Kombination zur Therapie bei benigner Prostatahyperplasie
Gebiet der Erfindung
Diese Erfindung bezieht sich auf eine neuartige Methode zur Behandlung der Symptome bei benigner Prostatahyperplasie. Dabei werden gleichzeitig verwendet: (1) Ein α-1 Adrenoceptor Antagonist in kontrolliert freisetzender Formulierung oder ein 5-α Reduktase — Inhibitor und (2) ein cGMP PDE 5 - Inhibitor in kontrolliert freisetzender Formulierung oder ein cGMP PDE 5 - Inhibitor mit langer Halbwertszeit.
Hintergrund der Erfindung
Zur medikamentösen Therapie der benignen Prostatahyperplasie (BPH) werden zur Zeit möglichst selektive α-1 und α-lA Adrenoceptor Antagonisten eingesetzt. Ihre Wirkung beruht auf einer

Abflusshinderung in der Harnröhre durch eine vergrößerte Prostata symptomatisch verbessert wird. Beispiele entsprechender Verbindungen sind Tamsulosin, Alfuzosin, Doxazosin und
Terazosin. Allerdings ist das Ausmaß der symptomatischen Verbesserung der BPH begrenzt und es wird nach Möglichkeiten gesucht, die Peakflussrate des Harns weiter zu steigern sowie das
Restharnvolumen weiter zu reduzieren.
Eine weitere Möglichkeit stellt die Therapie mit 5-α Reduktase - Inhibitoren, beispielsweise Finasterid oder Dutasterid, dar. Dieses hemmt die Reduktion von Testosteron zu Dihydrotestosteron. Als Folge kommt es nach längerer Therapie zu einer Reduktion des Prostatavolumens und zu einer Verbesserung der Symptome der BPH. Auch bei dieser Therapie ist das Ausmaß der Verbesserungen begrenzt und es ist eine Wirkungssteigerung wünschenswert. Ferner treten unter der Therapie mit 5-α Reduktase - Inhibitoren Nebenwirkungen auf, beispielsweise eine Beeinträchtigung der männlichen Sexualfunktion.
Auch die Verwendung von PDE 5 - Inhibitoren zur Behandlung von Symptomen der benignen Prostatahyperplasie wurde vorgeschlagen (Forssmann et al., US Anmeldung 2003/0199517 Al). Wie im Abschnitt 0027 auf Seite 3 der Anmeldung beschrieben, erfolgt deren orale Applikation jedoch in „üblichen galenischen Zubereitungen wie Tabletten, lackierten Tabletten, Kapseln ....". Diese Form der Therapie hat neben unzureichender Wirksamkeit und zu geringer Wirkungsdauer das Problem der mäßigen Verträglichkeit.
Auch Kombinationen der genannten Medikamentengruppen wurden teilweise beschrieben. So wurde zur Behandlung von Impotenz die Kombination eines selektiven α-adrenergen Rezeptor Antagonisten mit einem cGMP PDE - Inhibitor beschrieben (Wyllie, US 2003/0040514 Al).
Allerdings konnte sich diese Kombination zur Behandlung von Impotenz nicht durchsetzen. Als Hauptursache ist hierfür zu sehen, dass diese Kombination aufgrund von Verträglichkeitsnachteilen als kontraindiziert gilt. So führt das US amerikanische Arzneimittelverzeichnis „Physicians' Desk Reference" beim Präparat Levitra® Tabletten (Vardenafil) unter Contrainidications aus: „Because the co-administration of alpha-blockers and LEVITRA can produce hypotension, LEVITRA is contraindicated in patients taking alpha-blockers." Ferner bezieht sich die Anmeldung US 2003/0040514 Al auf ein Therapieschema, das als „on demand, as opposed to needing to be taken chronically" bezeichnet wird (Absatz 0023 auf Seite 2) und bevorzugt „fast acting" Kombinationen (Absatz 0024 auf Seite 2). Dadurch ist die vorgeschlagene Kombination zur Behandlung anderer Erkrankungen wie benigner Prostatahyperplasie nicht geeignet.
Zusammenfassung der Erfindung
Es wurde nun gefunden, dass eine verbesserte therapeutische Wirkung bei der Behandlung der benignen Prostatahyperplasie erreicht werden kann, wenn
ein α-1 Adrenoceptor Antagonist in kontrolliert freisetzender Formulierung oder ein 5-α Reduktase - Inhibitor
und
ein cGMP PDE 5 - Inhibitor in kontrolliert freisetzender Formulierung oder ein cGMP PDE 5 - Inhibitor mit langer Halbwertszeit
kombiniert werden. Diese Zubereitungen haben den Vorteil deutlich verbesserter Wirksamkeit bei Blasenentleerungsstörungen bei benigner Prostatahyperplasie wie verbessertem Peakharnfluss, geringerer Restharnmenge, verringerter Notwendigkeit zum nächtlichen Wasserlassen, geringerer
Blasenreizung oder imperativem Harndrang. Darüber hinaus besteht auch noch die Möglichkeit im
Vergleich zur jeweiligen Monotherapie die Dosierung jedes der Kombinationspartner eher gering zu halten und somit eine verbesserte Verträglichkeit der Therapie zu erreichen. Die gegenwärtige Lehrmeinung, dass die erfindungsgemäßen Kombinationen kontraindiziert sind, konnte durch die
Formulierung in den erfindungsgemäßen Zubereitungen überwunden werden. Ferner kann mit der erfindungsgemäßen Kombination eines 5-α Reduktase - Inhibitors mit einem cGMP PDE 5 -
Inhibitor in kontrolliert freisetzender Formulierung oder mit einem cGMP PDE 5 — Inhibitor langer
Halbwertszeit der nachteilige Effekt des 5-α Reduktase - Inhibitors auf die männliche Sexualfunktion vermindert werden.
