Abstract
本发明公开了一种激素透皮递送系统(THDS)。THDS有效用于生育控制和可通过加强递送孕激素和雌激素,尤其是孕激素,左炔诺孕酮而治疗的多种疾病和不适的治疗。THDS包括衬垫层,连接性的包含有效量的至少一种孕激素的粘性聚合物基质,激素的递送被以预定量存在的一种或多种皮肤渗透增强剂而增强。THDS能够从与皮肤接触的小的表面区域,即小于20平方厘米提供孕激素和雌激素的有效日剂量。本发明还公开了利用THDS的生育控制和各种激素的替代疗法。
Description
本申请要求2003年7月17日提交的美国专利申请10/621,711的优先权,后者是2002年5月23日提交的美国专利申请10/130,913的部分接续案,其是2000年11月22日提交的国际申请PCT/USOO/32043的美国国家阶段申请,其要求1999年11月24日提交的美国临时申请60/167,535的优先权,上述申请各自的内容在此引为参考。
发明领域
本发明涉及控制生育的激素治疗以及用于多种疾病和不适的治疗。具体来说,本发明提供了与加强递送类固醇激素的激素透皮递送系统有关的制剂和使用方法。
发明背景
此处引用各种科学文章和专利出版物来描述本发明相关的现有技术。这些出版物各自以其全文引为参考。
使用合成的雌激素和/或孕激素的激素治疗目前被用于控制生育和用于治疗或预防多种激素相关的不适或缺陷,包括控制痤疮,治疗子宫内膜异位,诱导或预防闭经,辅助怀孕和治疗乳溢等。传统上,合成的雌激素和合成的孕激素的组合在过去已经以口服剂量形式施用。虽然合成的孕激素和雌激素的组合可有效抑制排卵,但伴随这种口服避孕药还是存在一些不想要的副作用。例如,在使用口服避孕药的女性中,血栓和相关的血管失调包括中风和心肌梗塞的发病率较高;使用者的相对危险度可以比对照人群高11倍。此外,该危险度在年龄超过35岁的女性中急剧增加。避孕药应用也与良性肝肿瘤的发病率增加和与胆囊疾病的危险度增加有关。另外,也如果妇女在怀孕后继续服用药丸则可能导致胎儿畸形。
激素透皮递送提供了许多优点并且避免了与口服避孕药和激素治疗有关的一些缺点。具体的,透皮控速施用避免了与口服疗法有关的吸收和代谢的变动。它还提供持续的施用,允许使用具有短生物半衰期的药物活性因子。此外,在透皮方案中超过剂量或不足的可能性更少,病人对多日易用的透皮方案的接受程度超过每日口服剂量给药。
因此提供制剂和透皮系统是非常需要的,该系统允许1)使用高水平孕激素,2)使用合成的或天然的雌激素,3)对每个月经周期使用最低数量的剂量单位,并且提供合适水平的孕激素和雌激素以完全确保生育控制或其他的治疗目标而产生最少的或不产生不需要的代谢或化学降解产物。
近年来开发了各种用于女性生育控制的透皮避孕递送系统。美国专利5,296,230描述了透皮生育控制聚合物基质剂量单位,其包括衬垫层,粘附到衬垫层的含有微量分散剂量雌激素和孕激素的聚合物层和粘性层。美国专利5,560,922公开了天然雌激素,17β-雌二醇,或乙炔基雌二醇或其组合物以剂量单位内的天然孕酮或孕激素的量递送,该剂量单位包括衬垫层和含有释放激素的微贮库的相连聚丙烯酸酯粘合性聚合物层。
美国专利5,788,983公开了透皮聚合物剂量单位,衬垫层和库层,库层具有多个在使用中接触皮肤的区域并且任选含有提供不同吸收率的不同药物治疗剂。美国专利5,762,956描述了一种透皮避孕药递送装置和使用该装置的生育控制方法。该系统包含衬垫层和粘性聚合物基质,其内分散有有效控制生育的激素,以及特定相对重量比的三种皮肤渗透增强剂的组合物。
上述透皮递送系统的缺陷在于它们为一种或多种目的单独地或与选定的雌激素以合适的配重递送足量孕激素尤其是左炔诺孕酮的能力不足。例如U.S.5,296,230,U.S.5,560,922和U.S.5,788,983描述的系统包括了尺寸庞大的剂量单位,即表面积高达100cm2。U.S.5,762,956描述的剂量单位据称要小一些,但递送的孕激素不够加强。
因此,需要激素透皮递送系统和用于此的药物递送制剂,可以容易地实现难递送的孕激素例如左炔诺孕酮的高血清水平,以及所需的孕激素和选择用于避孕和其他目的的雌激素的分布谱图而使副作用最小。此外也需要剂量单位大小舒适,外观不显眼并且粘附牢固。
发明简述
本发明涉及用于增强类固醇激素透皮吸收的皮肤渗透增强剂组合物,涉及激素递送制剂,涉及包含衬垫层和其中分散有至少一种有效控制生育或用于其他激素治疗的粘性聚合物基质的激素透皮递送系统(THDS),涉及相关的组合物和方法。激素递送制剂,其可以在THDS的粘性聚合物基质中被利用,包含一种或多种此处详细说明的皮肤渗透增强剂,湿润剂/增塑剂和聚合物。THDS中各皮肤渗透增强剂的相对和绝对量通过采用具体的起始物质量和控制生产THDS的加工参数例如干燥时间和温度而被调整为使激素递送最优化。
根据本发明的一个方面,皮肤渗透增强剂组合物被提供用于构建激素透皮递送系统以递送孕激素,雌激素或睾酮激素中的一种或多种。该组合物包括药物可接受的有机溶剂(如二甲亚砜),羟基酸(如乳酸)的脂肪(C8-C20)醇酯,羟基酸(如乳酸)的低级(C1-C4)烷基酯和C6-C18饱和或不饱和的脂肪酸,例如癸酸的组合。在利用DMSO的优选实施方案中,乳酸脂肪醇酯,乳酸低级烷基酯和癸酸和四种赋形剂分别以重量比2∶1∶1∶0.8到6∶1∶1∶0.8,更具体分别为3∶1∶1∶0.8到4∶1∶1∶0.8进行组合。在具体的实施方案中,乳酸脂肪醇酯是乳酸月桂酯并且乳酸低级烷基酯是乳酸乙酯。
