Abstract
本发明公开了一种肺科营养素,包含以下重量百分含量的组分:粉末中链甘油三酯10~20%、浓缩乳清蛋白粉10~18%、大豆分离蛋白粉11~15%、粉末葵花籽油8~12%、粉末亚麻籽油6~8%、复配矿物质3~5%和复配维生素0.3~0.6%;所述粉末中链甘油三酯中包含50~70wt%的中链甘油三酯;所述粉末葵花籽油中包含50~80wt%的葵花籽油;所述粉末亚麻籽油包含45~55wt%的亚麻籽油。本发明所述肺科营养素采用动植物蛋白复合,添加了中链脂肪酸、亚麻籽油、葵花籽油和复配维生素和矿物质,与现有的营养素相比,本发明所述肺科营养素具有以下优点:(1)低糖,不增加呼吸负担;(2)最新的中长链脂肪酸,即可提供最高的能量,又不会拉肚子;(3)全方位强化免疫营养素,有助于抗炎抗氧化。

技术领域
本发明涉及一种营养保健品,具体涉及一种肺科营养素。
背景技术
慢性肺病患者,例如慢阻肺,即使应用最好的药物治疗,也无法逆转疾病的发展,其肺功能呈不可逆地下降。每次急性加重住院,不仅产生庞大的医疗费用(平均住院费用24,372元),而且会对肺功能造成剧烈的打击,仅1/3的患者能恢复到住院前的水平。研究显示,慢阻肺患者的病情越严重,BMI(体重指数=体重÷身高2)越低。而且,营养不良将导致院内感染率增加1-2倍,使住院期间的死亡率增加77%,使30天内因为肺炎而再入院的风险增加29%。因此,营养支持是慢性肺病患者不可或缺的治疗组成之一,对于增强肺部抵抗力,减少肺炎发生率,减少肺病急性加重,降低整体治疗费用具有至关重要的作用。
肺病营养治疗当前的局限性:
与老年人营养不良相比,肺病患者具有独特的病理生理特点。而传统全营养素是针对健康人的营养需求而设计的,慢性肺病患者服用后可能导致诸多不利影响:
1)增加呼吸负担:慢性肺病患者均存在呼吸功能下降,而传统全营养中糖类(最终会分解为二氧化碳和水)的能占比高达50%-60%,导致二氧化碳生成增加,进一步增加患者的呼吸负担。
2)增加胃肠负担,吸收少,易腹泻。慢性肺病患者因为长期供氧不足以及合并心功能不全,导致胃肠道淤血和缺氧而影响营养素的消化和吸收。传统全营养素的脂肪以长链脂肪酸为主,①高脂时会导致胃排空延迟,增加胃肠负担,也不利于肺癌术后胃肠功能的恢复;②长链脂肪酸的分子量大,消化吸收路径长,需要更多消化酶的参与,因此吸收慢,吸收少,这在消化吸收障碍者更为明显;③易导致腹泻(脂肪痢),而影响患者的依从性;④长链脂肪酸必须经过淋巴系统转运才能入血。肺癌患者淋巴结清扫时损伤淋巴管道,导致长链脂肪酸和免疫蛋白丢失,从而增加胸腔引流量,不利于术后早期拔除引流管。
3)影响血糖、血脂,不适用于合并心血管疾病和代谢疾病的患者。慢阻肺患者中,①50%合并心血管疾病。摄入过多的长链脂肪酸会导致甘油三酯和胆固醇水平上升,不利于心血管疾病患者;②18.7%合并糖尿病。传统全营养素为高糖配方,导致血糖明显升高。
4)抗炎、抗氧化能力有限。慢性肺病多存在全身炎症,需要强化抗炎、抗氧化,而传统全营养素无法满足这一需求。
因此,国家卫生健康委员会已经将肺病全营养纳入特殊医学用途配方食品的范畴,以区别于传统的全营养配方食品。但截至2021年4月24日,尚无肺病全营养产品获批特医食品。
发明内容
本发明的目的在于克服现有技术存在的不足之处而提供一种肺科营养素。
为实现上述目的,本发明采取的技术方案为:一种肺科营养素,包含以下重量百分含量的组分:
粉末中链甘油三酯10~20%、浓缩乳清蛋白粉10~18%、大豆分离蛋白粉11~15%、粉末葵花籽油8~12%、粉末亚麻籽油6~8%、复配矿物质3~5%和复配维生素0.3~0.6%;
所述粉末中链甘油三酯中包含50~70wt%的中链甘油三酯;
所述粉末葵花籽油中包含50~80wt%的葵花籽油。
