通常,本公开的系统的示例包括用于将机械能从手柄传递到匣盒的机械设计和构造,以用于操纵人体或动物受试者体内的组织和解剖结构或其他结构,以达到治疗疾病或病症的目的。手柄和匣盒配合以在组织内递送植入物或锚组件。该匣盒被配置为承载锚组件的部件。多个匣盒可以与单个手柄一起使用,使得在对个体对象进行手术期间,可以使用该单个手柄将多个锚组件部署在组织内。

在一个示例中,远侧锚部件70的中段80提供从头部72到尾部74的结构过渡,并且在中段80的区域中去除了侧壁的一部分。侧壁的另一部分被移除以限定从中段80延伸的尾部74的连接器段82。在一个示例中,该连接器段82包括产生正交定向构型的弯曲部。因此,在其预植入形式中,锚组件可以包括远侧锚部件70,其与缝线部分78的初始接合是大致同轴的。
仍参照图3,在一个示例中,近侧锚部件84包括夹住缝线部分78的尖头96。尖头96的内部结构用于破坏缝线部分78的表面,既压入缝线部分78中又在尖头96之间压缩缝线部分78。凸片98可以从一个或多个尖头96延伸,以帮助在近侧锚部件84和缝线部分78之间形成牢固的接合。

当针组件210相对于轴组件202滑动时,针组件210和轴组件202之间的摩擦有多种来源。例如,沿轴组件202的内表面和/或针组件210的外表面的不连续性或缺陷可以增加针组件210和轴组件202之间的摩擦。摩擦的一个重要来源是施加在针远侧部分212上以使其沿其近侧长度的大部分大致笔直的约束力。即,针远侧部分212的定形构型包括针远侧部分212在处于不受约束状态时呈现的预定曲率半径。该预定曲率半径被设计成使得针远侧部分212相对于针远侧部分212从轴远侧部分204的远侧出口点以特定角度(或角度范围)和特定位置(或位置范围)而穿透组织。由本文描述的系统的示例执行的治疗良性前列腺增生的方法依赖于针相对于轴组件202的长轴线穿过组织的大致横向路径。
预定的曲率半径使针远侧部分212能够沿着这种大致横向的路径穿透组织。然而,如本文所述,当针远侧部分212在轴远侧部分204内时,针远侧部分212的近侧长度的大部分被限制为基本上笔直。虽然针远侧部分212相对柔性,但限制针远侧部分212确实沿轴组件202的内表面和轴远侧部分204的内表面产生多个接触点。

如本文别处所述,在一些示例中,多个匣盒可以以串行方式与单个手柄一起使用。参照图12A、12B、13A和13B说明和解释的步骤和机构公开了对于一系列匣盒与单个手柄的多次接合,如何重复执行产生锚组件的推动和切割步骤。一个显著的特征是植入物弹簧作为其初始状态处于拉伸状态。因此,对于匣盒而言,重要的是具有防止在搬运匣盒期间植入物弹簧中的能量意外释放的特征。
图14是用于治疗良性前列腺增生的系统的匣盒的一个示例内的某些机械特征的侧视图。图14示出了图13A中(在图13A的构型中)描绘的机构,添加了盒旋钮203以显示这些特征之间的操作关系。图14示出了处于存储位置的匣盒旋钮203。与图1所示的接合位置相比,在存储位置,握持部分292横向于匣盒的长轴线,在所述接合位置中,握持部分292与匣盒的长轴线对准。当处于存储位置时,匣盒旋钮203用作防止推动器块244移动的安全联锁装置。在如图14所示的存储位置中,匣盒旋钮203上的推动器安全凸片293与植入物致动器246的远端接合并防止植入物致动器246枢转以释放推动块244(如关于图12A和12B所示和解释)。推动器安全凸片293允许在植入物弹簧处于其延伸构型时安全地搬运和储存匣盒。
图15A是用于治疗良性前列腺增生的系统的匣盒的一个示例内的某些机械特征的分解俯视图,并且15B是相同特征的分解透视图。图15A和15B示出了另一种安全联锁装置,其中切割器安全凸片286存在于切割器爪236上。当匣盒旋钮203处于存储位置时切割器安全凸片286与匣盒旋钮203接合并且防止切割器爪236枢转以释放切割器块(如图13A和13B所示和解释)。切割器安全凸片286允许在植入物弹簧处于其拉伸构型时安全地搬运和储存匣盒。图15A和15B以分解图示出以使相关特征清楚。然而,在操作中,切割器爪236与匣盒旋钮203紧密接合以产生本文所述的机械相互作用。
推动器安全凸片293和切割器安全凸片286通过将安全互锁装置与匣盒旋钮203的存储位置相关联来解决匣盒中的推动器组件和/或切割器组件意外触发的问题。
图16A、16B、17、18A和18B示出了手柄和匣盒系统中的另一个安全互锁机构的示例。图16A是手柄100的一部分的透视图,并且图16B是手柄100的侧视图。手柄100包括匣盒仓101,由于匣盒已被移除,匣盒仓101被示为空的。在该示例中,手柄触发器组件110包括匣盒锁定凸片111,其当手柄触发器组件110枢转远离手柄握把115时突出到匣盒仓101中。匣盒仓101中的匣盒锁定凸片111的存在防止匣盒与手柄100接合,这是因为匣盒由于匣盒仓101中的匣盒锁定凸片111而无法物理地装配在匣盒仓101中。该机构防止匣盒插入手柄中,除非在适当的条件下——当手柄触发器组件110处于其最靠近手柄握把115的初始位置。该初始位置对应于手柄中的所有机构都被复位到它们的初始位置。

