Abstract
ANILLO DE ANULOPLASTIA DISTENSIBLE QUE PUEDE ALARGARSE, IN SITU, MEDIANTE LA APLICACION DE PRESION DILATORIA MEDIANTE UN GLOBO U OTRO APARATO DE DILACION. EL ANILLO DE ANULOPLASTIA DISTENSIBLE DE LA INVENCION ES UTILIZABLE EN PACIENTES PEDIATRICOS CUYO CRECIMIENTO, TRAS LA IMPLANTACION QUIRURGICA DEL NUCLEO, NECESITARA EL CONSIGUIENTE ALARGAMIENTO DEL ANILLO PARA AJUSTAR EL CRECIMIENTO DEL ANILLO. LA INVENCION INCLUYE UN METODO TRANSLUMINAL Y/O TRANSEPTICO PARA EL ALARGAMIENTO POSTERIOR AL IMPLANTE DEL ANILLO DE ANULOPLASTIA MEDIANTE CATETER.
Description
Anilla extensible para anuloplastia.
La presente invención se refiere en general a
dispositivos médicos, y más en particular a una anilla para
anuloplastia utilizable para corregir, mediante cirugía, ciertos
trastornos de las válvulas auriculoventriculares (es decir, mitral
y tricúspide) del corazón humano, como se especifica en el preámbulo
de la reivindicación 1. Tales anillas para anuloplastia se conocen
por las patentes estadounidenses A-4489446 y
A-4602911.
Una alternativa conveniente para muchos pacientes
que sufren trastorno de la(s) válvula(s)
mitral(es) y/o tricúspide(s) del corazón, consiste en
restaurar la válvula (es decir realizar una valvuloplastia) en vez
de sustituirla por otra. Un grupo específico de pacientes que son
normalmente candidatos a tal cirugía son los niños que sufren
defecto septal auriculoventricular congénito (AVSD).
El remodelado de los anillos de la válvulas (es
decir la "anuloplastia") es fundamental para muchos
procedimientos de valvuloplastia reconstructiva. Tal remodelado de
los anillos de las válvulas puede llevarse a cabo implantando una
anilla protésica (es decir, una anilla para "anuloplastia") con
el fin de estabilizar el anillo y corregir o evitar la
insuficiencia valvular que puede producirse debido al defecto o mal
funcionamiento del anillo de la válvula.
El estado de la técnica incluye numerosas anillas
para anuloplastia, por ejemplo las que se describen en las patentes
estadounidenses 4.042.979 (de Angell); 4.290.151 (de Massana);
4.898.446 (de Reed); 4.602.911 (de Ahmadi y otros); 5.061.277 (de
Carpentier y otros); y 5.201.880 (de Wright y otros), y en la
publicación de patente SU197710.
Un problema asociado a las anillas para
anuloplastia del estado de la técnica es que cuando tales anillas
se implantan en niños (pacientes pediátricos con AVSD) el posterior
crecimiento del paciente puede hacer que la anilla para
anuloplastia quede demasiado pequeña para la función que tiene que
desempeñar. Así, puede que sea necesaria una operación posterior
para sustituir la anilla para anuloplastia implantada originalmente
por una anilla más grande adecuada para el tamaño del paciente en
ese momento.
Aunque algunas de las anillas para anuloplastia
del estado de la técnica tienen incorporados medios para ajustar el
tamaño de la anilla en el momento de la implantación, el
solicitante no tiene conocimiento de ninguna anilla para
anuloplastia del estado de la técnica que esté construida y equipada
para ajustar el tamaño, in situ, después de la implantación,
con el fin de acomodar los cambios del tamaño anular debidos al
crecimiento del paciente.
La presente invención proporciona una anilla
extensible para anuloplastia que puede expandirse, in situ,
mediante un aparato de expansión de válvula posicionable
transvascularmente y/o transeptalmente, como se especifica en la
reivindicación 1.
Según una realización actualmente preferida de la
invención, la anilla para anuloplastia puede estar formada por una
pluralidad de segmentos u hojas separados que se aseguran entre si
por deslizamiento o desplazamiento para formar una anilla que tiene
la configuración deseada del anillo de válvula mitral o tricúspide.
