Description
- Diese Erfindung bezieht sich auf einen Katheter mit einer Nadelelektrode für die Hochfrequenzablation, und insbesondere für einen derartigen Katheter für die Hochfrequenzablation des Myokards des Herzens.
- Die Hochfrequenzablation wird bei dem Versuch eingesetzt, Rhythmusstörungen des Herzens dadurch zu beseitigen, dass Läsionen im Myokard des Herzens erzeugt werden. Es hat sich jedoch gezeigt, dass es schwierig ist, Läsionen zu erzeugen, die groß genug und tief genug sind, insbesondere in einigen Bereichen des Herzens, beispielsweise in der Kammer, wo der Apex 1 cm dick sein kann und in der die abweichende Bahn, die die Rhythmusstörung verursacht, sich anstatt auf der endokardialen Seite auf der epikardialen Seite des Myokards befinden kann. Bei Einsatz der vorliegenden Verfahren ist die Tiefe der Läsion auf 2 bis 6 mm begrenzt. Es besteht deshalb ein Bedürfnis für einen neuen verbesserten Katheter, der tiefere und größere Läsionen im Myokard des Herzens ermöglicht.
- Die US 5,083,565 offenbart einen Katheter mit einem Paar von im Abstand angeordneten Elektroden, die aus dem distalen Ende des Katheters in das Gewebe vorgestoßen werden können, um ihnen so HF-Energie zuzuführen. Die DE 36 16 616 offenbart eine chirurgische Schneidvorrichtung mit einem Messer, das gleitend verschiebbar an dem distalen Ende angeordnet ist und dem HF-Energie zugeführt werden kann. Aus der US-A-4 641 649 ist eine Vorrichtung nach dem Oberbegriff des Anspruchs 1 bekannt.
- Insgesamt ist es ein Ziel der vorliegenden Erfindung, einen Katheter mit einer Nadelelektrode für eine Hochfrequenzablation des Myokards des Herzens zum Erzielen tieferer und größerer Läsionen in dem Myokard bereitzustellen.
- Ein weiteres Ziel der Erfindung besteht darin, einen Katheter der obigen Art bereitzustellen, bei welchem die gleiche Nadel für die Penetration und zum Zuführen der HF-Energie zum Myokard verwendet wird.
- Ein weiteres Ziel der Erfindung besteht in der Bereitstellung eines Katheters der obigen Art mit einer Nadel, die zurückziehbar ist.
- Ein weiteres Ziel der Erfindung besteht darin, einen Katheter der obigen Art bereitzustellen, der unabhängig von dem Katheter vorwärts bewegt werden kann.
- Ein weiteres Ziel der Erfindung besteht darin, einen Katheter der obigen Art bereitzustellen, dessen distales Ende gesteuert werden kann.
- In Übereinstimmung damit stellt die vorliegende Erfindung einen Katheter zur Verwendung in der Hochfrequenzablation des Myokards des Herzens bereit, wie in Anspruch 1 angegeben ist.
- Weitere Ziele und Merkmale der Erfindung ergeben sich aus der folgenden Beschreibung, in der bevorzugte Ausgestaltungen im Einzelnen in Verbindung mit den beiliegenden Zeichnungen dargelegt sind.
- Fig. 1 ist eine teilweise geschnittene Seitenansicht, die einen Katheter mit einer feststehenden Nadelelektrode für die Hochfrequenzablation des Myokards des Herzens zeigt.
- Fig. 2 ist ein Querschnitt längs der Linie 2-2 von Fig. 1.
- Fig. 3 ist eine Seitenansicht des Katheters und der bei ihm eingesetzten Steuerungen sowie einer zurückziehbaren Nadelelektrode, wobei das distale Ende des Katheters gesteuert werden kann.
- Fig. 4 ist ein Querschnitt längs der Linie 4-4 von Fig. 3.
- Fig. 5 ist eine vergrößerte Schnittansicht des distalen Endes des in Fig. 3 gezeigten Katheters, wobei die Nadelelektrode in einer zurückgezogenen Stellung gezeigt ist.

- Der Katheter mit einer Nadelelektrode für eine Hochfrequenzablation des Myokards des Herzens besteht insgesamt aus einem flexiblen langgestreckten Element, das wenigstens ein sich durch ihn hindurch erstreckendes Lumen sowie ein proximales und ein distales Ende hat. An dem distalen Ende des flexiblen langgestreckten Elements ist eine nadelartige Elektrode aus einem leitenden Material befestigt. Durch das Lumen erstreckt sich ein Leiter, der mit der Elektrode verbunden ist. Zum Zuführen von Hochfrequenzenergie zum Leiter und zur Elektrode ist eine Einrichtung vorgesehen.
