背景
1.技术领域
本发明总体上涉及支架,具体来说涉及具有带特殊曲率半径的节点的支架。
2.背景技术
支架被广泛地用于多种支架被放置在患者管腔内然后膨胀的应用场合。所述支架可被用于冠脉或其他脉管系统,以及其他体腔。

发明内容

当支架100被收缩到导管内时(即,它的展开前构型),支架100的直径相比支架100还没有被装载到导管内时(即,它的加载前构型)缩小。通过导致弯曲应变增加(例如,尤其是相邻节点)的段间夹角的减小来适应支架周长上的相应缩小。随着单元的变形(例如,当支架100被装载到导管内后,或者当输送系统正被操纵通过脉管系统到达治疗部位)、尤其在弯曲或轴向装载期间,相邻段之间的某些夹角比其他夹角增大或减小更多,原因在于在最柔性的支架设计中天然地缺少对称性。因此,在连接所述相邻段的节点中存在更大的应变。能通过例如有限元分析计算集中在节点附近的应变量。
在已变形的支架中的最大应变被称为峰值应变。峰值应变通常发生在节点附近的单个段上,但是依据支架设计也可能发生在支架上的其他位置。支架上的主要应变可以是拉伸(一般被定义为正的)或压缩(一般被定义为负的)。应变被分为两类:法向应变和剪切应变。法向应变是正的(例如,拉伸)或负的(例如,压缩)。也存在正或负的剪切应变,但是在所述情况下正负约定是随意的,并且在物理上正负剪切应变之间没有实质区别。法向应变(也被称为主应变)基本上是支架应变分析和耐久性分析的基础。
高拉伸应变可能导致裂缝出现并经支架传播,从而导致疲劳寿命减少和支架破裂(即,失效模式)。压缩应变不倾向于引起裂缝,所以一般不会引起支架寿命的减少,除非该应变是超常规量级的。已变形支架的某些部分在使用期间可能发生高度应变,而其他部分可能根本没有应变。已变形支架被认为是微小体积区域的集合体,每个区域都具有一个应变水平;这些区域的应变水平共同地在一个最大负值到一个最大正值的范围内。对于在身体内工作的支架而言,假设支架材料经过正确的材料处理和表面修整,如果支架应变被维持在支架材料的耐受极限以下,那么高疲劳寿命是可期的。但是,如果在身体内工作的支架承受高于支架材料的耐受极限的支架应变,那么无论怎样的支架材料处理和表面修整,高疲劳寿命都是不可期的。
通常,支架被设计成使得:支架上的应变在脉动的负载状况下(即在振荡的周向压缩应变下)保持低水平。但是,已经确定的是,在其他位置被植入的支架(例如在患者关节处或附近的患者脉管系统内)承受比之前所预测的数量更大的应变。
支架100的弯曲和任何相应的伸长导致拉伸应变在特定节点处或节点内集中,从而使节点附近的段以及节点本身面临更低的疲劳寿命。在其他节点上的压缩应变的集中允许这些节点附近的段维持较高的疲劳寿命。因此,平衡支架100所承受的应变提高了支架100的总体疲劳寿命。应变平衡可涉及改变支架100的设计,以使得没有暴露给高水平拉伸应变的节点具有分担应变的途径,进而有助于提高特定节点和相邻段的疲劳寿命,因此有助于提高整个支架100的疲劳寿命。
本发明的支架100有助于通过设置具有彼此不同的曲率半径的节点来平衡应变。由于应变被集中在曲率半径最小的区域,所以通过增大该半径,峰值应变被降低。每个单元的承受较大应变的节点具有与之相关的较大半径。因此,具有较大曲率半径的节点具有与之相关的较低数量的峰值应变,由此提高了疲劳寿命(即,有助于防止支架破裂)。另外,支架100的具有较大曲率半径的特定节点是在各种测试程序中被发现承受最高峰值应变的节点。所以,提高了支架的总体寿命和性能。
但是,虽然改变不同节点的半径可以改善支架设计的某些方面,但是这种半径改变可能降低支架的至少一部分的强度。更具体来说,为了能将支架100装配到穿过患者脉管系统的输送系统内,支架100被收缩到相对小的直径。一般地,当节点半径增加和段宽度保持不变时,最小的收缩直径与半径的增加成比例地增加。如果所要求的收缩直径理想地是一个固定值,那么随着节点半径被增大,段宽度将不得不缩小。段宽度的所述缩小可引起支架强度的下降。因此,对于每种支架设计和所需的应用而言,可以调整总体支架设计,以保证耐久性收益超过由于增大节点半径而导致的强度损失或者收缩直径的增加。举个非限制性示例,对于静脉支架(与动脉支架比较)来说,输送系统的直径可能不那么关键,原因有两个:1)静脉的直径一般大于动脉;2)静脉中的血压低于动脉,因此,允许输送装置进入静脉的入口孔的尺寸不是很关键,原因在于一旦手术完成,制止血从静脉上的孔流出比动脉容易。因此,通过考虑支架的所需强度、所需耐久性、以及支架收缩直径的所需尺寸、以及这些要求之间的相互影响,能够针对静脉应用来调整支架的设计。
