッド無添加RPMI 1640)で、Baf3/KZ134/huIL-21Rをトランスフェクトされた細胞を2回洗浄した。次いで、試験溶液を調製する間、組織培養インキュベーター中に細胞を置いた。別々のプレートで、ヒトIL-21を培地または一連のIL-21アンタゴニスト(モノクローナル抗体もしくは可溶性ヒトIL-21受容体/γc-Fcのいずれか)のいずれかと混合した。混合した後、このプレートもまた、加湿した37℃組織培養インキュベーターに移した。30分後、試験溶液を細胞プレートに移し、混合した。次いで、このプレートをインキュベーターに戻して24時間置いた。24時間後、細胞をインキュベーターから取り出し、室温まで放冷させた。次いで、各ウェルを体積100μLのSteady-Gloルシフェラーゼ試薬(Promega, Madison, WI)で1:1希釈し、完全に混合した。プレートをカバーし、室温で10分間振盪し、ルシフェラーゼ相対単位(RLU)を照度計で測定した。
種間交差反応研究(特に非ヒト霊長類の交差反応性に関する)は、治療的アンタゴニストを開発する場合、前臨床薬理学/毒物学研究の前に完了することが重要である。本明細書において説明する抗ヒトIL-21中和性実体が、カニクイザルIL-21、マウスIL-21、またはラットIL-21と交差反応し、それらによって誘導される活性を中和し得るかどうかを判定する(したがって、カニクイザル、マウス、および/またはラットのいずれかを実用的な試験種として正当と認める)ために、組換えカニクイザルIL-21、マウスIL-21、およびラットIL-21の生物活性を最初に実証する必要があった。実施例8で説明したIL-21 STAT-ルシフェラーゼ活性アッセイ法のための方法を用いて、組換えヒトIL-21、カニクイザルIL-21、マウスIL-21、およびラットIL-21(すべてZymoGenetics社製)のEC50値およびEC90値を決定した。結果から、このアッセイ法においてヒトIL-21、カニクイザルIL-21、およびマウスIL-21によって誘導されるSTAT-ルシフェラーゼ活性のレベルは広範囲に渡って様々であることが示される。後続の実験で使用されるEC90値は、次のとおりであると決定された:ヒトIL-21は961pM;カニクイザルIL-21は102pM;マウスIL-21は6.41nM;およびラットIL-21は1.08nM。IL-21可溶性受容体(hIL-21R/γc-Fc)により、カニクイザル、マウス、およびラットの作用が中和された。
*注意:クローン362.78-CHO(実施例1b)は、他の実験で使用した100pMの代わりに濃度800pMのcIL-21を用いて試験した。N/T=試験未実施。
治療的サイトカインアンタゴニストを開発する場合、構造的に関係のあるサイトカインとそれが交差反応し、中和するかを知ることが重要である。この初代B細胞増殖アッセイ法は、本明細書において説明するIL-21中和性実体をヒトIL-4に対する交差反応および中和に関して試験するために設計した。
11A. 細胞ベースのアッセイ法におけるIL-2およびIL-15に対する潜在的交差反応の評価
治療的サイトカインアンタゴニストを開発する場合、構造的に関係のあるサイトカインとそれが交差反応し、中和するかを判定することが重要である。マウスT細胞株CTLL-2は、ヒトIL-2またはIL-15に応答して増殖するように誘導することができる。したがって、本明細書において説明するIL-21中和性実体をヒトIL-2およびIL-15に対する交差反応および中和に関して試験するためにこのアッセイ法を選択した。
ヒトIL-2、ヒトIL-4、ヒトIL-7、ヒトIL-9、およびヒトIL-15に対する抗IL-21 mAb 362.78-CHOの潜在的交差反応性を表面プラズモン共鳴によって評価した。
初代B細胞アッセイ法
IL-21中和性実体の活性をさらに試験するために、2つの初代B細胞アッセイ法を開発した。B細胞増殖アッセイ法を用いて、4日間に渡るIL-21によって誘導された増殖の中和を実証した。B細胞分化アッセイ法では、8日間に渡るIL-21によって誘導された形質細胞分化の中和が実証された。これらの実験から、長期の生物学的に関連するアッセイ法におけるIL-21の中和が実証された。
*クローン366.552.11.31の場合の実際のアンタゴニスト:リガンドの比が14.4:1、3.6:1、0.9:1、および0.22:1であったことに留意されたい。