- Wie insbesondere in Fig. 1 und 2 der Zeichnungen gezeigt ist, besteht der Katheter 11, der eine Nadelelektrode für die Hochfrequenzablation des Myokards des Herzens hat und die vorliegende Erfindung einschließt, aus einem flexiblen langgestreckten Element 12, das aus einem geeigneten Material, wie Kunststoff, hergestellt ist und das mit wenigstens einem, sich durch es hindurch erstreckenden Lumen 13 versehen ist. Das flexible langgestreckte Element 12 ist mit einem proximalen Ende 14 und einem distalen Ende 16 versehen. Das distale Ende 16 trägt eine HF-Ablationselektrode 21 und ist aus einem geeigneten leitenden Material, wie rostfreiem Stahl, hergestellt. Die Elektrode ist mit einem Mantel 22 versehen, der sich über das distale Ende 16 des flexiblen langgestreckten Elements 12 erstreckt und daran durch ein geeignetes Mittel, wie Klebstoff, festgelegt ist. Die Nadelelektrode 21 hat eine Nadel 23, die im Querschnitt zylindrisch ist und sich distal vom Mantel 22 erstreckt. Gewöhnlich kann die Nadelspitzenelektrode 23 eine geeignete Länge von beispielsweise 5 mm bis 10 mm und einen Durchmesser von 0,127 bis 0,508 mm (0,005 bis 0,020 Zoll) haben.
- Das flexible langgestreckte Element 12 kann einen geeigneten Außendurchmesser von beispielsweise 0,254 mm (0,010 Zoll) haben. Das Lumen 13 kann einen geeigneten Durchmesser von beispielsweise 0,178 mm (0,007 Zoll) haben, um eine Wandstärke für das rohrförmige Element 12 von etwa 0,038 mm (0,0015 Zoll) zu bilden. In dem Lumen 13 ist ein Leiter 26 angeordnet, der einen geeigneten Durchmesser von beispielsweise 0,102 mm (0,004 Zoll) hat. Der Leiter 26 kann aus einem geeigneten Material, wie Kupfer, hergestellt sein und ist mit einem Ende mit der Nadelspitze 23 durch geeignete Mittel, wie eine Verlötung bei 27, verbunden. Der Leiter 26 tritt aus dem proximalen Ende 14 des flexiblen langgestreckten Elements oder Glieds 12 aus und ist mit einem Kabel 31 verbunden, das an eine herkömmliche oder Hochfrequenz-Energieversorgung 32 angeschlossen ist.
- Im Folgenden werden nun kurz die Arbeitsweise und Verwendung des Katheters 11 beschrieben. Es sei angenommen, dass eine Kartographierung und eine Ablation beispielsweise in Verbindung mit einer Vorrichtung und einem Verfahren der anhängigen Anmeldung 071656,764, eingereicht am 15. Februar 1991, jetzt US-Patent 5,156,151 erfolgen und dass die Kartographierung ausgeführt wurde, um die genaue Stelle zu ermitteln, wo eine Ablation erreicht werden soll. Sobald eine gewünschte Position ermittelt worden ist, kann der Katheter 11 der vorliegenden Erfindung durch das Lumen vorwärts bewegt werden, das in dem Kartographiekatheter vorgesehen ist. Alternativ kann er außerhalb des Kartographiekatheters vorgeschoben werden oder er kann durch sich selbst vorgeschoben werden. Der Katheter 11 kann durch Drücken auf das flexible langgestreckte Element 12 vorgeschoben werden. Während seines Vorschiebens kann seine Vorwärtsbewegung fluoroskopisch beobachtet werden. Die Vorwärtsbewegung wird fortgesetzt, bis die gewünschte Stelle im Myokard erreicht ist. Dann wird der Katheter weiter vorwärts bewegt, damit die Nadelspitze 23 dazu gebracht wird, in das Myokard bis auf eine geeignete Tiefe von beispielsweise 2 bis 3 mm einzustechen. Sobald die Nadelelektrode ausreichend weit eingedrungen ist, wird aus der Energiezuführung 32 Hochfrequenzenergie zugeführt. Auf diese Weise können Läsionen mit einer Tiefe von 8 mm bis 10 mm und einem Durchmesser von etwa 5 mm bis 7 mm auf einfache Weise erreicht werden, um anomale Bahnen im Myokard zu zerstören, auch wenn das Myokard eine Dicke von 1 cm hat und sich die anomale Bahn anstatt auf der endokardialen Seite auf der epikardialen Seite des Myokards befindet.
