Description
- Die vorliegende Erfindung betrifft eine biologisch aktive Gelenkprothese. Eine Prothese gemäß den Merkmalen des Oberbegriffs von Anspruch 1 ist aus dem Dokument EP-A-0 645 149 bekannt.
- Es ist auf diesem technischen Gebiet bekannt, zum Ersatz von Gelenken mit beschädigtem Gewebe, insbesondere von Gelenken, die kleine Knochen in den Händen und Füssen verbinden, synthetische, elastische Gelenkimplantate zu verwenden, die aus nicht biologisch abbaubaren Kunststoffen bestehen. Eine typische biostabile elastische Gelenkprothese besteht aus einem Zwischenstückabschnitt, der zwischen den zu verbindenden Knochen angeordnet wird, und zwei länglichen Befestigungsabschnitten, die in den zu verbindenden Knochen verankert werden. Ein derartiges künstliches Gelenk ist beispielsweise von Dow Corning S.A., Valbourne Cedex, France, unter der Handelsbezeichnung Silastic® erhältlich. Das Silastic®-Implantat besteht aus einem Elastomermaterial.
- Die Verwendung von Gelenkprothesen, die aus biostabilen Polymeren, Polymergemischen und Elastomeren hergestellt sind, kann allerdings dem Patienten Probleme bereiten. Ein solches Problem bei biostabilen Gelenkprothesen besteht darin, dass das operierte Glied nach der Operation lediglich einer festgelegten Beanspruchungsstärke widerstehen kann. Wenn zum Beispiel eine Silastic®-Gelenkprothese zum Ersatz eines Fingergelenks verwendet wird, kann die operierte Hand keine Belastung von mehr als 5 kgf (~9,81 N) aushalten, und eine Überbelastung kann zum Bruch oder zum Verschleiß des Implantats führen, das die Gelenkprothese bildet. Ein weiteres Problem solcher biostabiler Gelenkprothesen besteht darin, dass aufgrund von Verschleiß, Ermüdung und/oder Korrosion lose Partikel aus der Gelenkprothese freigesetzt werden können, die eine chronische Entzündungsreaktion, z.B. eine Synovitis, und/oder osteologische Veränderungen im Knochen hervorrufen können. Die Entzündungsreaktion kann ferner eine Schwellung und Schmerzen im Gelenk hervorrufen, und zwar oft in einem Maße, dass eine Entfernung der Gelenkprothese erforderlich wird.
- Die PCT-Veröffentlichung WO 96/41596 der vorliegenden Patentanmelderin beschreibt eine biologisch abbaubare Gelenkprothese, die aus einem Zwischenstück sowie einem proximalen und einem distalen Befestigungsteil, die an den mit einem Gelenk zu versehenden Knochen befestigt werden, besteht. Eine Gelenkprothese gemäß WO 96/41596 kann im Bereich der Hand, des Handgelenks oder des Fußes implantiert werden, um ein beschädigtes Gelenk vollständig oder teilweise zu ersetzen. Das Zwischenstück der Gelenkprothese von WO 96/41596 hält die zu verbindenden Knochen in einem gewünschten Abstand voneinander. Die Gelenkprothese wird an den Knochen durch Verankerung des proximalen beziehungsweise des distalen Befestigungsteils in den beiden gelenkig zu verbindenden Knochen verankert. Die Festigkeits- und Duktilitätswerte der Befestigungsteile der Gelenkprothese von WO 96/41596 sind so gewählt, dass sie ein Biegen des operierten Gelenks kurz nach der Implantation erlauben.
- Das Zwischenstück der Gelenkprothese von WO 96/41596 ist für die Funktion der Prothese wichtig. Zur Erzielung einer erwünschten Porosität (typischerweise einer Porengröße von 50 bis 1000 μm) besteht das Zwischenstück von WO 96/41596 bevorzugt aus einer faserigen, dreidimensionalen, teilweisen porösen Struktur aus biologisch abbaubaren Fasern, die eine typische Dicke von 1 bis 300 μm aufweisen.
