Abstract
Gelenkprothese, insbesondere Kniegelenk-Endoprothese, zum Ausbilden einer Gelenkverbindung zwischen einem ersten Knochen und einem zweiten Knochen, mit einer ersten Komponente (4), die zumindest mittelbar mit dem ersten Knochen verbunden ist, einer zweiten Komponente (2), die zumindest mittelbar mit dem zweiten Knochen verbunden ist, und mit einem zwischen der ersten Komponente (4) und der zweiten Komponente (2) beweglich gelagerten Zwischenelement (3), welches mit einer ersten Fläche (7) an der ersten Komponente (4) und mit einer zweiten Fläche (6) an der zweiten Komponente (2) anliegt, dadurch gekennzeichnet, daß die erste Fläche (7) des Zwischenelements (3) eine nicht-sphärische Fläche ist, wobei ein erster Krümmungsradius (R1) der ersten Fläche (7) des Zwischenelements (3) in medial-lateraler Richtung und ein zweiter Krümmungsradius (R3) der ersten Fläche (7) des Zwischenelements (3) in ventral-dorsaler Richtung verschieden sind.
Description
- Die Erfindung betrifft eine Gelenkprothese zum Ausbilden einer Gelenkverbindung zwischen einem Paar menschlicher oder tierischer Knochen.
- Beispielsweise ist aus der
EP 0 442 330 A2 eine Kniegelenk-Endoprothese bekannt, welche ein Femurteil, ein Meniskuselement und ein Tibiateil umfaßt. Das Femurteil und das Tibiateil liegen mit Lagerflächen an zugeordneten Lagerflächen des Meniskuselementes an. Das Femurteil ist dabei mit einer kugelförmigen Lagerfläche versehen, welche an der gleichfalls kugelförmigen und konzentrisch angeordneten Lagerfläche des Meniskuselements anliegt. Das Tibiateil ist konvex ausgebildet und weist eine zylindrische Lagerfläche auf, an welcher die zweite entsprechend ausgebildete und koaxial angeordnete Lagerfläche des Meniskuselements anliegt. - Weiterhin ist aus der
DE 195 29 824 A1 eine bikondyläre Knie-Endoprothese mit einer am unteren Femurkopf zu befestigenden, in seitlicher Ansicht im wesentlichen U-förmigen Femurkomponente bekannt, wobei die Femurkomponente einen Schenkel und Steg aus zwei mit gegenseitigem Abstand angeordneten, im wesentlichen parallelen Kondylusabschnitten besteht, deren stetig in die Außenseite des anderen Schenkels übergehende, gekrümmte Außenseiten die Femurlaufflächen bilden, und von deren Innenseite im Bereich des Steges wenigstens ein Verankerungszapfen vorsteht. Eine am Tibiaplateau zu befestigende Tibiakomponente besteht aus einem plattenförmigen Tibiatray und einem sich im wesentlichen senkrecht von dessen Unterseite erstreckenden Tibiateil, wobei zwischen der Femurkomponente und der Tibiakomponente ein relativ zu beiden bewegliches, im wesentlichen plattenförmiges Meniskusteil angeordnet ist, dessen Unterseite gleitbar auf der Oberseite des Tibiatrays aufliegt und dessen Oberseite zwei mit gegenseitigem Abstand angeordnete Lagerschalen ausgebildet sind, in denen jeweils ein Kondylusabschnitt der Femurkomponente gleitbar abgestützt ist. - Nachteilig bei den aus den obengenannten Druckschriften bekannten Endoprothesen ist insbesondere, daß bedingt durch die nicht naturgetreue Form der Meniszi die physiologischen Kniegelenksfunktionen bei Bewegung und Belastung nicht befriedigend erfüllt werden. Insbesondere treten örtliche Belastungsspitzen in Folge von Linien- oder Punktberührungen auf. In der Folge können Bewegungsstörungen, ungleichmäßige Abnützung der Prothese, Luxationen oder Lockerungen der tibialen oder femuralen Komponente der Endoprothese auftreten.
