Abstract
【解決手段】 カテーテル26上において同軸に装着された固定部材は、ステントを搬送するために内シャフト又はガイドワイヤルー面に取着された金属若しくは突起物とステントの間でバルーン材料が挟まれるのを防ぐため、バルーン14直下のカテーテルの周上に取着される。この固定装置は、ステントと好適に摩擦嵌合し、搬送径にあるステントをカテーテル上の適位に固定すべく、ステントに圧力を付与する。

Description
本発明は、特に体内脈管の管腔内に膨張拡張型ステントを搬送し、拡張する装置及び方法に関する。より詳細には、本発明は、カテーテルのバルーンと内シャフトとの間にとりわけ配置されるステント固定装置に関する。
ステント及びステント搬送装置は、多数の医療処置及び状況において利用されており、またそのような用途におけるそれらの構造及び機能も広く知られている。ステントは、カテーテルを介して体内脈管の管腔へ搬送される通常は円筒形のプロテーゼであり、一般に縮径した形状で搬送された後、体内脈管の径まで拡張される。拡張した状態において、ステントは、体内脈管を開通非閉塞状態に維持する一方で該脈管の管壁を支持し、補強するものである。
本発明は、血管形成術の施術及び体腔内へのステント搬送に好適なカテーテル装置に関する。一般にステントはプロテーゼ(補綴)装置であり、体腔、例えば血管、あるいは人間又は動物の生体において接近し難い場所内に配置することが可能なものである。ステントプロテーゼの本体は一般に管状に構成され、その径は縮小又は拡大可能なものである。ステントは、狭窄状態にある血管、あるいは、動脈瘤により損傷を受けた血管を体内で支える血管を永久的に拡張することに関して特に有用である。このようなステントは、典型的に、カテーテルを利用して体腔内へ搬送される。通常、そのようなカテーテルはバルーンカテーテルであり、バルーン周上に装着されて搬送されたステントを拡張し、体腔内の選択部位に該ステントを留置するものである。本発明は、ステント搬送を容易にするために、ステントをカテーテルへ脱着可能に装着し固定する改良された装置、具体的にはバルーン内に具備される固定装置を伴う装置の提供を主な目的とするものである。以下に例示される態様は、搬送施術中におけるカテーテルへステントを固定する改良された手段を十分に開示するものである。
本発明は、ステント固定装置、とりわけバルーンとカテーテル内シャフトとの間に配置されるステント固定装置に関する。以下に開示する複数態様の個々の要素は総じて、所望により相互に置換することが可能である。図1乃至4を参照すると、血管形成術用及びステント搬送用のカテーテル装置が総括的に符号10で示され、総括的に符号16で示される遠位端部にバルーン14を具備するバルーンカテーテル12が示されている。図1は、カテーテルの近位端部12a及び遠位端部12bの拡大図である。図2及び3は、遠位端部16の更なる拡大図であり、両図の縮尺は同一である。図示のカテーテル12は、ラピッドエクスチェンジ又はシングルオペレーターカテーテルとして知られる型のものである。しかしながら、他の型のカテーテルを使用することも可能であり、例えばオーバーザワイヤ型及びフィクスドワイヤ型を挙げることができる。バルーン14は、標準的な手段によりカテーテル12に固定される。バルーンは収縮した状態で示されている。ステント18は、バルーン周上に巻縮されて固定されている。ステントは、バルーンが公知方法により拡張されたときに、その径が拡張されるように構成される。図1及び2においては、血管形成施術用に構成されたカテーテルが示されており、図3には、ステント挿入用に構成されたカテーテルが示されている。
図9は、所望により二以上の分割片からなる装着支持体を利用することも可能であることを示すものである。この態様において、装着支持体は内部要素26周上で相互に間隔を置いて連結された三つの支持体30a、30b及び30cからなる。好ましくは図7及び9に係る態様においては、装着支持体はリング様形状又は円筒形状であるが、他の形状も当業者であれば容易に想到しうる。
また、この形状の装着支持体30は、図12で示される態様に類似した二つの分割片からなる内部要素26の間に挿入されてもよい。上記全ての態様において、アコーディオン様折畳式装着支持体を構成する材料は、任意の好適ポリマーとすることができ、例えばポリエチレンを挙げることができる。例えば、約0.002インチの壁厚を有する管を使用することができる。アコーディオン様の折り目又はひだは、加熱されたチャンバー内に間隔を置いて複数配置されたブレードを利用する圧力成形手段により、上記のような管に容易に形成することが可能である。
管要素216は、熱可塑性材料、好ましくはサーリン(Surlyn;商標)、ピーバックス(Pebax)及びウレタン等の低モジュラスポリマーカーらなる。ポリプロピレン、低密度ポリエチレン(LDPE)、高密度ポリエチレン(HDPE)、エチレンビニルアセテート(EVA)、ナイロン、ポリエステル及びポリエチレンテレフタレート(PET)等からなる装置は、型又はコイル内でブロー成形により製造される。管要素は比較的薄壁に押出し成形され、次いで型、コイル又は畝若しくは波形を形成する他の賦形具内でブロー成形される。
本発明の各種態様において使用されるカップ又はスリーブは、ステントの一端又は両端を収容するエラストマー又は硬質材料からなる。本発明の好適態様において、カップはバルーン下に配置される一又は複数のステントカラーと共に使用される。カラーは、リングとして、ステントに隣接して、あるいはステント及び介在バルーンの下側に配置されるか、円筒体として、ステント及び介在バルーンのほぼ全長にわたってその下に配置される。本発明によるステント拘束は、ステントが予め装着される搬送機構において特に、より確実なステント固定を実現するものである。本発明の各種態様において例示されたカップ及びスリーブは、接着剤又は熱融着によりカテーテルに固定されうるものであり、あるいは、摺動型のカップ又はスリーブとして構成されてもよい。カップがカテーテル上を随意に摺動する場合、カップはステントと摩擦嵌合するように、常にカラー上で使用される必要がある。
搬送中、バルーンカテーテルは患者の脈管構造内を進行し、処置されるべき脈管部にステントが隣接するように該構造内に配置される。バルーンはステントをその拡張径まで拡張させるために膨張する。このとき、バルーンの拡張により、スリーブ末端部がステント上から軸沿いに摺動し、その結果ステント端部がカテーテルから解放される。種々のスリーブ436収縮態様が図49乃至52に示されている。これら図面には、バルーン414が完全に拡張した後、収縮したスリーブ436及び438形状が図示されている。図面には遠位側のスリーブ438のみが描かれている。図49には、好適な収縮形状が図示されている。収縮をより容易にするために、スリーブはシリコーン被覆されている。スリーブは位置440、442において、外シャフト424及び内シャフト426と好適に接着しているが、バルーンウエスト44まで延長されて接着されていてもよい。スリーブに予め折り目を設けるか、バルーンウエストに重なる位置までスリーブを接着することにより、収縮形状を制御することができる。拡張状態のバルーン414及び収縮状態のスリーブ436、438を図示した図52には、バルーンに関して45゜の好適円錐角が示されている。ステントが所望の径に達した後、バルーンが収縮し、カテーテルが抜出されて、ステントが所定位置に留置される。
図54乃至56には、本発明に係る他の態様が示されている。具体的には、装着支持体430の数及び位置を変更したものである。これら図面には、内部に装着支持体430を伴う非拡張状態のバルーンが示されている。ここで図示される構造は、基本的には図448に示される構造と同一であり、装着支持体430の数及び位置のみが異なるものである。図54に係る態様において、リング状装着支持体430は単独である。三つのリング状装着支持体430を包含する別の類似態様が図55に示されている。四つのリング状装着支持体430を包含する態様が図56に示されている。

