Abstract
Invention concernant un implant intervertébral destiné à être placé par voie chirurgicale entre les apophyses épineuses des vertébres et caractérisé en ce qu'il comporte un noyau textile, une coque en titane, quatre appendices de fixation en titane et quatre rubans textiles de fixation sertis dans des aiguilles métalliques.
Description
Le dispositif objet du présent brevet vise à stabiliser les vertébres, en les fixant entre elles deux par deux au moyen d'un implant semi-rigide contraint.
On a proposé pour fixer les vertébres des plaques ou tiges d'ostéosynthèse, en métaux divers (aciers, titane, alliages spéciaux),ces plaques ou ces tiges s'appuyant soit sur des crochets, soit sur des vis pédiculaires.
On a proposé également des procédés de ligamentoplastie vertébrale, soit en reliant des vis pédiculaires par des anneaux en fibres synthétiques, soit en bloquant entre les épineuses vertébrales, des cales en matériaux divers, ces cales étant fixées par des ligament en matière synthétique.
Tous ces procédés appellent des critiques.
Les plaques et tiges sont rigides, de mise en place difficile. La pose du matériel n'est pas exempte de pathologie iatrogène, telles des plaies des racines nerveuses ou de la dure-mère. Leur utilisation s'accompagne obligatoirement d'une greffe osseuse qui allonge encore la durée de l'intervention et la rend plus hémorragique.
Les simples ligaments associés à des cales souples interépineuses sont d'une mise en place imprécise. La fixation du ligament dans les espaces inter-vertébraux est rendu difficile par l'absence de dispositif approprié. La mise en place des cales qui sont distinctes des ligaments entraine des manipulations parfois difficiles et qui peuvent entrainer des problèmes septiques. Les déplacements secondaires du matériel sont fréquents car les cales sont trop déformables et n'ont pas de stabilité primaire géométrique.
Entre les deux procédés, les ligaments appuyés sur des vis pédiculaires ont les désavantages des deux systèmes ; les vis pédiculaires sont de mise en place délicate, nécessitant un abord plus large que les simples ligamentoplasties ou même des contre-incisions et les ligaments peuvent s'user par frottement sur les parties osseuses.
Il nous a donc paru judicieux de proposer une cale interépineuse semi-rigide avec des dispositifs d'amarrage incorporés de mise en place facile.
L'implant, objet du présent brevet est composite en cela que sa structure, comprend d'une part un noyau en tissu synthétique, tressé et enroulé sur lui même et d'autre part une coque en titane enveloppant de toute part le noyau en tissu synthétique. Sa forme trapézoïdale permet d'obtenir une grande précision de mise en place de l'implant et une stabilité primaire suffisante pour éviter un déplacement de l'implant pendant la période post-opératoire précoce. Les processus de cicatrisation assurent une stabilité définitive au cours de la période postopératoire secondaire et tardive.
L'implant est monobloc ce qui évite les manipulations per-opératoires excessives lors de sa mise en place. Il est destiné à être placé avec un ajustage précis entre les apophyses épineuses des deux vertébres adjacentes que l'on se propose de fixer et qui auront été abordées par voie postérieure médiane.
L'implant comprend trois parties principales.
A) Un noyau en textile synthétique, par exemple en polyester.
Ce noyau est formé d'un ruban tressé dont la largeur varie avec la taille de l'implant, ce ruban est replié sur lui même, enroulé et fixé par soudure, de manière à former un bloc trapézoïdal.
B) Une coque en titane mince de la même forme que le noyau en textile, qu'il contient. Elle se présente comme un trapézoïde comportant six faces.
C) Des appendices de fixation (4 métalliques, 4 en textile)

- une petite base postérieure, rectangulaire avec
un côté cranio-caudal, appelé épaisseur de l'implant pouvant varier en fonction de la morphologie vertébrale de 5 mm à 25 mm (de 4 mm en 4 min) un côté latéro-latéral ou transversal mesurant environ 10 min; cette base se poursuit vers les faces latérales de l'implant de manière arrondie, les arêtes vives pouvant en effet traumatiser les tissus mous périvertébraux

- Deux faces latérales égales et symétriques, avec une dimension cranio-caudale variable égale à l'épaisseur de l'implant comme décrit plus haut et trois dimensions antéro-postérieures, grande (20 mm environ), moyenne (17 mm environ), et petite (14 mm environ), permettant de choisir un implant adapté aux différentes morphologies des vertébres ; ces faces latérales se raccordent avec la grande base et la petite base comme vu plus haut.
