Fra EP 0052072 Al, EP 0105838 og US 4.115.409 kendes beslægtede forbindelser, som dog adskiller sig strukturelt fra de 20 foreliggende forbindelser. Således har forbindelserne i EP 0052072 Al en rent carbocyklisk gruppe endestillet i amino-propoxysidekæden. Det har overraskende vist sig, at forbindelserne ifølge opfindelsen har en forøget forkammer/luft-rørs-selektivitet i forhold til de kendte forbindelser.

En foretrukken klasse er forbindelser, i hvilke Ab, Rb1, Rb2 og Rb er som defineret ovenfor og 30 W er en binding, og X er imino, og fysiologisk hydrolyserbare estere deraf med mindst én hydroxygruppe i esterificeret form.
En yderligere foretrukken klasse er forbindelser, i hvilke 35 Rblf Ab, Rb og Rb2 er som defineret ovenfor, og 4 DK 171484 B1 W1 er imino eller en binding, og X er imino eller en binding, og fysiologisk hydrolyserbare estere deraf med mindet én hy-droxygruppe i esterificeret form.
5 Fysiologisk hydrolyserbare estere er derivater, i hvilke de esterificerede hydroxygrupper under fysiologiske betingelser deesterificeres til dannelse af den tilsvarende hydroxyfor-bindelse.
Der foretrækkes forbindelser, i hvilke hydroxygruppen i 10 3-aminopropoxysidekædens 2-stilling er i uesterificeret form.
Opfindelsen angår også en fremgangsmåde til fremstilling af forbindelserne med formlen I, ved hvilken fremgangsmåde en tilsvarende forbindelse med den almene formel II

Fremgangsmåden ifølge opfindelsen kan udføres på konventionel måde til fremstilling af analoge 3-amino-2-oxypropoxyarylfor-bindelser. Valget af de mest hensigtsmæssige varianter bør 20 naturligvis tage de tilstedeværende substituenters reaktiviteter i betragtning.
En precursorform af forbindelsen med formlen II er en forbindelse, der er i stand til at blive omdannet til en forbindelse med formlen II. Fx ved hensigtsmæssig etherifi-25 cering, aromatisk substitution og/eller afbeskyttelse; hvis R2 er hydroxysubstitueret phenyl, fortrinsvis i beskyttet form.

Fremgangsmådetrinet ifølge opfindelsen kan således udføres i mere end ét trin. Fx kan der anvendes en forbindelse med formlen II i beskyttet form, eller der kan indføres en 3-amino- 3- oxopr opy Idel i beskyttet form, og efter at 3-amino-2-5 oxopropylering er blevet effektueret, kan den tilstedeværende beskyttelsesgruppe spaltes fra.
Benzyl, methyl eller 2-tetrahydropyranyl, men fortrinsvis benzyl, er eksempler på en beskyttelsesgruppe på fx en hy-droxysubstitueret ring.
10 I én form af fremgangsmåden ifølge opfindelsen udføres 3-amino-2-oxopropyleringen i to hovedproces trin.
I det første proces trin indføres en gruppe -CH2Rx, hvor Rx er en gruppe, som er i stand til at reagere med en primær amin til dannelse af en 2-amino-l-hydroxyethylgruppe, ved hjælp af 15 O-alkylering i 1-stillingen i en forbindelse med formlen II til dannelse af den tilsvarende forbindelse med formlen III