- Der Erfindung liegt demnach die Aufgabe zugrunde, eine Gelenkprothese zu schaffen, welche möglichst naturgetreue physiologische Bewegungsabläufe ermöglicht, ohne daß es zu Fehlstellungen und nachfolgenden Fehlfunktionen oder Zerstörung der Prothese kommen kann.
- Die Aufgabe wird durch eine Gelenkprothese gemäß den kennzeichnenden Merkmalen des Hauptanspruchs in Verbindung mit den gattungsbildenden Merkmalen gelöst.
- Der Erfindung liegt die Erkenntnis zugrunde, daß der Krümmungsradius in dorsal-ventraler Richtung ungleich dem Krümmungsradius in medial-lateraler Richtung zu bemessen ist. Dadurch ergibt sich eine nicht-sphärische Berührungsfläche zwischen dem Zwischenelement (Meniskuselernent) und der ersten Komponente (Tibiaimplantat). Folglich ist die Berührungsfläche zwischen den Implantat teilen nicht auf Punkte oder Linien beschränkt, sondern bleibt auch bei Bewegung des Kniegelenks flächig, so daß eine verbesserte Führung, eine höhere Stabilität und eine verbesserte Fähigkeit zur Selbstzentrierung erreicht werden. Daraus resultiert eine längere Lebensdauer der Implantate, des Zwischenelements (Meniskuselements) und ein höherer Komfort für den prothetisch versorgten Patienten.
- Die Krümmungsradien können außerdem unabhängig voneinander optimiert werden. Der Krümmungsradius kann in dorsalerventraler Richtung so groß bemessen werden, daß eine Luxation sicher vermieden wird. In medialer-lateraler Richtung kann der Krümmungsradius davon unabhängig auf gute Selbstzentrierung und bequeme Kippbewegung optimiert werden.
- Vorteilhafte Weiterbildungen der Erfindung sind durch die in den Unteransprüchen angegebenen Maßnahmen möglich.
- Vorteilhafterweise stimmen dabei die lateral-medialen Krümmungsradien sowie die dorsal-ventralen Krümmungsradien des Meniskuselements und des Tibiaimplantats überein. Dadurch kann eine große Anlagefläche und eine stabile Lage des Meniskuselements bei Bewegung erzielt werden.
- Weiterhin ist: von Vorteil, daß das Tibiaimplantat mit einer medialen Erhöhung versehen ist, welche das Meniskuselement in medialer Richtung stabilisiert und Fehlstellungen vorbeugt.
- Die Erfindung wird nachfolgend anhand von Ausführungsbeispielen unter Bezugnahme auf die Zeichnung näher beschrieben. In der Zeichnung zeigen:

3A-3B eine schematische Darstellung der Krümmungsradien. - Die in den
1 und2 dargestellten Kniegelenk-Endoprothesen. 1 weisen als gemeinsame Merkmale ein Femurimplantat2 , ein die Funktion eines Meniskus übernehmendes Zwischenelement3 sowie ein Tibiaimplantat4 auf. Die in den1 und2 dargestellten Kniegelenk-Endoprothesen sind dabei in Form einer Kniegelenk-Endoprothese1 für jeweils eine der beiden Kniegelenkskondylen ausgeführt. Müssen beide Kondylen prothetisch versorgt werden, ist es in einfacher Weise möglich, die in den1 und2 dargestellten Kniegelenk-Endoprothesen1 entweder beidseitig zu implantieren oder die Kniegelenk-Endoprothese1 in geeigneter Weise einstöckig auszubilden. - Das Femurimplantat
2 ist dabei teilschalenförmig ausgebildet und weist eine Gleitfläche5 auf. Die Gleitfläche5 wirkt mit dem Meniskuselement3 zu einem Gleitlager zusammen. Eine dem Femurimplantat2 zugewandte proximale Fläche6 des Meniskuselements3 ist dabei konkav gewölbt und weist vorzugsweise den gleichen Krümmungsradius auf wie das Femurimplantat2 . - Das Femurimplantat