所述粉末亚麻籽油包含45~55wt%的亚麻籽油。
本发明所述肺科营养素专为肺病患者而设计,具体特征如下:
1、添加了上述含量的葵花籽油和亚麻籽油,使本发明所述肺科营养素为高脂低糖配方,能够快速供能,不增加呼吸负担。1g脂肪可提供能量37kJ,呼吸商仅0.7,而1g蛋白质和糖类仅提供17kJ能量,呼吸商分别为0.7和1.0,因此,高脂低糖配方意味着更高的能量密度,高效供能,弥补慢性肺病患者的高代谢损耗,同时不增加呼吸负担;
2、添加了上述含量的中长链脂肪酸,使本发明所述肺科营养素能够有吸收好,腹泻少的特点。中链脂肪酸分子量小,代谢路径短,吸收速率是长链脂肪酸的6倍,脂肪吸收系数比长链脂肪酸高5%-47%,24小时粪便脂肪较长链脂肪酸减少2g-56g,因此中长链脂肪酸(将50%的长链脂肪酸替换为中链脂肪酸)是消化吸收障碍患者的首选,这样既能保证人体必须脂肪酸的供给,又能有效改善吸收,减少腹泻。此外,中长链脂肪酸还有助于减少肺癌术后患者因淋巴管道损伤导致的漏出液过多,从而减少胸腔引流量,缩短引流管留置时间;明显缩短肛门排气时间,说明胃肠功能恢复更快,是肺癌术后患者的营养治疗首选。
3、改善糖脂代谢。配方中,中链脂肪酸可改善血脂,降低肝脏总胆固醇,改善脂蛋白水平,减少45%的餐前血糖生成,使胰岛素抵抗指数降低8.7%;大豆蛋白是最有利于心血管健康的蛋白,对血脂、血压均会产生积极的影响。因此,所述肺科营养素同样适用于合并心血管疾病和糖尿病的肺病患者。
因此,本发明所述肺科全营养素,是根据肺病患者独特的病理生理特点而设计的全营养素,很好地弥补了传统全营养素的不足。
本发明所述肺科营养素可用温开水冲服,可以根据患者的能量需求来确定使用量。
所述粉末中链甘油三酯、粉末亚麻籽油、粉末葵花籽油可购自市场,其中含有一定量的赋形剂和添加剂,优选地,所述粉末中链甘油三酯为中链甘油三酯、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯和二氧化硅的混合物;所述粉末亚麻籽油为亚麻籽油、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯、二氧化硅和维生素E的混合物;所述粉末葵花籽油为葵花籽油、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯、二氧化硅和维生素E的混合物。
优选地,所述浓缩乳清蛋白粉中包含18~22wt%的乳清蛋白;优选地,所述大豆分离蛋白粉中包含88~92wt%的蛋白质。
所述复配矿物质和复配维生素均可购自市场,优选地,所述复配矿物质为磷酸氢钙、碳酸镁、葡萄糖酸锌、焦磷酸铁和麦芽糊精的混合物。优选地,所述复配维生素为L-抗坏血酸、dl-α-醋酸生育酚、烟酸、醋酸维生素A、D-泛酸钙、盐酸吡哆醇、盐酸硫胺素、核黄素、维生素D3、叶酸、氰钴胺和麦芽糊精的混合物。
优选地,还包含以下重量百分含量的组分:麦芽糊精15~25%、椰浆粉7.0~11.0%、左旋肉碱0.7~1.0%、二氧化硅0.5~0.8%、黄原胶0.2~0.5%和牛磺酸0.09~0.14%。
优选地,所述椰浆粉包含椰浆、麦芽糊精和酪朊酸钠。

n-6为脂肪酸增加炎症水平(促炎),n-3为脂肪酸抗炎,n-6:n-3>10:1时具有促炎的效果;n-6:n-3<5:1时具有抗炎效果,对心血管疾病,风湿性关节炎和哮喘等有利(参考文献:Ricker MA,Haas WC.Anti-Inflammatory Diet in Clinical Practice:AReview.Nutr Clin Pract.2017Jun;32(3):318-325.doi:10.1177/0884533617700353.Epub 2017Mar 28.PMID:28350517.)本发明所述肺科营养素采用独特的脂肪酸抗炎配比,免疫维生素含量加倍,实现强化抗炎、抗氧化功效,降低CRP、TNF-α、IL-6、IL-8的水平,利于缓解肺病患者的全身炎症。