本发明的另一方面特征是用于构建激素透皮递送系统的聚合物制剂,该系统包括衬垫层和聚合物基质,优选粘性聚合物基质,其中分布有待透皮递送的孕激素,雌激素或睾酮中的一种或多种。该制剂包括聚合物和基于重量百分比从大约0%到大约5%的湿润剂/增塑剂和从大约10%到大约30%的皮肤渗透增强剂组合物,该组合物包括药物可接受的有机溶剂(如二甲亚砜),羟基酸(如乳酸)的脂肪(C8-C20)醇酯,羟基酸(如乳酸)的低级(C1-C4)烷基酯和C6-C18饱和或不饱和的脂肪酸,例如癸酸。在利用DMSO的优选实施方案中,乳酸脂肪醇酯,乳酸低级烷基酯和癸酸和四种赋形剂分别以重量比2∶1∶1∶0.8到6∶1∶1∶0.8,更具体分别为3∶1∶1∶0.8到4∶1∶1∶0.8组合。优选聚合物是粘性聚合物,并且在某些实施方案中粘性聚合物是聚丙烯酸酯粘性共聚物。更具体地,聚丙烯酸酯粘性共聚物包含丙烯酸2-乙基己酯单体并进一步包含大约3%到大约60%w/w的乙酸乙烯酯。在某些实施方案中,湿润剂/增塑剂是聚乙烯吡咯烷酮/乙酸乙烯酯。在某些实施方案中,乳酸脂肪醇酯是乳酸月桂酯并且乳酸低级烷基酯是乳酸乙酯。
在本发明的某些实施方案中,聚合物制剂包含孕激素,其在优选的实施方案中是左炔诺孕酮。聚合物制剂还可以包括孕激素和雌激素,具体是左炔诺孕酮和乙炔基雌二醇或17β-雌二醇。这种类型制剂的示例性的实施方案包括,基于重量百分比,大约79.65%的聚丙烯酸酯粘合性共聚物,大约1.25%的聚乙烯吡咯烷酮/乙酸乙烯酯,大约9.51%的二甲亚砜,大约3.10%的乳酸月桂酯,大约3.10%的乳酸乙酯,大约2.39%的癸酸,大约0.58%的左炔诺孕酮和大约0.28%的乙炔基雌二醇。在另外的实施方案中,聚合物制剂包括孕激素和雌激素和睾酮。在另外的实施方案中,该制剂只包括睾酮。
本发明另一方面的特征是包含基本上对皮肤渗透增强剂和待透皮递送的孕激素和雌激素不可透过的衬垫层的THDS。激素分散于固定到衬垫层的粘性聚合物基质中。粘性聚合物基质由上述的粘性聚合物制剂制成。在完成构建THDS后,粘性聚合物基质包括基于粘性聚合物基质的最终重量百分比为:从大约0%到大约5%的湿润剂/增塑剂;从大约12%到大约36%的皮肤渗透增强剂的组合,该组合是一种混合物,其包含从大约4%到大约12%的药物可接受的有机溶剂,例如二甲亚砜,从大约4.2%到大约12.6%的的羟基酸(如乳酸)的脂肪(C8-C20)醇酯,从大约0.7%到大约2.3%的羟基酸(如乳酸)的低级(C1-C4)烷基酯和从大约3%到大约9%的C6-C18脂肪酸,例如癸酸;和在大约一到大约九天内有效提供预定的日剂量的各激素的一种或多种激素。在某些实施方案中,孕激素是左炔诺孕酮,而雌激素是乙炔基雌二醇或17β-雌二醇。可选的实施方案包括THDS调整用于递送(1)只有孕激素,(2)睾酮及孕激素和雌激素,或(3)只有睾酮。
在本发明具体的实施方案中,湿润剂/增塑剂是聚乙烯吡咯烷酮/乙酸乙烯酯。该粘性共聚物包括聚丙烯酸酯共聚物,优选是一种包含丙烯酸2-乙基己酯单体进一步包含大约3到60%w/w的乙酸乙烯酯的共聚物。
除了前述置于构建后的THDS上的具体量的皮肤渗透增强剂,某些实施方案需要增强剂中以重量比存在大约4-8份的DMSO,大约4-8份的乳酸脂肪醇酯,大约1份乳酸的低级烷基酯和大约3-6份的癸酸。优选DMSO和乳酸脂肪醇酯以重量比大约1.5∶1到大约1∶1.5存在。
在具体的实施方案中,THDS被配制用于递送乙炔基雌二醇和左炔诺孕酮,其中乙炔基雌二醇以每天大约10μg和50μg的速率透皮递送大约一天到大约九天的时间段,而左炔诺孕酮以每天至少20μg的速率透皮递送,更具体以每天至少30μg,递送大约一天到大约九天。在使用中,THDS透皮递送足量的左炔诺孕酮以产生稳定状态的至少1,000pg/ml的血清浓度。
本发明的THDS能够加强递送孕激素,雌激素和睾酮,甚至从相对小的表面积。因此,本发明另一方面的特征是包含衬垫层和粘性聚合物基质的THDS,其中粘性聚合物基质最大表面尺寸为大约20cm2并且最大截面尺寸为大约300μm并且其能够以至少20μg/天,更优选至少30μg/天递送左炔诺孕酮持续大约一天到大约九天。在优选的实施方案中,粘性聚合物基质最大表面尺寸为17.5cm2或15cm2。在具体的实施方案中,THDS被配制成用于递送左炔诺孕酮并递送足以产生至少1,000pg/ml左炔诺孕酮血清浓度量的的左炔诺孕酮。
在另外的实施方案中,THDS被配制成用于递送左炔诺孕酮和雌激素,例如乙炔基雌二醇或17β-雌二醇。更具体地,雌激素是乙炔基雌二醇并且以每天10μg到50μg递送大约1到大约9天。
本发明的THDS通过组合合适量的粘性聚合物,湿润剂/增塑剂,皮肤渗透增强剂和激素,然后将混合物涂布到衬垫层上并在预定的温度下干燥该混合物一段预定的时间而制成。在另外的实施方案中,粘性聚合物制剂被涂布到一条可撕离衬里(releasable liner)上。成分的起始量和加工参数(如涂布厚度和干燥时间和温度)被调整为达到各皮肤渗透增强剂的最终重量比和重量百分数。具体地,该方法包括:(1)通过将基于粘性聚合物制剂的重量百分比的粘性共聚物溶液与下列物质组合而制备粘性聚合物制剂:从大约0%到大约5%的湿润剂/增塑剂;从大约10%到大约30%的皮肤渗透增强剂组合,该组合是一种混合物,该混合物包括药物可接受的有机溶剂,例如DMSO,羟基酸例如乳酸的脂肪(C8-C20)醇酯,羟基酸例如乳酸的低级(C1-C4)烷基酯和C6-C18脂肪酸,例如癸酸;和一定量的一种或多种在大约一天到大于九天内有效提供预定的日剂量的各激素的选定激素,从而形成粘性聚合物基质起始溶液;(2)将粘性聚合物起始溶液涂布到衬垫层;和(3)将涂布过的衬垫层在足以产生上述激素透皮递送系统的温度下干燥一段时间。在具体的实施方案中,粘性聚合物起始制剂被以厚度为大约300μm到大约800μm涂布到衬垫层上,涂布的材料在大约40℃到大约80℃的温度下干燥大约5分钟到大约25分钟。
