发明背景
1.发明领域
本发明涉及修复性支架。本发明具体涉及含螺旋形元件的支架以及制造本发明支架的方法。
2.相关领域介绍
支架是植入身体导管内腔中的修复性部件,用来为管壁提供支撑。来自支架的结构性支撑在血管成形手术中尤为重要。一般地,支架被植入于导管系统中,以增强部分封闭、坍缩、弱化或非正常扩张的导管。更一般地,支架可用于任何生理导管,包括例如动脉管、静脉管、胆管、泌尿管、食管、气管支气管树、大脑导管或泌尿生殖系统。支架既可用于人,也可用于动物。
通常有两种类型的支架:自扩张型支架和气囊扩张型支架。自扩张型支架一旦放开就自动扩张,呈撑开、扩张的状态。气囊扩张型支架要用充气气囊导管使它扩张。给气囊充气,可使支架产生塑性变形。气囊扩张型支架可以通过将处于非扩张或皱缩状态的支架设置在充气导管的气囊部分进行植入。皱缩的支架放上去以后,将充气导管插入手术导管壁上的切口,沿着手术导管移动,直到位于手术导管上需要修复的部位。然后顶着手术导管内壁向充气导管充气,使支架扩张。具体地说,支架由于气囊充气而产生塑性变形,所以支架的直径增大并保持在此增大的状态。在某些情况下,植有支架的手术导管在支架扩张后被支架撑大。
Patrick W.Serruys等人所著的《冠状支架手册》(Martin Dunitz,LTD,1998)中介绍的Palmaz-Schatztm支架就是气囊扩张型支架的一个例子,该种支架已经植入在成千上万的患者体中。像其他已知的支架一样,Palmaz-Schatztm支架存在有些缺陷,包括但不限于:(1)支架—手术导管比例的均匀性较低,(2)支架在皱缩状态和张开状态下都相对较硬,(3)其柔顺性有限使得在窄导管中的安置困难。支架—手术导管比例一般是指管壁受扩张状态的支架支撑的程度,较好是在支架的全部长度上都是均匀的。此外,因为Palmaz-Schatztm支架由一个或多个连接若干连续槽型管的桥组成,所以支架扩张后在手术导管中存在有些未受支撑的区域。这些缺点是许多支架常见的,见上述手册,第36页。
发明概述
本发明涉及具有相对均匀的扩张后支架—导管比例和其他需要性能的可扩张支架,以及制造这些支架的方法。本发明的支架包含一个通常呈圆管形的主体,该主体上有许多个可扩张螺旋形片段。该主体由多个圆环形主体元件组成,这些元件通过上述螺旋形片段连接在一起。所述圆环形元件具有与主体的轴在同一条线上的轴。这些圆环形元件由多个周面元件通过可扩张螺旋形片段连接在一起而形成。在有些实施方式中,所述支架包含在主体两端的两个端区。

发明详述
本发明涉及可扩张支架以及制造所述支架的方法。在一种实施方式中,如图1和2所示,支架通常包含圆管形主体部分11,它具有轴5和壁厚103。壁厚103在整个支架中可以都是相同的。主体部分11由多个螺旋形片段30和40以及多个主体圆环形元件100组成,每个元件都有与主体的轴5在同一条线上的轴(未示出)。每个主体圆环形元件100由多个周面元件50组成,周面元件通过螺旋形片段30和40连接在一起形成一个个圆环100。
所述支架也可以具有位于主体部分11两端的第一端区10和第二端区20。在有些实施方式中,如图1所示,端区10和20宜为支架提供正方形外边缘8。所述支架可由不锈钢或其他合适的材料制造。在大多数实施方式中,所用材料或者部分材料宜为辐射不透明的材料,而且形成支架的各个片段是相邻的。不过,在有些实施方式中,组成支架的各个片段可以是连接在一起的独立元件。
