骨髓移植后嵌合现象的检测
非遗传方法
材料与方法
受试者与样品
四个从女性供体接受骨髓的男性骨髓移植受体和17个正常的健康受试者进入本研究。如(Lo et al.,Am J Hum Genet
62:768-75(1998))所述的收取所有的EDTA-血液样品的血沉棕黄层血沉棕黄层(BC)并在-20℃储存。
DNA的分离
按照生产商的推荐使用Nucleon DNA提取试剂盒(Scotlabs)从BC中提取DNA。
亚硫酸氢盐转化
按照生产商的指示使用CpGenome DNA修饰试剂盒(Intergen)进行DNA样品的亚硫酸氢盐修饰。通过亚硫酸氢盐转化,非甲基化的胞嘧啶残基被转化为尿嘧啶,而甲基化的胞嘧啶残基保持不变(Herman et al.,Proc Natl Acad Sci U S A
93:9821-6(1996))。在亚硫酸氢盐转化之后,再使用不同的PCR引物将甲基化和非甲基化DNA之间的序列差异区分开来。在亚硫酸氢盐转化反应中使用1μg的BC DNA。
甲基化特异性PCR(MSP)
按Herman et al.,supra所描述的方法对MSP分析进行了改良。引物M-for (5’-GCGAGCGTAGTATTTTTCGGC-3’)和M-rev(5’-AACCAAATAACCTATAAAACCTCTACG-3’)被设计用于甲基化的序列,而引物U-for(5’-GTTGTGAGTGTAGTATTTTTTGGT-3’)和U-rev(5’-CAAATAACCTATAAAACCTCTACA-3’)被设计用于非甲基化的序列。5μl的亚硫酸氢盐处理的DNA加入到包含5μl的10X TaqMan缓冲液A(PE Applied Biosystems)、2mM MgCl2、10pmol dNTPs、20pmol各个相应的MSP引物、1.25U AmpliTaq Gold DNA聚合酶(PE AppliedBiosystems)的50μlPCR反应物中。将反应混合物进行45次热循环(甲基化等位基因:95℃45秒、58℃30秒、72℃20秒;非甲基化等位基因:95℃45秒、50℃30秒、72℃20秒),其中在95℃进行8分钟的初始变性步骤。然后通过琼脂糖凝胶电泳对PCR产物进行分析。
结果
本试验提供MSP分析方法以检测雄激素受体基因的甲基化和非甲基化的DNA序列。总共有6名男性和11名女性健康受试者进入本研究。在所有的男性对照受试者中,正如所预料的,在这些样品中只检测到了非甲基化的雄激素受体基因(图1A)。与此相反,在女性对照受试者中,观察到了非甲基化的和甲基化的雄激素受体基因DNA序列(图1A)。在这些女性受试者中甲基化的和非甲基化的雄激素受体基因的检测率分别为100%和82%。而当从MSP分析中省略DNA样品时,未观察到阳性信号(图1A)。有趣的是,在所有性别失配的男性骨髓移植受体中(100%),都观察到了甲基化和非甲基化的DNA序列的阳性信号,表明来自于女性供体的细胞存在于男性受体的血液循环中。

实施例2
胎儿和母亲之间的DNA甲基化差异用作检测母体血浆中胎儿DNA
的方法
本试验表明,通过使用人IGF2-H19位点中的甲基化差异区域作为母体血浆中的非遗传标志物,对胎儿从母亲遗传的等位基因的检测是可能的。这些结果极大地扩展了对在母体血浆中的胎儿DNA的产前诊断的可能性,从而允许发展在母体血浆中不依赖于性别和不依赖于多态性的胎儿特异性标志物,及发展产前诊断印迹疾病和某些染色体异倍体性的新方法。
材料与方法
受试者与样品
取得同意后从怀孕妇女处收集样品,总共有21名和18名分别处于第二个三月期(17-21周)和第三个三月期(37-42周)的怀孕妇女进入本研究。进入研究的受试者在当前怀孕期中无一例具有先兆子痫或早产阵痛。EDTA母体血液和胎儿羊水样品按如前所述的(Lo et al.,Am J Hum Genet62:768-775(1998))从第二个三月期的受试者中收集。对于第三个三月期的受试者,在正常阴道分娩前2-3小时收集EDTA母体血液样品。如所述的(Lo et al.,Clin Chem

DNA分离
