Abstract
Description
Solche Therapiegeräte sind bereits in verschiedenen Ausführungen als Handgelenk-Bewegungsschiene oder als Finger-Bewegungsschiene bekannt. Sie dienen beispielsweise nach Operationen im Handbereich zur kontinuierlichen passiven Gelenkmobilisation.
Auf den Querbügel lassen sich Fingerbetätigungselemente unterschiedlicher Länge in Anpassung an die unterschiedlichen Fingerlängen aufschieben. Die anderen Enden der Fingerbetätigungselemente sind mit Clipsen verbindbar, die über Klebestreifen mit den jeweiligen Fingerspitzen befestigbar sind. Nach einem Wechsel von der einen Hand zur anderen müssen die Fingerbetätigungselemente vom Querbügel abgenommen und in umgekehrter Reihenfolge wieder aufgeschoben werden. Mit dieser Finger-Bewegungsschiene sind Bewegungsübungen der vier Finger möglich.
Damit ist ein Universalgerät geschaffen, das mit wenigen, einfachen Umstellungen für praktisch alle im Bereich der Hand in Frage kommenden Bewegungsübungen einsetzbar ist. Es ist somit gegenüber dem Stand der Technik, wo mehrere Geräte für die unterschiedlichen Bewegungsübungen erforderlich sind, nur noch ein Gerät notwendig. Dies ist wesentlich kostengünstiger und ein einziges Gerät ist auch einfacher handhabbar, da das Umsetzen der verschiedenen Geräte an der Hand bzw. dem Arm entfällt.
Durch entsprechenden Seitenabstand können die Bewegungsübungen mit den vier Fingern ausgeführt werden, ohne daß der dazwischen befindliche Daumen stört.
Damit kann von der seitlich der Hand angeordneten Schwenkhebelanordnung die Schwenkbewegung quer zu den einzelnen Fingern übertragen werden. Durch das Querschwenkgelenk ist das Anschlußteil etwa rechtwinklig zur Bewegungsrichtung der Schwenkhebel verschwenkbar und so auf unterschiedlich lange Finger einstellbar.
Diese Gelenk-Kette läßt sich um ihre Querschwenkgelenke in eine Bewegungsrichtung frei bewegen, während quer dazu eine Kraftübertragung wie mit einem starren Hebel möglich ist. Durch die Anordnung der Gelenk-Kette mit den Schwenkachsen ihrer Querschwenkgelenke etwa rechtwinklig zu den Schwenkhebel-Schwenkachsen und zur Längserstreckung des Schwenkhebels, an dem die Gelenk-Kette angeschlossen ist, kann so die Schwenkbewegung der Schwenkhebelanordnung quer zu den Fingerspitzen übertragen werden.
Bei kleineren Händen bzw. Fingern kann die Gelenk-Kette in Schleifen verlaufen, ohne daß dadurch die Übertragung der Schwenkbewegung zu den Fingern beeinträchtigt ist.
Dabei ist es zweckmäßig, wenn die Gelenk-Kette länger ist als die Breite der direkt nebeneinander befindlichen vier Finger der Hand. Die Finger haben dadurch in Spreizrichtung Bewegungsfreiheit, so daß eine entspannte Fingerhaltung und in Spreizrichtung eine freie Bewegbarkeit vorhanden ist.
Andererseits besteht aber dafür auch die Möglichkeit, daß mehrere, auswechselbare Anschlußteile vorzugsweise in Form unterschiedlich langer Gelenk-Ketten vorgesehen sind.
Diese Anordnung der Schwenkhebel bildet praktisch einen Gelenkstrang nach, welcher der anatomischen Anordnung mit Handgelenk, Mittelhandknochen, Fingergrundgelenk, Fingergrundglied, Fingermittelgelenk und Fingermittelglied entspricht und parallel dazu angeordnet ist. Entsprechend der natürlichen Fingerbeugebewegung werden auch die einzelnen Schwenkhebel für die Bewegungsübungen bewegt.
Der Handgriff ist in diesem Falle zweckmäßigerweise als Knopf oder Knauf ausgebildet.
Gerade bei Bewegungsübungen des Daumens kann dieser durch unterschiedliche Stellungen des Gerätes in seiner Ausgangslage mehr oder weniger vom Zeigefingerbereich abgespreizt werden, so daß das Sattelgelenk des Daumens in unterschiedlicher Form bewegt wird. Dadurch können nicht nur Daumengrund- und Endgelenke trainiert werden, sondern auch das für die Bewegung des Daumens wichtige Sattelgelenk.

