本发明通常涉及使用天然人子宫珠蛋白(hUG)或重组人子宫珠蛋白(rhUG)对炎症和纤维变性疾病的治疗。已证实了UG(hUG或rhUG)的新的生理作用和疗法。具体地说,本发明涉及通过给药hUG或rhUG抑制PLA2和/或预防纤连蛋白沉积来治疗炎症和纤维变性疾病。本发明进一步提供一种治疗为严重的肺临床疾病的新生儿呼吸窘迫综合征(RDS)和支气管肺发育不良(BPD)以及为一种肾病的肾小球肾病,这两种疾病的特征均在于炎症和纤维变性疾病。
本申请所引用的参考文献涉及本发明所涉及的现有技术的状况,其均被本文引作参考。
发明背景炎症和纤维变性疾病
近些年来,对改进的用于治疗炎症和纤维变性疾病的治疗剂的研究已受到广泛的注意。为肺表面活性剂缺乏疾病的新生儿RDS为尤其令人感兴趣的一种疾病,因为它是早期新生儿死亡的一个主要原因。虽然引入表面活性剂的治疗显著地增加了RDS病人的存活率,但这类病人人群中有很大比例的人出现慢性炎症和纤维变性疾病是一个主要问题。同样,当病人肾被阻塞并不再过滤血液时,遗传性纤连蛋白沉积肾小球肾病导致晚期肾衰竭。肾病的特征在于纤连蛋白沉积和肾的纤维化,这使得器官丧失功能并最终不能维持生命。
PLA2(磷脂酶A2),一类水解甘油磷脂Sn2位置酯键的内源性酶,是涉及炎症和纤维变性疾病的许多蛋白中的一种。它还负责肺内表面活性剂磷脂的水解。子宫珠蛋白(还称作CC10、CC16、CC17、尿蛋白-1、P-1、孕酮结合蛋白、PCB-结合蛋白、克拉拉细胞分泌蛋白(CCSP)、胚动素、视黄醛结合蛋白、磷脂结合蛋白和α2微珠蛋白)在体外抑制PLA2的活性。
子宫珠蛋白为小的球状同型二聚体蛋白。它具有15.8kDa的分子量,但它在凝胶电泳中以相当于10kDa的大小迁移。人子宫珠蛋白在成人肺中大量存在,占全部可溶性蛋白的约7%。然而,它的表达在发育的人胎儿中直至妊娠后期才完全激活。结果,早产婴儿的细胞外肺液包含比成人肺液少得多的UG。UG还由胰腺表达。
由于它们从细胞磷脂库中释放花生四烯酸(AA),因此PLA2在炎症反应中起着关键的作用。AA在称为花生四烯酸级联的过程中被代谢为大量强效炎症介体。
通过升高的血清或局部PLA2活性已确定一些急性和慢性临床疾病(参见下面表1)。

目前没有有效的可用于临床应用的PLA2抑制剂。至今,仅一些PLA2抑制剂已进入临床试验,但均未获准用于商业销售。
纤连蛋白(Fn)为以一些不同形式存在并由不同组织分泌的200kDa糖蛋白。Fn为一种必需的蛋白,并且在小鼠中Fn基因的靶定破坏表明它在胚胎发生中具有关键作用。Fn还在炎症、细胞粘附、组织修复和纤维变性中起着关键的作用,并在损伤部位沉积。血浆纤连蛋白(pFn)由肝脏分泌并在血浆中循环。在肺中,细胞Fn(cFn)在炎症和损伤时分泌。两种类型的Fn是炎症细胞和成纤维细胞的趋化因子。炎症发生期间,大量的炎症细胞和成纤维细胞渗入肺,这可导致肺纤维变性和最终死亡。在人临床疾病,如新生儿RDS和BPD等肺部疾病以及肾小球肾炎等肾脏疾病中已检测到升高水平的Fn。UG的作用
纯化的人UG的氨基酸分析表明它与其它“UG样”蛋白,如兔UG的结构类似但不相同。人与兔UG之间70个氨基酸中39个相同(参见图1)。“UG样”蛋白,包括人UG/CC10、大鼠CC10、小鼠CC10和兔UG显示种特异性和组织特异性抗原差异以及它们的组织分布和体外生化活性的不同。根据组织和来源物种,在许多不同的文章中描述了UG样蛋白,包括大鼠肺、人尿、痰、血液成分、兔子宫、大鼠和人前列腺和人肺。目前,仍未知这些蛋白的生理作用。
尽管多年的研究,这些蛋白在体内的生物学作用仍然不清楚。UG样蛋白之间缺乏结构的相同性使得不可能根据某蛋白在体外或其它由结构相关蛋白显示的活性来预测其是否在人体内将具有治疗功效。例如,兔UG与5%的孕酮结合,而在相同试验中,人子宫珠蛋白结合的孕酮的量小于5%。人UG具有比兔UG(5.4)低的等电点(4.6)。
Stripp等(1996)报道了关于被制备用来消除子宫珠蛋白的表达的子宫珠蛋白剔除小鼠的研究。该小鼠具有克拉拉细胞,其中该细胞显示异常的细胞内结构来代替子宫珠蛋白分泌颗粒,但没有其它表型。这种观察结果极其重要,因为预测了伴随有肺炎症和纤维变性的肺功能。而且,这种剔除小鼠没有显示肾、胰腺或生殖异常,这表明子宫珠蛋白在体内控制炎症或纤维变性中不具有显著的作用。
发明目的
因此,本发明的一个目的是提供包含PLA2抑制有效量的重组人子宫珠蛋白(rhUG)或其片段或衍生物以及可药用载体或稀释剂的药物组合物。
本发明的另一个目的是提供包含纤连蛋白结合有效量的重组人子宫珠蛋白或其片段或衍生物以及可药用载体或稀释剂的药物组合物。
本发明的另一个目的是提供包含PLA2抑制或纤连蛋白结合有效量的用于治疗靶适应症的活性剂的药物组合物。
而且,本发明的一个目的是提供一种在需要这种治疗的哺乳动物体内抑制PLA2酶的方法,其中该方法包括对哺乳动物给药PLA2抑制有效量的天然或重组人子宫珠蛋白或其片段或衍生物。
进一步,本发明的一个目的是提供一种在需要这种治疗的病人中治疗或预防炎症疾病的方法,其中该方法包括给药抗炎有效量的天然或重组人子宫珠蛋白或其片段或衍生物。
本发明的一个目的是提供一种在需要这种治疗的病人中治疗或预防纤维变性疾病的方法,其中该方法包括给药纤连蛋白结合有效量的天然或重组人子宫珠蛋白或其片段或衍生物。
本发明的另一个目的是提供一种治疗或预防特征在于内源UG缺乏的炎症或纤维变性疾病的方法,其中该方法包括给药补偿量的天然或重组人子宫珠蛋白或其片段或衍生物。
本发明的另一个目的是提供一种包含PLA2抑制或纤连蛋白结合有效量的天然或重组人子宫珠蛋白或其片段或衍生物的化妆品组合物。
另外,本发明的一个目的是提供一种包含PLA2抑制或纤连蛋白结合有效量的天然或重组人子宫珠蛋白或其片段或衍生物的血液补充物。