使用QIAamp Blood试剂盒(Qiagen)从血浆和羊水样品中提取DNA,通常800μl的血浆或羊水被用于每柱的DNA提取。使用50-100μl体积的洗脱液。按照生产商的推荐使用Nucleon DNA提取试剂盒(Scotlabs)从血沉棕黄层血沉棕黄层中提取DNA。
DMR多态性区域的基因型
人IGF2-H19位点中的DMR包含两个450碱基对的重复单元和七个400碱基对的重复单元(Nakagawa et al.,Proc Natl Acad Sci USA98:591-596(2001))(图2)。在我们的研究中选择DMR内的A/G SNP(Nakagawa et al.,supra)为标志物(图2)。聚合酶链反应(PCR)被用于扩增在母体和胎儿DNA样品中的SNP。使用人类H19基因的序列(基因库登录号AF125183)设计引物。通常2-5μl经洗脱并从母体血沉棕黄层血沉棕黄层、脐带血沉棕黄层血沉棕黄层或羊水中纯化的DNA被加入到包含2.5μl的10X TaqMan缓冲液A(PE Applied Biosystems)、3mM MgCl2、6.26pmol dNTPs、5pmol的引物(正向:5’-ggACGGAATTGGTTGTAGTT-3’;反向:5’-AGGCAATTGTCAGTTCAGTAA-3’)、0.625U AmpliTaq GoldDNA聚合酶(PE Applied Biosystems)的25μl PCR反应物中(95℃8分钟,随后35次循环,95℃1分钟、56℃20秒、72℃20秒)。对于正向引物,以大写字母表示的核苷酸相应于H19序列(基因库登录号AF125183)的7927-7944位置。对于反向引物,核苷酸与H19序列的8309-8329位置互补。然后通过琼脂糖凝胶电泳和DNA测序对PCR产物进行分析。

按照生产商的指示使用CpGenome DNA修饰试剂盒(Intergen)进行DNA样品的亚硫酸氢盐修饰。通过亚硫酸氢盐转化,非甲基化的胞嘧啶残基被转化为尿嘧啶,而甲基化的胞嘧啶残基将保持不变(Herman et al.,Proc Natl Acad Sci U S A
93:9821-9826(1996))。在亚硫酸氢盐转化之后,再使用不同的PCR引物将甲基化和非甲基化DNA之间的序列差异区分开来。通常在亚硫酸氢盐转化反应中使用1μg来自于母体或脐带血的血沉棕黄层DNA,或93μl从母体血浆或羊水纯化的经洗脱的DNA。然后经亚硫酸氢盐处理的DNA在25-50μl 1μTris-EDTA中洗脱。
甲基化特异性PCR(MSP)
按Herman et al.,supra所描述的方法对MSP分析进行了改良。5μl的亚硫酸氢盐处理的DNA加入到包含5μl的10X TaqMan缓冲液A(PEApplied Biosystems)、2.5mM MgCl2、10pmol dNTPs、20pmol各个相应的MSP引物(图2)、1.25U AmpliTaq Gold DNA聚合酶(PE Applied Biosystems)的50μl PCR反应物中。引物M-for和M-rev(图2)被设计用于甲基化的序列,而引物U-for和U-rev(图2)被设计用于非甲基化的序列。将反应混合物进行50次(血沉棕黄层和羊水DNA)或56次(血浆DNA)热循环(甲基化等位基因:95℃45秒、55℃20秒、72℃20秒;非甲基化等位基因:95℃45秒、49℃20秒、72℃20秒),其中在95℃进行8分钟的初始变性步骤。然后通过琼脂糖凝胶电泳对PCR产物进行分析。使用MicrospinS-300 HR柱(Amersham Pharmacia)纯化反应产物以进行DNA测序或引物延伸分析。
DNA测序
使用ABI Prism dRhodamine Terminator Cycle Sequencing ReadyReaction试剂盒(PE Applied Biosystems)和PCR产物的相应正向引物对纯化的PCR产物进行测序。用ABI Prism 310基因分析仪(PE AppliedBiosystems)对测序产物进行分析。
引物延伸分析
