Abstract
免疫系统无应答的人的T细胞应答常常减弱。我们开发了一种分离、刺激和扩增针对抗原特异性效应细胞的原初细胞毒性T淋巴细胞前体(CTLp)的方法,能够体内裂解肿瘤细胞。该活体外方法产生完全功能化的效应细胞。用人I类HLA分子和限定性辅助分子转染人工抗原提呈细胞(AAPC;果蝇),使用其刺激正常供体和癌症患者的CD8+T细胞。在果蝇细胞表面高密度表达的I类分子是空的,使得可以有效装载多种肽,这导致产生识别内源性表达特定肽的肿瘤细胞的多克隆应答。如果肽表位是确定的免疫原,则产生的应答是强烈的、抗原特异性的和可重复的应答。这种人工抗原表达系统可适于治疗绝大多数人的大多数癌症。
Description
本发明背景
尽管在癌症治疗方面取得了显著进步,但癌症仍然是严重的健康问题。标准治疗方案化疗、放疗、外科手术以及三者联合通常不能获得持久的痊愈。在许多情况下,进行治疗的癌症患者经常在一段时间后旧病复发,使病情进一步恶化,这些治疗方案对患者存在严重问题。
困扰癌症治疗进展的另一个因素是已经发现癌症不是由单一生物因素引起,而是由综合因素引起。与治疗集中于单一病原体或致病因子的大多数药物治疗不同,癌症治疗需要处理多种生物因素。
近年来,研究定位于开发利用患者自身免疫系统的癌症疗法。过继免疫治疗是这类方法其中一种。过继免疫治疗需要利用患者自身细胞产生细胞毒性T淋巴细胞(CTL)来对抗肿瘤细胞或癌细胞。但是,该技术基本上仍未批准作为可行的临床治疗方案用于人类患者。除鉴定免疫产生CTL的合适表位的问题之外,现有技术不能提供一种将足够数目的不同表位提呈给APC以便充分靶向多种抗原从而有效治疗癌症的方法。本发明满足了这些未被满足的需求,并有其它有益之处。
本发明概述
本发明提供一种能同时提呈高达10个或更多个不同肽的非天然抗原提呈细胞(nnAPC)、生产nnAPC的方法、使用所述nnAPC治疗癌症的方法。

本发明详述
本发明提供一种治疗癌症患者的方法,该方法包括:
a.制备一种非天然抗原提呈细胞系(nnAPC),其中所述nnAPC能够同时提呈高达约15种不同的癌症相关肽分子,优选约10种不同肽分子,其中每种肽约6-12个氨基酸长,优选约8-10个氨基酸长,浓度范围约10nM至100μM;
b.由所述患者或合适供体收集CD8+细胞;
c.用所述nnAPC细胞系刺激所述CD8+细胞;
d.将所述CD8+细胞加入含细胞因子(例如IL-2、IL-7)的培养基中或条件生长培养基(CGM)中,优选IL-2或IL-2和IL-7联合应用;
e.将收集自所述患者或合适供体的非悬浮外周血单核细胞或者去除CD-8的外周血单核细胞和约10-50μg/ml的肽混合;
f.用剂量必须足以防止悬浮液中的这些细胞增殖的γ-射线照射所述外周血单核细胞悬浮液,例如约3,000-7,000rad的剂量,优选约5,000rad,或者可用细胞抑制剂(包括但不限于丝裂霉素C)处理外周血单核细胞悬浮液;
g.分离粘附外周血单核细胞;
h.将约10ng/ml至10μg/ml的所述每种肽加入所述粘附外周血单核细胞;

f.用剂量必须足以杀死悬浮液中所有细胞组分(需要的外周血单核细胞除外)的γ-射线照射所述外周血单核细胞悬浮液,例如约3,000-7,000rad的剂量范围,优选约5,000rad;
g.分离粘附的外周血单核细胞;
h.将约10ng/ml至10μg/ml所述每种肽装载至所述粘附外周血单核细胞;

本发明提供一种非天然抗原提呈细胞(nnAPC),该细胞得自用人I类HLA分子、结合分子和共刺激分子表达编码DNA转染的果蝇(Drosophila melanogaster)细胞,其中所述nnAPC能够提呈高达约15种不同的肽分子,优选10种肽分子。

Her-2/neu(p185)是Her-2/neu癌基因的蛋白产物。Her-2/neu基因和Her-2/neu蛋白在各种各样的癌症(包括乳癌、卵巢癌、结肠癌、肺癌和前列腺癌)中扩增和过表达。Her-2/neu与恶性肿瘤转化有关。其可见于50-60%的管型原位癌和所有乳腺癌的20-40%,并可见于大部分卵巢、前列腺、结肠和肺腺癌。Her-2/neu不仅与恶性表型密切相关,而且与恶性肿瘤侵蚀有关,其可见于全部浸润性乳癌的1/4。Her-2/neu过表达与乳癌和卵巢癌的预后不佳相关联。Her-2/neu是一种相对分子量为185kd、长度大约为1255个氨基酸的跨膜蛋白。其具有一个大约645个氨基酸的与表皮生长因子受体(EGFR)大约40%同源的胞外结合结构域、一个高度疏水的跨膜锚式结构域(TMD),以及一个大约580个氨基酸的与EGFR80%同源的羧基末端胞质结构域(CD)。
正在进行的癌基因相关研究鉴定了至少40个在恶性肿瘤细胞中有效并且负责转化或与转化相关的癌基因。根据推定的其基因产物(例如癌基因表达的蛋白)的功能或位置将癌基因分为不同的类别。据信癌基因对正常细胞生理的某些方面是必须的。