图18A和18B分别是用于治疗良性前列腺增生的系统的匣盒的一个示例内的某些机械特征的透视图和侧视图。图18A和18B示出了存在于手柄触发器组件110上的匣盒锁定凸片111与存在于轴支撑件297上的匣盒锁定表面291之间的相互作用。当手柄触发器组件110处于枢转远离手柄握把的位置时,匣盒锁定凸片111通过物理地阻止匣盒的移除来防止匣盒从匣盒仓中移除。尽管在该示例中匣盒锁定表面291存在于轴支撑件297上,但类似的锁定表面可以存在于匣盒的其他部分上,例如匣盒底座298,并且以基本相似的方式起作用。
存在于手柄触发器组件110上的匣盒锁定凸片111提供了一种在手柄处于复位、初始状态之前防止匣盒插入的方法,并且在使用手柄和匣盒系统时防止匣盒的移除(通过与匣盒锁定表面291的相互作用)。存在用于防止盒的移除和/或插入的替代示例。例如,匣盒可以包含锁定接收特征并且手柄可以包含具有一系列锁定特征的附加轮。当拉动手柄触发器时,附加轮旋转,使得锁定特征与锁定接收器接合。如果一个完整的植入循环需要多次触发器挤压,那么附加轮包括足够的锁定特征以在整个循环期间保持匣盒锁定。在循环结束时,不包含锁定特征的附加轮的一部分与锁定接收器对准。以这种方式,匣盒将被解锁并从手柄上移除。作为另一个示例,用户可以接合锁定滑块以将匣盒锁定在手柄中。当触发器被挤压时,滑块将被机械接合以防止滑块移动,直到循环完成为止。
图19A和19B是用于治疗良性前列腺增生的系统的一个示例的匣盒的一部分的侧视图。匣盒旋钮203与匣盒盖299接合,匣盒盖299包括匣盒指示器窗口290。在图19A中,切割器块234通过匣盒指示器窗口290可见,这表明切割器块234仍处于其初始位置并且没有向近端移动以在形成锚组件的最后步骤中切割缝线。图19A显示了切割器块234填充了匣盒指示器窗口290。在图19B中,只有分切割器块234的一小部通过匣盒指示器窗口290可见。也就是说,切割器块234不再填充匣盒指示器窗口290。这表明切割器块234已经向近侧移动并且切割缝线以用于形成锚组件的最后步骤。