- Sobald der Ablationsprozess abgeschlossen ist, kann der Katheter 11 zum Abschließen des Vorgangs entfernt werden.
- Eine weitere Ausführungsform des Katheters der vorliegenden Erfindung, der eine zurückziehbare Nadelelektrode verwendet, ist in den Fig. 3 bis 6 gezeigt. Der Katheter 41 und die Steuerkonsole 42, die mit ihm verwendet wird, sind in Fig. 3 gezeigt. Der Katheter besteht aus einem flexiblen langgestreckten Element 43, das aus einem geeigneten Material, wie Kunststoff, hergestellt ist und mit einer Vielzahl von Lumina, beispielsweise sechs Lumina 46 bis 51, die am Umfang um den Außenrand des flexiblen langgestreckten Elements 43, wie in Fig. 4 gezeigt, angeordnet sind, und mit einem größeren zentralen Lumen 52 versehen ist. Das flexible langgestreckte Element 43 hat ein proximales Ende 56 und ein distales Ende 57. In dem distalen Ende 57 ist ein Betätigungselement 61 für eine Gleitverschiebungsbewegung in dem zentralen Lumen 52 angeordnet. Das Betätigungselement 61 kann aus einem geeigneten Material, wie Metall oder Kunststoff, hergestellt sein. Es besteht aus einem zylindrischen, relativ starren Rohr oder einer Stange 62, das/die mit einem Paar von Kreisscheiben 63 und 64 an gegenüberliegenden Enden versehen ist, die sich in Längsrichtung des Lumens 62 zwischen einer Kreisscheibe 66 bewegen, die an der das Lumen 52 bildenden Seitenwand durch ein geeignetes Mittel, wie Klebstoff, befestigt sind. Die Scheibe 66 ist mit einem Loch 67 versehen, durch welches sich das Rohr oder die Stange 62 hindurch erstreckt. Das Rohr oder die Stange 62 ist als rohrförmiges Element gezeigt, das mit einer sich axial erstreckenden Bohrung 68 versehen ist.
- An dem distalen Ende des Betätigungselements 61 ist eine nadelartige Elektrode 71 angebracht und sitzt, wie gezeigt, an der Scheibe 64 und erstreckt sich distal davon fluchtend ausgerichtet zur Längsachse des Lumens 52 der Längsachse für das Rohr oder die Stange 62. Wie gezeigt, kann die Elektrode 71 in einem Stück mit dem Betätigungselement 61 ausgebildet sein. Die nadelartige Elektrode 71 sowie das Betätigungselement 61 können aus einem geeigneten elektrodenartigen leitenden Material hergestellt sein. Sie kann beispielsweise aus rostfreiem Stahl oder alternativ aus goldplattiertem Kupfer bestehen. Die nadelartige Elektrode 71 hat einen Durchmesser im Bereich von 0,127 bis 0,203 mm (0,005 bis 0,008 Zoll) und kann eine Länge im Bereich von 5 mm bis 1,0 cm haben. Wie gezeigt, ist die nadelartige Elektrode 71 mit einer scharfen Spitze 72 versehen. An dem distalen Ende des flexiblen langgestreckten Elements 43 ist eine kreisförmige Dichtung 76 angebracht und wird darin durch geeignete Mittel, wie Klebstoff, gehalten. Die Dichtung 76 ist mit einem sich diametral erstreckenden Schlitz 77 versehen, durch den sich die nadelartige Elektrode 71 erstrecken und zurückgezogen werden kann. Die Dichtung 76 dient dazu, das Eintreten von Flüssigkeiten, wie Blut, in das zentrale Lumen 52 zu verhindern.