2 weist weiterhin geeignete Vorrichtungen zur Fixierung an dem entsprechend vorbereiteten Femurknochen auf. In den Ausführungsbeispielen sind eine Führung11 sowie ein in den Markkanal des Femur einschiebbarer Stift12 vorgesehen. - Das Tibiaimplantat
4 ist vorzugsweise flach ausgebildet und in ähnlicher Weise mit einer Führung13 ausgestaltet, welche in den entsprechend vorbereiteten Tibiakopf eingesetzt wird. - Das Tibiaimplantat
4 weist an einer medialen Seite 9 eine Erhöhung10 auf, welche eine Stabilisierung des Meniskuselements3 in medialer Richtung ermöglicht. Dadurch kann vermieden werden, daß das Meniskuselement3 in medialer Richtung verrutscht und zu Fehlstellungen führt. - Das Meniskuselement
3 ist erfindungsgemäß nicht nur an seiner dem Femurimplantat 2 zugewandten proximalen Fläche6 gewölbt, sondern auch an einer dem Tibiaimplantat4 zugewandten distalen Fläche7 . Dabei ist den1A bis1D ein konkav-konvex ausgebildetes Meniskuselement3 dargestellt, während die2A bis2D ein konkav-konkav oder bikonkav ausgestaltetes Meniskuselement3 darstellen. - In
1A und2A ist dabei jeweils eine Schrägansicht mit einem dorsal-ventral geschnittenen Meniskuselement3 und Tibiaimplantat4 dargestellt, während die1B und2B den entsprechenden Schnitt in einer Seitenansicht zeigen. In den1C und2 .C ist ein Schnitt senkrecht zu der in1A und2A dargestellten Schnittrichtung, also ein mediallateraler Schnitt, durch das Meniskuselement3 und das Tibiaimplantat4 in Schrägansicht dargestellt. Entsprechend zeigen die1D und2D eine Rückansicht des entsprechenden Schnittes. - In
1A ist das Meniskuselement3 an der dem Femurimplantat 2 zugewandten proximalen Fläche6 konkav und an der dem Tiibiaimplantat4 zugewandten distalen Fläche7 konvex gewölbt=.1B zeigt in der Seitenansicht nochmals den entsprechenden Schnitt. Dabei sind mit den Pfeilen die Krümmungsradien R1 und R2 der distalen Fläche7 des Meniskuselemens3 sowie einer Gleitfläche8 des Tibiaimplantats4 , auf welcher das Meniskuselement3 aufliegt, bezeichnet. Dabei bezeichnet der Vektor R1 den ventral-dorsalen Krümmungsradius des Meniskuselements3 , während der Vektor R2 den ventral-dorsalen Krümmungsradius des Tibiaimplantats4 bezeichnet. Der dorsal-ventrale Krümmungsradius der distalen Fläche7 des Meniskuselements3 entspricht dabei dem dorsal-ventralen Krümmungsradius der Gleitfläche8 des Tibiaimplantats4 . -
1C zeigt einen senkrecht zu dem in1A dargestellten Schnitt in medial-lateraler Richtung geführten Schnitt. In1D ist die entsprechende Schnittrichtung aus dorsaler Perspektive dargestellt. Wiederum wird durch die Pfeile der Krümmungsradius der distalen Fläche7 des Meniskuselements3 und der Krümmungsradius der Gleitfläche8 des Tibiaimplantats4 dargestellt. Dabei bezeichnet der Vektor R3 den medial-lateralen Krümmungsradius des Meniskuselements 3, während der Vektor R4 den mediallateralen Krümmungsradius des Tibiaimplantats4 bezeichnet. Auch in diesem Fall gilt, daß die Krümmungsradien der Flächen7 und8 gleich sind. - In
