Abstract

在医院中胃肠外供药的流体控制及供药装置通常要用连接器或适配件,以便于流体在两点之间流动。大多数的流体连接器和适配器应用针刺穿封盖消毒管的隔膜或刺穿流体药物容器的隔膜。流体然后从容器或装有流体的管中流入注射器或第二管装置。这些连接器或适配件通常具有机械的或活动的部件。流体可以迅速穿过连接器和适配件通常对病人的生存是特别重要的,所以必不可少的是连接器和适配件的操作要可靠和可重复。在使用期间适配件和连接器的误操作可能危及生命。机械的或活动的部件例如弹簧和隔膜越多,则它们的操作越可能出错。不正确的操作可能导致将空气栓塞引入到病人身上。因此机械部件越少,则这些连接器愈可靠,愈容易为医务界接受。

这样一种封闭的病人专用系统对医务界来说将有很大益处,即该系统易于使用,只应用与病人连通的阀装置,该阀装置不加盖或不用针和适配件与医用装置连接,该系统易于擦洗,充分耐用,在操作若干次之后仍保持其功能,而且在高压下仍保持液密密封。
发明概要

用在本发明中的密封件的一个优选实施例包括一系列叠置在一起并连接形成单一构件的O形件。该O形件具有逐渐增加的直径,最小直径的件靠近腔的近端。最靠近密封件近端的O形件在密封件处于未受压缩状态时接触钉形件的在通孔近侧的壁,因此防止流体从钉形件内通过外壳上的近侧开口泄漏,最好至少下一个中间O形件也接触在通孔近侧的钉形件。这种设计可以防止流体在密封件处于未受压缩状态时将足够大的压力施加在狭缝上,使狭缝打开。对于优选实施例,流体可存在于钉形件中,并位于钉形件和通孔远侧的密封件之间,而不会打开在密封盖上的狭缝。密封件被如此设计,即如果该流体将密封件稍为往下推,则会使第一和第二O形件往下移,并离开钉形件,由此使远侧的紧靠第一和第二O形件的O形件向上移并接触钉形件,从而保证流体不流过密封盖和流出阀门。保持与钉形件四周的接触可以避免使狭缝上的流体压力打开狭缝,从而造成阀的渗漏。

优选实施例的详细说明
术语“近”用于表示位于或靠近图2~5、10~12、14、16、24、25和27中钉形件尖端32的,和位于或靠近图6中钉形件尖端60的,以及位于或靠近图8、9、13~19、23、24、25、和27中密封盖92的阀和其它部件的端部。而术语“远”用于表示阀或钉形件尖端或密封件的相对端部。术语“医用装置”用于表示技术人员熟知的便于流体,具体为液体,流过的任何医用工具。所考虑的医用装置的例子包括管子、导管、注射器、Ⅳ型装置(外围管和中心管二者)、集装管、医用阀和其它部件,但不限于此。标准尺寸的医用装置可以在市场上买到。因此阀的一端或两端均设有接纳这种标准尺寸医用装置的配合件。

第二特征是钉形件具有尖端及位于该尖端上或附近的至少一个孔,还具有与孔连通的通道,该通道使流体可以流过该孔。该孔最好在钉形件的侧面上,靠近其尖端,该孔是细长的,具有18号的尺寸或更大。对于很多应用,该孔最好在一个以上,最好是三个对称配置的孔,位于近端的里面。钉形件位于腔的内部,该尖端嵌入位于密封件近端的密封盖上。钉形件的尖端是钝的和圆形的,以避免钉形件重复穿刺时损坏密封件。钉形件包括至少一个肋,该肋使得空气可以进入密封件和钉形件之间的空间,以利于取下医用装置时密封该开口。钉形件大体为锥形,密封件具有大体互补的锥形腔,腔内部与钉形件形状一致。
第三个特征是,弹性密封件适合于在将医用装置的头部插入开口时进入压缩状态,而在移出头部时回到未受压缩状态。在未受压缩状态的密封件其一部分基本上完全充满腔的邻接开口的部分。在压缩状态,密封件受到医用装置输送端的推压,从开口推到腔中。作为密封盖的密封部分具有预切的狭缝,在该狭缝中嵌入钉形件的近端部。医用装置的输送端和密封件适合于啮合,使得当钉形件的尖端刺穿密封件时,在上述输送端和密封件之间基本上没有任何死空间。因此预定量的药物剂量采用本发明可以完全地输送给病人,没有任何的配给的剂量停留在死空间中。在一些情况下,当输送化学治疗剂或治疗小儿时,输送准确的药量是很重要的。
图1和2清楚示出,阀10的第一实施例包括阀体或外壳12、钉形件24和密封件36。密封件36用弹性材料制作,该材料是柔性的和惰性的,不渗透液体,容易由钉形件刺穿。在图13所示的实施例中,示出另外形状的密封件36d,该密封件36d的近端部具有预切开的狭缝11。这提供了一种极细的开口,钉形件24的尖端32容易通过该开孔,在抽出钉形件时,还形成不渗透流体的密封。如图3所示,将此三个部件组装起来,使钉形件被封闭,以防止偶然的粘附。图2示出外壳12、密封件35和钉形件24如何被连接起来,而不需要使用任何粘合剂,或其它的粘合剂或方法。如随后要说明,可以获得提供不渗漏流体密封的机械连接。如图4和5所示,密封件36可在外壳12内移动,由此由钉形件24刺穿,露出钉形件24的尖端32,从而使流体流过阀10。
