Description
- Diese Erfindung liegt auf dem Gebiet der Stents zur Aufrechterhaltung der Durchgängigkeit irgendeines einer Vielzahl von Gefäßen des menschlichen Körpers.
- In den letzten zehn Jahren wurden viele verschiedene Stentgestaltungen verwendet, um die Durchgängigkeit von Arterien und anderer Gefäße des menschlichen Körpers aufrechtzuerhalten. In allen solchen Vorrichtungen ist die Umfangsfestigkeit eine wichtige Eigenschaft. Speziell muß der Stent genügend Umfangfestigkeit haben, um elastischem Rückprall zu widerstehen, der durch das Gefäß hervorgerufen wird, in welchem der Stent plaziert wurde. Der in der US-Patentschrift Nr. 4 553 545 beschriebene Mass-Stent und der in der US-Patentschrift Nr. 4 503 569 beschriebene Dotter-Stent haben jeweils offene spiralförmige Wicklungen. Der in der US-Patentschrift Nr. 4 733 665 beschriebene Palmaz-Stent hat die "Chinesenfinger"-Gestalt. Der derzeit von der Cook, Inc. vertriebene Gianturco-Rubin-Stent ist eine andere Stentgestaltung, die wie die Stents von Mass, Dotter und Palmaz kein geschlossenes rundes Teil haben, um die Umfangsfestigkeit zu optimieren.
- Die EP-A-0.566.807 wird als Grundlage für den Oberbegriff des Anspruches 1 verwendet.
- Der ideale Arterienstent benutzt eine minimale Drahtgröße der Stentelemente, um Thrombose an der Stentstelle nach der Implantation zu minimieren. Der ideale Arterienstent besitzt auch genügend hohe Umfangsfestigkeit, um elastischem Rückprall der Arterie zu widerstehen. Obwohl die optimale Gestaltung zur Maximierung der Umfangsfestigkeit eine geschlossene runde Struktur ist, wurde kein bekannter Stent beschrieben, der einen kleinen Durchmesser hat, wenn er perkutan in ein Gefäß eingesetzt wird, und der sich in die Form mehrerer geschlossener runder Strukturen (d. h. Ringe) ausdehnt, wenn er nach außen gegen die Gefäßwand expandiert wird.
- Nach der Erfindung ist eine Stentstruktur vorgesehen zur Aufrechterhaltung der Durchgängigkeit eines Gefäßes eines menschlichen Körpers, wobei die Stentstruktur eine Längsachse hat und erste, Längselemente bildende Strukturen aufweist, welche durch zweite Strukturen um die Längsachse verbunden sind, mit dem Kennzeichen, daß mindestens zwei Längselemente eine Wellenform besitzen, um die Längsflexibilität zu verbessern.
- Eine Ausführungsform der Erfindung ist nachfolgend nur beispielshalber unter Bezugnahme auf die beiliegenden Zeichnungen beschrieben, in welchen
- Fig. 1 eine Seitenansicht eines Stents ist, nachdem dieser entfaltet wurde, d. h. in seiner Nachentfaltungsform,
- Fig. 2 ein Querschnitt an der Schnittlinie 2-2 von Fig. 1 ist und erläutert, wie die Längselemente an den Ringen befestigt sind,
- Fig. 3 ein Querschnitt an der Schnittlinie 3-3 von Fig. 2 ist und die Verbindung eines einzelnen Ringes mit den Längselementen zeigt,
- Fig. 4 eine Seitenansicht eines Stents vor der Befestigung auf einem Stentabgabekatheter ist, d. h. in der Form einer Ausgangsstruktur,
- Fig. 5 ein Querschnitt an der Schnittlinie 5-5 von Fig. 4 ist und erläutert, wie die Längselemente mit den Ovalen verbunden sind,
- Fig. 6 eine Seitenansicht einer Vorentfaltungsform der Stentstruktur ist, in welcher die Ovale zu einem Zylinder mit kleinem Durchmesser gefaltet sind, der um einen entleerten Ballon plaziert ist, welcher nahe dem distalen Ende eines Stentabgabekatheters angeordnet ist,
- Fig. 7 eine Teilseitenansicht eines Vorentfaltungsstents ist, die nur zwei von mehreren gefalteten Ovalen zeigt, welche um einen ausdehnbaren Ballon ausgebildet sind, in welchem die Ovale in einer alternativen Weise gegenüber Fig. 6 gefaltet sind,
- Fig. 8 eine Seitenansicht eines Nachentfaltungsstents gemäß der Erfindung ist, welcher zwei wellenförmige Längselemente auf gegenüberliegenden Seiten des Stents für verbesserte Plazierung in gekrümmten Gefäßen benutzt, und
- Fig. 9 eine Seitenansicht eines Stents ist, der aus einem Metallzylinder mit kleinem Durchmesser als einzelnes Metallstück geätzt ist.
