Abstract
Description
- Die Erfindung bezieht sich auf eine Vorrichtung für die aktive und/oder passive Beinbewegung mit einem Tretlager, einem diesen tragenden Bodengestell, je einer auf beiden Seiten des Tretlagers angeordneten Kurbel mit Pedalen und mit einer oberhalb des Tretlagers befindlichen, vom Bodengestell ausgehenden sowie mit zwei Handgriffen endigenden Griffstange.
- Man kennt derartige Vorrichtungen als Trainingsgeräte, die nach dem Vorbild eines Fahrrades, jedoch ohne Räder, ausgeführt sind, wobei für den Fall einer aktiven Beinbewegung der Tretwi-derstand oft noch einstellbar ist. Diese Geräte besitzen demgemäß entsprechend dem Aufbau eines Fahrrades oberhalb des Tretlagers einen Sattel und eine starre Lenkstange, die dem Benutzer das Festhalten gestattet. Derartige Vorrichtungen sind indes bei verschiedenen Erkrankungen, die den Patienten an einen Rollstuhl binden, nicht anwendbar. Vor allem scheiden sie für die Beinbewegung bei Querschnittsgelähmten und bei Multiple-Skerose-Kranken aus, bei welchen lediglich eine passive Beinbewegung vorgenommen werden kann, und die darüber hinaus auch weitergehend in ihrer Beweglichkeit eingeschränkt sind. Andererseits ist der therapeutische Wert einer wenn auch nur passiven Beinbewegung unbestreitbar, weil es dadurch gelingt, die Rückbildung der Beinmuskulatur zu verzögern oder sogar zu verhindern. Daneben gibt es auch für die Heilungsphase nach Verletzungen der Wirbelsäule und nach Beinbrüchen die Möglichkeit der aktiven Beinbewegung mit gegebenenfalls variabler Belastung, für welche bekannten Vorrichtungen oft gleichfalls noch nicht geeignet sind, so daß mit der aktiven Beinbewegung erst zu einem verhältnismäßig späten Zeitpunkt begonnen werden kann.
- Diese Aufgabenstellung wird durch die Erfindung, wie sie in den Patentansprüchen gekennzeichnet ist, gelöst. Auf diese Weise kann ein Patient die Pedalen des Tretlagers unmittelbar von einem Rollstuhl aus erreichen. Vor allem besteht dank des die Beine getrennt haltenden Körpers, mit welchem die Griffstange verkleidet ist, in keinem Falle die Gefahr, daß sich die Beine und insbesondere die Knie gegenseitigen berühren. Die Beine können zwar an den glatten Wandungen des sie trennenden Körpers anliegen, jedoch können hierdurch keine Beschädigungen eintreten. Dies ist vor allem deshalb wichtig, weil im Falle der passiven Beinbewegung, bei welcher die Pedalkurbelwelle motorisch angetrieben ist, sonst leicht Unfälle eintreten können, die auf unkontrollierte Beinbewegungen zurückgehen.
- Bei einer Ausführungsform der Erfindung kann der Körper in Gestalt einer Verkleidung auch bereits vom Bodengestell ausgehen. In diesem Fall ist der Querschnitt dort durch die Länge des Bodengestells einerseits und durch eine Breite festgelegt, die durch den Austritt der Pedalkurbelwelle gekennzeichnet ist. Somit sind die Beine auch im Bereich der Unterschenkel voneinander durch den verkleidenden Körper getrennt. Zweckmäßig verjüngt sich dabei der Querschnitt in Richtung nach oben.
- In vielen Fällen ist es jedoch zweckmäßiger, wenn der verkleidende Körper in einen oberen Abschnitt und in einen davon beabstandeten, unteren Abschnitt aufgeteilt ist, welche beiden Abschnitte selbständig sind und durch die Griffstange bzw. durch ein vom Bodengestell ausgehendes Rohr miteinander verbunden sind. Eine solche Gestaltung läßt es nämlich zu, den oberen Abschnitt gegebenenfalls auch getrennt anzuschließen, so daß der untere Abschnitt alleine bei bettlägerigen Patienten beispielsweise im Bereich des Fußendes eines Bettes aufgestellt werden kann, ohne daß dabei der dann nicht benötigte obere Abschnitt stört. Die von der Grundfläche ausgehende Verkleidung bzw. der von dort ausgehende Abschnitt des verkleidenden Körpers sind im übrigen vorteilhaft schräg nach oben in Richtung auf den Benutzer geneigt.