- Zum Vorwärtsschieben und Zurückziehen des Betätigungselements 61 mit der davon getragenen nadelartigen Elektrode 71 ist eine Einrichtung vorgesehen, die aus Schraubenfedern 81 und 82 besteht, die koaxial auf dem Betätigungselement 61 angeordnet sind, wobei die Feder 81 mit ihrem proximalen Ende an der Scheibe 63 und mit ihrem distalen Ende an der Scheibe 6f3 in Eingriff steht, die an dem flexiblen langgestreckten Element 43 festgelegt ist. In ähnlicher Weise steht die Feder 82 mit ihrem proximalen Ende mit der Scheibe 66 und mit ihrem distalen Ende mit der Scheibe 64 in Eingriff. Die Federn 81 und 82 bestehen aus einem Material mit Formgedächtnis, beispielsweise aus einer Nickel-Titan-Legierung, wie z. B. Nitinol.
- Zur Zuführung elektrischer Energie zu den Federn 81 und 82 ist eine Einrichtung vorgesehen, die aus einem Leiter 86 besteht, der in dem Lumen 47 angeordnet ist, in die Bohrung 68 eintritt, durch ein Loch 84 in dem Rohr 62 hindurchgeht und mit dem proximalen Ende der Feder 81 verbunden ist. In ähnlicher Weise ist in dem Lumen 51 ein Leiter 86 vorgesehen, der in die Bohrung 68 eintritt, durch ein Loch 87 hindurchgeht und mit dem distalen Ende der Feder 82 verbunden ist. Das proximale Ende der Feder 82 und das distale Ende der Feder 81 sind miteinander durch ein Verbindungsglied 88 verbunden, das sich durch ein Loch (nicht gezeigt) in der Scheibe 66 erstreckt. Das Verbindungsglied 88 ist mit einem gemeinsamen Rückkehrleiter 89 verbunden, der in dem Lumen 49 angeordnet ist. Die Leiter 83, 86 und 89 erstrecken sich zum proximalen Ende des flexiblen langgestreckten Elements 41 und sind in einem flexiblen Kabel 92 verbunden, das mit einem Stecker 92 versehen ist. Die Leiter 83 und 136 sind mit gewickelten Abschnitten 83a und 86a versehen, um die Hin- und Herbewegung des Betätigungselements 61 aufzunehmen. Der Stecker 92 ist für eine Verbindung mit einem passenden Stecker 93 ausgelegt, der mit einem weiteren Kabel 94 verbunden ist, das an die Steuerkonsole 42 angeschlossen ist. Die Steuerkonsole 42 ist so gebaut, wie sie in der anhängigen Anmeldung Nr. 071893,770, eingereicht am 12. Dezember 1991, beschrieben ist, und hat einen Gleitmechanismus 96, der, wie in Fig. 3 gezeigt ist, nach oben und nach unten bewegt werden kann, um eine Bewegung der Nadelelektrode 71 zwischen einer ausgefahrenen und einer zurückgezogenen Stellung herbeizuführen, indem sie vorwärts oder rückwärts oder, mit anderen Worten, in eine Vorwärtsrichtung und eine entgegengesetzte Richtung bewegt wird. Auf diese Weise kann durch Zuführen von Energie zur Feder 81 durch Bewegen des Gleitmechanismus in der entgegengesetzten Richtung die Feder 81 zum Expandieren gebracht werden, um eine Kompression der Feder 82 herbeizuführen und um die Nadelelektrode 71 in die in Fig. 5 gezeigte Position zurückzuziehen, in der sie sich hinter der Dichtung 76 befindet. Wenn umgekehrt der Gleitmechanismus 96 zur Vorwärtsrichtung bewegt wird, wird die Feder 82 zum Expandieren gebracht, um die Nadelelektrode 72 durch den Schlitz 77 in der Dichtung 76 auszufahren und um über das distale Ende des flexiblen langgestreckten Elements 43 vorzustehen.
