Claims (33)
- 循環系を有する生きている対象に結合される神経刺激システムであって、

それぞれが所定のタイプのタイミング基準事象を含む心臓周期を示す基準信号を感知する埋込み可能な基準信号センサであって、前記循環系の外部に置かれるように構成された埋込み可能な基準信号センサと、
前記埋込み可能な基準信号センサに結合された基準事象検出回路であって、前記所定のタイプのタイミング基準事象を検出するように適合された基準事象検出回路と、
前記刺激出力回路と前記基準事象検出回路とに結合された刺激制御回路であって、前記刺激制御回路は、前記神経刺激パルスの前記送出を制御するように適合され、前記神経刺激パルスの前記送出を前記所定のタイプのタイミング基準事象に同期化するように適合された同期化モジュールを含む、刺激制御回路と
を含む神経刺激システム。 - 少なくとも前記刺激出力回路、前記基準事象検出回路、前記刺激制御回路を含むように適合された埋込み可能なハウジングをさらに含む請求項1に記載の神経刺激システム。
- 前記基準信号は皮下心電図(ECG)信号を含み、前記所定のタイプのタイミング基準事象は所定のタイプの心臓事象を含み、前記埋込み可能な基準信号センサは皮下ECG信号を感知するように適合された1つまたは複数の皮下電極を含み、前記基準事象検出回路は前記皮下ECG信号から所定のタイプの心臓事象を検出するように適合された心臓事象検出回路を含む前記請求項のいずれかに記載の神経刺激システム。
- 前記心臓事象検出回路は、心室脱分極(R波)を検出するR波検出器を含む請求項3に記載の神経刺激システム。
- 前記心臓事象検出回路は、心房性脱分極(P波)を検出するP波検出器を含む請求項3に記載の神経刺激システム。
- 前記P波検出器は、複数の心臓周期にわたって前記皮下ECG信号の平均をとる信号平均化回路を含む請求項5に記載の神経刺激システム。
- 前記1つまたは複数の皮下電極は、前記埋込み可能なハウジングに組み込まれる請求項3から6のいずれかに記載の神経刺激システム。
- 前記皮下ECG信号から不整脈を検出する不整脈検出回路をさらに含み、前記刺激制御回路は、前記不整脈が検出されるときに、前記神経刺激パルスの前記送出を抑制するか調整するように適合される請求項3から7のいずれかに記載の神経刺激システム。
- 前記皮下ECG信号から1つまたは複数の心臓パラメータを測定する心臓パラメータ測定回路をさらに含み、前記刺激制御回路は、前記測定された1つまたは複数の心臓パラメータに基づいて前記神経刺激パルスの前記送出を調整するように適合される請求項3から8のいずれかに記載の神経刺激システム。
- 前記基準信号は、心音を示す音響信号を含み、前記所定のタイプのタイミング基準事象は所定のタイプの心音を含み、前記埋込み可能な基準信号センサは前記音響信号を感知する音響センサを含み、前記基準事象検出回路は、前記音響信号を使用して所定のタイプの心音を検出するように適合された心音検出回路を含む請求項1および2のいずれかに記載の神経刺激システム。
- 前記音響センサは埋込み可能な加速度計を含む請求項10に記載の神経刺激システム。
- 前記心音検出回路は、第一心音(S1)を検出するS1検出器、第二心音(S2)を検出するS2検出器、第三心音(S3)を検出するS3検出器、第四心音(S4)を検出するS4検出器のうちの1つまたは複数を含む請求項10、11のいずれかに記載の神経刺激システム。
- 前記基準信号は、血液動態機能を示す血液動態信号を含み、前記所定のタイプのタイミング基準事象は所定のタイプの血液動態事象を含み、前記埋込み可能な基準信号センサは前記血液動態信号を感知する血液動態センサを含み、前記基準事象検出回路は、前記血液動態信号を使用して所定のタイプの血液動態事象を検出するように適合された血液動態事象検出回路を含む請求項1および2のいずれかに記載の神経刺激システム。
- 前記血液動態事象検出回路に結合された導関数計算機をさらに含み、前記導関数計算機は、前記血液動態信号の時間導関数を計算することによって導関数血液動態信号を作るように適合され、前記血液動態事象検出回路は、前記導関数血液動態信号から前記所定のタイプの血液動態事象を検出するように適合される請求項13に記載の神経刺激システム。
- 前記基準事象検出回路は、前記血液動態信号内の所定のタイプのピークを検出するピーク検出器を含む請求項13に記載の神経刺激システム。