Auch eine Dreierkombination aus selektivem α-1 Adrenoceptor Antagonist in kontrolliert freisetzender Formulierung, 5-α Reduktase - Inhibitoren und PDE 5 - Inhibitor ist möglich,
beispielsweise Tamsulosin in kontrolliert freisetzender Formulierung, Finasterid und Vardenafil in kontrolliert freisetzender Formulierung.
Die Kombination der genannten Verbindungen kann frei erfolgen, beispielsweise durch eine Kombinationspackung, welche Arzneiformen der jeweiligen Substanzen enthält und auf der Packung, dem Beipackzettel oder einem Etikett eine Anweisung zur Einnahme der Arzneiformen am selben Tag enthält. Dabei können die einzelnen Arzneiformen annähernd zum selben Zeitpunkt oder zu verschiedenen Zeitpunkten im Verlaufe eines Tages eingenommen werden.
Bevorzugt sind jedoch fixe Kombinationen, also Arzneiformen, die alle zu kombinierenden Arzneistoffe enthalten. Diese können beispielsweise Tabletten, Hartgelatine oder Weich- gelatinekapseln sein.
Zur Kombination eines α-1 Adrenoceptor Antagonisten mit einem PDE 5 — Inhibitor kurzer Halbwertszeit, beispielsweise Vardenafil oder Sildenafil, werden zweiseitig retardierte Arzneiformen verwendet. Diese stellen eine Kombination von kontrolliert freisetzendem α-1 Adrenoceptor Antagonist - Anteil mit einem kontrolliert freisetzendem PDE 5 - Inhibitor - Anteil dar. Hierzu eignen sich beispielsweise Kapseln, die zwei Arten von Pellets, Minitabletten oder Tabletten enthalten, nämlich solche, die den α-1 Adrenoceptor Antagonisten und solche, die den PDE 5 - Inhibitor kontrolliert freisetzen. Eine weitere Möglichkeit stellen Retardtabletten dar, die aus mindestens zwei unterschiedlichen Wirkstoffschichten bestehen. Eine davon setzt den α-1 Adrenoceptor Antagonisten kontrolliert frei, eine andere den PDE 5 - Inhibitor. Daneben sind auch osmotisch freisetzende Tabletten möglich. Diese enthalten in der Wirkstoffschicht beide Kombinationspartner. Optional ist auch noch eine osmotisch wirksame, wirkstofffreie Quellschicht möglich. Die resultierende Ein- bzw. Zweischichttablette wird mit einem wasserunlöslichen, aber permeablen Lack, beispielsweise aus Celluloseacetat, überzogen und auf der wirkstoffhaltigen Seite mit mindestens einem Loch zur Wirkstofffreigabe versehen.
Zur Kombination eines α-1 Adrenoceptor Antagonisten mit einem PDE 5 - Inhibitor langer Halbwertszeit, beispielsweise Tadalafil, oder zur Kombination eines 5-α Reduktase - Inhibitors mit einem PDE 5 - Inhibitor kurzer Halbwertszeit, beispielsweise Vardenafil oder Sildenafil, werden einseitig retardierte Arzneiformen verwendet. Im ersten Fall gibt die Arzneiform den α-1 Adrenoceptor Antagonisten in kontrollierter Form frei und enthält das Tadalafil in schnell frei- setzender Form. Im zweiten Fall gibt die Arzneiform den PDE 5 - Inhibitor in kontrollierter Form frei und enthält den 5-α Reduktase - Inhibitor in schnell freisetzender Form. Einseitig retardierte Arzneiformen sind beispielsweise Kapseln, die den kontrolliert freizusetzenden Kombinationspartner in Form von Pellets, Minitabletten oder Tabletten enthalten und den schnell freizu-

-4 - setzenden Kombinationspartner in Form eines Pulvers, Komprimates, Tablette oder Pellet. Eine weitere Möglichkeit stellen Retardtabletten dar, die aus mindestens zwei unterschiedlichen Wirkstoffschichten bestehen. Die eine enthält den kontrolliert freizusetzenden Kombinationspartner. Dessen Freisetzung wird durch beispielsweise durch Einbettung in eine unlösliche Matrix oder erodierbare Matrix kontrolliert. Eine andere Wirkstoffschicht der Tablette enthält den nicht kontrolliert freizusetzenden Kombinationspartner in üblicher schnell freisetzender Form. Eine weitere Möglichkeit stellen einschichtig aufgebaute Retardtabletten dar. Diese enthalten den α-1 Adrenoceptor Antagonisten in Form von Diffusionspellets sowie den zu kombinierenden PDE 5 - Inhibitor langer Halbwertszeit oder 5-α Reduktase - Inhibitor. Alternativ enthalten diese den PDE 5 — Inhibitor kurzer Halbwertszeit in Form von Diffusionspellets sowie den zu kombinierenden 5-α Reduktase — Inhibitor. Daneben sind osmotisch freisetzende Tabletten möglich. Diese sind beispielsweise dreischichtig aufgebaut und enthalten eine erste Schicht mit dem nicht kontrolliert freizusetzenden Kombinationspartner, eine zweite Schicht mit dem kontrolliert freizusetzenden Kombinationspartner und eine osmotisch wirksame, wirkstofffreie Quellschicht. Die resultierende Dreischichttablette wird mit einem wasserunlöslichen, aber permeablen Lack, beispielsweise aus Celluloseacetat, überzogen und auf der wirkstoffhaltigen Seite mit mindestens einem Loch zur Wirkstofffreigabe versehen.
