Abstract
Cet implant interépineux (1) est propre à être engagé entre les apophyses épineuses de deux vertèbres et comprend un bloc (10) de matériau déformable élastiquement.Selon l'invention, l'implant comprend, sur ses deux côtés latéraux opposés, c'est-à-dire destinés à se trouver longitudinalement de part et d'autre des apophyses épineuses après implantation, deux parties (11) en un matériau plus rigide que ledit matériau déformable élastiquement, reliées audit bloc (10) de matériau déformable élastiquement.
Description
1 La présente invention concerne un implant interépineux. II est connu par
le document N WO 99/42051, au nom du demandeur, de réaliser un implant interépineux déformable élastiquement, propre à amortir le mouvement postérieur des vertèbres lors de l'extension de la colonne vertébrale.
Cet implant est substantiellement en forme de "H", c'est-à-dire comprend une paire de branches saillantes supérieures et une paire de branches saillantes inférieures, délimitant des évidements respectifs supérieur et inférieur propres à recevoir les apophyses épineuses des deux vertèbres ; il est formé d'un bloc de matériau viscoélastique déformable, notamment en silicone, éventuellement contenu dans une enveloppe en matériau tissé, notamment en polyester. Cet implant donne satisfaction en pratique, constituant une alternative aux autres méthodes de traitement chirurgical de vertèbres, plus invasives, notamment les méthodes d'ostéosynthèse inter-pédiculaire consistant à mettre en place un matériel rigide à vis pédiculaires et tiges de liaison, réunissant les vertèbres traitées. Il apparaît cependant que, dans certains cas, en particulier en cas de patients forts ou présentant une hyperlordose compensatoire locale, l'implant existant présente une capacité d'amortissement insuffisante. La mise en place chez de tels patients de l'implant de plus grande taille existant dans la gamme d'implants disponibles n'est pas une solution satisfaisante puisqu'elle risque de produire une distraction trop importante des vertèbres, conduisant à une surpression notamment antérieure du disque intervertébral. Par ailleurs, il apparaît que le maintien de l'implant en position risque de ne pas être toujours parfaitement assuré, notamment lors des mouvements de torsion longitudinale de la colonne vertébrale ou des mouvements de flexion latérale de celle-ci. Ce risque est partiellement atténué au moyen de ligaments engagés au travers de la partie médiane interépineuse de l'implant et autour des apophyses épineuses, ce qui rend la mise en place de l'implant plus invasive. La présente invention vise à remédier à ces inconvénients.
L'implant concerné est, de manière connue en soi, propre à être engagé entre les apophyses épineuses de deux vertèbres et comprend un bloc en un premier matériau élastiquement déformable. Selon l'invention, l'implant comprend, sur ses deux côtés latéraux opposés, c'est-à-dire destinés à se trouver longitudinalement de part et d'autre des apophyses épineuses après implantation, deux parties en un deuxième matériau 2908035 2 élastiquement déformable, plus rigide que ledit premier matériau, reliées audit bloc. L'inventeur a en effet pu constater que la capacité d'amortissement insuffisante de l'implant existant résultait, dans certaines situations anatomiques 5 cliniques, d'un fluage latéral du matériau de l'implant sous l'effet desdites forces de compression. Dans l'implant selon l'invention, lesdites parties latérales en matériau plus rigide permettent de s'opposer à ce risque de fluage latéral du matériau constituant ledit bloc. De préférence, les deux parties en un matériau plus rigide sont reliées l'une 10 à l'autre au travers de la portion médiane interépineuse de l'implant, par une partie de liaison. Cette partie de liaison permet, en supplément de la rigidité propre desdites parties en matériau plus rigide, de parfaitement s'opposer au fluage latéral précité de l'implant.