- Dem Erfordernis nach glatten Wandungen wird in besonderem Maße dadurch Rechnung getragen, daß die Verkleidung bzw. der Körper und seine Abschnitte aus Kunststoff, zweckmäßig aus Polymethacrylsäuremethylester, bestehen.
- Um eine beim Drehen der Kurbel stets optimale Haltung der Füße sicherzustellen, sind die Pedalen in weiterer Ausgestaltung der Erfindung zweckmäßig unterhalb ihres Drehpunktes hängend an den Kurbeln angeordnet.
- Der Antrieb läßt sich mittels eines Motors vorsehen, der zweckmäßig mit variabler Drehzahl zu betreiben ist. Diese Drehzahleinstellung läßt sich leicht bewirken, indem ein Dimmer verwendet wird. Dieser kann auf einer ebenen Platte angebracht sein, mit welchem die Verkleidung bzw. der obere Abschnitt des Körpers in Höhe der Handgriffe abgeschlossen ist. Eine derartige Platte kann im übrigen auch Handschalter für die Inbetriebnahme des Motors tragen. Es ist aber auch möglich, einen der Handgriffe drehbar auszuführen und auf diese Weise über einen Bowdenzug die Einstellung der Drehzahl des Motors vorzunehmen.
- Aus Gründen der Betriebssicherheit ist es von beträchtlichem Vorteil, wenn der Motor die Pedalkurbelwelle über ein Schneckengetriebe und eine Rutschkupplung antreibt, die weiterhin mittels eines Bowdenzuges von der Griffstange her betätigbar ist.
- Die Zugänglichkeit der verschiedenen Einstellmittel für den Patienten ist dadurch gegeben, daß sich die Verkleidung bzw. die Abschnitte der Körper, von der Grundfläche ausgehend, schräg nach oben in Richtung auf den Benutzer geneigt erstrecken. Somit können auch in ihren Bewegungen weitgehend Behinderte die neue Vorrichtung selbst bedienen.
- Eine weitere Anpassung an die Bewegungsvorgänge der Beine wird noch dadurch erreicht, daß die seitlichen Wände der Verkleidung bzw. des Körpers im Bereich der Knie des Benutzers nach innen eingewölbt sind.
- Um die Vorrichtung mühelos an den Benutzer heranbringen und nach Gebrauch wieder von ihm weg transportieren zu können, ist das Bodengestell entlang seiner vorderen oder entlang seiner hinteren Kante mit einer Rolle derart versehen, daß diese in Kippstellung der Vorrichtung Berührung mit dem Boden erlangt.
- Erhebliche Bedeutung hat noch die Standsicherheit der Vorrichtung. Sie wird dadurch stabilisiert, daß das Bodengestell die Verkleidung bzw. den unteren Abschnitt des Körpers beidseitig überragende, vordere und hintere Auslegerstützen für den Bodenkontakt aufweist.
- Schließlich läßt sich noch eine Gefährdung der Füße des Patienten bei den Bewegungen der Pedale dadurch vermeiden, daß letzere aus Fußplatten mit seitlich hochgezogenen Rändern bestehen, die eine Berührung der Fußränder mit den sich bewegenden Teilen ausschließen.
- Nachstehend wird die Erfindung anhand von sich auf Ausführungsbeispiele beziehenden Zeichnungen näher erläutert. Darin zeigen:
- Figur 1 eine perspektivische Darstellung der erfindungsgemäßen Vorrichtung in schräger Vorderansicht,
- Figur 2 eine perspektivische Darstellung der erfindungsgemäßen Vorrichtung in schräger Hinteransicht,
- Figur 3 eine Querschnittsdarstellung des Getriebes der neuen Vorrichtung,
- Figur 4 eine teilweise Querschnittsdarstellung einer Pedalanordnung,
- Figur 5 eine andere Ausführungsart der erfindungsgemäßen Vorrichtung in geschnittener seitlicher Ansicht und
- Figur 6 die Ausführungsart gemäß Figur 5 in Draufsicht.
- Die Figuren 1 und 2 zeigen die aus dem unteren Abschnitt 32 des verkleidenden Körpers austretende Pedalkurbelwelle 11, an die sich zu beiden Seiten eine Kurbel 1 anschließt. Am endseitigen Drehpunkt 8 der Kurbeln 1 sind die Pedale 4 bzw. 4' hängend angeordnet. Im Innern des verkleidenden Abschnittes 33 befindet sich weiterhin noch ein später zu beschreibender Antriebsmotor mit Kupplung und Getriebe, so daß die Pedalkurbelwelle 11 motorisch bewegbar ist.