Zur Kombination eines 5-α Reduktase - Inhibitors mit einem PDE 5 - Inhibitor langer Halbwertszeit, beispielsweise Tadalafil sind Tabletten oder Kapseln ganz besonders bevorzugt, die beide Kombinationspartner in schnell freisetzender Form enthalten. Die Tabletten können aus einem Granulat oder einer Pulvermischung hergestellt werden, die bereits beide Kombinationspartner enthält. Alternativ können Granulate oder Pulver gemischt und anschließend gemeinsam verpresst werden, die jeweils nur einen der Kombinationspartner enthalten. Auch die Fertigung einer Zweischichttablette, in der die Wirkstoffe getrennt in den beiden Schichten enthalten. Analog können zur Fertigung von Kapseln separate Pulver, Granulate, Pellets oder Tabletten eingesetzt werden oder solche Pulver, Granulate, Pellets oder Tabletten, die bereits beide Kombinationspartner enthalten. Auch Weichgelatinekapselformulierungen mit beiden Kombinationspartnern sind möglich. Diese enthalten eine Suspension oder Lösung beider Kombinationspartner in einem Öl oder wassermischbaren Vehikeln wie Polyethylenglykol.
Detaillierte Beschreibung der Erfindung
α-1 und α-lA Adrenoceptor Antagonisten sind bekannt und werden im Rahmen dieser Erfindung unter dem Begriff α-1 Adrenoceptor Antagonist zusammengefasst. Besonders bevorzugt sind Tamsulosin, Alfuzosin, Doxazosin und Terazosin. Die Verbindungen können als Base oder Salz und diese jeweils in unterschiedlicher Hydratstufe und Modifikation eingesetzt werden. Die
bevorzugten Dosen und Formen sind 0,2 - 0,8 mg Tamsulosin HCl, 5 - 20 mg Alfuzosin HCl, 1 - 16 mg Doxazosin mesilat und 2 — 20 mg Terazosin HCl.
5-α Reduktase - Inhibitoren sind bekannt. Besonders bevorzugt ist Finasterid und Dutasterid. Die bevorzugten Dosen sind 1 - 10 mg Finasterid und 0,2 - 1 mg Dutasterid.
cGMP PDE 5 - Inhibitoren sind bekannt. Besonders bevorzugt sind Vardenafil, Sildenafil und Tadalafil. Die Verbindungen können als Base oder Salz und diese jeweils in unterschiedlicher Hydratstufe und Modifikation eingesetzt werden. Als cGMP PDE 5 - Inhibitoren mit langer Halbwertszeit sollen solche verstanden werden, deren Halbwertszeit mehr als 8 Stunden beträgt. Als cGMP PDE 5 - Inhibitoren mit kurzer Halbwertszeit sollen solche verstanden werden, deren Halbwertszeit gleich oder weniger als 8 Stunden beträgt Ein bevorzugter cGMP PDE 5 — Inhibitor mit langer Halbwertszeit ist Tadalafil. Die besonders bevorzugten Dosen und Formen sind: Vardenafil Hydrochlorid Trihydrat 5 - 30 mg ( berechnet als Vardenafil), Sildenafilcitrat 25 - 150 mg (berechnet als Sildenafil) und Tadalafil 5 — 30 mg.

Gegenstand der Erfindung ist ferner eine Arzneimittelformulierung, die einen α-1 Adrenoceptor Antagonisten kontrolliert mit einer mittleren Freisetzungsrate von 80 % in länger als 45 Minuten und einen PDE 5 - Inhibitor mit langer Halbwertszeit sowie einen 5-α Reduktase - Inhibitor enthält. Besonders bevorzugt sind Arzneimittelformulierungen, die einen α-1 Adrenoceptor Antagonisten kontrolliert mit einer mittleren Freisetzungsrate zwischen 80 % in 2 Stunden und 80 % in 16 Stunden abgeben und einen PDE 5 - Inhibitor mit langer Halbwertszeit sowie einen 5- α Reduktase - Inhibitor enthalten.
Zur Ermittlung der mittleren Freisetzungsrate gemäß der Definition der Erfindung werden die Arzneimittelformulierungen der vorliegenden Erfindung in der „Apparatur 2" der USP geprüft (The United States Pharmacopeia). Als Testmedium wird 900 ml Phosphatpuffer mit pH 6,8 unter Zusatz von 0.1 % Natriurnlaurylsulfat verwendet. Die Umdrehungsgeschwindigkeit des Rührers beträgt 75 Umdrehungen pro Minute. Proben werden durch ein 8 μm Filter gezogen und deren Wirkstoffgehalt bestimmt. Die auf diese Weise als aufgelöst bestimmte Wirkstoffmenge wird in Gewichts-Prozent der eingesetzten Wirkstoffmenge umgerechnet.