15 Ladite partie de liaison est de préférence réalisée dans le même matériau que les deux parties en matériau plus rigide. Ladite partie de liaison peut être tubulaire, afin de permettre le cas échéant le passage d'un ou plusieurs ligaments au travers de l'implant, qui permettent d'assurer un maintien supplémentaire de l'implant par rapport aux vertèbres. La 20 partie de liaison peut également être pleine en section transversale, permettant alors d'augmenter le degré d'amortissement de la partie interépineuse de l'implant. L'implant pourrait présenter une forme parallélépipédique ou cylindrique, auquel cas la résistance de l'implant au fluage latéral est obtenue par la liaison 25 desdites parties en matériau plus rigide au travers de la portion médiane de l'implant. De préférence toutefois, l'implant selon l'invention présente une forme de "H", c'est-à-dire comprend une paire de branches saillantes supérieures et une paire de branches saillantes inférieures, délimitant des évidements respectifs supérieur et inférieur propres à recevoir les apophyses épineuses des deux 30 vertèbres, et lesdites parties en un matériau plus rigide s'étendent au moins sur une partie de la hauteur d'au moins une desdites branches saillantes. Lesdites parties en un matériau plus rigide ont dans ce cas l'avantage de renforcer la rigidité d'une ou plusieurs desdites branches saillantes, et donc d'augmenter la stabilité et par conséquent d'augmenter le degré de maintien en 35 position de l'implant après mise en place, notamment en cas des mouvements 2908035 3 de torsion de la colonne vertébrale ou de flexions latérales de celle-ci. L'inventeur a pu constater que le risque précité de déplacement de l'implant résultait d'une rigidité insuffisante des branches saillantes de l'implant, particulièrement en cas de fluage latéral important du matériau de l'implant.
5 Lesdites parties en un matériau plus rigides selon l'invention permettent de solutionner ce problème. Ces parties en un matériau plus rigide s'étendent de préférence sur l'ensemble de la hauteur desdites branches saillantes. La rigidité dudit deuxième matériau constituant ces parties peut alors être suffisante pour assurer la 10 contention latérale précitée de l'implant à l'égard du fluage du matériau constituant ledit bloc. Ces mêmes parties peuvent également être reliées l'une à l'autre au travers de la portion médiane interépineuse de l'implant, par une partie de liaison. Lesdites parties en un matériau plus rigide sont de préférence liées 15 intimement audit bloc de matériau déformable élastiquement, sur l'ensemble de l'interface d'assemblage de chaque partie en un matériau plus rigide avec ledit bloc. Cette liaison sur l'ensemble de cette interface contribue à s'opposer au fluage latéral de l'implant étant donné que le glissement d'une face d'assemblage par rapport à l'autre est empêché.
20 L'implant peut, dans ce cas, être obtenu par surmoulage sur ledit bloc desdites parties en matériau plus rigide, ou, lorsqu'il y a une liaison desdites parties en matériau plus rigide l'une à l'autre au niveau de la partie médiane interépineuse, par surmoulage dudit bloc sur lesdites parties en matériau plus rigide et sur la partie formant ladite liaison.
25 Lorsque l'implant présente une forme de "H", au moins une desdites parties en matériau plus rigide peut présenter une épaisseur réduite au niveau de la portion médiane interépineuse de l'implant, de manière à permettre une flexion des branches saillantes supérieure et inférieure correspondantes l'une vers l'autre, afin de rendre possible une insertion latérale de l'implant dans l'espace 30 interépineux. Une telle possibilité d'insertion latérale est avantageuse étant donné qu'elle préserve mieux l'intégrité du système ligamentaire qu'une insertion postérieure. Ledit premier matériau peut notamment être un matériau viscoélastique. Ce matériau peut avoir une rigidité variable, augmentant à mesure qu'augmente le 35 degré de compression subie par ce matériau.
