Abstract
De uitvinding heeft betrekking op een farmaceutisch preparaat, bevattende als actief bestanddeel Clara celeiwit CC10 of een werkzaam derivaat daarvan, en een farmaceutisch acceptabele drager of verdunmiddel, en op Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik : i) ter voorkoming van weefselafstoting na transplantatie, ii) ter behandeling van infertiliteit, septische shock, allergie, biliaire cirrhosis, myastenia gravis; of iii) ter preventie van vroeggeboorte, abortus door Gramnegatieve septicemie, en voor gebruik als diagnosticum.

<Desc/Clms Page number 1> FARMACEUTISCH PREPARAAT OP BASIS VAN CLARA CELEIWIT CC10, EN HET GEBRUIK DAARVAN ALS GENEESMIDDEL EN DIAGNOSTICUM De onderhavige uitvinding heeft betrekking op een farmaceutisch preparaat op basis van Clara celeiwit CC10, en op het gebruik van Clara celeiwit CC10 als geneesmiddel en diagnosticum. EMI1.1 Clara celeiwit CC10, ook genoemd urinair eiwit 1, en kortweg CC10 of CC16, is bekend uit de literatuur (Singh G., et al., Biochimica Biophysica Acta 950 [1988] 329-337). Clara celeiwit CC10 is een homodimeer, door disulfide verbonden polypeptide (Mr 2 x 7900) dat voorkomt in een aantal biologische vloeistoffen, zoals sputum, urine, sperma en in de slijmvliezen van luchtwegen. In het sputum bedraagt de Clara celeiwit CC10 concentratie gemiddeld 15 mg/l. De concentratie in de urine is groter bij mannen (30 gag/1) dan bij vrouwen (0, 5 all), welke verschillen maximaal worden in de puberteit. Verder stijgt de concentratie van het Clara celeiwit in de urine tijdens de zwangerschap. Over de biologische functie van Clara celeiwit CC10 is op dit moment weinig bekend. De onderhavige uitvinding is gebaseerd op de vinding, dat Clara celeiwit CC10 de produktie van interferon-gamma (= IFN-gamma) remt van geactiveerde mononucleaire cellen. Deze remming is maximaal in het geval interleukine 2 (IL-2) was gebruikt voor de stimulatie. Clara celeiwit CC10 inhibeert enerzijds de IL-2 produktie, maar in het bijzonder blokkeert het de interactie van IL-2 met doelwitcellen hetzij een combinatie van beide. <Desc/Clms Page number 2> Uit deze vinding blijkt, dat Clara celeiwit CC10 werkt als een immunosuppressief middel als gevolg van zijn anti-cytokine werking. De onderhavige uitvinding verschaft een farmaceutisch preparaat dat bevat als actief bestanddeel Clara celeiwit CC10 of een werkzaam derivaat daarvan, en een farmaceutisch acceptabele drager of verdunmiddel. Dit farmaceutisch preparaat kan worden gebruikt bij de behandeling van organismen, met name mens en dier, als geneesmiddel en als diagnosticum. Het farmaceutisch preparaat kan het Clara celeiwit CC10 bevatten in combinatie met elke farmaceutisch acceptabele drager of verdunmiddel. De dosisvormen kunnen bestaan uit tabletten, korrels, inhaleerbaar poeder, aërosolen, parenteraal, intramusculair, intraveneus toedienbare vloeistoffen, en dergelijke. Bijzondere voorkeur hebben injecteerbare vloeistoffen die isotoon zijn en eventueel op gebruikelijke wijze gebufferd. Het farmaceutisch preparaat kan in een geschikte doseringsvorm worden toegediend in een hoeveelheid afhankelijk van de concentratie van het actieve bestanddeel Clara celeiwit CC10. Deze hoeveelheid bedraagt in het algemeen minder dan 10 mg/kg lichaamsgewicht, in het algemeen minder dan 5 mg/kg lichaamsgewicht, bijvoorbeeld in het bereik van maximaal 0. 05 tot 5 mg/kg lichaamsgewicht. Deze dosis kan eenmalig of in meervoudige doses verdeeld over de dag worden toegediend, afhankelijk van het type ziekte, de aard van de ziekte, leeftijd en geslacht van de patiënt. Als gevolg van de anti-cytokine werking van het Clara celeiwit CC10, heeft Clara celeiwit CC10 vooral toepassing bij de behandeling van auto-immuunziekten, zoals thyroidytis Hashimoto, lupus erythematosus, reumathoide arthritis, multiple sclerose, chronische ontsteking. Verder kan Clara celeiwit CC10 worden toegepast voor gebruik ter voorkoming van weefselafstoting na <Desc/Clms Page number 3> transplantatie ; ter behandeling van infertiliteit, septische shock, allergie, biliaire cirrhosis, myastenia gravis ; of ter preventie van vroeggeboorte, abortus door Gram-negatieve septicemie. Bovendien kan Clara celeiwit CC10 worden gebruikt als diagnosticum, omdat de concentratie daarvan in lichaamsvloeistoffen, zoals urine, bloed, vruchtwater, gewrichtsvocht, long-uitwassen, cerebrospinaalvocht, zaadvloeistof, en dergelijke, een maat vormt voor de