Abstract
Description
本願は、2001年4月4日出願の係属中の米国特許出願第09/826,070号(名称:「超音波外科装置用の機能的なバランス非対称性を備えたブレード(Blades with Functional Balance Asymmetries for Use with Ultrasonic Surgical Instruments)」)の一部継続出願であり、2002年9月23日出願の米国仮特許出願第60/412,845号の優先権を主張するものである。これらの特許文献は、言及することを以ってその開示の全てを本明細書の一部とする。
本発明は、組織を切断、凝固、把持、及び/またはブラント切開(blunt-dissecting)するための超音波外科器具に関し、詳細には、外科部位に対する吸引やイリゲーションなどの流体、蒸気、及び/またはデブリの管理を行うためのポートまたは内腔を有する超音波外科器具に関する。本発明は、一実施形態では、特に内視鏡下外科手術に適合されているが、他の医療用途にも用いることができる。
従来技術では、水晶体超音波吸引器の先端部に吸引システムが配置されている場合が多い。これにより、吸引/イリゲーションが可能であるが、水晶体超音波吸引器に吸引されるすなわち排出される流体が超音波ブレードに直接接触するためその流体が加熱されてしまうという問題がある。超音波ブレードと直接接触する流体は、温度を相当低下させて器具の焼灼効果を低下させてしまう。超音波ブレードに直接接触する流体により、利用できる超音波エネルギーが減少する。つまり、このような流体がブレードシステムを弱めるすなわち負荷をかけるため、所望の組織効果(tissue effect)を達成するためにはより大きな電力が必要となる。加えて、ブレードのキャビティを介して移送される組織及び/または流体は、ブレードの節に集積される傾向にある。この集積により、ブレード内に妨害物が形成され、更に流れが悪くなってブレードに負荷がかかり更なる電力損失が起こる。
本発明は、超音波ブレードとの接触が最小になるように配置される1つの内腔または複数の内腔からなる先端側/基端側に移動可能な流体管理システムを有する超音波外科装置を提供する。本発明は、ブレードに対する負荷を最小にしながら、ブレードの組織効果部分に対する流体、デブリ、または蒸気の制御された送達/除去のための手段を提供する。好適な実施形態のブレードは、少なくとも1つの平面で非軸対称であり、長手方向、横方向、及び/またはねじれの運動パターンによって特徴付けられる所定の自然の励起周波数におけるモード形状を有する。ブレードの組織効果部分に沿って少なくとも1つの節点が存在し、ある方向のブレードの最小(約0)振動運動によって画定され、同じ方向の運動が最大である少なくとも1つの腹の位置が組織効果部分(例えば、先端部)に沿って存在する。本発明は、上記した任意の形式(長手方向、横方向、及びねじれ)またはその組合せで振動運動を伝播する、組織効果のために少なくとも1つの運動節点が利用できる軸対称ブレードにも用いることができる。好適な器具は、1または複数の横方向(モードまたは運動)の節に流体管理システムを配置して、軸対称でない超音波外科ブレードのこのような節に集積する傾向にある組織または流体の除去の効率を改善するようにデザインされている。
本発明の新規の特徴は、特に添付の特許性球の範囲に記載する。しかしながら、本発明自の作機構及び動作方法、並びに発明の更なる目的及び利点は、添付の図面を参照しながら以下の詳細な説明を読めばよりよく理解できるであろう。
Claims (20)
- 基端部及び先端部を画定する超音波外科器具であって、
前記外科器具の前記先端部に配置された非対称ブレードと、
前記ブレードの上に重なるように配置された先端部を有する内腔とを含むことを特徴とする超音波外科医具。 - 前記内腔によって、外科部位から流体、デブリ、または蒸気を吸引して除去できることを特徴とする請求項1に記載の超音波外科器具。
- 前記内腔によって、イリゲーション流体を外科部位に供給できることを特徴とする請求項1に記載の超音波外科器具。
- 前記内腔の前記先端部が移動可能であることを特徴とする請求項1に記載の超音波外科器具。
- 前記ブレードが、横方向モードまたはねじれモードと共に長手方向モードで振動するようにデザインされていることを特徴とする請求項1に記載の超音波外科器具。
- 前記内腔が、横方向の節またはねじれの節に近接した位置で固定されていることを特徴とする請求項7に記載の超音波外科器具。
- 前記内腔が、横方向の節またはねじれの節に近接した位置に対して移動可能であることを特徴とする請求項7に記載の超音波外科器具。
- 前記横方向の節またはねじれの節により、吸引によるデブリまたは流体の除去が促進されることを特徴とする請求項6に記載の超音波外科器具。
- 前記内腔が前記ブレードの前記先端部の先端側に移動可能であることを特徴とする請求項1に記載の超音波外科器具。
- 前記内腔が、前記ブレードに対して径方向に移動可能であることを特徴とする請求項1に記載の超音波外科器具。
- 前記外科器具の前記基端部が、吸引機能及び/またはイリゲーション機能の制御部を含むことを特徴とする請求項1に記載の超音波外科器具。
- 前記ブレードが溝を画定していることを特徴とする請求項1に記載の超音波外科器具。
- 前記溝が、吸引除去するためにデブリ及び/または流体を前記内腔に向かって案内することを特徴とする請求項12に記載の超音波外科器具。
- 前記溝が、前記内腔から外科部位にイリゲーション流体を案内することを特徴とする請求項12に記載の超音波外科器具。
- 前記溝が、前記内腔を支持するまたは拘束することを特徴とする請求項12に記載の超音波外科器具。
- 基端部及び先端部を画定する超音波外科器具であって、
前記外科器具の前記先端部に配置された、少なくとも1つの平面において対称なブレードと、
前記ブレードの上に重なるように配置された少なくとも1つの内腔とを含むことを特徴とする超音波外科器具。 - 前記内腔によって、外科部位から流体、デブリ、または蒸気を吸引して除去できることを特徴とする請求項16に記載の超音波外科器具。
