Opfindelsen angår en analogi-fremgangsmåde til fremstilling af hidtil ukendte isoquinolineddikesyre-forbindelser eller farmaceutisk acceptable salte deraf, som er nyttige til behandling af visse kroniske komplikationer stammende fra dia-5 betes mellitus, især diabetiske katarakter.
Der er tidligere gjort forskellige forsøg på at opnå nye anti-diabetiske midler. Almindeligvis har disse anstrengelser involveret syntese af nye organiske forbindelser, især sulfonylurinstoffer, og bestemmelse af deres evne til væ-10 sentligt at sænke blodsukkerniveauer, når de indgives oralt. Imidlertid vides der kun lidt om virkningen af organiske forbindelser med hensyn til at forhindre eller lette kroniske komplikationer ved diabetes, såsom diabetiske katarakter, neuropathia og retinopathia. US patent skrift nr.
15 3 821 383 angiver, at aldosereductase-inhibitorer som 1,3- dioxo-lH-benz[d,e]isoquinolin-2(3H)-eddikesyre og derivater deraf er nyttige til behandling af disse tilstande.
Sådanne aldosereductase-inhibitorer fungerer ved at inhi-bere aktiviteten af enzymet aldosereductase, som er hoved-20 ansvarligt for regulering af reduktionen af aldoser, såsom glucose og galactose, til de tilsvarende polyoler, såsom sorbitol og galactitol, hos mennesker og andre dyr.
På denne måde forhindres eller reduceres uønskede opsamlinger af galactitol i linsen hos galactosæmiske individer 25 og af sorbitol i linsen, de perifere nervestrenge og nyrerne hos forskellige diabetiske individer. Som følge deraf er sådanne forbindelser af terapeutisk værdi som aldosereductase-inhibitorer til forhindring eller lettelse af visse kroniske diabetiske komplikationer, især sådanne af 30 oculær art, da det er kendt inden for faget, at tilstedeværelsen af polyoler i øjets linse fører til katarakt-dannelse med ledsagende tab af linseklarhed.
Ved fremgangsmåden ifølge opfindelsen fremstiller hidtil ukendte aldosereductase-inhibitorer, der er nyttige som 2 149569 terapeutiske midler til forhindring eller lettelse af kroniske diabetiske komplikationer, især diabetiske katarakter.

hvori R betyder hydrogen eller alkyl med 1-3 carbonatomer, R3 betyder -X eller -CH2X, hvor x betyder phenyl eller monoeller disubstitueret phenyl, hvor substituenterne er valgt blandt chlor, brom og fluor, og R^ betyder hydrogen eller 10 methyl, eller farmaceutisk acceptable salte deraf.
R er fortrinsvis hydrogen eller methyl, er fortrinsvis methyl, og X er fortrinsvis phenyl, monochlorphenyl eller di chlorphenyl.
Af særlig interesse er de forbindelser med formlen III, 15 hvori R^ er -X eller -CH^X/ og X er phenyl, 4-chlorphenyl eller 3,4-dichlorphenyl, især hvor R er hydrogen og R^ er methyl.
De ved fremgangsmåden ifølge opfindelsen fremstillede forbindelser kan sammensættes i farmaceutiske præparater omfattende en farmaceutisk acceptabel bærer og en forbind- else med formlen III i en effektiv mængde til behand-.
20 149569 3 ling af diabetiske katarakter. Foretrukne forbindelser til anvendelse i sådanne farmaceutiske præparater er dem med de foretrukne substituenter for R, R-j, R4 og x som defineret ovenfor. Foretrukne farmaceutiske præparater 5 har form af et ophthalmisk præparat.
De ved fremgangsmåden ifølge opfindelsen fremstillede forbindelser kan anvendes ved en fremgangsmåde til behandling af diabetiske katarakter, der består i at indgive et individ, som har behov for behandling, en effektiv mængde af en 10 'forbindelse med formlen III, fortrinsvis en forbindelse med de foretrukne substituenter .for R, R3, R4 og X som defineret ovenfor.
Fremgangsmåden ifølge opfindelsen til fremstilling af forbindelserne med formlen III er ejendommelig ved det i 15 kravets kendetegnende del anførte. Udgancrsforbindelserne fremstilles let ud fra de passende kendte 2-R4-l-oxo-3-in-daneddikesyrer, se Bull. Soc. Chim. Fr., 1970, 1143, og disse kan f.eks. syntetiseres ud fra de kendte forbindelser 3-phenylgiutarsyre (Braun et al., Chem Ber. 64B, 20 1785 (1931)), når skal være hydrogen, eller 2-phenyl- butan-l,3-dicarboxylsyre (Swan, J. Chem. Soc. 1955, 1039)» når skal være methyl. Den passende dicarboxylsyre opvarmes i en stærk syre, såsom koncentreret svovlsyre, p-toluensulfonsyre og phosphorsyre, ved en temperatur på fra 25 omkring 75 til 150°C, fortrinsvis fra omkring 90 til omkring 120°C, i et tidsrum på fra omkring 12 til omkring 30 timer, fortrinsvis fra omkring 20 til omkring 24 timer.

