Abstract
Description

本出願は、治療装置と共に使用するための凍結防止剤、および皮下の脂質リッチ細胞から熱を除去するためのシステムおよび方法に関する。
過剰な体脂肪または脂肪組織は、人の外観および運動能力を損なう可能性がある。過剰な脂肪組織は、例えば大腿、臀部、腹部、膝、背中、顔面、腕および他の領域を含む身体の種々の場所に存在し得る。更に、過剰な脂肪組織は、脂肪性浮腫(セリュライト)の美しくない外観を誇張すると思われ、該セリュライトは皮下脂肪が真皮の中に突出するときに形成され、また皮膚が下地構造の繊維状ストランドに結合する場所で窪みを形成する。セリュライトおよび過剰量の脂肪組織は、魅力のないものとみなされることが多い。更に、より大過剰の体脂肪量には顕著な健康上のリスクが付随する可能性がある。従って、過剰の体脂肪を抑制または除去する効果的な方法が必要とされている。
本発明の開示は、熱交換素子および熱伝導性凍結防止剤を用いて、皮下の脂質リッチ細胞を冷却するための装置、システムおよび方法を既述するものである。「皮下組織」の用語は、真皮の下に存在する組織を意味し、主に脂質リッチ細胞または脂肪細胞で構成される皮下脂肪または脂肪組織を含むものである。以下に記載する詳細の幾つかは、関連技術における当業者が、ここに開示された実施形態を製造および使用することを可能にするために充分な仕方で、後述の実施形態を説明するために提供されるものである。しかし、以下に記載する幾つかの詳細および利点は、本発明の一定の実施形態を実施するために必要でないかもしれない。加えて、本発明は、特許請求の範囲内にはあるが、図を参照して詳細に説明されない他の実施形態を含んでもよい。
図1は、患者101における皮下の脂質リッチ細胞から熱を交換するための、本発明の一実施形態に従う治療システム100の斜視図である。該治療システム100は、患者101の腹部領域102、または皮下脂肪もしくは脂肪層の減少が望まれる他の領域に配置される治療装置104を含んでよい。該治療装置104は、例えば機械的ファスナー(例えばベルト105)、接着剤(例えばエポキシ)、吸引(たとえば真空もしくは減圧)、または他の何れかの機構を使用して、患者101に固定されてよい。該治療装置104は、患者101を加熱および/または冷却するように構成されてよい。一定の実施形態において、治療装置104は、とりわけ、氷の形成を防止しながら治療装置104をほぼ水の凍結点(0℃)以下の温度に予備冷却することを可能にするために、非凍結性の凍結防止剤を含んでよい。治療装置104の種々の実施形態については、図7〜図10を参照して、以下で更に詳細に説明する。他の実施形態では、図2および図3を参照して以下でさらに詳細に説明するように、治療システム100は、治療装置104または患者101の皮膚に凍結防止剤を供給するためのカップリング装置(図1には示されていない)を含んでもよい。
図2は、図1の治療システム100に使用するのに適し、且つ本発明の一実施形態に従って構成されたカップリング装置502を示す側面図である。該カップリング装置502は、患者101の治療領域に隣接して配置されてよい。カップリング装置502は、治療装置104の熱交換素子130にカップリング装置502を取外し可能または固定して取り付けるための、アタッチメント構造510を含んでいてよい。図示の実施形態において、該アタッチメント構造510は、張力付加クリップを含んでいる。組立てに際し,該カップリング装置502は、背面部分504を治療装置104に向けて、熱交換素子130上にスナップされてよい。他の実施形態において、該アタッチメント構造510は、ネジ、ピン、ヒンジ、および/または何れか他の適切な取付け器具を含んでよい。
図4は、図1の治療システム100において、また本発明の実施形態に従って実施するのに適した方法を示すフローチャートである。この方法は、治療装置に含まれる熱交換素子に凍結防止剤を適用すること(ブロック19)を含んでいる。一定の実施形態において、凍結防止剤は患者の皮膚に適用され、または該皮膚および熱交換素子の両方に適用される。熱交換素子の温度は望ましい温度に低下されてよい(ブロック12)。熱交換素子の温度が望ましい温度、例えば凡そ水の凍結点(0℃)以下に低下されたら、該熱好感素子は患者の皮膚に隣接して配置されてよい。熱交換素子を患者の皮膚に隣接して配置することにより、ある領域の温度が低下して、該領域における脂質リッチ細胞が選択的に侵される一方、上皮および/または真皮における非脂質リッチ細胞は一般的には影響を受けないこととなる(ブロック16)。一定の実施形態では、熱交換素子が患者の皮膚に隣接して配置されたら、治療装置の温度は必要に応じて更に低下されてよい(ブロック15)。