- Zur Herstellung dieses Zwischenstücks können die biologisch abbaubaren Fasern gewebt oder gewirkt oder zu einem Faservlies verarbeitet werden, das dann zu einer dreidimensionalen kissenförmigen porösen Struktur gefaltet oder gewickelt wird. Das Zwischenstück nach WO 96/41596 kann auch hergestellt werden aus (i) einem kontinuierlichen Rohfaden unter Anwendung von Techniken zum dreidimensionalen Verweben, Wirken oder Verschlingen, wobei eine erwünschte Länge des kontinuierlichen Rohfadens für jedes Zwischenstück geschnitten wird; (ii) biologisch abbaubaren Fasern, die zu den gewünschten Längen geschnitten und unter Verwendung eines biologisch abbaubaren Rohfadens miteinander verbunden werden, oder (iii) einem biologisch abbaubaren kontinuierlichen Polymer-Rohmaterial, das zur Erzeugung einer offenen Porosität darin behandelt wurde.
- Die Form des Zwischenstücks von WO 96/41596 entspricht bevorzugt dem Zwischenraum zwischen den durch die Gelenkprothese zu verbindenden Knochen. Demgemäß kann dieses Zwischenstück die Form eines Kissens, eines Zylinders, eines Ellipsoids, einer (abgeflachten) Kugel, eines kubischen oder rechteckigen Prismas oder eine andere dreidimensionale Struktur aufweisen, die den für das Zwischenstück vorgesehenen Zwischenraum zwischen den zu verbindenden Knochen so gut wie möglich ausfüllt. Das Zwischenstück von WO 96/41596 ist vorteilhaft elastisch und soll nicht irgendwelche scharfen und/oder harten Kanten oder Ecken aufweisen, die eine mechanische Reizung des umgebenden Gewebes hervorrufen könnten.
- EP-A-645 149 beschreibt eine vollständig absorbierbare Prothese zur Reparatur von beschädigten Bändern und/oder Sehnen in Form einer aus mehreren Schichten bestehenden spiraligen Walze, die spiralige Schichten, eine perforierte Schicht aus einem synthetischen bioabsorbierbaren Material, einen bioabsorbierbaren Film sowie eine Schicht aus einem bioabsorbierbaren Biopolymer-Schwamm aufweist.
- Die vorliegende Erfindung gibt eine Prothese zur Implantation zwischen zu verbindenden Knochen an, die einen aus einem gewickelten bioabsorbierbaren Gewebe hergestellten zylindrischen Körper aufweist; sie ist dadurch gekennzeichnet, dass sie ein erstes poröses Gelenkzwischenstück (
21 ) in Form eines zylindrischen Körpers aus einem gewickelten bioabsorbierbaren Gewebe und ein zweites poröses Gelenkzwischenstück (22 ) in Form eines zylindrischen Körpers aus einem gewickelten bioabsorbierbaren Gewebe aufweist und dass nach der Implantation der Prothese in einen Patienten das erste Gelenkzwischenstück und das zweite Gelenkzwischenstück derart in teilweisem Kontakt stehen, dass ein Hohlraum zwischen einer Oberfläche des ersten Gelenkzwischenstücks und einer Oberfläche des zweiten Gelenkzwischenstücks vorliegt. - Das Gewebe ist typischerweise relativ schmal (z.B. 1 bis 10 mm breit) und dünn (z.B. 0,1 bis 1,0 mm dick), je nach der beabsichtigten Anwendung der Prothesenvorrichtung, die aus dem Gewebe herzustellen ist. Gemäß der vorliegenden Erfindung wird das Gelenkzwischenstück durch Wickeln des Gewebes zu einem zylindrischen Körper und Fixieren des freien Endes des Gewebes auf der Oberfläche des so erzeugten zylindrischen Körpers hergestellt.
- Das Gelenkzwischenstück der vorliegenden Erfindung kann in Verbindung mit einem oder mehreren Befestigungsteilen implantiert werden, um das Gelenkzwischenstück zwischen den zu verbindenden Knochen an seinem Platz zu halten.
- Es folgt eine Beschreibung einiger spezieller Ausführungsformen der Erfindung, wobei auf die beigefügten Zeichnungen Bezug genommen wird.