2μl纯化的MSP产物被加入到包含50μM ddATP(2’,3’-二脱氧三磷酸腺苷)、50μM dGTP、50μM dTTP、0.2pmol Cys-5标记的引物(5’-GGGTTATTTGGGAATAGGATATTTA-3’)、4U Thermo Sequenase(Amersham Pharmacia)、1.43μl浓缩缓冲液的25μl反应物中。将反应物进行40次热循环(95℃30秒、51℃20秒、72℃20秒)。Cys-5标记的引物长度为25个核苷酸(nt),且多态性位点与引物的3’-端相距2nt。对于A等位基因,在该多态性位点上引入ddATP将产生链终止,因此得到27nt的延伸产物(即25+2nt)。对于G等位基因,链延伸将继续直到与引物的3’-端相距5nt的下一个A残基为止,因此得到30nt的延伸产物(即25+5nt)。使用14%的变性聚丙烯酰胺凝胶对反应产物进行电泳,并使用ALFExpress Sequencer(Amersham Pharmacia)进行分析。数据用AlleleLinks程序(Amersham Pharmacia)进行分析。
结果
DMR的基因型
39名怀孕妇女进入本研究,通过直接测序从血沉棕黄层DNA获得的PCR产物来确定在DMR内的SNP的母体基因型(图2)。具有每个可能的基因型的怀孕妇女的数目为17(GG,43.6%)、16(AG,41.0%)和6(AA,15.4%)。
二等位基因多态性杂合的妇女血浆中胎儿DNA的检测
选择16名SNP杂合(即AG)妇女以进行进一步检验。由于这是二等位基因多态性,按照以前的标准这些妇女将不会被认为可对母体血浆中胎儿DNA的检测在该多态性位点处能够提供信息(Lo et al.,Ann NY AcadSci
731:204-213(1994);Bianchi Am J Hum Genet
62:763-764(1998))。为证实在此基因组区域的甲基化差异将允许我们克服这一限制,用亚硫酸氢盐处理母体DNA并使用如图2中所示的引物通过MSP进行扩增。与此相似,从羊水(第二个三月期样品)或脐带血(第三个三月期样品)的血沉棕黄层中分离的胎儿DNA经过PCR和MSP以确定胎儿等位基因的印迹状态。
在16个选择的受试者中,从四个第三个三月期和七个第二个三月期胎儿样品得到的甲基化的(即从父亲遗传的)等位基因与各自的母亲的甲基化的等位基因不同(图3a、图3b,比较1号板和2号板)。为检验是否在胎儿和母亲之间的甲基化差异将允许从母体血浆中检测出胎儿等位基因,来自于这些受试者的母体血浆DNA经过亚硫酸氢盐转化,再进行MSP。有趣的是,从父亲遗传的甲基化的胎儿等位基因可以在两个第三个三月期和四个第二个三月期母体血浆样品中检测到(图3a、图3b,3号板)。为排除这些观察值仅是由于母体血浆中异常甲基化的母体DNA存在而引起的可能性,我们从这些阳性受试者中的一个受试者中收集了产后母体血浆样品(约分挽后3.5年)。在此产后样品中我们没有观察到其它的甲基化等位基因(图3a,4号板),表明在怀孕期间母体样品中其它的甲基化等位基因是来自于胎儿。另外,在没有提供信息的受试者(n=4,数据未显示)的血浆中没有观察到阳性信号,因此进一步表明该MSP分析的特异性。总而言之,这些数据表明,即使在按照现有标准不被认为能提供信息的情况下,利用母亲和胎儿之间的甲基化差异也允许检测母体血浆中的胎儿DNA。
从母体血浆中检测来源于胎儿的从母亲遗传的DNA
然后我们检验是否使用母亲和胎儿之间的甲基化差异可以允许我们检测胎儿从母亲遗传的等位基因。这种类型的分析以前被认为是不可能的(Lo et al..Ann NY Acad Sci
731:204-213(1994);Bianchi,Am J HumGenet
62:763-764(1998))。由于从母亲遗传的等位基因是非甲基化的,所以引物U-for和U-rev(图2)在亚硫酸氢盐转化后被用于扩增非甲基化的等位基因。在16个被分析的受试者中,三个第三个三月期和五个第二个三月期母体样品可以提供信息。在这些受试者中,胎儿拥有不同于母亲的非甲基化等位基因的非甲基化等位基因。这些结果意味着,在这些受试者中,母亲具有最初从她的父亲遗传的胎儿等位基因,然后传递到她的胎儿。在这8个提供信息的受试者中,通过直接测序只在一个第三个三月期样品中观察到微弱的阳性信号(图4a,比较1号板和2号板)。