还能预期的是,支架100除了具有曲率半径彼此不同的多个节点以外,支架100的至少一个节点包括非恒定曲率半径。在某些情况下,具有不同曲率半径节点和非恒定曲率半径节点的组合的支架可以进一步提高作用力的平衡分布。在与本申请同时提交的美国专利申请No.______(代理案号No.H-KN-02622(1847-141))中公开了具有非恒定曲率半径节点的支架的更多细节,其全部内容以提及的方式纳入本文。
能预期的是,每个单元120和整个支架100具有与在患者脉管系统内使用的预期目的相称的任意合理尺寸。同样地,单元120的总数是任意合理的值。此外,如图4所示,例如,连接器构件180可以被包含在支架100上,从而将支架100临时固定到输送装置。
在所公开的实施例中,加载前支架100(即,插入导管之前)的内径在大约0.5000英寸到大约0.6000英寸之间,展开前支架100(即,被收缩在导管内)的内径在大约0.0500英寸到大约0.0600英寸之间。
在使用中,支架100被推进到体腔内的某个部位。然后支架100在该部位扩张。可以通过任何常规手段来膨胀支架100。例如,支架100可以被放置在导管的球囊末端上。此时,球囊在该部位膨胀,从而在支架100的内部产生径向力。径向力迫使支架100在例如没有明显的纵向膨胀或收缩的情况下径向地膨胀。支架100的材料(例如,不锈钢)的塑性变形使支架100在接下来的球囊放气之后仍保持膨胀后的形状。替代地,支架100可以由超弹性或形状记忆材料(例如,镍钛诺)形成。
可以有很多的改型方案。例如,支架100可以被加衬一个内套管或外套管(比如聚酯织物或ePTFE)以促进组织生长。支架100的至少一部分也可以被涂覆不透辐射的涂层,比如铂、金、钨、或钽。除了之前所讨论的材料,支架100可以由其他材料形成,包括但不限于MP35N、钽、铂、金、埃尔基洛伊耐蚀游丝合金和Phynox。
尽管在图4中示出了三个单元120沿支架100的直径周向连接,但是设计者也可以选择其他的数量被如此连接以改变支架100的特性。同样地,尽管图4中每一列单元120被示出具有三个周向连接单元120,但是周向连接单元120的数量能变化以调整支架100的特性。
当支架100由形状记忆材料(比如镍钛诺)形成时,支架100能从镍钛诺管上被激光切割下来。然后,支架100经历定形过程,其中切割下的管在心轴上膨胀并被加热。多个膨胀和加热循环用于将支架100定形为最终膨胀直径。能预期的是,最终膨胀直径等于支架100的所需展开直径。在膨胀期间,能预期的是支架100受到轴向约束,从而支架100的长度在膨胀期间不变。还能预期的是,最终的支架100具有低于体温的奥氏体结束温度。此时,由于金属所制支架100的形状记忆特性,在体温下支架100将自膨胀到所需的展开直径。
在使用中,支架100能被安装在输送导管上。如现有技术中已知的那样,支架100在输送导管上被可缩回护套保持在压缩构型下。如现有技术中已知的那样,输送导管被用于将支架100推进到展开位置(例如,血管的收缩区域)。在展开位置,缩回护套,从而释放出支架100。释放后,支架100自膨胀到展开直径。尽管附图中公开的特征的预期用途涉及到自膨胀支架的使用,但是所述特征配合非自膨胀支架(例如由不锈钢材料制成的球囊可膨胀支架)被使用时也是有益的。
通过前文和对各种附图的参见,本领域技术人员将明白在不脱离本发明范围的前提下还能对本发明做出某些改型。这不表示本发明被限制为附图中所示的具体实施例,因为本发明的范围像本领域技术所允许的那样宽泛并且说明书被同样地理解。所以,以上描述不构成限制,而仅是具体实施例的示范。本领域技术人员在后附权利要求的范围和实质内将预期到其他改型。
Claims (17)
1.一种支架,包括:
限定了长度并包括多个单元的支架体;
每个单元包括两个以波状图案延伸的结构构件;以及
每个结构构件包括多个单元段,所述多个单元段限定了它们之间的多个节点;
其中,第一节点的曲率半径不同于第二节点的曲率半径。
2.根据权利要求1所述的支架,其中,第一节点的曲率半径比第二节点的曲率半径大大约200%到大约700%。
3.根据权利要求1所述的支架,其中,第一节点的曲率半径比第二节点的曲率半径大大约500%到大约600%。
4.根据权利要求1所述的支架,其中,第一节点的曲率半径比第二节点的曲率半径大大约250%。
5.根据权利要求1所述的支架,其中,第一节点的曲率半径在大约0.0030英寸到大约0.0040英寸之间。