- Enfin deux faces planes, parallèles et symétriques l'une craniale et l'autre caudale, destinées à être en contact avec les deux épineuses adjacentes ; ces deux faces doivent avoir une surface rugueuse afin d'augmenter l'adhérence aux surfaces osseuses avec lesquelles elles sont en contact ; les autres faces sont lisses pour éviter de traumatiser les parties molles peri et intra-vertébrales.
Sur ce trapézoide qui forme le corps de l'implant sont implantés, à l'union des faces craniale et latérale et des faces caudale et latérale, 4 appendices symétriques en titane se dirigeant perpendiculairement aux faces craniale et caudale de l'implant et parallèlement aux faces latérales, de manière à former, deux par deux, un tenon métallique dans lequel vient se loger l'apophyse épineuse vertébrale correspondante.
Chacun de ces appendices présente une face axiale rugueuse, destinée à être en contact avec l'os de l'apophyse épineuse et une face latérale lisse destinée à être en contact avec les parties molles périvertébrales.
Ces deux tenons métalliques permettent, lors de la mise en place de l'implant, d'obtenir une bonne stabilité primaire.
Enfin, 4 fentes parallèles aux faces craniales et caudales de l'implant sont pratiquées sur les faces latérales de la coque en titane en regard des appendices métalliques de stabilisation.
Par ces fentes font issue quatre rubans d'environ 5 mm de large sur 1 mm d'épaisseur, pour 120 mm de longueur, terminés par une forte aiguille courbe en acier sur laquelle les rubans sont sertis. Ces rubans sont soudés au noyau en textile synthétique.
Ils sont en matière synthétique tressée par exemple du polyester.
Ces rubans permettent la fixation solide de l'implant en passant les aiguilles dans les espaces inter-épineux sus et sous-jacents à l'espace où l'implant est mis en place.
Les rubans sont ensuite mis en traction et suturés entre eux et au tissu fibreux péri-vertébral par quelques points de fil non résorbable.
Du fait de leur disposition, la mise en tension des rubans applique rigoureusement les deux languettes des tenons contre les faces latérales des apophyses épineuses correspondantes.
La mise en place de l'implant est simple. Lors de la chirurgie vertébrale, l'espace interépineux où l'on désire faire l'implantation est préparé au bistouri et à la pince gouge jusqu'à ce que l'un des gabarits correspondant à la taille d'un des 15 implants disponibles puisse entrer à frottement doux dans l'espace interépineux préalablement préparé. On s'assure que l'implant # implant ne traumatise pas les parties molles péri et intravertébrales. On met alors en place l'implant correspondant au gabarit. L'implant sera légèrement plus grand. Sa mise en place devant se faire à frottement dur.
La fixation est alors assurée, d'une part en resserrant les languettes de titane au contact des apophyses épineuses au moyen d'un instrument approprié, d'autre part en fixant par faufilage dans les espaces interépineux sus et sous jacents à l'espace appareillé, les quatre rubans textiles sertis dans des aiguilles.
Claims (4)
1) Implant destiné à être placé entre les apophyses épineuses de deux vertébres adjacentes pour les stabiliser, et caractérisé par une structure composite associant un noyau en tissu synthétique enroulé sur lui-même et une coque en titane de même forme contenant le noyau.
2) Dispositif selon la revendication 1, caractérisé en ce que 2 tenons sont disposés symétriquement de part et d'autre de l'implant, ces tenons étant destinés à se bloquer sur les apophyses épineuse vertébrales et à stabiliser l'implant.
3) Dispositif selon les revendications 1 et 2, comportant 4 rubans textiles sertis dans des aiguilles chirurgicales, les rubans étant eux même solidaires du noyau textile de l'implant, et destinés à compléter la fixation en étant passés dans les espaces interépineux sus et sous-jacents puis suturés sur eux mêmes.
4) Dispositif selon les revendications précédentes, suivant lequel l'implant intervertébral, les appendices métalliques de stabilisation et les ligaments textiles sertis dans des aiguilles, servant à la fixation, forment un tout monobloc pour faciliter la pose chirurgicale de l'implant.