如图1和2所示的主体11可以通过许多方法形成。例如,主体11可以包含两个或多个通常相互平行的第一螺旋形片段30和40。在有些实施方式中,它们可以彼此处于180°的相反位置。一般地,第一螺旋形片段30和40在主体11的周边110上等间距隔开。第一螺旋形片段30和40通过多个周面片段50连接起来,形成多个主体圆环形元件100,它们至少大致上呈圆环形。在一种实施方式中,所述周面片段50组成了每个圆环形元件100的大部分周环110。所述螺旋形片段30和40除了将周面元件50连接形成圆环形元件100外,还将每个圆环形元件100与相邻的圆环形元件100连接起来,形成主体11。

第一周面片段50可以是由多个按照一定图案连接在一起的片段形成的可扩张片段。所述图案,如图1-3所示的那种,可以是类似于含弯曲形峰和谷的方形波的重复图案。也可以用其他重复或非重复图案。例如(非限制性),第一周面片段50可以像三角波形状、正弦波形状、其他重复图案,或者其他任何图案,只要当从内部向支架施加径向力时能使片段扩张,或者当从外部施加压缩力时能使片段径向坍缩。
第一周面元件50可以具有丝宽420(见图4)。在一种实施方式中,丝宽可以在0.002英寸和0.007英寸之间变化,但较好是约0.0050英寸。根据支架参数的情况,可以采用其他丝宽。
在图1-5所示实施方式中,第一周面元件50包含直线部分320和曲线部分328,曲线部分328与直线部分320连接在一起形成重复图案。在有些而非所有实施方式中,直线部分320可以与支架的轴平行。第一周面片段50具有幅度350和周期380。在一种实施方式中,所述幅度可以在0.5mm-2.0mm之间,周期可以在0.5mm-2.0mm之间。在有些实施方式中,幅度可以小于周期。根据支架的总体设计和性能要求,可以采用上述范围以外的幅度和周期。
第二周面元件60除了如图6所示以螺旋形图案连接在一起构成一个或多个螺旋形片段30或40外,也可以采用许多其他形式连接。在图6所示实施方式中,第二周面元件60包含具有丝宽407的直线部分412和曲线部分414,并且总体上类似于S形结构。此外,第二周面元件60可以含有弯曲部分417,它同直线部分412在直线部分412与曲线部分414连接端相背的一端相连。弯曲部分的方向可以相对于支架的轴5呈一角度α,范围为0-45度。在至少一个实施方式中,较好的角度α约为10度。在有些实施方式中,第二周面元件60的直线部分412相对于支架的轴成一角度Ω,其较好为0-45度。当像图2那样从平面视角观察时,直线部分412在有些实施方式中可以相对于支架的轴成一角度Ω。在有些实施方式中,Ω可以大致等于第一螺旋形片段30和40的螺旋角。在一种实施方式中,第二周面元件60的幅度300(见图3、4和6)可在0.5mm-2.0mm之间,周期可在0.5mm-2.0mm之间。根据具体的支架所采用的尺寸和设计情况,可以采用其他范围。在一种实施方式中,周期宜为约0.82mm,直线部分412的长度宜为约0.5mm,幅度300宜为约0.38mm。第二周面元件60的幅度可以大于、等于或小于第一周面元件50的幅度。在一种实施方式中,无论就周长还是就周面表面积而言,第一周面元件对整个主体11的贡献大于第二周面元件60的贡献。在一种实施方式中,支架的总外表面积约为0.029平方英寸。
如图7所示,支架可以具有由两个或多个第一螺旋形片段30和40以及两个或多个第二螺旋形片段200和210组成的主体11。第一和第二螺旋形片段30、40和200、210分别相连,构成总体上呈圆管形的主体11。在某些而非所有实施方式中,第一和第二螺旋形片段可以共有一个连接元件250。在有些实施方式中,共有的连接元件250可以呈H形,该H形连接元件250大体相互平行的两个直线部分相对于轴5可以成角度δ(见图6)。在一种实施方式中,δ可以是约14度。如图7所示,第一螺旋形片段30和40与第二螺旋形片段200和210可以具有不同的节距,即单位长度上的螺旋数,这就使得第一和第二螺旋形片段具有不同的螺旋角度(分别为θ和β),即螺旋形片段相对于支架轴5的角度。在一种实施方式中,第二螺旋形片段200和210的节距约为第一螺旋形片段的两倍。在一种实施方式中,θ可以在0-45度之间变化,宜约为40度,β宜约为θ的两倍。在其他实施方式中,角θ可以在0-99度范围内。