図6は、図1の治療システム100において且つ本発明の一実施形態に従って実行するのに適した、もう一つの方法を示すフローチャートである。図6の方法80は、別途、または図4および/または図5に示した方法と組み合せて適用されてよい。例えば,凍結防止剤は、患者の皮膚(皮膚を凍結損傷から保護するため)および治療装置の熱交換表面(治療装置を予備冷却するため)の両方に適用してよい。
図1の治療システム100に使用するのに適した凍結防止剤は、患者の生物学的組織を凍結損傷(例えば氷形成による損傷)から保護し得る物質である。凍結防止剤は、濃厚化剤、pH緩衝剤、吸湿剤、表面活性剤、および/または他の添加剤と共に、温度降下剤を含有してよい。該凍結防止剤は、液体(例えば水溶液または非水溶液)、ゲル、ヒドロゲル、またはペーストとして処方されてよい。凍結防止剤は吸湿性、熱伝導性であってよく、理想的には生体適合性である。一定の実施形態において、凍結防止剤は、超音波が該凍結防止剤を通過することを可能にするように、例えばその全体の開示を本明細書の一部として援用するBuchalterの米国特許第4,002,221号およびRichardson等の米国特許第4,459,854号に記載された水ベースのゲルのように、超音波音響的であるように処方されてよい。
図7は、治療システム100に使用するために適した、本発明の一実施形態に従う治療装置104の斜視図である。この実施形態において、治療装置104は、第一の部分129aおよび第二の部分129bを有する支持体128、第一の部分129aに位置する第一の熱交換素子130a、第二の部分129bに位置する第二の熱交換素子130bを含んでいる。該治療装置104は、一般に、手動操作のための手持ち型ユニットとして構成されており、および/または、それはストラップで患者に縛るか、または取外し可能に患者に取り付けるように構成されてよい。第一の熱交換素子130aおよび/または第二の熱交換素子130bは、動作に際して治療領域に圧力を加えるために、支持体128に沿って移動し、および/または回転してこれら熱交換素子を位置決めするように構成されてよい。

図9は、本発明のもう一つの実施形態に従う治療装置104の分解斜視図である。治療装置104は、ハウジング302、少なくとも一部はハウジング302内に配置された冷却アセンブリー308、および冷却アセンブリー308をハウジング302に固定するために構成された保持装置318を含んでよい。治療装置104はまた、図10を参照して以下で更に詳細に説明するように、ハウジング302の中に配置された振動部材を含んでよい。
出願人は、図9に示した治療装置および凍結防止剤を使用して、豚における皮下の脂質リッチ細胞を冷却する実験を行った。この実験で使用した第一の凍結防止剤組成物は、約30%のポリプロピレングリコール、約30%のグリセリン、および約40%のエタノールを含んでいた(凍結防止剤I)。また、この実験で使用した第二の凍結防止剤組成物は、約40%のプロピレングリコール、約0.8%のヒドロキシエチルセルロース、および約59.2%の水を含んでいた(凍結防止剤II)。研究した皮膚表面の温度には、−11℃、−12℃、−14℃、−16℃、および−20℃が含まれる。

を含んでなるシステム。 - 請求項1に記載の冷却システムであって、前記凍結防止剤が更に、凍結防止剤におけるpHを約6〜約7.5の範囲に維持するためのpH緩衝剤を含んでなるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤がポリプロピレングリコールを含有するシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤が水溶性であるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤がヒドロゲルであるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤が更に、約1cP〜約10,000cPの範囲の凍結防止剤の粘度を与えるための濃厚化剤を含んでなるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤が更に、吸湿剤を含んでなるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤の凝固点が約−10℃以下であるシステム。