参照图1,外壳12的一个优选实施例具有钟形裙部16和最好为圆筒形的上部导管20。裙部16与圆环14形成整体并由该环连接于上部导管20。裙部16形成钉形件24的内部导管18的屏蔽件。内部导管18最好是圆筒形的并稍呈圆锥形。内部导管18和上部导管20包括对准的空心管,使得当钉形件24刺穿密封件36时内部导管18和上部导管20彼此流体相通。在导管20的顶端有一个包围圆形开口的25a的环形唇部25(图2)。
在阀的第一实施例中,上部导管20适合于接纳ANSI标准注射器46的端部或鼻部48(见图4和5)。然而应当考虑到,上部导管20的外径可以为任何能接纳其它连接器的连接件的尺寸。上述导管20的近端部最好装有锁定机构,以利于将阀10锁定在各种医用装置上。例如参照图1,靠近外壳12的近端唇部25的锁定耳22最好这样形成,使得外壳12可以锁定在技术人员周知的任何相配的Luer锁定装置上。例如参照图19,可以在上部导管20的外直径上设置常规的Luer锁定螺纹180。
参照图2,钉形件24的远端具有内部导管18,在其近端具有空心钉形件26,该空心钉形件26与内部导管形成整体。在使用期间内部导管18和空心钉形件26为流体提供连续的流体通道。在钉形件24的中间部分上的环形套28与内部套管18和空心钉形件26形成整体并使该导管18和空心钉形件相互连接。如图3所示,套28的边缘28a靠在内环14的下表面,并具有环形制动件28b,该制动件可以快速卡入到内环下侧的环形槽14b中。该套28起两个作用。首先它起固定装置的作用,固定在环形环14上。其次,对密封件36,它起支承和固定装置的作用。
空心钉形件26为圆锥形,结束于尖锐的尖端32。沿空心钉形件的长度最好形成凸出的脊部30。这些凸出的脊部30从空心钉形件的表面伸出,最好伸出0.2~2.0mm。脊部30最好沿空心钉形件的长度对齐,如图2所示。这些脊部30可用来破坏在密封该空心钉形件26时所形成的真空,如下面要说明的。在下面结合这些脊部的功能说明脊部的准直和取向的变化。在钉形件尖端32的远侧,配置至少一个纵向通孔34,以使内部导管18和上部导管20之间流体相通。距钉形件尖端约10mm,最好约5mm,最好有三个通孔34。这些通孔可以为任何尺寸,通孔的尺寸越大,流体流过阀10的流率便越大。在优选的阀实施例中,通孔34的尺寸是18号,以提供3倍于18号标准针的流率。
密封件36最好具有密封盖40,该盖具有大致平坦的顶表面40b、向外成锥形的侧壁38和下部唇部42。其内部是空心的,以形成锥形腔37(图3)。因此密封件36易于在钉形件24上滑移,以便将之紧贴地装在腔37内。密封件唇部42位于环形套28内,并嵌在套和环14的下侧之间。沿密封件36的长度有纵向槽43(图2),该槽形成空气袋,以利于在使用期间压缩密封件36。槽43可以具有利于密封件压缩的各种形状和尺寸。在阀的第一实施例中,仅有一个槽43,该槽完全包围密封盖40和唇部42之间的密封件36。

如图3清楚示出,具有相连之内部导管18的钉形件24通过环形套28的外部和环形环14的内部的结合而固定在外壳12上。虽然不一定需要,但这两个部件可以用技术人员熟知的各种方法中的任何一种方法固定在一起,这些方法包括热封、胶粘、压力锁定、粘接等,但不限于这些方法。密封件36嵌入环形套28,并沿外壳12的环形环14内部的内部唇部27固定就位。钉形件24的长度是使得在组装之后,钉形件的尖端停于由外壳12的唇部25确定的平面的下面。钉形件尖端32最好在外壳12的唇部25的下面约0.525~1英寸。密封件36与钉形件24紧密配合,基本上与外壳12的唇部25齐平。钉形件尖端32因此在使用之前嵌入密封盖40,或在阀10处于封闭位置时距离密封盖40约0.025英寸。内部导管18由外壳12的钟形裙部16部分屏蔽(见图1~3)。钟形裙部16的内表面上最好具有凸出的螺纹44,用作将医用装置固定在其上的可选锁定机构。另外,其它的医用装置也可以压配合在内部导管18上而不直接与凸出螺纹44接合。