我们推测,在该单独的阳性受试者中的微弱信号和来自于母体血浆中的非甲基化胎儿等位基因的低检测率可能是由于直接测序方法低灵敏度的原因。为增强检测的灵敏度,我们采用了更为灵敏的引物延伸分析方法以检测来自于MSP反应产物的未甲基化胎儿等位基因。由于SNP是A/G多态性,所以ddATP在引物延伸分析被用作反应底物。从A和G等位基因延伸的反应产物分别为27和30nt长。在相应的母体血沉棕黄层样品中没有出现胎儿特异性的反应产物(图4b、图4c;母体BC)。让人注意的是,在二个第三个三月期(图4b,箭头)和一个第二个三月期(图4c,箭头)的母体血浆样品中观察到了胎儿特异性的延伸产物,表明非甲基化的胎儿DNA出现在母体血浆中。至于对照组,在此分析中检验的不提供信息的受试者中没有一个是阳性的(n=5,数据未显示)。这些结果第一次表明有可能使用非遗传标志物从母体血浆中检测来自于胎儿的从母亲遗传的DNA序列。
讨论
这些结果表明使用非遗传标志物克服了常规检测在母体血浆中的胎儿DNA的局限性。通过使用母亲和胎儿之间的非遗传差异,从母亲的血浆中检测在遗传上与母体等位基因不可区分开的从父亲遗传的等位基因是可能的。同样地,从母体血浆中检测从母亲遗传的胎儿等位基因是可能的。因此这种新的非遗传方法将扩展可以使用在母体血浆中的胎儿DNA的疾病种类。
即使使用了相对不灵敏的方法如直接测序和引物延伸,本结果也表明从母体血浆中检测甲基化有差异的胎儿DNA序列是可能的。与类似的甲基化等位基因分析相比(图3),在母体血浆中检测非甲基化胎儿DNA的灵敏度较低(图4)。使用更为灵敏的检测系统,如等位基因特异性的PCR(Newton et al,Nucleic Acids Res
17:2503-2516(1989))和实时甲基化特异性PCR(Lo et al.,Cancer Res
59:3899-3903(1999);Eads et al.,NucleicAcids Res
28:E32(2000))可以增强血浆基非遗传分析的灵敏度。实时甲基化特异性PCR的发展特别使人感兴趣,因为它开启了在母体血浆中定量胎儿特异性甲基化的可能性,正如早已实现的在血液循环中对肿瘤DNA的检测那样(Kawakami et al,J Natl Cancer Inst
92:1805-1811(2000))。
在母体血浆中胎儿DNA的可能引入作为常规的产前诊断工具已经引起一些关于在循环的胎儿DNA中需要非遗传标志物的问题(Lo et al.,AmJ Hum Genet
62:768-775(1998);Avent et al,Vox Sang
78:155-162(2000))。因此对于这种标志物大多数的方案更集中于母亲和胎儿之间的基因多态性的使用上(Tang et al.,Clin Chem
45:2033-2035(1999);Pertl et al.,HumGenet
106:45-49(2000))。当前对于非遗传标志物用于母体血浆中胎儿DNA检测可能性的证实开启了发展母体血浆中不依赖于性别和不依赖于多态性的胎儿标志物的新方法。可以达到这一目的的一种途径是利用组织特异性甲基化现象(Grunau et al.,Hum Mol Genet
9:2651-2663(2000))。由于滋养层被认为是用于将胎儿DNA释放进入母体血浆中的占优势细胞群体,对滋养层特异性的甲基化模式的说明允许发展母体血浆中一般的非遗传胎儿标志物。在生物学上,组织特异性甲基化标志物的应用也可以允许直接解决关于胎儿细胞类型应负责向母体血浆中释放胎儿DNA类的问题。
母体血浆的非遗传分析在与基因组印迹有关的疾病,如Prader-Willi综合症(Pfeifer,Lancet
356:1819-1820(2000))中具有显而易见的应用,这种方法也具有诊断疾病如先兆子痫的可能性,其中印迹基因被推测发挥了作用(Graves,Reprod Fertil Dev