最后,本发明的一个目的是提供临床样品中子宫珠蛋白纤连蛋白复合物的定量分析法。
发明概述
现已发现子宫珠蛋白在体内抑制PLA2和预防纤连蛋白沉积和纤维变性中起关键的生理作用。在新品系的转基因子宫珠蛋白“剔除”小鼠和涉及肺炎症和纤维变性的新生儿呼吸窘迫综合征(RDS)的猴模型中进行的联合实验中证实了这些作用。本发明的子宫珠蛋白剔除小鼠(下文称“UG KO小鼠”)在早期和晚期发作疾病时分别显示致死的肾小球肾病和肾实质纤维变性。对正常的小鼠给药外源Fn导致Fn在肾中的沉积,但给药等摩尔量的Fn和rhUG未导致沉积。
已证实在存在rhUG时PLA2在体内活性降低。在第一个实验中,UG KO小鼠的表型表明与具有功能性UG基因的同窝幼畜中的PLA2活性相比,在不存在UG时,血清PLA2活性显著升高。在第二个实验中,对患有RDS的早产猴给药rhUG显示在肺的细胞外液中抑制PLA2活性。
其它实验证实在体外PLA2可降解用于治疗RDS的人工表面活性剂(通常为Survanta),并且UG可抑制这种降解。这些实验证实在气管内或静脉内给药后,在体内UG介导PLA2抑制和Fn沉积。
使用子宫珠蛋白剔除小鼠的实验证实rhUG可用来治疗其中子宫珠蛋白被发现缺乏或蛋白本身具有导致功能丧失的突变的疾病。现已发现rhUG可用来治疗或预防其中在循环中或炎症或纤维变性位点功能性内源子宫珠蛋白缺乏的炎症或纤维变性疾病。在一些肺炎症或纤维变性疾病中发现血清和/或支气管一肺泡渗出液中的hUG水平降低,包括处于发展成新生儿BPD的危险中的早产婴儿。发现UG可用来补充缺乏或缺陷性内源子宫珠蛋白以预防或治疗这些炎症和纤维变性疾病。
根据一个方面,本发明提供一种体内治疗炎症疾病的方法,包括对需要这种治疗的病人给药抗炎有效量的UG。
根据另一个方面,本发明提供一种体内抑制可溶性PLA2酶的方法,包括对需要这种治疗的病人给药PLA2抑制有效量的UG。
根据另一个方面,本发明提供一种治疗或预防纤维变性疾病的方法,它包括对需要这种治疗的病人给药纤连蛋白结合有效量的UG。
本发明的另一个方面提供一种通过加入纤连蛋白结合量的UG治疗或预防原纤维形成的方法。
根据另一个方面,本发明提供一种治疗或预防特征在于内源功能性UG缺乏的炎症或纤维变性疾病的方法,它包括对需要这种治疗的病人给药补偿量的UG。
本发明还提供包含有效量的rhUG与可药用载体或稀释剂的药物组合物。该组合物可为可注射溶液剂的形式和用于气管内给药的液体或半气雾剂的形式。
根据另一个方面,本发明提供包含UG和肺表面活性剂(例如Survanta(来自Abbott实验室的牛肺提取物)和Exosurf(来自Glaxo-Wellcome的化学合成肺表面活性剂))以及可药用的载体或稀释剂的药物组合物。
本发明还包括包含PLA2抑制或纤连蛋白结合有效量的rhGU、治疗靶适应症的活性剂和载体的药物组合物。PLA2抑制或纤连蛋白结合有效量的rhUG减少炎症,并从而确保有效量的活性剂到达治疗部位。
另一个方面,本发明提供定量临床样品中子宫珠蛋白纤连蛋白复合物的分析法,其中将怀疑包含子宫珠蛋白-纤连蛋白复合物的临床样品与抗原捕获剂接触,例如固定在不溶载体上的单特异性的兔多克隆抗体;向样品中加入抗原检测剂,例如纤连蛋白特异性抗体;使用例如第二抗体通过标准酶反应检测与载体结合的任何复合物的存在,第二抗体例如为与诸如辣根过氧化物酶缀合的抗-IgG抗体,在酶反应中酶底物被转化为可使用标准分光光度或荧光设备定量的产色或荧光化合物。
本发明还提供包含PLA2抑制和/或纤连蛋白结合有效量的人子宫珠蛋白和可药用的载体或稀释剂的化妆品组合物和血液补充物。

优选实施方案的详细描述rhUG
本发明的rhUG基本上具有与天然人UG蛋白相同的氨基酸序列。具有与天然人蛋白“基本上相同的”氨基酸序列的氨基酸序列包括至少75%与天然人蛋白相同的rhUG。在一个优选的具体实施方案中,rhGU具有至少85%的同一性,在一个最优选的具体实施方案中,rhGU与天然UG具有至少98%的同一性。
还包括在本发明方法中的是UG片段或衍生物的使用。UG的“片段”指具有天然蛋白序列的六个或更多个连续氨基酸的天然hUG氨基酸序列的一部分。术语“衍生物”指UG的肽类似物,包括一个或多个氨基酸取代和/或一个或多个化学部分的加入,例如酰化剂或磺化剂,前提是衍生物保留母体分子的生物学活性。
而且,用于本发明方法中的UG基本上为纯的。术语“基本上为纯的”指具有约75%至约100%纯度的UG。在一个优选的具体实施方案中,UG具有约90%至约100%的纯度,在一个最优选的具体实施方案中,UG具有至少95%的纯度。UG的临床用途
另一方面,本发明提供一种用于治疗或预防炎症或纤维变性疾病的方法,包括对可能为动物或人的哺乳动物给药有效量的UG。

在人炎症/纤维变性疾病中UG缺乏、PLA2活性和纤连蛋白聚集与沉积之间的典型的相互关系总结在下面。新生儿支气管-肺发育异常(新生儿BPD)
新生儿BPD特征在于新生婴儿中肺组织的严重炎症和不可逆的纤维变性,这通常是呼吸窘迫综合征(RDS)的结果。然而,该疾病还可能由胎粪吸入综合征或感染导致。
由于肺hUG的合成可与表面活性剂共调节,并在妊娠后期开始,故该疾病与hUG缺乏相关。因此,严重早产的新生儿可能缺乏UG以及表面活性剂。hUG的缺乏可能导致与在新生儿BPD中见到的炎症和纤维变性相关的增加的PLA2活性和Fn相关的纤维变性。一些婴儿对合成的表面活性剂不反应,这可能是由于过量的PLA2活性。因此,UG可用来治疗新生儿BPD。
优选的给药途径为借助于气管内或全身性途径直接滴注。多器官功能衰竭(MOF)
过量的PLA2活性与由于细菌脓毒症或外伤的MOF有关。该疾病的特征在于全身性的炎症反应,包括迅速大量的组织损伤以及肺、肾、胰腺、肠和脉管系统器官功能的丧失。最近的证据指出MOF的启动为升高的全身性可溶的磷脂酶A2活性、其对组织细胞膜的直接裂解以及对如肺表面活性剂的必需磷脂的水解。在临床实验中直接抑制PLA2的努力已经成功。