尽管在癌症治疗方面取得了显著进步,但癌症仍然是严重的健康问题。标准治疗方案化疗、放疗、外科手术以及三者联合通常不能获得持久的痊愈。在许多情况下,进行治疗的癌症患者经常在一段时间后旧病复发,使病情进一步恶化,这些治疗方案对患者存在严重问题。就黑素瘤来说,使用常规化疗还没有治愈转移性黑素瘤。据报道,组合化疗的Dartmouth方案(DTIC、顺铂、BCNU和他莫西芬)的反应率为35%-50%,但生存时间维持在6-10个月。已经报道了强烈“高剂量强度”化疗自体骨髓移植增强造血的缓解率高。尽管如此,存活的平均时间长度短暂,大约为4个月。
Rosenberg和同事尝试使用活化淋巴细胞输注治疗各种癌症。最初,使用由IL-2活体外激活的淋巴因子激活的杀伤细胞(LAK)和后期癌症浸润淋巴细胞(TIL),但有效的证据不可靠。实际上,对照临床实验未显示出使用活体外激活细胞超越直接给予患者IL-2的优势。因此,LAK和TIL治疗的利益是最低限度的,副作用通常非常严重,以至于许多实验过早停止。
小鼠肿瘤模型研究已证明,肿瘤抗原特异性T细胞体内免疫的过继免疫治疗非常有效,毒性最小。应用该策略治疗人肿瘤的主要障碍是鉴定使肿瘤细胞对细胞毒性T淋巴细胞(CTL)介导的杀伤敏感的免疫原性抗原。由黑素瘤患者分离肿瘤反应性T细胞引致鉴定出某些CTL直接针对的肿瘤抗原(表位)。这些表位包括酪氨酸酶(Brichard等,J.Exp.Med.(1993)178:489-495;Robbins等,Cancer Res.(1994)54:3124-3126)、MART 1/Melan A(Kawakami等,J.Exp.Med.(1994)180:347-352)、gp 100(Bakker等,J.Exp.Med.(1994)179:1005-1009;和Kawakami等,J.Immunol.(1995)154:3961-3968)和MAGE(Gaugler等,J.Exp.Med.(1994)179:921-930)。其中,酪氨酸酶和MART-1几乎普遍表达于黑素瘤,因此它们是过继免疫治疗的必然选择。
近些年,人们注意到小百分比的进行免疫治疗的黑素瘤患者几年来的存活率显著改善。这包括用“癌症疫苗”主动特异性免疫治疗以及利用免疫系统非特异性增强剂,例如IL-2、α-干扰素和γ-干扰素之类的细胞因子。但是,经常伴随细胞因子使用出现的副作用(例如恶心、发烧)降低了细胞因子的利益。
细胞溶解T细胞(CD8+)是对抗病毒感染的主防御线。CD8+淋巴细胞特异性识别和杀伤感染病毒的宿主细胞。理论上讲,应当有可能利用免疫系统抗击其它类型的疾病(包括癌症)。但是,几乎不能获得特异性激活CTL的体外/活体外方法。现在,鉴定出以上提及的关键黑素瘤抗原和以下描述的特异性体外激活CTL的方法使得可以验证过继免疫治疗转移性黑素瘤这个想法。
所有原初T细胞都需要两个活化信号来激发免疫应答。对于CD8+淋巴细胞(CTL)来说,第一个信号赋予特异性,包括将抗原的免疫原性肽片段(表位)提呈给CD8+细胞,所述表位与存在于抗原提呈细胞(APC)表面的MHC I类(HLA)复合体结合。该复合体由在胞内传递信号的T细胞抗原受体(TCR)特异性识别。
结合T细胞受体对于诱导T细胞活化是必须的,但并不足够,通常不能使细胞增殖或分泌细胞因子。完全激活需要用于进一步增强活化级联的第二个共刺激信号。在抗原提呈细胞上的共刺激分子中,B7和细胞粘附分子(整合素)如ICAM-1分别通过结合T细胞上的CD28和LFA-1参与该过程。在存在合适共刺激分子作用的情况下,CD8+细胞与携带结合MHC I类分子的免疫原性肽(表位)的抗原提呈细胞相互作用,此时CD8+细胞成为完全活化的溶细胞型T细胞。
淋巴细胞介导的细胞杀伤作用包括一系列生物事件,起始事件是CD8+CTL通过上述T细胞活化的识别方法结合携带抗原的靶(肿瘤)细胞。
图1图示了上述CD8+细胞与抗原提呈细胞或靶(肿瘤)细胞之间的相互作用。该相互作用起始于与APC或靶细胞上的MHC I类分子相接的抗原结合T细胞抗原受体(TCR)。辅助分子如淋巴细胞功能性抗原(LFA-1、LFA-2和LFA-3)、胞间粘附分子1(ICAM-1)、T细胞共刺激因子(CD2、CD28、B7)增强细胞-细胞粘附或转导另外的细胞活化信号。
在细胞-细胞作用后,CTL通过活化可溶性溶细胞介导物(储存在T细胞胞质颗粒中的穿孔素和粒酶)和CTL表面分子(Fas配体)杀伤靶细胞。在溶细胞攻击后,靶细胞死于坏死(膜穿孔和细胞器损坏)或凋亡(染色质凝聚、DNA片段化和膜起泡)。
图2图示了淋巴细胞介导的溶细胞机制。在图2的图A中,在结合靶细胞后,CTL中的胞质颗粒再次快速定向于靶细胞,以将含穿孔素和粒酶的颗粒释放到胞间隙。这些蛋白水解酶在靶细胞的胞质膜上形成孔,最终导致细胞坏死。在图B中,在结合靶细胞后,CTL上Fas配体的表达水平增加。Fas配体和靶细胞上Fas受体的相互作用导致凋亡。CPP32之类的蛋白酶和与IL-1b-转化酶(ICE)有关的其它酶参与诱导凋亡。天然抗原提呈细胞(例如树突细胞、巨噬细胞、自身肿瘤细胞)有可能用于CD8+活化。但是,遵循该方法的活化效率低。这是因为I类天然APC分子在肿瘤表位旁包含许多其它类型的肽表位。这些肽有许多来自正常无毒细胞蛋白,导致将实际上可有效抗肿瘤的活性天然APC数量稀释(Allison等,Curr.Op.Immunol.(1995)7:682-686)。