在使用期间,阀被设计成用作两通阀。阀的取向不依赖于流体的流向,但取决于先存连接的优选取向。因此阀门可以在两个取向用作静脉内的中心或外围组合连接器的阀连接器。不经肠胃的流体通过管道输送给病人,使得流体从容器经刺穿的部件流到病人身上。容器频繁更换或增加附加的流体瓶子。本文公开的阀被设计成使医用装置沿流体输送路线连接于病人。然而阀还可用于任何需要再密封流体阀的环境中。在使用期间,将适当尺寸的连接器装在内部导管18上。如上所述,可以采用技术人员熟知的Luer锁定机构、压配合或任何其它锁定机构进行锁定。因此在一个例子中,流体从内部套管18流入钉形件。然而流体流动由密封件36封闭就位。
图4和5示出阀的起动。图4中,连接于阀10近端的医用装置是注射器46。然而所连接的医用装置可以是技术人员熟知的任何一号医用装置。将注射器46的鼻部48放在外壳12的唇部25里面的密封盖40上。沿箭头方向对注射器46施加压力,而在密封盖40上产生压力,如图所示。总的向下压力压缩密封件36。这样便推动钉形件26的尖端32穿过密封盖40,露出通孔34。槽38有助于这种压缩。现在取决于是从病人身上抽出流体还是将药物注入病人,流体可以流入注射器46或相反。图5示出阀10由于将注射器46的鼻部48插入开口25a而被打开。将注射器中的注射活塞抽回,由此形成真空,使流体经阀10抽入注射器。对于静脉内注射,阀10可以以图4和5示出的位置取向,或可以转动180°,使流体沿相反方向流动。
在从钉形件26上取下注射器时,如图4所示,密封件36自由地返回到其原来形状并覆盖通孔34。制造密封件36的材料的弹性决定了密封件36可以返回到其原来的形状。另外,形成在钉形件外表面上的凸出脊部30有助于密封件36返回到原来的形状。在压缩期间,在空心钉形件26和密封件36之间的区域内可能形成真空,因此妨碍密封件36返回到其原来位置。凸出的脊部30使空气可以沿着空心钉形件/密封件的界面流动,从而可阻止真空的形成,允许密封件36自由地恢复其形状。密封件36能够可逆地变形并回到其原来的位置是特别有用的,因为(1)它直接地停止了流体流过阀10的流动;(2)它覆盖了凹进的钉形件;(3)它减少了钉形件偶然刺伤其它物件或人的危险性。另外,因为阀10除密封件外缺少活动部件,所以在往下推压密封件36时,阀10不大可能发生错误操作。
通孔34最好位于空心钉形件26上相对较低的位置。因而当阀10关闭时通孔34在密封件36恢复到其原来形状的过程中可以较早地被密封。在一个阀的优选实施中,通孔34在钉形件尖端32下面0.075英寸处(见图2)。另外,即使密封件36没有完全回到其如图4所示的原来形状,通孔34也能够被密封。另外,密封件36逆向返回到其原来位置的能力使得可以重新使用阀10。在脱开连接后和重新使用之前,被刺穿的密封盖40的表面基本上与外壳12齐平。这种平齐的表面有利于用酒精或其它的表面去污剂消毒。裙部16和上部导管20最好使两个连接处与四周环境隔离,以保持连接处的无菌性。另外,裙部16和上部导管20均起着收集容器的作用以防止在操作期间流体渗出阀10。
可以在上部导管20上装一个帽盖(未示出),以便在不使用期间保护密封表面。然而这种帽盖不必保持无菌,因为密封件36可在每次使用后用杀菌剂擦洗。密封件36的可逆性使得阀10特别适合于用作使两个流体管连通的连接阀。因此采用本文公开的阀可以使第一流体管通与第二流体管道连通。阀10的可逆性允许,例如,相继地增加多个流体管到一个直接与病人的静脉连通的流体管上。因为阀容易消毒和密封,所以可以增加流体管和除去流体管而不必脱开与静脉的连接。
阀10最好用硬塑料例如ABS塑料制作,但还可以考虑用其它技术人员熟知的药物上为惰性的材料制作。钉形件24最好用外壳12的材料制作。然而最好用更硬的材料例如聚碳酸酯制作钉形件24,以使它能穿透各种连接隔膜和密封件。这种阀的一个特别的优点是,它不依赖于使用金属钉。这样便减少了使用和制造期间刺伤皮肤的危险。另外,上部导管20用作空心钉形件26的屏蔽件,从而进一步减少了皮肤与空心钉形件26的接触。钉形件只需要足以刺穿密封盖40,或如果需要再刺穿连接隔膜。
在图2~4所示的阀的实施例中,通孔34位于钉形件尖端32的远侧。这种位置形成两个重要优点。第一,通孔34的位置有利于在使用后重新密封阀10。第二,如果通孔位于钉形件尖端32,则孔34可能由密封盖填入,由此可能将密封件的颗粒子引入到流体流中或可能堵住孔34。因此通孔在钉形件尖端32远侧的纵向位置可以防止将颗粒引入流体路径中和/或堵住通孔34,还另外考虑到,可以调节通孔34的数目和直径,以调节不同的流速。在阀的优选实施例中,穿过通孔34的流体的优选速度等于或大于穿过18号针的流速。大于18号针的通孔当然有利于较大的流速。