Die Verschwenkung auf die andere Seite erfolgt durch eine Schwenkbewegung des Wendeteiles 14, das mit seiner Welle in dem hülsenförmigen Klemmhalter 13 drehgelagert ist. Durch Lösen der Klemmschraube 31 kann der Querträger 15 des Wendeteiles 14 auf die andere Seite in eine beliebige bzw. für die Daumenbewegungsübungen passende Stellung verschwenkt und durch Anziehen der Klemmschraube 31 wieder arretiert werden.
Die Endstellung der Bewegungskinematik 2 kann so variiert werden, daß sich der Daumen in Ausgangslage in einer mehr oder weniger abgespreizten Lage vom Mittelfinger befindet.

Für diese Bewegung besteht auch die Möglichkeit, daß die Schwenkachse des Basis-Schwenkhebels 3 so eingestellt wird, daß die Gelenkachse des Handgelenkes etwa mittig und rechtwinklig geschnitten wird und daß dann zumindest der Basis-Schwenkhebel 3 für eine Schwenkbewegung motorisch angetrieben ist und die weiteren Schwenkhebel gegebenenfalls festlegbar sind.

- Therapiegerät zur Durchführung einer kontinuierlichen, passiven Bewegung zumindest eines Teiles der Hand eines Patienten, wobei das Gerät eine Halterung (8) insbesondere mit einer am Unterarm anbringbaren, sich bis zum Handgelenk erstreckenden Unterschale (10) und gegebenenfalls einer das Handgelenk überdeckenden Oberschale (11) aufweist, mit der eine Bewegungskinematik (2) verbunden ist, die einen mit einer Steuereinrichtung in Steuerverbindung stehenden Bewegungsantrieb (6) aufweist, der mit Schwenkhebeln verbunden ist, dessen antriebsfernster Schwenkhebel wenigstens ein Anschlußteil (7) für ein zu bewegendes Teil der Hand hat, dadurch gekennzeichnet, daß das Gerät (1) wahlweise zur Bewegung zumindest eines der vier Finger oder des Daumens oder des Handgelenkes (9) ausgebildet ist, daß die die Schwenkhebel (3,4,5) aufweisende Bewegungskinematik (2) zumindest zur Bewegung der Finger einschließlich des Daumens seitlich neben der Hand angeordnet ist, daß am antriebsfernen Ende der Schwenkhebelanordnung auswechselbare Anschlußteile (7) vorgesehen sind, daß zur Bewegung der Finger einschließlich des Daumens das Anschlußteil (7) wenigstens einen Fingeranschluß (19) aufweist und daß für die Bewegung des Handgelenkes bei abgenommener Oberschale (11) der Halterung (8), das Anschlußteil (7) als Handgriff (32) ausgebildet ist.
- Therapiegerät nach Anspruch 1, dadurch gekennzeichnet, daß zur wahlweisen Behandlung des Daumens oder der vier Finger die Bewegungskinematik (2) von der einen Seite der Hand auf die andere verstellbar ist.
- Therapiegerät nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das mit dem Schwenkhebel (5) verbundene, zumindest einen Fingeranschluß aufweisende Anschlußteil (7) wenigstens ein Querschwenkgelenk (25) hat, dessen Schwenkachse (24) quer, insbesondere etwa rechtwinklig zur Längserstreckung des damit verbundenen Schwenkhebels (5) und den Schwenkhebel-Schwenkachsen (20,21,22) verläuft.
- Therapiegerät nach Anspruch 3, dadurch gekennzeichnet, daß das Anschlußteil (7) zur Bewegung der Finger einschließlich des Daumens als Gelenk-Kette (23) mit bezüglich ihren Schwenkachsen parallel verlaufenden Querschwenkgelenken ausgebildet ist, an der beabstandet zueinander ein oder mehrere zueinander beabstandete Fingeranschlüsse vorgesehen sind.
- Therapiegerät nach Anspruch 4, dadurch gekennzeichnet, daß die Gelenk-Kette (23) länger ist als die Breite der direkt nebeneinander befindlichen vier Finger der Hand.
- Therapiegerät nach Anspruch 4 oder 5, dadurch gekennzeichnet, daß die Gelenk-Kette (23) mittels Schnelltrennverbindungen im Bereich der Querschwenkgelenke verbundene Kettenglieder aufweist und daß die Fingeranschlüsse vorzugsweise trennbar mit den Kettengliedern verbindbar sind.
- Therapiegerät nach einem der Ansprüche 4 bis 6, dadurch gekennzeichnet, daß mehrere auswechselbare Anschlußteile (7) vorzugsweise in Form unterschiedlich langer Gelenk-Ketten (23) vorgesehen sind.
