実施例
他に示されない限り、使用される装置および試薬を、本明細書中に以前に列挙された供給源から入手している。
本発明の方法における使用に適切な粉体は、単純な吸入器デバイスからの良好なエアロゾル化を示す特性を有することが必要である。この特性を決定するために、出願人らは、異なる脱凝集化特性を有すると考えられる3つの異なる乾燥粉体を特徴付けした。試験するべき第1の粉体は、Spectrum Labs(Laguna Hills、CA)から入手したサブミクロン硫酸アルブテロール粒子であった。第2および第3の粉体を、賦形剤および生物活性薬剤の組み合わせをエタノール/水溶媒系に溶解し、そしてこの混合物を噴霧乾燥することによって調製した。
プラセボ粒子組成物は、70/20/10% DPPC/クエン酸ナトリウム/塩化カルシウムであった。0.2gのクエン酸ナトリウムおよび0.1gの塩化カルシウムを、0.11Lの水に溶解した。DPPCエタノール溶液を、0.7g DPPC(DL−α−ホスファチジルコリンジパルミトイル、Avanti Polar Lipids、Alabaster、AL)を0.89Lの95%エタノールに溶解することによって調製した。次いで、クエン酸ナトリウム/塩化カルシウム溶液およびDPPC/エタノール溶液を、一緒に混合した。最終の総溶液濃度は、1.0g/L(85%エタノール/15%水中に0.70g/L DPPC、0.2g/Lクエン酸ナトリウムおよび0.1g/L塩化カルシウムで作成した)であった。
Nitro Atomizer Portable Spray Dryer(Niro、Inc.、Columbus、MD)を使用して、乾燥粉体を生成した。可変圧力(1〜5bar)で圧縮された空気が、ドライヤーの上に配置した回転アトマイザー(2,000〜30,000rpm)を動かした。速度を変えた液体供給(20〜66mL/分)を、電気的測定ポンプ(LMI、モデル番号A151−192s、Acton、MA)によって連続的にアトマイザーへポンピングした。入口温度および出口温度の両方を、測定した。入口温度を、手動で制御した;100℃〜400℃の間で変わり得、そして100、110、150、175、または200℃で、5℃の制御限界で安定させた。出口温度を、入口温度ならびに気体供給速度および液体供給速度のような因子によって決定した:50℃〜130℃の間で変化した。容器を、粉体産物を収集するためにサイクロンへきつく取り付けた。

高分散性粉体が効率的に放出し、そして呼吸活性化乾燥粉体吸入器(DPI)の範囲から肺へ浸透し得ることを示すために、出願人らは、クエン酸ナトリウム、DPPC、塩化カルシウム緩衝液および微量のローダミン蛍光標識から構成される噴霧乾燥粉体を調製した。この粉体は、2.1μmのメジアン空気力学的直径(AeroDisperserおよびAerosizerによって測定される)ならびに11.0μmの幾何学的直径(本明細書中に記載されるような上記のRODOS乾燥粉体分散器およびHELOSレーザー回折計を使用して測定される)を有し、そして、実施例1の噴霧乾燥粉体と同様に優れた脱凝集化特性を表した。
ヒト沈着研究を実施して、単純な呼吸作動吸入器から放出される高分散性粉体が、肺への高効率送達(名目上の用量の50%より多い)を生じ得るか否かを決定した。以下の特徴を有する粉体を使用した:Dg=6.7μm;ρ=0.06g/cc;Da=1.6μm。
γシンチグラフィーは、吸入された粒子の沈着のパターンを評価するための確立された方法論である。本実施例において、試験物質を、InAMed laboratories(Gauting、Germany)で低用量の放射性同位体99m Tcを用いて標識する。肺の縁の決定を、81m Kr(クリプトン)換気スキャンを管理することによって高める。呼吸流速をモニターして、深い、十分な吸入を沈着研究の間に実施したことを確実にする。呼吸活性化吸入器を介する深い、十分な吸入に対する最高吸入流速(PIFR)の範囲を、研究を始める前に評価した。特定の範囲以外のPIFRを、繰り返した。