阀10的重要优点是、它基本上没有死空间,因此进入阀10的流体量基本上等于流出阀10的流体量。另外,阀中总的流体等效体积是很小的,因而为了使阀10与医用装置例如注射器46流体连通而流过系统的流体体积基本上为零。
在阀的另一优选实施例中,如图6和7所示,提供了一种起流体容器(未示出)的可再密封盖作用的一次性无菌适配阀50。因此可以从流体容器中抽出流体,或使流体从容器中抽入适合于以无菌方式容纳流体的医用装置。按照常规作法,容器的开口通常用覆盖件(未示出)密封。
图6示出适配阀50,该阀具有包括适配阀裙部52的外壳。适配阀裙部52最好紧密地装在容器的开口上。裙部52可具有能接纳各种尺寸容器的任何尺寸。最好在至少一个沿裙部长度的位置上设置一纵向槽54,以保证在裙部52和容器之间的紧密配合。形状最好为管形的腔56从裙部52向上延伸,它在结构上和设计上类似于阀的第一优选实施例的上部导管20。与阀的第一实施例类似,阀的近侧部分可以包括锁定机构59,该机构最好包括Luer锁定装置或其它技术人员熟知的锁定装置。
如图7所示,钉形件58向上延伸通过管状腔56。钉形件尖端60最好凹进管形腔56的近侧唇部62内。在封闭位置时,该尖端60由密封件64覆盖,该密封件64基本上与密封件36相同。突出的脊部66和密封槽68有利于密封件的压缩和使用后的密封。因此在图7所示的密封位置,密封件64覆盖通孔70,从而防止液体从容器中流出。适配阀50包括第二钉形件72,该钉形件的指向与钉形件58相反。这些钉形件52和72彼此流体相通。钉形件72向下伸入适配阀裙部52。两个钉形件最好形成阀50的一个部件,而裙部52和上部腔形成第二部件。这两个部件的组装方式与阀10相同。钉形件72如钉形件58一样具有纵向通孔74和尖端76。通孔74位于尖端76的内侧。因此适配阀50可与装有无菌药物的容器联用,在该容器的开口上具有盖和隔膜密封件。可与这种阀联用的具有这种密封盖的容器例子包括用于肌肉注射器的配料瓶、抗菌素容器等。然而还可想到,阀50可以装上自身的密封装置和锁定机构,以使阀可以用于各种药物或其它流体的容器上。这些容器中的药物最好保持在无菌条件下,并且药物的体积和特性是使得可以在时间过程中断续取出多个等分部分。如果在使用期间重新配药,则可以除去盖在容器开口上的盖子,露出橡皮膜。将适配阀50放在该膜上,并直接施加压力,使远端钉形件72刺穿该膜并进入容器。然后如图5所示,使注射器等与第一优选阀的实施例相结合,将流体从容器中抽出。鼻部48压在钉形件58上的压力使钉形件尖端60穿过密封件64。在此同时密封件64被压缩。密封件的槽68接纳压缩的料。从容器中抽出流体并从钉形件58上取下注射器。松开施加在密封件64上的压力可使密封件64恢复到原来的形状。钉形件的脊部有利于密封件64的运动。
装在容器中的成份通常是在采购时可冻干的成份。这种冻干的成份在使用前需要配制。如果使用前药物需要重新配制,则在抽出流体之前需要将无菌水、盐水或其它流体加入到容器中。阀的两通特点允许作此操作而不用其它特殊适配连接。在取下注射器以后,适配阀可以自动密封。随后用注射器等从容器中取出等分药液。可以用酒精或其它相容的表面消毒剂在每次使用前擦洗唇部62和密封件64。与阀的第一实施例相同,还可以考虑在不使用期间将一个盖覆盖在上部腔的唇部62上。
适配阀50适合于用作静脉药物容器的药物适配件。在这种情况下,适配阀50放在向静脉内送药的药物容器上,并用管子连接于通向静脉内的输送器。因此适配阀50可以配置成与图1所示的连接阀流体相通,以利于使药物从静脉滴瓶中流出。
密封件的另一个实施例,即密封件36a示于图9。密封件36a包括其近端的密封盖92和其远端的密封盖96。环形的环形法兰95位于密封盖92的近侧。密封盖92和96由密封壁连接。该壁包括许多环形壁部分94,该壁可膨胀和收缩成折皱状。在压缩密封件36a期间,环形壁部分94的直径沿径向方向向外膨胀。空气袋13a(图10)存在于环形部分94和外壳之间,而空气袋13b存在于空心钉形件24和密封件36a之间。密封件包括位于密封盖92远侧并邻接环形壁部分94的腔98。密封件36a与钉形件26(图2)和阀的其它部件以类似于图2的密封件36的方式相互作用。
参考图10,杯状环形法兰95可以沿上部导管20伸长并用圆环97固定就位。这样便形成一种蹦床效果,有助于在抽出注射器(未示出)之后使密封件36a恢复到减压的状态。此实施例具有两个优点。第一,阀10的近端部可以用酒精或其它消毒剂擦洗,而消毒剂不会渗入阀10。第二,用圆环97将杯状环形法兰95固定在上部导管20的近端部上有助于密封件36a的重复变形和回复。