解决该问题更直接有效的方法是仅用与对抗特定疾病(例如癌症)有关的那些表位或肿瘤特异性抗原(例如黑素瘤特异性抗原)特异性激活CD8+细胞。为此,通过在果蝇细胞中表达MHC I类分子产生人工抗原提呈细胞。因为果蝇不具有免疫系统,所以不存在参与将肽表位运载至I类分子上的TAP-1,2肽转运蛋白。因此,I类分子作为空壳出现在果蝇细胞表面。通过将这些转染果蝇细胞与结合I类分子的外源肽(例如癌症或肿瘤特异性表位,包括但不限于黑素瘤特异性抗原)温育,有可能用所述肽占据每个I类分子。在这些果蝇APC中高密度表达含单个肽的I类分子使得可以体外产生细胞毒性CD8+T细胞,其对所述抗原肽是完全特异性的。1996年6月25日颁发的题为“使用表达人MHC I类分子和β2-微球蛋白的昆虫细胞体外活化细胞毒性T细胞”的美国专利第5,529,921号和1994年5月24日颁发的题为“表达MHC I类抗原和β2-微球蛋白编码基因并能够装配空复合物的果蝇细胞系及所述细胞系的制备方法”的美国专利第5,314,813号讲述了果蝇细胞的制备方法和步骤。具体地说,美国专利第5,529,921号在第26栏56行至第28栏22行公开了分离和/或富集前体细胞培养物的各种方法。
另外,该特征消除了对用高剂量的各种细胞因子体内刺激免疫系统的需要。由此产生一种避免了由细胞因子引起的副作用的治疗方法。或者,在适宜以及患者可获得利益的合适情况或条件下,可用低水平剂量的α干扰素、γ干扰素和/或IL-2同时治疗患者。
消除对用细胞因子体内刺激的需要使患者治疗质量提高。包括给予患者细胞因子的治疗方案经常导致患者出现流感样症状,例如恶心、呕吐和发烧。尽管在已经很虚弱的患者身上发生的特别严重的反应可能危及生命,但这些副反应一般不致命。另一个考虑是这种副反应对其它有益治疗方案的患者接受度和适应性有不利影响。消除细胞因子体内刺激的需要产生的治疗方案改善了患者的舒适性,并提供给临床医师一种其患者更愿意服从的有效治疗方案。
已经使用作为APC的转染果蝇细胞和来自T细胞受体(TCR)转基因小鼠2C系的CD8+细胞在小鼠中证明了该方法对肿瘤过继免疫治疗有用。在该系统中,纯化的CD8+2C细胞对还携带有共刺激分子B7-1和ICAM-1的MHC I类分子(Ld)转染的果蝇细胞体外提呈的肽高度起反应。转染的果蝇细胞作为确定最小刺激需要的探测物用于刺激未激发CD8+T细胞(Cai等,P.N.A.S.USA(1996)93:14736-14741)。或者,当使用未分离的小鼠脾细胞代替纯化的2C细胞作为应答物时,不需要使用共刺激分子。在此情况下,脾细胞群中的CD8+细胞接受活化B细胞的“旁观者”共刺激。利用此发现,有可能证明MHC I类分子(Ld)转染的果蝇细胞能够诱导正常的DBA/2小鼠脾细胞在没有加入淋巴因子的情况下体外对同源P815肥大细胞瘤特异性肽产生应答。将这些CTL注射入患P815肥大细胞瘤的DBA/2小鼠中导致肿瘤快速退化(Sun等,Immunity(1996)4:555-564)。
从程序上讲,体外培养正常DBA/2小鼠脾细胞和装载P1A.35-43肽的MHC I类分子(Ld)转染的果蝇细胞,P1A.35-43肽是一种得自DBA/2产生的P815肥大细胞瘤细胞系的肿瘤特异性表位。5天后由培养物收集淋巴细胞,其显示出对P815肿瘤细胞的强体外细胞毒性T淋巴细胞(CTL)活性,但不能裂解P1024(一种不表达P1A.35-43的P815成熟细胞系),如图3的图A所示。当这些CTL注射到3天前接种P815细胞的DBA/2小鼠中时,第一周肿瘤生长未受阻碍,但随后在第二周中肿瘤消除,如图3的图B所示。当用不相关抗原(例如病毒核蛋白肽)体外免疫CTL时,对P815生长没有任何作用,这证明了特异性,如图3的图B所示。总之,主要组织相容性复合体I类分子(Ld)转染的果蝇细胞诱导正常的DBA/2小鼠脾细胞在没有加入淋巴因子的情况下体外对同源P815肥大细胞瘤特异性肽产生应答。将这些CTL注射入带有P815肥大细胞瘤的DBA/2小鼠中导致肿瘤快速退化(Wolfel等,J.Exp.Med.(1993)178:489-495)。
体外人体研究
体外对健康测试者的人CTL进行抗酪氨酸酶免疫。在仅用果蝇细胞初次刺激后,以测试的全部CTL效应细胞对JY靶细胞的比率证实携带酪氨酸酶的JY细胞特异性裂解。使用全刺激/再刺激方法诱导健康受试者的酪氨酸酶特异性CTL,并测试其杀伤Malme 3M黑素瘤细胞系的能力。除了一个或两个可能的例外,在所有供体中都不同程度地诱导出抗Malme 3M的特异性CTL活性。一般来说,针对对照Malme 3肿瘤细胞的反应性最小。还对黑素瘤患者的细胞进行体外抗酪氨酸酶表位免疫,以产生针对健康志愿者细胞的相似活性和特异性的CTL。