Alkyleringsprocestrinet i 1-stillingen kan udføres på kendt måde til fremstilling af analoge ethere. En forbindelse med formlen II omsættes fortrinsvis i ionisk form. Amineringspro-cestrinet kan udføres på konventionel måde til fremstilling 5 af analoge 3-amino-2-hydroxypropoxyarylforbindelser. Fx kan
Rx Λ være en gruppe med formlen -CH-CH2 eller et derivat af denne gruppe, fx en gruppe med formlen CHOHCH2L, hvor L er chlor, 10 brom eller en gruppe RyS020, hvor Ry er phenyl, tolyl eller alkyl. L er især chlor. Reaktionen udføres fortrinsvis i ethanol eller i en passende ether såsom dioxan. Der kan eventuelt anvendes et overskud af aminen som opløsningsmiddel. Alternativt kan reaktionen udføres i en smelte. Passende 15 reaktionstemperatur kan være fra ca. 20 til ca. 200°C, hensigtsmæssigt reaktionsblandingens tilbagesvalingstemperatur, hvis der anvendes et opløsningsmiddel.
Den fakultative esterificering i en resulterende forbindelse med formlen I kan udføres på kendt måde til fremstilling af 20 analoge estere, om nødvendigt under anvendelse af selektive betingelser, hvis der er andre reaktive grupper til stede, fx hydroxy eller amino.
Forbindelserne ifølge opfindelsen kan eksistere i fri form, dvs. normalt som en base eller i saltform. Frie former af 25 forbindelserne ifølge opfindelsen kan omdannes til saltformer. Fx syreadditionssaltformer og omvendt på konventionel måde. Hensigtsmæssige syrer til syreadditionssaltdannelse omfatter saltsyre, malonsyre, ravsyre, oxalsyre og fumarsyre.
I forbindelserne ifølge opfindelsen er carbonatomet i fx 30 3-aminopropoxysidekædens 2-stilling asymmetrisk substitueret,
og hvis A er -CH(CH-,)CHo-, er a-carbonatomet i a J
alkylensidekæden også asymmetrisk substitueret. Forbindelserne kan således eksistere i racemiske former eller individuel 7 DK 171484 B1 le optiske isomerformer. Den foretrukne optiske isomer har S-konfiguration på det asymmetrisk substituerede carbonatom i 3-aminopropoxysidekæden, og ved forgrening på alkylenside-kæden, har den foretrukne optiske isomer R-konfigu^ation.
5 Individuelle optiske isomerformer kan fås på konventionel måde, fx ved at anvende optisk aktive udgangsmaterialer eller ved fraktioneret krystallisation af racematsalte under anvendelse af optisk aktive syrer.
For så vidt fremstilling af eventuelle specifikke udgangs-10 materialer ikke er specifikt beskrevet, kan denne udføres på konventionel måde. Opfindelsen angår også et farmaceutisk præparat, der omfatter en forbindelse ifølge opfindelsen i fri form eller i farmaceutisk acceptabel saltform i kombination med en farmaceutisk acceptabel bærer eller diluent. I 15 nedenstående eksempler er alle temperaturangivelser ukorrigerede .
EKSEMPEL 1 N-[2-[3-[2-Cyano-4-(2-propoxyethoxy)phenoxy]-2-hydroxypropyl-amino]-ethyl]-Ν' -(lH-indol-5-yl)-urea 20 4,07 g 2-(2,3-epoxypropoxy)-5-(2-propoxyethoxy)benzonitril, 3,2 g N-(2-aminoethyl)-Ν' -(lH-indol-5-yl)urea og 200 ml methanol tilbagesvales sammen i 5 timer. Efter fjernelse af opløsningsmidlet ved inddampning oprenses den skumagtige remanens ved silicagelchromatografi under reduceret tryk (0,5 25 bar) under anvendelse af ammoniak/methanol/dichlormethan (1:9:90) som elueringsfase. Efter omkrystallisation af ethyl-acetat/ether er smeltepunktet 122-124°C.
Udgangsmaterialerne fås på følgende måde: a) 5 g lH-indol-5-amin opløses i 50 ml pyridin, og under 30 kraftig omrøring sættes der dråbevis 6,1 g chloreddike- syrephenylester til den ved 0-5°C holdte blanding.
Efter 3 timers omrøring ved stuetemperatur fjernes 8 DK 171484 B1 pyridinet ved vakuumdestillation ved 50°C, og remanensen blandes med isvand og methylenchlorid/isopropanol (3:1). Efter tørring af den organiske fase med magnesiumsulfat og fjernelse af opløsningsmidlet ved ind-5 dampning fås den olieagtige lH-indol-5-carbamino- syrephenylester (anvendt uden yderligere oprensning).
b) 9,2 g lH-indol-5-carbaminosyrephenylester, 12 ml 1,2- diethylamin og 70 ml dioxan omsættes med hinanden under omrøring i 2 timer i et oliebad ved 100°C. Efter af-10 køling og fjernelse af opløsningsmiddel blandes rema nensen med vinsyreopløsning og ethylacetat. Den vandige fase gøres alkalisk ved hjælp af NaOH-opløsning og ekstraheres med dichlormethan/isopropanol (2:1). Ekstrakten tørres over magnesiumsulfat og inddampes. Den 15 krystallinske remanens oprenses på silicagel under reduceret tryk (0,5 bar) under anvendelse af koncentreret ammoniak/isopropanol/ethylacetat (5:35:45).
Klare fraktioner omkrystalliseres af methanol/-ether, hvilket giver N-(2-aminoethyl)-N'-(lH-indol-5-yl)-urea 20 (smeltepunkt 156-158°C).