在图11所示的密封件的另一实施例中,示出了阀10的密封件36b。密封件36b类似于图9和10所示的密封件36a,如密封件36a由密封盖92、由环形壁部分94构成的侧壁以及密封唇部96构成。密封件36a还也具有向外伸出的环99,该环与阀10的纵轴成直角。环99用于将密封件36a连接于上部导管20。上部导管的环形塞20’插在上部导管20内,从而在垂直环99,上部导管20上的凸缘101和环形塞20’之间形成密封配合。环99有助于密封件36b在抽出注射器(未示出)后使密封件36b复原而封闭钉形件36。
如图12所示,杯状环形法兰95和环99二者均可用在阀10上,形成密封件36c。该密封件36c在抽出注射器(未示出)后可以很快复原,并具备密封件36a和36b二者的优点。
图13示出密封件另一个替代实施例,即密封件36d。在此实施例中,密封件36d由密封盖92、盖封唇部96和侧壁150构成,该侧壁由串联叠置在一起的圆形胎100构成,一个胎位于相邻的较大直径的下部胎上。圆形胎100最好在其横截面的整个直径上是实心的。这些圆形胎100在分别压缩和减压密封件36d时便发生变形和复原,因而可以根据需要而使钉形件(未示出)露出或被覆盖。
如上所述,密封件36d最好具有预切的狭缝11,该狭缝位于盖92上,沿着阀10的纵轴定位。密封盖92具有独特的构形,一旦抽出注射器(未示出)和密封件36d复原后该构形可以保证狭缝11关闭和密封。它包括加大的内部压力响应件200,该响应件与密封盖92形成整体。在侧壁150的近端部和响应件200之间是环形空间102,该空间由腔98中的流体充满。该流体处于压力状态下,例如处于阀10所连接的病人的血压下,参照图14,例如病人的血液流过钉形件26上的孔34,充满腔102。该流体压靠在件200的外侧,使得在密封件不受压缩时可以封闭狭缝11,如图14和19所示。该流体的压力形成高压密封,这种密封可以防止流体通过狭缝逸出阀10。在响应件200的端部有半圆筒形的环形凸出撕开环104,该环有利于延长密封件36d的寿命。
该撕开环104最好与部件200形成整体,沿着部件200的内表面的周边,并在密封件的外表面上有一稍呈盘形的凹部204。压力响应件在未受压缩的状态下可以密封在密封件36d上的任何开孔,从而在未受压缩状态下形成大体液密密封。压力响应部件200即使在施药过程中有时遇到的很高压力时也能使阀保持液密密封,特别是在阀10与病的动脉相连接时保持密封。部件200的中心和环形空间102与狭缝11的进入通道11a是共轴的。压力流体充满环形空间102,从而施加压缩该部件200的压力,从而完全封闭狭缝11的进入通道11a。在优选的阀实施例中,从入口通道11a到密封盖92近端的距离在0.500~0.075英寸之间,最好约为0.100英寸。
如图22所示,尖端32被设计成避免撕裂密封件。尖端32具有三个沿分隔线a、b和c彼此结合的平圆面210、212和214。该平圆面210、212和214的该结合部经常是参差不齐的并将撕开该密封件36d。利用分隔线a、b和c或结合部可以防止这种撕开,这些分隔线或结合部分别配置在凹部220、222和224中,形成“埋入的分离线”。
在图8和19~21中示出采用密封件36d的另一替代实施例的阀10。在此实施例中,导管20上端部的内壁160具有至少一个,最好具有多个径向凹槽107。凹槽107是细长的,一般配置成对称的星形结构,平行于阀10的纵轴。每个凹槽具有相对的侧边缘162,在压缩密封件36d时该边缘啮合密封件36d。该凹槽提供了密封件36d压缩时膨胀进入的空间。
图23~25和27示出密封件36h的另一个优选实施例。在此实施便中,密封件36h包括密封盖92,该密封盖92具有盘形的凹部204(图23)。密封件36h具有狭缝11,该狭缝具有靠近凹部204的近端部和位于密封盖92远端部的远端部11a。参看图23,设置类似于图13中的圆形胎100。密封件36h具有内腔98。另外,密封件36h最好具有如下面要详细说明的密封唇部96。
图8清楚地示出,上部导管20近端部的壁181向内成圆锥形,其锥角与注射器46的鼻部48的锥角相同。按照ANSI标准,该圆锥是0.006英寸每直线英寸。注射器鼻部48的壁182在鼻部滑入开口25a中时靠着壁181,从而向内推压密封件36d,将其压缩并迫使钉形件36的尖端32进入狭缝11。压缩时该密封件36d膨胀,从而基本上完全充满凹槽107的上部分密封件36d的一些部分嵌在边缘162之间,而其它一些部分充满凹槽107。当液体径孔34流过鼻部48时,在鼻部48中的空气被压出鼻部48,并经壁181和182之间从阀10排出。因此整个的处方配剂基本上通过阀10输送给病人。流体流过通孔34,但不管在密封件36d和壁181之间还是在相接壁181和182之间均不发生渗漏。