- Wie aus
1 ersichtlich ist, wird das Gelenkzwischenstück der vorliegenden Erfindung aus einem faserigen Gewebe1 hergestellt, das zu einem zylindrischen Körper2 gewickelt wird. Die Größe des so erzeugten zylindrischen Körpers ist durch seinen Durchmesser "D" und seine Dicke "t" gekennzeichnet, wie in1 veranschaulicht ist. Gemäß den1 ,2 und3 wird das freie Ende3 des Gewebes an der Oberfläche des zylindrischen Körpers befestigt, z.B. durch Nähen mit einer bioabsorbierbaren Naht4 oder durch Kleben mit einem bioabsorbierbaren Polymer-Schmelzkleber oder einer Polymerlösung5 . - Die Herstellung des zylindrischen Körpers
2 durch Wickeln des Gewebes führt zu mehreren Vorteilen gegenüber dem Stand der Technik. Der Durchmesser D des zylindrischen Körpers kann sehr leicht und bequem durch Veränderung der Länge des zur Erzeugung des zylindrischen Körpers verwendeten Gewebes variiert werden. Der Chirurg kann ferner, falls erforderlich, im Operationsraum den Durchmesser D durch Öffnen der Befestigung (4 oder5 ) des zylindrischen Körpers, Abwickeln des Gewebes bis zu einem gewünschten Punkt, Abschneiden des abgewickelten Teils und Anbringen des neuen freien Endes des Gewebes auf dem zylindrischen Körper leicht kleiner machen. In ähnlicher Weise kann die Dicke t des zylindrischen Körpers2 leicht durch Auswahl von Geweben mit unterschiedlicher Breite t' variiert werden, wie in1 gezeigt ist. - Die Porosität des zylindrischen Körpers
2 kann ferner vorteilhaft auf verschiedene Weise variiert werden. So kann zum Beispiel die Porosität des Gewebes1 durch Variieren der Textilstruktur des Gewebes eingestellt werden. Die Porengröße und die Porenverteilung des Gewebes1 kann ferner auch durch Variieren der Dicke der zur Herstellung des Gewebes verwendeten Fasern variiert werden, d.h., durch Verwendung von dünneren Fasern (z.B. 0,1 bis 10 μm), dickeren Fasern (z.B. 10 bis 500 μm) oder von Gemischen solcher Fasern im Gewebe. - Die Porosität des Gewebes
1 (und des zylindrischen Körpers2 ) kann ferner auch durch Variieren der Verteilung und/oder des Durchmessers der zur Herstellung des Gewebes verwendeten Fasern variiert werden. In4 sind schematisch Beispiele für einige typische, in der vorliegenden Erfindung zu verwendende Gewebe dargestellt, die verschiedene Verteilungen und/oder Durchmesser der Fasern aufweisen. Zusätzlich zu den in4 dargestellten Möglichkeiten sind auch zahlreiche andere Faserstärken und Faserverteilungen im Gewebe im Rahmen der vorliegenden Erfindung möglich. Durch Variation der Dicke und/oder der Verteilung und/oder der Orientierung der Fasern und/oder durch Variieren der Wirktechnik, der Webtechnik oder der Technik zur Vliesherstellung, die zur Herstellung des Gewebes aus solchen Fasern herangezogen werden, können Gelenkzwischenstücke gemäß verschiedenen Spezifikationen bezüglich der Belastbarkeitseigenschaften, der Reibungseigenschaften und der Gewebewachstumseigenschaften hergestellt werden. - Wie aus
5 hervorgeht, können die Fasern vor der Herstellung des Gewebes1 gekräuselt oder spiralförmig verdrillt werden. Wenn ein solches Gewebe aufgewickelt wird, ist der resultierende zylindrische Körper2 besonders porös und elastisch. Die Porosität des zylindrischen Körpers2 kann ferner auch durch Einstellen der während des Wickelvorgangs auf das Gewebe wirkenden Spannung variiert werden. Wenn eine niedere Spannung angewandt wird, ist der gewickelte zylindrische Körper lose und weich, und die Dicke t des zylindrischen Körpers kann fast die gleiche sein wie die Breite t' des Gewebes. Wenn während des Wickelvorgangs eine hohe Spannung angewandt wird, wodurch das Gewebe dichter, härter und weniger porös wird, kann die Dicke t des resultierenden zylindrischen