在某些实施方案中,THDS剂量单位提供有覆盖层。该覆盖层可以固定到衬垫层或者其可以单独提供,根据使用者的判断而应用。覆盖层涂布有粘合剂并且延伸超出部分或全部衬垫层和粘性聚合物基质的边缘之外。在另外的实施方案中,非粘性聚合物代替粘性聚合物,并且通过覆盖层上的粘合剂进行皮肤粘附。
根据本发明的另一方面,提供了控制生育的方法,其包括对需要这一治疗的受试者的皮肤施用上述的THDS。THDS每周更换一次,持续一个月经周期的三到四周,作为生育控制持续超过连续月经周期是合意的。在具体的实施方案种,THDS递送左炔诺孕酮和雌激素,优选乙炔基雌二醇或17β-雌二醇。在另外的实施方案中,THDS适于仅仅递送孕激素,优选为左炔诺孕酮。
在优选的实施方案中,该方法包括透皮递送乙炔基雌二醇和左炔诺孕酮,其中乙炔基雌二醇以每天大约10μg到50μg的速率递送大约一天到到大约九天,而左炔诺孕酮以每天至少20μg的速率递送,优选以每天至少30μg递送大约一天到大约九天。在该方法的这些实施方案中,左炔诺孕酮以足以产生至少1,000pg/ml的血清浓度递送,这超过生育控制所需的血清水平。
上述的方法适于希望完全消除月经的受试者。在这种情况下,作为生育控制,THDS每周更换一次持续连续的周数,并实现消除月经。
在另外的调整中,本发明的THDS被配制用于只递送睾酮。THDS被用于治疗导致性欲下降(男性和女性)的循环睾酮水平不足,而该治疗包括每周施用一次THDS,持续该治疗所需的连续的周数。
本发明另外的特征和优点将通过如下的详细描述和实施例而被理解。
示例实施方案的详述
下面叙述的是本发明的各种实施方案,包括用于制备此处描述的THDS的粘性聚合物基质的制剂,以及用于构建THDS的加工参数使得其可以可靠地获得难以递送的孕激素,例如左炔诺孕酮的高血清水平,以及用于避孕和其他目的的所需的孕激素和选择的雌激素和/或其他激素的分布谱图。这些制剂和使用这些制剂产生的THDS与目前公开或可得到的制剂不同,并且构成激素透皮递送领域的一个重大进步。在一个方面,本发明的THDS包括分散在固定于衬垫层的粘性聚合物基质中的孕激素,最合适的是左炔诺孕酮,以及雌激素,最合适的是乙炔基雌二醇或17-β雌二醇。在另一方面,THDS只包括孕激素。在另一方面,THDS包括孕激素,雌激素和睾酮,或只有睾酮。
本发明THDS的剂量单位,有时称作“贴片”,其最简单的形式包括衬垫层,其上固定有含有待递送的激素的粘性聚合物基质和促进这些激素以合适的速率透皮递送的其它赋形剂。简单地说,该剂量单位通过将粘性聚合物,激素和赋形剂组合到起始制剂中,将该制剂涂布到衬垫层上(或者在另外的实施方案中涂布到撕离里衬上),并将涂布的衬垫层在一定的温度下干燥一段时间而制备,该温度被设计产生其中以对于递送激素是最优化的具体量存在的激素和赋形剂的剂量单位。出于此处详细描述的原因,起始制剂中各组分的量与构建过程完成后的最终THDS中存在的各组分的量不同。为加以区分,此处使用的术语“粘性聚合物制剂”,“粘性聚合物溶液”或“起始溶液”指在涂布到衬垫层并且干燥之前的起始制剂。此处使用的术语“粘性聚合物基质”是指已经涂布到衬垫层上并干燥后(即,“构建后”)的粘性聚合物溶液。
粘性聚合物制剂的组分:
皮肤渗透增强剂:从透皮递送系统释放的药物分子必须能够渗透各层皮肤。为提高药物分子的渗透率,药物透皮递送系统必须能够特别提高最外层皮肤,角质层的渗透性,其提供对分子渗透最大的阻碍。就此而言,本发明提供了利用一定量的一种或多种皮肤渗透增强剂的激素透皮递送系统。正是皮肤渗透增强剂绝对量和相对量的控制提供渗透的激素足够的流量。皮肤渗透增强剂还提供所需这些激素的渗透速率以达到从透皮避孕递送系统释放所需的量并递送到身体中以产生所需的效果。
皮肤渗透增强剂的组合优选用于本发明的实践中。该组合包括以下物质的混合物:(1)药物可接受的有机溶剂,例如二甲亚砜(DMSO),(2)羟基酸的脂肪(C8-C20)醇酯,例如乳酸月桂酯,(3)羟基酸的低级(C1-C4)烷基酯,例如乳酸乙酯和(4)C6-C18脂肪酸,例如癸酸。为使激素递送最优化,这些皮肤渗透增强剂以一定量存在于前面详细描述的粘性聚合物基质中。在某些实施方案中,可以从聚合物基质中除去一种或多种皮肤渗透增强剂。然而,剩余的增强剂的特定的量和它们的重量百分比(在起始溶液以及最终剂量单位中)的相对比率应当保持在此处所述的范围内。
在优选的实施方案中,药物可接受的有机溶剂是DMSO。其他适合用于本发明的有机溶剂包括但不限于C1-C8分支或不分支的醇,例如乙醇、丙醇、异丙醇、丁醇、异丁醇等以及氮酮(月桂氮卓酮:1-十二烷基六氢-2H-氮杂卓-2-酮)和甲磺酰甲烷等。
羟基酸脂肪醇酯优选是乳酸脂肪醇酯,例如乳酸月桂酯。然而,其他的羟基酸和脂肪醇也可以使用。另外的羟基酸包括但不限于α-羟基酸例如乙醇酸,酒石酸,柠檬酸,苹果酸和扁桃酸以及β-羟基酸,唾液酸。另外的脂肪醇包括任何C8-C20饱和的或不饱和的脂肪醇,例如肉豆蔻醇,棕榈醇和油醇等。
羟基酸低级烷基酯也优选使用乳酸,最优选是乳酸乙酯。然而,其他的羟基酸,例如乙醇酸,酒石酸,柠檬酸,苹果酸,扁桃酸和唾液酸也可以使用。此外异丙基肉豆蔻酸(IPM)可以用作羟基酸低级烷基酯的替代物。
发明人已经发现在皮肤渗透增强剂制剂中包含中到长链脂肪酸改进了本发明中采用的激素的透皮递送分布谱图。癸酸被优选使用。然而,可以使用其他的C6-C18饱和的或不饱和的脂肪酸,包括但不限于己酸,辛酸(caprytic acid),月桂酸和肉豆蔻酸等。