图15、16、17和18分别示出密封件的实施例,即密封件36e、密封件36f和封闭件36g,这些密封件除应用圆形胎100的侧壁150取代褶状壁部分94而外基本上与密封件36a(图10)、密封件36b(图11)和密封件36c(图12)相同。
阀的其它部件和密封件的各种实施例的相互作用方式类似于与图2的密封件36的相互作用方式。在使用阀10之前,最好沿轴向方向用钢针居中刺穿密封盖40或92,预切密封件形成狭缝11,以便可以在用钉形件26进行刺穿时使密封件更快地进行减压和复原。密封件最好用这样一种材料制作,这种材料可以重复地再密封并防止流体沿密封材料流动。密封件36还应当能够向下压,然后弹性返回到原来位置,重新密封阀门。太软的材料不能有效地再密封,而且在打开阀门之后不能弹性返回。太硬的材可以提供充分的弹力,然而不能有效地密封。因此在优选实施例中,密封件用硅酮制作,该材料的硬度的范围在30~70肖氏硬度计单位内,最好在40~50肖氏硬度计单位的范围内。在优选硬度范围内的硫化硅酮聚合物可从密执根州的Adrian市的Wacher Silicone Corp.公司得到。在有些实施例中,还希望给密封件36提供附加的润滑性,以使它能更有效地返回和重新密封。Dowchemical Co.公司生产一种其中含有硅油的硅酮制品,可提供这种附加的润滑性。
一般地讲,阀10的密封不是由直接覆盖通孔34的密封件36的侧壁提供的,而是由塞满腔98的近端部和开口25a的密封盖40或密封盖92提供的。密封盖40和92应当充分厚,以便在阀封闭之后有效地再密封开口25a。然而密封盖40和92还应当充分薄,以便它们容易返到封闭位置。盖40和92的厚度在0.075和0.500英寸之间比较好,最好约为0.100英寸。
该阀可以作成无菌和一次性使用的形式,使得在一给定的装置内用完之后,便将阀装置扔掉。然而,如上所述,在任何一种给定的装置上,阀可以重复多次使用。因为阀不用针,所以阀装置基本上没有可能发生偶然刺伤皮肤的事情。从上述说明可以明显看出,该阀已几乎完全不用医务上所用的针。采用上述阀,除直接插在病人身上的针而外,已完全不需要其它针。
阀10被用来提供一种封闭的用于将预定量的药物从远距离的药源输送给病人的病人专用系统。阀10通过将其远端部连接于病人,例如连接于与阀流体相通的静脉或动脉。血液充满阀,但例如密封件36d将防止任何血液渗出阀。医用装置的输送端或鼻部48插入到图8所示的阀中,使鼻部48推压密封件,由此密封件受到充分压缩,使空心钉形件24的尖端32刺穿密封件并进入上述输送端。现在预定量的药物便完整地经鼻部48传送到阀10,并送给病人。因为鼻部48和密封件36d啮合的方式是使得空心钉形件24的尖端32在刺穿密封件时将接触密封件,从而避免在鼻部48和密封表面40b之间的界面上形成任何死空间。因而可以达到基本上整个预定量的药物经阀直接从注射器46传送到病人,基本上没有任何上述预定量的药物存留在阀的任何死空间中。从阀10中抽出鼻部48时,密封件36d回到未受压缩状态,从而关闭阀,同时在上述未受压缩状态下保持液密密封,即使在高压条件及重复使用后也保持密封。
图23示出另一个密封件的替代实施例,即密封件36h。在此实施例中,密封件36h类似于36d,由密封盖92、密封唇部96和侧壁150组成,该侧壁由串联叠置在一起的圆形胎100组成,其中一个胎置于一个相邻的较大直径的下部胎上。侧壁150形成腔98。圆形胎在其整个横截面直径上最好是实心的。这些圆形胎在对密封件36h进行压缩和减压时将分别发生变形和复原,由此分别露出和覆盖钉形件(未示出)。
密封件36h在密封盖92上具有预切开的狭缝11,该狭缝沿着密封件36h的纵轴。狭缝11在密封件36h处于未受压缩状态时保持密封。如上所述,预切密封件形成的狭缝11可以在钉形件穿刺时更快地减压和复原。然而和密封件36d不同,密封件36h的密封盖92基本上是实心的,没有如用在密封件36d的密封盖92中的任何压力响应部件。