Körpers kleiner als die Breite t' des Gewebes gemacht werden. Darüber hinaus kann durch Variieren der während des Wickelvorgangs auf das Gewebe einwirkenden Zugspannung ein zylindrischer Körper erzeugt werden, der variierende Dicke und Porosität besitzt. Wenn beispielsweise zu Beginn des Wickelvorgangs eine hohe Zugspannung angewandt wird und diese Zugspannung während des Wickelns fortschreitend verringert wird, entsteht ein zylindrischer Körper, der einen Mittelabschnitt hoher Dichte und geringer Dicke aufweist, wie in6 erläutert ist. - Das zur Herstellung des Gelenkzwischenstücks der vorliegenden Erfindung verwendete Gewebe kann unter Verwendung von Fasern aus einem biologisch abbaubaren Polymer, Copolymer, Polymergemisch oder einer entsprechenden Polymerzusammensetzung oder durch Kombination verschiedener biologisch abbaubarer Polymersubstanzen hergestellt werden. Wenn Fasern aus verschiedenen biologisch abbaubaren Polymeren zur Herstellung des erfindungsgemäßen Gewebes kombiniert werden, kann ein Gelenkzwischenstück aufgebaut werden, das im Gewebe in einer nicht gleichmäßigen Weise abgebaut wird, da die verschiedenen Fasern, aus denen das Gewebe aufgebaut ist, unterschiedliche Abbaugeschwindigkeiten im Gewebe aufweisen können. Alternativ kann ein derartiger nicht gleichmäßiger Abbau des Gelenkzwischenstücks im Gewebe gemäß der Erfindung durch Beschichten des Gewebes mit einem biologisch abbaubaren Material erzielt werden, das im Gewebe eine Abbaugeschwindigkeit besitzt, die von der des zur Herstellung des Gewebes verwendeten Polymers oder der verwendeten Polymeren verschieden ist.
- In der medizinischen und technischen Literatur sowie in der Patentliteratur wurde bereits eine Vielzahl von biologisch abbaubaren Polymeren angegeben, die sich als Rohmaterialien zur Herstellung von Gelenkprothesen gemäß der vorliegenden Erfindung eignen. Hierzu gehören beispielsweise biologisch abbaubare aliphatische Polyester (vgl. z.B. Vainionpää, S., Rokkanen, P. und Törmälä, P., in Progr. Polym. Sci. 14 (1989) pp. 679-716; die Patente
US 4 743 257 ,US 5 084 051 ,US 4 968 317 ; die europäische PatentanmeldungEP 0 423 155 und die PCT-Anmeldung PCT/FI93/00014) sowie Polyesteramide, Polyorthoester, Polyanhydride und Polyphosphazene (vgl. z.B. C.T. Laurensin et al., J. Biomed. Mater. Res. 27 (1993) pp. 963-973). - Wenn sich das Gelenkzwischenstück der vorliegenden Erfindung in einem Gelenkhohlraum befindet, wird es relativ schnell von Bindegewebe bedeckt und/oder gefüllt. Während dieses biologischen Abbauprozesses wird das Gelenkzwischenstück durch ein biologisches Fasergewebe ersetzt. Im Ergebnis wird ein neues, biologisches, elastisches Fasergewebegelenk erhalten, das eine Bewegung der damit verbundenen Knochen durch die umgebenden Muskeln erlaubt. Da sich das neue Gelenk während des Abbauprozesses des Gelenkzwischenstücks bildet, werden keine Fremdpartikel abgegeben, die für das Patientensystem chronisch schädlich sind, wie dies bei den sogenannten biostabilen Gelenkprothesen der Fall sein kann. Daher vermeidet das Gelenkzwischenstück der vorliegenden Erfindung die Risiken solcher chronischer Komplikationen, die durch lose Fremdpartikel hervorgerufen werden, die bei Verwendung biostabiler Gelenkprothesen möglich sind, vollständig.
- Damit nach der Implantation innerhalb des Gelenkzwischenstücks Gewebe wachsen kann, ist der zylindrische Körper der vorliegenden Erfindung vorteilhaft porös, wobei die Porengröße beispielsweise zwischen 50 und 1000 μm variiert. Die Porengröße des zylindrischen Körpers kann, wie oben erläutert, gemäß der erwünschten mechanischen Festigkeit der Prothese und je nach dem Abstand zwischen den zu verbindenden Knochen variiert werden.
