Description
- Die Erfindung betrifft die perioperative Anwendung von razemischen und Dextro-4-[1-(2,3-dimethylphenyl)ethyl]-1H-imidazol und deren nichttoxischen pharmazeutisch annehmbaren Salzen. In Gegenwart dieser 4-substituierten Imidazolverbindungen benötigt man weniger Anästhetikum, um ein gegebenes Niveau der operativen Anästhesie zu erzielen. Diese Verbindungen vermindern auch die zirkulierenden Catecholamine und verringern dadurch den Streß, dem das cardiovaskuläre System in der perioperativen Periode ausgesetzt ist.
- Razemisches 4-[1-(2,3-Dimethylphenly)ethyl]-1H-imidazol, das unter dem Namen Medetomidine bekannt ist, hat die Formel
- und ist ein selektiver und potenter alpha&sub2;-Adrenorezeptoragonist. Es wurde früher schon beschrieben, z.B. in der Europäischen Patentveröffentlichung Nr. 72615 als ein antihypertensives Mittel und in der Europäischen Patentveröffentlichung Nr. 187471 als ein in der Tiermedizin sedatives-analgetisches Mittel.
- Es wurde weiterhin festgestellt, daß diese Verbindung auch anxiolytische Wirkungen besitzt und deshalb bei der Behandlung von allgemeinen Angstzuständen, panischen Störungen und verschiedenen Entzugssymtomen verwendet werden kann.
- Da die Verbindung der Formel (I) ein asymmetrisches Kohlenstoffatom besitzt, kann diese Verbindung, die eine razemische Mischung von zwei optisch aktiven Stereoisomeren ist, in die d- und l-Enantiomeren aufgespalten werden. Insbesondere das d-Enantiomer zeigt eine überraschend hohe Selektivität und Potenz als ein alpha&sub2;-Adrenorezeptoragonist.
- Von Clonidin, einem bekannten antihypertensiven Arzneimittel (U.S.-Patent 3202660) wurde kürzlich berichtet, daß es wertvolle Eigenschaften bei der Verwendung auf dem perioperativen Gebiet entwickelt. Diese Beobachtungen wurden in zwei Schriften berichtet: Flacke J.W., Bloor B.C., Flacke W.E., Wong D., Dazza S., Stead S.W., Laks H.: Reduced narcotic requirement by clonidine with improved hemodynamic and adrenergic stability in patients undergoing coronary bypass surgery; Anethesiology 67: 909-917, 1987 and Ghignone M., Calvillo O., Quintin L.: Anesthesia and hypertension: The effect of clonidine on perioperative hemodynamics and isoflurane requirements; Anesthesiology 67: 901-908, 1987.
- Das erste Papier bezieht sich auf eine Untersuchung von Patienten, die Bypass-Operationen am Herzen unterworfen wurden. Clonidine wurde sowohl als Prämedikation als auch intraoperativ verwendet. Die Verwendung von Clonidine verursachte eine Verringerung der anästhetischen Erfordernisse und eine verringerte Plasmakonzentration von Catecholaminen. Das zweite Papier bezieht sich auf eine Untersuchung des Einflusses von Clonidine-Pramedikation auf perioperative Hämodynamik bei hypertensiven Patienten. Clonidine verringert die Labilität des Herzschlages und des Blutdruckes bei hypertensiven Patienten und die Erfodernis für Anästhesie um etwa 40 %.
- Es wurde nun festgestellt, daß Medetomidine und das d-Enantiomer von 4-[1-(2,3-Dimethylphenyl)ethyl]-1H-imidazol und deren pharmazeutisch annehmbaren Salze Eigenschaften entwickeln, die sie brauchbar für die perioperative Versorgung von Säugern machen. Das d-Enantiomer ist in dieser Hinsicht besonders geeignet.
- Die vorliegende Erfindung betrifft somit die Verwendung einer Verbindung, die razemisches oder Dextro- 4-[1-(2,3-Dimethylphenyl)ethyl]-1H-imidazol ist oder eines nichttoxischen pharmazeutisch annehmbaren Salzes davon bei der Herstellung eines Arzneimittels für die perioperative Behandlung von Säugern zur Verringerung der Ansprechung des autonomen Nervensystems auf Streßreize während der Operation.
- Die vorliegende Erfindung stellt auch eine pharmazeutische Zusammensetzung zur Verfügung, umfassend Dextro- 4-[1-(2,3-Dimethylphenyl)ethyl]-1H-imidazol oder ein nichttoxisches pharmazeutisch annehmbares Salz davon und einen pharmazeutisch annehmbaren Träger oder ein Verdünnungsmittel.