如图2、3、4和6所示,螺旋形片段30、40是可以周面扩张的(即支架的圆周表面可以扩张),并且由多个周面元件60构成,所述多个周面元件60依次由直线412和/或曲线414片段(见图6)组成,每个片段具有丝宽407(见图6),小于周面元件60的周长尺寸66(见图3)。在有些实施方式中,每个螺旋形片段30或40对每个圆环形元件100的周长的总贡献大于丝宽407。每个螺旋形片段30或40对支架(图1中110或图11中105)总周长的贡献可以大于片段(如412和414)的丝宽407对周长的贡献,所述片段组成了周面元件60,周面元件60则又组成螺旋形片段。(即在有些实施方式中,螺旋形片段30和40对每个圆环形元件100的周边110的周长贡献不只和丝宽407有关,例如,它也可以和元件60的几何参数有关。)对于图1和图11所示实施方式,当支架处于非扩张状态和扩张状态时,就是这种情况。螺旋形片段30和40的几何参数是确定其可扩张性的一个因素。
类似地,螺旋形片段200、210是可以周面扩张的,可由其他周面元件50组成,而周面元件50则由直线320和/或曲线片段328组成(见图3和图5),它们具有丝宽420(见图4)。螺旋形片段200、210对每个圆环形元件100的总周长尺寸110的贡献大于单个片段320和328的丝宽420的贡献,片段320和328组成元件50,元件50则组成螺旋形片段200、210。组成螺旋形片段200、210的元件50的几何参数是决定螺旋形片段200和210对支架总周长的贡献大于丝宽420的一个重要因素。因此,在本发明的一种实施方式中,处于非扩张状态时的支架的周边110和支架处于扩张状态时的周边105主要和分别组成螺旋形片段30、40和200、210的元件50和60的几何参数有关。
本发明的某些而非所有实施方式可以使用端区10和20(见图1、2和11)。使用端区的支架通常具有位于支架中部的中央区或两端的两个端部区域。支架在端区和中央区之间也可以有个过渡区。过渡区是用来帮助支架移植之后扩张的中部区域和不扩张的支架端部实现平缓过渡。过渡区的尺寸和特点随支架材料和几何参数而不同。例如,在各种影响因素中,过渡区的性质随着第一螺旋形片段的螺旋角、位于两上端区的曲线片段数目和构成两个端区的片段的直线部分的角度ε而变化(例如,见图8)。
端区10和20可呈许多形式。在有些实施方式中,端区可由一个或多个环17组成(见图8)。环17一般呈圆环形,在有些实施方式中呈正圆环形。在一种实施方式中,这些环由直线片段28与曲线片段29按一定图案连接形成。所述图案可以有许多形式,包括图中所示的,较好但不一定是重复图案。端区10和20可以是多个环17相互连接而成。撑杆15可以用来将环连接形成端区,并将端区连接于主体11。在有些实施方式中,这些撑杆起悬臂弹簧的作用,因此其硬度(随其宽度和厚度而异)就会决定支架相对于其轴5的弯曲性能。