本发明的有益效果在于:本发明提供了一种肺科营养素,本发明所述肺科营养素采用动植物蛋白复合,添加了中链脂肪酸、亚麻籽油、葵花籽油和复配维生素和矿物质,与现有的营养素相比,本发明所述肺科营养素具有以下优点:(1)低糖,不增加呼吸负担;(2)中长链脂肪酸(长链脂肪酸源自粉末葵花籽油和粉末亚麻籽油,中链脂肪酸源自中链甘油三酯)既可提供最高的能量,又不会拉肚子;(3)全方位强化免疫营养素,有助于抗炎抗氧化。

为更好地说明本发明的目的、技术方案和优点,下面将结合具体实施例对本发明作进一步说明。
本发明实施例所述物质成分的来源如下:
麦芽糊精购自山东西王糖业有限公司;
粉末中链甘油三酯购自广州汇健生物科技公司,其中,中链甘油三酯的含量为70%;
浓缩乳清蛋白粉购自广州汇健生物科技公司的WPC80;
大豆分离蛋白粉购自临沂山松生物制品有限公司的SD-100;
粉末葵花籽油购自西安欣禄生物科技有限公司;其中,葵花籽油的含量为50%;
椰浆粉购自海南南派实业有限公司的椰浆粉A106;
粉末亚麻籽油购自西安海斯夫生物科技公司,其中亚麻籽油的含量为50%;
复配矿物质购自上海绝尘生物科技有限公司;
左旋肉碱购自辽宁科硕营养科技股份有限公司;
二氧化硅购自上海绝尘生物科技有限公司;
黄原胶购自山东阜丰发酵有限公司;
复配维生素购自广州汇健生物科技公司;
牛磺酸购自青岛青尔源药业有限公司的66-12。
实施例1
本发明所述肺科营养素的一种实施例,本实施例所述肺科营养素包含以下重量百分含量的组分:
麦芽糊精22%、粉末中链甘油三酯17%、浓缩乳清蛋白粉15%、大豆分离蛋白粉13%、粉末葵花籽油10%、椰浆粉9%、粉末亚麻籽油7%、复配矿物质4.38%、左旋肉碱1%、二氧化硅0.7%、黄原胶0.35%、复配维生素0.45%和牛磺酸0.12%。
实施例2
本发明所述肺科营养素的一种实施例,本实施例所述肺科营养素包含以下重量百分含量的组分:
麦芽糊精15%、粉末中链甘油三酯20%、浓缩乳清蛋白粉18%、大豆分离蛋白粉13%、粉末葵花籽油8%、椰浆粉11%、粉末亚麻籽油8%、复配矿物质4.6%、左旋肉碱1%、二氧化硅0.6%、黄原胶0.3%、复配维生素0.41%和牛磺酸0.09%。
传统的全营养素用于肺病患者,副作用主要有3个:1、碳水化合物摄入过多,增加二氧化碳生成和呼吸做功;2、糖脂代谢异常,增加心血管风险;3、吸收差,增加腹泻的风险;参见参考文献:1、Pumonary Rehabilitation:Guidelines to Success,4thEdition.Elsevier(Singapore)Pte Ltd.2009;2、Maltais F,etal.am.J.Respir.Crit.Care Med.2014May;189(9):e15-e62。
实施例1所述肺科营养素属于低糖高脂高蛋白的配方,可以用于代餐。实施例2所述肺科营养素属于超低糖高脂高蛋白的配方,可以用于患者的营养补充,患者日常还可进食部分碳水化合物,如米饭、面条等。
产品封装为20克一小袋。使用时用35-40度的温开水冲调至100-150毫升。根据患者的能量需求来确定用量。
用于营养补充时,200-500千卡/天,餐间补充。服用实施例2,x2袋/天(约200千卡),x3袋/天(约300千卡),x4袋/天(约400千卡),x5袋/天(约500千卡)。
用于代餐时,需求为能量25-30千卡/公斤体重/天、蛋白质1.2-2.0克/公斤体重/天。服用实施例1,根据患者的需求来确定每日用量。例如:体重50公斤的患者x10袋/天(约1100千卡)、x12袋/天(约1320千卡)或更多袋/天。