* Det nødvendige mellemprodukt IH-indol-6-eddikesyre- 25 ethylester fås ved omsætning af ethyliodid med 1-indol- 6-eddikesyreethylester i tetrahydrofuran i nærværelse af 1,8-diazabiazalo[5,4,0]undec-7-an (1,5-5).
** Det nødvendige mellemprodukt 2,3-dihydro-2-oxo-lH- indol-5-eddikesyremethylester fås ved omsætning af 5-30 methoxymethyl-lH-indol-2(3H)-on med saltsyre efterfulgt af omsætning af det vundne chlorid med kaliumcya-nid/vand og forsæbning af 2,3-dihydro-2- oxo-(IH-indol- 5-acetonitril) med kaliumhydroxid i ethanol og esteri-ficering af den vundne 2,3-dihydro-2-oxo-lH-indol-5-35 eddikesyre med methanol i svovlsyre.
EKSEMPEL 38 15 DK 171484 B1 4-[3-[2-Cyano-4-(2-propoxyethoxy)phenoxy]-2-hydroxypropy1amino ] -N- (IH-indol-5-yl)butanamid 3,2 g 2-(2,3-epoxypropoxy)-5-(2-propoxyethoxy)benzonitril, 5 2,5 g 4-amino-N-(IH-indol-5-yl)butanamid og 160 ml methanol blev tilbagesvalet sammen i 4 timer. Efter fjernelse af opløsningsmidlet blev remanensen oprenset over silicagel over reduceret tryk (0,5 bar) under anvendelse af koncentreret ammoniak/methanol/dichlormethan (1:14:85) som elueringsop-10 løsningsmiddel. De klare fraktioner omkrystalliseres af methanol/ether, hvilket giver titelforbindelsen (smeltepunkt - 98-100°C).
Udgangsmaterialerne fås på følgende måde: (a) 3 g 4-aminosmørsyre opløses i 2,33 g natriumhydroxid og 15 30 ml vand. Efter tilsætning af 30 ml chloroform af køles blandingen til 10°C, og under kraftig omrøring tilsættes der dråbevis 5,46 g chloreddikesyrebenzyl-ester. Efter henstand under omrøring ved stuetemperatur natten over tilsættes der dichlormethan, og blandingen 20 får lov at dele sig. Den vandige fase mættes med natri- umchlorid og ekstraheres med methylenchlorid. Efter tørring over magnesiumsulfat fjernes opløsningsmidlet ved inddampning i vakuum, hvilket giver den olieagtige 4- (benzyloxycarbamoyl-amino)smørsyre.
25 (b) 6,76 g 4-(benzyloxy-carbonylamino)smørsyre opløses i 70 ml dichlormethan og omsættes med 2,94 g dicyclo-hexylcarbodiimid ved stuetemperatur. Efter 2 timers omrøring filtreres bundfaldet fra, vaskes med 25 ml dichlormethan og omsættes med 3,6 g natriumhydrogen-30 carbonat og 3,44 g 5-aminoindol. Efter 17 timers omrø ring ved stuetemperatur tilsættes der 150 ml vand, og efter 1/2 times kraftig omrøring fjernes dichlormethan-fasen, og den vandige fase genekstraheres endnu to 16 DK 171484 B1 gange med dichlormethan, som til sidst tørres over magnesiumsulfat. Efter fjernelse af opløsningsmidlet oprenses den olieagtige remanens over silicagel under reduceret tryk (0,5 bar) under anvendelse af toluen/et-5 hylacetat (2:1), hvilket giver 4-(benzyloxycarbonylami- no)-N-(IH-indol-5-yl)butanamid (olie).