上述皮肤渗透增强剂的组合可以用于增强从任何类型的透皮递送装置透皮递送类固醇激素。优选使用此处详细描述的粘性聚合物基质型系统;然而,增强剂组合也可以在非粘性聚合物,以及在多层或库型透皮递送系统等中使用。
激素:利用上述皮肤渗透增强剂的THDS可以用于递送能够透皮递送的任何类型的激素。更具体的,本发明的THDS被配制成递送动物类固醇激素。在一个实施方案中,孕激素和雌激素的组合被用于以下一种或多种目的:(1)生育控制;(2)痤疮控制,(3)治疗子宫内膜异位和(4)诱导闭经。在另外的实施方案中,仅孕激素被用于以下一种或多种目的:(1)生育控制,(2)辅助怀孕,(3)作为禁忌使用雌激素(如,泌乳中的雌性)的受试者替代的激素疗法;和(4)防止乳溢。在另一个实施方案中,孕激素,雌激素和睾酮的组合被用作治疗缺乏这些激素的雌性的激素替代疗法。另一个实施方案涉及配制用于只递送睾酮的THDS,睾酮优选用于治疗由于雄性和雌性睾酮缺乏而导致的性欲下降。
在发明包含左炔诺孕酮的THDS优选用于激素组合的递送以及仅仅孕激素的递送。左炔诺孕酮已知难以透皮递送;然而,该激素可通过本发明的THDS被有效递送。随着激素以相对稳定的速率在一段延长时间内,通常为数天,并优选为一周到九天的控制释放,受试者被提供在一段长时间内稳定诸如激素的优点。
左炔诺孕酮是基于重量剂量基础上的潜在的孕激素,其是重要的因素,因为孕激素常常表现出比雌激素更低程度的透皮吸收。其他能够部分或完全使用的孕激素是炔诺孕酮,炔诺肟酯,去氧孕烯,孕二烯酮,炔诺酮,异炔诺酮,氢化孕酮,双醋炔诺醇,羟孕酮己酸酯,甲羟孕酮醋酸酯,炔诺酮醋酸酯,孕酮,醋酸甲地孕酮,助孕素和某些其他生物相同性的和可透皮吸收的孕激素。这些包括生物相容的孕激素衍生物,其可以被透皮吸收,有些可以有利的在透皮吸收后被生物转化为原始的孕激素。孕激素和选择的其他激素应该彼此具有高相容性。
对于孕激素与雌激素的组合而言,合成的激素乙炔基雌二醇特别适合。该激素可以与特别合适的孕激素,左炔诺孕酮,通过本发明的TDHS以两种激素合意的每日速率透皮递送。乙炔基雌二醇和左炔诺孕酮是相容的并且可以分散在粘性聚合物制剂中。通常,设计用于一周治疗的透皮剂量单位应该递送至少大约20μg/天的左炔诺孕酮(或相当有效量的另一种孕激素)和10-50μg/天的乙炔基雌二醇(或相当有效量的另一种雌激素)。孕激素和雌激素的这些各自的量据信是抑制排卵和维持正常的女性生理功能和特征所必须的。在本发明中,左炔诺孕酮透皮递送的量优选为每天30μg以15cm2透皮递送装置递送超过一天到大约一周。
如果吸收量符合雌激素组分日剂量的要求并且如果激素组分是相容的,也可以使用17β-雌二醇生物相容性的能被透皮吸收并优选生物转化为17β-雌二醇的衍生物。这样的雌二醇衍生物包括酯,单酯或是二酯。单酯可以是3-或17-酯。雌二醇酯可以是,比方说,雌二醇-3,17-二乙酸酯;雌二醇-3-乙酸酯;雌二醇17-乙酸酯;雌二醇-3,17-二戊酸酯;雌二醇-3-戊酸酯;雌二醇-17-戊酸酯;3-单-,17-单-和3,17-dipivilateesters;3-单-,17-单-和3,17-二丙酸酯;3-单-,17-单-和3,17-二环戊基-丙酸酯;相应的cypionate,庚酸酯,苯甲酸酯和类似的酯;thinyl雌二醇;雌酮;和其他可透皮吸收的雌激素类固醇及其衍生物。
上述物质与雌二醇本身的组合(例如,雌二醇和雌二醇-17-戊酸酯的组合或另外雌二醇-17-戊酸酯和雌二醇-3,17-二戊酸酯的组合)可以用于获得有利的结果。例如,基于雌激素组分总重量15-80%的各组分可以用于获得所需的结果。其他的组合也可以用于获得所需的吸收和治疗受试者体内17β-雌二醇的水平。
包含睾酮的制剂可以利用可以透皮吸收的天然的睾酮或合成的睾酮。例如,甲基睾酮适合用于本发明。在绝经期前的女性中,睾酮的产率为大约300μg/天。因此,用于睾酮递送的THDS应当配制成递送补充睾酮的部分或完全缺乏,即多至大约300μg/每天的量递送。相似地,为治疗男性的睾酮缺乏,THDS应当被配制为递送多至每天大约3-6mg。
应当理解激素不仅可以以纯化学化合物形式利用,还可以与其他能透皮应用的药物或与其他和上述的物质相容的组分混合使用。因此,如果合适可以使用激素的简单药物可接受的衍生物,例如醚,酯,酰胺,乙缩醛,盐等。在一些情况下,这样的衍生物可以是优选的。孕激素化合物和雌激素类固醇通常同时分散或溶解以构建含激素的粘性聚合物基质或者它们可以单独分散或溶解。
粘性聚合物:一般来说,用于形成生物可接受的粘性聚合物基质的聚合物是那些能够形成薄膜或涂层的聚合物,激素可以以受控制的速率通过它们。合适的聚合物是生物学和药物相容的,非致敏原性的,不溶于体液并与体液相容的,与该装置接触的组织相容的。由于基质的分解或腐蚀会影响激素的释放速率以及剂量单位保留在原位便于除去的能力,所以避免使用可溶性的聚合物。
用作粘性聚合物制剂的合适的材料包括聚乙烯,聚丙烯,乙烯/丙烯共聚物,乙烯/丙烯酸乙酯共聚物,乙烯/乙酸乙烯酯共聚物,硅酮弹性体,尤其是医用等级的聚二甲硅氧烷,氯丁橡胶,聚异丁烯,聚丙烯酸酯,氯化聚乙烯,聚氯乙烯,氯乙烯-乙酸乙烯酯共聚物,交联的聚甲基丙烯酸酯聚合物(水凝胶),聚偏氯乙烯,聚(对苯二甲酸乙二酯),丁基橡胶,表氯醇,乙烯乙烯醇共聚物,乙烯乙烯氧基乙醇共聚物;硅酮共聚物,例如,聚硅氧烷聚碳酸酯共聚物,聚硅氧烷聚乙烯氧化物共聚物,聚硅氧烷-聚甲基丙烯酸酯共聚物,聚硅氧烷-亚烷基共聚物(如,聚硅氧烷-乙烯硅烷共聚物),等等;纤维素聚合物,例如甲基或乙基纤维素,羟丙甲基纤维素和纤维素酯;聚碳酸酯;聚四氟乙烯;等等。