图20A是用于治疗良性前列腺增生的系统的匣盒的一个示例内的某些机械特征的分解透视图。图20B是处于其接合构型的那些机械特征的侧视图。如本文关于图13A和13B所描述的,在正常操作中,切割器爪236枢转并与切割器块脱离,从而允许切割器块在植入物弹簧的力下向近侧移动。切割器块的近侧运动导致缝线被切割以形成锚组件的缝线部分。然而,在一些情况下,用户可能希望在部署远侧锚组件之后但在部署近侧锚组件和切割缝线之前中断部署程序。在这种情况下,则可以从患者移除手柄和匣盒系统,而无需完全组装和部署植入物。
图20A和20B示出切割器爪236包括切割器手动凸片284,用户可通过匣盒旋钮203上的切割器进入窗口288接触该切割器手动凸片284。切割器手动凸片284配置为偏心斜面,使得通过切割器进入窗口288在切割器手动凸片284上的推动产生垂直于推动方向的切割器爪236的旋转(或枢转)。用户可以用插入通过切割器进入窗口288的小工具推动切割器手动凸片284。匣盒旋钮203中靠近切割器进入窗口288的凹口将工具导向切割器进入窗口288。如本文所述,切割器爪236的这种枢转使切割器爪与切割器块脱离接合并且允许切割器块在植入物弹簧的力下向近侧移动以切割缝线。
所公开的装置可以在用于各种医疗目的的各种治疗装置中实施,包括但不限于缩回、提升、压缩、逼近、支撑、重塑、重新定位、消融或以其他方式改变组织、器官、解剖结构、移植物或在人类或动物受试者体内发现的其他材料。在某些示例中,治疗装置旨在置换、压缩、缩回或破坏前列腺组织以促进疾病或病症例如BPH的治疗。
其他治疗装置可受益于本文公开的示例的使用。配备有各种工具的治疗设备可以操纵、消融或以其他方式改变组织,其中这些工具由机械能移动、部署或驱动,可以受益于安全联锁装置和机构的使用以及在此公开的部署指示器的使用。此类工具可包括但不限于针、切割刀片、真空器、抓握臂组件、可扩张切割构件、钝解剖器、套索或结扎夹、具有一体式或可缩回刀片的铰接头、螺旋刀片、用于输送射频能量的电极、切割线或环、电灼探针或钉或缝线输送头。
在一些示例中,该系统包括承载至少一个植入物的匣盒和配置为接收匣盒的手柄。手柄包括致动器和至少一个加载有机械能的弹簧机构。手柄还包括与匣盒配合以将机械能从弹簧机构传递到匣盒以部署植入物的构件。手柄和匣盒系统包括第一击发滑动件,该第一击发滑动件具有与针组件上的推片对准的狭槽。第一击发滑动件的槽和针组件的推片是互补的机构,其允许来自弹簧机构的能量通过第一击发滑动件传递以在匣盒中击发针。手柄和匣盒系统还可以包括第二击发滑动件,该第二击发滑动件具有与缝线管或连接管上的推片对准的狭槽。第二击发滑动件的狭槽和缝线管的推片是互补的机构,其允许来自弹簧机构的能量通过第二击发滑动件传递以同时推进缝线管和针管。