- 前記血液動態センサは、血流量を示す血液動態信号を感知するドップラ心エコー・センサを含む請求項13から15のいずれかに記載の神経刺激システム。
- 前記血液動態センサは、血圧を示す圧力信号を感知する圧力センサを含む請求項13から15のいずれかに記載の神経刺激システム。
- 前記血液動態センサは、血流量を示すインピーダンス信号を感知するインピーダンス・センサを含む請求項13から15のいずれかに記載の神経刺激システム。
- 前記基準事象検出回路は、前記心臓周期のそれぞれについて前記基準信号から前記所定のタイプのタイミング基準事象を抽出するように適合された信号プロセッサを含む前記請求項のいずれかに記載の神経刺激システム。
- 前記同期化モジュールは、前記心臓周期のそれぞれについて前記神経刺激パルスの前記送出を前記所定のタイプのタイミング基準事象に連続的に同期化するように適合された連続的同期化モジュールを含む請求項19に記載の神経刺激システム。
- 前記基準事象検出回路は、複数の心臓周期に関連する前記基準信号のセグメントから前記所定のタイプのタイミング基準事象のそれぞれを抽出するように適合された信号プロセッサを含む請求項1から18のいずれかに記載の神経刺激システム。
- 前記同期化モジュールは、所定の個数の前記心臓周期のうちの心臓周期について前記神経刺激パルスの前記送出を周期的に同期化するように適合された周期的同期化モジュールを含む前記請求項のいずれかに記載の神経刺激システム。
- 前記刺激制御回路は、前記所定のタイプのタイミング基準事象のうちの1つと共に始まるオフセット間隔を作るように適合されたオフセット間隔ジェネレータと、前記オフセット間隔が満了するときに複数の神経刺激パルスのバーストの送出を開始するように適合されたパルス送出コントローラとを含む前記請求項のいずれかに記載の神経刺激システム。
- 循環系を有する生きている対象に結合される神経刺激システムを動作させる方法であって、
前記循環系の外部に置かれた埋込み可能な基準信号センサを使用して、それぞれが所定のタイプのタイミング基準事象を含む心臓周期を示す基準信号を感知することと、
前記基準信号から前記所定のタイプのタイミング基準事象を検出することと、
神経刺激パルスの送出を前記検出された所定のタイプのタイミング基準事象に同期化することと
を含む方法。 - 前記基準信号を感知することは、埋込み可能な電極を使用して皮下ECG信号を感知することを含む請求項24に記載の方法。
- 前記基準信号から前記所定のタイプのタイミング基準事象を検出することは、前記皮下ECG信号から心房性脱分極(P波)を検出することを含む請求項25に記載の方法。
- 前記基準信号を感知することは心音を示す音響信号を感知することを含み、前記基準信号から前記所定のタイプのタイミング基準事象を検出することは、前記音響信号から所定のタイプの心音を検出することを含む請求項24に記載の方法。
- 前記基準信号を感知することは、血流量または血圧を示す血液動態信号を感知することを含む請求項24に記載の方法。
- 前記基準信号から前記所定のタイプのタイミング基準事象を検出することは、前記血液動態信号から所定のタイプのピークを検出することを含む請求項28に記載の方法。
- 前記検出されたタイミング基準事象のうちの1つと共にオフセット間隔を開始することをさらに含み、神経刺激パルスの前記送出を前記検出されたタイミング基準事象に同期化することは、前記オフセット間隔が満了するときに、複数の神経刺激パルスのバーストの送出を開始することを含む請求項24から29のいずれかに記載の方法。
- 神経刺激パルスの前記送出を同期化することは、前記神経刺激パルスの前記送出を連続する心音の前記タイミング基準事象に連続的な基礎で同期化することを含む請求項24から30のいずれかに記載の方法。
- 神経刺激パルスの前記送出を同期化することは、前記神経刺激パルスの前記送出を選択された心音の前記タイミング基準事象に周期的な基礎で同期化することを含む請求項24から30のいずれかに記載の方法。
- 前記基準信号から前記タイミング基準事象を検出することは、複数の心臓周期中に記録された前記基準信号のセグメントから前記タイミング基準事象のそれぞれを抽出することを含む請求項24から32のいずれかに記載の方法。