Die zuvor beschriebenen Arzneimittelformulierungen liegen beispielsweise in einer Kapsel vor. Diese besteht beispielsweise aus Glycerin, Gelatine und Farbstoffen, aus Hypromellose, Pullulan oder aus den anderen bekannten Materialien und kann enthalten:

Zur Formulierung des α-1 Adrenoceptor Antagonisten und des PDE 5 - Inhibitors in kontrolliert freisetzender Form zur nachfolgenden Abfüllung in eine Kapsel eignen sich insbesondere diffusionskontrollierte Pellets. Zur Herstellung der diffusionskontrollierten Pellets mit einem α-1 Adrenoceptor Antagonisten oder PDE 5 - Inhibitor werden beispielsweise Neutralpellets aus Saccharose oder mikrokristalliner Cellulose mit einer Mischung des Wirkstoffes, üblichen Bindemitteln, erforderlichenfalls einer Säure und weiteren üblichen Hilfstoffen beschichtet und anschließend mit einem Diffusionslack, der einen Weichmacher enthalten kann, überzogen. Als Bindemittel werden bevorzugt Hydroxypropylmethylcellulose oder Polyvinylpyrrolidon ver- wendet. Ebenso können andere natürliche, synthetische oder halbsynthetische Polymere wie beispielsweise Methylcellulose, Hydroxypropylcellulose, Natriumcarboxymethylcellulose, PoIy- acrylsäuren, Polyvinylalkohole oder Gelatine eingesetzt werden. Als Diffusionslack eignet sich besonders Ethylcellulose, wie sie z.B. als wässrige Dispersion unter der Bezeichnung Aquacoat® oder Surelease® im Handel ist. Aber auch andere Materialien wie Poly[(methacryl- säure)(ethylacrylat)] (1:1) oder andere Acrylate (Eudragit®), Celluloseacetat oder Celluloseacetat- butyrat können verwendet werden. Als Weichmacher eignen sich beispielsweise Phthalsäurederivate (z.B. Dimethylphthalat, Diethylphthalat, Dibutylphthalat), Citronensäure- derivate (z.B. Triethylcitrat, Tributylcitrat, Acetyltriethylcitrat), andere Ester (z.B. Diethylsebacat, Triacetin), Fettsäuren und Derivate (Glycerolmonostearat, acetylierte Fettsäureglyceride, Rizinusöl und andere native Öle, Miglyol), PoIy ole (Glycerol, 1,2- Propandiol, Polyethylenglycol unterschiedlicher Kettenlänge). Des weiteren werden Art und Menge des Weichmachers so eingestellt, dass die oben definierte erfmdungsgemäße Freisetzung und die erforderliche Stabilität der Pellets erreicht werden. Die Einstellung der oben definierten Freisetzung erfolgt ferner durch Steuerung der Porengröße des Diffusionslackes und/oder seiner Dicke. Als Porenbildner zur Steuerung der Porengröße können gegebenenfalls lösliche Polymere, wie z.B. Polyethylenglycole, Polyvinyl- pyrolidone, Hydroxypropylmethylcellulosen, Carboxymethylcellulosen oder deren Salze, Methyl- cellulosen, Dextrine, Maltodextrine, Cyclodextrine, Dextrane oder andere lösliche Verbindungen, wie z.B. Salze (Kochsalz, Kaliumchlorid, Ammoniumchlorid usw.), Harnstoff, Zucker (Glucose, Saccharose, Fructose, Lactose usw.), Zuckeralkohole (Mannit, Sorbit, Lactitol usw.) eingesetzt werden. Der Anteil des Porenbildners an der Lackmenge beträgt dabei 0 bis 50 % (G/G) (G = Masse). Bei den Pellets ist es besonders wichtig, ein bestimmtes Gewichts-Verhältnis von wirkstoffüberzogenen Pellets zur Diffusionsmembran sowie ein bestimmtes Verhältnis von Diffusionslack zur Weichmachermenge zu verwenden. Teile des eingesetzten Weichmachers können während des Lackierens und Nachtemperns abdunsten. Bei Änderung der Randbedingungen ist eine Veränderung der Beschichtungsmenge an Diffusionslack erforderlich. So ist beispielsweise
eine höhere Beschichtungsmenge erforderlich, wenn die gewünschte Freisetzungsrate verringert wird, die Menge Porenbildner erhöht wird oder bei bestimmten Weichmachern der Weichmacheranteil verringert wird. Eine niedrigere Beschichtungsmenge ist erforderlich, wenn die gewünschte Freisetzungsrate erhöht wird, die Menge Porenbildner erniedrigt wird oder bei bestimmten Weich- machern der Weichmacheranteil erhöht wird. Die Diffusionspellets können beispielsweise hergestellt werden, indem man den Wirkstoff in Wasser suspendiert bzw. löst und mit einer konzentrierten Hydroxypropylmethylcellulose-Lösung verdickt. Die so erhaltene Suspension oder Lösung wird in einer Wirbelschichtanlage in einem Sprühprozess auf Neutralpellets aufgezogen. Es folgt die Beschichtung der Pellets mit einer Diffusionsmembran bevorzugt in einer Wirbelschichtanlage durch Aufsprühen beispielsweise einer wässrigen Ethylcellulosedispersion oder organischen Ethylcellulose - Lösung, die einen geeigneten, physiologisch verträglichen Weichmacher enthält. Die Pellets werden anschließend bei Temperaturen von 50 bis 125°C, vorzugsweise 60 bis 1100C getempert. Dabei führen höhere Temperaturen der Temperung dazu, dass zur Erzielung der erfindungsgemäßen Freisetzung eher niedere Lackauftragsmengen aus- reichen und die entstehenden Pellets bei Lagerung physikalisch stabiler sind. Die Dicke der Diffusionsmembran, Weichmachertyp, Weichmachermenge und Pelletgröße werden so gewählt, dass eine Freisetzungsgeschwindigkeit des α-1 Adrenoceptor Antagonisten und des PDE 5 - Inhibitors von 80 % in mehr als 45 Minuten, bevorzugt zwischen 80 % in 2 Stunden bis 16 Stunden resultiert. Die einer Tagesdosis von beispielsweise 0,4 mg Tamsulosin HCL und 10 mg Vardenafϊl HCl Trihydrat entsprechende Menge Pellets wird in eine Hartgelatinekapsel gefüllt. Neben der beschriebenen Beschichtung von Neutralpellets sind auch andere Methoden der Pelletherstellung gangbar wie die Feuchtextrusion und Ausrunden, die Rotorgranulation, die Wirbelschichtagglomeration oder die thermische Extrusion. Alternativ können auch Minitabletten mit einem Durchmesser von 1 — 4 mm hergestellt werden. Nachfolgend werden die wirkstoffhaltigen Pellets oder Minitabletten wie beschrieben mit einer Diffusionsmembran überzogen.
Zur Formulierung des α-1 Adrenoceptor Antagonisten und des PDE 5 - Inhibitors in kontrolliert freisetzender Form zur nachfolgenden Abfüllung in eine Kapsel eignen sich in einer anderen Ausgestaltungsform der erfindungsgemäßen Arzneimittelformulierung Tabletten, die den Wirkstoff in einer Matrix eines wasserquellbaren Polymers enthalten. Die Größe dieser Tabletten ist so bemessen, dass eine oder mehrere Tabletten im Inneren der Kapsel Platz finden. Die Tabletten können in nicht überzogener Form in die Kapsel gefüllt werden oder zuvor mit einem Lack überzogen werden, beispielsweise einem in Magensaft unlöslichen Lack.