在MOF中,内源UG的量不足以对抗PLA2的超活化。外源施加的UG可用来治疗MOF。
远器官功能衰竭(ROF)包括对不是首先被外伤或感染影响的器官的器官的损伤。通常远器官功能衰竭涉及多于一种的远器官,导致多器官功能衰竭。例如胰腺炎是对醇摄取、感染或外伤反应的胰腺炎症,它可能导致成人呼吸窘迫综合征(ARDS),急性肾衰竭(ARF)和全身性休克。炎性肠疾病或腹膜炎的发作可导致ROF/MOF。ROF/MOF与高水平的循环活化的PLA2相关。全身性给药hUG可防止ROF/MOF。在患有ROF/MOF的病人中立即注射UG可降低严重性或消除PLA2介导的器官衰竭和休克。胰腺炎
所有形式的胰腺炎都涉及全身性和局部升高的Ⅰ型可溶性PLA2活性。胰腺炎通常导致特征在于肺中升高的可溶性PLA2活性的肺机能不全或ARDS。因此,作为体内可溶性Ⅰ型PLA2抑制剂,UG为治疗两种形式的急性胰腺炎的极好的代表,并且作为所有急性形式的胰腺炎中肺机能不全的预防性措施。
优选的给药途径为静脉内途径。炎性肠疾病
炎性肠疾病(IBD),包括溃疡性结肠炎、直肠炎和节段性回肠炎,特征在于Ⅱ型可溶性PLA2的增加的局部产生和活性。在IBD中循环的可溶性PLA2活性也可能升高。作为增加的PLA2活性的后果,IBD在严重的情况下导致肺机能不全或ARDS(这类似于胰腺炎)。
在IBD中给药外源UG的合理性与胰腺炎相同:通过抑制PLA2、Fn聚集和/或沉积下调炎症反应和防止牵连远器官(肺和肾)。
优选的给药途径为在住院病人中通过静脉内途径。细菌性肺炎
患过细菌肺炎的病人的BAL液显示具有比死于该病者高2-3X水平的UG。肺的细菌性感染可能过度活化内源可溶性PLA2。可给药UG以抑制或控制这种作用。
如果病人插管,优选的给药途径为借助于气管内途径,如果不是,则通过静脉内注射。透析并发症
透析主要的并发症为血栓形成,即自发形成血块。在病人中,这通常阻塞血管经过部分,破坏治疗以及导致局部缺血,有时危及生命。血液透析的第二个问题是邻近静脉的炎症和/或纤维变性,它将透析的血液返回至病人主循环中。邻近静脉的纤维变性通常以对透析血液的返回的耐受性和压力的增加来检测。第三个问题是血管经过部位的纤维变性以及关闭或瘘管。第四个问题是加快的动脉硬化,第五个问题是剩余肾功能的丧失,这主要是由于Fn的沉积。
在此过程中内源性UG被透析掉的可能性提供了对这些问题的一个解释。选择性地除去内源UG使得循环的Fn游离,聚集形成血凝块形成的病灶,或沉积于红细胞上,激发它们通过相互粘附或粘附于血管腔中形成血凝反应。转谷氨酰胺酶(TGs)为负责建造在基膜、皮肤和血块中发现的大分子网格的酶。在不存在竞争作为活化TGs的底物的游离UG时,Fn和其它血块成分发生交联。
邻近静脉和血管到达部位的炎症和纤维变性以及加速的动脉粥样硬化可通过Fn在血管腔的沉积来解释。纤连蛋白在血管内皮的沉积促进血小板和白细胞的粘附,在不存在PLA2抑制时它们可被恶化。Fn的血管沉积还可促进在动脉粥样硬化斑块中发现的脂肪、胆固醇和蛋白的局部沉积。纤连蛋白已知为动脉粥样硬化斑块以及与肾病和初级和剩余肾功能丧失相关的肾小球沉积的主要成分。因此,通过减少炎症和纤连蛋白的沉积,UG的给药可减少或消除这些问题。
给药UG的优选途径为在透析前、期间和之后静脉灌注。
另一种方法,内源性UG的丧失可通过向透析缓冲液中加入UG或用UG预覆盖透析膜或这两种方法来预防。器官移植
术语“器官”指例如实体器官,如肾、肝脏和心脏以及骨髓、角膜和皮肤。
有两种类型的器官移植排斥:急性的和慢性的。急性排斥为涉及PLA2活性和被通常损伤移植物的炎症细胞浸润的炎症过程。
慢性排斥涉及Fn介导的移植物的纤维变性,包括局限于移植物的动脉粥样硬化。因此,给药UG可用来治疗或预防急性和慢性移植排斥。
优选的给药途径为通过注射。
器官移植的另一个方面为在从供体移出之前、移植期间和在受体中时的器官的缺血,这导致急性排斥。已知缺血导致升高的PLA2活性和组织坏死。因此,UG可用来预防此缺血。UG的优选形式为灌注液体或储存缓冲液,其中来自体内的器官保存在此。Ⅰ型糖尿病的预防
Ⅰ型糖尿病来自通过自身免疫反应对胰腺组织的损伤。胰腺通常分泌可溶性的PLA2和hUG至血液循环中。在本发明的子宫珠蛋白剔除(KO)小鼠(本文称为“UG KO小鼠”)的胰腺中报道了坏死性损伤。
在不存在子宫珠蛋白时,UG KO小鼠显示可引发自身免疫反应的类似的胰腺组织损伤。因此,UG可用来预防或终止Ⅰ型糖尿病的缓慢进程。优选的给药途径为通过注射。肾病的预防和治疗
UG KO小鼠中的肾Fn沉积和纤维变性类似于人肾病中Fn的沉积和纤维变性。因此,UG的给药可预防或减缓处于如Ⅱ型糖尿病危险中的病人的肾病发展进程。预防和治疗眼炎症
眼炎症,包括眼色素层炎、视网膜炎和术后炎症的特征在于增加的PLA2活性。因此,UG可被局部、眼内或全身性给药以减少眼的炎症。动脉硬化
动脉硬化为全身性血管纤维化增厚。它通过Fn在脉管系统壁的沉积来启动和/或介导。动脉粥样硬化是除开Fn沉积外还涉及胆固醇沉积的动脉硬化的一种形式。因此,UG可被给药以预防或减少动脉硬化。急性肾衰竭
急性肾衰竭(ARF)通常是远器官炎症、感染或直接损伤的结果,它导致可溶性PLA2在血液循环中的释放和活化。ARF期间对肾的损伤可相当严重,具有由炎症促进的急性组织损伤并可归入肾的纤维变性中,从长期来看导致降低的肾功能。如UG KO小鼠所示,UG的抗炎症和抗纤维化的特性在肾中尤其相关。
优选的给药途径为通过注射或全身性给药。
总之,下面非限制性的疾病表与PLA2和/或纤连蛋白沉积的抑制相关,每一种为通过本发明方法治疗或预防的疾病的代表:

而且,UG可单独给药或与常用于治疗或预防上述疾病的其它活性剂或组合物混合给药。这些活性剂或组合物包括,但不局限于类固醇、非类固醇抗炎剂(NSAIDs)、化疗剂、止痛剂、免疫治疗剂、抗病毒剂、抗真菌剂、疫苗、免疫抑制剂、造血生长因子、激素、细胞因子、抗体、抗血栓形成剂、心血管药物或生育药物。而且还包括口服耐受性药物、维生素和矿物质。
而且,UG可作为用于在注射部位抑制炎症或给药其它治疗剂或预防剂以抑制炎症或由这些物质导致的免疫反应的载体来给药。在此方面,UG使得能对靶适应症给药有效量的活性剂。
UG还可以化妆品组合物的形式给药用于限制由伤口愈合或在皮肤炎症反应期间导致的纤维变性、疤痕或瘢痕形成。
而且,UG可用作血液补充物,即作为合成或捐献的人血的添加物。
最后,UG可用来通过加入至精子、体外受精卵和在转移至人和其它哺乳动物的母体子宫之前的胚胎中来提高人工受精的比率。
本发明涉及UG在预防或治疗PLA2和纤连蛋白相关的疾病中的用途。在预防疾病方面,“预防”指在易感或可能易感人群中预防疾病发生,或限制其严重性或发展,而术语“治疗”指改善疾病或病理学状况。
UG可静脉内给药,或在治疗新生儿RDS/BPD和成人RDS的情况下,借助于气管内管以液体或半气雾剂形式给药。其它可行的给药途径包括局部、眼、经皮肤、透皮、肛门、全身性、肌肉内、缓释、口服、阴道、十二指肠内、腹膜内和结肠内给药。这些组合物可以药物学、营养和兽医学领域技术人员熟知的剂量和方法对需要这种给药的个体或病人给药,给药时考虑这些因素,如年龄、性别、体重和特定个体或病人的情况以及给药途径。本发明的组合物还可以控制释放的剂型给药。再次考虑到如年龄、性别、体重以及特定个体或病人的情况以及给药途径,组合物可与其它活性剂同时给药或顺序给药。
本发明的组合物的实例包括用于口服给药的可食用的组合物,如固体或液体制剂,例如胶囊剂、片剂、丸剂以及类似的给药于管口的液体制剂,例如口、鼻、肛门、阴道等,制剂如悬浮剂、糖浆或酏剂;和用于胃肠外、皮下、皮内、肌肉内或静脉内(如注射给药)给药的制剂,如无菌悬浮剂或乳剂。然而,由于组合物中的活性成分可与蛋白形成复合物,使得当其给药至血流中时,由于血液蛋白的沉淀,可能形成血块;本领域技术人员应考虑这种情况。
在这些组合物中,UG可与适宜载体、稀释剂或赋形剂混合,如无菌水、生理盐水、葡萄糖、DMS0、乙醇等。UG可以用于再配制在等渗水溶液、盐水、葡萄糖或DMSO缓冲液中的冷冻干燥的形式提供。在一些盐溶液中,观察到一些rhUG沉淀;这种现象可用作一种分离本发明化合物的方法,例如通过“盐析”法。
进一步,本发明还设想提供UG的试剂盒。该试剂盒还包括一个包含适宜载体、稀释剂或赋形剂的单独的容器。试剂盒可包括用于同时或顺序给药的降低或减轻上述疾病的致病作用的其它试剂。其它试剂可以单独的容器或与UG混合的形式提供。另外,试剂盒可包括关于混合或混合成分和/或给药的说明书。
本发明还设想一种治疗或预防特征在于内源功能性UG缺乏的炎症或纤维变性疾病的方法,它包括对需要这种治疗的病人给药补偿量的UG。术语“补偿量”指使总UG(内源功能性UG和外源UG)的肺局部或全身性浓度至其正常的范围内所需的UG的量。更具体地说,内源UG的肺局部浓度的正常范围为约>50毫克UG/微克血清白蛋白或>50毫克/升。血清UG浓度的正常范围为>15毫克/升。
本发明的组合物包含获得预期目的有效量的天然和/或重组hUG,即提高的UG血浆或组织水平以产生选择性抑制PLA2和降低的炎症和/或纤连蛋白的结合以减轻其在纤维变性疾病中聚集和/或沉积的所需效果。组合物包含与可药用的载体或稀释剂混合的有效量的基本上纯化的天然和/或重组人UG。
在另一个方面,本发明的组合物包含与可药用的载体或稀释剂混合的有效量的天然和/或重组hUG和肺表面活性剂。对肺局部气管内给药足以抑制PLA2活性和/或纤连蛋白沉积的UG需要在单剂或复剂中范围为0.2μg/kg至500mg/kg基本纯化的UG蛋白。UG通常以单剂为20ng/kg至500mg/kg的量一次或多次给药,或连续灌注高达10g。
天然和/或重组hUG还可借助于气管内途径与人工肺表面活性剂,如Survanta结合给药。UG与Survanta(5mls/kg)共同给药,UG不结合表面活性剂,阻止它发挥治疗功效。肺表面活性剂通常在组合物中以约10-90%(重量计)存在,较通常为约20-80%(重量计)。由于其极低的表面张力,表面活性剂携带有UG扩展在肺的内表面。借助于静脉内注射全身性给药足以抑制PLA2活性和/或纤连蛋白沉积的UG在较长的时间内(天)需要范围为0.5μg至连续灌注数克蛋白的基本上纯的天然和/或重组hUG的量。
用于注射和半气雾剂气管内送递的适宜的制剂包括任选具有粘度调节剂和稳定剂的天然和/或重组hUG的水溶液。
如本文所采用的术语“PLA2抑制有效量”指抑制PLA2活性和降低或减轻病人组织或身体炎症的UG的量。术语“纤连蛋白结合有效量”指结合纤连蛋白以降低其聚集和/或沉积,并防止或降低原纤维形成或纤维变性的UG的量。如本文所使用的术语“抗炎量”指降低或减轻组织或身体炎症的量。通常,给药至成人用于治疗炎症和纤维变性疾病的UG的量为单剂0.2μg/kg至500mg/kg或在较长的时间内给药的高达数克。对于新生儿,在治疗新生儿RDS中,范围通常为单剂为50ng/kg至100mg/kg或在较长的时间内连续给药高达10g。连续灌注的有效安全速率为50ng/kg/小时至500mg/kg/小时。
实施例
现将进一步参考下面非限制性的实施例来说明本发明。除非另有说明,份和百分数以重量计。
实施例1:体内实验
通过Mantile等的方法(1993)获得重组人UG。
在妊娠142天通过剖腹产产下体重各为约400g的一只雄性和雌性P.cyanocephalus品种。这是建立的RDS模型(Coalson,J.J.等,BPD的狒狒模型Ⅱ:病理学特性。实验分子病理学37:355-350(1982))。