- Fig. 1 ist eine Seitenansicht einer Ausführungsform eines zylindrischen Stents 1, der in seiner Nachentfaltungsgestalt gezeigt ist. Der Stent 1 hat mehrere Ringe 2, die durch vier Drähte, die sogenannten Längselemente, in Abstand voneinander gehalten werden. Wie in Fig. 1 und 2 zu sehen, befindet sich am oberen Ende des Stents ein Längselement 4T, am Boden ein Längselement 4B, an der linken Seite ein Längselement 4L und an der rechten Seite ein Längselement 4R. Obwohl die Fig. 1 und 2 sieben Ringe und vier Längselemente zeigen, liegt es auf der Hand, daß der Stent auch länger gemacht werden kann, indem man Ringe hinzufügt oder den Abstand zwischen Ringen vergrößert. In ähnlicher Weise kann der Stent kürzer gemacht werden, indem man die Anzahl der Ringe reduziert oder den Abstand zwischen Ringen vermindert. Auch ein variabler Abstand der Ringe wird ins Auge gefaßt, um verschiedenen Zwecken zu dienen, einschließlich erhöhter Umfangsfestigkeit an einem bestimmten Abschnitt des Stents. Auch wird ins Auge gefaßt, daß die zwei oder mehr Längselemente für diese Stentgestaltung benutzt werden können, wobei die maximale Anzahl 32 ist.
- Die Fig. 2 und 3 erläutern die Verbindung der Längselemente mit den Ringen. Speziell können die Längselemente in Ausschnitte in der Form von Kerben 5 plaziert werden, die auf dem Außenumfang des Ringes 2 angeordnet sind. Die Längselemente können dann punktgeschweißt, mit Klebstoff verbunden oder durch irgendwelche anderen Mittel an den Ringen 2 befestigt werden. Es wird auch ins Auge gefaßt, daß die Längselemente auf dem Innenumfang des Ringes 2 angeordnet werden oder Löcher mechanisch oder mit Laser durch den Ring 2 gebohrt werden können, um die Längselemente hindurchzuführen.
- Die Fig. 4 und 5 erläutern einen Stent 1', der in einer speziellen Form gezeigt ist, in welcher er hergestellt werden könnte, d. h. in einer Ausgangsform. Speziell zeigen die Fig. 4 und 5, daß diese Ausgangsform des Stents 1' mehrere parallele Ellipsen oder Ovale 2' hat, wobei jedes Oval die gleiche kleinere Achsenabmessung m und größere Achsenabmessung M hat. Die kleinere Achse des Ovals geht durch dert Mittelpunkt der Längselemente 4L und 4R. Die größere Ovalachse geht durch den Mittelpunkt der Längselemente 4T und 4B. Es ist wichtig zu bemerken, daß, wenn es erwünscht ist, einen Außenenddurchmesser D (wie in Fig. 2 gezeigt) des Ringes 2 nach seiner vollen Entfaltung zu haben, gezeigt werden kann, daß D der Gleichung D2 = ¹/&sub2;(m² + M²) gehorcht.