Tabletten zur nachfolgenden Abfüllung in eine Kapsel, die den α-1 Adrenoceptor Antagonisten oder PDE 5 - Inhibitor in einer Matrix eines wasserquellbaren Polymers enthalten, werden wie folgt hergestellt. Diese sogenannten Matrixformulierungen enthalten zweckmäßig von 0,1 bis
.

70 Gew.-%, bevorzugt 0,2 bis 60 Gew.-% des Wirkstoffs. Der Mengenanteil der Matrix des wasserquellbaren Polymers beträgt zweckmäßig von 10 bis 95 Gew.-%, bevorzugt 20 bis 60 Gew.-%. Besonders bevorzugt sind erfindungsgemäße Arzneizubereitungen in Form von Erosionstabletten. Diese Tabletten sind dadurch gekennzeichnet, dass sie neben üblichen Hilfs- und Trägerstoffen sowie Tablettierhilfsstoffen, eine bestimmte Menge an wasserquellbaren, hydrogelbildenden Polymeren enthalten, wobei diese Polymere eine Viskosität von mindestens 15, bevorzugt mindestens 50 cps (gemessen als 2 %ige wässrige Lösung bei 200C) haben müssen. Übliche Hilfs- und Trägerstoffe sind beispielsweise Laktose, mikrokristalline Cellulose, Mannit oder Calciumphosphate. Übliche Tablettierhilfsmittel sind beispielsweise Magnesiumstearat, Talkum oder hochdisperses Siliciumdioxid (Aerosil®). Diese liegen im Falle des Magnesium- stearats zweckmäßig in einer Menge von 0,5 bis 3 Gew.-%, im Falle des hochdispersen Silicium- dioxids zweckmäßig in einer Menge von 0,1 bis 1 Gew.-% vor. Als wasserlösliche, hydrogelbildende Polymere werden bevorzugt Hydroxypropylcellulosen, Hydroxypropylmethyl- cellulosen (HPMC), Methylcellulosen, Carboxymethylcellulose, Alginate, Galaktomannane, PoIy- acrylsäuren, Polymethacrylsäuren oder Copolymerisate aus Methacrylsäure und Methyl- methacrylat, Guar, Agar, Pektin, Tragant, Gummi arabicum, Xanthan bzw. Mischungen dieser Substanzen eingesetzt. Besonders bevorzugt ist die Verwendung von HPMC. Hierbei sollten die erfindungsgemäßen Erosionstabletten bevorzugt mindestens 10 Gew.-% eines Hydroxy- propylmethylcellulosetyps bezogen auf die Masse einer Tablette enthalten, dessen Viskosität (gemessen als 2 %ige wässrige Lösung bei 200C) mindestens 15, bevorzugt mindestens 50 cps beträgt. Die Arzneimittelformulierung, die den Wirkstoff in einer Matrix eines wasserquellbaren Polymers umfasst, wird hergestellt, indem man den Wirkstoff, das Polymer und geeignete Hilfsund Trägerstoffe (wie oben beschrieben) sowie übliche Tablettierhilfsmittel (wie oben beschrieben) mischt und direkt tablettiert. Ferner ist es möglich, den Wirkstoff, das wasser- quellbare Polymer und geeignete Trägerstoffe in der Wirbelschicht zu granulieren. Dabei wird die Menge und Viskosität des wasserquellbaren Polymers so gewählt, dass Tabletten mit den oben beschriebenen mittleren Freisetzungsgeschwindigkeiten des α-1 Adrenoceptor Antagonisten oder PDE 5 — Inhibitors resultieren. Das trockene Granulat wird gesiebt, mit einem Schmiermittel, wie zum Beispiel Magnesiumstearat, vermischt und tablettiert. Die Tablette wird gegebenenfalls noch lackiert. Zur nachfolgenden Füllung einer Kapsel sind Erosionstabletten mit einem Durchmesser von 3 mm bis 7 mm bevorzugt.
Der PDE 5 - Inhibitor mit langer Halbwertszeit oder der 5-α Reduktase - Inhibitor kann als Pulver, Granulat, Pellet oder Tablette in die Kapsel eingebracht werden. Hierzu eignen sich übliche schnell freisetzende Formulierungen.
In einer weiteren Ausfuhrungsform der Arzneimittelformulierung liegen die Kombinationspartner in einer Zweischichttablette vor. Diese besteht aus zwei kontrolliert freisetzenden Schichten, aus einer kontrolliert und einer schnell freisetzenden Schicht oder zwei schnell freisetzenden Schichten. Diese können sein:
Einerseits eine α-1 Adrenoceptor Antagonist kontrolliert freisetzende Schicht oder eine 5- α Reduktase - Inhibitor schnell freisetzende Schicht
und
andererseits eine PDE 5 - Inhibitor kontrolliert freisetzende Schicht oder eine PDE 5 - Inhibitor mit langer Halbwertszeit schnell freisetzende Schicht
Die Formulierung der jeweils kontrolliert freisetzenden Schicht orientiert sich an den Prinzipien, die oben für die Matrixformulierung zur nachfolgenden Abfüllung in eine Kapsel dargelegt wurde. Zur Formulierung der jeweils schnell freisetzenden Schicht wird der Wirkstoff mit geeigneten Hilfs- und Trägerstoffen (wie oben beschrieben) sowie üblichen Tablettierhilfsmitteln (wie oben beschrieben) gemischt und direkt tablettiert. Ferner ist es möglich, den Wirkstoff und geeignete Trägerstoffe in der Wirbelschicht, im Mischgranulator oder im Walzenkompaktor zu granulieren. Das trockene Granulat wird gesiebt, mit einem Schmiermittel, wie zum Beispiel Magnesium- stearat, vermischt und tablettiert. Zur Tablettierung eignet sich insbesondere eine mit 2 Füll- und Pressstationen versehene Zweischichtpresse. Die Tablette wird gegebenenfalls noch lackiert. Um eine zu hohe initiale Freisetzungsrate eines der Kombinationspartner zu verhindern, kann die Zweischichttablette auch noch mit einer dritten, wirkstofffreien Schicht versehen werden.