使用任何天然或人工抗原提呈细胞(APC)系统体外产生细胞毒性T淋巴细胞受限于这些系统能够产生的抗原特异性。
利用以下的APC系统产生针对单个表位的抗原特异性CTL:1)用限定肽脉冲刺激人树突细胞(DC);2)用肽脉冲刺激由原淋巴细胞产生的外周血单核细胞(PBMC);3)其中的天然肽用酸萃取并装载目的肽的类淋巴母细胞系(LCL);4)工程为表达空I类分子的果蝇细胞;用人I类分子和共刺激分子转染的小鼠3T3细胞(J.B.Latouche和M.Sadelain,Nature Biotech(2000)18:405-409)。
人们认为树突细胞(DC)是人体中提呈触发抗原的细胞系统,因为其作为触发抗原提呈细胞广泛应用。自体或外源蛋白在DC中加工。产生的肽表位由HLA分子提呈,并转运至DC表面。但是,已发现DC不能始终如一地在体外产生抗4种不同肽的CTL。这应当提供对4种肽每一种都具有活性的CTL。另外,还发现在肽脉冲刺激时DC表型成熟或不成熟不影响结果。
另外,果蝇细胞刺激经常产生抗高达10种不同类型肽的CTL。这提供了对10种肽的每一种都有活性的CTL。
按照对每种肽的标准刺激方案,开始评价果蝇细胞和DC激发针对单个肽表位的CTL应答的能力。为了对比DC和转染的果蝇细胞,在IL-4和GM-CSF存在下通过培养自体单核细胞1周产生不成熟DC。在收获前24小时通过向培养基中加入TNFα,由不成熟DC获得成熟DC。收获(成熟或不成熟)DC,用肽脉冲刺激,按照用于刺激CD8细胞和肽脉冲刺激的果蝇细胞的方法将DC与纯化的CD8细胞混合。当对酪氨酸酶肽表位689评价时,发现一般来说果蝇细胞是比DC更好的刺激物,如图7所示。此外,当使用限定肽脉冲刺激APC时,显示出不成熟或成熟表型的DC(图8)在激发特异性CTL应答方面没有果蝇细胞有效。这特别令人惊奇,因为DC在免疫系统中起主要作用。进行了一个使用1个供体的对比研究,如图9所示。当使用果蝇细胞作为刺激物时,产生抗4种不同肽的特异性杀伤作用,而不成熟DC产生微量的特异性杀伤作用,成熟DC产生仅针对用于刺激的4种肽中的一种的特异性杀伤作用。
制备细胞毒性淋巴细胞
通过用抗CD8抗体正选择由去除白细胞的样品中分离CD8+细胞,用表达人I类分子(HLA-A2.1)、B7.1、ICAM-1、LFA-3和B7.2的果蝇细胞提呈的4种不同黑素瘤相关肽刺激CD8+细胞。在IL-2和IL-7存在下,用装载所述肽表位的自体单核细胞再刺激CD8+细胞两轮。用OKT3和IL-2非特异性扩增CTL。检测抗Malme 3M细胞的CTL活性,并通过流式细胞仪测定CD8+T细胞的纯度。
按照GLP和GMP实施生产工艺和步骤。“GLP”和“GMP”是由美国FDA制订的实验室和生产管理标准,本领域技术人员很容易理解。监测CTL的特性、生存能力、CTL活性、无菌性和内毒素含量。
以下表1显示了适用于本发明的乳癌和卵巢癌治疗方法的肽表位清单。对本领域一般技术人员来说显而易见的是,除了以下表1列出的那些肽表位之外,还有各种各样的肽表位适用于本发明的乳癌和卵巢癌治疗方法,只要这种肽是T细胞表位。

实施例1
生产果蝇抗原提呈细胞
按照公开的方法由果蝇(Oregon-R)卵制备Schneider S2细胞系,并已将其保藏在ATCC(CRL 10974)。用补加10%胎牛血清的市售Schneider果蝇培养基培养S2细胞。
在S2细胞中表达MHC I类分子和共刺激蛋白的pRmHa-3质粒载体得自如文献所述构建的pRmHa-1表达载体。该载体含有金属硫蛋白启动子、金属效应共有序列和携带有分离自果蝇的聚腺苷酸化信号的乙醇脱氢酶基因。
如下制备用于转染的互补DNA:

分别将互补DNA插入到pRmHa-3载体中。使用磷酸钙沉淀法,用HLA-A2.1、B7.1和ICAM-1质粒DNA和phshneo质粒的混合物转染S2细胞。通过用含遗传霉素的Schneider培养基培养选择稳定转染的细胞。使用前24小时,通过加入CuSO4诱导转染基因表达。使用抗HLA-A2.1、抗B7.1和抗ICAM-1的抗体,通过流式细胞仪测定表达水平。细胞的HLA表达超过30%对CD8+淋巴细胞的体外有效活化是必须的。
分离人CD8
+
细胞
使用DynabeadsTM分离法(Dynal),通过正选择分离去除白细胞样品中的CD8+细胞。将抗人CD8小鼠单克隆抗体(50μg/ml的人γ球蛋白溶液[])加入到已清洗细胞的Dulbecco PBS溶液中,Dulbecco PBS溶液中补加有1%人血清白蛋白(Baxter-Hyland)和0.2%柠檬酸钠。温和搅拌下于4℃温育45分钟后,清洗细胞,并重悬浮在相同的缓冲液中,该缓冲液含有1∶1珠/细胞比率的Dynal磁珠,磁珠上包被有绵羊抗小鼠IgG。将细胞和珠放入无菌试管中,于4℃温和搅拌45分钟。搅拌结束时,使用分离器,按照生产商(Dynal)的说明,以磁力取出结合抗体的细胞。在CD8肽59-70(AAEGLDTQRFSG;SEQ ID NO:12)存在下,通过于37℃温育45分钟解离CD8细胞-珠复合物。以磁力去除游离的珠,通过流式细胞仪计数并分析CD8细胞,以评价纯度。CD8+细胞的回收率通常超过80%。表2概述了通过抗CD-8抗体正选择由正常人PBMC制品分离的14种CD8+制品的细胞组成。