- Medetomidine und dessen d-Enantiomer mindern wesentlich die anästhetischen Anforderungen und andere Streßfaktoren während der Operation. Die MAC-einsparenden (MAC = minimale alveolare Konzentration) und cardiovaskuläre Wirkungen dieser Verbindungen bei Haloethan-anästhetisierten Hunden wird wie folgt berichtet: Anästhesie wurde durch Maskeninhalation von Haloethan in Sauerstoff bei männlichen Mischlingshunden (8 bis 11 kg) eingeleitet. Im Anschluß an eine tracheale Intubation wurde die Ventilation überwacht, um eine Normocarbie aufrechtzuerhalten (CO&sub2; ET -4,5 %). Perkutan wurden Katheter eingelegt für: 1) intraarterielle Blutgasbestimmung und Blutdruckaufzeichnung (femoral Arterie); 2) Überwachung des pulmonaren arteriellen und zentralvenösen Druckes und Bestimmung des Herzminutenvolumens (Thermodilution); und 3) für die intravenöse Verabreichung von Flüssigkeiten und Arzneimitteln. Die endotidalen Halothan- und CO&sub2;-Konzentrationen (Infrarotanalyse), Herzschlag und -rhythmus (Ableitung II des EKG), der systemische arterielle Druck, der zentrale venöse Druck und der pulmonare arterielle Druck wurden kontinuierlich angezeigt und aufgezeichnet. Mittels Isolierdecken und Wärmelampen wurde die Temperatur bei 37ºC gehalten. Nach einer zweistündigen Gleichgewichtsperiode wurde die MAC für Halothan bestimmt, wie vorher beschrieben, (Nichaolls E.A., Louis G.L., Prokotimer P.G., Maze M.: Haloethane anesthetic requirements are not affected by aminophylline treatment in rats and dogs. Anethesiology 65:637-641, 1986) und die Grundlinie für die hämodynamische Funktion (mittlerer arterieller Blutdruck, Herzschlag, zentral venöser Druck, pulmonaler arterieller diastolischer Druck, pulmoneller Druck der verschlossenen Arterie, Herzminutenvolumen und der abgeleitete systemische vaskuläre Widerstand) wurden bestimmt. Medetomidine in der DL- (n-7), D- (n-5) und L-Form wurde in getrennten Versuchen bei jeder der drei Dosen (1, 3 und 10ug/kg) durch den rechten arteriellen Eingang während 15 Minuten verabreicht, wobei die Hunde auf ihren individuellen MAC für Halothan gehalten wurden. Zehn Minuten nach Beendigung der Infusion (zu diesem Zeitpunkt war das Profil stabil) wurden die hämodynamischen Funktionen wieder bestimmt. Zu diesem Zeitpunkt wurde auch das arterielle Blut gesammelt für 1) Messung der Gasspannung und des Säure/Base-Status; und 2) des umlaufenden Noreepinephrins. Die MAC-Bestimmung wurde dann wiederholt und daran anschließend wurd die Medetomidine-Dosis verabreicht, und diese Routine wurde wiederholt. Es wurden die Daten mittels ANOVA für die wiederholten Messungen anschließend durch einen gepaarten t-Test mit Bonferroni-Korrektion verglichen. Ein p-Wert von < 0,05 wurde als Niveau für die statistische Signifikanz angesehen. MAC für Halothan nahm signifikant im Anschluß an die DL-Medetomidine-Verabreichung in einer Dosis-abhängigen Weise in einem solchen Maße ab, daß bei der höchsten Dosis (10ug/kg) der Halothan-MAC weniger als < 0,1 % betrug.
- Diese Wirkung konnte auch mittels des D-Isomers erzielt werden, während das L-Isomer ohne Wirkung war. Keines der Isomeren veränderte den mittleren arteriellen Druck, während nur das D-Isomer signifikant den Herzschlag und das Herzminutenvolumen verringerte.
- Ergebnisse: MAC für Halothan nahm signifikant im Anschluß an die Medetomedine-Verabreichung in einer Dosis-abhängigen Weise ab. Das D-Enantiomer zeigte den gleichen MAC-einsparenden Effekt (Tabelle 1). - Tabelle 2 und 3 zeigen den verringerten Herzschlag und das Herzminutenvolumen, das sich bei der Verabreichung der Verbindungen ergibt. - Die Verbindungen können i.v., i.m. oder oral verabreicht werden. Der bevorzugte Dosisbereich ist 1 ug/kg bis 10 ug/kg.
- Razemisches Medetomidine (50 ug, intravenös) wurde zehn gesunden Patienten als Prämedikation 20 bis 30 Minuten vor einer chirurgischen Extraktion des dritten Hackenzahns in statistischer Weise, im Kreuzversuch und bei einer Placebo-kontrollierten Phase II einer klinischen Untersuchung verabreicht. Eine Lokalanästhesie erfolgte mit Lidocain/Epinephrin. Jeder Patient wurde zweimal im Abstand von wenigstens drei Wochen operiert. Medetomedin wurde als deutlich überlegen gegenüber dem Placebo sowohl bei den Patienten als auch bei den Zahnchirurgen eingeschätzt (s. Tabelle 4). Es wurden keine nachteiligen Wirkungen festgestellt, und zwar weder bei den hämodynamischen Parametern, bei den klinischen-chemischen Tests noch bei dem subjektiven Befinden der Patienten. Claims (4)
1. Verwendung einer Verbindung, die ein racemisches oder
rechtsdrehendes 4-[1-(2,3-Dimethylphenyl)ethyl]-1H-imidazol oder ein
nicht-toxisches pharmazeutisch akzeptierbares Salz davon ist, beim
Herstellen eines Medikamentes zur perioperativen Behandlung von
Säugern zur Verringerung der Reaktion des autonomen Nervensystemes
auf Streßreize während der Operation.
2. Verwendung einer Verbindung nach Anspruch 1, die
rechtsdrehendes 4-[1-(2,3-Dimethylphenyl)ethyl]-1H-imidazol oder ein nicht-
toxisches pharmazeutisch akzeptierbares Salz davon ist.
3. Verwendung einer Verbindung nach Anspruch 1 oder 2, bei der
das Medikament zur Benutzung als anxiolytische analgetische

4. Verwendung einer Verbindung nach einem der Ansprüche 1 bis 3,
bei der das Medikament zur präoperativen Benutzung als
beruhigendes Analgetikum zum Verstärken des Effektes von Anästhesiemitteln
und somit zum Verringern der anästhetischen Anforderung dient.