- Der untere Abschnitt 33 wird mittels der zu beiden Seiten austretenden Aulegerstützen 34 fest auf dem Boden gehalten. An der vorderen Kante erkennt man eine Rolle 26, die normalerweise ohne Bodenberührung ist. Wenn jedoch die Vorrichtung nach vorne gekippt wird, erlangt die genannte Rolle 26 Bodenberührung, so daß ein leichtes Verfahren möglich ist.
- Vom Abschnitt 33 erstreckt sich das Rohr 23 nach oben, um mit etwas Abstand vom unteren Abschnitt den oberen Abschnitt 32 des verkleidenden Körpers zu tragen. Dieser obere Abschnitt ist insbesondere so gestaltet, daß er zwischen den Knien des Patienten eine Trennung bewirkt. Die glatten Wandungen 29, mit denen dieser obere Abschnitt 32 im wesentlichen verschalt ist, lassen ein leichtes Entlanggleiten der Knie zu, so daß auch Verletzungen unterbleiben. Die Längserstreckung 30 muß also so bemessen sein, daß der Abschnitt 32 sich zwischen den Knien befindet. Von verhältnismäßig geringer Ausdehnung ist die Quererstreckung 31. In Nähe der vorderen oberen Kante durchdringen die Handgriffe 6 und 6' den Abschnitt 32. Einer der Handgriffe ist drehbar und erlaubt über einen Bowdenzug die Einstellung der Drehzahl des hier nicht gezeichneten Motors. Oben ist der Abschnitt 32 von der im wesentlichen ebenen Platte 14 verschlossen, die, wie Figur 2 zeigt, noch Schalter und Anzeigegeräte tragen kann. Der Benutzer sitzt in der Regel hinter der Vorrichtung, so daß seine Hände bei ausgestreckten Armen die Handgriffe 6 und 6' umschließen können. Die Füße werden in die Pedale 4 so gestellt, daß die Zehen zum offenen Ende der Pedalen weisen. Der hochgezogene Rand 35 schützt die Fußränder und gegebenenfalls auch die Knöchel vor unbeabsichtigten Berührungen.
- Die bereits erwähnten, endseitigen Drehpunkte 8 der Kurbeln 1 tragen, wie Figur 4 erkennen läßt, mittels vorstehender Bolzen Radialkugellager 7, mit deren Hilfe die Pedale 4 hängend angeordnet sind. Die Pedalkurbelwelle 11, an welcher die Kurbeln 1 endseitig befestigt sind, wird gemäß Figur 3 über die Schnecke 18 und das damit im Eingriff stehende Zahnrad 19 und schließlich die Rutschkupplung 20 angetrieben. Für die Rutschkuppelung besteht ein Bowdenzug 21, der durch das Rohr 23 nach oben führt, so daß er von dort betätigbar ist. Wie Figur 3 weiterhin zeigt, ist die gesamte Anordnung in einem Gehäuse untergebracht, so daß Getriebe und Kuppelung einerseits nicht verschmutzen können, und daß andererseits aber auch kein Ö1 bzw. keine Schmierstoffe austreten können.
- Die weitere Ausgestaltungsform der Erfindung gemäß den Figuren 5 und 6 zeigen, wie das oben verlaufende Rohr 23 mit Stützen 24 ein Bodengestell ausbildet. Ferner zeigt Figur 5 die Lage des Motors 3, der an das Tretlager 2 angeschlossen ist und dessen Abtrieb von der bereits erwähnten Schnecke 18 gebildet ist. Für das Bodengestell besteht die Grundfläche 13, von welcher sich die Verkleidung 5 schräg nach oben verlaufend erstreckt. Sie schließt den gesamten, vom Tretlager 2 einschließlich seines Motorgetriebes eingenommenen Raum im unteren Teil in sich ein und verkleidet sodann das Rohr 23 noch bis zur Höhe 12, im Anschluß an welche die im wesentlichen ebene Platte 24 die Verkleidung abschließt. Die im Bereich der Grundfläche 13 bestehende Breite 10 und Länge 9 erfahren nach oben zu eine fortschreitende Verjüngung. Durch die obere Platte 14 ist die Griffstange 22 herausgeführt, an deren beiden Enden sich die Handgriffe 6, 6' befinden. Einer dieser Handgriffe wird zweckmäßig mit einem Bowdenzug für die Antriebsbedienung versehen. ZusätzlicheSchalter 15, 16 und 17 auf der Platte 14 sind von beiden Handgriffen aus leicht erreichbar. Einer der erwähnten Schalter läßt sich als Dimmer ausführen, der die Einstellung einer variablen Drehzahl des Motors 3 erlaubt.