体外免疫纯化的人CD8+细胞
初次刺激
将转染的果蝇S2细胞在补加10%胎牛血清和CuSO4的Schneider培养基(106细胞/ml)中于27℃培养24小时。收获细胞,用含100μg/ml人酪氨酸酶369-377的昆虫X-press培养基(BioWhittaker)清洗并重悬浮。于27℃温育3小时后,以1∶10的比率在补加10%自体血清的RPMI培养基(Gibco)中混合S2细胞和CD8+细胞。将细胞混合物于37℃温育4天,在此期间果蝇细胞相继死亡。在第5天,加入IL-2(20U/ml)和IL-7(30U/ml)选择性扩增酪氨酸酶特异性CTL群。
再刺激
将去除白细胞时获得的冷冻自体CD8缺失PBMC解融,用含10%自体血清(作为β2微球蛋白源)和20μg/ml酪氨酸酶369-377的RPMI培养基清洗以106细胞/ml重悬浮。γ辐射(5,000rad)后于37℃温育细胞2小时。通过用Dulbecco PBS清洗去除未粘附细胞。通过在含10%自体血清和10μg/ml酪氨酸酶369-377的Hepes缓冲的RPMI培养基中温育90分钟使粘附单核细胞装载酪氨酸酶表位。去除上清液,以10CD8+细胞/1粘附单核细胞比率加入果蝇细胞-活化CD8+细胞悬浮液(3×106细胞/ml补加10%自体血清的RPMI培养基)。于37℃培养3-4天后,加入IL-2(20U/ml)和IL-7(30U/ml)改变培养基,以选择性扩增酪氨酸酶特异性CTL群。

活性和纯度测定
CTL测定
Malme 3M细胞在51Cr释放测定中用作靶细胞。用0.1mCi51Cr于37℃对在含4%胎牛血清、1%HEPES缓冲液和0.25%庆大霉素的RPMI培养基中的5×106Malme 3M细胞标记1小时。用含10%胎牛血清(HyClone)的RPMI清洗细胞4次并稀释至105细胞/ml。以100∶1、20∶1和4∶1(效应细胞∶靶细胞)的比率将100μl效应CTL和100μl装载肽的51Cr标记的Malme 3M靶细胞混合在一个96孔微量滴定板中。以20∶1(K562:Malme 3M)的比率加入K562细胞,以降低天然杀伤细胞的基础裂解。使用非肿瘤HLA-A2.1成纤维细胞系Malme 3评价非特异性裂解。平行测定包括检测51Cr自发释放和最大释放的对照。于37℃温育6小时后,离心滴定板,并计数上清液,以检测51Cr释放。

实施例2
抗黑素瘤的细胞毒性T细胞输注试验
试验目的
本实施例讲述了细胞毒性T细胞输注治疗黑素瘤的效果,其评价根据以下因素:
1.体外免疫后再输注自体CTL的安全性和耐受性;
2.在有限稀释分析中输注的CTL在体循环分解中的动力学;
3.放射性闪烁照相法测定的CTL全身分布;
4.免疫组织学检测的活组织结节的细胞组成(CTL、TH、NK、B细胞);和
5.两个月中可检测病灶的消退和反应时程。
患者群
适合治疗需要的患者具有历史记录的、不可切除的、可检测或可评价的恶性黑素瘤以及HLA-A2单倍型。治疗前评价包括通过MRI或CT扫描放射性评价大脑,胸部和腹部CT扫描以及物理检查尤其是皮肤和淋巴结。治疗的患者总数是15人(9男6女)。年龄由33岁至75岁,平均58岁。转移性肿瘤的平均病程为1.5年。对14/15的患者进行治疗前皮试,以测定是否存在无变应性状态,对于评价的全部7种普通抗原,5/14的患者测试为阴性。通过采用HLA-A2特异性单克隆抗体(BB7.2)的FACS分析筛选患者的HLA-A2单倍型。通过PCR分析进行再分类。除了1名患者之外,其他全部患者都是HLA-A*0201,例外者(患者08)是HLA-A*0205。
用活体外产生的自体CTL治疗
按照该临床方案治疗15名患者。所有患者都接受至少1次自体CTL输注。表1概述了给予每名患者的轮次和细胞剂量。体外产生的细胞数取决于与患者相关的因素,例如由回输血方法分离的PBMC数和每个PBMC群中存在的CD8+T细胞数。因为全部体外产生的细胞都回输入供者体内,所以给予每名患者的剂量必然有所不同。在对患者之间给予剂量归一化的尝试中,记录每种剂量的理论“效力”得分。通过将细胞总数乘以由装载肽的靶细胞获得的溶细胞活性获得该值。T细胞输注剂量由最小的4×107(患者08)至最大的3.2×109(患者13)。患者根据其在每轮治疗末期的临床状态进入第2、3或4轮治疗。由回输血样品获得的PBMC数在进行另外轮次治疗的患者中往往较低,特别是如果随后轮次治疗紧接着前一轮次治疗结尾的话。这要归因于在先前轮次治疗过程中由于给予IFNα-2b而产生的持久性淋巴细胞减少。分离的原初天然CD8+T细胞总数取决于其在每个PBMC群中的百分比。患者中CD8+T细胞的百分比在8%-31%之间变化。所获得的扩增率也影响最终细胞数,其在0.1-6.0倍之间变化。以上的说明书和实施例1讲述了活体外产生CTL的方法。