- Bei einer Ausführungsform der vorliegenden Erfindung werden die gewickelten Gewebelagen des zylindrischen Körpers zusammengeklebt, und zwar entweder durch Beschichten der Oberfläche des Gewebes mit einem anderen bioabsorbierbaren Polymer, das eine niedrigere Schmelztemperatur als das Gewebe aufweist, vor dem Wickeln des Gewebes oder durch Verwendung eines klebenden bioabsorbierbaren Films
6 , der mit dem Gewebe1 gewickelt wird, wie in7 dargestellt ist. Das Verkleben des Gewebes kann auch unter Verwendung einer Lösung eines anderen bioabsorbierbaren Polymers vorgenommen werden. Durch Anwendung von Hitze und/oder Druck kann die Gewebebeschichtung oder der verklebende bioabsorbierbare Film erweicht und/oder geschmolzen werden, um so das aufgewickelte Gewebe zu binden und einen festen zylindrischen Körper zu erzeugen. Insbesondere, wenn der oben erwähnte bioabsorbierbare Klebefilm verwendet wird, ist es möglich, eine orientierte Porosität und/oder Kanäle7 in senkrechter Richtung in Bezug auf die ebenen Oberflächen des zylindrischen Körpers zu erzeugen, wie schematisch in8 dargestellt ist. Derartige orientierte Poren und/oder Kanäle sind vorteilhaft, wo eine gelenkte Geweberegeneration (d.h., ein in spezifische Richtungen gelenktes Gewebewachstum) erwünscht ist. - Gemäß der vorliegenden Erfindung können die Steifigkeit, die Flexibilität, die Oberflächenqualität und die Porosität des zylindrischen Körpers durch Erhitzen des zylindrischen Körpers auf höhere Temperaturen (typischerweise auf eine Temperatur T > Tg, wobei Tg die Glasübergangstemperatur einer Faserkomponente des zylindrischen Körpers bedeutet) kontrolliert werden, optimal in einer geeigneten Form und unter mechanischem Druck. Das Erhitzen und der gleichzeitig angewandte mechanische Druck machen den zylindrischen Körper steifer; ferner kann, wenn die Behandlung in einer Form vorgenommen wird, die Form des zylindrischen Körpers bleibend geändert werden; so kann z.B. die kreisförmige Geometrie des zylindrischen Körpers abgeflacht werden, oder es können seine ebenen Oberflächen gekrümmt gemacht werden.
- Das Gewebezwischenstück der vorliegenden Erfindung verhält sich nach der Implantation überraschend gut, und zwar unabhängig davon, ob die Gelenkfläche von einem Knochen oder von beiden zu verbindenden Knochen entfernt wurde.
9A zeigt einen Querschnitt durch Gelenkflächen von zwei Fingerknochen oder Zehenknochen,8 und9 , wobei die Gelenkfläche beider Knochen entfernt wurde und der Gelenkraum mit einem zylindrischen Körper10 gefüllt wurde, der planare Oberflächen10' und10'' aufweist.9B zeigt eine Ausführungsform, bei der die Gelenkfläche lediglich von einem Knochen11 , nicht jedoch von dem anderen, damit zu verbindenden Knochen12 entfernt wurde. Zur Anpassung an diese Situation kann eine Oberfläche11' des implantierten zylindrischen Körpers11 konkav und die andere Oberfläche11' planar ausgeführt werden.9C erläutert einen Fall, in dem die beiden Oberflächen12' und12'' des zylindrischen Körpers12 konkav sind, um sie an die konvexen Gelenkflächen der beiden zu verbindenden Knochen anzupassen. - Das Gelenkzwischenstück (zylindrischer Körper) der vorliegenden Erfindung kann ferner auch in Kombination mit unterschiedlichen Typen von Befestigungsteilen verwendet werden (z.B. einem oder mehreren Stiften, Schrauben, Bolzen oder Stäben, allein oder in Kombination), die so ausgebildet sind, dass das Gelenkzwischenstück mit den zu verbindenden Knochen verbunden werden kann, wie in WO 96/41596 in Bezug auf das Zwischenstück und entsprechende Befestigungsteile beschrieben ist. Die Befestigungsteile der Gelenkprothese gemäß der vorliegenden Erfindung sind dadurch gekennzeichnet, dass sie mit dem Gelenkzwischenstück verbunden werden, um eine feste und integrale Einheit zu erzeugen, und zwar so, dass die Befestigungsteile und das Gelenkzwischenstück zusammen eine flexible Gelenkprothese bilden. Durch die Befestigungsteile wird das Gelenkzwischenstück zwischen den zu verbindenden Knochen gehalten, wobei es durch die Muskelkraft möglich ist, die zu verbindenden Knochen in Bezug zueinander abzubiegen.