- 請求項1に記載の冷却システムであって、更に、前記凍結防止剤を含有するカップリング装置を含んでなり、該カップリング装置は、前記治療装置に取外し可能に結合され且つ前記熱交換表面に近接しているシステム。
- 請求項9に記載の冷却システムであって、前記カップリング装置が、前記熱交換表面に近接した背面部分、前記背面部分から離間した前面部分、および前記凍結防止剤を保持するための前記背面部分と前記前面部分の間の中間部分を含んでいるシステム。
- 請求項10に記載の冷却システムであって、前記前面部分が、前記中間部分と液体流通した少なくとも一つのアパーチャを含んでいるシステム。
- 請求項10に記載の冷却システムであって、前記中間部分が、メッシュ材料、フォーム材料、多孔質プラスチック材料および/または多孔質金属材料で構成されるシステム。
- 請求項10に記載の冷却システムであって、更に、前記中間部分が、メッシュ材料、発泡体材料、多孔質プラスチック材料、および/または多孔質金属材料で構成されるシステム。
- 請求項10に記載の冷却システムであって、前記中間部分が、治療の前に熱伝導性の凍結防止剤を含んでいるシステム。
- 請求項1に記載の冷却システムであって、更に、前記凍結防止剤を予め充填した吸収材を含んでなり、該吸収材は、少なくとも部分的には前記熱交換表面と標的領域の間にあるシステム。
- 請求項1に記載の冷却システムであって、前記凍結防止剤が、プロピレングリコール、グリコール、ポリエチレングリコール、エチレングリコール、ジメチルスルホキシド、ポリビニルピリジン、酢酸カルシウムマグネシウム、酢酸ナトリウム、エタノール、プロパノール、および蟻酸ナトリウムの少なくとも一つを含んでいるシステム。
- 皮膚を有する患者の皮下の脂質リッチ細胞から熱を除去するためのシステムにおいて:
ハウジングと、熱交換表面との間で熱的に導通した熱的質量とを有する治療装置であって、前記熱的質量は、前記領域における脂質リッチ細胞が侵される一方、前記熱交換表面に近接した非脂質リッチ細胞を保存するように、前記皮膚の領域の温度を低下させるために構成されている治療装置と;
前記治療装置と前記皮膚との間の界面を実質的に覆うように構成された吸湿性の凍結防止剤であって、前記治療装置および前記皮膚の少なくとも一方に接触するように構成された凍結防止剤と
を含んでなるシステム。 - 請求項17に記載の冷却システムであって、更に、前記治療装置に取外し可能に結合されたカップリング装置を含んでなり、該カップリング装置は、前記熱交換表面に近接した背面部分、前記背面部分から離間した前面部分、および前記凍結防止剤を保持するために前記背面部分と前記前面部分の間の中間部分を含んでいるシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤が更に、pHを約3〜11の範囲に維持するためのpH緩衝剤を含んでなるシステム。
- 請求項19に記載の冷却システムであって、前記pH緩衝剤は、pHを約6〜約7.5の範囲に維持するシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤がポリプロピレングリコールを含有するシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤が水溶性であるシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤がヒドロゲルであるシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤が更に、約1cP〜約10,000cPの範囲の凍結防止剤の粘度を与えるための濃厚化剤を含んでなるシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤が更に吸湿剤を含んでなるシステム。