In einer weiteren Ausführungsform der Arzneimittelformulierung liegen die Kombinationspartner in einer Einschichttablette vor. Diese enthält:
Einerseits einen α-1 Adrenoceptor Antagonisten oder PDE 5 - Inhibitor kurzer Halbwertszeit in kontrolliert freisetzender Formulierung
und
andererseits einen PDE 5- Inhibitor mit langer Halbwertszeit (zur Kombination mit einem α-1 A Adrenoceptor Antagonisten) oder einen 5-α Reduktase - Inhibitor (zur Kombination PDE 5 - Inhibitor kurzer Halbwertszeit) in schnell freisetzender Form
Als kontrolliert freisetzende Formulierung zur nachfolgenden Einarbeitung in eine Einschicht- tablette eignen sich insbesondere die oben beschriebenen diffusionskontrollierten Pellets. Diese
werden mit dem in schnell freisetzender Form zu kombinierenden Wirkstoff und weiteren Hüfsstoffen, Trägerstoffen und Tablettierhilfsmitteln gemischt und zu einer Einschichttablette ver- presst. Auch eine Granulation des schnell freisetzenden Hilfsstoffes sowie eine nachfolgende Lackierung der Tablette sind möglich.
In einer weiteren Ausfuhrungsform der Arzneimittelformulierung der vorliegenden Erfindung handelt es sich um ein osmotisches Arzneimittelfreisetzungssystem. Solche osmotischen Arzneimittelfreisetzungssysteme sind grundsätzlich im Stand der Technik bekannt und werden z.B. ausführlich abgehandelt in Richard W. Baker, "Osmotic Drug Delivery: A Review of the Patent Literature", Journal of Controlled Release 35 (1995) 1-21. Die Arzneimittelformulierung als osmotisches Arzneimittel-Freisetzungssystem besteht bevorzugt aus
• a) einem Kern, der die Wirkstoffe, gegebenenfalls ein hydrophiles polymeres Quellmittel und gegebenenfalls einen wasserlöslichen Stoff zur Induzierung der Osmose enthält, und
• b) einer für Wasser durchlässigen und für die Komponenten des wirkstoffhaltigen Kerns undurchlässigen Hülle
• c) einer Öffnung durch die Hülle b) für den Transport der im Kern enthaltenen Bestandteile in die umgebende Körperflüssigkeit.
Dieses spezielle osmotische Arzneimittelfreisetzungssystem ist grundsätzlich im Stand der Technik beschrieben, beispielsweise in der DE-A-2 328 409 oder der US- A-3 85 770. Bezüglich der Materialien für die Hülle sei auf die EP- A-O 277 092 und die dort erwähnten US-A-3 916 899 und US-A-3 977 404 Bezug genommen.
Bezüglich geeigneter hydrophiler polymerer Quellmittel sei beispielsweise auf die in der EP-A- 0 277 092 sowie der WO 96/40080 genannten polymeren Quellmittel verwiesen. Beispielsweise können Ethylenoxidhomopolymerisate (Polyethylenglycol) mit verschiedenen Polymerisationsgraden, die beispielsweise unter der Bezeichnung Polyox bekannt sind, mit Molekulargewichten zwischen 100.000 bis 8.000.000 sowie Vinylpyrrolidon-vinylacetat-copolymerisate sowie weitere in der US-A-3 865 108, US-A-4 002 173 und US-A-4207 893 genannte wasserquellbare Polymere verwendet werden. Wasserlösliche Stoffe zur Induzierung der Osmose sind im Prinzip alle wasserlöslichen Stoffe, deren Verwendung in der Pharmazie unbedenklich ist, die z.B. in den Pharmacopöen oder in "Hager's Handbuch der Pharmazeutischen Praxis, 1990-1995, Springer Verlag" sowie Remington's Pharmaceutical Sciences als wasserlösliche Hilfsstoffe erwähnt sind. Spezielle wasserlösliche Stoffe sind Salze von anorganischen oder organischen Säuren oder nichtionische organische Stoffe mit hoher Wasserlöslichkeit wie z.B. Kohlenhydrate wie Zucker
etc. Die Herstellung einer Öffnung in die Hülle der Tablette ist an sich im Stand der Technik bekannt und beispielsweise in den US Patentschriften 3 485 770 und 3 916 899 beschrieben. Die Einstellung der Freisetzungsrate erfolgt durch Art und Menge des die Hülle bildenden semipermeablen Materials, durch Art und Menge des ggf. enthaltenen hydrophilen polymeren Quellmittels sowie des gegebenenfalls vorhandenen wasserlöslichen Stoffes zur Induzierung des Osmose. Die Kombinationspartner der vorliegenden Erfindung können in unterschiedlicher Weise in ein osmotisches Arzneimittelfreisetzungssystem eingebracht werden. Zur kontrollierten Abgabe beider Wirkstoffe werden diese mit den Hilfsstoffen gemischt und zu einer gemeinsamen Wirkstoffschicht verpresst. Soll nur ein Kombinationspartner kontrolliert freigegeben werden, kann dieser entweder separat in die Lackhülle der Tablette eingebracht werden oder der nicht kontrolliert freizugebende Wirkstoff wird zu eine separaten Wirkstoff Schicht verpresst, welche zunächst vor dem kontrolliert freizugebendem Kombinationspartner aus dem Arzneimittelfreisetzungssystem freigepumpt wird.