- Um die Stentgestaltung der Fig. 4 und 5 auf einem Ballon zu plazieren, der nahe dem distalen Ende eines Stentabgabekatheters befestigt wird, ist es erforderlich, die Ovale 2' um jenen Ballon zu falten. Speziell kann der zylindrische Vorentfaltungsstent 1" auf einem ausdehnbaren Ballon 6 ausgebildet werden, wie in Fig. 6 zu sehen ist, indem man die Ovale 2' um die gestrichelte Linie F (welche die kleinere Achse des Ovals 2' ist) faltet, wie in Fig. 5 gezeigt ist. Speziell könnte, wie in Fig. 4 zu sehen ist, das obere Ende und der Boden der Ovale 2' ortsfest gehalten werden, während die Seitenlängselemente 4R und 4L nach links gestoßen werden, was zu der Vorentfaltungsstruktur führt, die als Stent 1" in Fig. 6 gezeigt ist. Eine optimale Gestaltung hat die gefalteten Ovale 2", wie in Fig. 6 gezeigt, wobei der Stent 1" ein Zylinder ist, dessen Außendurchmesser gleich der Größe der Abmessung m der kleineren Achse ist. Wenn der Ballon 6 von Fig. 6 ausgedehnt wird, bildet der Vorentfaltungsstent 1" den Nachentfaltungsstent 1 mit runden Ringen 2, wie in den Fig. 1 und 2 gezeigt ist.
- Der Stent 1''' ist eine alternative Ausführungsform für eine Vorentfaltungsstruktur des Stents, wie er auf einen Ballon aufgesetzt wird. Speziell zeigt Fig. 7 zwei gefaltete Ringe 2''' eines Stents 1 "' mit mehreren Ringen. Der Stent 1''' wird gebildet, indem man das obere Ende und den Boden des Stents 1' von Fig. 4 ortsfest hält, während man das Längselement 4R nach links und das Längselement 4L nach rechts stößt. Wie der Stent 1" von Fig. 6 hat nach dem Befestigen auf einem Ballon der Stent 1''' eine zylindrische Form mit einem Durchmesser gleich der Abmessung m.
- Die Fig. 1 bis 7 einschließlich erläutern Stents, die Längselemente verwenden, welche aus allgemein geraden Drähten gebildet sind. Fig. 8 zeigt eine Ausführungsform eines Stents 10 nach der vorliegenden Erfindung, der zwei wellenförmige Längselemente hat. Speziell sind das linke Seitenlängselement 14L (als gestrichelte Linien gezeigt) und das rechte Seitenlängselement 14R jeweils wellenförmige Längselemente. Ein solcher Stent würde sich beim Einsetzen in ein Gefäß leichter biegen und wäre leichter für eine Plazierung in gekrümmte Gefäße, wie einige Kranzarterien, geeignet.
- Typischerweise bestehen die Ringe und Längselemente der Stents aus dem gleichen Material. Typische für einen solchen Stent verwendete Metalle wären rostfreier Stahl, Tantal, Titan oder eine Form von Metall mit Formerinnerungsvermögen, wie Nitinol. Wenn Nitinol verwendet wird, würde der Stent zu der Form bei Körpertemperatur mit runden Ringen 2 hitzebehandelt werden, wie in den Fig. 1 und 2 gezeigt. Die Ringe könnten dann, wie in den Fig. 4 und 5 gezeigt, zu Ovalen verformt und dann auf einem Stentabgabekatheter befestigt werden, welcher keinen Ballon verwendet, sondern die allgemeinere Form besitzt, die in der obenerwähnten US- Patentschrift Nr. 4,553,545 von C. T. Dotter beschrieben ist. Eine solche Gestaltung würde die erwünschte Stentstruktur mit mehreren allgemein runden Ringen statt der Dotter-Gestalt einer Spiralfeder liefern, welche inhärent eine geringere Umfangsfestigkeit hat.
- Es sollte verstanden werden, daß, wenn die Ovale auf einen Stentabgabekatheter gefaltet sind, wenn sie voll entfaltet werden, keine perfekt runden Ringe bilden, wie in Fig. 2 gezeigt, sondern eher eine allgemein runde Form haben. Solche vergleichsweise kleinen Abweichungen von einer exakt runden Form vermindert die Umfangsfestigkeit nicht merklich, da sie in der Tat geschlossene Strukturen haben, die fast genau rund sind.