2908035 4 Ledit premier matériau peut notamment être un silicone, d'une dureté par exemple de 65 Shore A. Ledit deuxième matériau a une dureté plus élevée que celle du premier matériau. Il peut s'agir par exemple d'un silicone de dureté de l'ordre de 85 5 shore A. L'implant peut être contenu dans une gaine en matériau textile, notamment en polyester. L'invention sera bien comprise, et d'autres caractéristiques et avantages de celle-ci apparaîtront, en référence au dessin schématique annexé, représentant, 10 à titre d'exemples non limitatifs, plusieurs formes de réalisation possibles de l'implant qu'elle concerne. La figure 1 en est une vue en coupe selon la ligne 1-1 de la figure 2, selon une première forme de réalisation ; la figure 2 en est une vue en coupe selon la ligne II-II de la figure 1 ; 15 la figure 3 en est une vue similaire à la figure 1, selon une deuxième forme de réalisation ; la figure 4 en est une vue selon cette deuxième forme de réalisation, dans une configuration d'implantation ; la figure 5 en est une vue similaire à la figure 1, selon une troisième forme 20 de réalisation, et la figure 6 en est une vue similaire à la figure 1, selon une quatrième forme de réalisation. Par simplification, les parties ou éléments d'une forme de réalisation qui se retrouvent de manière identique ou similaire dans une autre forme de réalisation 25 seront identifiés par les mêmes références numériques et ne seront pas à nouveau décrits. Les figures 1 et 2 représentent un implant interépineux 1, propre à être inséré entre les apophyses épineuses de deux vertèbres de manière à pouvoir amortir le mouvement postérieur de ces vertèbres lors de l'extension de la 30 colonne vertébrale. L'implant 1 est substantiellement en forme de "H", c'est-à-dire comprend une partie médiane 2 destinée à être placée entre lesdites apophyses épineuses, une paire de branches saillantes supérieures 3 délimitant un évidement supérieur 4, et une paire de branches saillantes inférieures 5, délimitant un évidement inférieur 6, ces évidements 4 et 6 étant propres à 35 recevoir les apophyses épineuses des deux vertèbres.
2908035 5 Comme le montre la figure 2, les branches 3 et 5 présentent des bords supérieurs antérieurs 3a, 5a tronqués, respectivement inclinés et arrondis, de telle sorte que l'implant 1 puisse être mis en place à la base des apophyses épineuses des vertèbres, au plus près des lames.
5 L'implant 1 comprend également un conduit 7 aménagé au travers de la partie 2, dont les extrémités sont évasées. Ce conduit 7 sert, si nécessaire, à la mise en place de ligaments engagés au travers de lui et autour des apophyses épineuses des deux vertèbres. L'implant 1 peut comprendre une enveloppe en matériau tissé, notamment 10 en polyester, qui le recouvre entièrement. Comme cela est représenté, l'implant 1 est formé de parties en matières différentes, à savoir : - un bloc central 10 en un premier matériau élastiquement déformable, par exemple en un silicone à dureté de 65 Shore A, et 15 - sur ses deux côtés latéraux opposés, c'est-à-dire destinés à se trouver longitudinalement de part et d'autre des apophyses épineuses après implantation, deux parties 11 en un deuxième matériau élastiquement déformable, plus rigide que ledit premier matériau, reliées au bloc 10. Les parties 11 s'étendent sur l'ensemble de la hauteur de l'implant 1 ; elles 20 ont, dans la forme de réalisation montrée sur la figure 1, une épaisseur variant peu mais légèrement plus importante du côté des extrémités libres des branches 3 et 5, et sont liées intimement au bloc 10 sur l'ensemble de l'interface d'assemblage avec ce bloc. Elles peuvent notamment être en silicone de dureté Shore A plus élevée que celle du matériau du bloc 10, par exemple de l'ordre de 25 85 shore A, et être surmoulées sur ce bloc 10. Ces parties latérales 11 plus rigides permettent de s'opposer à un risque de fluage latéral du matériau constituant le bloc 10 sous l'effet de forces de compression élevées susceptibles d'être exercées sur la partie 2 par les apophyses épineuses, et donc de conserver à cette partie 2 une capacité 30 d'amortissement suffisante. Les parties 11 ont également pour résultat de renforcer la rigidité des branches 3 et 5, et donc d'augmenter le degré de maintien en position de l'implant 1 après mise en place, notamment en cas des mouvements de torsion de la colonne vertébrale ou de flexions latérales de celle-ci, et particulièrement 35 en cas de compression importante de la partie 2.
2908035 6 La figure 3 montre que les parties 11 peuvent présenter une portion 11 a d'épaisseur réduite au niveau de la portion médiane interépineuse de l'implant 1, de manière à permettre, comme le montre la figure 4, une flexion l'une vers l'autre des branches 3 et 5 situées sur un même côté de l'implant 1, afin de 5 rendre possible une insertion latérale de l'implant dans l'espace interépineux. Les branches 3 et 5 peuvent être maintenues dans cette position fléchie au moyen d'un capuchon de maintien 15. La figure 5 montre que les parties 11 peuvent également être reliées l'une à l'autre au travers de la partie 2 de l'implant 1. Ce dernier comprend alors un 10 élément constitué par les deux parties 11 et par la partie médiane de liaison 12, sur lequel est surmoulé le bloc 10. Dans ce cas, la partie de liaison 12 est tubulaire, afin de permettre le cas échéant le passage d'un ou plusieurs ligaments au travers d'elle permettant un maintien supplémentaire de l'implant 1 par rapport aux vertèbres.