Forbindelserne ifølge opfindelsen i fri form eller som 20 fysiologisk acceptabel salte deraf har enestående farmakologisk virkning og kan anvendes som terapeutiske midler til mennesker.
Forbindelserne har især Ø-adrenoceptorblokerende virkning som vist ved standardtests, fx i spontant bankende 25 marsvinehjerteforkamre (A. Bertolet et al., Postgradua- 17 DK 171484 B1 te Medical Journal. 57. Suppl. 1, 9-17, 1981). De inhiberer den positive kronotropiske isoproterenolef-fekt ved badkoncentrationer på fra ca. 10'9 til 10'8 mol/liter.

Betaxolol 10 20 Forbindelserne er særlig nyttige som Ø-adrenoceptorblo-kerende midler og kan derfor anvendes ved profylakse og terapi af sygdomme, som er almindelig kendte for at respondere på blokade af β-adrenoceptorer såsom dem, der findes i hjertet. Eksempler på sådanne sygdomme er 25 hypertension, angina pectoris, thyrotoxicose, migræne, glaucoma og til behandling af forstyrrelser i hjerterytmen såsom superventrikulær tachykardi.
Endvidere har forbindelserne mere udtalte og vidtspred-te gunstige farmakologiske egenskaber, end der ville 30 forventes for forbindelser med denne type struktur.

Især er deres virkning mere cardioselektiv end hidtil kendt for lignende forbindelser.
Dette kan påvises in vivo i trachealpræparationer fra marsvin, som fremstilles ifølge standardprocedurer, i 5 hvilke en del af trachealmusklen tillades at slappe af under indflydelse af isoproterenol og i nærværelse af kendte koncentrationer af den forbindelse, der skal afprøves. Udvalgte eksempler er vist nedenfor:

Atenolol 3 x 10'6 25 Propranolol 5 x 10'9 I disse præparationer er potensen af alle eksempler meget mindre end i forkammerpræparationerne beskrevet ovenfor, og de fleste har ingen blokerende virkning i 30 tracheapræparatet ved koncentrationer på 1 x 10*5 molær, og for mange forbindelser kræves der meget højere koncentrationer end 1 x 10'5 molær for at udvise blokerende virkning i trachea, hvilket indikerer, at disse forbindelser i praksis har fuldstændig selektivitet for 20 DK 171484 B1 forbindelser i praksis har fuldstændig selektivitet for de cardiale (atriale) β-adrenoceptorer. I modsætning hertil har de cardioselektive antagonister betaxolol og atenolol signifikante blokeringsvirkninger på isopro-5 terenolresponset i trachea. Selektivitetsindexer for de beskrevne forbindelser er for det meste umulige at beregne på grund af deres cardiospecificitet. For de mere potente forbindelser, fx eksemplerne l og 4, er selektiviteterne henholdsvis 17.000 og 45.000, medens 10 selektiviteterne for betaxolol og atenolol er henholdsvis 29 og 20. Dette indikerer, at de beskrevne eksempler i praksis er yderst potente cardiospecifikke Ø-ad-renoceptorantagonister.
I præparerede rotter, der har fået rygmarven skåret 15 over, kan mange af forbindelserne fremkalde en 100%'s inhibering af virkningerne af isoproterenol administreret i en dosis på 0,1 /xg/kg intravenøst på hjerterytmen, men inhiberer ikke faldet i blodtryk fremkaldt af isoproterenol ved doser så høje som 300 ^g/kg intra-20 venøst. Dette fald i blodtryk reflekterer i stor udstrækning en β2-virkning af isoproterenol på de perifere blodkar og er et yderligere bevis på den høje /3-^ selektivitet hos forbindelserne in vivo.
Den høje blokeringsselektivitet for disse forbindelser 25 er af stor vigtighed ved behandling af hypertension, hvor for tiden kommercielt tilgængelige forbindelser kan fremskynde en forværring af en eksisterende astmatisk tilstand. Høj selektive forbindelser ville forventes at have færre ugunstige metaboliske virkninger 30 såsom forøgelse af blodcholesterol og færre vasospasti-ske virkninger såsom Raynauds symptomer.
Nogle af forbindelserne har også en grad af iboende sympatomimetisk virkning (ISA) , en egenskab, som er nyttig til at forhindre unødvendig bradycardi og hjæl-35 per til med at reducere incidensen af hjerteinfarkt i 21 DK 171484 B1 patienter med muskelsygdomme. Denne egenskab menes også at være nyttig til at reducere incidensen af rebound-hypersensitivitet, der forekommer efter ophør med β-blokkerlægemidler, og som er blevet beskrevet for 5 lægemidler, der ikke har nogen ISA, såsom propranolol, samt reducerer incidensen af ugunstige metaboliske virkninger såsom det forøgede plasmacholesterol, der fremkaldes af lægemidler såsom propranolol, som mangler ISA.
10 Denne egenskab kan påvises som en forøgelse i hvilepulsen i en præpareret rotte, som har fået rygmarven skåret over, under anvendelse af standardprocedurer, ved hvilke den maksimale virkning af kumulative doser af lægemiddel på op til 3 mg/kg observeres. Udvalgte 15 eksempler, der viser denne virkning, er vist nedenfor.

Pindolol +89 30 I visse af disse forbindelser er graden af jS-agonist-virkning således, at når den lægges sammen med de selektive Ø-antagonistegenskaber, er forbindelserne nyttige som cardiotonika, fx til behandling af hjerteinsufficiens, specielt i situationer, hvor der ønskes 22 DK 171484 B1 en positiv inotrop virkning uden signifikant indflydelse på blodtrykket. Balanceforholdet mellem de agonistiske og antagonistiske virkninger er særlig favorabelt for disse forbindelser; den agonistiske komponent 5 bidrager til den cardiotoniske virkning, medens den antagonistiske komponent beskytter mod en overdreven forøgelse i sammentrækningskraften, hvilket kan føre til arryhythmier. Forbindelser, i hvilke R2 er hydroxy, udviser en yderligere grad af delvis agonistvirkning, 10 hvilket er yderst nyttigt ved behandling af hjerteinsufficiens, især i situationer, hvor der ikke ønskes en forlænget lægemiddelvirkning.
Disse forbindelser har også den yderligere fordel, at de sænker det intraokulære tryk, når de instilles som 15 en pufret opløsning til øjnenes cornea. Disse forbindelser kan derfor anvendes ved behandling af glaucoma.
Denne egenskab kan påvises under anvendelse af standardtests i kaniner. Der observeres reduktioner i intraokulært tryk på så meget som 4 mm Hg over en 20 periode på 2 timer.
Til de ovenfor beskrevne anvendelser vil dosen variere alt efter den anvendte forbindelse, administrationsformen og den ønskede behandling. I almindelighed vil den nødvendige dosis til behandling af hypertension og 25 beslægtede coronarhjertesygdomme med et oralt præparat af forbindelserne 1-36 imidlertid opnås med en daglig dosis på 0,5-5 mg pr. kg legemsvægt; administration kan foretages il, 2, eller 3 delte doser eller i retard-form. Til større pattedyr såsom mennesker er den totale 30 daglige dosis af disse forbindelser fra ca. 1 til 600 mg; egnede farmaceutiske præparater til oral administration indeholder i almindelighed fra 25 mg til 250 mg af disse forbindelser sammen med faste eller flydende bærere og/eller diluenter, som er velkendte for 35 fagfolk.