Cuando se ejerce una presión dilatadora o hacia fuera sobre la
superficie interna de la anilla, como se puede llevar a cabo
introduciendo un elemento radialmente inflable (por ejemplo un
globo o una jaula de alambre expansible) en un anillo de la válvula
remodelada, tal presión hace que los segmentos y hojas se deslicen o
separen entre sí. Tal deslizamiento o separación de los segmentos u
hojas hace que la anilla se expanda hasta conseguir un tamaño
anular mayor.
Se prefiere que los segmentos individuales u
hojas que forman la anilla incorporen lengüetas y cortes de
fijación u otro dispositivo de colocación adecuado o de fijación
por fricción, para controlar la cantidad de extensión que resulta
de cada aplicación de presión dilatadora y para evitar que los
segmentos u hojas se desprendan o separen entre si de manera
inadvertida.
La anilla se puede cubrir con una cubierta
estirable o extensible para evitar que entre sangre y/o se estanque
la misma en los espacios que hay entre las superficies de
articulación de los segmentos u hojas individuales. Además, una
anilla de sutura estirable o extensible, formada con un material que
puede ser atravesado por una aguja tal como Dacron Mesh (marca
comercial registrada), está montada en la anilla para facilitar la
sutura in situ de la anilla en el momento de la
implantación.
Otros objetos y ventajas de la invención,
quedarán claros para aquellos versados en la materia, durante la
lectura de la siguiente descripción detallada de sus realizaciones
preferidas con referencia a los dibujos adjuntos.
La figura 1, es una vista en perspectiva parcial
de una primera realización de una anilla para anuloplastia
ajustable de la presente invención.
La figura 2, es una vista en perspectiva en corte
parcial de una segunda realización de la anilla para anuloplastia
ajustable de la presente invención.
La figura 3, es una vista en perspectiva reducida
de una tercera realización de la anilla para anuloplastia ajustable
de la presente invención.
La figura 3a, es una vista en perspectiva
aumentada de una parte de la anilla para anuloplastia de la figura
3.
La figura 4, es una vista en corte de un corazón
humano con una anilla para anuloplastia ajustable de la presente
invención implantada en posición mitral, y muestra el modo en el
que el aparato de dilatación (por ejemplo un catéter inflable o
jaula expansible) puede hacerse avanzar a través de un catéter
posicionado transeptalmente y se utiliza para agrandar in situ la
anilla para anuloplastia ajustable.
La siguiente descripción detallada y los dibujos
que se adjuntan, tienen la finalidad de describir y mostrar
algunas realizaciones preferidas de la invención.
Con referencia a los dibujos, las figuras 1 a 3
muestran modos alternativos para construir las anillas ajustables
10, 10a y 10b de la presente invención. Las anillas 10, 10a y 10b
que se muestran en las figuras 1 a 3 tienen normalmente forma de
''D'' que se corresponde con la forma anatómica normal del anillo de
la válvula mitral. Se debe apreciar que, si estas anillas se
utilizan para remodelar la válvula tricúspide, tendrían
generalmente la configuración redonda de la forma anatómica normal
del anillo de válvula tricúspide.
La anilla 10 que se muestra en la figura 1
comprende unos segmentos tubulares primero 12, segundo 14 y tercero
16. Cada segmento 12, 14, 16 está unido a los otros dos segmentos
para formar una estructura anular sustancialmente autónoma. El
primer segmento 12 tiene una configuración tubular con dos extremos
abiertos A y B en los que se insertan los extremos correspondientes
de los segmentos segundo y tercero 14, 16. El segundo segmento 14
tiene un primer extremo redondeado o cerrado C y un segundo extremo
tubular abierto D. El tercer segmento 16 tiene unos extremos
primero y segundo redondeados o cerrados E y F,
respectivamente.
El primer extremo C del segundo segmento 14 está
insertado en el segundo extremo abierto B del primer segmento 12.
Una serie de lengüetas erectas o dientes 18a sobresalen de un lado
del segundo segmento 14 que se inserta en el segundo extremo B del
primer segmento 12. Una serie de aberturas o huecos
correspondientes 20a están formados en la pared lateral del primer
segmento 12. Los dientes individuales 18a se ajustan a presión y
enganchan por fricción en los huecos individuales 20a, como se
muestra.