Beispiele
Beispiel 1
12,5 g Tamsulosin HCl wird zusammen mit 12,5 g Hypromellose in einer Mischung von 47,5 Wasser und 427,5 g Methanol gelöst. Diese Lösung wird auf 2 500 g Neutralpellets aus mikrokristalliner Cellulose mit einem mittleren Partikeldurchmesser von 125 μm in einer Wirbelschichtanlage aufgesprüht. 2 500 g der so beschichteten Pellets werden in einer Wirbelschichtanlage mit einer Dispersion besprüht, die aus folgenden Bestandteilen besteht: 1167 g Poly(ethylacrylat-methylmethacrylat) - Dispersion 30 %, 350 g Talkum und 1260 g Wasser. Die mit dieser Diffusionsmembran beschichteten Pellets werden bei 400C nachgetrocknet.
741 g mikronisiertes Vardenafil Hydrochlorid Trihydrat und 625 g gemahlene Weinsäure werden in einer Lösung aus 156 g Hypromellose und 6 250 g Wasser suspendiert, bzw. gelöst. Diese Suspension wird in einer Wirbelschichtanlage auf 2 500 g Neutralpellets aus Saccharose aufgesprüht. Die so beschichteten Pellets werden in einer Wirbelschichtanlage mit einer Dispersion besprüht, die aus folgenden Bestandteilen besteht: 1473 g Poly(ethylacrylat-methylmethacrylat) - Dispersion 30 %, 442 g Talkum und 1 593 g Wasser. Die mit dieser Diffusionsmembran beschichteten Pellets werden bei 400C nachgetrocknet.
103,4 mg der diffusionskontrollierten Tamsulosinpellets und 157 mg der diffusionskontrollierten Vardenafüpellets werden in eine Hartgelatine - Steckkapsel abgefüllt.
Beispiel 2
Eine Dreischicht - Tablette wird wie folgt hergestellt: Für die Schicht 1 werden 75,4 kg Hypromellose, 5 kg Ethylcellulose und 15 kg hydriertes Rizinusöl gemischt und mit einer Lösung bestehend aus 3,2 kg Povidon K 30 und 28,8 kg Ethanol granuliert. Das getrocknete Granulat wird mit 0,5 kg hochdispersem Sicliciumdioxid und 1 kg Magnesiumstearat nachgemischt. Für die Schicht 2 werden 5 kg Alfuzosinhydrochlorid, 15 kg Hypromellose und 75 kg mikrokristalline Cellulose in der Wirbelschicht mit einer Lösung aus 3 kg Povidon K 30 und 97 kg Wasser granuliert. Das Granulat wird getrocknet und mit 0,5 kg hochdispersem Sicliciumdioxid und 1,5 kg Magnesiumstearat nachgemischt. Für die Schicht 3 werden 23,7 kg Vardenafil Hydrochlorid Trihydrat, 130 kg Hypromellose und 3.1 kg mikrokristalline Cellulose gemischt und auf einer Walze trocken granuliert. Das Granulat wird mit 0,8 kg hochdispersem Sicliciumdioxid und 2,4 kg Magnesiumstearat nachgemischt. Die drei Granulate werden so in die Fülltrichter einer Dreischicht - Tablettenpresse gegeben, dass Schicht 2 die mittlere Tablettenschicht darstellt. Es
werden runde Dreischicht - Tabletten von 8 mm Durchmesser gepresst, wobei Schicht 1 eine Masse von 100 mg, Schicht 2 von 100 mg und Schicht 3 von 160 mg aufweist.
Beispiel 3
Es werden Tabletten folgender Zusammensetzung hergestellt: 5 mg Finasterid, 10 mg Tadalafü, 50 mg Laktosemonohydrat, 47,625 mg mikrokristalline Cellulose, 0,625 mg Natriumdodecylsulfat, 7,5 mg Croscarmellose Natrium, 3 mg Hyprolose, 1,25 Magnesiumstearat. Hierzu werden die einer Charge von 1,6 Mio. Tabletten entsprechende Mengen an Finasterid, Tadalafü, Laktosemonohydrat, mikrokristalline Cellulose und die halbe Menge der Croscarmellose Natrium im Wirbelschichtgranulator mit einer wässrigen Lösung aus Natriumdodecylsulfat und Hyprolose granuliert. Das getrocknete Granulat wird mit Magnesiumstearat nachgemischt und auf einer Rundlaufpresse zu runden Tabletten von 7 mm Durchmesser und 125 mg Masse verpresst. Die Tabletten werden mit einem Lack überzogen bestehend aus: 2,391 mg Hypromellose, 0,797 mg Macrogol 3350, 0,653 mg Titandioxid und 0,144 mg Eisenoxid gelb.
Beispiel 4
In einem ersten Ansatz werden 70,225 kg Sildenafilcitrat, 35,075 kg mikrokristalline Cellulose und 37,5 kg Hypromellose in einem Wirbelschichtgranulator mit einer Lösung von 5,7 kg Hypromellose in 136,8 kg Wasser granuliert. Das getrocknete Granulat wird mit 1,5 kg Magnesiumstearat nachgemischt und zu runden Tabletten von 7 mm Durchmesser und einer Masse von 150 mg verpresst. In einem zweiten Ansatz werden 5 kg Finasterid, 60 kg Laktosemonohydrat, 47,625 kg mikrokristalline Cellulose und 3,75 kg Croscarmellose Natrium in einem Wirbelschichtgranulator mit einer Lösung aus 0,625 kg Natriumdodecylsulfat und 3 kg Hyprolose in 100 kg Wasser granuliert. Das getrocknete Granulat wird mit 3,75 kg Croscarmellose Natrium und 1,25 kg Magnesiumstearat nachgemischt. Die Mischung wird zu runden Tabletten von 7 mm Durchmesser und einer Masse von 125 mg verpresst. Je eine Tablette des ersten Ansatzes und eine Tablette des zweiten Ansatzes werden in eine Hartgelatinekapsel der Größe 00 verkapselt.