immuunstatus, en een beeld geeft van de werking van het zogenaamde cytokinine-netwerk, waardoor de concentratie van het Clara celeiwit een voorspellende waarde heeft met betrekking tot de voornoemde ziektebeelden. Het zal de deskundige duidelijk zijn, dat naast Clara celeiwit CC10 ook afgeleide eiwit-en polypeptidestructuren geschikt zijn voor gebruik overeenkomstig de uitvinding, voor zover deze structuren in hoofdzaak dezelfde biologische activiteit vertonen. De anti-cytokinine werking van Clara celeiwit CC10 zal hierna verduidelijkt worden aan de hand van een aantal voorbeelden waaruit duidelijk blijkt de immuunregulerende werking op het niveau van een blokkering van de IFN-gamma produktie, met name wanneer geïnduceerd door IL-2. VOORBEELD 1 Mononucleaire cellen (MNC) afkomstig van een enkelvoudige donor werden geïsoleerd uit verse buffy coats door centrifugeren over lymphoprep (Nycomed Pharma AS, Oslo, Noorwegen) en gekweekt in RPMI 1640 medium (Gibco Ltd., Paisley, Schotland) voorzien van 10% (v/v) door warmte-geinactiveerd foetaal kalverserum met 0. 1% (v/v) gentamicine en L-glutamine (2mM). Indien noodzakelijk werden monocyten verwijderd door hechting aan plastic (1 uur, 37 C, 5% (v/v) CO2), waarna de lymphocyt-zuiverheid werd vastgesteld door een nietspecifieke esterasekleuring. De cellen werden geincubeerd in aanwezigheid van Clara celeiwit CC10 en gestimuleerd <Desc/Clms Page number 4> na 3 uur door de toediening van een inductiemiddel. Uit de supernatant werden elke 24 uur monsters genomen gedurende 4-7 dagen. Figuur 1 toont de IFN-gamma produktie door deze mononucleaire cellen (2 x 106 cellenjml) na stimulatie met phitohemagglutinine (PHA, Welcon, Beckingham, UK), IL-2 en een gemengde leukocytkweek (MLC). Zoals blijkt uit figuur 1 is de produktie aan IFN-gamma duidelijk geremd. De IFN-gamma produktie was minimaal bij aanwezigheid van 1. 5 gjml Clara celeiwit CC10, wanneer IL-2 of MLC werden gebruikt. De mate van maximale remming varieerde van 75% (PHA) tot 95% (IL-2 en MLC). De IFN-gamma concentratie werd gemeten met behulp van een standaard Elisa test en de verkregen waarden werden gecorrigeerd ten opzichte van een internationaal standaard preparaat Gg 23-901-530-g (National Institute of Allergy and Infections Diseases, NIH, Bethesda, USA). Verder werd de IFN-gamma concentratie bepaald in een anti-virale test, onder gebruikmaking van humane A459 cellen die na incubatie gedurende 24 uur met verdunningen van de supernatant werden geïnfecteerd met encephalomyocarditis-virus (EMC-virus). De waarden werden opnieuw gecorrigeerd tegen een internationale standaard (National Biological Standards Board, National Institute of Standards and Control, Hertfordshire EN6 3QG UK, Human IL-2 Standard 86-504). VOORBEELD 2 MNC ontdaan van monocyten door hechting aan kunststof (lymphocyt-zuiverheid 95%) werden gestimuleerd met IL-2. Zoals getoond in figuur 2 trad slechts een lichte verhoging op in de totale produktie van IFN-gamma. Daarentegen bleef de mate van remming geïnduceerd door het opnemen van Clara celeiwit CC10 in een kweekmedium constant. Derhalve is de remming door Clara celeiwit CC10 niet het gevolg van inactivering van de monocytfunctie.
Claims (9)
- CONCLUSIES 1. Farmaceutisch preparaat, bevattende als actief bestanddeel Clara celeiwit CC10 of een werkzaam derivaat daarvan, en een farmaceutisch acceptabele drager of verdunmiddel.
- 2. Farmaceutisch preparaat volgens conclusie 1 in de vorm van een injecteerbare oplossing.
- 3. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik als geneesmiddel.
- 4. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik als remmer van cytokine werking.
- 5. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik in de behandeling van een autoimmuunziekte.
- 6. Clara celeiwit CC10 voor gebruik in de behandeling van thyroidytis Hashimoto, lupus erythematosus, reumathoide arthritis, multiple sclerose, chronische ontsteking.
- 7. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik : i) ter voorkoming van weefselafstoting na transplantatie ; ii) ter behandeling van infertiliteit, septische shock, allergie, biliaire cirrhosis, myastenia gravis ; of iii) ter preventie van vroeggeboorte, abortus door Gram- negatieve septicemie.
- 8. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik als diagnosticum.
- 9. Clara celeiwit CC10 of een werkzaam derivaat daarvan voor gebruik bij de bepaling van de immuunstatus van het organisme.