DTH応答は、CD4+T細胞によって開始され、かつ、T細胞、好中球、およびマクロファージによって媒介される古典的な免疫応答である。DTH応答は、CD4+T細胞によって媒介される応答の好適な指標である。2種類のアジュバント、すなわち完全フロイントアジュバント(CFA; Sigma)またはRibi(Sigma;MPL+TDM+CWSアジュバントとしても知られる)のいずれかに溶かしたニワトリ卵白アルブミンタンパク質(OVA)を皮下投与して、マウスを免疫化する。この段階は、感作期(0日目〜6日目)と呼ぶ。7日後に耳測定値を測定する。次いで、マウスの耳に対照のPBS(左耳)またはOVA(右耳)を注射する。この段階は、攻撃接種期(7日目〜8日目)と呼ぶ。OVAに対して生じさせた免疫応答により、耳に炎症が誘導され、結果として、OVAで処置した耳は24時間で耳の厚さが増すが、PBSで処置した耳では耳の厚みは増さない。これは、カリパスを用いて測定する。
抗IL-21が多発性硬化症に対して何らかの作用を有するかどうかを試験するために、MSのマウスモデルである実験的自己免疫性脳脊髄炎(EAE-MS)を抗IL-21抗体が抑制する能力を試験する。十分に特徴付けられているC57BL/6マウスのT細胞依存性ミエリン乏突起膠細胞糖タンパク質(MOG)35-55ペプチド免疫化モデルを使用する。この実験は、抗IL-21抗体が、DCを介した抗原提示を阻害することによって、またはCD8 T細胞応答を増強することによってEAEを遅延させるか、かつ/またはその疾患スコアを抑制し得るかを決定するために実施する。このモデルにおいて効率的なCD8 T細胞応答が無いと、EAEが増悪する(Malipiero et. al., Eur. J. Immunol., 27:3151-3160, 1997)。EAEモデルの疾患の発症が用量依存的な様式で遅延することから、抗IL-21抗体の使用がMSにおいて有益であり得ることが示唆される。
非主要組織適合性が不一致なマウスまたは同系の免疫無防備状態マウスにナイーブT細胞を養子移入すると、大腸炎(Leach MW et al 1996, Powrie F et al, 1997)ならびに乾癬に似た皮膚病変(Schon MP et al., Nat Med. 2:183-8, 1997; Davenport CM et al., Int Immunopharmacol. 5:653-72, 2002)が発症する。BALB/CマウスまたはB10.D2マウスに由来するCD4+CD25- T細胞をわずか20万個だけ免疫無防備状態のC.B-17 SCIDマウスに移植すると、体重減少、ヘモカルト陽性の便、および皮膚病変の発症がもたらされる。これらのマウスにおける症状は群によって様々である。
接触過敏症は、ジニトロフルオロベンゼン(DNFB)およびオキサゾロンを含む様々な接触アレルゲンを用いて、マウスにおいて誘発することができる。アセトンおよびオリーブ油からなるビヒクル中のアレルゲンでマウスを局所的に感作し、次いで、オリーブ油のみに溶かしたアレルゲンを耳に攻撃接種する。耳の厚さの変化は、アレルゲンに対する免疫応答の尺度である。感作期(0日目〜5日目)に、または攻撃接種期(5日目〜6日目)の間に抗IL-21抗体を投与する。IL-21に拮抗することによって耳の厚さが抑制されることから、接触過敏症を誘発する際のIL-21の役割が示唆される。
A. 水素-重水素交換(HDx)実験
抗IL-21 mAbである362.78.1.44、362.597.3、366.328.10、366.552.11、および可溶性hIL-21R/γc-Fcを中和することによって認識されるIL-21のエピトープ領域を同定しようとして、イムノアフィニティーに基づいた水素重水素交換(HDx)方法を適用し、続いて質量分析による解析を行った。具体的には、CNBr活性化セファロースビーズ上に精製mAbを固定し、重水素緩衝液に変更した。重水素化 IL-21をインキュベーションによってイムノアフィニティービーズに結合させ、重水素化緩衝液でビーズを洗浄して未結合タンパク質を除去した。次いで、抗原-抗体複合体をPBS溶液に供して、IL-21の未結合領域におけるアミド水素への逆交換(back-exchange)を開始した。次いで、重水素-水素交換を停止し、低pH緩衝液中にIL-21を溶出させ、次いでそれを固定ペプシンによるタンパク質分解消化に供した。次いで、ペプチド質量マップをMALDI-TOF質量分析法を用いて作成し、PBS溶液での希釈によって重水素化IL-21からアミド水素に逆交換された遊離状態のIL-21から作成した対照試料のものと比較した。図3は、遊離状態のIL-21(図3Aおよび3C)および抗体が結合したIL-21(図3Bおよび3D)の両方のペプシン消化ペプチドの質量スペクトルの拡大図を示す。図3Aに示すように、重なった(overlapping)ペプチドアイソトープを、質量精度10 ppm以内の理論上のペプチド質量を用いて、遊離状態のIL-21の対応するペプチドEKKPPKEF(SEQ ID NO: 2の残基129番目〜136番目)(m/z、1002.5619Da)およびLERFKSLL(SEQ ID NO: 2 の137番目〜144番目)(m/z、1005.6091Da)に割り当てた。不完全なアミド水素交換が原因で、少量の残存する重水素化ペプチドがm/z1002.5619Daの付近で観察された。