Til behandling af glaucoma administreres dosen sædvanligvis som sterile øjendråber som en 0,5-2% opløsning i en pufret opløsning indeholdende borsyre.
Sammenlignende forsøg 5 Sammenlignende forsøg er gennemført med forbindelser i henhold til nærværende opfindelse og kendte forbindelser på isolerede hjerteforkamre og luftrør fra marsvin.
I disse eksperimenter fik de anvendte væv lov til at komme i ligevægt i 60 minutter i Krebs' Ringer fysiolo-10 giske saltopløsning med følgende sammensætning; NaCl, 120; KC1, 5,6; MgS02, 1,2; CaCl2, 2,5; KH2P04, 1,4;
NaHC03, 25; glucose 11,2; EGTA, 0,0025 mmol/1.
Kumulative koncentrations-responskurver blev opnået for hver opstilling som beskrevet af Van Rossum (Archiv.
15 Int. Pharmacodyn. 143, side 299-329, 1963), og kurverne blev tilpasset med computeranalyse i henhold til Zabo-rowsky et al, (J. Pharmacol. Meth., 4, side 165-178, 1980). Til måling af antagonisteffekten blev en hensigtsmæssig forbindelse tilsat til organbadet mindst 45 20 minutter efter den første koncentrations-responskurve var opnået, og den resulterende testblanding fik lov til at komme i ligevægt i 20 minutter, før den næste koncentrationsresponskurve blev tilvejebragt. Forskydningen til højre på denne kurve blev beregnet som en 25 pA2-værdi (se Mackay, J. Pharm. Pharmacol.. 30, side 312-313, 1978).
Marsvinehjerter blev fjernet fra voksne dyr (500-700 g) og placeret i Krebs' Ringer opløsning (pH 7,4) med en sammensætning som beskrevet ovenfor, og opstillingen 3 0 blev luftet med 5% C02 i 02. Hjerteforkammeret blev dissekeret frit af ventriklerne og overliggende væv og blev placeret i et 30 ml bad opbevaret ved 37°C og forbundet til en isotonisk transducer. En spænding på 2 24 DK 171484 B1 g blev anvendt. Kronotropisk aktivitet blev forstærket og registreret på en Grass polygraf.
Luftrøret blev udskåret fra voksne marsvin (500-700g), dissekeret frit af overliggende væv og skåret på tværs 5 i segmenter ca. 3 mm brede. Et segment blev anbragt i et 30 ml bad opbevaret ved 3°C. Afslapning af segmentet ved hjælp af isoprenalin blev målt med en isotonisk transducer forbundet til en Grass polygraf, efter at tonus var foranlediget ved administration af 1 mmol/1 10 carbachol (15 minutter før koncentrations-responskurverne) . Spændingen for hvert væv var 1 g.
Resultaterne af disse eksperimenter blev målt og udtrykt som pA2-værdier, som er den negative logaritme til potensen. Jo højere pA2værdien er, jo højere er 15 potensen for en given forbindelse for det pågældende væv. En forskel på 1 enhed i pA2 svarer til en forskel i potens på 10 gange. Modsat gælder det, at jo lavere pA2-værdien er, jo mindre potent er denne forbindelse for det væv. Eksempelvis viser eksempler på forbindel-20 ser ifølge opfindelsen kun lidt eller ingen aktivitet på luftrøret (beta-2), hvorimod propanolol, metoprolol, stenolol og betaxolol udviser signifikant antagonist-effekt på denne receptor. Alle de afprøvede forbindelser ifølge opfindelsen viser aktivitet overfor beta-25 l-adrenoceptoren (hjerteforkammeret). Selektivitets- indekset (forhold mellem potens for hjerteforkammeret i forhold til luftrøret), som er et kvantitativt separationsmål i aktiviteter for forbindelser for disse to adrenoceptorer, viser, at forbindelserne ifølge opfind-30 elsen er umådeligt mere selektive end de kendte forbindelser.