分娩后,将幼仔用盐酸氯氨酮(10mg/kg)麻醉并用2.5mm直径的气管内管插管。借助于通过经皮肤注射入桡动脉放置的动脉线监测血气和压力。将一深的静脉线经皮肤放置入隐静脉内,通过它给药液体、抗生素和药物。将动物放在饲服控制的红外温室中并用带有加湿器的标准循环时间、压力调节的通风器进行通风,保持在36-37℃。最初设置为适宜胸运动所需的FiO2 1.0,速率为40/分钟,I/E比例1∶1.5,4cm H2O正向末端呼气压力(PHEP)和峰吸气压力(PIP)。FiO2保持在1.O,调节PIP以保持PaCO2为40+10托。每小时监测血气、血细胞比容、电解质、凝血酶原时间、部分促凝血酶原激酶时间和血糖试条。抽取用于研究的血液容量性地被加入肝素的成年狒狒血液代替。通过电解质以10cc/kg/小时给药静脉内液体,并当心率超过180次/分钟时如所需地增加。当碱不足超过-10时加入碳酸氢钠(2meq/kg)。实验期间连续给药氨苄青霉素(50mg/kg/天,以两次分开的剂量)和庆大霉素(5mg/kg/天,以两次分开的剂量)。
一只动物接受表面活性剂加上PBS(处理号1),第二只动物(处理号2)接受表面活性剂加上两倍剂量的1mg/kg rhUG。经气管内管直接将表面活性剂和rhUG对肺给药。使用的表面活性剂为Survanta(RossLabs),表面活性剂制剂来自牛肺组织,除开磷脂还包含表面活性剂脱辅基蛋白B和C。第一次剂量的rhUG与表面活性剂一起给药,第二次在第一次之后4小时给药。监测动物的动脉血气、电解质和EKG。在开始表面活性剂治疗后50小时杀死它们。在24和48小时的时候,用包含蛋白酶抑制剂(PMSF,10μg/ml亮抑蛋白酶肽,10μg/ml胃蛋白酶抑制剂和杆菌肽)的PBS灌洗肺。将其冷冻于-80℃下直至分析PLA2活性。通过Bradford方法(Biorad)测定总蛋白。根据Levin等的方法(1986;见上文)测定肺洗出物中的PLA2活性并示于下表。

上面给出的数据为两次测定的平均值。结果表明气管内给药rhUG在体内抑制PLA2。接受了表面活性剂和rhUG的动物与接受了表面活性剂而没有rhUG的动物相比在其肺洗出物液体中具有显著较低的PLA2活性。数据证实与表面活性剂结合给药rhUG在保护表面活性剂磷脂中有益。实施例2:在体外可溶性PLA2对人工表面活性剂水解的抑制
在体外rhUG抑制可溶性PLA2对人工表面活性剂的水解。Survanta为来自牛肺的人工表面活性剂并被用来治疗患有RDS的早产新生儿和患有RDS的成人(ARDS)。组Ⅰ可溶性PLA2,即猪胰腺PLA2(BoehringerMannheim)对Survanta的水解特征在于其作为底物与荧光磷脂酰胆碱底物(Cayman Chemicals)竞争,产生花生四烯酸产物的能力。
Survanta是组Ⅰ可溶性PLA2体外降解的底物。在体外Survanta迅速地被在人肺的细胞外液体中发现的PLA2降解。在体外RHUG抑制Survanta的降解。实施例3:UG剔除小鼠的构建
制备转基因UG KO小鼠,目的在于用于确定子宫珠蛋白在哺乳动物生理学中的作用以及产生在严重炎性临床疾病中UG作为治疗剂的模型。第一步是构建适宜的DNA载体,通过它来靶定和打断内源小鼠子宫珠蛋白基因。将包含来自129/SVJ小鼠品系的外显子3和子宫珠蛋白基因的旁侧序列(Ray,1993)的3.2kb BamHI-EcoRI DNA片段亚克隆至如Lei等(1996)描述的pPNW载体的相应位置。通过PCR(引物Primer-L(来自内显子1):5’-TTCCAAGGCAGAACATTTGAGAC-3’;Primer-R(来自外显子2):5’-TCTGAGCCAGGGTTGAAAGGC-3’)扩增包含部分外显子2和其上游序列的0.9kb片段,NotI和XhoI限制性位点被遗传改造至末端用于定向亚克隆至基因靶定载体。在该构建物中,编码27个氨基酸的79bp的外显子2缺失。将PCR片段放置于pPNW中编码新霉素抗性基因的上游,产生基因靶定载体pPNWUG。载体示于图2A中,其中PGK-neo盒打断子宫珠蛋白基因,破坏蛋白质编码序列。
根据Nagy,A.等,PNAS 90:8424(1993)用NotI线性化pPNWUG基因靶定载体并电穿孔至ES R1细胞中。电穿孔细胞的9-(1,3-二羟-2-丙氧甲基)鸟嘌呤和G-418选择产生156个克隆,DNA印迹分析证实野生型子宫珠蛋白等位基因的5.1kb HindⅢ片段和来自156个克隆中的3个的同源重组的另外8.2kb HindⅢ片段,其示于图2B中。根据Capecchi,科学244:1288(1989),将这些ES R1克隆注射至C57BL/6胚泡中。生成两种不同的传代自不同嵌合起始物的小鼠品系。让携带靶定子宫珠蛋白基因座的杂合后代(UG+/-)交配,通过图2C所示的PCR以及图2D所示的DNA印迹分析后代的基因型。实施例4:UG基因剔除和小鼠UG(mUG)蛋白不存在的证实
为了证实纯合剔除小鼠(UG-/-)不具有任何可检测的mUG,检测子宫珠蛋白基因靶定小鼠在一些器官,包括肺中的UG-mRNA和mUG蛋白的表达。实验方法通过规定的动物保护和使用委员会的批准。从UG+/+、UG+/-和UG-/-小鼠的不同器官分离全部RNA。使用逆转录聚合酶链反应(RT-PCR)检测mUG-mRNA。使用mUG特异性引物,mPr(5’-ATCTTGCTTCACAGAGGACTTG-3’)逆转录靶分子,使用PCR引物mPr和mPl(5’-ATCGCCATCACAATCACTGT-3’)扩增产生的cDNA。将PCR产物与来自UG基因序列外显子-2的mPp(5’-ATCAGAGTCTCTGGTTATGTGGCATCC-3’)寡核苷酸探针杂交。用于小鼠GAPDH RT-PCR的引物和探针如下;mGAPDH-r(5’-GGCATCGAAGGTGGAAGAGT-3’);mGAPDH-1(5’-ATGGCCTTCCGTGTTCCTAC-3’);mGAPDH-p(5’-GAAGGTGGTGAAGCAGGCATCTGAGG-3’)。图2E显示在UG+/+、UG+/-,但未在UG-/-小鼠肺中检测到mUG-mRNA。类似的数据(未显示)表明mUG-mRNA不存在于UG-/-小鼠的前列腺或子宫中,但存在于具有完整子宫珠蛋白基因的小鼠中。