使用したプラセボ粉体は、70/20/10重量% DPPC/クエン酸ナトリウム/塩化カルシウムから構成され、以下の特徴を有した:Dg=6.7μm;ρ=0.06g/cc;Da=1.6μm。一次空気力学的粒子サイズ特徴を、飛行時間(AeroSizer/AeroDisperser)を使用して得、そして幾何学的粒子サイズ特徴を、1および2barで操作したレーザー回折(本明細書に記載されるような、RODOS乾燥粉体分散器およびHELOSレーザー回折計を使用して測定される)を使用して得た。放出された空気力学的粒子サイズ特徴を、2Lの総空気容積について28.3L/分で操作したAndersenカスケードインパクション(重量測定分析)を使用して得た。幾何学的粒子サイズ特徴を、60L/分で操作したレーザー回折(IHA/HELOS、Sympatec、NJ)を使用して得た。
プラセボ粉体を、0.2μmフィルターで塞いだリザーバへ充填した。99m Tc溶液(100mLの脱イオン水に0.5mL 99m Tc含有等張生理食塩水を添加する)を、乾燥チャンバに配置されるPari Jetネブライザー中に充填した。Pari Jetネブライザーを、3分間作動して、1.5mLの99m Tc溶液を霧状にした。99m Tc粒子を、このチャンバ内で乾燥し、そして粉体を含むリザーバへ誘導した。標識チャンバの湿度を制御して、そして30%相対湿度を決して上回らないようにした。
標識された多孔性粒子の吸入を、被験体がγカメラに対して背中を向けて座っている間に、実行した。吸入後、γシンチグラフィ画像を、被験体がカメラの前に背中を向けて真っ直ぐに座っている間に、撮った。吸入時間および呼吸待機期間を記録した。肺のサイズを81Krスキャンによって決定した。被験体は、この放射能ガスを、研究の前または終わりに吸入した。
単純な吸入器からの優れたエアロゾル化特性を有する高分散性粉体と同じ調製物を使用して、単回の吸入において驚くほどの高用量を送達し得ることを示すため、出願人らは、高分散性粉体を調製して、そして予め測定した高用量(50mg)または予め測定した低用量(6mg)のいずれかを得るように粉体を装填した。この粉体の粒子サイズ特徴は、以下のようであった:Dg=10.6μm;ρ=0.11g/cc;Da=3.5μm。
L−ドパを含み、かつ吸入に適した粒子を、以下のように生成した。2.00123g DPPC(Avanti Polar Lipids、ロット番号G160PC−25)を、2.80Lのエタノールヘ添加し、そして溶解するまで攪拌した。0.0817g L−ドパ(Spectrum、ロット0Q0128、Laguna Hills、CA)、0.9135g クエン酸ナトリウム(無水物)(Spectrum ロットNX0195)、および0.5283g 塩化カルシウム(無水物)(Spectrum ロットNT0183)を、1.2Lの水へ添加して、そして溶解した。この水溶液をエタノール溶液へ添加することによって、これらの溶液を合わせて、次いでこの溶液を、溶液が澄明になるまで攪拌した。この製剤の重量%は、約:20% L−ドパ、50% DPPC、20%クエン酸ナトリウム、10%塩化カルシウムであった。
ケトプロフェン/DPPC/マルトデキストリン(maltodextrin)粒子を調製し、そしてインビボで投与した。
以下の実施例の方法および器具を、L-ドパを含み、肺送達に適切である粒子の物理的特徴を決定するために使用した。
L-ドパの血漿レベルの測定を、IV注入、経口胃管栄養法、または肺への吸入の後に行った。カルビドパは、一般に、末梢のデカルボキシラーゼ活性が完全に停止されることを確認するために投与される。本実施例では、L-ドパの投与の1時間前に末梢デカルボキシラーゼインヒビターカルビドパ(200mg/kg)の腹腔内(IP)注射を動物に与えた。ケタミン麻酔下で、動物を3つの群に分けた。第1の動物群では、L-ドパ(2mg)を、1%メチルセルロースおよび1%アスコルビン酸を含有する生理食塩水に懸濁し、経口胃管栄養法により与えた。第2群では、吸入技術を、L-ドパ(20%負荷密度)を負荷したAIR 粒子の肺投与のために使用した。