- Gemäß der vorliegenden Erfindung können die Befestigungsteile aus einem biologisch abbaubaren Polymer, Polymergemisch oder einer entsprechenden Zusammensetzung unter Anwendung von Schmelzformverfahren, wie etwa Spritzgießen oder Extrudieren, hergestellt werden, oder sie können durch mechanische Bearbeitung aus einem Polymerrohling in die gewünschte Form gebracht werden. Selbstverstärkte biologisch abbaubare Zusammensetzungen stellen besonders vorteilhaft als Rohmaterialien anwendbare Zusammensetzungen bei der Herstellung der Befestigungsteile gemäß der vorliegenden Erfindung dar. Derartige Zusammensetzungen sind auf dem vorliegenden Gebiet bekannt und wurden in Veröffentlichungen wie etwa dem Patent
US 4 743 257 und der Anmeldung WO 99/05312 beschrieben. Andere biologisch abbaubare Zusammensetzungen können ebenfalls zur Verstärkung der Befestigungsteile gemäß der vorliegenden Erfindung verwendet werden, wobei das Befestigungsteil durch Fasern, geschnittene Fasern, Filamente oder daraus erzeugte Strukturen, wie Geflechte, Garne, Schnüre, Vliesstrukturen, Gewebe, Gewirke, etc., verstärkt wird, wobei die Verstärkung auch aus einem biologisch abbaubaren Polymer, Copolymer oder Polymergemisch besteht. Biologisch abbaubare Zusammensetzungen zur Erzeugung solcher Verstärkungen für das Befestigungsteil sind auf diesem Gebiet bekannt, und zahlreiche derartige Polymere sind in den hier zitierten Literaturreferenzen angegeben. Das Befestigungsteil der vorliegenden Erfindung kann auch mit Fasern verstärkt werden, die aus einem biologisch abbaubaren keramischen Material, wie Bioglas oder Calciumphosphat (vgl. z.B. Vainionpää, S., Rokkanen, P. und Törmälä, P., in Progr. Polym. Sci. 14 (1989) pp. 679-716), bestehen. - Die erfindungsgemäßen Befestigungsteile, die mit biologisch abbaubaren organischen und/oder anorganischen Fasern oder daraus erzeugten Strukturen verstärkt sind, können nach verschiedenen in der Kunststofftechnologie bekannten Verfahren hergestellt werden, z.B. durch zumindest teilweises Verbinden der verstärkten Strukturen miteinander durch ein biologisch abbaubares Polymer, Copolymer oder Copolymergemisch (Matrix) unter Bedingungen, unter denen die Matrix sich in Lösung (oder in geschmolzener Form) befindet und eine ausreichend homogene Zusammensetzung aus der Matrix und dem Verstärkungsmittel gebildet wird. Spritzgießen, Extrusion, Wickeln, Formpressen und andere Methoden können zum Kombinieren von verstärkenden Fasern oder dergleichen mit der Matrix und bei der Umwandlung dieser Kombination in Halbfabrikate und/oder zu Implantaten angewandt werden. Gewünschtenfalls können die Befestigungsteile der vorliegenden Erfindung in ähnlicher Weise wie das Gelenkzwischenstück porös sein, wobei Gewebewachstum in der offenen Porosität des Befestigungsteils aus dem umgebenden Gewebe stattfindet und als Ergebnis das Befestigungsteil rasch fest an seinem Platz verankert wird.