优选地,粘性聚合物应该选自玻璃化转变温度低于室温的聚合物。这些聚合物可以但不必须在室温下具有一定程度的结晶性。交联的单体单位或位点可以掺入到这样的聚合物中。例如,能掺入到聚丙烯酸酯聚合物中的交联单体包括多元醇的聚甲基丙烯酸酯,例如二丙烯酸丁烯酯和二甲基丙烯酸丁烯酯,三甲基丙烯酸三甲醇丙烷酯等。其他提供这样的位点的单体包括丙烯酸烯丙基酯,甲基丙烯酸烯丙基酯,马来酸二烯丙基酯等。
优选地,粘性聚合物制剂包含通式(I)的聚丙烯酸酯粘性聚合物:
其中x表示足以提供粘性聚合物中所需性质的重复单元的数目,R是H或低级(C1-C10)烷基,例如乙基,丁基,2-乙基己基,辛基,癸基等。更具体地,优选粘性聚合物基质包括具有丙烯酸2-乙基己酯单体和大约50-60%w/w的作为共单体的乙酸乙烯酯的聚丙烯酸酯粘性共聚物。本发明合适的聚丙烯酸酯粘性共聚物的例子包括但不限于National Starch and Chemical Co.,Bridgewater,N.J.出售的商品名为Duro Tak 87-4098的共聚物,其包括一定百分比的乙酸乙烯酯共单体。
湿润剂/增塑剂:优选湿润剂/增塑剂分散在粘性聚合物制剂中。在制剂中掺入湿润剂允许剂量单位吸收皮肤表面的水分而这可以帮助降低皮肤刺激并防止递送系统的粘性聚合物基质的失效。增塑剂/湿润剂可以是用于药物行业的传统的增塑剂,例如聚乙烯吡咯烷酮(PVP)。具体地,PVP/乙酸乙烯酯共聚物,例如那些分子量从大约50,000的适合用于本发明。PVP/乙酸乙烯酯用作增塑剂以控制聚合物基质的硬度,还用作湿润剂调节基质的水分含量。优选地,PVP/乙酸乙烯酯是International Specialty Products,Inc.(ISP)的Wayne,New Jersey提供的PVP/VA S-630,其中PVP和乙酸乙烯酯各自基本以相等重量百分比存在。
衬垫层:衬垫层可以由对激素和粘性聚合物基质的其他赋形剂无法透过的任何合适的材料制成。衬垫层用作基质层的保护性覆盖层并提供支持功能。还可以形成衬垫层使得其与含有激素的粘性聚合物基质大小基本相同或者其可以是更大的尺寸使得其可以延伸到粘性聚合物基质的外侧从而使衬垫层的延伸表面可以作为粘性覆盖层的基础,以下将详细描述。衬垫层可以是提供所需的保护和支持功能的任何合适的厚度。合适的厚度从大约10到大约300微米。更具体地,厚度小于大约150微米,更具体地,其小于大约100微米并且最具体地,厚度小于大约50微米。
适合用于制备衬垫层的材料的例子是高和低密度的以下物质的膜:聚乙烯,聚丙烯,聚氨酯,聚氯乙烯,聚酯例如聚(邻苯二甲酸乙烯酯),金属箔,这样的合适的聚合物膜的金属箔片等。聚酯膜,例如Scotchpak9732(3M公司),尤其适合用于本发明。
粘性聚合物制剂的制备:根据本发明,考虑在完成构建过程后贴片内组分的浓度而制备THDS剂量单位。因此,可以改变参数以获得包括起始制剂的组分合适的终含量和比例,以及用于构建贴片的加工参数,如下所述。
在制备含激素的粘性聚合物制剂时,优选利用上述通式的聚丙烯酸酯粘性聚合物。激素以激素剂量和在各剂量单位内所需的治疗时间所确定的量添加。
一般来说,优选起始制剂包括基于粘性聚合物起始溶液的重量大约10到大约30保粉笔的皮肤渗透增强剂组合。更优选使用基于粘性聚合物起始溶液的重量大约13到27%,更优选大约16-24%或再优选大约19-21%的皮肤渗透增强剂组合。在优选的实施方案中,皮肤渗透增强剂以DMSO,乳酸脂肪醇酯,乳酸低级烷基酯和癸酸重量比例(粘性聚合物基质的重量百分比)分别为2∶1∶1∶0.8到6∶1∶1∶0.8在粘性聚合物起始溶液中配制。更具体地,比例为3∶1∶1∶0.8到4∶1∶1∶0.8。这些重量比例是特别适合使用包括以下加工参数构建的THDS:将粘性溶液以500μm到700μm之间的厚度涂布到衬垫层并在大约60℃干燥15分钟。
从实施例1-3的数据可以看出,各种皮肤渗透增强剂在构建THDS的过程中产生不同的增加/损失。例如,在实施例1-3设定的加工参数下,构建后的粘性聚合物基质中的DMSO的重量百分比为加入到起始制剂的量的大约65到90%。相似地,乳酸乙酯降低到其初始量的大约50-75%。与之相反,乳酸月桂酯的终含量比起始百分比大大约2.6到4倍,而癸酸类似地增加到其起始百分比的大约2.5-3.9-倍。这些不同的改变可能来自与构建后基质体积的总体下降,以及各皮肤渗透增强剂的不同挥发性。无论观察到的改变的原因是什么,起始制剂中各增强剂的量应该考虑在加工过程中的不同增加或损失。本领域技术人员将能够利用此处提供的信息以及制药和药物化学领域公知的信息和对起始材料进行合适的调整。
优选含有激素的粘性聚合物基质包含超过所需递送的剂量的分散的激素。为实现此效果,起始制剂可以包含超过所需剂量大约5.0到大约50倍的激素。更优选地,超过待透皮吸收的所需剂量的大约10到大约25倍。
在配制用于递送雌激素和孕激素的THDS中,优选起始制剂包括比例为至少大约1.8∶1的孕激素和雌激素,更优选大约2∶1。不受任何具体作用基质的束缚,据信含有的较少量的雌激素就可增加血液中自由孕激素的量,因为雌激素已知诱导性激素结合球蛋白的产生,该球蛋白结合孕激素。因此,据信不仅孕激素和雌激素的比例是显著的,而且制剂中雌激素的总量优选保持在最低值,例如少于基于此处所述的起始粘性聚合物重量的大约0.3%。
根据利用的激素和所需的药物递送,合适量的增塑剂可以在基于粘性聚合物基质的重量的0到大约10%之间变化。优选地,存在于终剂量单位的湿润剂/增塑剂的量小于5%。当使用PVP/乙酸乙烯酯时,其可以作为水溶液被添加,而PVP/乙酸乙烯酯的含量基于最终贴片的干燥基质的重量在0.5%到大约5%之间变化,注意终产物中的湿润剂/增塑剂的重量百分比大于起始制剂中的百分比。