De manera similar, el primer extremo E del tercer
segmento 16 se inserta en el segundo extremo abierto D del segundo
segmento 14. Una serie de lengüetas erectas o dientes 18b
sobresalen de un lado del tercer segmento 16 que se inserta en el
segundo extremo D del segundo segmento 14. Una serie de aberturas o
huecos correspondientes 20b están formados en la pared lateral del
segundo segmento 14. Los dientes individuales 18b se ajustan a
presión y enganchan por fricción en los huecos individuales 20b,
como se muestra.
Además, el segundo extremo F del tercer segmento
16 se inserta en el primer extremo abierto A del primer segmento
12. Una serie de lengüetas erectas o dientes 18c sobresalen desde
un lado del tercer segmento 16 que se inserta en el primer extremo
A del primer segmento 12. Una serie de aberturas o huecos
correspondientes 20c están formados en la pared lateral del primer
segmento 12. Los dientes individuales 18c se ajustan a presión y
enganchan por fricción en los huecos individuales 20c, como se
muestra.
Los dientes individuales 18 están configurados y
construidos de manera que cuando se aplica una presión dilatadora
suficiente en la superficie interna de la anilla 10, los segmentos
12, 14, 16 se separan, lo que hace que los dientes 18 se salgan de
los huecos 20 dentro de los cuales estaban colocados y se deslicen
y ajusten a presión en los siguientes huecos disponibles de la
serie, aumentando así el tamaño anular de la anilla. Si se aplica
más presión dilatadora, los dientes 18 se mueven hasta los
siguientes huecos disponibles 20 de la serie, produciéndose así un
segundo aumento de tamaño, y así sucesivamente.
La figura 2 muestra una anilla alternativa 10a
que comprende unos segmentos tubulares semianulares primero y
segundo 30, 32, que están unidos entre si por los extremos, como se
muestra, para formar una configuración anular deseada de la anilla
10a. Unas cremalleras 34, 36 se insertan en los extremos abiertos
opuestos de los segmentos tubulares primero y segundo 30, 32. Los
dientes 18 sobresalen lateralmente desde cada cremallera 34, 36
insertada en los extremos yuxtapuestos de los segmentos tubulares
semianulares primero y segundo 30, 32, como se muestra. Unas
aberturas o huecos correspondientes 20 están formados en las
paredes laterales de los elementos tubulares 30, 32. Los dientes
individuales 18 se ajustan a presión y enganchan por fricción en los
huecos individuales 20, como se muestra. Como se ha descrito antes
con respecto a la realización de la figura 1, la aplicación de
presión dilatadora contra la superficie interna de la anilla 10a
hace que los segmentos semitubulares 30, 32 se separen y que los
dientes individuales 18 avancen hasta, y se asienten dentro de los
siguientes huecos disponibles 20, haciendo así que aumente el tamaño
de la anilla 10a una magnitud determinada.
Se debe apreciar que los componentes que forman
la anilla 10 no necesitan tener una configuración tubular como la
que se muestra en las realizaciones de las figuras 1 y 2. De hecho,
como se muestra en la figura 3, la anilla 10b puede comprender una
pluralidad de hojas arqueadas no tubulares 40, 42, 44 solapadas
entre si y que se encuentran dentro de una cubierta externa
extensible 70, como se muestra.
En una realización de la invención, una anilla de
sutura 52, formada con material tal como Dacron Mesh (marca
comercial registrada), está montada alrededor de la periferia de la
anilla 10, 10a, 10b para facilitar la sutura in situ de la anilla
10, 10a, 10b con el fin de rodear el tejido anatómico.
La figura 4 es una ilustración esquemática del
corazón humano con una anilla para anuloplastia ajustable 10 de la
presente invención implantada en posición mitral. Las estructuras
anatómicas y los vasos sanguíneos principales del corazón se han
señalado, en la figura 4, según los signos siguientes:

Según se muestra en la figura 4, un método con el
que se puede ajustar el tamaño de la anilla para anuloplastia 10
consiste en introducir un catéter guía 50, mediante cateterismo por
la vena cava superior de manera que el extremo distal del catéter
atraviese el septo interauricular IS, usando una técnica de
penetración septal conocida, y se introduce en la aurícula izquierda
LA. Después, se hace avanzar un catéter de dilatación de inflable
54, tal como un catéter valvuloplastia del tipo que se puede
adquirir comercialmente, a través del lumen del catéter guía 50, y
se coloca de manera que el balón 60 del catéter inflable 54 queda
dentro del anillo de la válvula mitral MV. Después, se infla el
balón 60, como se muestra, para hacer que la anilla para
anuloplastia ajustable 10 se distienda hasta conseguir una
configuración anular más grande.