图4示出本发明的采用密封件36h的可选实施例。图中示出的钉形件26a更像管子,与其它实施例中示出的钉形件26相比具有较小的截头锥形,该钉形件26a装在腔98中并具有带尖端32的近端部,尖端32嵌入密封盖92内。另外,钉形件26a的尖端32和钉形件26的锐尖端不同,它是钝的圆形端部。因为端部是圆的,所以密封盖不会因钉形件尖端32的刺划而受损。因此例如如图14所示的密封件的撕裂环对于本实施例是不需要的。
本实施例的另一特征是当密封件36h处于未受压缩状态时钉形件26a与密封件36h的配置。在这种状态下,钉形件26a的圆端部32配置成嵌入狭缝的入口通道11a内,同时狭缝11则对任何流体保持密封。图24示出与密封盖92远端部接触的整个圆端部32。另外,最靠近密封件近端部的侧壁圆形胎即胎100a接触钉形件26a的侧壁。还希望至少下一个挨着的远端圆形胎即胎100b也接触在通孔34近侧的钉形件26a。使多个胎与在通孔34近侧的钉形件26a接触可以防止流本从腔98流到阀10的近端部。没有这种设计,流体将经通孔34泄漏,由此将足够大的流体压力加在狭缝11上,迫使狭缝打开,而此时密封件仍处于未受压缩状态。通孔34应当位于接触钉形件26a的胎100a、100b的远侧,使得穿过通孔34的流体不能将压力加在狭缝上,而是利用圆胎进行密封,从而在钉形件26a和密封件36h之间形成密封。
在送药期间,例如当阀10连接于病人的动脉时,病人的血液流过钉形件26a的通孔34,充满腔98中的在第二胎100b远侧的区域。因为存在于头两个胎100a和100b之间和密封盖92和胎100a之间的流体体积很小,所以流体不能将足够大的压力加在密封盖上,使狭缝11打开。预切的密封盖92被设计成直到流体压力达到20磅/英寸2仍保持关闭。因此血压不能打开阀10。
然而当阀10的远端连接于病人动脉时,若血液向上推密封件36h,则流体可能迫使密封盖92向近侧移动,由此也向近侧方向推压侧壁胎100。该压力可能使血液流过胎100a和100b,使压力作用在狭缝11上。然而,由于增加的流体压力,在第一和第二胎100a和100b远侧的最靠近的胎将向近侧移动,并接触钉形件26a,占据胎100a和100b原来的位置,从而保证有多个胎总是与钉形件26a接触。因为密封件36h的侧壁胎100被设计成从近端到远端向外侧弯,所以在胎100a和100b远侧的最近的胎当其位于其原始位置时可能不与钉形件26a接触。然而,技术人员很清楚,如果流体流过钉形件26a、通孔34,并进入密封件36h的腔98,迫使密封件36h向近侧方向移动,因而在第一胎100a和第二胎100b远侧的胎也将向近侧方向移动,并在通孔34的近侧接触钉形件26a,由此增强钉形件26a和密封件36h之间的密封。即,在阀10的腔内不包含流体时,仅有第一胎100a和第二胎100b接触钉形件26a。然而一当流体进入阀10的腔98中时,密封件36h可能向近侧方向移动。如果发生这种移动,则除第一胎100a和第二胎100b外,在第二胎100b远侧的最近的胎也接触密封件36h,从而增强密封件36h和钉形件26a之间的密封,并防止流体通过钉形件26a、通孔34流入腔98,并流过胎100,将压力加在密封件36h的密封盖92上的狭缝11上。
图25示出外壳的替代实施例即外壳12a。在该部分截面图中,除沿上部导管20内壁的纵轴线配置的槽303、304外,外壳12a类似于外壳12。槽303、304作为流体逸出空间设置,从而保证在密封盖92和上部导管20的内壁305之间不形成完全的密封。槽303、304最好从上部导管20的近端伸向远端,通过上部导管20与密封盖92相接触的部分。从图28可以清楚看到,槽303最好从外壳12a的上部导管20的近端部向远侧伸向径向凹槽107的近端部。
设置流体逸出空间的优点是,可使停留于密封件36h和上部导管20之间的空间内的任何流体在压缩密封件36h时可以排出外壳。参照图25,在例行使用阀10输送流体期间,流体可能渗入在密封件36h和上部导管20的壁305之间的外壳12a的部分中。当此区域充满流体和密封盖92由医用装置(未示出)压缩到远端时,使用者在将密封盖92向远侧推过钉形件26a的通孔26a时将会遇到困难,因为侧壁胎100在上部导管20内由于液体的存在而不再有任何可被压缩的空间。很不希望要求使用者用特别大的力来压缩密封件,因为使用者通常将向下对着密封件转动医用装置,这将导致密封件的损坏,最后被撕裂。另外,在密封件36h和外壳12a的上部导管20的内壁305之间的流体可能妨碍密封件36h在钉形件26a的通孔34的远侧被压缩。结果,阀10将不能正确操作。