在图1、7、8和9所示实施方式中(仅起示例作用),端区10的直线片段28相对于支架的轴成一角度ε。角度ε可以在0-45度范围内,在一种实施方式中宜为约10度。端区的片段具有0.002-0.007英寸之间的丝宽13。在一种实施方式中,端区的重复图案具有约0.027英寸的周期2和约0.043英寸的幅度21,也可以是其他数值。如图1所示,在一种实施方式中,仅作为将端区10和20连接到主体11手段的撑杆15具有0.002英寸至0.08英寸之间的宽度,而且宽度不宜超过壁厚,所述壁厚通常但不一定在约0.002至0.008英寸之间。
本发明支架在插入导管之后,可以使之扩张,从而由非扩张状态塑性变形到扩张状态,直径增加约300%-400%,结果得到增大的周边105(见图11)。图11所示为图1所示支架处于扩张状态的情况。在一个具体实施方式中,支架扩张后,其外径由1.0mm增加为3.00mm,扩张状态下的支架—导管比例平均增大16%。
虽然端区10和20可以用来提供正方形边缘,但不是本发明的所有支架都需要端区。图12-15显示的是无端区的支架。像图1-9所示支架一样,图12-15中的支架包含多个相邻的圆环形元件100。所述圆环形元件100由多个第一周面元件50’和第二周面元件60组成。图12-15中支架的第一周面元件50’基本上与第二周面元件60相同,只是它们旋转呈不同角度。周面元件一般呈S形,含有直线部分412、半径为R的曲线部分414和弯曲部分417。R可依据支架的总体特征在较大范围内不同,在一种实施方式中为0.001至0.02英寸,较好约为0.0083英寸。弯曲部分417与直线部分有一段距离499。在一个具体实施方式中,距离499可以为0.002至0.020英寸,且较好为约0.007英寸。在一种实施方式中,所述元件的丝宽407可以为约0.13mm。
相邻的圆环形元件100可以通过连接每个圆环形元件100中的第一周面元件50’和相邻的圆环形元件100中的第一周面元件50’而连在一起,这样,相邻的两个圆环形元件100中的第一周面元件50’在整个支架中形成螺旋形,同样第二周面元件在整个支架中以相对于轴5成θ角的方向形成螺旋形。在有些实施方式中,利用连接片段250(见图7)而连接相邻圆环形元件100中的第一周面元件,并连接相邻圆环形元件100中的第二周面元件。此外,连接片段将每个圆环形元件100中的第一周面元件50’与每个圆环形元件100中的两个第二周面元件60连接起来。在一种实施方式中,各个圆环形元件100彼此毗邻,相隔一个距离666。在一种实施方式中,较好的间距为0.002至0.020英寸,更好约为0.009英寸。
上面对本发明的描述是说明性的,并未函盖全部情况。在不背离本发明权利要求中所界定的范围和主旨的前提下,可对所述支架作各种修改以改变其总体特征。举一个非限定性例子,增加直线片段的长度,而且(或者)增加第二周面元件60的弧度可以减少使每个环节扩张所需的径向力,并增加其柔顺性。增加第二周面元件60的角度Ω可以:(1)增加扩张所需的径向力,(2)增加表面积,(3)减少柔顺性。与此类似,可对撑杆15作各种修改(见图2)。增加撑杆宽度和壁厚会:(1)增加表面积,(2)增加径向强度,(3)增加使支架径向扩张所需的压力,(4)减少柔顺性,此外在增加壁厚的情况下,(5)增加辐射不透明度。
本发明支架可以用许多方法制造。可以由一根金属管子除去其管壁上的各个部分形成本发明所描述的图案,来形成支架。因此,得到的支架可由一段整体的材料形成,省得将各段连接在一起。管壁上的材料可用各种技术除去,包括激光(例如YAG激光)、放电、化学蚀刻、金属切割、这些技术的组合或其他已知技术。例如,可参见Moriuchi等人的美国专利第5879381号和Becker的美国专利第6117165号,它们完全参考结合于此。用这种方式形成支架,可以得到基本上没有应力的结构,其中的螺旋形片段与周面元件形成整体。在一种实施方式中,形成支架的管子内径约3.0mm,壁厚约1.0mm,长度约30mm。其他尺寸的管子也可以采用。具体言之,可以调整长度使之适合要放置支架的内腔的病变部分。这样就以避免对整个病变区域使用几个支架。