En realizaciones tales como las que se han
descrito y mostrado en las figuras 1 a 3, se puede apreciar que el
balón 60 puede hincharse hasta conseguir un diámetro específico que
produce un único aumento incremental (es decir, desde un corte hasta
el siguiente) del sistema mecánico de control de expansión de
dientes y cortes formados en la anilla para anuloplastia 10.
De manera similar, cuando la anilla para
anuloplastia 10 se implanta en la válvula tricúspide TrV, es
deseable hacer avanzar el catéter guía 50 a través de la vena cava
superior SVC hasta un punto donde el extremo distal del catéter
guía 50 está situado dentro de la aurícula derecha RA del corazón.
Después, el catéter de dilatación inflable 54 se hace avanzar
hasta un punto en el que la parte distal del catéter inflable 54 se
extiende a través de la válvula tricúspide TrV. A continuación, el
balón 60 se infla para extender la anilla para anuloplastia de la
presente invención (no se muestra) cuando está implantada dentro de
la válvula tricúspide TrV.
Claims (9)
1. Anilla extensible para anuloplastia (10, 10a,
10b) con una superficie interna que define una abertura anular, que
comprende:
una pluralidad de segmentos biocompatibles (12,
14, 16, 30, 32) que definen la periferia de la anilla;
una primera estructura cooperante (18, 18a, 18b,
18c) formada en un primer segmento; y
una segunda estructura cooperante (20, 20a, 20b,
20c) formada en un segundo segmento,
caracterizada porque dichas estructuras
cooperantes primera y segunda están acopladas para impedir la
contracción radial de la anilla pero permiten la extensión de la
anilla al aplicar una fuerza dilatadora contra su superficie
interna.
2. Anilla extensible para anuloplastia según la
reivindicación 1, en donde dichas estructuras cooperantes primera
(18, 18a, 18b, 18c) y segunda (20, 20a, 20, 20c) consisten en una
estructura de trinquete que permite una extensión radial
incremental.
3. Anilla extensible para anuloplastia según la
reivindicación 1, en donde el primer segmento comprende un extremo
tubular abierto y el segundo segmento comprende un extremo
cerrado;
en donde la primera estructura cooperante
comprende una pluralidad de aberturas (20, 20a, 20b, 20c) separadas
adyacentes al extremo tubular abierto del primer segmento; y
en donde la segunda estructura cooperante
comprende una pluralidad de dientes (18, 18a, 18b, 18c) separados
adyacentes al extremo cerrado del segundo segmento; y
en donde los dientes están configurados para que
se inserten en una o más aberturas.
4. Anilla extensible para anuloplastia según la
reivindicación 3, en donde cada diente (18, 18a, 18b, 18c)
comprende además un borde inclinado en dirección contraria al
extremo cerrado del segundo segmento y un borde sustancialmente
vertical hacia el extremo cerrado del segundo segmento.
5. Anilla extensible para anuloplastia según la
reivindicación 1, en donde los segmentos comprenden una pluralidad
de hojas arqueadas (40, 42, 44, 46).
6. Anilla extensible para anuloplastia según la
reivindicación 5,
en donde la primera estructura cooperante
comprende una pluralidad de aberturas en la primera hoja que están
separadas;
en donde la segunda estructura cooperante
comprende una pluralidad de dientes en una segunda hoja que están
separados; y
en donde los dientes están configurados para
insertarse en una o más de las aberturas.
7. Anilla extensible para anuloplastia según la
reivindicación 1 que comprende además un material suturable (52)
dispuesto en dicha anilla para facilitar la sutura de dicha anilla
.
8. Anilla extensible para anuloplastia según la
reivindicación 7, en donde dicho material suturable (52) consiste en
una malla de tela.
9. Anilla extensible para anuloplastia según la
reivindicación 1 que comprende además una cubierta sustancialmente
impermeable situada alrededor de dicha anilla para evitar que la
sangre se ponga en contacto con dicha anilla.