实施例1提出了本发明的示例的起始制剂。该制剂适合以下面所述的优选的构建加工参数使用。
优选在与粘性聚合物混合前,使用的激素被溶解和分散在含有增塑剂和皮肤渗透增强剂组合的溶液中。更优选,增强剂组合物和增塑剂溶液被组合,激素被加入到其中并进行混合。
通常,粘性聚合物溶液被添加到分散在增强剂组合物/增塑剂溶液中的激素溶液中。然后使用高转矩搅拌器,将粘性聚合物和增塑剂/增强剂/激素溶液的混合物充分混合以形成激素在粘性聚合物中均匀的分散系或溶液。然后混合的溶液不加搅动放置直至脱气,例如至少1小时直至24小时的时间段。
THDS剂量单位的构建:脱气后,粘性聚合物溶液被施用到衬垫层材料,并随后在设定的温度干燥设定的时间。在另外的实施方案中,粘性聚合物基质可以施用到代替衬垫层的撕离里衬。因此,此处对施用粘性聚合物基质到衬垫层的参考将理解为包括这个可选的实施方案。将脱气的粘性聚合物基质施用到衬垫层可以使用商业获得的常规用于此目的的实验室涂布/干燥设备完成。例如,可以使用WernerMathis Model LTSV/LTH设备,以及来自Werner Mathis AG(Zurich,Switzerland)的其他实验室涂布装置。其他合适的装置包括但不限于Cheminstruments,Inc.(Cincinnati,OH)生产的器械。
施用到衬垫层的粘性聚合物的厚度,以及干燥的温度和时间都是可以改变以获得贴片内的激素和渗透增强剂的终浓度和比例的加工参数。例如,在实施例3详细描述的,已经发现施用到衬垫层的粘性聚合物基质的厚度(如,从300到800μm)当其他两个加工参数干燥时间和干燥温度保持恒定时产生对皮肤渗透增强剂总体较强的保留。与之相反,改变干燥时间,例如从5到25分钟,或改变干燥温度,例如从40-100℃,产生对皮肤渗透增强剂保留的总体降低,其程度大小取决于增强剂。
因此,本领域技术人员将理解除了选择粘性聚合物起始制剂中合适量的起始材料,还可以选择(1)涂布在衬垫层上的脱气的粘性聚合物溶液初始厚度,(2)干燥时间和(3)干燥温度的合适的组合以获得在THDS中皮肤渗透增强剂和激素的最终组合物,如下文所述。脱气的粘性聚合物基质合适的初始厚度范围可以从300到800μm,更具体地从400或500到700μm,平均厚度为大约600-700μm。合适的干燥时间应当至少为5分钟,但可以扩展至10,15,20,25或更多分钟,取决与选择的其他加工参数。相似地,合适的干燥温度从室温扩展到40,50,60,70或80℃,同样取决于选择的其他加工参数。在优选的实施方案中,涂布的溶液的初始厚度为大约500-700μm,更优选600-700μm;干燥温度为大约50-80℃,更优选55-65℃,更优选大约60℃;而干燥时间为大约10-20分钟,更优选大约15分钟。本发明示例的实施例采用初始涂布厚度600-700μm,干燥温度60℃和干燥时间15分钟。
在完成构建后,干燥的粘性聚合物基质中增强剂的总量为聚合物基质的大约12-36%w/w,更具体大约15到33%,更具体大约18到30%,更具体在大约21到27%,最具体大约23-25%w/w,尤其是当使用丙烯酸酯共聚物时。
构建后,本发明的THDS还包括基于粘性聚合物基质重量的具体量的单独的皮肤渗透增强剂,出于上述的原因其与干燥前的量不同。因此,在干燥的粘性聚合物层中,DMSO包括基于聚合物基质大约4%到12%,更具体大约5%到11%,更具体为大约6-10%,更具体为大约7-9%和最具体为大约8%的粘性聚合物基质。乳酸脂肪醇酯包括基于因聚合物基质重量大约4.2%到12.6%的粘性聚合物基质,更具体为大约5.2%到11.6%,更具体为大约6.2-10.6%,更具体为大约7.2-9.6%,最具体为大约8.4%。乳酸的低级烷基酯包括基于聚合物基质的重量大约0.7%到2.3%的粘性聚合物基质,更具体大约1.0%到2.0%,更具体大约1.2-1.8%,最具体为大约1.5%。癸酸包含基于聚合物基质的重量大约3%到9%的粘性聚合物基质,更具体大约4%到8%,更具体大约5-7%,最具体大约6%。
构建后,还优选皮肤渗透增强剂以如下的重量比例存在于粘性聚合物基质中:对每份乳酸低级烷基酯而言,大约4-8份DMSO,大约4-8份乳酸脂肪醇酯和大约3-6份癸酸。换句话说,通常的比例为DMSO∶乳酸脂肪醇酯∶乳酸低级烷基酯∶癸酸为(4-8)∶(4-8)∶1∶(3-6)。在利用乳酸月桂酯作为乳酸脂肪醇酯和乳酸乙酯作为乳酸低级烷基酯的优选的实施方案中,DMSO对乳酸月桂酯的重量比在大约1∶1.5到1.5∶1之间,有利的是大约1∶1。
干燥的粘性聚合物基质与一片撕离里衬(例如Scotchpak1022或9744,3M Co.,St.Paul Minn.)(或者衬垫层,如果采用可选的实施方案)相邻分层放置,优选具有相同的大小以形成一层激素透皮递送系统。产生的层可以用钢尺模和和水压机切割以形成圆盘或方形等,具有所需的形状和大小。该圆盘或方形通常面积不超过60cm2,优选,圆盘或方形大约为5到50cm2,更优选大约8到大约40cm2。最优选,圆盘为大约10到大约20cm2。优选15cm2的圆盘,因为其相对较小而仍能够分散高浓度激素。本发明具体的实施方案特征在于贴片表面积为10,12.5,15,17.5或20cm2。然而,可以采用其他的大小。本发明THDS的优点在于其可以用薄的,透明的材料构建,例如此处示例的Scotchpakt9732衬垫层。贴片外观较小和不显眼对于使用者具有美学上的吸引力。