本领域的熟练技术人员应该理解,上述支架和其制造方法是说明性的,并未函盖本发明的全部情况,在不背离下面的权利要求中所界定的发明范围和主旨的情况下,可以进行修改和变通。
Claims (42)
1.包含一个主体的可扩张支架,其特征在于,当支架未扩张时,所述主体包含:
多个可扩张的螺旋片段;
多个共轴的主体圆环形元件,所述主体圆环形元件相互毗邻且通过螺旋形片段相互连接,每个主体圆环形元件包含:
一个与相邻圆环形元件基本上相同的周面;
多个通过螺旋形片段的部分连接在一起的周面片段,由此形成圆环形元件,所述多个周面片段构成每个圆环形元件的大部分周面。
2.如权利要求1所述支架,其特征在于所述周面片段由多个片段按重复图案连接起来组成。
3.如权利要求1所述支架,其特征在于所述重复图案是方波形式,所述方波形式含有曲线形的峰和谷。
4.如权利要求1所述支架,其特征在于它还包含第一和第二端区,其中第一和第二端区位于在支架主体的两端。
5.如权利要求1所述支架,其特征在于它还包含多个将各端区连接到主体上的撑杆。
6.如权利要求5所述支架,其特征在于所述每个端区由多个环组成。
7.如权利要求6所述支架,其特征在于每个端区中的环由多个撑杆连在一起。
8.如权利要求7所述支架,其特征在于所述环由多个交替的直线和曲线片段组成。
9.如权利要求8所述支架,其特征在于所述直线片段相对于圆环形元件的轴呈一个大于0°的角。
10.可扩张支架,包含
第一非螺旋形端区;
第二非螺旋形端区;
具有轴且大体上呈圆柱形的主体,位于第一和第二端区之间,它包含:
多个相邻圆环形主体元件,其轴与主体的轴在同一条线上,相邻的圆环形主体元件连接在一起,且包含:
多个具有一定环形尺寸的第一可扩张周面片段;
多个具有一定环形尺寸的第二可扩张周面片段,其尺寸小于第一可扩张周面片段的环形尺寸,每个第二可扩张片段与两个第一可扩张片段相连;
其中,圆环形主体元件通过将相邻的圆环形主体元件的第二可扩张周面片段按螺旋形图案连接在一起而相连,由此在主体中形成多个大体平行的螺旋。
11.如权利要求10所述支架,其特征在于支架的至少一部分是辐射不透明的。
12.如权利要求10所述支架,其特征在于每个端区通过多个撑杆与主体相连。
13.如权利要求10所述支架,其特征在于支架由已经邻接成一段整体的材料制造。
14.如权利要求10所述支架,其特征在于支架还包含在主体中的两个螺旋形片段,其中螺旋形片段错开180°。
15.如权利要求10所述支架,其特征在于每个端区包含一个由多个邻接片段形成的环。
16.如权利要求15所述支架,其特征在于所述邻接片段包含直线和曲线片段,且直线和曲线片段连接在一起,形成重复图案。
17.如权利要求16所述支架,其特征在于所述第一可扩张周面元件包含多个连接在一起的直线和曲线片段,以形成重复图案,这种重复图案大体上像具有曲线形峰和谷的方波形式。
18.如权利要求17所述支架,其特征在于所述第二可扩张元件包含多个连接在一起的直线和曲线片段,其中直线片段相对于支架的轴形成一个角度,该角度大约等于主体中至少一个螺旋的螺旋角。
19.可扩张支架,当其处于非扩张状态时,包含:
一个轴;
围绕所述轴的圆管形主体,所述主体包含:
多个具有第一节距的第一可扩张螺旋片段;
多个具有不同于第一节距的第二节距的第二可扩张螺旋形片段,至少一个第一螺旋形片段横穿至少一个第二螺旋形片段。
