Abstract
Un procedimiento de captura de células tumorales circulantes de una muestra, que comprende (i) poner en contacto la muestra con un reactivo de afinidad dirigido a seprasa de mamífero, (ii) poner en contacto la muestra con un reactivo de afinidad dirigido a EpCAM de mamífero, y (iii) capturar las células retenidas en las etapas (i) y (ii), capturando así las células tumorales circulantes de la muestra, en el que el reactivo de afinidad dirigido a seprasa y el reactivo de afinidad dirigido a EpCAM se inmovilizan cada uno sobre una superficie de un soporte sólido.
Description
Las células seleccionadas (aquellas que permanecen en la superficie después del lavado con PBS del microchip) se tiñeron directamente usando DAPI (tinción nuclear), anticuerpos marcados con fluoresceína dirigidos contra CD45, anticuerpos anti-citoqueratina (8,18,19) marcados con ficoeritrina (PE) y anticuerpos anti-EpCAM marcados con Cy7 (para el chip seprasa+) o anticuerpos anti-seprasa marcados con Cy7 (para el chip EpCAM+). Después de la tinción
5 y el lavado, se obtuvieron imágenes de los chips en un Zeiss Axiovert 200M utilizando un objetivo de microscopio 40X y una cámara EMCCD. Se encontró que > 95 % de las células seprasa+ no expresaron niveles visibles de EpCAM, como se discernió a partir de la tinción en el chip usando anticuerpos anti-EpCAM marcados con colorante fluorescente Cy7. Las células EpCAM+ también se inmunofenotiparon usando también anticuerpos anti-seprasa marcados con Cy7 con resultados similares garantizados (> 95 % de células EpCAM+ no expresaron seprasa).
10 Los resultados de varias muestras clínicas se muestran en la Tabla 1. En la Tabla 1, la columna «Patología» enumera el estadio de la enfermedad en el momento en que se tomó la muestra. «Ab de afinidad» indica el anticuerpo utilizado para capturar las CTC de la muestra. El número de células se proporciona para 7,5 ml de sangre. La pureza se calcula como una proporción de CTC (seprasa+ o EpCAM+) con respecto al número total de células seleccionadas (WBC
15 (CD45+) + CTC + células de tinción triple -dichas células (CD45+, DAPI+ y CK+)). Las muestras analizadas incluyeron muestras de pacientes metastásicos y de pacientes con enfermedad localizada. Además, se realizó el seguimiento de un solo paciente con enfermedad localizada antes y 3 semanas después de la cirugía (resección del tejido enfermo). La proporción de CTC seprasa+ y CTC EpCAM+ también se muestra como una función de la patología.

25 Determinación de la proporción de CTC seprasa(+) y EpCAM(+) en la sangre periférica de pacientes con diversos cánceres
Los chips microfluídicos para CTC se fabricaron en sustratos de COC. El diseño del chip consistió en una huella de 26,3 x 20,5 mm con canales principales de entrada y salida (20,5 mm de longitud, 400 µm de anchura y 150 µm de
30 profundidad) que conectaban una serie de 50 canales curvilíneos que formaban de manera conjunta el lecho de selección de células. La Figura 1 muestra el diseño del chip de selección de CTC. Cada canal de selección curvilíneo tenía 30,6 mm de longitud, 150 �m de profundidad y 25 �m de anchura. El área de superficie del lecho de selección de CTC era de 596 mm2 (11 mm2/canal) con 45,1 mm2 de dicha área de superficie en los canales principales. El volumen total del chip era de 9,4 �l (138 nl/canal) con un volumen de 2,5 �l para los canales principales.

Los procedimientos descritos en el presente documento también se pueden usar para comparar el número de células
seprasa+ con el de CTC EpCAM+ derivadas de un tumor. La proporción de CTC que expresan seprasa y CTC que
expresan EpCAM se puede usar para determinar el potencial metastásico del tumor. El análisis de las CTC contadas
5 en ambos lechos de afinidad para pacientes individuales que presentaban diferentes enfermedades mostró una
subpoblación de células dominante variable. Los datos se resumen en la Tabla 3. Se observó un número mayor de
CTC Fap+ en PDAC localizado y cáncer colorrectal metastásico, mientras que se observó que las células EpCAM+
eran dominantes en melanoma metastásico y PDAC metastásico. La mediana para la proporción FAPα/EpCAM fue
de 1,21, 1,16, 0,87, 0,69 para PDAC local, cáncer colorrectal metastásico, PDAC metastásico y melanoma 10 metastásico, respectivamente. El recuento de estas dos subpoblaciones y sus proporciones pueden servir como un
marcador que indica el estadio de la enfermedad.

Análisis de expresión de las CTC capturadas con el reactivo de afinidad dirigido a seprasa
20 Usando los procedimientos descritos en el presente documento, las CTC se capturaron, se liberaron, se contaron mediante detección de impedancia (no se realizó tinción) y se recogieron en un microtubo. Las CTC recogidas se centrifugaron y se lisaron usando ~ 20 �l de solución de lisis de un kit Cell-to-CT™ disponible comercialmente (Life Technologies, Grand Island, N.Y.). La tecnología Cells-to-CT™ permite la transcripción inversa de lisados de 10– 105 células sin aislar ni purificar el ARN. La eliminación de la etapa de aislamiento del ARN agiliza y simplifica
25 sustancialmente el análisis de la expresión génica de las células. De forma alternativa, la lisis de las CTC y el aislamiento del ARN se realizaron directamente usando el lecho de captura de CTC. Esto se puede realizar cuando se usa otro conjunto de chips conectados en serie, modificado con FAPα y EpCAM, para contar las células y caracterizar su fenotipo mediante inmunotinción.
30 Después de la lisis, las muestras se trataron con DNasa para eliminar el ADNg residual de la muestra de ARN total (ARNT). Se realizaron reacciones de transcripción inversa tanto positivas como negativas (sin enzima RT) con 10 �l de ARNT de CTC en un volumen total de 50 �l. La reacción de RT se realizó con M-MulV a 37 °C d urante 60 minutos. Se realizó una PCR cuantitativa en tiempo real para evaluar 15 niveles de expresión génica usando un ensayo SYBR Green en un instrumento 7900HT Applied Biosystems equipado con una placa de 384 pocillos. Se recogieron los
35 perfiles de expresión génica de las CTC seprasa+ y EpCAM+ capturadas. Se evaluaron los siguientes genes (es decir, marcadores epiteliales, mesenquimatosos, EMT y CSC (células madre cancerosas)): EpCAM, KRAS, CD133, CD146, KRT19, CD34, GAPDH, CD24, FAP, ACT2, VIM, NANOG, CD44, CXCR4 y TWIST1 (Tabla 4).