本发明优选的实施方案中,THDS贴片具有覆盖膜,其也可以选自多种本领域可获得的薄的,优选透明的膜。在一个实施方案中,覆盖层被设计为在各方向延伸到贴片周边之外,通常超出贴片周边的边缘为大约0.1到1.0cm,更具体为大约0.3到0.7cm,更具体为大约0.5cm。在另外的实施方案中,覆盖层被涉及为部分超出贴片的边缘,即形成延伸到贴片边缘外的覆盖层材料的“标签”。该覆盖层可以用THDS的其他元件构建,即其可以在构建THDS时固定到衬垫层。或者,覆盖层可以分别构建,如,在单独的小袋中,用其自己的可撕离里衬,使得覆盖层可以根据使用者的判断来施用。覆盖层系统通常用于贴片和其他皮肤装置,并可以根据任何标准方法制备。
尽管没有覆盖层的贴片也适合用于本发明,包含覆盖层的贴片提供了THDS在其使用期间(通常为一周)更可靠粘附的优点。此外,在没有覆盖层的THDS中,贴片的周边可以与延伸到贴片边缘的粘性聚合物基质胶粘。胶粘的周边会积累来自外界环境的灰尘,衣物棉绒等,在贴片的边缘产生美学上难看的“暗圈”。在覆盖层形式的贴片中,THDS的周边由于覆盖层而隔绝外界环境而覆盖层的周边暴露到外界环境的是窄得多的边缘(如,在优选的实施方案中覆盖层和粘合剂组合的厚度大约75-100μm)。这使得外界物质在贴片上的积累最小化,而基本避免了“暗圈”,构成覆盖层系统的另一个美学优点。
然后将得到的用或不用覆盖层制备的THDS剂量单位放置在合适的包装中储存,例如纸和/或箔袋,直至它们被用于透皮治疗。
由于THDS贴片中皮肤渗透增强剂的最终绝对和相对量的重要性,如果一个或者多个前面的制剂或加工参数被改变了,则构建过程中重要的最终步骤是在过程已经完成后对这些组分进行定量。这可以根据本领域已知的标准分析技术完成,例如溶剂提取组分,然后进行液体或其他色谱。合适的方法的例子显示于实施例2。
使用方法:本发明的THDS采用向受试者皮肤施用与需要治疗的具体不适相称的预定量贴片预定的时间的通常方法。该受试者通常是哺乳动物并且更通常为人,尽管本发明可以在动物上实践用于各种兽医学的目的。通常,尽管不一定出现的情况下,贴片被配制成递送有效量的一种或多种激素持续1到7-9的时间段以达到所需的效果。对于持续递送激素而言,在此处的激素损耗低于有效量之前用新鲜的贴片更换该贴片。在通常的治疗过程进行中,贴片每周更换一次。
妇女的生物控制是本发明的THDS的优选的应用。对于采用孕激素和雌激素的避孕系统而言,本发明的THDS提供了速率增加的激素释放,从而提供高速率的激素递送。能够通过本发明的THDS递送的左炔诺孕酮的水平超过避孕所需的大约1,000pg/ml的水平。实际上,利用本发明的系统达到超过1500-2000pg/ml的水平(参见实施例4)。此外,乙炔基雌二醇的血清水平为大约20-80pg/ml,从而提供对子宫内膜的有效支持。激素合适的血清水平可以通过使用上述相对小的贴片而获得,优选面积为10到20cm2,尤其是10,11,12,13,14,15,16,17,18,19或20(或中间值的增量,例如12.5或17.5)cm2,其增加了对使用者的方便程度。
在某些条件下(如,其他健康条件要求避免使用雌激素的情况),可以只使用孕激素控制生育。通常优选使用左炔诺孕酮,但是足够量的透皮左炔诺孕酮的透皮递送是之前存在的问题。本发明的THDS在这方面尤其具有优点,产生可以通过其使用达到高血清水平的左炔诺孕酮。对于不同雌激素的生育控制而言,血清水平持续300pg/ml被发现在皮下左炔诺孕酮植入物中是有效的。该血清水平使用本发明的THDS可以很容易地达到。
透皮施用激素用于控制生育的方案是公知的。贴片通常被配制为在1到7-9天内递送控制生育有效量的激素。在包括28天周期的通常的方案中,贴片每周更换一次持续三周。在第四周,没有贴片或使用安慰剂贴片。
在备选的方案中,贴片可以在第四周佩戴,从而提供持续的激素透皮供应。该方案可以被希望完全避免月经的妇女采用。月经可以不确定地持续或在数月后终止(例如3,4或6个月)。
本发明的THDS可以用于生育控制之外的其他症状。例如,生育控制量的雌激素/孕激素组合也可以用于控制痤疮,控制子宫内膜异位症和诱导闭经。只用生育控制量的孕激素(如左炔诺孕酮)配制的THDSs也可以用于辅助怀孕和用于雌激素禁忌使用的受试者(例如,具有乳腺癌危险度的女性),以及用于抑制乳溢,这是在男性或非哺乳女性中的乳汁分泌。
本发明的THDS提供与目前使用的透皮激素和避孕系统相多优点。如上文详细讨论的,主要的优点包括来自较小表面积高量的生育控制激素,尤其是左炔诺孕酮的递送,该贴片可以和透明的衬垫和覆盖层材料制成,从而提高了它们的美观性。征增加了本发明的THDS用于此处描述的多种目的的实用性和吸此外,通常与这样的治疗有关的胸部的压痛和充血已经被发现本发明的THDS与使用其他系统相比的病人中显著降低,上述现为恶心和/或呕吐。

方法:
激素溶解并分散在含有PVP/乙酸乙烯酯和皮肤渗透增强剂的组合的溶液中。添加Duro-Tak 87-4098(固体含量33%)粘性聚合物溶液,并密封容器。使用磁性搅拌棒以大约200rpm在室温下搅拌溶液3小时,以形成均匀的溶液。通过将搅拌的溶液不搅拌放置一个小时或直至所有的空气气泡消失而使搅拌的溶液脱气。脱气的溶液使用实验室涂布/干燥器(型号LTSV/LTH,Werner Mathis,Switzerland)被涂布到一张厚度为700μm的衬垫层上,然后在60℃干燥15分钟。干燥的粘性聚合物基质用与衬垫层相同大小的撕离里衬层压,以形成一片THDS。使用钢尺模和4000psi的水压将这些片切割为15cm2的剂量单位。各剂量单位用纸或箔袋单独包装并在4℃储存。
实施例2
干燥的粘性聚合物基质的定量分析