本产品为存在营养不良和营养风险的肺病患者提供全面的营养素,包括脂肪、蛋白质、碳水化合物、多种维生素和矿物质、左旋肉碱、硫磺酸。
实施例1和实施例2所述肺科营养素通过患者志愿使用,目前实施例1使用人数为20人,实施例2的使用人数为20人,使用三个月后,患者的使用情况反馈见表1。

从表1可以看出,实施例1和实施例2所述肺科营养素经患者试用,有较大比重的体重增加反馈和继续使用意愿反馈,且均未出现腹泻的情况。
最后所应当说明的是,以上实施例仅用以说明本发明的技术方案而非对本发明保护范围的限制,尽管参照较佳实施例对本发明作了详细说明,本领域的普通技术人员应当理解,可以对本发明的技术方案进行修改或者等同替换,而不脱离本发明技术方案的实质和范围。
Claims (10)
1.一种肺科营养素,其特征在于,包含以下重量百分含量的组分:
粉末中链甘油三酯10~20%、浓缩乳清蛋白粉10~18%、大豆分离蛋白粉11~15%、粉末葵花籽油8~12%、粉末亚麻籽油6~8%、复配矿物质3~5%和复配维生素0.3~0.6%;
所述粉末中链甘油三酯中包含50~70wt%的中链甘油三酯;
所述粉末葵花籽油中包含50~80wt%的葵花籽油;
所述粉末亚麻籽油包含45~55wt%的亚麻籽油。
2.如权利要求1所述肺科营养素,其特征在于,所述粉末中链甘油三酯为中链甘油三酯、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯和二氧化硅的混合物。
3.如权利要求1所述肺科营养素,其特征在于,所述粉末亚麻籽油为亚麻籽油、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯、二氧化硅和维生素E的混合物。
4.如权利要求1所述肺科营养素,其特征在于,所述粉末葵花籽油为葵花籽油、低聚异麦芽糖、酪蛋白酸钠、单,双硬脂酸甘油酯、二氧化硅和维生素E的混合物。
5.如权利要求1所述肺科营养素,其特征在于,所述浓缩乳清蛋白粉中包含18~22wt%的乳清蛋白。
6.如权利要求1所述肺科营养素,其特征在于,所述大豆分离蛋白粉中包含88~92wt%的蛋白质。
7.如权利要求1所述肺科营养素,其特征在于,所述复配矿物质为磷酸氢钙、碳酸镁、葡萄糖酸锌、焦磷酸铁和麦芽糊精的混合物。
8.如权利要求1所述肺科营养素,其特征在于,所述复配维生素为L-抗坏血酸、dl-α-醋酸生育酚、烟酸、醋酸维生素A、D-泛酸钙、盐酸吡哆醇、盐酸硫胺素、核黄素、维生素D3、叶酸、氰钴胺和麦芽糊精的混合物。
9.如权利要求1所述肺科营养素,其特征在于,还包含以下重量百分含量的组分:麦芽糊精15~25%、椰浆粉7.0~11.0%、左旋肉碱0.7~1.0%、二氧化硅0.5~0.8%、黄原胶0.2~0.5%和牛磺酸0.09~0.14%。
10.如权利要求9所述肺科营养素,其特征在于,每100克所述肺科营养素中包含以下营养成分:能量500~600kcal、蛋白质23~27g、脂肪25~30g、碳水化合物40~48g、钠225~240mg、维生素A 900~1100μg RE、维生素D 1.4~1.8μgRE、维生素E 12~15mg a-TE、维生素B1 1.0~1.2mg、维生素B2 1.2~1.4mg、维生素B6 1.2~1.4mg、维生素B12 3.2~3.6μg、维生素C 120~160mg、烟酸20~24mg、叶酸330~370mg、泛酸5~5.5mg、钙780~850mg、磷600~700mg、镁130~200mg、铁13~19mg、锌12~18mg、左旋肉碱700~1000mg和牛磺酸90~140mg。