2A ist das Meniskuselement3 im Gegensatz zu dem in den1A bis1D beschriebenen Ausführungsbeispiel an der dem Femurimplantat2 zugewandten proximalen Fläche6 konkav und an der dem Tibiaimplantat4 zugewandten distalen Fläche7 ebenfalls konkav gewölbt.2B zeigt in der Seitenansicht nochmals den entsprechenden Schnitt. Dabei sind mit den Pfeilen die Krümmungsradien R1 und R2 der distalen Fläche7 des Meniskuselements3 sowie der Gleitfläche8 des Tibiaimplantats4 , auf welcher das Meniskuselement3 aufliegt, bezeichnet. Wie im vorigen Ausführungsbeispiel bezeichnet der Vektor R1 den ventraldorsalen Krümmungsradius des Meniskuselements3 , während der Vektor R2 den ventral-dorsalen Krümmungsradius des Tibiaimplantats4 bezeichnet. Der ventral-dorsale Krümmungsradius der distalen Fläche7 des Meniskuselements3 entspricht dabei dem dorsal-ventralen Krümmungsradius der Gleitfläche8 des Tibiaimplantats4 . -
2C zeigt einen senkrecht zu dem in2A dargestellten Schnitt in medial-lateraler Richtung geführten Schnitt. In2D ist die entsprechende Schnittrichtung aus dorsaler Perspektive dargestellt. Wiederum wird durch die Pfeile der Krümmungsradius der distalen Fläche7 des Meniskuselements3 und der Krümmungsradius der Gleitfläche8 des Tibiaimplantats4 dargestellt. Dabei bezeichnet der Vektor R3 den medial-lateralen Krümmungsradius des Meniskuselements3 , während der Vektor R4 den mediallateralen Krümmungsradius des Tibiaimplantats4 bezeichnet. Auch in diesem Fall gilt, daß die Krümmungsradien der Flächen7 und8 gleich sind. - Aus den bechriebenen Maßnahmen geht hervor, daß die Krümmungsradien R1, R2 in dorsal-ventraler Richtung ungleich den Krümmungsradien R3, R4 in medial-lateraler Richtung sind. Dadurch ergibt sich nicht eine sphärische, sondern eine teiltorusförmige Berührungsfläche zwischen dem Meniskuselement
3 und dem Tibiaimplantat4 . Folglich ist die Berührungsfläche zwischen den Implantatteilen3 und4 nicht auf Punkte oder Linien beschränkt, sondern bleibt auch bei Bewegung des Kniegelenks flächig, so daß eine verbesserte Führung, eine höhere Stabilität und eine verbesserte Fähigkeit zur Selbstzentrierung erreicht werden. Daraus resultiert eine längere Lebensdauer der Implantate2 und4 , des Meniskuselements3 und ein höherer Komfort für den prothetisch versorgten Patienten. - Die Krümmungsradien R1, R2, R3 und R4 liegen bevorzugt im Bereich zwischen 80 mm und 250 mm, besonders bevorzugt zwischen 100 mm und 200 mm. Der Krümmungsradius R3 unterscheidet sich vom Krümmungsradius R1 bevorzugt um mindestens 5 %, weiter bevorzugt um mehr als 10 %, besonders bevorzugt um mehr als 20 %.
- Die
3A und3B zeigen schematische Darstellungen zur Veranschaulichung der Krümmungsradien R1 und R3 der Fläche7 . Die Anordnungsbereiche des Femurimplantats2 , des Meniskuselements3 und des Tibiaimplantats4 sind in den3A und3B im Vergleich zu den1 und2 auf dem Kopf stehend dargestellt, so daß die Fläche7 in3A und3B oben liegt. Veranschaulicht ist, daß die Fläche7 durch Überlagerung des medial-lateralen Krümmungsradius R1 sowie des dorsal-ventralen Krümmungsradius R3 zustande kommt, wobei die Fläche7 eine Teilfläche eines virtuellen Torus20 ist. Der Torus20 hat entweder einen kreisförmigen Querschnitt oder, wie in den3A und3B dargestellt, einen elliptischen Querschnitt21 . Der Radius R3 ist dabei in den3A und3B in Richtung auf die Fläche7 gerichtet. Der kreisförmige bzw. elliptische Teiltorus20 kommt somit durch virtuelle Führung des Führungskreises bzw. - der Führungsellipse
22 entlang des Radius R1 zustande. Auf diese Weise entsteht die in den1 und2 dargestellte Fläche7 .