1,2-dihydro-2-hydroxy-3-R^-isoquinolineddikesyren (6) omdannes direkte til l-halogen-3-R4-4-isoquinolineddike-15 syren (9) ved opvarmning med et phosphortrihalogenid, såsom phosphortrichlorid eller phosphortribromid, ved en temperatur på fra omkring 50 til omkring 120°C, fortrinsvis fra omkring 60 til omkring 90°C, i et apro-tisk organisk opløsningsmiddel, fortrinsvis et lavere 20 alkylacetat, såsom ethylacetat, i relativt lange reak tionstider, f.eks. fra omkring 14 til omkring 24 timer. Alternativt kan man ved gennemførelse af reaktionen under de samme betingelser, men i relativt kortere reaktionstider, f.eks. fra omkring 1 til omkring 3 timer, 25 isolere l,2-dihydro-3-R4-l-oxo-4-isoquinolineddikesyren (7). l-Halogen-3-R^-isoquinolin-eddikesyren (9) dannes · derpå ved omsætning af 1,2-dihydro-3-R4-l-oxo-4-isoquino-lineddikesyren (7) med et phosphoroxyhalogenid, såsom phosphoroxychlorid eller phosphoroxybromid, i et organisk 30 opløsningsmiddel, såsom et lavere alkylacetat, f.eks.

Ved fremgangsmåden ifølge opfindelsen dannes så forbind-35 eiser med formlen III, hvori R^ er -X (11) eller -C^X
(12), ved omsætning af henholdsvis en passende substi- 149569 6 tueret phenol eller benzylalkohol med l-halogen-3-R4“4-iso-quinolineddikesyren (9) i nærvær af en base, fortrinsvis et alkalimetalalkoxid, såsom natrium- eller kalium-t-butoxid. Reaktionen gennemføres almindeligvis i et organisk opløs-5 ningsmiddel, såsom dimethylformamid, toluen eller xylen, ved en temperatur på fra omkring 50 til omkring 15°cj fortrinsvis fra omkring 100 til omkring 120°C.
Forbindelser med formlen III, hvori R er alkyl med 1-3 carbonatomer, fremstilles let 'ud fra de frie syrer ved 10 konventionelle esterificeringsmetoder, f.eks. ved omsætning med den passende alkohol under syrekatalyserede betingelser eller ved omsætning med den passende diazolkan.
Farmaceutisk acceptable salte kan let fremstilles ud fra forbindelser med formlen III, hvori R er hydrogen, 15 yed konventionelle metoder således, at syrehydrogenet erstattes med en egnet kation. Således kan disse salte let fremstilles ved behandling af isoquinolineddikesyrerne med en vandig opløsning af de ønskede farmaceutisk acceptable kationer, f.eks. en opløsning af hydroxidet eller carbona-20 tet, og inddampning af den resulterende opløsning til tørhed, fortrinsvis under formindsket tryk. Alternativt kan en lavere-alkanolopløsning af carboxylsyren med formlen III, blandes med et alkoxid af det ønskede metal, og opløsningen inddampes til tørhed. Egnede farmaceutisk ac-25 ceptable salte inkluderer, men er ikke begrænset til sådanne med kalium, natrium, ammonium, calcium eller magnesium som kation. Desuden .kan de ifølge opfindelsen fremstillede forbindelser med formlen III danne syreadditionssalte Ved omsætning med en farmaceutisk acceptabel syre 30 til dannelse af f.eks. hydrochloridet, hydrobromidet, sulfatet, hydrogensulfatet, lactatet, oxalatet eller citratet. Betegnelsen "farmaceutisk acceptable salte" skal i denne beskrivelse med krav omfatte både metalsaltene og syreadditionssaltene af de ifølge opfindelsen frem- 149569 7 stillede forbindelser som beskrevet ovenfor.