(SEQ ID NO: 2の残基129番目〜144番目)を含む可能性が高いことが示唆された。
(SEQ ID NO: 2の残基141番目〜162番目)(m/z、2519.2451)と同定された。図3Dに示すように、このアミノ酸残基22個のペプチドの質量シフト(Δ質量=9.0Da)を2種類の上流残基(図2B、EKKPPKEF(SEQ ID NO: 2の残基129番目〜136番目)およびLERFKSLL(SEQ ID NO: 2の残基137番目〜144番目)の質量シフトと比較したところ、この領域はmAbの結合の際にごくわずかしか保護されなかったことから、このペプチドのごく一部のみがmAbへの結合に関与し得ることが示唆された。実際には、このペプチド配列はC末端尾部であり、mAbによって顕著に保護されると思われたペプチド(LERFKSLL (SEQ ID NO: 2の残基137番目〜144番目)と重複するアミノ酸残基4個を含む。重水素保持のこれらの質量分析測定値に基づいて、本発明者らはmAbを結合するIL-21エピトープを
(SEQ ID NO: 2の残基129番目〜144番目)および
(SEQ ID NO: 2の残基141番目〜162番目)の上流配列であると推定した。
HDxアッセイ法を用いて、IL-21 mAb結合エピトープ領域を推定した。推定された結合エピトープ領域にリジン残基5個(IL-21中の合計12個のリジン残基に由来する)が存在することが観察された。リジン残基は帯電が特徴であるのでタンパク質の溶媒接近可能領域に存在する可能性が最も高いため、リジンは、抗原エピトープ領域を並行して決定するために選択的化学修飾を施すのに理想的な選択と思われる。化学修飾戦略の背景にある概念は、抗体の存在下および不在下での抗原のリジン修飾の保護がその結合エピトープと相互に関係するというものである(Scholten et al., J. Amer. Soc. Mass Spectr. 17: 983-994, 2006)。したがって、エピトープ領域をさらに特徴付けするために、およびmAb結合に関与しているリジン残基を決定するために、親和性結合した状態のIL-21および遊離状態のIL-21の両方においてリジン残基の選択的アセチル化を実施した。リジン修飾/保護の部位は、MALDI-TOF質量分析およびエレクトロスプレーイオン化法(ESI)質量分析を用いた全質量解析およびペプチドマッピング解析によって決定した。
(SEQ ID NO: 2の残基119番目〜136番目)(m/z、1986Da)と同定した。一方、非アセチル化ペプチドの質量は1860Da(m/z)である。質量の差(アセチル化ペプチド/非アセチル化ペプチド)は126Daであったことから、遊離状態のIL-21ではこのペプチド中の3つのリジン残基すべてがアセチル化されていることが示された。しかしながら、親和性結合されたIL-21の選択イオンクロマトグラムはペプチドのトレースを示さなかったことから(図4B)、3つのリジン残基はIL-21抗体結合によって完全に保護されていることが示された。
(SEQ ID NO: 2の残基141番目〜162番目)のC末端尾部の上流のリジンが抗体結合によって保護される可能性が最も高いことが示されていた。したがって、K119ではなくK113が抗体結合に関与している可能性が最も高い。
実施例3および9の種交差反応性の結果をさらに良く理解するために、IL-21 mAbが結合したIL-21上の所定のエピトープの視点から(実施例17Aおよび17B)、アミノ酸配列を獲得し、複数の種に渡ってIL-21を比較した(表13)。ヒト配列全体は、カニクイザル配列およびアカゲザル配列との同一性は96%を超えたのに対し、げっ歯動物IL-21配列との同一性は61〜65%に過ぎなかった。特に、実施泥17で説明したクローン362.78.1.44および362.597.3によって結合された不連続なエピトープ(表13では下線を引いた)はヒトIL-21、カニクイザルIL-21、およびアカゲザルIL-21において同一であったのに対し、ラットIL-21配列ではこれらの領域中の残基6個がヒトIL-21とは異なり、マウスIL-21配列では残基7個が異なる。