- Die Steuerkonsole 42 ist mit einem Ein-Aus-Schalter 97 und mit einer Anzeigeleuchte 98 in Form einer lichtemittierenden Diode versehen, um anzuzeigen, wenn Strom eingeschaltet ist. Die Steuerkonsole ist ferner mit einem Steuerhebel 101 versehen, der um 360º geschwenkt werden kann, um die Richtungsbewegung des distalen Endes des flexiblen langgestreckten Elements 43 herbeizuführen. Um dies zu erreichen, sind drei langgestreckte Elemente aus einem Material mit einem negativen Ausdehnungskoeffizienten, wie Flexinol, geliefert von Toki, Japan, in den Lumina 46, 48 und 50 vorgesehen und mit den Leitern 102, 103 und 104 verbunden, die jeweils in den Lumina 46, 48 und 50 vorgesehen sind. Diese Leiter sind in das Kabel 91, in die Stecker 92 und 93 und das Kabel 94 für eine Verbindung mit der Schaltung geleitet, die in der Steuerkonsole 42 vorgesehen ist. Wie in der anhängigen Anmeldung 07/793,858, eingereicht am 18. November 1991, beschrieben ist, sind die distalen Enden der langgestreckten Elemente des Materials mit dem negativen Koeffizienten mit einem gemeinsamen Rückkehrleiter 87 verbunden. Wie in der anhängigen Anmeldung 07/793,858, eingereicht am 18. November 1991, erläutert ist, kann die Bewegung des distalen Endes des flexiblen langgestreckten Elements 43 durch Betrieb des Steuerhebels 101 wie gewünscht gesteuert und gerichtet werden.
- Der Katheter 41 kann zur Ausführung eines Ablationsvorgangs genauso wie der in den Fig. 1 und 2 gezeigte Katheter 11 verwendet werden. Er kann zusammen mit der Vorrichtung zum Kartographieren und für die Ablation verwendet werden, die in der anhängigen Anmeldung 07/656,764, eingereicht am 15. Februar 1991 beschrieben ist. Der Katheter hat den zusätzlichen Vorteil, dass die Nadelelektrode 72 während ihres Einführens in das Gefäß eines Patienten sich in der zurückgezogenen Stellung befindet, so dass sie die Seitenwand des Lumens, in welchem sie vorwärts bewegt wird, nicht versehentlich durchdringen kann. Außerdem hat der Katheter 41 den Vorteil, dass sein distales Ende durch Verwendung des Steuerhebels 101 so positioniert werden kann, dass es in die gewünschte Position bewegt werden kann, wenn das proximale Ende von dem Chirurgen vorwärts bewegt wird. Wenn der Kartographievorgang den zu ablatierenden Bereich bestimmt hat, kann der Katheter 41 in diese Position vorwärts und in Kontakt mit dem Myokard bewegt werden. Der Gleitmechanismus 96 kann dann betätigt werden, indem er in die Vorwärtsposition bewegt wird, um die Feder 82 dazu zu bringen, die Nadelelektrode 72 durch den Schlitz 77 in der Dichtung 76 nach vorne zu bewegen und in das Myokard auf die gewünschte Tiefe einzudringen. Sobald das Eindringen erreicht ist, kann aus der Steuerkonsole 42 Hochfrequenzenergie zur Nadelelektrode 72 geführt werden, um die gewünschte Ablation auszuführen. Wie vorstehend erläutert, ist es aufgrund der Verwendung der tief eindringenden Elektrode 72 möglich, eine relativ große und tiefe Läsion bei relativ kleinen Energiebeträgen zu erreichen. Beispielsweise können, wie vorher beschrieben, Läsionen mit einer Tiefe von 10 mm und einem Durchmesser von 5 bis 10 mm erreicht werden, wobei eine Energie von nur 15 Joules verwendet wird. Nach dem Ausführen der Ablation kann der Katheter 41 auf herkömmliche Weise entfernt werden.
- Eine weitere Ausführungsform eines Katheters mit einer Nadelelektrode für eine Hochfrequenzablation ist in Fig. 7 und 8 gezeigt. Wie dort dargestellt ist, besteht der Katheter 121 aus einem flexiblen langgestreckten Element 122 aus einem geeigneten Kunststoff der vorher beschriebenen Art und hat ein proximales Ende 123 und ein distales Ende 124. Es ist mit einem großen zentralen Lumen 126, das sich über seine Länge erstreckt, und mit zusätzlichen kleineren Lumina 127, 128, 129 und 130 versehen, die am Umfang um das zentrale Lumen 12c3 herum im Abstand angeordnet sind. Die Lumina 127, 128 und 130 haben einen Abstand von etwa 120º voneinander. Wie bei den vorherigen Ausführungsformen erläutert wurde, sind für die Steuerbarkeit des distalen Endes 124 Leiter 136, 137 und 138 in den Lumina 127, 128 und 130 und ein Rückführleiter 139 am Lumen 129 vorgesehen. Es sind wenigstens drei solcher Leiter vorgesehen, die in einem Abstand von 120º voneinander oder weniger angeordnet sind. Die distalen Enden der Leiter 136, 137 und 138 sind aus einem Material hergestellt, das einen negativen Ausdehnungskoeffizienten hat, beispielsweise aus Flexinol, geliefert von Toki, Japan. Die Leiter 136, 137, 138 und 139 sind über ein Kabel 141 mit der Energiezufuhr 142 verbunden, die eine Joystick-Steuerung 143 zum Steuern der Bewegung des distalen Endes 124 des flexiblen langgestreckten Elements 122 aufweist.