由于形成槽303、304作流体逸出空间,所以在密封件36h和外壳12a的上部导管20的内壁305之间存在的液体在用医用装置(未示出)压缩密封件36h时将向近侧流过槽303、304。当流体从阀10经槽303、304从外壳12a的近端部排出时,便可以正常压缩密封件而无需在阀10上加过大的力。
图26a是图25所示阀的顶视平面图。在阀10的外壳12a的上部导管20上示出槽303、304。重要的是,当密封件36h由医用装置(未示出)向远侧压缩时,密封件36h不膨胀进入槽304、304而防止流体流过。
图29示出外壳的另一替代实施例即外壳12b。外壳12b应用通道307作流体逸出空间,该通道大体垂直于阀10的纵轴线。通道307是一个孔,它横向穿过上部导管20的侧壁,并位于Luer锁定螺纹309或其锁定机构的远侧,这些锁定机构在上部导管20的远端围绕上部导管20。与槽303、304相类似,通道307在密封件36和上部导管20的内壁305之间的区域内形成一个流体通道,因此在侧避100被压缩并膨胀进入径向凹槽107时流体流出。因为存在通道,可使流体流出该区域,所以使用者在将医用装置推入阀10向远侧推进时不需要施加过大的力。
图26b是图29所示阀10的顶视平面图。通道307用虚线示出,并最好配置阀10的外壳12b的上部导管20上。在医用装置(未示出)压缩密封件36h时,在上部导管20和密封件36h之间的流体将从阀10经通道307排出,流到上部导管20的侧壁的外侧。因此,通道307与其它槽的明显不同之处在于,流体通过侧壁排出而不是通过外壳12a的近端部排出,例如不像槽303那样。
技术人员容易理解,可以联合使用槽和通道,以便于在医用装置压缩密封件时从阀中排出流体。例如,在压缩密封件时,流体可以流过在近侧的槽,然后流过与该槽连通的通道。通道可以配置在阀近端部的远侧。技术人员可以很容易地看出,在本发明的阀中可以包括一个单一的槽或通道,或应用多个槽或通道。
如上所述,缺少通道或槽可能造成密封件36的损坏并防碍密封盖92被完全地推压到通孔34的下面。如果通孔没有完全打开,则病人不能接受到恒定流速的药物。在某些情况下,以预定的流量输送准确量的药物对治疗是很重要的,因此通孔34必须完全打开,以便流过从医用装置来的药物。通道和/或槽可以保证将密封盖推到通孔的远侧,并且可以压缩密封件,而不需要过大的可能造成密封件损坏的力。
Claims (9)
1.一种医用阀,包括:
包括壁构件的主体,该壁构件限定了一个具有内部和外部的内腔,上述主体还具有近端部和远端部,上述近端部具有充分大的开口,以便接纳医用装置输送端部的顶部,该医用装置使流体流过上述输送端部,上述主体在上述壁构件上具有流体逸出空间;
钉形件,具有尖端、在上述尖端上或附近设置至少一个孔和与该孔连通的通道,该通道使流体可以流过上述钉形件;
弹性密封件,在上述腔内包围上述钉形件,上述密封件在医用装置的顶部插入上述开口时进入压缩状态,上述密封件有充分的弹性,在从上述开口中取下医用装置的顶部时,该密封件可以返回到未受压缩的状态;
其中,上述流体逸出空间在上述密封件处于上述压缩状态时与上述腔的外部流体连通。
2.如权利要求1所述的医用阀,其特征在于,上述流体逸出空间包括至少一个槽。
3.如权利要求2所述的医用阀,其特征在于,上述槽位于上述主体的上述壁构件上,并结束于上述主体的近端部。
4.如权利要求1所述的医用阀,其特征在于,上述流体逸出空间包括穿过上述主体的通道,上述通道位于上述主体近端部并远离上述开口。
5.一种使第一流体流过医用阀的方法,上述阀包括具有内腔的主体,该腔具有内部和外部,上述腔还具近端部和远端部,上述近端部具有充分大的用于接纳医用装置输送端部的开口,该医用装置将流体传送通过上述输送端部,上述阀还包括位于上述主体内腔内的密封件,上述阀包含在上述腔内的在上述密封件和上述主体之间的第二流体,上述方法包括以下步骤:
a.将医用装置的顶部插入上述主体近端部上的开口中;
b.向医用装置施加力,由此向远侧方向压缩上述密封件;
c.通过在上述阀的上述主体中的流体逸出空间将上述第二流体排
出上述腔之外;
d.通过上述医用阀输送上述第一流体。
6.如权利要求6所述的方法,其特征在于,在步骤(c)包括通过在上述主体的上述壁构件上的槽排出上述第二流体。
7.如权利要求6所述的方法,其特征在于,上述槽从上述主体的近端部向远侧延伸;
8.如权利要求5所述的方法,其特征在于,步骤(c)包括通过在上述主体的上述壁构件上的通道排出上述第二流体。
9.如权利要求8所述的方法,其特征在于,上述通道位于上述主体近端部的远侧。