前臨床イムノアッセイ法および臨床イムノアッセイ法に応用するために、362.78-CHOに特異的な抗イディオタイプmAbを作製した。これにより、この治療的抗IL-21 mAbで治療した個体における潜在的な抗362.78-CHO抗体応答を特異的に測定できる。
捕捉アッセイ法:抗血清中のマウス抗362.78-CHO(Ab2、抗イディオタイプ)抗体の362.78-CHO(Ab1、CHO細胞中で作製、ロット番号E10569)への結合能力を、捕捉スタイルのELISAアッセイ法を用いて評価した。このアッセイ法において、コーティング緩衝液(0.1M Na2CO3、pH9.6)に濃度1μg/mLで溶かしたヤギ抗ヒトIgG Fc特異的抗体(Jackson ImmunoResearch Laboratories, West Grove, PA)をウェル当たり100μL用いて、96ウェルポリスチレンELISAプレートのウェルを最初にコーティングした。プレートを4℃で一晩インキュベートした後、未結合抗体を吸引し、かつ、洗浄緩衝液(0.137M NaCl、0.0022M KCl、0.0067M Na2HPO4、0.0020M KH2PO4、0.05%(v/w)ポリソルベート20、pH 7.2と定義されるPBS-Tween)をウェル当たり300μL用いて、プレートを2回洗浄した。200μL/ウェルのブロッキング緩衝液(PBS-Tweenに加えて1%(w/v)ウシ血清アルブミン(BSA))でウェルを室温(RT)で60分間ブロックし、吸引し、これらのプレートを300μL/ウェルのPBS-Tweenで2回洗浄した。ウェルを濃度1μg/mlの(1%BSA含有PBS-Tween溶液に溶かした)362.78-CHO(Ab1、CHO細胞において作製したZG1、ロット番号E10569)と共にインキュベートした。室温で1時間インキュベーションした後、これらのウェルを吸引し、かつ、前述したようにプレートを2回洗浄した。1:1000の初期希釈率で始まって1:1,000,000に及ぶ(1%BSA含有PBS-Tween溶液に溶かした)抗血清(Ab2)の10倍段階希釈物を調製した。次いで、各希釈物の2つ1組の試料を100μL/ウェルでアッセイプレートに移した。正常マウス血清は陰性対照としての機能を果たした。室温で1時間インキュベーションした後、これらのウェルを吸引し、かつ、前述したようにプレートを2回洗浄した。次いで、HRPを結合させた希釈率1:5000のFc特異的なヤギ抗マウスIgG抗体(Jackson ImmunoResearch Laboratories)を100μL/ウェルでウェルに添加した。室温で1時間インキュベーションした後、未結合の検出抗体をウェルから吸引し、プレートを2回洗浄した。吸引後、100μL/ウェルのテトラメチルベンジジン(TMB)(BioFX Laboratories, Owings Mills, MD)を各ウェルに添加し、プレートを室温で1分間インキュベートした。100μL/ウェルの停止試薬(BioFX Laboratories, Owings Mills, MD)を添加することによって発色を停止し、かつ、ウェルの吸光度の値をMolecular Devices Spectra MAX 340機器によって450nmで読み取った。
ヒト免疫グロブリン遺伝子を発現する、組換えヒトIL-21で免疫化したトランスジェニックマウスにおいて本明細書において説明する中和性抗IL-21モノクローナル抗体を作製した(実施例1を参照されたい)。IL-21 mAbクローン362.78.1.44が組換え型IL-21に加えてネイティブなヒトIL-21に結合し中和できることを確認することが重要であった。さらに、前臨床毒物学研究を支援するために、様々な種のネイティブなIL-21に対するIL-21 mAbクローン362.78.1.44の結合能力を理解することは役立つ。これを試験するための1つのアプローチは、IL-21 mAbクローン362.78.1.44を蛍光色素で標識し、それを用いて活性化T細胞中の細胞内IL-21をフローサイトメトリーによって検出することである。この研究において、新しく単離したヒトおよびカニクイザルの末梢血白血球ならびにラットおよびマウスの脾細胞をインビトロでPMAおよびイオノマイシンで24時間活性化してIL-21産生を誘導した。細胞を回収、固定、透過処理し、ALEXA FLUOR-647(AF-647)で標識したIL-21 mAbクローン362.78.1.44を用いてCD3またはCD4(ヘルパーT細胞集団を明確にするため)およびIL-21の発現に関して染色し、アイソタイプを一致させた対照抗体によって誘導された染色強度と比較した。このアッセイ法の陽性シグナルが対照アイソタイプmAbで観察されたシグナルを上回っていたことから、これはIL-21中和活性の指標ではないものの、試験種の内因性IL-21へのIL-21 mAbクローン362.78.1.44の特異的結合が実証される。試験の結果、IL-21と抗ヒトIL-21 mAbクローン362.78.1.44との結合に関して陽性と判定される種におけるIL-21中和を実証するためには、さらなる研究が必要である(実施例20を参照されたい)。