- Es sollte auch verstanden werden, daß wenigstens ein Teil der Endringe des Stents aus einem radiopaken Metall hergestellt oder mit ihm beschichtet werden könnte, wie mit Tantal oder Gold, um eine fluoroskopische Anzeige der Stentposition in einem Gefäß zu liefern. Die anderen Ringe und Längselemente könnten jedoch aus einem viel weniger dichten Metall bestehen, welches geringere Verdunkelung des Mittelbereiches in dem Stent ergeben würde. Beispielsweise könnten die Stentringe und -längselemente alle aus Titan oder einer Titanlegierung bestehen, ausgenommen die Endringe, die aus Gold gebildet werden könnten, welches dann mit Titan plattiert wird. So wäre die gesamte Außenoberfläche des Stents aus Titan, welches bekanntermaßen ein ver gleichsweise nichtthrombogenes Metall ist, während das Gold in den Endringen ein verbessertes fluoroskopisches Bild der Stentextremitäten liefert.
- Die Abmessungen der Stentringe sind typischerweise 0,1 bis 0,3 mm Dicke mit einer Breite von 0,1 bis 0,5 mm und einem Außendurchmesser D zwischen 2,0 und 30,0 mm je nach dem Lumendurchmesser des Gefäßes, in welches er eingesetzt wird. Die Länge des Stents könnte zwischen 1 und 10 cm liegen. Der Drahtdurchmesser für die Längselemente wäre typischerweise zwischen 0,05 und 0,5 mm.
- Obwohl die oben beschriebenen Gestaltungen getrennte Längselemente erläutern, die an mehreren Ringen befestigt sind, betrachtet diese Erfindung auch eine Ausgangsstentstruktur, die chemisch aus einem dünnwandigen Rohr mit einem ovalen Querschnitt geätzt wird. So würden das Oval und die Längselemente aus einem einzigen Metallstück gebildet, was die Notwendigkeit ausschließt, die Längselemente an den Ringen zu befestigen. In einer ähnlichen Weise könnte Laserbearbeitung oder maschinelle EDM-Bearbeitung angewendet werden, um den Stent aus einem dünnwandigen Rohr zu bilden.
- Es wird weiterhin unterstellt, daß eine Vorentfaltungsstentstruktur 20, wie sie in Fig. 9 gezeigt ist, aus einem dünnwandigen zylindrischen Rohr gebildet werden könnte, dessen Innendurchmesser etwas kleiner als der Außendurchmesser des Ballons 6 ist, der in Fig. 6 gezeigt ist. Ein Muster wie jenes, das in Fig. 6 oder 7 gezeigt ist, könnte auf einem dünnwandigen Metallzylinder photogeätzt werden. Die einstückige Struktur 20, die in Fig. 9 gezeigt ist, hat gefaltete Ovale 22 und Längselemente 23T 24B, 24R und (nicht gezeigt) 24L. Diese Vorentfaltungsstentstruktur 20 könnte auf dem ausdehnbaren Ballon befestigt werden, wobei der Stent genügend elastischen Rückprall hat, um fest nach unten auf den Ballon zu greifen.
- Demgemäß ist hier eine Stentstruktur beschrieben worden zur Aufrechterhaltung der Durchgängigkeit eines Gefäßes eines menschlichen Körpers, wobei die Stentstruktur eine Vielzahl von Strukturen hat, welche Längselemente bilden mit einer Wellenform, um die Längsstabilität zu verbessern.
- Zahlreiche andere Modifikationen, Anpassungen und alternative Ausgestaltungen sind selbstverständlich im Lichte der vorstehenden Lehre und im Rahmen der Erfindung möglich, wie in den anliegenden Ansprüchen offenbart ist.
Claims (16)
1. Stentstruktur (10) zur Aufrechterhaltung der Durchgängigkeit eines Gefäßes eines
menschlichen Körpers, wobei die Stentstruktur (10) eine Längsachse hat und erste
Strukturen aufweist, welche Längselemente (14L, 14R) bilden, wobei die ersten Strukturen
durch zweite Strukturen um die Längsachse verbunden sind, dadurch gekennzeichnet,
daß mindestens zwei Längselemente eine Wellenform besitzen, um die Längsflexibilität
zu verbessern.