喉頭鏡を用いてラットの喉頭蓋を可視化し、平滑チップ吸入デバイス(PennCentury Insufflation粉体送達デバイス)を気道に挿入した。付属のシリンジからの空気のボーラス(3cc)を用いてデバイスのチャンバーから予め負荷した粉体を動物の肺に送達した。総計10mgの粉体(2mg L- ドパ)を送達した。第3の群において、以前に配置した大腿カニューレを用いてL-ドパ(2mg)のボーラス(2〜3秒)を送達した。血液サンプル(200 μL )を、以下の時間点:L-ドパ投与の0(L-ドパ投与の直前)、2、5、15、30、60、120 および240 分後に大腿カニューレを用いて各動物から回収した。全てのサンプルをHPLCを用いてL-ドパ測定のために処理した。
また、L-ドパを与えたラットの薬力学評価を行った。ラットに、中央前脳束へのニューロトキシン6-OHDAの一側性注射を与えた。次いで、ラットを、標準的なアポモルフィン誘導性回転パラダイムを用いて、首尾よい線条体ドパミン枯渇を評価するためにスクリーニングした。手術の2週間後に開始して、動物をアポモルフィン誘導性回転挙動について3週間の間毎週試験した。この試験のために、動物にアポモルフィン(第1試験については0.25mg/kg および2回の試験の後に0.1mg/kg)のIP注射を与え、円柱状のPlexiglassバケットに配置した。各360 度回転を、30分間計数し、>200 回転/30分を示す動物(12/30 障害ラット)のみを挙動試験に使用した。
以下の実験の目的は、少なくともキャリア粒子および必要に応じて薬剤を含む種々の組成物の相対的なバイオアベイラビリティを試験することである。他に示されない限り、噴霧乾燥粒子を使用する場合、これらを、上の実施例における工程に従って調製した。調製したこの粒子の特徴は、以前に開示した範囲内に含まれる。この製剤を、下の表10に示す。
上記で生成した乾燥粒子製剤(AIR粒子)を、投与のために調製した。サルメテロールなしで噴霧乾燥したF1およびF2の場合のAIR粒子を、カプセルに置いて、そして重量測定した。その後、所望の活性化合物(F1、F2)を、AIR粒子の上に置き、その重量を記録した。詳細には、F−1製剤を、サルメテロールを含まないF1製剤と共に置き、そしてF−2製剤を、サルメテロールを含まないF2製剤と共に置いた。このカプセルの内容物の最終質量は、総計で1.0mgであった。このカプセルを閉じて、内容物を、カプセルを繰り返し転がすことによって混合した。このプロセスは、これらの実験において投与するカプセル内の「ブレンド」を生じた。
モルモットにおける肺機能を評価するために全身体プレチスモグラフィ方法を使用した。麻酔をかけた動物に、気管内吸入によって試験製剤を投与した。この系は、個々のモルモットに霧状化によって得られるメタコリンを用いて経時的に繰り返しチャレンジすることを可能にした。流れパラメータに基づく気道抵抗の計算した測定値、PenH(強化休止)を、メタコリン誘導性気管支収縮からの保護のマーカーとして特異的に使用した。
Pharmacological and Toxicological Methods、39(3):163−168(1998))に記載される。ベースライン肺機能(気道過剰反応)値を、いずれの実験処理の前にも測定した。次いで、生理食塩水およびメタコリンに応じる気道過剰反応を、サルメテロール製剤の投与後の種々の時点(2〜3、16、24および42時間)で評価した。平均PenHを、生理食塩水またはメタコリンを用いたチャレンジ後の4分と9分の間に収集したデータから計算した。各時点でのベースラインPenHの割合を、各実験動物について計算した。その後、同じ製剤を受けた動物からの値を、平均して、各時点での平均群応答(±標準誤差)を決定した。