ヒト免疫グロブリン遺伝子を発現する組換えヒトIL-21で免疫化したトランスジェニックマウスにおいて、本明細書において説明する中和性抗IL-21モノクローナル抗体(IL-21 mAb)を作製した(実施例1を参照されたい)。IL-21 mAbクローン362.78.1.44が組換え型IL-21に加えてネイティブなヒトIL-21に結合し中和できることを確認することが重要であった。ネイティブなIL-21の中和を実証するために、先に説明したBaf3/IL-21R pSTAT細胞ベースのアッセイ法(実施例7を参照されたい)を使用し、活性化CD4+T細胞順化培地試料をネイティブなIL-21の供給源として使用した。この実験では、T細胞順化培地試料を様々な量のIL-21 mAbクローン362.78.1.44と共にプレインキュベートし、次いで、Baf3/hIL-21RトランスフェクタントにおいてIL-21によって誘導されたSTAT3リン酸化(pSTAT3)のレベルを測定した。別々の健常ヒトドナー4名からの活性化T細胞順化培地試料を用いて、ネイティブなIL-21の中和を実証した。
21A. カニクイザルにおけるIL-21 mAb 362.78-CHOを用いた毒性パイロット研究
IL-21 mAb 362.78-CHOのエピトープ特異性はヒト、アカゲザル、およびカニクイザルに共通しており(実施例17および17bを参照されたい)、したがって、安全性評価のために関連種であるカニクイザルにおいてIL-21 mAbの許容性および毒性を試験した。

マウスコラーゲン誘発関節炎(CIA)モデル
当技術分野において公知である、いくつかの関節リウマチ動物モデルがある。例えば、コラーゲン誘発関節炎(CIA)モデルにおいて、マウスはヒトの関節リウマチに非常に似た慢性炎症性関節炎を発症する。CIAはRAと同様な免疫学的特徴および病理学的特徴を共有するため、これは、潜在的なヒト用抗炎症性化合物をスクリーニングするための理想的なモデルとなる。CIAモデルは、その発生が免疫応答および炎症応答の両方に依存する、周知のマウスモデルである。免疫応答は、抗原として与えられるコラーゲンに応答したB細胞およびCD4+T細胞の相互作用を含み、かつ、抗コラーゲン抗体の産生を招く。炎症相は、これらの抗体のいくつかがマウスの生来のコラーゲンと交差反応し、かつ補体カスケードを活性化させた結果としての炎症のメディエーターからの組織応答の結果である。CIAモデルを使用する際の利点は、病因の基本的なメカニズムが公知であることである。II型コラーゲン上の関連するT細胞エピトープおよびB細胞エピトープが同定されており、かつ、免疫介在性関節炎に関する様々な免疫学的パラメーター(例えば、遅延型過敏症および抗コラーゲン抗体)ならびに炎症性パラメーター(例えば、サイトカイン、ケモカイン、およびマトリックス分解酵素)が決定されており、したがって、これらを用いて、CIAモデルにおける試験化合物の有効性を評価することができる(Wooley, Curr. Opin. Rheum. 3:407-20, 1999; Williams et al., Immunol. 89:9784-788, 1992; Myers et al., Life Sci. 61:1861-78, 1997;およびWang et al., Immunol. 92:8955-959, 1995)。ZymoGeneticsによって作製されたラット抗マウスIL-21 mAbの潜在的な有効性を、後述するようにCIAモデルにおいて試験した。
動物は、2回目のコラーゲン注射後すぐに足の炎症の徴候を示し始める場合があり、一部の動物は、2回目のコラーゲン注射の前に足指炎症の徴候を持ち始めることさえあり得る。ほとんどの動物は、追加免疫注射から1〜2週間以内に関節炎を発症するが、一部の動物は、より長い期間を必要とする場合がある。このモデルにおける疾患の発生率は、典型的には90%〜100%であり、動物60匹を用いた研究では、典型的には非応答動物0匹〜5匹(6週間の観察後に判定)が認められる。この研究は、疾患の確立したマウスのみを含んだため、関節炎を発症しないマウスは使用しなかった。炎症が始まると、多様な軽度の足炎症または足指炎症が一般的に一過性に発生し得ることに留意されたい。この理由から、動物は、顕著で持続的な足の腫脹を発症するまでは、確立された疾患を有するとみなさなかった。