上调I类分子和黑素瘤相关抗原对IFNa-2b的应答
在增强抗原特异性CTL体内溶解黑素瘤细胞能力的尝试中,在CTL输注之前连续5天给予低剂量IFNα-2b,再以每周3次给予4周。检测对细胞因子体内应答的一种方式是通过用特异性抗体阳性染色的免疫组织化学分析评价在连续时间点获得的活组织切片。由1名多皮肤病灶患者(患者04)获得连续活组织切片,以评价I类分子和抗原表达。活组织切片检查表明,在任一治疗前I类分子和MART-1表达为弱阳性(活组织切片A)。皮下注射10MU/m25天后,观察到这两个标记物急剧增加(活组织切片B)。对于酪氨酸酶和gp100来说,治疗前样品(活组织切片A)的免疫组织化学染色分别是阴性和弱阳性。最初的5天IFNα给药以及13次追加给药之后,染色组织样品中的后期抗原表达增加(活组织切片C)。
活体外产生的CTL的抗原特异性
如果可获得活组织物质建立细胞系,则在释放当天评价所有患者产生的抗装载肽的T2靶、HLA-A2黑素瘤细胞系(Malme3M)和自体黑素瘤系的CTL。评价每种制备剂量细胞的溶细胞活性。使用单独提呈每种肽或同时提呈全部4种肽的装载肽的T2细胞测定每名患者产生的CTL应答的特异性。评价HLA-A2匹配细胞系或自体肿瘤系溶解携带内源表达的黑素瘤相关抗原的细胞的能力。除了溶细胞活性之外,用已确立的检测在特定肽刺激物作用下产生的胞内γ干扰素产量的方法评价抗原特异性。通过该方法评价活体外方法结束时产生的CTL。分别记录对每种肽特异性的细胞百分比。通过加上T细胞群中检测的每种肽的特异性计算患者13的每批CD8培养物的特异性细胞数。每个连续治疗周期都可检测到特异性细胞总数增加。
检测CTL治疗后CD8和CD4细胞浸润性肿瘤活组织切片
理想的是治疗前、中、后全部患者的活组织切片样品。但是,实验条件使得活组织切片样品仅限于一定数量的患者。由研究登记的15名患者中的5人获得肿瘤组织。在T细胞治疗后第5和第6周分别获得两名患者(患者08和13)的活组织检查样品。组织样品检测揭示,同时存在浸润性CD8和CD4细胞。其中一个肿瘤样品取自头皮枕骨区域的皮肤病灶,该病灶在T细胞第2次输注后4周,即到随后的检测时其尺寸增加。活组织切片检查揭示淋巴细胞严重浸润的组织坏死。其它活组织切片得自臀移植手术过程中取出得大腿骨顶部。患者08的皮肤病灶一般性I类分子和特异性HLA-A2标记都是强阳性(4+)。酪氨酸酶和gp100是弱阳性(分别是1+和2+),而同一样品中的MART-1是阴性。患者13的活组织切片区也坏死,外源染色更多;不同肿瘤细胞群的HLA-A2.1以及一种或多种MAA表达缺乏。但是,完整组织区显示强I类分子(4+)和全部黑素瘤相关抗原。后一样品中的淋巴细胞浸润似乎环绕在肿瘤块周围,而不是深深地浸润在肿瘤中。但是,与肿瘤直接相关的最高百分比细胞是CD8细胞。这两名患者没有治疗前活组织样品阻碍了对治疗前组织样品中相似类型浸润细胞的验证。
T细胞治疗后CT扫描证实目的应答
CT扫描是治疗前筛选标准和治疗后跟踪检查的一部分。患者10在治疗前扫描(99年6月23日)后5周(99年7月27日)接受单剂8×108CTL输注,输注后1个月(8/27/99)重复胸部CT扫描时,观测到肺病灶尺寸急剧减小。同样,患者14在首次输注6.6×108细胞(99年10月5日)前3.5周进行胸部CT扫描(99年9月10日),作为登记过程的一部分。第2次输注11.5×108细胞后1个月的后续CT扫描(99年1月7日)揭示三个单独的病灶急剧收缩。根据CT扫描前和扫描后的检测,患者13也具有目的应答。I期治疗后气管旁腺病由7.8cm2(研究前)变为4.4cm2,并在II期治疗后消失。
无变应态存在不妨碍产生CTL的能力或阻碍临床反应
按照该方案治疗的大多数患者以前均接受了医学治疗。进行治疗前皮试来测定对一组7种普通抗原的无变应性反应是否与不能活体外产生CTL有关,或预防记录到的临床反应。活体外产生CTL的能力与患者的治疗前皮试结果无关。应当指出的是,患者03和04(都是混合反应者)在第二轮治疗开始之前重复进行了皮试,仍有无变应性反应。
实施例3
产生能够溶解乳癌和卵巢癌细胞的HER-2/neu特异性CTL
我们感兴趣的是将我们的CTL生产技术应用于其它类型的肿瘤,以确定是否可用该方法治疗所有形式的癌症。HER-2/neu是一种与EGFR同源的原癌基因,在许多人类癌症(主要是乳房、卵巢和结肠的腺癌)中扩增和过表达。其经常与侵袭性疾病有关,并可能是预后不佳的指标。已经有几个以这些类型的癌症作为可能标靶的临床实验对其进行了研究。
在20世纪90年代早期,或者通过计算机辅助性肽结合算法,或者通过分离自卵巢癌患者腹水的CTL作图,鉴定了HER-2/neuHLA-A2.1限制性肽表位(表3)。