在本公开的第五方面中,匣盒还包括切割器爪,切割器爪位于匣盒的主体内并且位于切割器块和推动器块之间。在本发明的第五方面,切割器爪具有与切割器块接合的接合位置,使得切割器爪防止切割器块向推动器块滑动,以及与切割器块脱离的脱离位置,使得切割器爪允许切割器块向推动器块滑动。在本发明的第六方面,推动器块配置为滑动以与切割器爪接触并且使切割器爪枢转而脱离与切割器块的接合。
在本公开的第七方面,匣盒还包括在匣盒的主体的盖上的指示器窗口。在本发明的第八方面,指示器窗口指示切割器块的位置。在本公开的第九方面,指示器窗口被配置为提供到滑动切割器块的通路。
在本公开的第十方面,匣盒还包括在旋钮上的通路窗口,该通路窗口提供接近切割器爪的通路,以允许切割器爪从接合位置移动到脱离位置,而推动器块不接触切割器爪。
在本公开的第十一方面,一种系统包括手柄和匣盒,其中手柄配置为接受一系列这种匣盒并且将机械能传递给匣盒以递送植入物。匣盒包括匣盒主体,该匣盒主体联接到具有长轴线的轴组件和轴组件的远侧部分,该远侧部分具有贯穿远侧部分的管腔。该管腔具有由管腔随其从在平行于长轴线的第一方向上的延伸到在横向于长轴线的第二方向上的延伸而弯曲而限定的管腔曲率半径。针组件可滑动地设置在轴组件和匣盒主体内并且具有针远侧部分,使得针远侧部分配置为从管腔终止的出口端口退出轴组件。针远侧部分具有由针从在平行于长轴线的第一方向上的延伸到在横向于长轴线的第二方向上的延伸而弯曲而限定的针曲率半径,并且管腔曲率半径和针曲率半径不同。
在本公开的第十二方面,匣盒包括位于轴组件上的切口,该切口配置为允许针远侧部分的至少一部分弯曲通过切口。在本公开的第十三方面,匣盒还包括位于轴组件上的附加切口,该附加切口配置为允许针远侧部分的至少一部分弯曲通过附加切口。
在本公开的第十四方面,匣盒包括远侧管腔壁,该远侧管腔壁配置为具有与针远侧部分的期望退出轨迹相切的壁曲率半径。
在本公开的第十五方面,一种系统包括手柄和匣盒,其中手柄配置为接受一系列这种匣盒并且将机械能传递给匣盒以递送植入物。手柄包括在手柄内的凸轮,并与手柄中包含的触发器组件连接。手柄还包括连接到凸轮的轮致动器,使得凸轮上的特征配置为当特征接触轮致动器时使轮致动器沿第一方向枢转。轮致动器具有弯曲部,当轮致动器沿第一方向枢转时接合该弯曲部,并且当特征不再接触轮致动器时使轮致动器沿与第一方向相反的第二方向枢转。

Claims (18)

其中,所述弹簧处于拉伸位置,施加的力趋向于将切割器块和推动器块拉向彼此。
2.根据权利要求1所述的匣盒,还包括在匣盒上的旋钮,该旋钮从解锁位置移动到锁定位置,使得锁定位置将匣盒固定至手柄。
3.根据权利要求2所述的匣盒,还包括在所述旋钮上的推动器安全凸片,所述推动器安全凸片配置为当所述旋钮处于所述解锁位置时接合所述植入物致动器,使得所述植入物致动器在如此接合时不能枢转。

5.根据权利要求2所述的匣盒,还包括切割器爪,其位于匣盒的主体内并且位于切割器块和推动器块之间。
6.根据权利要求5所述的匣盒,其中,所述切割器爪具有与所述切割器块接合的接合位置,使得所述切割器爪防止所述切割器块向所述推动器块滑动,并且所述切割器爪具有与所述切割器块脱离的脱离位置,使得所述切割器爪允许所述切割器块向推动器块滑动。
7.根据权利要求6所述的匣盒,其中,所述推动器块配置为滑动以与所述切割器爪接触并且使所述切割器爪枢转而脱离与所述切割器块的接合。
8.根据权利要求7所述的匣盒,还包括在匣盒的主体的盖上的指示器窗口。

10.根据权利要求9所述的匣盒,其中,所述指示器窗口被配置为提供用于滑动所述切割器块的通路。
11.根据权利要求6所述的匣盒,还包括在所述旋钮上的通路窗口,所述通路窗口提供到所述切割器爪的通路,以允许所述切割器爪从所述接合位置移动到所述脱离位置而不使所述推动器块接触所述切割器爪。
12.一种用于输送植入物的匣盒,包括:
切割器块,其通过拉伸弹簧与推动器块连接,其中切割器块和推动器块被配置为在匣盒的主体内沿直线方向移动;和
可枢转的植入物致动器,其在匣盒的主体内,并且当植入物致动器处于第一位置时与推动器块接合,使得植入物致动器防止推动器块移动,其中所述弹簧处于拉伸位置,施加的力趋向于将切割器块和推动器块拉向彼此。
13.根据权利要求12所述的匣盒,还包括切割器爪,其在匣盒的主体内并且位于切割器块和推动器块之间。
14.根据权利要求13所述的匣盒,其中,所述切割器爪具有与所述切割器块接合的接合位置,使得所述切割器爪防止所述切割器块朝向所述推动器块移动,并且具有与所述切割器块脱离的脱离位置,使得所述切割器爪允许所述切割器块向推动器块移动。