- Die Implantate (Gelenkzwischenstück und Befestigungsteil) der vorliegenden Erfindung können ferner auch verschiedene Additive enthalten, um die Verarbeitbarkeit des Materials zu erleichtern (zum Beispiel Stabilisatoren, Antioxidationsmittel oder Weichmacher) oder seine Eigenschaften zu verändern (zum Beispiel Weichmacher oder keramische chemische Produkte in Pulverform oder biostabile Fasern, wie etwa Kohlenstofffasern) oder seine Anwendung zu erleichtern (z.B. Färbemittel). Das Befestigungsteil der vorliegenden Erfindung kann auch aus dem eigenen Fasergewebe des Patienten aufgebaut werden, wie etwa aus dem Gewebe von Sehnen oder Bändern, indem ein ausreichend langer Teil einer Sehne oder eines Bandes, der aus einem Knochen herausragt, der mit einem anderen derartigen Knochen zu verbinden ist, in der Weise manipuliert wird, dass das Gelenkzwischenstück zwischen den Knochen angeordnet wird und die Sehne oder das Band durch das Zwischenstück hindurchgeht.
- Gemäß einer vorteilhaften Ausführungsform der Erfindung kann das Gelenkzwischenstück und/oder das Befestigungsteil ein oder mehrere bioaktive Mittel enthalten, wie etwa Antibiotika, Chemotherapeutika, Mittel zur Beschleunigung der Wundheilung, Mittel zur Induzierung der Bildung von Knorpelkollagen, Wachstumshormone, Anticoagulantien (wie Heparin), etc.. Bioaktive Medien dieses Typs sind in der klinischen Anwendung besonders vorteilhaft, da sie, zusätzlich zur mechanischen Wirkung, biochemische Wirkungen (zum Beispiel Beschleunigung des Wachstums von Fasergewebe und/oder Knorpelgewebe), medizinische und andere Wirkungen in menschlichen Geweben aufweisen.
- Das Befestigungsteil der vorliegenden Erfindung kann einen starren oder einen flexiblen Aufbau besitzen. Ein starres Befestigungsteil ist typischerweise ein Stab, eine Stange, eine Schraube oder ein Stift, wobei der Schaft mit dem Gelenkzwischenstück verbunden wird und die Spitze sich innerhalb des zu verbindenden Knochens befindet. Eine Profilierung (wie etwa ein Schraubengewinde, verschiedene Schuppenstrukturen oder Stufen) auf der Oberfläche eines Befestigungsteils dieses Typs erleichtert die Festlegung des Befestigungsteils in dem im Knochen vorgesehenen Loch. Ein elastisches Befestigungsteil kann beispielsweise aus einem Gewebe (einem "Schleierstoff") bestehen, das am Gelenkzwischenstück oder einem Teil davon angebracht und auf der Oberfläche des zu verbindenden Knochens befestigt wird (mit kleinen absorbierbaren Stiften oder einer Naht). Ein solches elastisches Befestigungsteil kann auch aus einer oder mehreren Schlingen von absorbierbarem Nahtmaterial bestehen, das zur Befestigung des Gelenkzwischenstücks an dem zu verbindenden Knochen verwendet wird.