尽管转染的果蝇细胞具有独特的提呈高达10种不同肽表位的能力(图10),但我们选择4种HER-2肽826、835、861和863,原因是活体外产生针对这些肽的CTL的频率。这4种不同的HER-2肽代表弱至中等的HLA-A2.1(存在于转染的果蝇细胞表面)结合物。我们倾向于包括弱的A2结合物,因为我们在使用黑素瘤相关肽方面的经验提示,如果肿瘤细胞确实是天然T细胞表位,则弱的I类结合物一般产生有效识别该肿瘤细胞的CTL。我们作为靶的大部分肿瘤相关蛋白是自身抗原,因此,预期用病毒肽可观测到它们对I类分子具有高亲和力。低至中等的结合物一般产生非常有效溶解肿瘤细胞的CTL。这可由MART-1肽得到证实,MART-1肽是果蝇细胞上的一种低亲和力结合物(图3),还是一种常规生产能够有效溶解装载肽的靶细胞(T2)或更重要的黑素瘤细胞(Malme3M)(图12)的CTL的表位。
HER-2/neu是EGF-R家族一员,起生长因子受体作用。HER-2蛋白在人胎儿发育时表达。在成年人中,在许多正常组织的上皮细胞中可微弱检测出该蛋白。正常细胞中HER-2基因以单拷贝存在。在许多人类癌症中都鉴定出该基因扩增和/或相关蛋白过表达,包括乳癌、卵巢癌、子宫癌和肺腺癌。HER-2和EGF-R受体之间的序列差异示于表5。我们评价的4种HER-2肽中的3种在两种蛋白之间具有三个或更多个氨基酸变化。单氨基酸改变足以区分两种蛋白。

一旦在4周的活体外刺激方案之后产生了CTL,我们就评价使用免疫肽制备的HLA-A2.1四聚体分子是否提呈肽特异性细胞。如图13所示,产生肽特异性CTL的能力是供体依赖性的。在图A(供体261)中,供体对肽835产生强CTL应答(37.55%)。在图B(供体262)中,用835和861四聚体分子可检测到肽特异性CTL(分别为3.6%和15.1%)。这支持在刺激方案结束时使用多种肽来保证肽特异性CTL。该活体外方案使得人们可以相对较容易地产生多种特异性CTL。
抗肽和抗肿瘤反应
完成全部活体外方案之后,评价产生的CTL的抗原特异性。为产生CTL,在第0天使果蝇细胞装载组合的4种HER-2肽。在4周活体外刺激方案结束时,评价整批CD8培养物的抗原特异性。装载每种免疫性肽的T2细胞用作靶细胞。在图14中,图示了典型的反应。整批培养物含有对4种HER-2肽每一种的特异性。用卵巢肿瘤细胞系(ATCC;HTB-77)评价抗肿瘤反应。当靶细胞系不是HLA-A2.1限制性细胞系时,我们将细胞系转染为具有+/-测定系统。当将HLA-A2.1转染入HTB-77细胞系中时,观测到CD8效应细胞的杀伤作用增强(图15,图A-D)。评价代表各个肽的HER-2特异性效应细胞,以证实将每种肽表位都提呈至该肿瘤细胞系。
还评价了用HLA-A2.1转染的乳腺癌细胞系(ATCC,HTB-131)显示HER-2特异性肽效应细胞溶解肿瘤的能力。当用HLA-A2.1转染时,肽861特异性CTL可以溶解该肿瘤细胞系(图16)。
肿瘤细胞裂解需要IFNγ处理
HTB-77/A2.1细胞系需要用IFNγ预处理,以显示肽特异性裂解。在51Cr-释放测定开始前24小时用500U/ml IFNγ(比活25nG/ml)处理细胞。在图17中,加入IFNγ导致HLA-A2.1转染细胞系的裂解增强。为测定该剂量IFNg对HLA-A2.1和HER-2二者表面表达的影响,进行FACS分析,以测定诱导24小时和48小时后这些分子的水平。图18的图A和B显示了FACS分析结果。在图A中,用IFNg诱导后24和48小时HTB-77细胞表面上的HER-2分子没有增加。在HLA-A2.1转染细胞中,在相似处理方法之后HER-2和HLA-A2.1的表面表达水平都没有显示增加。要注意的是,当通过微阵列DNA芯片分析评价mRNA水平时(图19),TAP-1以及HLA-DM和HLA-DR、组织蛋白酶S和D以及天冬氨酸特异性半胱氨酸蛋白酶5(Caspase 5)的表达水平增加。这应当可以解释为什么HTB-77/A2.1细胞的杀伤作用在IFNγ存存在下增强。这种特殊分子的上调导致HER-2分子的加工更有效,使得可以更好地提呈目的肽。
肽
使用肽合成仪(Gilson Company,Inc.),通过标准Fmoc化学制备合成肽。通过C-8柱反向HPLC将所有肽都纯化至大于95%纯度。使用电喷离子化质谱确定纯度和身份。黑素瘤相关肽包括:MART-1特异性的肽819(AAGIGILTV SEQ ID NO:6)、都是gp100肽的肽817和853(分别为ITDQVPFSV SEQ ID NO:4和KTWGQYWQV SEQ IDNO:5)、酪氨酸特异性的肽689和792,其中肽689是天然序列(YMNGTMSQV SEQ ID NO:1),肽792是天然序列的翻译后修饰型(YMDGTMSQV SEQ ID NO:2)。肽826(CLTSTVQLV SEQ ID NO:7)和835(KIFGSLAFA SEQ ID NO:8)分别代表p185蛋白胞内和胞外结构域的HER-2/neu序列。Pec6020(ALALAALLVV SEQ ID NO:10)和Pec6025(ALLVVDREV SEQ ID NO:11)是重叠序列,代表在卵巢肿瘤细胞系中检测到的粘蛋白。C-凝集素也是一种在卵巢肿瘤细胞系中检测到的蛋白,由其序列获得的肽(C-凝集素8)以KMASRSMRL SEQ D NO:9表示。