- In dem distalen Ende des zentralen Lumens 126 ist eine Nadel 146 angeordnet, die aus einem geeigneten Material hergestellt ist, beispielsweise aus rostfreiem Stahl. Die Nadel 146 ist mit einer Nabe 147 versehen, die am distalen Ende eines flexiblen Druck-Zug-Drahts 148 angeordnet ist, der aus einem geeigneten Material hergestellt ist, beispielsweise aus einem rohrförmigen Element aus rostfreiem Stahl, wie beispielsweise aus einem Hyporohr. Das rohrförmige Element 148 erstreckt sich über der Länge des zentralen Lumens 126 und in ein Gehäuse 151, das an dem proximalen Ende 123 des flexiblen langgestreckten Elements 122a angeordnet ist. Das Gehäuse 151 ist mit einem langgestreckten Schlitz 152 versehen, der in eine zylindrische Aussparung 153 mündet, die das proximale Ende des Druck-Zug- Drahts 148 aufnimmt. Durch die Aussparung 153 erstreckt sich ein Schieber 154, der mit einem Fingereingrifiselement 156 an einem Ende und mit einem Bund 157 am anderen Ende in der Aussparung 153 angeordnet ist. Der Druck-Zug-Draht 148 erstreckt sich durch den Bund 157 und ist auf seinem Durchgang durch geeignete Mittel, wie eine Madenschraube 158, festgelegt. Am distalen Ende des flexiblen langgestreckten Elements 122 ist eine Dichtung 161 vorgesehen, die einen Schlitz 162 aufweist. Die Dichtung 161 dient dazu, den Eintritt von Flüssigkeit, wie Blut, in das zentrale Lumen 126 zu verhindern, und erlaubt das Ausfahren und Zurückziehen der Nadel 146 durch Betätigung des Schiebers 154. Wenn der Katheter 121 nach vorne in eine gewünschte Stellung bewegt worden ist und das distale Ende in der vorstehend beschriebenen Weise durch die Verwendung eines Steuerhebels 143 gesteuert wird, kann die Nadel 146 von Hand in eine Position vorwärts bewegt werden, um in das Myokard auf die gewünschte Tiefe einzudringen. Anschließend kann HF-Energie aus der Energiequelle 142 zur Nadel zugeführt werden, wie es in Verbindung mit den vorherigen Ausführungsformen beschrieben ist. Der Katheter der vorliegenden Ausführungsform hat den Vorteil, dass er relativ einfach ist und die Nadel von Hand vom Arzt vorwärts bewegt und zurückgezogen werden kann. Nach Abschluss des Ablationsvorgangs kann die Nadel 146 von der Rückführeinrichtung oder dem Schieber 154 zurückgeführt werden. Dann kann der Katheter entfernt werden.
- Aus den vorstehenden Ausführungen wird ersichtlich, dass ein Katheter bereitgestellt ist, der eine Nadelelektrode hat, die für eine Radiofrequenzablation des Myokards des Herzens verwendet werden kann. Durch Einsatz einer Nadelelektrode, die tief in das Myokard eindringt, ist es möglich, tiefe Läsionen zu erreichen, die breite Bereiche abdecken, um den gewünschten Effekt des Zerstörens von abweichenden Wegen zu erreichen, wodurch Rhythmusstörungen des Herzens beseitigt werden. Durch Bereitstellen einer zurückziehbaren Nadelelektrode kann ein unbeabsichtigtes Durchbohren einer Gefäßwand ausgeschlossen werden.