Claims
1. Arzneiform oder Kombinationspackung zur Behandlung der benignen Prostatahyperplasie enthaltend mindestens einen α-1 Adrenoceptor Antagonisten in kontrolliert freisetzender Formulierung und mindestens einen PDE 5 - Inhibitor in kontrolliert freisetzender " Formulierung oder mindestens einen PDE 5 — Inhibitor mit langer Halbwertszeit.
2. Arzneiform gemäß Anspruch 1 enthaltend eine kontrolliert freisetzende Formulierung von Tamsulosin, Alfuzosin, Doxazosin oder Terazosin sowie eine kontrolliert freisetzender Formulierung von Vardenafil oder Sildenafil.
3. Arzneiform gemäß Anspruch 1 enthaltend eine kontrolliert freisetzende Formulierung von Tamsulosin, Alfuzosin, Doxazosin oder Terazosin sowie Tadalafil.
4. Arzneiform oder Kombinationspackung zur Behandlung der benignen Prostatahyperplasie enthaltend mindestens einen 5-α Reduktase - Inhibitor und mindestens einen PDE 5 -
. Inhibitor in kontrolliert freisetzender Formulierung oder mindestens einen PDE 5 - Inhibitor mit langer Halbwertszeit.
5. Arzneiform gemäß Anspruch 4 enthaltend Finasterid oder Dutasterid sowie eine kontrolliert freisetzende Formulierung von Vardenafil oder Sildenafil.
6. Arzneiform gemäß Anspruch 4 enthaltend Finasterid oder Dutasterid sowie Tadalafil.
7. Arzneiform oder Kombinationspackung zur Behandlung der benignen Prostatahyperplasie enthaltend mindestens einen α-1 Adrenoceptor Antagonisten in kontrolliert freisetzender Formulierung, mindestens einen PDE 5 - Inhibitor in kontrolliert freisetzender
Formulierung oder mindestens einen PDE 5 - Inhibitor mit langer Halbwertszeit und mindestens einen 5-α Reduktase - Inhibitor.
8. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen α-1 Adrenoceptor Antagonisten in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind
und
einen PDE 5 - Inhibitor in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind.
9. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen α-1 Adrenoceptor Antagonisten als Matrixtablette mit kontrollierter Wirkstofffreisetzung
und
einen PDE 5 - Inhibitor als Matrixtablette mit kontrollierter Wirkstofffreisetzung.
10. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Zweischichttablette darstellt, welche enthält:
eine Schicht mit einem α-1 Adrenoceptor Antagonisten in Form einer kontrolliert frei- setzenden Matrixformulierung
und
eine Schicht mit einem PDE 5 - Inhibitor in Form einer kontrolliert freisetzenden Matrixformulierung.
11. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen α-1 Adrenoceptor Antagonisten in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind
und
einen PDE 5 - Inhibitor mit langer Halbwertszeit.
12. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen α-1 Adrenoceptor Antagonisten als Matrixtablette mit kontrollierter Wirkstofffreisetzung
und
einen PDE 5 - Inhibitor mit langer Halbwertszeit.
13. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Zweischichttablette darstellt, welche enthält:
eine Schicht mit einem α-1 Adrenoceptor Antagonisten in Form einer kontrolliert freisetzenden Matrixformulierung
und
eine Schicht mit einem PDE 5 - Inhibitor mit langer Halbwertszeit.
14. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie eine Einschichttablette darstellt, welche enthält:
eine Schicht mit einem α-1 Adrenoceptor Antagonisten in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind
und
eine Schicht mit einem PDE 5 — Inhibitor mit langer Halbwertszeit.
15. Arzneiform gemäß Anspruch 1 dadurch gekennzeichnet, dass sie ein osmotisches Arzneimittelfreisetzungssystem darstellt, welches einen α-1 Adrenoceptor Antagonisten und einen PDE 5 - Inhibitor enthält.
16. Arzneiform gemäß Anspruch 2 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen 5-α Reduktase — Inhibitor und
einen PDE 5 - Inhibitor in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind.
17. Arzneiform gemäß Anspruch 2 dadurch gekennzeichnet, dass sie eine Kapsel darstellt, welche enthält:
einen einen 5-α Reduktase - Inhibitor und
einen PDE 5 - Inhibitor als Matrixtablette mit kontrollierter Wirkstofffreisetzung.
18. Arzneiform gemäß Anspruch 2 dadurch gekennzeichnet, dass sie eine Zweischichttablette darstellt, welche enthält: eine Schicht mit einem 5-α Reduktase - Inhibitor
und
eine Schicht mit einem PDE 5 - Inhibitor in Form einer kontrolliert freisetzenden Matrixformulierung.
19. Arzneiform gemäß Anspruch 2 dadurch gekennzeichnet, dass sie eine Einschichttablette darstellt, welche enthält:
eine Schicht mit einem PDE 5 - Inhibitor in Form von Pellets, Granulat oder Tablette(n), die mit einer diffusionskontrollierenden Membran überzogen sind
und
eine Schicht mit einem 5-α Reduktase - Inhibitor.
20. Arzneiform gemäß Anspruch 2 dadurch gekennzeichnet, dass sie ein osmotisches Arzneimittelfreisetzungssystem darstellt, welches einen 5-α Reduktase — Inhibitor und einen PDE 5 - Inhibitor enthält.
21. Arzneimittelformulierung gemäß Anspruch 2 dadurch gekennzeichnet, dass sie einen PDE 5 — Inhibitor langer Halbwertszeit und einen 5-α Reduktase — Inhibitor enthält.