Claims (12)
- Gelenkprothese, insbesondere Kniegelenk-Endoprothese, zum Ausbilden einer Gelenkverbindung zwischen einem ersten Knochen und einem zweiten Knochen, mit einer ersten Komponente (
4 ), die zumindest mittelbar mit dem ersten Knochen verbunden ist, einer zweiten Komponente (2 ), die zumindest mittelbar mit dem zweiten Knochen verbunden ist, und mit einem zwischen der ersten Komponente (4 ) und der zweiten Komponente (2 ) beweglich gelagerten Zwischenelement (3 ), welches mit einer ersten Fläche (7 ) an der ersten Komponente (4 ) und mit einer zweiten Fläche (6 ) an der zweiten Komponente (2 ) anliegt, dadurch gekennzeichnet, daß die erste Fläche (7 ) des Zwischenelements (3 ) eine nicht-sphärische Fläche ist, wobei ein erster Krümmungsradius (R1) der ersten Fläche (7 ) des Zwischenelements (3 ) in medial-lateraler Richtung und ein zweiter Krümmungsradius (R3) der ersten Fläche (7 ) des Zwischenelements (3 ) in ventral-dorsaler Richtung verschieden sind. - Gelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß der erste Krümmungsradius (R1) und der zweite Krümmungsradius (R3) im Bereich zwischen 80 mm bis 250 mm, bevorzugt zwischen 100 mm und 200 mm liegen.
- Gelenkprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß sich der zweite Krümmungsradius (R3) vom ersten Krümmungsradius (R1) um mindestens 5 %, bevorzugt mehr als 10 %, besonders bevorzugt um mehr als 20 % unterscheidet.
- Gelenkprothese nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß der erste Knochen die Tibia und der zweite Knochen der Femur ist.
- Gelenkprothese nach Anspruch 4, dadurch gekennzeichnet, daß die erste Komponente (
4 ) als zumindest eine Tibiakondyle ersetzendes Tibiaimplantat (4 ) ausgebildet ist. - Gelenkprothese nach Anspruch 5, dadurch gekennzeichnet, daß die zweite Komponente (
2 ) als zumindest eine Femurkondyle ersetzendes Femurimplantat (2 ) ausgebildet ist. - Gelenkprothese nach Anspruch 6, dadurch gekennzeichnet, daß der medial-laterale Krümmungsradius (R1) der ersten Fläche (
7 ) des Zwischenelements (3 ) einem medial-lateralen Krümmungsradius (R2) einer proximalen Fläche (8 ) des Tibiaimplantats (4 ) entspricht. - Gelenkprothese nach Anspruch 6 oder 7, dadurch gekennzeichnet, daß der dorsal-ventrale Krümmungsradius (R3) der ersten Fläche (
7 ) des Zwischenelements (3 ) einem dorsal-ventralen Krümmungsradius (R4) der proximalen Fläche (8 ) des Tibiaimplantats (4) entspricht . - Gelenkprothese nach einem der Ansprüche 5 bis 8, dadurch gekennzeichnet, daß das Tibiaimplantat (
4 ) an einer medialen Seite (9) eine Erhöhung (10 ) aufweist. - Gelenkprothese nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, daß das Zwischenelement (
3 ) als Meniskuselement (3 ) aus einem Kunststoffmaterial, vorzugsweise Polyethylen, ausgestaltet ist. - Gelenkprothese nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, daß das Meniskuselement (
3 ) als konkav-konvexes Meniskuselement (3) ausgebildet ist. - Gelenkprothese nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, daß das Meniskuselement (
3 ) als bikonkaves Meniskuselement (3 ) ausgebildet ist.