De hidtil ukendte isoquinolineddikesyrer med formlen III er nyttige som aldosereductase-inhibitorer og er som sådanne af terapeutisk værdi til behandling af kro-5 niske komplikationer forbundet med diabetes, især til behandling af diabetiske katarakter. I denne beskrivelse med krav skal "behandling" inkludere både forhindring og lettelse af sådanne tilstande. Til behandling af diabetiske katarakter indgives de ifølge opfindelsen fremstil-10 lede forbindelser i øjet på det individ, som har behov for behandling, i form af et ophthalmisk præparat fremstillet i overensstemmelse med konventionel farmaceutisk praksis, se f.eks. "Remington's Pharmaceutical Sciences", 15th Edition, side 1488 - 1501 (Mack Publishing Co., 15 Easton, Pa., U.S.A.), Det ophthalmiske præparat vil indeholde en forbindelse med formlen III eller et farmaceutisk acceptabelt salt deraf i en koncentration på fra omkring 0,1 til omkring 5 .vægt-%, fortrinsvis fra omkring 0,5 til omkring 2 vægt-%, i en farmaceutisk accepta-20 hel opløsning, suspension eller salve. Nogen variation i koncentrationen vil nødvendigvis forekomme afhængigt af den bestemte anvendte forbindelse, tilstanden hos det individ, som skal behandles, og lignende, og den person, som er ansvarlig for behandlingen, vil bestemme den mest egnede kon-25 centration for det bestemte individ. Det ophthalmiske præparat vil fortrinsvis være i form af en steril vandig opløsning, der om ønsket indeholder yderligere ingredienser, f.eks. konserveringsmidler, puffer-midler, tonicitetsmidler, antioxidanter og stabilisatorer, ikke-ioniske befugtnings-30 eller klaringsmidler, viskositetsforøgende midler og lignende .
Det ophthalmiske præparat vil blive indgivet topisk i 1 øjet på det individ, som har behov for behandling ved konventionelle metoder, f.eks. i form af dråber eller 35 ved badning af øjet i den ophthalmiske opløsning.

Aktiviteten af de ved fremgangsmåden ifølge opfindelsen fremstillede forbindelser som aldosereductase-inhibitorer og som midler til behandling af diabetiske katarakter kan bestemmes ved farmakologiske standardprøvninger. Sådanne 5 prøvninger inkluderer f.eks. måling af deres evne til at inhibere enzymaktiviteten af isoleret aldosereductase, i-følge den almene procedure, som er beskrevet i US patentskrift nr. 3 821 383 og baseret på den procedure, som er angivet af Hayman et al., J. Biol. Chem. 240 , 877 (1965).
10 På basis af de ifølge opfindelsen fremstillede forbindelsers aktivitet ved denne prøvning forventes det, at forbindelserne også vil udvise aktivitet som aldosereductase-inhibitorer, når de prøves i egnede koncentrationer ved andre farmaceutiske standardprøvninger, f.eks. dem, der 15 måler deres evne til at reducere eller inhibere sorbitolopsamling i ischiasnerven hos akut streptozotocinerede (dvs. diabetiske) rotter; måling af deres evne til at reversere et allerede forhøjet sorbitol-niveau i ischiasnerven og linsen hos kronisk streptozotocin-induceret 20 diabetiske rotter; måling af deres evne til at forhindre eller inhibere galactitol-dannelse i linsen hos akut ga-lactosæmiske rotter; og måling af deres evne til at forsinke katarakt-dannelse og reducere alvorligheden af linseuklarheder hos kronisk galactosæmiske rotter.
25 Det forventes også, at de ifølge opfindelsen fremstillede forbindelser vil være nyttige til behandling af andre kroniske' diabetiske komplikationer, såsom retinopathia og neuropathia. Til behandling af sådanne tilstande vil forbindelserne blive indgivet enten alene eller i kombination 30 med farmaceutisk acceptable bærere eller fortyndingsmidler i doseringer på fra omkring 5 til omkring 250 mg/kg legemsvægt af det individ, som har behov for behandling, pr. dag.
Imidlertid vil der nødvendigvis forekomme nogen variation i doseringen afhængigt af det behandlede individs tilstand 35 og den bestemte anvendte forbindelse, og lægen vil i hvert 149569 9 tilfælde bestemme den passende dosis for det enkelte individ. Egnede farmaceutiske bærere eller fortyndingsmidler til dette formål inkluderer inerte faste fortyndingsmidler eller fyldstoffer, sterile vandige opløsninger eller sus-5 pensioner og forskellige organiske opløsningsmidler i overensstemmelse med farmaceutisk standardpraksis. Sådanne farmaceutiske præparater indgives derpå let i en række forskellige doseringsformer ad oral eller parenteral indgiv-ningsvej. Om ønsket kan de farmaceutiske præparater inde-10 holde andre ingredienser, såsom excipienter, aromamidler, bindemidler og lignende.
Fremgangsmåden ifølge opfindelsen belyses nærmere ved de efterfølgende eksempler, hvor udgangsmaterialet fremstilles som belyst i den efterfølgende præparation.