- 脈管内で膨張拡張可能なステントを搬送及び拡張する機構又は装置であって、
近位端及び遠位端を有してなるカテーテルと、
搬送径から拡張径まで膨張拡張可能なステントであって、その搬送径においてカテーテル遠位端付近でカテーテル上に同軸に装着されるように、拡張径から搬送径に縮径されてカテーテルと合致してなるステントと、
ステントより内軸側でカテーテル上に同軸に装着されてなり、流体による拡張圧力を受けてステントを搬送径から拡張径まで拡張させる拡張可能な膨張手段と、当該膨張手段はバルーンであることと、
カテーテル上において同軸に装着されるものであって、拡張部位まで搬送される間、ステントをカテーテル上の適位に維持するために、搬送径にあるステントに固定用圧力を与える、バルーンに支持されたシースと、該シースが、バルーン上のステント装着位置と、ステント装着位置から離れた位置との間で軸方向に可動に配置されることと

両カップ及びカラーが、バルーンが収縮状態にあるときカテーテル上のステント両端部をステント縮径状態に各々保持するように、連携して構成及び配置されてなり、バルーン及びカテーテルが、バルーンを収縮状態から拡張状態まで拡張させてステント両端部を両カップの端部から解放するように連携して構成及び配置されてなる請求項3乃至請求項21のいずれか一項に記載の装置。 - カップによって拘束されたステント端部とは逆側のステント端部において、カラーが、ステントと軸沿いに隣接して配置されてなる請求項22に記載の装置。