- 請求項17に記載の冷却システムであって、前記凍結防止剤が約−10℃以下の凝固点を有するシステム。
- 患者の皮膚に、熱交換素子および該熱交換素子と熱的に導通して熱交換表面に適用される凍結防止剤を有する治療装置を適用する方法であって:
(i)前記凍結防止剤を、前記治療装置と前記皮膚の間の界面に適用することと;
(ii)前記熱交換素子の温度を治療温度にまで低下させることと;
(iii)治療領域において、前記皮膚に対して前記熱交換素子を配置することと;
(iv)前記領域における脂質リッチ細胞が侵される一方、上皮における非脂質リッチ細胞を保存するように、前記領域において望ましい温度を達成することと
を含んでなる方法。 - 請求項27に記載の方法であって、更に、前記凍結防止剤を適用することは、前記凍結防止剤を前記熱交換表面に適用することを含み、また前記方法は更に、前記熱交換素子の温度を前記治療温度に低下させるときに、前記熱交換表面における氷形成を防止することを含む方法。
- 請求項27に記載の方法であって、更に、前記凍結防止剤を適用することは、前記凍結防止剤を皮膚に適用することを含み、また前記方法は更に、前記患者の皮膚の凝固点を低下させることを含む方法。
- 請求項29に記載の方法であって、更に、前記凍結防止剤を前記患者の皮膚に連続的に供給すること、および前記患者の皮膚において前記凍結防止剤の充分な濃度を維持することを含んでなる方法。
- 請求項29に記載の方法であって、前記凍結防止剤を適用することが、前記熱交換表面に近接した背面部分、該背面部分から離間した前面部分、および凍結防止剤を保持するための前記背面部分と前記前面部分の間の中間部分を有するカップリング装置を使用して、前記凍結防止剤を適用することを含む方法。
- 請求項31に記載の方法であって、更に、前記中間部分において一定の凍結防止剤圧力を維持することを含んでなる方法。
- 請求項31に記載の方法であって、更に、治療の前に、凍結防止剤を前記中間部分に予め充填することを含んでなる方法。
- 請求項27に記載の方法であって、前記凍結防止剤を適用することが、プロピレングリコール、グリコール、ポリエチレングリコール、エチレングリコール、ジメチルスルホキシド、ポリビニルピリジン、酢酸カルシウムマグネシウム、酢酸ナトリウム、エタノール、プロパノール、および蟻酸ナトリウムの少なくとも一つを含んだ凍結防止剤を適用することを含む方法。
- 皮膚を有する患者の皮下の脂質リッチ細胞から熱を除去するためのキットにおいて:
熱交換表面を備えた熱交換素子を有する治療装置であって、前記熱交換素子は、標的領域における脂質リッチ細胞を侵す一方、前記熱交換表面に近接した非脂質リッチ細胞を保存するように、前記領域の温度を低下させるために構成されている治療装置と;
約−40℃〜約0℃の領域に凝固点を備え、且つ前記熱交換表面と前記患者の皮膚の間の界面に適用されるように構成された凍結防止剤と;
(i)前記凍結防止剤を前記標的領域および/または前記治療装置の熱交換表面に適用する方法、および(ii)前記標的領域における脂質リッチ細胞を侵す一方、前記熱交換表面に近接した非脂質リッチ細胞を保存するように、前記領域の温度を低下させる方法に関する情報を含んだ説明文書と
を含んでなるキット。 - 請求項35に記載のキットであって、前記凍結防止剤が、ポリプロピレングリコール、ポリエチレングリコール、プロピレングリコール、エチレングリコール、グリセリン、エタノール、プロパノール、イソプロパノール、およびブタノールの少なくとも一つを含有するキット。
- 請求項35に記載のキットであって、更に、前記凍結防止剤を含有するように構成されたカップリング装置を含んでなり、該カップリング装置は、取外し可能に前記治療装置に結合可能であるキット。
- 請求項37に記載のキットであって、前記カップリング装置は、前記熱交換表面に近接した背面部分、前記背面部分から離間した前面部分、および前記凍結防止剤を保持するための前記背面部分と前記前面部分の間の中間部分を含んでいるキット。
- 請求項37に記載のキットであって、前記前面部分が、前記中間部分と液体流通した少なくとも一つのアパーチャを含んでいるキット。
- 請求項35に記載の冷却システムであって、更に、前記凍結防止剤を予め充填した吸収材を含んでなるシステム。