1つの実験において、実施例12の手順に従った。表11に記載する製剤F−1(0.5)、F−1(1.0)、F−1(2.0)、SX−1(0.5)およびSX−2(1.0)を、動物へ投与した。F−1シリーズの製剤は、サルメテロール、DPPC、クエン酸ナトリウムおよび塩化カルシウムを含む。流れパラメータを使用して、PenH(強化休止または気道抵抗の測定値)を、各動物について計算し、記録した。この動物を観察し、25時間試験した。この結果を、図19に示す。SX製剤は、SereventTM(サルメテロールの市販形態)を含む。サルメテロール含有AIR粒子(表10および11におけるF−1シリーズ)は、サルメテロールを含まないAIR粒子(ブランク粒子またはプラセボ粒子と時々呼ばれる)とブレンドした場合のSerevent含有製剤(表11のSX1(0.5)およびSX2(1.0))と有利に比較する。このF−1製剤は、一般的に、SX製剤より低い気道抵抗を示した。さらに、全てのF−1製剤は、一貫してSX−1(0.5)より低い気道抵抗を示した。投与後約10時間で始まるが、全てのF−1製剤は、SX−1またはSX−2のいずれかと比較した場合に、有意に低い気道抵抗を示し、それを維持した。
別の実験において、実施例12の手順に従って、表11に記載する製剤F−2(0.5)、F−2(1.0)、F−2(2.0)、SX−1(0.5)およびSX−2(1.0)を、動物へ投与した。F−2シリーズの製剤は、サルメテロール、DPPC、DPPE、クエン酸ナトリウムおよび乳糖を含む。流れパラメータを使用して、PenH(強化休止または気道抵抗の測定値)を、各動物について計算し、記録した。この動物を観察し、25時間試験した。この結果を図20に示す。SX製剤は、Serevent(サルメテロールの市販形態)を含む。サルメテロール含有AIR粒子(表10および11におけるF−2シリーズ)は、サルメテロールを含まないAIR粒子(ブランク粒子またはプラセボ粒子と時々呼ばれる)とブレンドした場合のSerevent含有製剤(表11のSX1(0.5)およびSX2(1.0))と有利に比較する。このF−2製剤は、一般的に、SX製剤より低い気道抵抗を示した。また、全てのF−2製剤は、一貫してSX−1(0.5)より低い気道抵抗を示した。
別の実験において、上の手順に従った。表11に記載する製剤F−1(0.5)、F−1(1.0)、F−1(2.0)、Serevent1(0.5)およびSerevent(1.0)を動物へ投与した。Serevent製剤をF−1シリーズと比較した結果(データは示さない)は、SX製剤をF−1シリーズと比較した場合の結果と一致した。重要なことに、この結果は、キャリアとして使用する場合、AIR粒子(ブランクまたはプラセボ)が、乳糖よりも優れていないとしても十分同等に機能することを示す。乳糖は、FDA許可の市販のキャリアである。しかし、乳糖は、深肺へ到着し得ない。実施例3に示すように、AIR粒子は、深肺へ到着し、薬剤の沈着部位へ所望の薬剤(例えば、本実験におけるサルメテロール)を送り届けるかまたは付随し得る。
別の実験において、上の手順に従った。表11に記載する製剤F−2(0.5)、F−2(1.0)、F−2(2.0)、Serevent1(0.5)およびSerevent(1.0)を動物へ投与した。再度、Serevent製剤とF−2シリーズとの比較において観察された結果(データは示さない)は、SX製剤をF−2シリーズと比較した場合の結果と一致した。これらの結果は、上の実施例15で記載した結論を支持する。

Claims (31)
- a)薬剤を含む粒子を提供すること(ここで、該粒子は、0.4g/cm3未満のタップ密度を有し、該粒子の少なくとも75.0%が、空気力学的粒子サイズ分布において6.8μm未満の細かい粒子画分を有する);および
b)粒子の集団を有する容器から、粒子を被験体の気道に投与すること(ここで、粒子は単回の呼吸活性化ステップで少なくとも50%の粒子の集団を肺に送達する)、
を含む、単回の呼吸活性化ステップでの肺系への薬剤の送達方法に使用される粒子であって、送達される粒子の集団の少なくとも50%が4μm未満の平均空気力学的直径を有する、粒子。 - 薬剤が、治療剤、予防剤、診断剤および生物活性剤からなる群より選ばれるものである、請求項1記載の粒子。

- a)薬剤を含む粒子を提供すること(ここで、該粒子は、0.4g/cm3未満のタップ密度を有し、該粒子の少なくとも75.0%が、空気力学的粒子サイズ分布において6.8μm未満の細かい粒子画分を有する);および