10C ist eine schematische Darstellung der Knochen15 und16 in Ansicht von oben, d.h., aus einer Position senkrecht zur Biegerichtung des Gelenks, wobei das mit einer Gelenkprothese versehene Gelenk gestreckt ist. - Wie aus
10 hervorgeht, geht ein stabartiges Befestigungsteil14 durch einen zylindrischen Körper13 hindurch, und die Enden14' und14'' des Befestigungsteils befinden sich innerhalb der Knochen15 und16 . Der Gegenstand der vorliegenden Erfindung kann in zahlreichen anderen Ausführungen der Befestigungsteile und der Gelenkzwischenstücke (Zwischenstücke) implantiert werden, wie auf diesem Gebiet bekannt ist und beispielsweise in den1 bis5 von WO 96/41596 dargestellt ist. - Wie aus
11A ferner hervorgeht, kann der zylindrische Körper (Gelenkzwischenstück) als Teilprothese auf einer Synovialgelenkfläche17 verwendet werden. Wie in der Querschnittsebene a-a in11B zu sehen ist, befindet sich der flache zylindrische Körper18 in einem Hohlraum19 in der Knorpeloberfläche17 auf der Oberfläche des Knochens20 . Der poröse, faserförmige zylindrische Körper18 induziert die Bildung von neuem Knorpel im Hohlraum19 während der Bioabsorption des zylindrischen Körpers und seines Ersatzes durch neugewachsenes Gewebe. Als Ergebnis dieses Prozesses bildet sich eine neue Knorpeloberfläche in dem beschädigten Hohlraum19 . Bei einer vorteilhaften Ausführungsform ist der zylindrische Körper18 mit Chondrozyten oder Zellen ähnlichen Typs vor der Implantation beladen. Auf dem Gebiet des Gewebe-Engineerings ist bekannt, dass verschiedene Typen von Zellen vom Patienten entnommen, unter Zellkulturbedingungen vermehrt, in ein poröses bioabsorbierbares Gelenkzwischenstück (zylindrischer Körper) eingebracht und in den Patienten implantiert werden können, um die Heilung des Gewebetraumas oder Gewebedefekts zu erleichtern. - Gemäß der vorliegenden Erfindung umfasst die Prothese zwei zylindrische Körper, die parallel zueinander im Gelenkhohlraum angeordnet werden können. Bei einer solchen Anordnung verbleibt ein vertikaler Hohlraum zwischen den zylindrischen Körpern, der den Hohlraum eines Synovialgelenks nachbildet. Wie in
12 dargestellt ist, sind zylindrische Körper21 und22 an Knochen25 und26 mit nagelartigen Befestigungsteilen23 und24 befestigt, so dass zwischen den zylindrischen Körpern21 und22 ein dem Synovialhohlraum ähnlicher Raum27 verbleibt. Wenn der Patient nach einer solchen Implantation das Gelenk bewegt, verschieben sich die zylindrischen Körper21 und22 gegenseitig durch Gleiten, und der dem Synovialhohlraum ähnliche Raum kann innerhalb des wachsenden faserförmigen Gelenks verbleiben. - Bei einer anderen vorteilhaften Ausführungsform der in
12 dargestellten Erfindung können die in Kontakt stehenden Oberflächen der zylindrischen Körper21 und22 , welche die Wände des Hohlraums27 bilden, mit Hyalinknorpelzellen und/oder Wachstumsfaktoren oder anderen bioaktiven Substanzen (oder mit einem anderen bioabsorbierbaren Polymer, das Wachstumsfaktoren abgibt) beschichtet werden, wodurch das Wachstum von Hyalinknorpel oder die Bildung einer Knorpelschicht auf den Hohlraumflächen des wachsenden Gewebes gefördert wird. Zusätzlich können die Oberflächen der zylindrischen Körper21 und22 , die in Kontakt mit dem Knochen25 und26 stehen, wie in12 dargestellt ist, mit knochenmorphogenen Proteinen (BMP) oder mit einem anderen, BMP freisetzenden bioabsorbierbaren Polymer beschichtet werden, um die Umwandlung jedes der zylindrischen Körper21 und22 durch Knochenbildung in den entsprechenden Knochen zu erleichtern. - Eine andere Art der Nachbildung eines Synovialhohlraums ist in
13 dargestellt. Wie aus dieser Figur ersichtlich ist, befindet sich ein flaches Loch oder ein kreisförmiger Spalt28 innerhalb des zylindrischen Körpers29 .
Claims (1)
- Prothese zur Implantation zwischen zu verbindenden Knochen, die einen aus einem gewickelten bioabsorbierbaren Gewebe hergestellten zylindrischen Körper aufweist, dadurch gekennzeichnet, dass die Prothese ein erstes poröses Gelenkzwischenstück (
21 ) in Form eines zylindrischen Körpers aus einem gewickelten bioabsorbierbaren Gewebe und ein zweites poröses Gelenkzwischenstück (22 ) in Form eines zylindrischen Körpers aus einem gewickelten bioabsorbierbaren Gewebe aufweist und dass nach der Implantation der Prothese in einen Patienten das erste Gelenkzwischenstück und das zweite Gelenkzwischenstück derart in teilweisem Kontakt stehen, dass ein Hohlraum (27 ) zwischen einer Oberfläche des ersten Gelenkzwischenstücks und einer Oberfläche des zweiten Gelenkzwischenstücks vorliegt.