肺中的mUG蛋白的免疫沉淀和蛋白质印迹分析产生示于图2F的类似确证的结果。来自UG+/+和UG-/-小鼠的肾、肝脏和肺的的组织裂解物通过在包含2mM苯基甲基磺酰氯和各20μg/mL抑蛋白酶肽、亮抑蛋白酶肽和胃蛋白酶抑制剂A的缓冲液(10mM Tris-HCl,pH7.5,1%TritonX-100,0.2%脱氧胆酸盐,150mM NaCl,5mM EDTA)中匀浆制备。将匀浆在4℃下以17,500×g离心30分钟,并如所描述的(E.Harlow和D.Lane,抗体;实验室手册,第一版,冷泉港实验室出版社,冷泉港,NY,1988)通过将组织裂解物或血浆蛋白(1mg/mL)与兔抗小鼠Fn抗体(1∶100稀释)一起保温进行免疫沉淀。通过在10%甘油,50mMTris-HCl,pH 7.5,250mM NaCl,4.3mM磷酸钠存在下,将等摩尔浓度的mFn与rhUG 4℃保温1小时,接着加入抗-mFn抗体(1∶100稀释),使纯小鼠Fn(mFn)与rhUG一起共免疫沉淀(Mantile,G等,生物化学杂志267:20343(1993))。还原条件下,在4-20%或6%SDS-聚丙烯酰胺凝胶上分辨等量的提取组织蛋白(30μg)或免疫沉淀物,接着用抗小鼠Fn(1∶2000稀释)或UG(1∶2000稀释)的兔抗体进行蛋白质印迹。在来自UG-/-小鼠的组织或体液中未检测到mUG,而来自UG+/+和UG+/-小鼠的组织的确包含mUG蛋白。
最后,对UG-/-小鼠肺的组织病理学分析,仅在支气管上皮细胞中缺乏mUG特异性免疫染色。将来自UG+/+、U+/-和UG-/-小鼠的肺组织固定在Bouin氏液或10%中性缓冲的福尔马林固定液中,包埋在石蜡中,切成4-6微米。将它们用苏木精和伊红(H&E)染色。将选择的组织染色通过Masson氏三色法检测胶原,PTAH检测纤维蛋白或Congo红检测淀粉状蛋白。对于mUG和mFn的免疫组织化学检测,使用Vectastain兔Elite ABC试剂盒(Vector实验室)。通过使用对应于mUG氨基酸序列(Lys28-Thr49,具体为KPFNPGSDLQNAGTQLKRLVDT)的合成肽(Peptide Technologies,Inc.)产生mUG的兔抗体(CytImmune)。以1∶1000的稀释度使用mFn的兔抗体(GIBCO BRL),mUG的抗体以1∶500使用。
这三组结果证实纯合子宫珠蛋白剔除小鼠UG-/-缺乏mUG蛋白或任何可检测的蛋白成分。实施例5:子宫珠蛋白剔除小鼠的表型
在179只源于UG+/-小鼠杂交的小鼠中,46只(26%)为+/+,90(50%)为+/-和43只(24%)为UG-/-表型,表明打破的mUG基因座以孟德尔方式遗传,UG+/+、UG+/-和UG-/-小鼠在出生时存活率相同。然而,UG-/-小鼠显示新的表型,它们发展成特征在于恶病质、严重蛋白尿和与体重大大降低相关的低血钙的进行性疾病。蛋白尿为在尿中排泄异常高水平的白蛋白和其它血清蛋白的疾病。它表明肾小球功能异常和肾衰竭。对受影响动物(如上所述对于肺)的肾的组织病理学研究揭示了示于图3的暴发性肾肾小球疾病。与UG+/+小鼠的肾小球比较,UG-/-小鼠的细胞过少并具有大量的嗜曙红的蛋白沉积。UG-/-小鼠中的致命肾病的时间进程为早期发生(4-5周时间)或后期发生(10个月时间)。在开始4周龄表现健康的那些UG-/-小鼠在两月龄时具有病灶性的肾小球沉积。在约10个月时,这些小鼠具有类似于死于早期发生疾病的小鼠的严重的恶病质。杂合子具有在UG-/-小鼠中观察到的较轻度形式的肾病。患有晚期发生疾病的小鼠的肾的组织病理学不仅显示如早期发生疾病中的严重的肾小球病,而且具有显著的肾实质纤维变性和肾小管增生(参见图3)。虽然在肾中发现了UG-/-小鼠的显著的病理学,但组织病理学研究还发现看来为血管来源的胰腺的偶然的坏死病灶区。而且,还发现了表明为编程性细胞死亡实体的胸腺和脾结构中的病灶区。令人感兴趣的是,胰腺表达mUG基因,并且该器官还为Ⅰ组细胞外PLA2的丰富来源;由于这为主要的消化酶,因此它的活化可能导致组织损伤。
由于子宫珠蛋白已被报道具有免疫调节和抗炎特性,并且由于已知反应性淀粉样变性应答于炎症而发生,mUG-裸鼠中肾小球的沉积可能是淀粉样蛋白。反应性淀粉样变性特征在于淀粉状蛋白和免疫复合物的沉积。UG-/-小鼠中肾沉积物的同一性通过肾切片的免疫组织化学来确定。来自UG-/-和UG+/+小鼠的肾切片用刚果红染色,并在偏振光下观察。在该实验中淀粉状蛋白产生阳性双折射;然而,UG-/-小鼠的肾小球明显为阴性。对UG-/-小鼠的肾小球中的IgA,IgG或IgM-免疫复合物存在的免疫荧光研究以及对主要淀粉状蛋白的存在的免疫组织化学分析也为阴性。因此,UG-/-小鼠的肾小球沉积物不包含淀粉状蛋白和免疫复合物,因此看来不是炎症应答的结果。实施例6:UG-/-肾中的Fn和胶原蛋白的检测
UG-/-小鼠的肾沉积物通过透射电子显微镜观察以说明它们的结构和形态学。将来自患有肾小球损伤的UG-/-小鼠的肾固定在福尔马林中并包埋在环氧树脂中。将薄的切片用乙酸双氧铀和柠檬酸铅染色以用于电子显微镜下观察。以6000X或60,000X进行显微照相。沉积物主要包含两种类型的纤维性结构;一种类型为相对不常见的长的具条纹的纤维,另一种更大量的短的分散纤维(图3E和3F)。由于ECM蛋白,如胶原蛋白和纤连蛋白产生类似的纤维性结构,因此UG-/-小鼠肾小球沉积物可能包含这些蛋白。