15 PRÆPARATION
2-Methyl-l-oxo-5-indaneddikesvre 2-Phenylbutan-l,3-dicarboxylsyre (Swan, J. Chem. Soc., 1955, 1039) (20,0 g, 90,0 mmol) blev opløst i 300 ml koncentreret svovlsyre og opvarmet' til 100°C i 24 timer. Den 20 varme opløsning blev hældt ud på 1 kg is, og blandingen ekstraheret med 5 x 250 ml. ether. Etherfaserne blev vasket med .saltvand, tørret over magnesiumsulfat, filtreret og filtratet vakuuminddampet til opnåelse af en bleggul sirup; 14,6 g (79%), m/e 222. NMR-spektret viste tilstedeværelsen 25 af kun én overvejende isomer som påvist ved en enkelt "up-field" dublet, tilskrevet 2-methylsubstituenten, og små mængder af tre andre isomere.

5 Syn. Col. Vol. II, side 108, 194-3) (1,38 ml, 12,20 mmol) blev omrørt ved stuetemperatur, medens der tilsattes 4,5 ml 5% natriummethoxid i methanol (9»8 mmol). Opløsningen blev orangerød, og efter fortynding med ether (50 ml) og filtrering gav den et lysorange bundfald. 1.2- Dihydro-3-methyl-l-oxo-4-isoquinolineddikesyre 15 1,2-Dihydro-2-hydroxy-3-methyl-l-oxo-4-isoquinolineddike- syre (4,0 g, 17,2 mmol) opløst i 30 ml ethylacetat ved 0°C blev omsat med phosphortrichlorid (1,50 ml, 17,2 mmol).
Blandingen blev opvarmet til 80°C i 1,5 timer og derpå hældt ud på 100 g isvand, omrørt i 16 timer og filtreret, 20 hvorved der blev opnået et lysegult råt produkt; 3,307 mg (89%), smp. 280 - 295°C (dec). Omkrystallisation fra eddikesyre gav et analytisk rent materiale med smp. 295 -300°C (dec).
1-Chlor-3-methy1-4-i soquinolineddikesyre 25 l,2-Dihydro-3-methyl-l-oxo-4-isoquinolineddikesyre (770 mg, 3,30 mmol) og phosphorosychlorid (1,0 ml, 10,7 mmol) blev opvarmet under tilbagesvaling i 8 ml ethylacetat i 16 timer og derpå vacuuminddampet til tørhed. Remanensen blev opdelt mellem vand og ethylacetat. Den vandig fase blev vasket 30 med ether. De kombinerede organiske faser blev vasket med 149569 11 saltvand, tørret over magnesiumsulfat, filtreret og vacuum-inddampet til opnåelse af en viskøs, homogen olie bestående af l-chlor-3-methyl-4-isoquinolineddikesyre; 772 mg (99%), EKSEMPEL 1 5 1-(3,4-Dichlorbenzyloxy)-3-methyl-4- isoquinolineddikesyre_ l-Chlor-3-methyl-4-isoquinolineddikesyre (623 mg, 2,65 mmol) i 6 ml dimethylformamid sattes til en opløsning af 3,4-di-chlorbenzylalkohol (2,75 g, 15,5 mmol) og kalium-tert-butoxid 10 (600 mg, 534 mmol) i 2 ml dimethylformamid, og blandingen blev opvarmet til 115°C i 3 timer. Efter afkøling blev blandingen hædt ud på knus is og vasket med 3 x 60 ml ether. Det basiske vandige lag blev gjort surt til pH 3 med 1 N saltsyre og ekstraheret med 3 x 100 ml ether.
15 Den organiske ekstrakt blev tørret over magnesiumsulfat, filtreret og vaccuminddampet til et olieagtigt fast stof; 568 mg (57%). Udrivning af denne remanens med methanol og derpå hexan og fraktioneret krystallisation fra chloroform gav den rene ovennævnte forbindelse; 44 mg (4%), 20 smp. 180-183°C.
EKSEMPEL 2 1-(4-Chlorphenoxy)-3-methyl-4-isoquinolineddikesyre
En blanding af l-chlor-3-methyl-4-isoquinolineddikesyre (237 mg, 10,01 mmol), 4-chlorphenol (260 mg, 2,02 mmol) 25 og kalium-tert-butoxid (226 mg, 2,02 mmol) blev opvarmet til 100°C i 3 timer. Efter afkøling blev blandingen fortyndet med ethylacetat og vand og gjort sur til pH 1-2.
Den fraskilte vandige fase blev ekstraheret med 20 ml ethylacetat. 5 BIOLOGISK PRØVNING