15 La figure 6 montre que la partie de liaison 12 peut également être pleine en section transversale, permettant alors d'augmenter le degré d'amortissement de la partie interépineuse de l'implant 1. Ainsi que cela apparaît de ce qui précède, l'invention fournit un implant interépineux ayant dans tous les cas une capacité d'amortissement suffisante, en 20 particulier en cas de patients forts ou présentant une hyperlordose compensatoire locale, et dont le maintien en place est renforcé, pouvant rendre inutile la mise en place de ligaments autour des apophyses épineuses des vertèbres traitées. II va de soi que l'invention n'est pas limitée à la forme de réalisation décrite 25 ci-dessus à titre d'exemple mais qu'elle s'étend à toutes les formes de réalisations couvertes par les revendications ci-annexées.
Claims (8)
1 û Implant interépineux (1), propre à être engagé entre les apophyses épineuses de deux vertèbres et comprenant un bloc (10) en un premier matériau élastiquement déformable, caractérisé en ce qu'il comprend, sur ses deux côtés latéraux opposés, c'est-à-dire destinés à se trouver longitudinalement de part et d'autre des apophyses épineuses après implantation, deux parties (11) en un deuxième matériau élastiquement déformable, plus rigide que ledit premier matériau, reliées audit bloc (10).
2 û Implant (1) selon la revendication 1, caractérisé en ce que les deux parties (11) en un matériau plus rigide sont reliées l'une à l'autre au travers de la portion médiane interépineuse de l'implant (1), par une partie de liaison (12).
3 û Implant (1) selon la revendication 2, caractérisé en ce que la partie de liaison (12) est réalisée dans le même matériau que lesdites deux parties (11) en matériau plus rigide.
4 û Implant (1) selon la revendication 2 ou la revendication 3, caractérisé en ce que la partie de liaison (12) est pleine en section transversale.
5 û Implant (1) selon l'une des revendications 1 à 4, caractérisé en ce qu'il présente une forme de "H", c'est-à-dire comprend une paire de branches saillantes supérieures (3) et une paire de branches saillantes inférieures (5), délimitant des évidements respectifs (4, 6) supérieur et inférieur propres à recevoir les apophyses épineuses des deux vertèbres, et en ce que lesdites parties (11) en un matériau plus rigide s'étendent au moins sur une partie de la hauteur d'au moins une desdites branches saillantes (3, 5).
6 û Implant (1) selon la revendication 5, caractérisé en ce que les parties (11) en un matériau plus rigide s'étendent sur l'ensemble de la hauteur desdites branches saillantes (3, 5).
7 û Implant (1) selon la revendication 6, caractérisé en ce que les parties (11) en un matériau plus rigide sont reliées l'une à l'autre au travers de la portion médiane interépineuse de l'implant (1), par une partie de liaison (12).
8 û Implant (1) selon l'une des revendications 1 à 7, caractérisé en ce que lesdites parties (11) en un matériau plus rigide sont liées intimement audit bloc (10) de matériau déformable élastiquement, sur l'ensemble de l'interface d'assemblage de chaque partie (11) en un matériau rigide avec ledit bloc (10). 2908035 8 9 ù Implant (1) selon l'une des revendications 5 à 8, caractérisé en ce qu'au moins une desdites parties (11) en un matériau plus rigide présente une portion (11a) d'épaisseur réduite au niveau de la portion médiane interépineuse de l'implant (1), de manière à permettre une flexion des branches saillantes (3, 5) 5 supérieure et inférieure correspondantes l'une vers l'autre, afin de rendre possible une insertion latérale de l'implant (1) dans l'espace interépineux. 10 ù Implant (1) selon l'une des revendications 1 à 9, caractérisé en ce que ledit bloc (10) et lesdites parties (11) en un matériau plus rigide sont réalisées en silicone. 10 11 ù Implant (1) selon l'une des revendications 1 à 10, caractérisé en ce que ledit bloc (10) et lesdites parties (11) en un matériau plus rigide sont contenus dans une gaine en matériau textile, notamment en polyester.