Claims (8)
1. Katheter (11, 41, 121) zur Verwendung bei der Hochfrequenzablation von Myokard
am Herzen eines Patienten
- mit einem flexiblen langgestreckten Element (12, 43, 122), das wenigstens ein
Lumen (13, 52, 126) hat, das sich durch das ein proximales und ein distales
Ende aufweisende Element erstreckt,
- mit einer Elektrode (21, 71, 146), die nur aus leitendem Material hergestellt und
an dem distalen Ende des flexiblen langgestreckten Elements (12, 43, 122)
angebracht ist,
- mit einem sich durch das Lumen (13, 52, 126) erstreckenden und mit der
Elektrode (21, 71, 146) verbundenen Leiter (26, 86, 151) und
- mit Einrichtungen (32, 42, 142) zum Zuführen von Hochfrequenzenergie zu dem
Leiter (26, 86, 151) und der Elektrode (21, 71, 146) um die Ablation des
Myokards herbeizuführen,
dadurch gekennzeichnet,
- dass die nur aus leitendem Material hergestellte Elektrode (21, 71, 146) die Form
einer einzelnen nadelartigen Elektrode aufweist, die einen Durchmesser von
0,051 bis 0,203 mm (.002 Zoll bis .008 Zoll) und eine Länge im Bereich von 0,5
bis 1 cm hat.
2. Katheter (11, 41, 121) nach Anspruch 1, der weiterhin dadurch gekennzeichnet ist,
dass das flexible langgestreckte Element (12, 43, 122) ausreichend starr ist, dass an
sein proximales Ende eine Kraft angelegt werden kann, um die nadelartige Elektrode
(21, 71, 146) zum Eindringen in das Myokard zu veranlassen.
3. Katheter (41, 121) nach Anspruch 1, welcher weiterhin von dem distalen Ende des
flexiblen langgestreckten Elements (43, 122) getragene Einrichtungen (81, 82, 156)
zum Ausfahren und Einziehen der nadelartigen Elektrode (71, 146) aufweist.
4. Katheter (41) nach Anspruch 3, welcher weiterhin dadurch gekennzeichnet ist, dass
die Einrichtungen zum Ausfahren und zum Einziehen der nadelartigen Elektrode (71)
ein erstes und ein zweites Federelement (81, 82) aus einem Material mit negativem
Ausdehnungskoeffizienten und Einrichtungen (62, 63, 64) für ein solches Festlegen
der Federelemente (81, 82) an der nadelartigen Elektrode (71) hat, dass, wenn das
eine Element des ersten und zweiten Federelements (81, 82) aktiviert wird, die
nadelartige Elektrode (71) eingezogen wird, und, wenn das andere Element des
artige Elektrode (71) eingezogen wird, und, wenn das andere Element des ersten und
zweiten Federelements (81, 82) aktiviert wird, die nadelartige Elektrode (71)
ausgefahren wird.
5. Katheter (121) nach Anspruch 4, welcher weiterhin dadurch gekennzeichnet ist, dass
die Einrichtungen zum Ausfahren und Einziehen der nadelartigen Elektrode (146)
einen Druck-Zug-Draht (148) mit einem proximalen und einem distalen Ende aufweist,
der sich durch das Lumen (126) in dem flexiblen langgestreckten Element (122)
erstreckt, wobei die nadelartige Elektrode (146) an dem distalen Ende des Druck-Zug-
Drahts (148) angebracht ist und an dem proximalen Ende des Druck-Zug-Drahts
(148) Einrichtungen (156) für sein Vorschieben und Zurückziehen von Hand festgelegt
sind.
6. Katheter (41, 121) nach Anspruch 1, welcher weiterhin eine Dichtung (76, 161)
aufweist, die an dem distalen Ende des flexiblen langgestreckten Elements (43, 122)
angeordnet ist und in sich eine abdichtbare Öffnung (76, 161) aufweist, durch welche die
nadelartige Elektrode (71, 146) ausgefahren und eingezogen werden kann.
7. Katheter nach Anspruch 1, welcher weiterhin Einrichtungen (101, 143) aufweist, um
eine Steuerbewegung des distalen Endes des flexiblen langgestreckten Elements (43,
122) herbeizuführen.
8. Katheter (41, 121) nach Anspruch 7, welcher weiterhin dadurch gekennzeichnet ist,
dass die Einrichtungen (101, 143) zum Herbeiführen einer Steuerbewegung
wenigstens drei langgestreckte Glieder (46, 48, 50, 136-138), die annähernd um 120º oder
weniger voneinander weg in dem flexiblen langgestreckten Element (43, 122)
angeordnet sind, und Einrichtungen zum selektiven Anlegen von Energie an die flexiblen
langgestreckten Glieder (46, 48, 50, 136-138) aufweist, um die Steuerbewegung des
distalen Endes des flexiblen langgestreckten Elements (43, 122) herbeizuführen.