2. Stentstruktur (10) nach Anspruch 1, wobei die zweiten Strukturen mindestens ein Paar
von längs versetzten Elementen (12) aufweisen, wobei die Längselemente (14) an
mindestens zwei der Elemente befestigt sind.
3. Stentstruktur (10) nach Anspruch 2, wobei die Elemente (10) in einer geschlossenen
Kontur geformt sind.
4. Stentstruktur (10) nach Anspruch 2, wobei die zweiten Strukturen ein Paar von
gegenüberliegenden Endelementen (12) aufweisen, die einen Strahlenundurchlässigkeitswert
haben, der sich von dem anderer Elemente (12), welche die zweiten Strukturen bilden,
unterscheidet.
5. Stentstruktur (10) nach einem der Ansprüche 2 bis 4, wobei die Elemente (12) eine
Metallzusammensetzung aufweisen.
6. Stentstruktur (10) nach einem der Ansprüche 2 bis 5, wobei die Elemente (12) Drahtteile
aufweisen.
7. Stentstruktur (10) nach einem vorhergehenden Anspruch, wobei die Längselemente eine
Metallzusammensetzung aufweisen.
8. Stentstruktur (10) nach einem vorhergehenden Anspruch, wobei die Längselemente (14)
Drahtteile aufweisen.
9. Stentstruktur (10) nach einem vorhergehenden Anspruch, wobei die Stentstruktur (10) aus
einem Metall gebildet ist, welches ein Formerinnerungsvermögen hat.
10. Stentstruktur (10) nach einem der Ansprüche 1 bis 4, wobei die Stentstruktur (10) als eine
Struktur aus einem Stück gebildet ist, die aus einem einzigen Metallstück durch
Photoätzen gebildet ist.
11. Stentstruktur (10) nach einem der Ansprüche 1 bis 4, wobei die Stentstruktur (10) aus
einer Struktur in einem Stück gebildet ist, die aus einem dünnwandigen Rohr durch
Funkenerosion bearbeitet (EDM machining) ist.
12. Stentstruktur (10) nach einem der Ansprüche 1 bis 4, wobei die Stentstruktur (10) aus
einer Struktur in einem Stück gebildet ist, wie aus einem dünnwandigen Rohr durch
Laserbearbeitung erstellt ist.
13. Stentstruktur (10) nach einem vorhergehenden Anspruch, wobei mindestens zwei der
Längsstrukturen (14L, 14R) mindestens einen geraden Abschnitt und mindestens einen
Abschnitt mit Wellenform haben, wobei jeder gerade Abschnitt kontinuierlich mit dem
mindestens einen Wellenformabschnitt verbunden ist, die geraden Abschnitte aller der
Längsstrukturen (14L, 14R) im allgemeinen parallel zu der Längsachse des Stent (10) liegen,
der wellenförmige Abschnitt jeder Längsstruktur (14L, 14R) im allgemeinen gekrümmte
Form hat, um jeder wellenförmigen Längsstruktur die Möglichkeit zu geben, leicht die
Länge während des Einführens der Stentstruktur in ein gekrümmtes Gefäß eines
menschlichen Körpers zu verändern.
14. Stentstruktur (10) nach Anspruch 13, wobei der wellenförmige Abschnitt der
Längsstrukturen (14L, 14R) sich erst in eine Umfangsrichtung erstreckt und sich dann in die
entgegengesetzte Umfangsrichtung erstreckt.
15. Stentstruktur (10) nach Anspruch 13 oder Anspruch 14, wobei jeder wellenförmige
Abschnitt an jedem seiner Enden mit einem geraden Abschnitt verbunden ist.
16. Stentstruktur (10) nach einem der Ansprüche 13 bis 15, wobei mindestens zwei der
Längsstrukturen jeweils eine Reihe von wellenförmigen Abschnitten aufweisen.