0=正常
0.5=1つまたは複数の足指が関与しているが、足指のみが炎症を起こしている
1=足を含む軽度の炎症(1つの領域)。1つまたは複数の足指を含んでよい
2=足における中等度の炎症。足指のうちいくつかおよび/または手首/足首を含んでよい(2つの領域)
3=足、手首/足首、および足指の一部または全体における重度の炎症(3つの領域)。
IL-21Rの発現は造血系由来の細胞に一般に限定されている。しかしながら、炎症性疾患の設定では、非造血細胞におけるIL-21R発現が、罹患組織の機能的変化をもたらす細胞型への直接的刺激を提供し得る。乾癬では、ケラチノサイトの増殖が正常に調節されず、乾癬状の表皮過形成、異常な最終分化、および角質層の不完全発達を伴う。乾癬性皮膚中の浸潤性Th1細胞およびTh17細胞によって産生されるIL-21は、細胞増殖、ケモカイン産生、および分化の変化を含む、ケラチノサイトの機能的変化を促進し得る。したがって、乾癬性皮膚病変中のケラチノサイト上のIL-21Rの存在を調査した。
Claims (62)
- SEQ ID NO: 29のアミノ酸残基20番目〜145番目に対して少なくとも80%の同一性およびSEQ ID NO: 37のアミノ酸残基21番目〜126番目に対して少なくとも80%の同一性を含む、抗ヒトIL-21モノクローナル抗体。
- 少なくとも80%の同一性を有する変化を含むアミノ酸残基が、SEQ ID NO: 31の重鎖可変領域CDR1中に存在する、請求項1記載のモノクローナル抗体。
- アミノ酸変化がいずれも保存的アミノ酸変化である、請求項1記載のモノクローナル抗体。

- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象においてTH1細胞媒介性疾患またはTH17細胞媒介性疾患を治療する方法であって、TH1細胞媒介性疾患またはTH17細胞媒介性疾患が、乾癬、脊椎関節症、反応性関節炎、腸疾患性関節炎、自己免疫性心筋炎、川崎病、セリアック病、ブドウ膜炎、ベーチェット病、冠動脈疾患、慢性閉塞性肺疾患(COPD)、および間質性肺疾患からなる群より選択される、方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象において炎症性腸疾患(IBD)を治療する方法であって、該炎症性腸疾患が、クローン病、潰瘍性大腸炎、および過敏性腸症候群からなる群より選択される、方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象において関節リウマチを治療する方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象において多発性硬化症を治療する方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象においてI型糖尿病(IDDM)を治療する方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象においてシェーグレン症候群を治療する方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって移植対象を処置する方法であって、移植拒絶が抑制されるか、移植前の治療レジメンにおける寛容性が確立されるか、または対象の同種抗体力価が低減される、方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象において自己免疫疾患を治療する方法であって、自己免疫疾患が、膵炎、炎症性筋疾患(多発性筋炎、皮膚筋炎)、顕微鏡的多発性血管炎、自己免疫性再生不良性貧血、自己免疫性甲状腺炎、自己免疫性肝炎、ウェゲナー症候群、憩室症、強直性脊椎炎、強皮症、全身性硬化症、乾癬性関節炎、変形性関節炎、アトピー性皮膚炎、白斑、移植片対宿主病(GVHD)、皮膚T細胞リンパ腫(CTCL)、糸球体腎炎、IgA腎症、感受性の高い移植患者、抗リン脂質症候群、および喘息、ならびに他の自己免疫疾患、またはIL-21アゴニストおよびIL-21受容体アゴニストによって媒介される他の疾患からなる群より選択される、方法。
- 治療的量の請求項1、4、12、20、23、31、39、または48のいずれか一項記載の抗ヒトIL-21モノクローナル抗体を投与することによって対象において全身性エリテマトーデス(SLE)を治療する方法。