10:23-29(1998))。
母体血浆中胎儿DNA在产前检测胎儿染色体异倍体性中的可能应用是由于对这一现象的发现而热烈讨论的结果(Lo et al.,Lancet
350:485-487(1997);Bianchi,Am J Hum Genet
62:763-764(1998))。与整倍体性怀孕相比(Lo et al.,Clin Chem
45:1747-1751(1999);Zhong et al.,Prenat Diagn20:795-798(2000)),在异倍体性中循环的胎儿DNA水平之间的定量差异可提供一种从母体血浆预计胎儿染色体异倍体性的可能性的方法。最近在母体血浆中发现凋亡胎儿细胞(“来源于血浆的细胞”)(Van Wijk et al.,Clin Chem
46:729-731(2000))还提供了另外一种从母体血浆检测异倍体性的方法(Poon et al.,Lancet
356:1819-1820(2000))。有趣的是,当前的数据还开启了另外一种检测胎儿异倍体性的可能方法。这是基于对异常DNA甲基化模式可能与染色体异倍体性有关的观察(Kuromitsu et al.,MolCell Biol
17:707-712(1997);Yu et al.,Proc Natl Acad Sci U S A94:6862-6867(1997))。因此,发展用于从母体血浆中检测这种甲基化异常的胎儿DNA序列的非遗传标志物是可能的。这种标志物与母体血浆中胎儿DNA的简单定量相比提供了特异性(Lo et al.,Clin Chem
45:1747-1751(1999);Zhong et al.,Prenat Diagn
20:795-798(2000))和与基于“来源于血浆的细胞”相比对于大规模应用提供了更好的适应性(Poon et al.,Lancet356:1819-1820(2000))。
胎儿非遗传标志物也可以用在从母体血液的细胞成分中分离的胎儿细胞的分析中。这里利用了最近的数据,表明甲基化分析可以以原位方式进行(Nuovo et al.,Proc Natl Acad Sci USA
96:12754-12759(1999))。
随着我们最近认识到胎儿母体运输是一个双向的过程(Lo et al.,Blood
88:4390-4395(1996);Maloney et al.,J Clin Invest
104:41-47(1999)),非遗传标志物也可以被用于研究细胞和DNA从母亲到胎儿的转移。这种方法也可以在其它类型的嵌合现象研究中应用,如移植后的血液嵌合现象(Starzl et al.,Curr Opin Nephrol Hypertens
6:292-298(1997))和尿液DNA嵌合现象(Zhang et al.,Clin Chem
45:1741-1746(1999))。
随着我们对人类基因组的理解的增加及高通量阵列基的甲基化分析技术的发展(Yan et al.,Clin Cancer Res
6:1432-1438(2000)),我们期望在未来几年中可以使用的胎儿非遗传标志物的数目迅速增加。这样的发展将为胎儿非遗传标志物提供与临床相关的平台,其能够与母体血浆中的常规遗传标志物以相互协同的方式使用。

Claims (38)
1.一种区分生物样品中来源于不同个体的DNA种类的方法,包括检测这些DNA种类间非遗传差异的步骤。
2.如权利要求1所述的方法,其中所述的非遗传差异是DNA甲基化差异。
3.如权利要求1所述的方法,其中所述的生物样品是血浆或血清。
4.如权利要求1所述的方法,其中所述的生物样品是血液。
5.如权利要求1所述的方法,其中所述的个体之一是怀孕的女性,而其它的个体是未出生的胎儿。
6.如权利要求1所述的方法,其中所述的个体之一是移植受体,而其它的个体是器官供体。
7.如权利要求6所述的方法,其中所述的移植是骨髓移植。
8.如权利要求1所述的方法,还包括测定具有非遗传差异的DNA种类浓度的步骤。
9.如权利要求2所述的方法,其中所述的非遗传差异是DNA甲基化差异。