20.如权利要求19所述支架,其特征在于第二节距的值约为第一节距的两倍。
21.如权利要求19所述支架,其特征在于它还包含多个大体上呈圆环形的端区,它们的轴与主体的轴在同一条线上。
22.如权利要求21所述支架,其特征在于所述端区具有正方形外边缘。
23.如权利要求20所述支架,其特征在于所述第一螺旋形片段由多个连接在一起形成螺旋的周面片段组成。
24.如权利要求23所述支架,其特征在于所述周面片段包含多个连在一起的直线和曲线片段。
25.如权利要求24所述支架,其特征在于所述直线片段与支架的轴呈0-45度角。
26.如权利要求23所述支架,其特征在于所述第二螺旋形片段由多个连接在一起形成第二螺旋的第二周面元件组成。
27.如权利要求23所述支架,其特征在于所述支架是辐射不透明的。
28.由具有第一端和第二端的管状元件制造支架的方法,所述方法包含如下步骤:
从靠近第一端的第一区域上除去材料,形成第一端区;
从靠近第二端的第二区域上除去材料,形成第二端区;
从第一和第二区域之间的中间区域上除去材料,形成主体,从中间区域除去材料后,在中间区域形成至少两个大体平行的可扩张螺旋形片段和多个周面片段,所述周面片段与可扩张螺旋形片段相邻,形成多个主体圆环形元件,它们通过可扩张螺旋形片段连接在一起,由此形成主体。
29.可扩张支架,包含:
多个第一可扩张片段;
多个第二可扩张片段;
多个共轴的相邻圆环形主体元件,所述圆环形主体元件通过连接第一可扩张片段和第二可扩张片段形成;
多个具有一定节距的第一螺旋形片段,第一螺旋形片段通过将相邻圆环形元件中的第一可扩张片段相互连接起来而形成;
多个第二螺旋形片段,它通过将相邻圆环形元件中的第二可扩张片段相互连接起来而形成,其中第二螺旋的节距不同于第一螺旋片段的节距。
30.如权利要求29所述支架,其特征在于第一和第二可扩张片段通过连接片段连接在一起。
31.如权利要求30所述支架,其特征在于所述连接片段是H形片段。
32.如权利要求29所述支架,其特征在于它还包含第一和第二端区,所述第一和第二端区在主体的两端。
33.如权利要求29所述支架,其特征在于至少一个螺旋形片段相对于支架的轴呈约40°的角。
34.如权利要求29所述支架,其特征在于第一和第二可扩张片段具有基本上相同的形状,但取向不同。
35.如权利要求34所述支架,其特征在于第一和第二可扩张片段包含通过曲线片段连接的直线片段,其中第一和第二可扩张片段的直线片段与轴呈斜角。
36.如权利要求34所述支架,其特征在于所述斜角具有相同的绝对值。
37.如权利要求36所述支架,其特征在于所述斜角具有约16°的绝对值。
38.由具有第一和第二末端部分及中间部分的管子制造支架的方法,所述方法包括:
从中间部分上除去材料形成一组第一可扩张螺旋形片段,第一组中的每个第一可扩张片段具有第一节距;
从中间部分上除去材料形成一组第二可扩张螺旋形片段,第二组中的每个第二可扩张片段具有不同于第一节距的第二节距。
39.如权利要求38所述方法,其特征在于除去材料形成第一可扩张螺旋形片段的步骤得到的是呈重复图案的第一螺旋形片段。
40.如权利要求39所述方法,其特征在于除去材料形成第二可扩张螺旋形片段的步骤得到的是呈不同于第一螺旋形片段的重复图案的第二螺旋形片段。
41.如权利要求38所述方法,其特征在于它还包括:
从第一末端部分上除去材料形成可在周面上扩张的第一端区;
从第二末端部分上除去材料形成可在周面上扩张的第二端区。
42.如权利要求41所述方法,其特征在于它还包括除去与第一和第二端区相邻的材料,形成多个处于两个端区和中间部分之间的撑杆。