- Bei dieser Ausführungsform wird zur Verbesserung der Standfestigkeit das Bodengestell im Bereich seiner Grundfläche 13 mit einer Standplatte 27 ausgebildet. An deren hinterer Kante erkennt man gemäß Figur 5 eine Rolle 26, mittels welcher die Vorrichtung in Kipplage verfahrbar ist.
Claims (16)
dadurch gekennzeichnet,
daß die Griffstange (22) bis etwa zur Höhe der Handgriffe (6, 6') mit einem Körper verkleidet ist, der im wesentlichen mit in zur Richtung der Pedalkurbelwelle (11) senkrechten Ebenen liegenden, sich abwärts und zur Seite bis in den Kniebereich des Benutzers hin erstreckenden, glatten Wandungen (29) verschalt ist und einen in horizontaler Ebene flachen Querschnitt aufweist, dessen Längserstreckung (30) erheblich größer als seine Quererstreckung (31) ist, wobei seine Quererstreckung (31) etwa parallel zur Richtung der Pedalkurbelwelle (11) und die Längserstreckung etwa senkrecht zur Richtung der Pedalkurbelwelle (11) verläuft.
dadurch gekennzeichnet,
daß der Körper in Gestalt einer Verkleidung (5) vom Bodengestell (23, 24) ausgeht, die dort von einem sich über dessen ganze Länge (9) und dessen bis zum Austritt der Pedalkurbelwelle (11) reichende Breite (10) erstreckendem Querschnitt ist.
dadurch gekennzeichnet,
daß der Querschnitt der Verkleidung (5) sich von der Grundfläche (13) ausgehend in Richtung nach oben verjüngt.
dadurch gekennzeichnet,
daß der verkleidende Körper in einen oberen, selbständigen Abschnitt (32) und einen davon beabstandeten unteren Abschnitt (33) aufgeteilt ist, die durch die Griffstange bzw. durch ein vom Bodengestell ausgehendes Rohr (23) miteinander verbunden sind.
dadurch gekennzeichnet,
daß die Verkleidung (5) unterhalb der Handgriffe (6, 6') mit einer ebenen Platte (14) abgeschlossen ist, die bei Ausführung der Vorrichtung mit einem Bewegungsmotor (3), Handschalter (15, 16, 17) für dessen Inbetriebnahme anzubringen gestattet.
dadurch gekennzeichnet,
daß sich die Verkleidung (5) von der Grundfläche (13) ausgehend schräg nach oben in Richtung auf den Benutzer geneigt erstreckt.
dadurch gekennzeichnet,
daß die Verkleidung (5) bzw. der Körper und dessen Abschnitte (32, 33) aus Kunststoff geformt sind.
dadurch gekennzeichnet,
daß die Verkleidung (5) bzw. der Körper und dessen Abschnitte (32, 33) aus Polymethacrylsäuremethylester bestehen.
dadurch gekennzeichnet,
daß die Pedalen (4, 4') unterhalb ihres Drehpunktes (8) hängend an den Kurbeln (1) angeordnet sind.
dadurch gekennzeichnet,
daß der Motor (3) mit variabler Drehzahl zu betreiben ist, und daß die Drehzahlsteuerung des Motors mittels eines an der Platte (14) befindlichen Dimmers einstellbar ist.
dadurch gekennzeichnet,
daß der Motor (3) mit variabler Drehzahl zu betreiben ist, und daß die Drehzahlsteuerung des Motors mittels eines drehbar ausgeführten Handgriffes (6') einstellbar ist.
dadurch gekennzeichnet,
daß der Motor (3) die Pedalkurbelwelle (11) über ein Schneckengetriebe und eine Rutschkupplung (20) antreibt, für die ein Bowdenzug (21) vonder Griffstange (22) her betätigbar ist.
dadurch gekennzeichnet,
daß die seitlichen Wände der Verkleidung (5) im Bereich der Knie des Benutzers nach innen eingewölbt sind.
daß das Bodengestell entlang einer seiner Kanten mit
Rollen (26) derart versehen ist, daß diese in Kippstellung der Vorrichtung Bodenberührung haben.
daß das Bodengestell die Verkleidung (5) beidseitig
überragende, vordere und hintere Auslegerstützen (34) für den Bodenkontakt aufweist.
daß die Pedale (4, 4') aus Fußplatten mit seitlich hochgezogenen Rändern (36) bestehen.