体外细胞毒性测定
进行标准51Cr-释放测定,以确定T2细胞上装载的黑素瘤相关肽表位的CTL效应细胞识别情况。使收获的3×106T2细胞在RPMI+10%FBS(培养基)中生长。加入0.1mCi的51Cr,在37℃水浴中温育。将标记细胞加入到10ml 4%清洗液(RPMI+4%FBS)和沉淀中,再清洗两次,用培养基重悬浮至终浓度为0.2×106/ml,记录自发裂解细胞/去污剂裂解细胞的放射性。用合适的肽以20μg/ml、30分钟脉冲刺激细胞。在96孔板的每个孔中加入50μl,每个孔含有10、2、0.4和0.08×106/ml的CD8效应细胞,将该板于37℃温育6小时,离心收获上清液。
流式细胞仪和四聚体染色
采用缀合FITC或PE的单克隆抗体,通过在FACS缓冲液(1%BSA、0.02%NaN3的PBS溶液)中于4℃温育30分钟,接着用相同缓冲液清洗,标记细胞。用0.5%甲醛固定细胞,然后用FACScan流式细胞仪(Becton Dickison)及其CellQuest软件进行数据采集和分析。用用于标记纯化一抗的相同二抗检测非特异性染色,或者当一抗为直接标记时用同种型匹配的对照检测非特异性染色。用作为阴性对照的具有序列SLYVTVATL SEQ ID NO:43的HLA-A2.1特异性HIVgag四聚体分子(Beckman Coulter)进行四聚体染色。制备的HER-2特异性四聚体肽序列为CLTSTVQLV(826 SEQ ID NO:7)、KIFGSLAFL(835 SEQ ID NO:8)或VMAGVGFSPYV(861 SEQ IDNO:16)。如包装说明书所述,PE标记的四聚体HLA-A2.1-肽复合物和异硫氰酸荧光素(FITC)-标记的抗人CD8a(BD PharMagin)单克隆抗体联合用于染色表位特异性CD8+ T细胞。用Becton DisckensonFACScan的双色流式细胞仪分析样品,并检测选通的CD8+T细胞的四聚体HLA-A2.1-肽复合物染色情况。
实施例4
用该活体外刺激方法产生其它的乳癌和卵巢癌特异性CTL
我们证实了对不同肿瘤来源的几种肿瘤抗原的所有已知HLA-A2.1-限制性肽表位产生CTL应答的能力。我们最初的研究集中于黑素瘤,对于黑素瘤我们能够证明在用CTL治疗的患者中产生目标临床应答,其中所述CTL对4种不同的肽表位是特异性的,具体为MART-1、gp100和酪氨酸酶、黑素瘤相关蛋白[Richards等,Amer.Soc.Clin.Oncol.,San Francisco,Calfornia(2001年5月)]。
为将该能力扩展至产生针对在各种各样的其它肿瘤中存在的其它肿瘤抗原的CTL,我们选择了公开序列和新序列为几种不同类型肿瘤共有的肿瘤抗原。这些抗原包括AES、MUC-1、CEA、FBP、C-凝集素、NY-ESO-1、Pec60、CA-125、MAGE-3、端粒酶和G250。
表7描述了这些抗原、其表达频率和表达这些抗原的癌症。用我们的活体外刺激方法产生的针对这些肽的应答频率列于表6。

1.非天然抗原提呈细胞系(nnAPC)在制备治疗黑素瘤患者的药物中的用途,其中所述nnAPC同时提呈高达10种所述黑素瘤相关肽分子,其中每种肽为8-10个氨基酸长,且其中所述肽选自:酪氨酸酶369-377YMNGTMSQV(SEQ ID NO:1)、酪氨酸酶369-377YMDGTMSQV(SEQ ID NO:2)、酪氨酸酶207-216FLPWE LFLL(SEQID NO:3)和gp100154-162KTWGQYWQV(SEQ ID NO:5)。

4.一种生产非天然抗原提呈细胞(nnAPC)的方法,其中所述nnAPC同时提呈高达15种癌症相关肽分子,所述方法包括以下步骤:
a.用黑腹果蝇卵制备昆虫细胞系;
b.使所述昆虫细胞在适于培养昆虫细胞的培养基中生长;
c.用pRmHa-1表达载体制备pRmHa-3质粒,其中所述pRmHa-3质粒包括金属硫蛋白启动子、金属效应共有序列和携带有分离自黑腹果蝇的聚腺苷酸化信号的乙醇脱氢酶基因;
d.将人I类HLA分子A2.1、B7.1、B7.2、ICAM-1、β-2微球蛋白和LFA-3的互补DNA插入到所述pRmHa-3质粒中,其中A2.1可用任何人I类DNA序列取代;
e.用phshneo质粒和所述含互补DNA的pRmHa-3质粒转染所述昆虫细胞;
f.通过使所述昆虫细胞接触CuSO4以诱导转染基因在所述昆虫细胞中表达来制备nnAPC;和