接着通过使用抗mFn抗体的免疫荧光分析肾小球沉积物。福尔马林固定的组织切片被用于使用兔抗-mFn和FITC-缀合山羊抗兔IgG的免疫荧光。类似地,还进行使用特异于mFn、胶原蛋白Ⅰ和Ⅲ、玻连蛋白、层粘连蛋白和骨桥蛋白的抗体的免疫荧光研究。使用ZeissAxiophot显微镜对落射荧光进行照相。野生型小鼠的肾小球中的Fn特异性免疫荧光基本上不可检测(图3G),在UG-/-同窝幼畜的肾小球中密集(图3H)。当使用Masson氏三色染色,UG+/+小鼠的肾小球为阴性(图3I),UG-/-小鼠(图3J)的为阳性,表明在肾小球沉积物中胶原蛋白的存在。使用胶原蛋白Ⅰ和胶原蛋白Ⅲ特异性抗体的免疫荧光证实了这些结果。由于已知Fn与其它细胞外基质(ECM)蛋白相互反应,我们还通过免疫组织化学检测了UG+/+和UG-/-小鼠的肾小球中的层粘连蛋白、玻连蛋白和骨桥蛋白的存在,其结果为阴性。实施例7:UG-/-小鼠的肾不超量产生Fn
为了确定是否Fn过量的产生可能导致其在肾小球中的沉积,我们通过RT-PCR和光密度法评价了UG-/-和UG+/+小鼠的肾、肺和肝脏中的Fn-mRNA的相对量。结果表明Fn-mRNA的相对量在UG+/+和UG-/-动物中基本上相同。因此,Fn-mRNA的超量产生可能不是UG-/-小鼠肾小球中Fn沉积的原因。接着我们通过还原条件下的SDS-PAGE和蛋白质印迹比较了UG-/-和UG+/+小鼠的血浆,肾和肝脏中的Fn-蛋白。在野生型小鼠的血浆、肾和肝脏中仅可检测到220kD Fn种类;然而,UG-/-小鼠的血浆和肝脏裂解物中具有220kD Fn带,肾裂解物包含另一种独特的共价交联的多聚Fn带(图4A)。实施例8:UG-/-小鼠中的提高的血清PLA2活性
基于目前的概念,至少在细胞表面的Fn基质装配和原纤维形成中关键的起始步骤被认为包括整合素活化和Fn自身聚集。由于UG是炎症途径中的关键酶-可溶性磷脂酶A2(sPLA2)的强抑制剂,UG-/-小鼠中mUG的缺乏可能导致发展为炎性肾病的肾小球性肾炎。因此,我们检测了年龄、性别和体重匹配的UG+/+(n=3)和UG-/-小鼠(n=3)的血清中的PLA2活性。杀死动物并根据制造商的说明,使用PLA2-分析试剂盒(Caymen Chemical)三次重复测定各样品的血清PLA2活性。通过Bradford分析(Bio Rad)确定血清中的蛋白浓度并计算PLA2的比活。UG-/-小鼠的血清PLA2的比活(umol/分钟/mg蛋白)[36+3.3(SEM)]明显高于(p<0.05)UG+/+小鼠[18+2.8(SEM)]。这些结果提出这种可能性,即在UG-/-小鼠中,较高的PLA2活性可能导致增加的溶血磷脂酸(LPA)的产生,结果促进整合素的活化和Fn自身聚集。实施例9:体外子宫珠蛋白与纤连蛋白的相互作用
为了进一步理解子宫珠蛋白可能如何防止Fn自身聚集,在体外测定rhUG破坏mFn-Fn相互作用的能力。将等摩尔浓度的rhUG和mFn一起保温以使得所有蛋白结合或其它相互作用,接着用抗Fn抗体免疫沉淀,并通过在还原条件下的SDS-PAGE分辨免疫沉淀。如前所述,通过使用mFn或mUG抗体的蛋白质印迹分别检测各种蛋白。结果表明纤连蛋白与rhUG共免疫沉淀(图4B)。为了证实这些结果,将125I-rhUG与mFn一起保温,并在非变性和非还原条件下,使用6%聚丙烯酰胺凝胶进行电泳来分辨复合物(图4C)。放射自显影中对Fn-UG异聚体的检测(泳道2)表明在体外可溶性Fn与UG相互作用。为了确定在体内是否发生Fn-UG异聚作用,将UG+/+和UG-/-小鼠的血浆用不与rhUG交叉反应的抗mFn抗体进行免疫沉淀(图4D)。抗mFn抗体与来自UG+/+,但不与来自UG-/-小鼠的血浆的mFn和rhUG共沉淀,这表明Fn-UG异聚体存在于UG+/+小鼠的血浆中。因此,Fn-UG复合物并不单纯是在体外形成的假象,而是天然产生于血清中。
为了确定UG对Fn结合的特异性和亲和性,我们在存在和不存在UG下将125I-Fn与未标记的Fn一起保温。所有复合物与二琥珀酰亚胺基底物(DSS)亲和交联。使用覆盖有人Fn(hFn)(CollaborativeBiomeical Products)的24孔平板,在不存在和存在UG或Fn(10-12-10-5M)下,将3ul125I-Fn(Sp.Act.6mCi/mg:ICN Biomedicals)在500ul HBSS中室温下保温2小时。SDS-PAGE以及使用UG抗体对所有Fn的蛋白质印迹未检测到任何UG污染。将放射标记的复合物用PBS洗涤两次,溶解在1N NaOH中,用IN HCl中和,通过γ计数器测定放射活性。在一个单独的实验中,在不存在或存在增加浓度的还原rhUG(5-500μg)下,将125I-hFn(3u1)与20ul(1mg/m1)小鼠Fn在40u1HBSS,pH7.6中一起室温下保温2小时。室温下将样品与0.20mM DSS交联20分钟,在SDS-样品缓冲液中煮沸5分钟,4-20%SDS-聚丙烯酰胺凝胶电泳并放射自显影。在不存在UG下,125I-Fn与未标记的Fn形成高分子量的放射活性复合物,但在存在UG时,Fn-Fn聚集物的形成被以依赖于UG浓度的方式抑制(图4E)。