对实施例1构建的THDS剂量单位进行定量分析以测定各组分在构建后的含量。如下测定左炔诺孕酮(LNG)和乙炔基雌二醇(EE)的量。从箔袋中取出剂量单位并除去撕离里衬。剂量单位在粘性侧折叠在一起并放置在提取容器中,向其中加入100ml甲醇。用手剧烈摇动容器,然后放置在轨道式摇床上12小时,直至剂量单位上的所有粘合剂被溶解。21ml提取的溶液被放置在离心管内,并添加9ml HPLC-级的水,以产生包括70%甲醇的甲醇水溶液。离心该样品并将上清经0.45μm(尼龙或Teflon)滤纸过滤到HPLC小瓶中。将滤出物注射到具有UV监测器的高压液相色谱。
如下测定其他的赋形剂的量。从箔袋中取出剂量单位并除去撕离里衬。剂量单位在粘性侧折叠在一起并放置在提取容器中,向其中加入2ml四氢呋喃(THF)以溶解粘合剂。四个小时后,向THF溶液(THF体积比5%)加入38ml含有0.01%(v/v)乙酸己酯(作为内参照)的己烷。振荡容器直至全部的计量单位溶解。离心样品并转移到HPLC小瓶中。滤出物被注射到具有FID检测器的气相色谱。结果显示如下。

实施例3
加工参数对粘性聚合物基质中皮肤渗透增强剂最终组成的影响
检测THDS构建参数对粘性聚合物基质中皮肤渗透增强剂的最终组成的影响。变化了四个加工参数:干燥温度,干燥时间,起始涂布厚度和起始制剂中总增强剂的重量百分比。结果显示于下表。各个值代表四个贴片的平均值。值以mg/g粘性聚合物基质的形式给出。

实施例4
临床研究
进行开放标记,剂量反应,4周期,多个中心的研究以测定用根据实施例1的方案制备的THDS治疗后的LNG,EE,孕酮,LH和雌二醇的水平。临床研究的目的是:(1)测定在健康排卵的妇女中7天LNG/EE THD的安全性;(2)测定用LNG/EE THDS治疗后LNG和EE的血清水平;(3)测定用LNG/EE THDS治疗后作为排卵指示的孕酮的血清水平;和(4)测定用LNG/EE THDS治疗后LH和雌二醇的水平。
在该研究中LNG和EE的血清水平每周测三次。在该研究中,LH,雌二醇和孕酮的血清水平也每周测三次。孕酮的水平等于或大于3ng/ml被假定为发生排卵。通过由日记卡确定点滴出血,突破性出血,消退性出血和闭经的发生而评估周期控制。点滴出血定义为需要最多两片卫生护垫的少量流血。突破性出血定义为需要使用两个或三个或更多卫生护垫的较大量流血。点滴出血和/或突破性出血定义为除了在第21天之前开始并持续到经过该日期的出血之外在第5-21天之间发生的出血。闭经定义为在整个28天周期中没有任何出血。消退性出血定义为在第22天或之后(在没有施用THDS的7天范围内)任何超过一天的点滴出血或出血。
通过无提示病人描述的副作用而评估安全性。还测定身体检查,病毒检测,生命体征和实验室功能的改变。
通过RIA分析LNG血清样品以及通过气相色谱/质谱分析EE血清样品以周期性测定在研究中受试者这些激素的血清水平。分别为15,17.5和20cm2的贴片的公式的初步结果显示于下表。

Claims (14)
1.激素透皮递送系统,包括对皮肤渗透增强剂和待透皮递送的类固醇激素不可透过的衬垫层和固定于衬垫层的粘性聚合物基质,其中粘性聚合物基质包括粘性聚合物,还包括基于粘性聚合物基质终重量百分比的以下物质:
a)从0%到5%的湿润剂/增塑剂;
b)从12%到36%的皮肤渗透增强剂的组合,该组合是包括从4%到12%的药物可接受的有机溶剂,从4.2%到12.6%的羟基酸脂肪(C8-C20)醇酯,从0.7%到2.3%的羟基酸低级(C1-C4)烷基酯和从3%到9%的C6-C18脂肪酸的混合物,其中有机溶剂为二甲亚砜,羟基酸脂肪(C8-C20)醇酯为乳酸脂肪醇酯,羟基酸低级(C1-C4)烷基酯为乳酸低级烷基酯,C6-C18脂肪酸为癸酸,以及其中二甲亚砜和乳酸脂肪醇酯以重量比为1.5∶1到1∶1.5存在;和
c)一定量的待透皮递送的类固醇激素,其在一天到九天内有效提供预定日剂量的激素,其中类固醇激素为左炔诺孕酮和乙炔基雌二醇,且左炔诺孕酮和乙炔基雌二醇的重量比至少1.8∶1。
2.权利要求1的激素透皮递送系统,其中左炔诺孕酮和乙炔基雌二醇的重量比至少2∶1。
3.权利要求1或2的激素透皮递送系统,其中乙炔基雌二醇少于起始粘性聚合物制剂重量的0.3%。
4.权利要求1的激素透皮递送系统,其中湿润剂/增塑剂是聚乙烯吡咯烷酮。
5.权利要求1的激素透皮递送系统,其中湿润剂/增塑剂是聚乙烯吡咯烷酮/乙酸乙烯酯。
6.权利要求1的激素透皮递送系统,其中粘性聚合物包括聚丙烯酸酯共聚物。
7.权利要求6的激素透皮递送系统,其中聚丙烯酸酯共聚物包括丙烯酸2-乙基己酯单体。
8.权利要求7的激素透皮递送系统,其中聚丙烯酸酯共聚物还包括3到60%w/w的乙酸乙烯酯。
9.权利要求1的激素透皮递送系统,其中乳酸脂肪醇酯是乳酸月桂酯,而乳酸低级烷基酯是乳酸乙酯。
10.权利要求1的激素透皮递送系统,其中皮肤渗透增强剂以重量比为4-8份二甲亚砜,4-8份乳酸脂肪醇酯,1份乳酸低级烷基酯和3-6份癸酸存在。
11.权利要求1的激素透皮递送系统,配制用于递送乙炔基雌二醇和左炔诺孕酮,其中乙炔基雌二醇以每天10μg到50μg的速率透皮递送一天到九天,并且左炔诺孕酮以每天至少20g的速率透皮递送一天到九天。
12.权利要求11的激素透皮递送系统,其中左炔诺孕酮以足以产生稳定状态血清浓度至少1,000pg/ml的量透皮递送。
13.权利要求1的激素透皮递送系统,其还包括覆盖层,其中覆盖层涂布有粘合剂并延伸超过衬垫层和粘性聚合物基质的部分或全部周边。
14.权利要求13的激素透皮递送系统,其中非粘性聚合物取代粘性聚合物,而其中通过涂布粘合剂的覆盖层实现皮肤粘附。