- Therapiegerät nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, daß drei miteinander verschwenkbar verbundene Schwenkhebel (3,4,5) in Funktionsstellung seitlich neben der zu behandelnden Hand mit ihren Schwenkachsen (20,21,22) etwa in Verlängerung der Gelenkachsen zumindest des Handgelenkes (9), der Fingergrundgelenke (16) und gegebenenfalls mit einem oder mehreren der Fingermittelgelenke (17) angeordnet sind.
- Therapiegerät nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, daß von den Schwenkhebeln (3,4,5) ein erster, an der Halterung, insbesondere dem Bewegungsantrieb (6) angebrachter Basis-Schwenkhebel (3) vorgesehen ist, der verschwenkbar und arretierbar ist und daß ein Mittel-Schwenkhebel (4) und ein End-Schwenkhebel (5) von dem Bewegungsantrieb (6) motorisch angetrieben sind.
- Therapiegerät nach Anspruch 9, dadurch gekennzeichnet, daß der Mittel-Schwenkhebel (4) und der End-Schwenkhebel (5) mittels Zahnriemen oder Stirnrädern als Getriebeelementen von dem Bewegungsantrieb (6) motorisch angetrieben sind und daß die Getriebeelemente innerhalb der Schwenkhebel-Umrißkontur untergebracht sind.
- Therapiegerät nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, daß der Antrieb des Mittel-Schwenkhebels (4) wahlweise zu- und abschaltbar und der Mittel-Schwenkhebel (4) in antriebsabgekuppelter Lage bezüglich seiner Schwenkposition einstellbar und arretierbar ist.
- Therapiegerät nach einem der Ansprüche 1 bis 11, dadurch gekennzeichnet, daß die Halterung (8) insbesondere mittig an ihrer Unterschale (10) einen in Längsrichtung der Unterschale (10) verlaufenen, hülsenförmigen Klemmhalter (13) zur längsverschiebbaren und drehbaren Aufnahme einer Welle eines L-förmigen Wendeteiles (14) aufweist, das einen Querträger (15) hat, an dessen freiem Ende der Bewegungsantrieb mit den Schwenkhebeln (3,4,5) angebracht sind.
- Therapiegerät nach Anspruch 12, dadurch gekennzeichnet, daß der am Querträger (15) angebrachte Bewegungsantrieb (6) mit Schwenkhebeln (3,4,5) um eine parallel zur Längserstreckung der gestreckten Schwenkhebel (3,4,5) verlaufende Achse verdrehbar und arretierbar angebracht ist.
- Therapiegerät nach einem der Ansprüche 1 bis 13, dadurch gekennzeichnet, daß die Halterung (8) so einstellbar ist, daß die Schwenkachse des Mittel-Schwenkhebels (3) die Gelenkachse des Handgelenkes (9) etwa mittig und quer, insbesondere etwa rechtwinklig schneidet, oder daß die Schwenkachse des Basis-Schwenkhebels (3) die Gelenkachse des Handgelenkes (9) etwa mittig und quer, insbesondere etwa rechtwinklig schneidet und daß im letzteren Fall zumindest der Basis-Schwenkhebel (3) für eine Schwenkbewegung motorisch angetrieben ist und die weiteren Schwenkhebel gegebenenfalls festlegbar sind und daß für eine Anund Abspreizbewegung der Hand ein Handgriff als Anschlußteil (7) vorgesehen ist.
- Therapiegerät nach einem der Ansprüche 1 bis 14, dadurch gekennzeichnet, daß die von der einen Handseite auf die andere umstellbare Bewegungskinematik (2) in beliebige Zwischenstellungen im Bereich zwischen der Lage auf der einen Seite der Hand und der Lage auf der anderen Seite der Hand verstellbar ist.
- Therapiegerät nach einem der Ansprüche 1 bis 15, dadurch gekennzeichnet, daß an dem End-Schwenkhebel (5) eine Klemmhülse (26) oder dergleichen Halter zur Aufnahme eines Anschlußstückes der unterschiedlichen Anschlußteile (7) vorgesehen ist.
- Therapiegerät nach einem der Ansprüche 1 bis 16, dadurch gekennzeichnet, daß der ein Anschlußteil (7) bildende Handgriff (32) mit einem Spannriemen oder Gurt (33) zum Sichern der Hand versehen ist.
- Therapiegerät nach einem der Ansprüche 1 bis 17, dadurch gekennzeichnet, daß die Fingeranschlüsse durch auf die Fingerkuppen aufschiebbare, insbesondere elastische Fingerringe (19) gebildet sind.