为了确定在Fn对UG的结合亲和性与Fn对其本身的结合亲和性之间是否存在任何差异,进行结合实验,其中将125I-Fn与未标记Fn一起保温,并与变化浓度的UG一起固定在多孔平板中。在独立实验中,还使用各种浓度的未标记的可溶性Fn进行了对125I-Fn与未标记固定的Fn的结合研究。来自两种类型的结合实验的数据的Scatchard分析产生具有UG对Fn的结合的解离常数(kds)为13nM,Fn对其本身的结合的解离常数为176nM的直线。这些结果表明由于UG对Fn的相对较高的结合活性,UG有效地对抗Fn自身聚集。如上面对Fn所描述的,还在不存在或存在UG下,进行了将放射碘标记(125I)-胶原蛋白Ⅰ与未标记的Fn一起保温的亲和交联实验。在存在或不存在还原UG(250μg)下,将15ul变性或未变性125I-胶原蛋白Ⅰ(Sp.Act.65.4mCi/mg)与Fn一起保温,亲和交联,电泳并放射自显影。结果表明UG对抗高分子量125I-胶原蛋白-Fn聚集物的形成(图4F)。实施例10:rhUG对肾小球hFn沉积的体内抑制
为了检测rhUG是否保护肾小球免于Fn聚集,对UG+/+和外表健康的UG-/-同窝仔静脉内单独给药可溶性人Fn(nFn)或与等摩尔浓度的rhUG混合的hFn。
将人Fn(500μg/150ul PBS)给药至两月龄,大约22g的UG+/+和外表健康的UG-/-小鼠的尾静脉中。类似地,对照鼠被注射500μg与等摩尔浓度的rhUG或白蛋白的150ul PBS混合物。末次注射24小时后,将小鼠杀死,并将各器官固定在缓冲的福尔马林中。通过使用单特异性的抗-hFn 抗体(GIBC0 BRL;克隆1)和FITC缀合兔抗小鼠IgG(Cappel)的免疫荧光观察肾和其它器官的组织切片。在一个单独的实验中,将UG+/+小鼠每天注射仅1mg hFn的150ul PBS达连续3天。
注射人Fn的原理能分辨内源小鼠Fn与给药的hFn。已描述了静脉内给药的方法和各组织中的hFn的免疫组织化学检测。
注射了hFn和rhFn(1∶1摩尔比例)的混合物或仅hFn的野生型UG+/+小鼠的肾小球中的人Fn的免疫荧光类似(图5A和5B)。然而,注射了hFn和UG混合物的UG-/-小鼠在肾小球中显示极少的hFn特异性免疫荧光(图5C),而仅注射了Fn的那些显示较高强度的免疫荧光(图5D)。作为对照,给药hFn和BSA的混合物不产生保护作用。
为了确定这种UG保护作用是否可通过在UG+/+小鼠中注射较大量的Fn来克服,我们每只动物每天注射1mg hFn达连续3天。虽然以较低的剂量(500μg/动物)对UG+/+小鼠静脉内给药在导致任何明显的肾小球沉积中不起作用(图5A),但给药较高剂量(3mg/动物)导致显著的积累。因此,UG防止肾小球Fn沉积,与UG-/-小鼠相反,由于内源UG的存在,UG+/+对于可溶性Fn的积累具有较高的阈值。实施例11:组织培养细胞中rhUG对纤维形成和Fn基质聚集的抑制
为了确定在通常的体外组织培养物分析中UG是否阻止Fn纤维形成和基质聚集,在仅包含可溶性hFn或等摩尔浓度的hFn和rhUG的混合物的培养基中培养小鼠胚胎成纤维细胞。如上测定培养细胞(CRL6336,ATCC)中的Fn基质的聚集和纤维形成。与接受hFn和rhUG的混合物的那些比较(图5F),在仅用hFn处理的培养物的细胞中观察到的纤维形成水平更高(图5E)。实施例12:临床样品中的UG-Fn复合物的检测
对体液的临床样品,如血清、BAL体液和痰中的UG-Fn复合物的检测在确定该复合物在人疾病中的作用非常重要。开发了用于检测UG-Fn复合物的液相诊断分析法,该分析方式示于图6。与固体载体共价连接的捕获抗体为针对人蛋白产生的单特异性的兔多克隆抗体。固体载体可为珠,如磁性珠,管和ELISA平板。固体载体在各结合反应后可以进行洗涤步骤以获得对于各种样品类型的更一致的结果。检测抗体为Fn特异性的,并可从许多商业来源获得。与酶,如辣根过氧化物酶(HRP)结合的抗-IgG抗体接着被用来检测在标准酶反应中分子链末端的抗-Fn IgG,其中酶底物被转化为用分光光度计或荧光光度计(Amersham)定量的显色或荧光化合物。该分析法的检测极限为500μgUG-Fn复合物/ml样品液体。实施例13:子宫珠蛋白缺陷
暂时但为急性hUG缺乏是由已知为临床透析,包括血液透析、腹膜透析和连续透析(CRRT)的血液清洗技术产生的。所有形式的临床透析包括使用半透膜从血液中过滤有毒的身体废产物,包括化学代谢物,如尿和小的蛋白,如β2-微球蛋白。
从其在SDS-PAGE中的特殊的迁移已知UG是一种极紧密的蛋白,其中特殊的迁移对应于约10-13kDa的分子量,尽管其真正的分子量为15.7kDa。因此,预期UG二聚体在透析实验中的行为类似于10-13kDa。令人惊奇的是,发现二聚体是如此致密以致通过8.0kDa MWCO透析膜。UG还通过14.0 kDa MWCO透析膜。
用于这些实例中的透析膜的组成与生产和用于临床透析的大多数膜的组成相同或类似。它们由再生纤维素或乙酸纤维素组成。
对于本实验,将不含缓冲添加物的两份部分纯化(一个纯度大于90%,一个约70%纯度)的rhUG细胞裂解物的1.0ml等份试样对1000ml未缓冲的50mM乙酸铵透析,使用三种大小的透析管:3.5kDa,8.0kDa和14.0kDa(Spectra/Pro;Thomas Scientific)。在48小时期间对各样品四次更换缓冲液,均在室温下(约25-27℃)进行。各透析样品的外观从清亮的黄色变为清亮的无色液体。在过程的开始和结束,通过简短地对管道(挤压系统)加压并观察任何渗漏来确保其中不存在渗漏的透析管道系统。管道系统在各末端是双重夹紧固定的以进一步确保渗漏。
图7显示这些SDS-PAGE分析结果。90%纯度的预透析的样品示于泳道7和8,紧接泳道1,2和3中的三个透析后样品。UG二聚体不再存在于代表用8.0kDa MWCO膜透析的样品的泳道中。这些结果后来用不同批次的部分纯化的UG制备物得以证实。
虽然本发明结合目前被认为是最可行和优选的具体实施方案进行了描述,但应理解本发明不局限于所公开的具体实施方案,相反,覆盖各种包括在所附权利要求的实质和范围内的各种修改和等同的方法。
