本発明の有利な実施形態と発展形を、従属請求項に記載した。
本発明の本質的な考えは、フラットなスペーサーファブリックからなる創傷用インサートを用いることにある。このスペーサー材は、少なくとも2つの表面層を有し、これら表面層の間にスペーサー糸があって、これらスペーサー糸は弾性バネとして働いて、両表面層を互いに間隔を取る位置に保持する。
スペーサーニットは、椅子やカウチの表面層や、衣類のパッドとして用いられる。
この関連では、織物スペーサーニットの上に患者を寝かせて、褥創(Decubitus)を減少させることが、すでに提案されている(Exogenous Dermatology 2002、1、276〜278ページ)
さらにDE19832634A1からは、鼠径ヘルニアの治療のために、フラットなスペーサーファブリックからなるインプラントを、腹腔と腹膜の間に挿入することが公知である。
このフラットなスペーサーファブリックは、様々に異なる方法で製造することができる。この場合重要なのは特に、弾性バネとして働くスペーサー糸が、十分大きな体積を有する中間スペースを形成することと、スペーサー材が希望の弾性特性を有することである。そのほかにも重要なのは、創傷と接触する表面層が、組織共生的であることと、液体の透過を可能とすることと、創傷組織が根付くのを妨げることである。これらの基準は、様々な方法で満足させることができる。
これらの表面層は、生体適合性ある材料からなるテキスタイル材料層、すなわちこの材料層の厚さが薄く、織り目が細かいため、希望の組織構造が得られる、このようなテキスタイル材料層とすることができる。
生体適合性材料としては、好ましくはポリマープラスチック、特にはポリビニルアルコールが用いられる。
図面に示した実施例を用いて、下記に本発明をさらに詳しく説明する。
第2の使用法を、図4および5を用いて説明する。
表面的な創傷22に、創傷の大きさに応じて切断された創傷用インサート10を載せる。この場合、創傷表面に接触する第1の表面層12は、上記に説明したと同様に生体適合性を有し、適切な組織構造を有するものとして形成されている。創傷22および創傷用インサート10は、気密な密封フィルム26で覆われている。密封フィルム26は、その周縁領域28が、創傷22の外で、皮膚30に密封性をもって接着されている。密封フィルム26は開口部を有し、この開口部を閉じることができる。この開口部は、例えば接着可能な紙片によって、または外部に対して開く逆止弁によって、閉じておくことができる。創傷用インサート10が挿入され、密封フィルム26によって密封された後、創傷用インサート10はフラットに圧縮され、その際、外側から例えば手のひらによって、圧力が密封フィルム26に加えられる。この圧縮の際、密封フィルム26の開口部を開くことによって、または逆止弁によって、創傷用インサート10から空気が排出される。圧縮後は開口部を閉じる。例えば接着し、または逆止弁によって閉じる。次に損傷インサート10は、スペーサー糸16の膨張による圧力の下で創傷表面に密着する。これについては前記に説明した。

創傷22に創傷用インサート10を挿入した後、この創傷を、密封フィルム26を用いて気密に閉じる。バルブ34を取り付けた管32を、密封された状態で、密封フィルム26の下の創傷用インサート10まで導く。バルブ34を開くと管32を経由して吸引が行われ、創傷22の中に負圧が生じる。互いに向かい合う創傷周縁を引き寄せて、創傷用インサート10を、スペーサー糸16の力に抗して圧縮する。この模様を図6に示した。これにより創傷組織の細胞に、吸引力が加えられる。バルブ34を閉じると、圧縮された創傷用インサート10が、互いに向かい合う創傷表面に圧力を加える。これにより創傷組織の細胞に圧力が加えられ、創傷液が搾り出されて、拡大する中間スペース18に吸収される。このとき創傷用インサート10は、向かい合う創傷面に支持される。

Claims (18)
- 創傷表面と接触する創傷用インサートと、創傷面と創傷用インサートとを覆うためのカバーとを有する創傷治療デバイスであって、該創傷用インサート(10)が、フラットなスペーサーファブリックからなり、該スペーサーファブリックは、少なくとも1つの第1の表面層(12)と、第2の表面層(14)と、これらの表面層(12、14)の間の中間スペース(18)とを備え、この場合、少なくとも第1の該表面層(12)が、生体適合性を有する表面と次のような組織構造とを有し、該組織構造は、液体の透過を可能とするが、創傷組織が根付くのを妨げるものであり、そしてまたこの場合、該中間スペース(18)にはスペーサー糸(16)が配置され、該スペーサー糸は弾性バネとして働いて、第1の表面層(12)と第2の表面層(14)とを、互いに間隔を取る位置に保持することを特徴とする、前記デバイス。
- 前記スペーサー糸(16)が、機械的特性に従って選択された材料からなることと、少なくとも第1の表面層(12)が、生体適合性を有する材料からなることとを特徴とする、請求項1に記載のデバイス。
- 少なくとも前記第1の表面層(12)が、生体適合性を有する材料の糸による、厚さが薄くて編み目が小さいテキスタイル層からなり、該表面層に前記のスペーサー糸(16)が編み込まれていることを特徴とする、請求項2に記載のデバイス。
- 少なくとも前記第1の表面層(12)が、生体適合性を有する材料でコーティングされていることを特徴とする、請求項1に記載のデバイス。
- 少なくとも前記第1の表面層(12)が、生体適合性を有する材料の被覆(20)を積層されていることを特徴とする、請求項1に記載のデバイス。
- 少なくとも前記第1の表面層(12)がスペーサー糸(16)で編まれており、被覆(20)の支持層として用いられることを特徴とする、請求項5に記載のデバイス。
- 前記被覆(20)が、孔径の小さい発泡体またはテキスタイル材からなることを特徴とする、請求項5または6に記載のデバイス。
- 前記生体適合性を有する材料が、ポリマープラスチック、特にはポリビニルアルコールであることを特徴とする、請求項1〜7のいずれかに記載のデバイス。
- 前記カバーが包帯材料(24)であることを特徴とする、請求項1〜8のいずれかに記載のデバイス。
- 前記カバーが密封フィルム(26)であり、該密封フィルムは、創傷(22)を囲む皮膚(30)に、密封された状態で固定可能であることを特徴とする、請求項1〜8のいずれかに記載のデバイス。
- 少なくとも1本の管(32)が前記中間スペース(18)に通じ、密封フィルム(26)の下から密封された状態で導き出すことができることを特徴とする、請求項10に記載のデバイス。
- 少なくとも1本の管(32)が、吸引管として負圧源に接続可能であることを特徴とする、請求項11に記載のデバイス。
- 少なくとももう1本の管が、液体の治療剤の供給管として形成されていることを特徴とする、請求項11および12に記載のデバイス。
- 少なくとも1本の管(32)が、吸引および/または必要があれば供給のため、制御されて開放および閉鎖できることを特徴とする、請求項11〜13のいずれかに記載のデバイス。
- 前記制御が指定可能な時間的間隔で行われることを特徴とする、請求項14に記載のデバイス。
- 前記制御が創傷(22)内の圧力に依存して行われることを特徴とする、請求項14に記載のデバイス。
- 前記創傷用インサート(10)の中に、圧力センサーまたは膨張センサーが配置されていることを特徴とする、請求項16に記載のデバイス。
- 前記被覆(20)が創傷に適用されるべき作用物質を含むことを特徴とする、請求項5〜7のいずれかに記載のデバイス。
